Acute kidney injury (AKI) resulting from toxic or ischemic insults and usually affect the tubular portion of the nephron, the use of potentially nephrotoxic drugs such as gentamycin one of the main causes. Early detection allows the AKI appropriate intervention may prevent or mitigate damage to the tubular cell and the development of acute renal failure (ARF). Diagnosis requires a combination of techniques, and be effective employment ultrasonography associated with laboratory tests. Ultrasound allows assessment of morphology and renal echogenicity by taking on bi-dimensional mode (B-mode) and renal hemodynamics through the assessment with pulsed Doppler with resistivity and pulsatility indices calculation. In this study, AKI was induced in six dogs with 30mg/kg of gentamicin once a day, for 10 days, in order to confirm the importance of B-mode and pulsed Doppler ultrasound for the diagnosis, monitoring and prognosis determination. Dogs were followed for 45 days by clinical examination, urinalysis, serum urea and creatinine, urinary GGT dosing and ultrasound. Six healthy dogs were used as control group and followed the same moments. The results of this study demonstrated that ultrasound is earlier than changes in laboratory diagnosis of ARI, and is useful for monitoring and prognosis determination, since the ultrasound aspect returned to normal in most dogs after recovery and dogs with ARF had higher values of the volume, of the Doppler indices and the degree of echogenicity.
A lesão renal aguda resulta principalmente da ação de agentes nefotóicos e de isquemia renal que geralmente afetam a porção tubular do néfron, sendo o uso de medicamentos potencialmente nefrotóxicos, como a gentamicina, umas das principais causas. A detecção precoce da lesão renal aguda permite a intervenção apropriada podendo prevenir ou atenuar o dano à célula tubular e o desenvolvimento da insuficiência renal aguda (IRA). O diagnóstico requer a associação de técnicas, sendo eficaz o emprego do exame ultrassonográfico associado aos exames laboratoriais. A ultrassonografia permite a avaliação da morfologia e da ecogenicidade renal por meio do exame em modo bidimensional (modo B) e da hemodinâmica renal por meio da avaliação com o Doppler pulsado pelo cálculo dos índices de resistividade e pulsabilidade. No presente estudo a lesão renal aguda foi induzida em seis cães, com o uso de 30mg/kg de gentamicina uma vez ao dia, durante dez dias, com o objetivo de confirmar a importância da ultrassonografia em modo B e Doppler pulsado para o diagnóstico, acompanhamento e determinação do prognóstico. Os cães foram acompanhados durante 45 dias por meio de exame clínico, exame de urina, dosagem sérica de ureia e creatinina, dosagem de GGT urinária e exame ultrassonográfico. Seis cães saudáveis foram utilizados como grupo controle e acompanhados nos mesmos momentos. Os resultados deste estudo demonstraram que a ultrassonografia foi capaz de diagnosticar a lesão renal aguda mais precocemente que as alterações laboratoriais, além de ser útil para o acompanhamento e determinação do prognóstico, uma vez que o aspecto ultrassonográfico voltou à normalidade na maioria dos cães após a recuperação e, os cães que desenvolveram IRA apresentaram valores mais elevados do volume, dos índices Doppler e do grau de ecogenicidade.

Abstract:
Numerous studies suggest that modulation of the cholinergic system through the use of nicotinic agonists can improve motor function in humans or animals with motor disorders. Specifically, although there are no approved therapeutics for patients with ataxia, the nicotinic receptor agonist varenicline has demonstrated efficacy to improve coordination and gait in several groups of patients with different subtypes of ataxia. Importantly, the mechanism underlying the varenicline’s mechanism of action to improve motor function remains to be elucidated. Thus, the purpose of these experiments was to first quantify gait impairments in rats with olivocerebellar ataxia utilizing an objective treadmill-based system to investigate temporospatial aspects of animals’ gait. These results were used to calculate an index that characterizes deviations from ‘normal’ gait, as similarly employed in clinical studies. The translational validity of this method of gait assessment was investigated by comparing gait impairments between these animals and those reported for humans with ataxia. It was next investigated whether varenicline could attenuate any gait impairments and thus improve motor functioning in these animals, as suggested by clinical findings. Finally, varenicline’s mechanism of action was investigated by attempting to block its effects by pretreating animals with the nicotinic antagonist mecamylamine. Thus, these studies demonstrate the involvement of nicotinic acetylcholine receptors in the mechanism of varenicline’s effects to improve motor functioning. Moreover, these results provide translational methods by which the efficacy of other, more selective nicotinic agonists to improve motor functioning can be tested preclinically prior to their use in humans with ataxia.

