Abstract:
ABSTRACT BACKGROUND: Endoscopic mucosal resection is one of the most frequent therapeutic alternatives for large colorectal lateral spreading tumors. There are few data on the prevalence of synchronous lesions on these patients. OBJECTIVE: To describe the prevalence of synchronous colorectal lesions in patients referred for endoscopic mucosal resection of lateral spreading tumors >20 mm. METHODS: We reviewed the endoscopic database of our Department and identified adult patients who were referred for the resection of a colorectal lateral spreading tumor >20 mm and had a diagnostic colonoscopy performed up to six months before. The proportion of patients with at least one synchronous lesion was estimated. The following features were compared between patients with and without synchronous lesions: age, gender, bowel preparation quality and cecal intubation on index colonoscopy and therapeutic colonoscopy, serrated adenoma as index lesion. RESULTS: From December 2016 to November 2017, we identified 70 patients who fulfilled inclusion criteria. Median size of lesions was 25 mm (20-45). Eighty percent were located in the right colon and 35.71% were serrated adenomas. Synchronous lesion rate was 38.57%. Bowel preparation quality was similar in both groups when comparing both index and therapeutic colonoscopies. Patients with synchronous lesions had a higher proportion of serrated adenoma as index lesion than patients without synchronous lesions [51.85% vs 25.58%, OR 3.13 (1.13-8.68), P=0.03]. CONCLUSION: We found a high prevalence of synchronous lesions among patients with a large colorectal lateral spreading tumor. This risk seems to be increased if index lesions are serrated adenomas.

Abstract:
Objective. To examine the relationship between the volume of the middle cerebral artery stroke lesion and functional status in the subacute phase of stroke. Methods. Infarctvolumes of 94 patients with a 1st middle cerebral artery stroke assessed on conventional MRI scans obtained in the 2nd week poststroke were related to a clinical measure of stroke severity (National Institutes of Health Stroke Scale [NIHSS]) and to functional status: motor impairment (Motricity Index [MI]) and limitation in activities (Barthel Index [BI] and modified Rankin Scale). Separate correlations were computed for patients with large (>30 ml) and small (30 ml) lesions, and to investigate the influence of lesion location on the relationship between volume and functional status, correlations were computed for patients with left and right hemisphere lesions and for patients with cortical and subcortical lesions. Results. Lesion volume correlated strongly with NIHSS scores (R = 0.61) and moderately with the patient’s functional status (MI [R between -0.42 and - 0.49], BI [R = -0.43], and Modified Rankin Scale [R = 0.45]). Right hemisphere lesions and cortical lesions had a stronger correlation with functional status. In patients with small lesion volumes (0-30 ml), no relationship between lesion volumes and functional status was seen at all. Conclusions. Lesion volume is moderately to strongly related to the functional status in the 2nd week poststroke.

Abstract:
(1) Background: this prospective study was focused on detailed analysis of the mutation heterogeneity in colorectal lesions removed during baseline (index) colonoscopy to identify patients at high risk of early occurrence of metachronous adenomas. (2) Methods: a total of 120 patients after endoscopic therapy of advanced colorectal neoplasia size ≥10 mm (index lesion) with subsequent surveillance colonoscopy after 10–18 months were included. In total, 143 index lesions and 84 synchronous lesions in paraffin blocks were divided into up to 30 samples. In each of them, the detection of somatic mutations in 11 hot spot gene loci was performed. Statistical analysis to correlate the mutation profiles and the degree of heterogeneity of the lesions with the risk of metachronous adenoma occurrence was undertaken. (3) Results: mutation in exon 7 of the TP53 gene found in the index lesion significantly correlated with the early occurrence of metachronous adenoma (log-rank test p = 0.003, hazard ratio 2.73, 95% confidence interval 1.14–6.56). We did not find an association between the risk of metachronous adenomas and other markers monitored. (4) Conclusions: the findings of this study could lead to an adjustment of existing recommendations for surveillance colonoscopies in a specific group of patients with mutations in exon 7 of the TP53 gene in an index lesion, where a shortening of surveillance interval may be warranted.

