Full textTiivistelmä Etelä-Karjalan allergiatutkimus (EKAT) käynnistettiin selvittämään lasten ruoka-allergioiden ja ruokayliherkkyysoireiden esiintyvyyttä ja niihin liittyviä tekijöitä. Tutkimusväestöön kuuluivat kaikki ne Etelä-Karjalan maakunnan alueella asuvat lapset, jotka olivat syntyneet huhtikuun 2001 ja maaliskuun 2006 välisenä aikana. Kyselyaineisto kerättiin yhteistyössä alueen neuvoloiden kanssa vuosina 2005–2006. Kyselytutkimuksen kanssa samanaikaisesti mutta siitä riippumattomasti kerättiin samaa lapsiväestöä koskevat allergiatestitulokset alueen potilasrekistereistä. Kyselytutkimuksen osallistumisaste oli vastasyntyneiden ikäluokassa 54 % (644/1194) ja isommilla lapsilla 69 % (3308/4779). Kolmannen ikävuoden loppuun mennessä vanhemmat ilmoittivat 9 %:lla lapsista todetun ruoka-allergiaa ja lisäksi 21 %:lla ruokayliherkkyys perustui ainoastaan vanhempien havaintoihin. Näiden lisäksi 19 % lapsista vältti yhtä tai useampaa ruoka-ainetta ilman, että minkään ruoka-aineen olisi todettu aiheuttaneen oireita. Dieettien noudattajien osuus väheni selvästi tarkasteltaessa ikäluokkia yksivuotiaista neljävuotiaisiin. Kolmannen ikävuoden loppuun mennessä ruoka-allergiatestattuja oli 19 % ja positiivisen testituloksen saaneita 8 % tutkimukseen osallistuneista. Lääkärin toteamat ja testeihin perustuneet ruoka-allergiat olivat tavallisempia maidolle, kananmunalle ja viljoille kuin muille ruoka-aineille. Allergiatestatuilla ja positiivisen testituloksen saaneilla oli vain lievä yliedustus kyselytutkimukseen osallistujien joukossa verrattuna kyselyyn vastaamattomiin. Allergiset sairaudet toisella biologisella vanhemmalla kaksinkertaistivat ja molemmilla vanhemmilla kolminkertaistivat positiivisen ruoka-allergiatestin ilmaantuvuuden. Raskauden ensimmäisen kolmanneksen lopun ajoittuminen kevääseen ennakoi syntyvän lapsen herkistymistä ruoka-aineille. Varhaislapsuuden ruoka-allergioiden ja -yliherkkyyksien todettiin olevan tavallisia väestössä. Näiden sairauksien periytyvyydestä saatiin uutta väestötietoa. Lisäksi havaittiin yhteys 11. raskausviikon ajoittumisen kevääseen ja ruoka-aineille herkistymisen välillä. Yhteyden yksityiskohtaiset syytekijät ja immunologiset mekanismit täytyy ehdottomasti vahvistaa jatkotutkimuksin

Full textBackground: The diagnostic criteria for generalized anxiety disorder (GAD) and its status as an independent mental disorder have been controversial. More recent epidemiological data provide a more precise picture of this disorder and its specific features. Methods: A systematic overview is given in regard to prevalence, course and comorbidity, impairment, and help-seeking behavior as well as to specificity of the core symptom (worries). Results: GAD is a frequent disorder with high incidence rates in middle-age groups, which are not seen in other anxiety disorders. Despite the high comorbidity GAD can be validly distinguished. Clinically relevant worries have been proven as specific for the disorder. The impairments are also considerable for patients without comorbid disorders. Conclusions: Research supports the independent status of GAD and the importance of this diagnosis. Unsolved questions are to be analyzed in future research