Abstract:
The global financial crisis has had a strong impact on most European countries. While at first the massive injection of resources helped the financial system not to collapse, the austerity policies implemented throughout the continent have not brought the expected economic growth. Interestingly, the type of intervention undertaken is rooted in the neoclassical tradition, which entails a specific understanding of the functioning of the financial system and the economy. In this context, the objectives of this paper are firstly, to analyze the main theoretical assumptions and the specific way in which the crisis is understood and the applied economic policies are developed under this tradition; and secondly, to account for the social and economic impact of the policies applied, and the different margins of action that each European country still has.
La crisis financiera global ha tenido un fuerte impacto en las economías europeas centrales y fundamentalmente en las periféricas. Si bien en un primer momento, la masiva inyección de recursos logró que el sistema financiero no colapse, las políticas de austeridad aplicadas no han podido consolidar las dinámicas de crecimiento esperadas. Lo interesante del tipo de intervención desarrollada, es que las medidas han sido justificadas en base a una serie de trabajos teóricos que presentan una visión particular del funcionamiento del sistema financiero y de la economía. Eneste contexto, los objetivos del presente trabajo son por un lado, analizar los principales supuestos teóricos y modo específico de entender a la crisis que han fundamentado las políticas económicas aplicadas; y por el otro, dar cuenta del impacto económico y social de estas medidas intentando dar cuenta los diferentes márgenes de acción que aún hoy poseen los países europeos.

Las leishmaniasis representan un serio problema de salud, con alrededor de 12 millones de casos a nivel mundial. Amenaza a 350 millones de mujeres, hombres y niños en 88 países del mundo, de los cuales 72 se encuentran en vías de desarrollo(3). En humanos, la leishmaniasis visceral causa parasitismo y patología importante en hígado, bazo y médula ósea y el 90% de la leishmaniasis visceral se dan en Bangladesh, Brasil, India, Nepal y Sudan. Según la OMS se estima que el 90,0% de los casos de leishmaniasis muco cutánea se presentan en Brasil, Bolivia y Perú; el 90,0% de los casos de leishmaniasis cutánea se encuentran en Afganistán, Brasil, Irán, Perú, Arabia Saudita y Siria(2,9). En Bolivia, aproximadamente 800,000 personas están en alto riesgo de infectarse(1) y se han reportado 2438 casos de leishmaniasis con una incidencia de 37.3%(1,4). Los parásitos asociados con leishmanisis en nuestro país son: L. braziliensis, L. amazonensis, L, chagasi y L. guyanensis. Las regiones endémicas de estas especies de leishmania son La Paz, Cochabamba, Santa Cruz, Beni, Pando y Tarija.

La tesis de este trabajo es que las predicciones sobre la dirección del desarrollo humano basándose en un solo evento ocurrido durante los primeros años de la vida generalmente tienen una débil validez interna. Las limitaciones de dichas predicciones no se deben a las limitaciones inherentes a los estudios que han puesto a prueba la validez de la predicción sino a una conceptualización errada sobre la naturaleza misma del desarrollo. Este no está determinado por efectos principales sino por complejas relaciones recíprocas entre los diferentes componentes del organismo y entre este y el ambiente físico y social. Se propone que hay que modelar el desarrollo como un proceso probabilístico cuya trayectoria se va forjando gradualmente.

El propósito de este estudio fue determinar el nivel de información, la experiencia previa en trauma dentario y la actitud de los padres y profesores frente a los traumas de avulsión y fractura dentaria en niños, se aplicó un cuestionario a 372 profesores y 400 padres del área central de Lima Metropolitana; se halló un regular nivel de información de padres y profesores, una actitud indecisa de padres y profesores frente a la avulsión, una actitud indecisa de los padres y una actitud adecuada de los profesores frente a la fractura dentaria, el 36,3% de los padres y el 29,6% de los profesores ha tenido experiencia previa en trauma dentario, la ocupación de los padres influye en su nivel de información, se observó que a mayor desarrollo profesional de los profesores aumenta el nivel de información sobre el trauma, el 68% de los padres y el 71.8% de los profesores no han recibido información de primeros auxilios dentales de ninguna fuente, a mayor edad aumenta la proporción de padres que tienen una actitud adecuada frente a la avulsión, el 81,8% de los padres con estudios superiores tienen una actitud indecisa frente a la avulsión; existe asociación estadísticamente significativa entre el nivel de información y la actitud de los padres y profesores frente al trauma de avulsión-fractura dentaria en niños, pero no existe asociación estadísticamente significativa entre la experiencia previa y la actitud de los padres y profesores; las campañas educativas son necesarias para mejorar los primeros auxilios dentales brindados por las personas inexpertas
The purpose of this study was to determine the level of information, the previous experience in dental trauma and the attitude of the parents and teachers about the emergency management of dental avulsion and tooth fracture in children, it was applied to a questionnaire to 372 teachers and 400 parents of Metropolitan the central area of Lima; results showed regulate level of information of parents and teachers, an uncertain attitude of parents and teachers on the management for an avulsed tooth, an uncertain attitude of the parents and an appropriate attitude of the teachers on the management of tooth fracture, 36,3% of the parents and 29,6% of the teachers have had previous experience in dental trauma, the occupation of the parents influences in its level of information, was observed that to greater professional development of the teachers it increases the level of information on the trauma, 68% of the parents and the 71,8% of the teachers have not received information of dental first aid of any source, to greater age increases the proportion of parents who have an appropriate attitude on the management for an avulsed tooth, the parents with great level education (81.8%) have an uncertain attitude on the management for an avulsed tooth; there was statistically significant association between the information level and the attitude of the parents and teachers on management of dental avulsion and tooth fracture in children, there was no statistically significant association between the previous experience and the attitude of the parents and teachers; educational campaigns are necessary to improve dental first aid of people lay