Abstract:
Objective. To explore the quantitative adjustment of ablation index (AI) under different baseline impedance to achieve similar lesion dimensions. Methods. (1) Keeping the AIs relatively constant, the lesion dimensions in different baseline impedances were studied. (2) According to Joule's law, Q = I2RT, keeping the current (I) unchanged, the powers corresponding to different baseline impedances can be obtained. Under different baseline impedances and corresponding powers, the swine hearts were ablated for 30 s in simulated human circumstances. The baseline impedances, the lesion dimensions, and AIs were recorded. And the derivation of empirical formula was achieved according to the AIs and baseline impedance values in similar lesions dimension. (3) Basic AI and baseline impedance (AI0/R0) were set as 400/120 Ω in the common AI groups and 550/120 Ω in the high AI groups, AI values in different baseline impedances were calculated using the empirical formula, and the corresponding lesion dimensions were measured to verify this formula. Results. (1) Higher baseline impedances were related to smaller lesion dimensions at similar AIs. (2) The lesion dimensions were roughly the same after modulating the baseline impedance and power to keep the electric current relatively constant. The relationship between AI and R fitted with experimental data is AI = 1.9933R + 203.61 (r = 0.9649), and the formula derived is ΔAI = (AI0 − 203)/R0 × ΔR. (3) Under the guidance of the empirical formula, there was no significant difference in lesion dimensions between the standard group and the formula guiding groups when AI0 = 400, but there was a shrinking tendence when AI > 700. Conclusion. The lesion depths are negatively correlated with baseline impedance at a certain AI. The relationship between baseline impedance and AI is “ΔAI = (AI0 − 203)/R0 × ΔR”. It is verified that when the AI is not too high, the empirical formula can be used to guide the quantitative adjustment of AIs at different baseline impedance, and the lesion depths achieved are roughly the same.

Abstract:
Skin lesion melanoma is the deadliest type of cancer. Artificial intelligence provides the power to classify skin lesions as melanoma and non-melanoma. The proposed system for melanoma detection and classification involves four steps: pre-processing, resizing all the images, removing noise and hair from dermoscopic images; image segmentation, identifying the lesion area; feature extraction, extracting features from segmented lesion and classification; and categorizing lesion as malignant (melanoma) and benign (non-melanoma). Modified GrabCut algorithm is employed to generate skin lesion. Segmented lesions are classified using machine learning algorithms such as SVM, k-NN, ANN, and logistic regression and evaluated on performance metrics like accuracy, sensitivity, and specificity. Results are compared with existing systems and achieved higher similarity index and accuracy.

Abstract:
The long-term efficacy of radiofrequency catheter ablation is highly dependent on the location and size of the lesions delivered via the ablation catheter. This is particularly the case for radiofrequency ablation for atrial fibrillation, where contiguous, transmural lesions are required for durable pulmonary vein isolation. Therefore, reliable lesion quality markers that accurately relate to lesion size in the context of 3D mapping have been sought to improve the reliability of conduction block. Force–time integral, Ablation Index and Lesion Size Index are metrics currently used in clinical practice. Several studies have shown better clinical outcomes using these indices compared with using contact force alone or no marker at all. A theoretical limitation of these composite lesion indices is that they rely on input values such as contact force, application duration and power. A recent technology assessessing local tissue impedance (an output measure) has shown promising results. Temperature, another output measure, is not routinely used due to the significant impact of catheter irrigation on readings, but two recent catheter technologies have sought to overcome this obstacle. This paper provides an update on the current knowledge on these lesion quality markers.