Full textEvidence-based medicine (ebm) can be described as the discipline bridging research and health care. This thesis covers 4 individual projects on the role of clinical epidemiology / evidence-based medicine in rational clinical decision-making. The first study "Observation or Experiment" addressed the methodological issue of the impact of observational studies versus randomised allocation to any intervention on the estimated effect of the intervention. The study population were systematic reviews including randomised and non-randomised studies on a broad spectrum of interventions and comprising more than 3000 primary studies. In the empirical assessment, we could demonstrate that lack of randomisation tended to exaggerate the estimated effect of the intervention, but could also result in underestimation of the effect, in similar effect sizes or even in inverse effects. Therefore randomisation is mandatory in intervention studies to generate comparable baseline groups and thereby ensure an unbiased assessment of the underlying treatment effect. The second project was a simulation study investigating the impact of bias on clinical decision-making. Based on empirical data from ITU-patients, various assumptions on baseline risks of gastrointestinal bleeding and an estimate of the relative risk reduction for bleeding by H2-antagonists from a recent meta-analysis we investigated the potential for erroneous clinical decisions induced by systematic errors in the performance of clinical trials. We could demonstrate that certain clinical situations are particularly susceptible for errors in decision-making, in particular, if a patient’s baseline risk for an adverse event or the effectiveness of the intervention is only moderate or small. As low baseline risk and / or moderate treatment effects tend to occur frequently in physician-patient-encounters, physicians need to be aware of the increased risk for errors and pay meticulous attention on a reliable evidence base. The third project was a clinical trial on the effectiveness of teaching evidence-based medicine to physicians. The trial comprises two phases: Instrument development and performance of the trial. Starting from a comprehensive curriculum of evidence-based-medicine we identified relevant core items of ebm. Based on this curriculum we developed and validated a before-after instrument with 15 questions each that was able to distinguish varying degrees of knowledge and skills of ebm. Over a period of 3 years, the instrument was randomly administered to 203 participants in consecutive ebm-courses. Prior to the course, the participants scored a mean of 6,3 + 2,9, after the course the number of correct answers increased to 9,9 + 2,4 (p< 0.001). The scores of ebm-experts or ebm-naïve controls was significantly higher resp. lower. Thereby we could demonstrate that short intensive courses in evidence-based medicine (such as the Berlin courses) can lead to a significant and clinically meaningful increase in knowledge and skills. The forth project focussed on the implementation of evidence-based medicine in the day-to-day practice of family doctors. In a cluster-randomised study we investigated whether short evidence-based drug information in hospital discharge letters can influence the prescribing behaviour of general practitioners. 178 practices participated in the study, 417 discharge letters with 59 different evidence-based drug information were sent out and 268 interviews were successfully performed after an interval of 3 - 4 months. Physicians in the intervention group were statistically less likely to depart from the hospital recommendations than physicians in the control group who only received a regular discharge letter group (absolute risk reduction 12,5%; p=0.039). Furthermore, physicians were very satisfied with this additional piece of information, which tended to assure their own knowledge and reminded them to apply it in this individual patient (rather than providing new information). It was concluded that short evidence-based information in discharge letters could have a positive impact on a rational prescribing behaviour of physicians.

Full textThe year 2000 is often called a milestone in the history of human genetics. The knowledge of the sequence of the human genome will only become relevant for clinical medicine when the function of genes within complex physiological systems as well as the genetic variability will be revealed. The studies reported here are based on the assumption that the influence of genetic variability does not only become obvious by comparison of affected and unaffected subjects but is as well detectable in the variability of physiological parameters in the general population. All studies are based on testing healthy mono- and dizygotic twins. We determined the heritability of various cardiovascular parameters. Furthermore selected candidate genes were tested by linkage and association analyses. We could demonstrate the influence of specific genes on blood pressure regulation, heart size, ECG and lipids. These studies are a proof of principle for the functional analysis of single genes in unselected random samples.

Full textThe findings of a number of recent major epidemiological studies in different countries document an increase in the cumulative lifetime prevalence estimates of major depression with each successively younger birth cohort and a decrease in the age of onset of this disorder (Cross-National Collaborative Group, 1992; Klerman & Weissman, 1989). At the same time comparably low depression estimates are found for the elderly. The paper presents an overview of the major studies conducted in this area in recent years. The validity of the findings, their significance for the aetiology of Major Depression and possible alternative explanations for the temporal trends are discussed. While much valid empirical evidence confirms the increase of Major Depression in recent birth cohorts, the low prevalence estimates in the elderly seen to represent at least in part an artifact of the research methodology