Abstract:
En el presente trabajo se estudió el efecto cicatrizante del quitosano, obtenido por desacetilación a partir de quitina de los caparazones del cangrejo de la variedad Cancer cetosus “Cangrejo Peludo”, bajo la forma farmacéutica de ungüento a diferentes concentraciones. Para ello se aisló la quitina de los caparazones de cangrejos, se obtuvo el quitosano por desacetilación y se identificó por método espectrofotométrico (IR), solubilidad y determinando el grado de desacetilación mediante titulación potenciométrica y posteriormente se formuló el ungüento. Para la determinación del efecto cicatrizante se utilizó el test de cicatrización descrito por Howes y col, para heridas incisas, en el cual se emplearon 48 ratones albinos hembras de la especie Mus músculos de 1 mes y medio de edad, con un peso dentro del rango de 25 a 33g. Las muestras se administraron cada 12 horas por un periodo de 72 horas, al término del cual se cuantificó la resistencia que ofrecían las heridas tratadas, comparándolas con sus respectivos controles y se realizaron cortes histológicos para observar el grado de evolución histológica del proceso de cicatrización. El quitosano, obtenido por desacetilación a partir de quitina presenta mayor efecto cicatrizante bajo la forma farmacéutica de ungüento al 0,25% (79,28% efecto cicatrizante) en comparación con el grupo control (base del ungüento) se concluye que en las condiciones de ensayo el quitosano posee un significativo efecto cicatrizante
In the present work the wound healing effect of the chitosan was studied, the chitosan obtained for desacetylation of chitin from the crab´s shells of the variety Cancer cetosus "Shaggy Crab ", under the pharmaceutical form of unguent at different concentrations. For it there was isolated the chitin from the shell of crabs, the chitosan was obtained for desacetylation and identified by spectrophotometric method (IR), solubility, determining the degree of desacetylatión by way of potentiometric qualifications and later the unguent was formulated. For the determination of the wound healing effect we used the test of wound healing described by Howes and col., for incision wounds; we used 48 female albino mice of the species Mus músculos of 1 month and a half of age, with a weight inside the range of 25 to 33g. The samples were administrated every 12 hours for a period of 72 hours., at the conclusion of the experiment the resistance that the treated wounds were offering, was quantified verifying them with their respective controls and histological cuts were realized to observe the degree of histological evolution of the process of wound healing. The chitosan, obtained by desacetylation from Chitin presents major wound healing activity under the pharmaceutical form of unguent at 0,25% (79,28% wound healing effect) in comparison with the group control (base of the unguent), it is concluded that under the test conditions chitosan possesses a significant healing wound effect

Abstract:
This paper examines the anti-terrorist strategy developed by Peru from the paradigm of the constitutional rule of law. One expression of this counter-insurgency fight was the nearly permanent establishment of states of emergency and the adoption of criminal emergency laws, which are critically analyzed, because they led to serious results on fundamental rights. In that sense, it is shown, with severe concern, the establishment by the Peruvian Government of legal “black holes” and “gray holes” at the expense of victims of enforced disappearance. The author concludes by assessing the position of the courts and the challenge they have in elaborating a democratic and constitutional model for combating terrorism.
El presente trabajo examina, desde el paradigma del Estado constitucional de derecho, la estrategia antiterrorista que desarrolló el Perú. Una de las expresiones de esta lucha antisubversiva fue la instauración casi permanente de los regímenes de excepción y la aprobación de las leyes penales de emergencia, las cuales son analizadas críticamente, pues propiciaron graves afectaciones a los derechos fundamentales. En ese sentido, se muestra, con  grave  preocupación,  la  creación por  parte  del  Estado  peruano  de«agujeros negros» y «agujeros grises» legales en desmedro de las víctimas de desaparición forzada. Finaliza el autor haciendo un balance de la postura de los órganos jurisdiccionales y el reto que tienen en la construcción del modelo democrático y constitucional de lucha contra el terrorismo.