Background The apparent diffusion coefficient (ADC), as determined by whole-body diffusion-weighted MRI, may be useful as an outcome measure for monitoring response to treatment in chronic non-bacterial osteitis. Purpose To test and demonstrate the feasibility of ADC-measurement methods for use as outcome measure in chronic non-bacterial osteitis. Materials and Methods Using data from a randomized pilot study, feasibility of change-score ADC between baseline and second MRI (ΔADC12) and third MRI (ΔADC13) as outcome measure was assessed in three settings: “whole-lesion,” “single-slice per lesion,” and “index-lesion per patient”. Bone marrow edema lesions were depicted on short tau inversion recovery sequence at baseline and copied to ADC maps at the three time-points. Correlations between the three settings were measured as were analysis of variances. Discriminant validity was assessed as inter- and intra-observer reproducibility and smallest detectable change. Results 12 subjects were enrolled, and MRI was performed at baseline and weeks 12 and 36. Pearson correlation was high ( r > 0.86; p ≤ 0.01) for ΔADC between single-slice—whole-lesion and whole-lesion—index-lesion and tended to be significant for single-slice—index-lesion settings ( p = 0.06). For ΔADC12 and ΔADC13, Bland–Altman plots showed small differences (0.02, 0.03) and narrow 95% limits-of-agreement (−0.13–0.09, −0.07–0.05 μm2/s) between whole-lesion and single-slice ROI settings. Inter-observer reproducibility measured by intra-class correlation coefficient was poor-to-fair (range: 0.09–0.31), whereas intra-observer reproducibility was good-to-excellent (range: 0.67–0.90). Smallest detectable changes were between 0.21–0.28 μm2/s. Conclusion ADC change-score as outcome measure was feasible, and the single-slice per lesion ROI setting performed almost equally to whole-lesion setting resulting in reduced assessment time.

44 Background: Central gland (CG) prostate cancers (CaP) are reported with lesser incidence and smaller tumor volume compared to the peripheral zone (PZ). Index tumor lesions defined by highest grade may be missed when in the CG. MRI/US fusion-guided biopsy allows targeting of lesions, potentially identifying cancer outside the traditional TRUS biopsy template. Methods: Retrospective review of 1,003 patients who underwent multiparametric MRI (mpMRI) found 2,119 suspicious lesions. Targets were biopsied and stratified by zonal distribution, CG or PZ. Cancer detection rates (CDR) were tabulated by location and correlated with PSA, Gleason score, prostate volume and MRI suspicion. Results: Fusion-guided biopsy targeted lesions in the central (711, 34%) or peripheral (1408, 66%) prostatic zones. CDR was similar between zones: 35.2% CG compared to 33.6% PZ (Table). CDR of clinically significant disease (Gleason >4+3) was similar in the CG and PZ despite higher prostate volume in those with CG lesions. In contrast to TRUS biopsy, upgrading occurred in 18.5% of CG patients versus 13.3% PZ (p=0.024). 36.6% (77/210) of CG lesions represented the highest risk lesion on MRI, translating to 13% (77/592) of the biopsy-proven CaP cohort. Conclusions: CG cancers occur at a similar frequency as PZ CaP. CG lesions were more likely to be upgraded from TRUS biopsy, frequently representing the index lesion. In upgraded patients, CG targets constituted the index lesion in a third of all males. MRI/US fusion-guided biopsy identifies clinically significant disease of the CG not captured on traditional biopsy. [Table: see text]

Abstract Context.—Two relatively unknown and recently described placental membrane hypoxic lesions (laminar necrosis and microscopic chorionic pseudocysts) have never been compared with time-honored, focal (infarction), and diffuse hypoxic lesions of placental parenchyma. Objective.—To compare the effect on placental diagnosis of the above placental membrane hypoxic lesions and chorionic disc hypoxic lesions (infarctions and global hypoxic pattern of placental injury). Design.—Twenty-three clinical (maternal and fetal) and 32 gross and microscopic placental features were retrospectively compared in 4590 placentas from a placental database built during a 13-year period: 168 placentas with at least one hypoxic disc lesion (infarct or global hypoxia) and at least one membrane lesion (microscopic chorionic pseudocysts or laminar necrosis (group 1), 750 placentas with at least one hypoxic villous lesion but no membrane lesion (group 2), 480 placentas with at least one membrane lesion but no villous lesion (group 3), and 3192 placentas with no hypoxic villous or membrane lesions (group 4). Results.—Several clinical and fetal conditions and placental features known to be associated with in utero hypoxia had a statistically significant correlation with the index hypoxic placental lesions, both villous and membranous. Of placentas from patients associated with clinical conditions at risk for hypoxia, 15% featured only hypoxic membrane lesions without a chorionic disc hypoxic lesion. Conclusions.—Recognizing placental membrane hypoxic lesions increases the sensitivity of placental examination in diagnosing placental hypoxia by at least 15%. The risk of in utero hypoxia is increased when microscopic chorionic pseudocysts and laminar necrosis occur in conjunction with villous hypoxic lesions.

Acute kidney injury (AKI) resulting from toxic or ischemic insults and usually affect the tubular portion of the nephron, the use of potentially nephrotoxic drugs such as gentamycin one of the main causes. Early detection allows the AKI appropriate intervention may prevent or mitigate damage to the tubular cell and the development of acute renal failure (ARF). Diagnosis requires a combination of techniques, and be effective employment ultrasonography associated with laboratory tests. Ultrasound allows assessment of morphology and renal echogenicity by taking on bi-dimensional mode (B-mode) and renal hemodynamics through the assessment with pulsed Doppler with resistivity and pulsatility indices calculation. In this study, AKI was induced in six dogs with 30mg/kg of gentamicin once a day, for 10 days, in order to confirm the importance of B-mode and pulsed Doppler ultrasound for the diagnosis, monitoring and prognosis determination. Dogs were followed for 45 days by clinical examination, urinalysis, serum urea and creatinine, urinary GGT dosing and ultrasound. Six healthy dogs were used as control group and followed the same moments. The results of this study demonstrated that ultrasound is earlier than changes in laboratory diagnosis of ARI, and is useful for monitoring and prognosis determination, since the ultrasound aspect returned to normal in most dogs after recovery and dogs with ARF had higher values of the volume, of the Doppler indices and the degree of echogenicity.
A lesão renal aguda resulta principalmente da ação de agentes nefotóicos e de isquemia renal que geralmente afetam a porção tubular do néfron, sendo o uso de medicamentos potencialmente nefrotóxicos, como a gentamicina, umas das principais causas. A detecção precoce da lesão renal aguda permite a intervenção apropriada podendo prevenir ou atenuar o dano à célula tubular e o desenvolvimento da insuficiência renal aguda (IRA). O diagnóstico requer a associação de técnicas, sendo eficaz o emprego do exame ultrassonográfico associado aos exames laboratoriais. A ultrassonografia permite a avaliação da morfologia e da ecogenicidade renal por meio do exame em modo bidimensional (modo B) e da hemodinâmica renal por meio da avaliação com o Doppler pulsado pelo cálculo dos índices de resistividade e pulsabilidade. No presente estudo a lesão renal aguda foi induzida em seis cães, com o uso de 30mg/kg de gentamicina uma vez ao dia, durante dez dias, com o objetivo de confirmar a importância da ultrassonografia em modo B e Doppler pulsado para o diagnóstico, acompanhamento e determinação do prognóstico. Os cães foram acompanhados durante 45 dias por meio de exame clínico, exame de urina, dosagem sérica de ureia e creatinina, dosagem de GGT urinária e exame ultrassonográfico. Seis cães saudáveis foram utilizados como grupo controle e acompanhados nos mesmos momentos. Os resultados deste estudo demonstraram que a ultrassonografia foi capaz de diagnosticar a lesão renal aguda mais precocemente que as alterações laboratoriais, além de ser útil para o acompanhamento e determinação do prognóstico, uma vez que o aspecto ultrassonográfico voltou à normalidade na maioria dos cães após a recuperação e, os cães que desenvolveram IRA apresentaram valores mais elevados do volume, dos índices Doppler e do grau de ecogenicidade.

Abstract:
Numerous studies suggest that modulation of the cholinergic system through the use of nicotinic agonists can improve motor function in humans or animals with motor disorders. Specifically, although there are no approved therapeutics for patients with ataxia, the nicotinic receptor agonist varenicline has demonstrated efficacy to improve coordination and gait in several groups of patients with different subtypes of ataxia. Importantly, the mechanism underlying the varenicline’s mechanism of action to improve motor function remains to be elucidated. Thus, the purpose of these experiments was to first quantify gait impairments in rats with olivocerebellar ataxia utilizing an objective treadmill-based system to investigate temporospatial aspects of animals’ gait. These results were used to calculate an index that characterizes deviations from ‘normal’ gait, as similarly employed in clinical studies. The translational validity of this method of gait assessment was investigated by comparing gait impairments between these animals and those reported for humans with ataxia. It was next investigated whether varenicline could attenuate any gait impairments and thus improve motor functioning in these animals, as suggested by clinical findings. Finally, varenicline’s mechanism of action was investigated by attempting to block its effects by pretreating animals with the nicotinic antagonist mecamylamine. Thus, these studies demonstrate the involvement of nicotinic acetylcholine receptors in the mechanism of varenicline’s effects to improve motor functioning. Moreover, these results provide translational methods by which the efficacy of other, more selective nicotinic agonists to improve motor functioning can be tested preclinically prior to their use in humans with ataxia.