Most women who desired women in the period 1910-1965 did not have the identity categories “lesbian” and “bisexual” available to them. Even in this linguistic vacuum, however, many were able to explore same-sex relationships, to engage in physical sexual activity with women, and even to form community on the basis of same-sex desire and behaviour. How were they able to understand themselves in relation to the homophobic world in which they lived? This dissertation examines the lives of lesbians and bisexual women in English Canada between 1910 and 1965, focusing particularly on the formation of subjectivity in relation to same-sex desires, relationships with partners and families of origin, sexual practices, and community. An analysis of oral testimonies, of journals, and of love letters shows that particular life events—the first awareness of same-sex attraction, physical exploration of that attraction, the finding of a language with which to describe same-sex desires and relationships, the first important same-sex relationship, and the finding of community—served as turning points in the formation of subjectivity. The story of that journey was later expressed as a linear and essentialist “coming out” narrative in which the individual triumphed over homophobia and ignorance and discovered her true self. That narrative structure is both understandable in the context of essentialist definitions of sexual orientation and a politically necessary one, given the need for a single identity category under which to campaign for legal and social recognition. The two dominant formulations of same-sex relationships between women before 1965—the “romantic friendship” and the “butch-femme relationship”— have obscured and made culturally unintelligible the lives of lower middle-class lesbians and bisexual women who were neither politically active nor fighting publicly for urban lesbian space. This dissertation analyses the lives of this neglected group of women and argues that their subjectivities were constructed not only in relation to sexual attraction, but also in relation to class. Middle-class ideas of respectability and an antagonism to bar culture resulted in the formation of class-specific lesbian subjectivities. This dissertation also suggests that women in same-sex relationships before the allegedly more liberal decades of the late twentieth century may actually have had slightly better relationships with families of origin than would later be the case. Greater adherence to notions of duty and obligation, fewer economic opportunities enabling women to live independently of family, the lack of a publicly available discourse of pathology with which families could define and reject their wayward daughters, and the lack of later notions of “alternative” lesbian families and community meant that many remained rather closer to their families than would lesbians after 1965.

O objectivo desta dissertação prende-se com a descrição e estudo da amostra de 50 esqueletos não-adultos dos vestígios antropológicos exumados do Largo do Convento do Carmo, em Lisboa, que datam do período entre o século XVI a meados do século XVIII. O que implicou a quantificação de número de peças ósseas por indivíduo através do registo do Índice de Conservação Anatómica (ICA) e do estado geral da amostra (ICAG). O valor calculado de 27,77% apresentou-se bastante baixo. Foi realizado o estudo dos parâmetros paleodemográficos, na estimativa da idade à morte estimou-se a idade à morte em 40 dos indivíduos da amostra e, na determinação sexual aplicou-se o método a 14 indivíduos. Na observação morfológica (não-métrica) foram observados os caracteres discretos cranianos e pós-cranianos mas sem grande expressão. Nos indicadores de stresse ou condicionamentos de crescimento observou-se a criba orbitalia, a hiperostose porótica mas, foi nas hipoplasias lineares do esmalte dentário que se verificou uma prevalência maior de 36%. Por fim, na patologia oral observaram-se as cáries, o desgaste dentário e o tártaro. A prevalência de cáries foi de 6,73% e de 681 dentes observados tinham cáries cavitadas 11 dentes.
The purpose of this dissertation was to make a description and study of a sample of 50 non-adult skeletons excavated from the Largo do Convento do Carmo, in Lisbon (16th to mid 18th century). It implied the quantification of bone pieces per individual in order to attain the anatomical conservational index, which was quite low 27,77%. It was also carried out a study of paleodemographic parameters, such as age at death which was obtained in 40 individuals, in the determination of sex the method could only be used on 14 individuals. On the non-metrical morphological approach was registered the epigenetic traits of the skull and of the axial skeleton but they did not show much expression. Regarding non-specific stress indicators, criba orbitalia and porotic hiperostoses was registered, but it was the linear enamel dental defects that with 36% prevalence stood out. Last but not least, on oral paleopathology was registered dental wear and dental calculus, but the prevalence went to 6,73% on carious lesions. Dental wear and dental calculus prevalence was very low and non-expressive.