Abstract:
The global financial crisis has had a strong impact on most European countries. While at first the massive injection of resources helped the financial system not to collapse, the austerity policies implemented throughout the continent have not brought the expected economic growth. Interestingly, the type of intervention undertaken is rooted in the neoclassical tradition, which entails a specific understanding of the functioning of the financial system and the economy. In this context, the objectives of this paper are firstly, to analyze the main theoretical assumptions and the specific way in which the crisis is understood and the applied economic policies are developed under this tradition; and secondly, to account for the social and economic impact of the policies applied, and the different margins of action that each European country still has.
La crisis financiera global ha tenido un fuerte impacto en las economías europeas centrales y fundamentalmente en las periféricas. Si bien en un primer momento, la masiva inyección de recursos logró que el sistema financiero no colapse, las políticas de austeridad aplicadas no han podido consolidar las dinámicas de crecimiento esperadas. Lo interesante del tipo de intervención desarrollada, es que las medidas han sido justificadas en base a una serie de trabajos teóricos que presentan una visión particular del funcionamiento del sistema financiero y de la economía. Eneste contexto, los objetivos del presente trabajo son por un lado, analizar los principales supuestos teóricos y modo específico de entender a la crisis que han fundamentado las políticas económicas aplicadas; y por el otro, dar cuenta del impacto económico y social de estas medidas intentando dar cuenta los diferentes márgenes de acción que aún hoy poseen los países europeos.

AbstractTeachers wishing to offer lessons in nature may hold back for fear of leaving students keyed up and unable to concentrate in subsequent, indoor lessons. This study tested the hypothesis that lessons in nature have positive—not negative—aftereffects on subsequent classroom engagement. Using carefully matched pairs of lessons (one in a relatively natural outdoor setting and one indoors), we observed subsequent classroom engagement during an indoor instructional period, replicating these comparisons over 10 different topics and weeks in the school year, in each of two third grade classrooms. Pairs were roughly balanced in how often the outdoor lesson preceded or followed the classroom lesson. Classroom engagement was significantly better after lessons in nature than after their matched counterparts for four of the five measures developed for this study. This nature advantage held for teacher ratings; third-party tallies of ‘redirects’ (the number of times the teacher stopped instruction to direct student attention back to the task at hand); independent, photo-based ratings made blind to condition; and a composite index of classroom engagement, but not for student ratings. The nature advantage held across different teachers and held equally over the initial and final five weeks of lessons. The nature advantage was large: after a lesson in nature, ‘redirects’ were roughly half as frequent, allowing teachers to teach for longer periods uninterrupted. In 48 out of 100 paired comparisons, the nature lesson was a full standard deviation better than its classroom counterpart; in 20 of the 48, the nature lesson was over two standard deviations better. Because the pairs of lessons were matched on teacher, class (students and classroom), topic, teaching style, week of the semester, and time of day, the advantage of the nature-based lessons could not be attributed to any of these factors. It appears that, far from leaving students too keyed up to concentrate afterward, lessons in nature may leave students more able to engage in the next lesson. Such “refueling in flight” argues for including more lessons in nature in formal education.

Introduction: Since 2013, after the Saint Gallen International Breast Cancer Conference, breast cancer is classified by its morphological, biological and prognostic features into three subgroups according to the immunohistochemichal expression, which is similar to the genetic expression of the tumor. These groups, known as luminal, HER2 enriched and triple negative, are classified according to the positivity or negativity of nuclear receptors (estrogen [ER] and progesterone receptors [PR]) or membrane receptors (HER2). Triple negative tumors do not express positivity for any of these receptors. This group is composed of tumors with a more adverse biology and more aggressive behavior, affecting women in a younger age than other groups, and they also have higher rates of recurrence and lower overall survival. Objectives: This study aims to assess morphological and angiogenic features of these tumors by ultrasound exams. Methods: This is a prospective study, in which 129 lesions with an immunochemistry study compatible with triple negative tumors were selected from 1,180 breast carcinomas diagnosed by ultrasound and core biopsy from 2012 to 2017. Features assessed were shape, margin, posterior acoustic effect, reaction of the surrounding tissue, presence of calcifications within the lesion, presence of vascular flow with resistance index (RI), pulsatility index (PI), and maximum velocity of the vascular flow (Vmax). Results: Among the 129 tumors included, oval shape was the most prevalent with 72 cases (56%), 43 tumors (33%) had an irregular shape, and only 14 (11%) were round. As for their margin, the most prevalent was microlobulated, observed in 60 cases (46%), followed by circumscribed (35 cases, 27%), indistinct (19 cases, 15%), and the less frequent margins were angled and spiculated, with 10 (8%) and 5 cases (4%), respectively. The most prevalent posterior acoustic effect was enhancement (50 cases, 39%), followed by absence of posterior effect (38 cases, 27%), mixed effect (23 cases, 18%) and the least frequent was acoustic shadowing, accounting for only 18 cases (14%). The absolute majority of the tumors, 92% lesions in total, did not present calcifications within the lesion and 89% did not have a hyperechogenic halo in the surrounding tissue, presenting with abrupt lesion limits. From all cases, 110 (85%) had normal peritumoral tissue, causing no tissue thickening. In relation to the angiogenic study, most lesions had vascular flow (81 cases, 63%). The mean PI was 1.5, RI was 0.7 and the average Vmax was 19.7 cm/s. Conclusions: From an ultrasonographic point of view, some prevalent feautres are noted, which can help in their radiogenomic determination. Oval shape, abrupt interface, microlobulated margins, posterior acoustic enhancement, absence of internal microcalcifications and hyperechogenic halo prevailed.

Argipidine ((2R,4R)-4-methyl-1-(N2-[(RS)-3-methyl-l,2,3,4-tetrahydroquinolinesulfonyl)-L-arginyl]-2-piperidinecarboxylie acid hydrate) is a potent and specific inhibitor of thrombin(Ki=0.039μM). We tested the effect of argipidine on a new animal model of peripheral arterial occlusion (PA0).PA0 was prepared by injection of 5% lactic acid into rat femoral artery. Macroscopic changes of legs progressed asfollows; change to violet color around whole paw (3-24hr), edema (3-24hr), gangrene of fingers (after 24hr) and mummification of whole paw (after 7 days). Pathological examination demonstrated partial deletion of arterial endotherial cells and subsequent occlusive arterial thrombosis. Coagulo-parameters in circulating blood did not change 3 hours after lactic acid injection. To evaluate the effect of tested antithrombotic drugs, sum of lesion grade of five fingers is used as an index of severity of. PA0.Single subcutaneous injection of argipidine 30 minutes before lactic acid injection, inhibited significantly development of lesions of the lower limb at doses lOmg/kg and 30mg/kg in a dose-dependent manner. The number of rats which showed moderate or severe lesion on their legs at 14 days after lactic acid injection was decreased to 2/8 in argipidine treated (30mg/kg) rats from 7/8 in control rats. But argipidine injected simultaneously with lactic acid showed weak inhibitory effect (not significant) and argipidine injected three hours after lactic acid injection did not showany inhibitory activity. Slight prophylactic effect was also found by subcutaneous injection of heparin at a dose of 900U/kg, but its effect were not significant.In a separate experiment, PT and APTT were assayed after subcutaneous injection of argipidine or heparin. Heparin (900U/kg) prolonged APTT much greater than argipidine (30mg/kg), but as for prolongation of PT, argipidine was more effective than heparin.These results suggest that extrinsic coagulation system plays an important role in initiation of lesions of this model.