Foi avaliada a reprodutibilidade entre examinadores do método de avaliação do Índice Funcional do Ciático (IFC), medido por um programa de computação desenvolvido para este fim. Foram empregados 20 ratos Wistar, cujo nervo ciático direito era abordado sob anestesia geral e esmagado num segmento de 5 mm proximal à sua trifurcação com um dispositivo especial, com carga fixa de 5 kgf por 10 minutos. Impressões das pegadas dos animais foram obtidas na fase pré-operatória e, depois, semanalmente, da 1ª à 8ª semana pós-operatória, em uma pista de marcha. As impressões eram digitalizadas, armazenadas e avaliadas, pela medida de parâmetros pré-determinados, por quatro examinadores, seguindo sempre a mesma seqüência de marcação dos parâmetros. Os resultados foram submetidos à análise estatística, que mostrou haver um alto índice de correlação entre examinadores na avaliação pré-operatória e nas 3a, 4a, 5a, 7a e 8a semanas (igual ou maior que 0,82), com queda casual na 6a semana, mas manteve significante como as demais (pF?0,01). Na 1a e 2a semanas, o índice de correlação foi próximo de zero, mostrando a pouca reprodutibilidade do método nesse período, em que a variabilidade entre os animais não diferiu da variabilidade entre os examinadores (pF?0,24 e 0,32, respectivamente), devido à pouca definição das impressões das pegadas.
The Sciatic Functional Index (SFI) is a quite useful tool to evaluate functional recovery of the sciatic nerve of rats in a number of experimental lesions and treatment. Although being an objective method, it depends on the examiner?s ability to adequately recognize and mark the previously established footprint landmarks, which is an entirely subjective step, thus potentially interfering with the calculations according to the mathematical formulae proposed by different authors. An inter-personal evaluation of the reproducibility of an SFI computer aided method was then carried out here to study data variability. A severe crush injury was produce on a 5 mm-long segment of the right sciatic nerve of 20 Wistar rats (5kgf load directly applied for 10 minutes) and the SFI was measured by four different examiners (2 experienced and 2 new-comers) either preoperatively and at weekly intervals from the 1st to the 8th postoperative week. Three measurements were made for each print and an average was drawn and used for the statistical analysis. The results showed that interexaminer correlation was high (0.82) on the 3rd, 4th, 5th, 7th and 8th weeks, with an unexpected (pF?0.01) drop on the 6th week. There was virtually no inter-personal correlation (correlation index nearly 0) on the 1st and 2nd weeks, period in which the variability between animals (pF?0.24) and examiners (pF?0.32) was similar, certainly due to the poor definition of the footprints. The authors conclude that the SFI method studied here is only reliable from the 3rd week on.

Foi realizado um estudo experimental da influência do ultra-som terapêutico na regeneração do nervo ciático submetido a esmagamento controlado, em ratos. Foram empregados no estudo 20 ratos da linhagem Wistar, de peso médio em torno de 250 g, divididos em dois grupos, conforme o tipo de procedimento realizado: 1) somente esmagamento, dez ratos; e 2) esmagamento e irradiação com ultra-som, 10 ratos. Sob anestesia, o nervo ciático era exposto na coxa direita do animal e esmagado com um dispositivo especialmente idealizado e confeccionado para essa finalidade, com uma carga fixa de 15 Kg, por 10 minutos, num segmento de 5 mm proximal à sua bifurcação. A irradiação com o ultra-som pulsado (1:5, freqüência de 1 MHz, intensidade de 0.4 W/'CM POT.2', duração de 2 minutos) era iniciada já no primeiro dia pós-operatório e realizada por dez dias consecutivos. Os resultados foram avaliados pela análise funcional da impressão plantar dos animais obtidas numa pista de marcha e determinação do índice funcional do ciático (De Medinaceli e Bain, Mackinnon e Hunter), e pela morfometria, através do cálculo da densidade de fibras nervosas, após sacrifício dos animais no vigésimo primeiro dia pós-operatório. Os resultados foram submetidos a análise estatística e mostraram que, nas condições do trabalho, o ultra-som acelerou a regeneração do nervo, demonstrável com significância no vigésimo primeiro dia pós-operatório
An experimental study of the influence of therapeutic ultrasound on the regeneration of the sciatic nerve submitted to controlled crush injury was carried out in rats. Twenty female Wistar rats weighing 250 g on average were used and distributed in two groups according to the procedure: 1) crush injury only, ten rats; 2) crush injury followed by ultrasound irradiation, ten rats. Under general anesthesia the sciatic nerve was exposed on the right thigh and crushed with a device especially devised and built for that purpose, with a 15 kg constant load for 10 minutes, affecting a 5 mm-long segment of the nerve proximal to its bifurcation. Pulsed ultrasound irradiation (1:5, 1 MHz frequency, 0.4 W/'CM POT.2' intensity, 2 minutes duration) was started the day after the operation and repeated for ten consecutive days. The results were evaluated by functional footprint analysis and determination of the sciatic functional index (DeMedinaceli's and Bain, Mackinnon and Hunter's methods) at weekly intervals, and by morphometry (nerve fiber density) of the resected sciatic nerve after killing the animals on the 21st postoperative day. Results were submitted to statistical evaluation and showed that, in the conditions of the study, ultrasound accelerated nerve regeneration, as demonstrated with significance on the 21st postoperative day