Introduction: Dermatofibrosarcoma Protuberans (DFSP) is a soft tissue tumor, with slow growth and low metastasis rate, but with a high chance of local recurrence. Epidemiologically, DFSP represents only 0.1% of all malignant tumors. It is more common in women aged between 30 and 50 years and the most affected site is the trunk. However, the presentation in the breast is rare. Clinically, DFSP is revealed as a slow and asymptomatic growth nodule that originates in the dermis and invades the subcutaneous tissue. The diagnosis is made by histopathology, with the result of uniform spindle cells with low atypia or mitotic activity, and immunohistochemistry, demonstrating, mainly, positivity for the CD34 antibody, and negativity for the S100 antibody. The standard treatment of DFSP is a complete surgical resection with negative margins. However, the minimum resection margin to achieve local control remains undefined. The present study aims to discuss a case of DFSP with a rare presentation in the mammary region and the steps for its diagnosis. Case report: A 37-year-old woman with an exophytic and ulcerated lesion of 5 cm in the lower inner quadrant (LIQ) of the right breast (RB). The patient reported the appearance of a skin lesion 10 years before in the same location, evolving with a superficial nodule over the last five years and with an accelerated growth of the lesion one month before. There are no other tangible nodules in the breasts and no armpit lymph nodes. Breast ultrasound: exophytic and vascularized nodule in RB LIQ, of 8.4 cm - BIRADS 4. Histopathological report after lesion excision: spindle cell neoplasm of mesenchymal origin, without necrosis, measuring 5.8x5.8 cm, mitotic index: 03 mitoses in 10 high-rise fields. The immunohistochemistry of the excised lesion indicated positivity for the CD34 and Ki-67 antibodies and negativity for the S-100, CD99, Desmina and Myogenin antibodies. The immunohistochemical profile, associated with the morphological findings, corroborates the diagnosis of Dermatofibrosarcoma protuberans. Surgical margins indicated involvement by the neoplasm. Thereafter, a surgical approach was discussed and indicated, which was not accepted by the patient.

For a better treatment of coronary artery disease in a catheterization lab, detection of the relative contributions of the epicardial stenosis (ES) and concomitant microvascular disease (MVD) is important. To diagnose ES, fractional flow reserve (FFR), the hyperemic stenosis resistance index (hSRv) and to diagnose MVD, hyperemic microvascular resistance index (hMRv) have been tested in cath labs. However, for concurrent assessment of ES and MVD, functional parameter utilizing flow and pressure values, pressure drop coefficient (CDP) and combined functional and anatomical parameter, lesion flow coefficient (LFC) are defined. To test the ability of CDP and LFC to account for ES and MVD, they were correlated with the hSRv and hMRv. We hypothesize that CDP and LFC will have a better combined correlation with hSRv and hMRv. Simultaneous pressure and flow readings were obtained in 11 Yorkshire swine. Single and multiple linear regression analyses were conducted between the FFR, CDP and LFC vs hSRv and hMRv. The correlation coefficient (r) was used to check the strength of correlation. The individual correlation between hSRv and hMRV with CDP (r = 0.90; r = 0.78) and LFC (r = 0.89; r = 0.95) was stronger compared to FFR (r = 0.63; r = 0.32). The combined correlation between hSRv and hMRv with CDP (r = 0.95) and LFC (r = 0.95) increased from the individual correlation. Therefore, we conclude that CDP and LFC can diagnose ES and MVD concurrently and might prove to be improved diagnostic parameters than FFR.

Abstract:
This report presents the results of RTI International Education’s study on teachers' guides across 13 countries and 19 projects. Using quantitative and qualitative methods, we examine how teachers’ guides across the projects differ and find substantial variation in the design and structure of the documents. We develop a scripting index so that the scripting levels of the guides can be compared across projects. The impact results of the programs that use teachers’ guides show significant impacts on learning outcomes, associated with approximately an additional half year of learning, showing that structured teachers’ guides contribute to improved learning outcomes. During observations, we find that teachers make a variety of changes in their classroom instruction from how the guides are written, showing that the utilization of structured teachers’ guides do not create robotic teachers unable to use their own professional skills to teach children. Unfortunately, many changes that teachers make reduce the amount of group work and interactivity that was described in the guides, suggesting that programs should encourage teachers to more heavily utilize the instructional routines designed in the guide. The report includes a set of research-based guidelines that material developers can use to develop teachers’ guides that will support effective instructional practices and help improve learning outcomes. The key takeaway from the report is that structured teachers' guides improve learning outcomes, but that overly scripted teachers' guides are somewhat less effective than simplified teachers' guides that give specific guidance to the teacher but are not written word for word for each lesson in the guide.