Abstract:
Objectives: To explore the reliability of frozen sections to diagnose prostate cancer (PCa) and to describe surgical steps of a 3D magnetic resonance imaging (MRI)– ultrasound (US)-guided prostate biopsy (PB) and focal cryoablation of the index lesion in a single setting procedure. Patients and Methods: Patients with suspicious PCa, based on prostatic specific antigen (PSA) value and on a PIRADS 4 or 5 single lesion, as well as the steadfastness of avoiding any kind of radical treatment, were considered for enrolment. IRB and written informed consent were obtained from the patients. The entire procedure was performed transperineally, in two consecutive surgical phases: 3D MRI–US-guided plus systematic template PB and real-time TRUS-guided focal cryoablation. Three cores were taken from the index lesion (one for frozen section and two for final pathology), three cores from the surrounding area and systematic sampling was performed for the rest of the gland. Focal cryoablation of the index lesion was performed once confirmation of PCa was obtained by means of frozen sections. Follow-up schedule included PSA test at 3-mo interval, MRI 3-mo and 1-yr postoperatively and prostate biopsy of the treated area at 1-yr. 31 Results: This report includes 14 patients with a minimum follow up time of 12 months. All patients were potent before treatment, complained no severe low urinary tract symptoms and denied consent to any radical treatment. PCa diagnosis was histologically confirmed in all patients by frozen sections. All other cores were negative. At final histology, there was a Gleason score upgrade in three patients, from 3+3 to 3+4. The postoperative course was uneventful and all patients were discharged on the first postoperative day. Mean PSA value decreased from 6.37 (baseline) to 0.83 ng/mL at 3-mo evaluation. Three-mo postoperative MRI images showed complete ablation of the index lesion in all patients. Urinary continence and erectile function were preserved in all patients, without clinically meaningful changes at EPIC questionnaire. At one-yr follow-up, eleven patients showed no signs of persistent or recurrent disease at MRI imaging and treated area biopsies; three patients had a suspicious area at MRI and they needed treatment for confirmed disease at biopsy. Conclusion: Single setting 3D MRI–US-guided frozen section and focal cryoablation of the index lesion could represent a step forward towards a “patient-tailored” minimally invasive approach to diagnosis and cure of low and intermediate risk PCa.

OBJECTIVE: To investigate differential item functioning or item bias of the Rivermead Mobility Index (RMI) and its impact on the drawing of valid comparisons with the RMI between subgroups of patients after stroke who differ with respect to age, sex, or side of lesion. DESIGN: Cross-sectional study. SETTING: A rehabilitation center in the Netherlands and 2 stroke rehabilitation units and the wider community in the United Kingdom. PARTICIPANTS: The RMI was completed for patients undergoing rehabilitation after stroke (N=620; mean age +/- SD, 69.2+/-12.5y; 297 [48%] men; 269 [43%] right hemisphere lesion, and 304 [49%] left hemisphere lesion). INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Mokken scale analysis was used to investigate differential item functioning of the RMI between subgroups of patients who differed with respect to age (young vs older), sex (men vs women), and side of stroke lesion (right vs left hemisphere). RESULTS: No differential item functioning was found for any of the comparison subgroups. CONCLUSIONS: The RMI allows valid comparisons to be made between subgroups of patients undergoing rehabilitation after stroke who differ with respect to age, sex, or side of lesion.

IntroduçãoO diagnóstico do carcinoma da próstata baseia-se na combinação da determinação do Antígeno Prostático Específico (PSA), toque retal e análise histopatológica de biópsia prostática sistemática, mas a baixa sensibilidade desta estratégia exige a repetição da biópsia, aumentando a iatrogenicidade. Por outro lado, a taxa de deteção de tumores sem significado clínico é ainda elevada. A ressonância magnética (RM) multiparamétrica está indicada, pela EAU, no estadiamento do carcinoma da próstata e demonstra bons resultados na deteção e orientação da biopsia. Para uniformizar os relatórios dos achados da técnica, foi lançado o score PI-RADSv2. Contudo, é uma técnica de aplicação recente, que carece de estudos e aperfeiçoamento. O objetivo deste estudo será avaliar a capacidade da ressonância magnética multiparamétrica na deteção e localização do carcinoma da próstata. Materiais e métodosNo estudo retrospetivo foram incluídos 30 indivíduos com carcinoma da próstata, sujeitos a ressonância magnética multiparamétrica previamente à prostatectomia radical. As localizações das lesões suspeitas na imagiologia e lesões tumorais na histopatologia foram assinaladas numa grelha de mapeamento de 24 regiões e comparadas. Foram estimadas a sensibilidade, especificidade, acuidade e os valores preditivos positivo e negativo para a capacidade de deteção de tumor em cada hemipróstata, quarto de secção e octante de secção, bem como da lesão índex, para a qual foi estimada a concordância  para a sua localização. Por fim, foi elaborada uma regressão logística para identificar a influência das covariáveis PSA sérico, grau ISUP, dimensão máxima e estadiamento T na deteção da lesão índex.ResultadosOs 30 doentes tinham 37 focos tumorais (80% grau Gleason ≥ 7) e 14,6% (105/720) dos setores foram positivos. A sensibilidade da RM multiparamétrica nas hemipróstatas, quartos de secção e oitavos de secção foi 77,8%, 52,27% e 51,53%, respetivamente. A sensibilidade foi superior no grupo de doentes com grau ISUP ≥ 3 e tamanho ≥ 1cm (60% vs. 51,52% e 59,46% vs. 46,15%, respetivamente). A lesão índex foi detetada em 80% dos casos e a sensibilidade foi de 92,31%. Reportou-se uma elevada especificidade em todas as avaliações (80% - 98,13%). 90% dos estadiamentos T3 foram detetados. A Dimensão Máxima foi a única covariável significativamente preditora da capacidade de deteção da lesão índex. ConclusãoA ressonância magnética multiparamétrica evidenciou uma boa performance na deteção de tumores da próstata >10 mm e com grau ISUP >3. Demonstrou-se útil no diagnóstico, estadiamento e planeamento de uma possível prostatectomia e potencialmente relevante no suporte à realização de terapias de ablação focal e na seleção dos doentes com indicação para tal.
IntroductionThe Prostate cancer (PCa) diagnostic pathway includes the PSA determination, digital rectal examination and the histopathological study after the systematic biopsy. Although it is the most common procedure, it is has a weak performance as it commonly leads to biopsy repetition, increasing iatrogenicity. In the other hand, there is an high percentage of low clinical significance tumors detected. The multiparametric prostate MRI (mp-MRI) is the EAU recommendation for staging prostate cancer and there is evidence of a good performance on detecting and staging prostate cancer and on biopsy orientation. Despite the benefits of the technique, it is relatively recent and only available in a few experienced medical centers. While the PI-RADSv2 classification makes efforts towards the standardization of the results, there is also the acknowledgement that these guidelines are still on work and will need more evidence base refinement. The main purpose of this study is to evaluate the multiparametric magnetic resonance’s performance on the detection and localization of the Prostate Cancer. Materials and methodsThis is a retrospective study of 30 PCa patients who underwent mp-MRI before radical prostatectomy. The suspicious lesions detected on the mpMRI images were localized using a previously standardized map of 24 regions of interest (ROI) per prostate and compared with the pathologic findings. Sensivity, specificity, accuracy, positive and negative predictive values were calculated. Index lesion detection was also evaluated. Index lesion localization and tumoral size estimate concordances were measured using Cohen’s Kappa. A binomial multivariate regression was performed for the covariates PSA, ISUP grade, maximum dimension and T staging, being the dependent variable the index lesion detection.ResultsThirty seven tumoral foci ( 80% Gleason grade ≥7) and 105 on 720 ROIs were positive. Hemiprostate, 12 ROI and 24 ROI sensivity evaluation were 77,8%, 52,27% and 51,53%, respectively. Superior sensivity was found with ISUP grade ≥3 and size ≥1cm (60% vs. 51,52% and 59,46% vs. 46,15%,respectively). Specificities were high (80% - 98,13%). Index lesion detection rate was 80% and sensivity 92,31%. T3 staging detection reached 90%. The maximum dimension was the only significant predictive covariate for the index lesion detection capacity. ConclusionMp-MRI has a good performance on detecting PCa with ISUP grade >3 or size >1 cm and a high sensivity for index lesion detection and localization. Therefore, besides being useful on PCa staging, it might be relevant for PCa diagnosis and therapeutical management, as it may improve radical prostatectomy planning and is probably the best tool to guide the focal therapy.

Valor Preditivo de Marcadores de Transição Epitelial-­‐Mesenquimatosa, PTEN, ERG, Ki67 e de Estudo Genético na Recorrência Bioquímica de Carcinoma da Próstata tratado com cirurgia Introdução: O carcinoma da próstata apresenta uma elevada incidência e prevalência. A maioria dos carcinomas da próstata localizados são curáveis, com as opções terapêuticas disponíveis. Contudo, a recorrência da doença está associada a morbilidade, a mortalidade e a custos elevados. Como tal, é importante otimizar o tratamento da doença localizada. Uma das possibilidades para o fazer, passa por uma melhoria do nosso conhecimento, acerca dos factores preditivos. Um reconhecimento precoce de uma maior probabilidade de recorrência poderia levar a uma otimização da abordagem terapêutica dos tumores primários, nomeadamente através de estratégias que incluam intensificação terapêutica. Material e Métodos: Numa amostra populacional constituída por doentes, com carcinoma da próstata localizado, e tratados com prostatectomia radical de intuito curativo, avaliámos o valor preditivo de factores clínicos, anatomopatológicos, imunohistoquímicos e genéticos. Utilizámos como “endpoint” a recorrência bioquímica (RBQ). Resultados: Em 147 doentes, e após um seguimento, cuja mediana foi de 20 meses (4-­‐55 meses), ocorreu RBQ em 15,6% dos casos. Na nossa amostra, o PSA pré-­‐ operatório (p=0,02); o tipo de cirurgia (p=0,01); o score de Gleason da biópsia (p=0,004) e da lesão índice (P<0,001); a classificação de Epstein da biópsia (p=0,005), da peça (p=0,006), e da lesão índice (p<0,001); o número de fragmentos de biópsia envolvidos por tumor (p=0,05), a extensão do envolvimento tumoral nos fragmentos de biópsia, em milímetros (p=0,01) e percentualmente (p=0,02); a existência de áreas de diferenciação intraductal (p=0,003); a presença de padrão de terciário (p<0,001); a dimensão linear da lesão índice (p=0,005); a existência de margem cirúrgica positiva (p=0,05); a existência de extensão extraprostática (p=0,02); a invasão vascular (p=0,002); a invasão das vesículas seminais (p=0,01); o número de gânglios isolados em doentes submetidos a linfadenectomia (p=0,01); o estádio patológico referente ao tumor primário (p=0,02) e a pErda de expressão da caderina E (p<0,001), mostraram significância estatística relativamente à ocorrência de recorrência bioquímica. Por análise multivariável, apenas a existência de áreas de diferenciação intraductal (p=0,01), a classificação de Epstein da lesão índice (p<0,001), a invasão vascular (p=0,02) e o número de gânglios isolados em doentes submetidos a linfadenectomia (p=0,05), mantiveram a significância estatística, após ajuste para as outras variáveis. Os restantes biomarcadores associados à transição epitelial-­‐mesenquimatosa (caderina N e vimentina), o PTEN, o Ki-­‐67 e o ERG, na nossa amostra, e por análise univariável, não estão associados à ocorrência de RBQ. A expressão de SChLAP1, quer em tecido tumoral, quer em tecido não tumoral, também não está associado à ocorrência de RBQ. Conclusões: O nosso trabalho confirma, na nossa população, o interesse preditivo de factores clínicos e anatomopatológicos normalmente associados a um comportamento biológico mais agressivo. Também sugere o interesse preditivo de alguns factores anatomopatológicos menos estudados. Por fim aponta para a possibilidade, de associação entre o fenómeno de transição epitelial e a ocorrência de RBQ.
Predictive value of epithelial-­‐mesenchymal transition markers, PTEN, ERG, Ki67, and genetic analysis in biochemical recurrence of prostate cancer treated with surgery Introduction: Prostate cancer is a contemporary health issue. It has a very high incidence and prevalence. The majority of patients with localized disease are cured with current standard therapeutic options. Nevertheless, disease recurrence is associated with morbidity, mortality and significant costs. It is, therefore, important to optimize therapeutic efficacy. One of the possibilities to achieve it, is through better understanding of predictive factors. Pinpointing in advance, the cases with worse prognosis, could allow a therapeutic adjustment, by adding adjuvant treatment. Methods: In a clinical cohort study, we evaluated the predictive impact of several clinical, pathological and molecular factors in the development of biochemical recurrence (BCR), in patients with localized prostate cancer treated with radical prostatectomy. Results: A total of 147 patients were included in our study. The median follow-­‐up time was 20 months. BCR occurred in 15.6% of the patients. In univariate analysis, pre-­‐operative PSA (p=0.02); the type of surgery (p=0.01); the Gleason score both in biopsy (p=0.004) and in the index lesion (p<0.001); the Epstein grading system in the biopsy (p=0.005), in the entire tumor (p=0.006), and in the index lesion (p<0.001); the number of positive cores in the biopsy (p=0.05), the maximum length of cancer in any one biopsy core (p=0.01) and the greatest percentage of cancer in any biopsy core (p=0.02); the presence of intraductal differentiation in the prostatic specimen (p=0.003); the presence of a minor high-­‐grade pattern (p<0.001); the index lesion dimension (p=0.005); positive surgical margin (p=0.05); the existence of extra prostatic extension (p=0.02); vascular invasion (p=0.002); seminal vesicle invasion (p=0.01); the total number of lymph nodes retrieved in patients submitted to lymphadenectomy (p=0.01); T stage (p=0.02) and loss of E-­‐Cadherine, were all predictive factors for BCR. In multivariate analysis, only the presence of intraductal differentiation (p=0.01), Epstein grading system in the index lesion (p<0.001), vascular invasion (p=0.02) and the total number of lymph nodes (p=0.05) remained independently predictive of BCR. The remaining epithelial-­‐mesenchymal transition markers (N-­‐Cadherine and Vimentine), PTEN, Ki-­‐67 and ERG, were not predictive factors for BCR. Neither was SChLAP1 expression, both in tumoral and non-­‐tumoral tissue. Conclusions: Our study, in this population, confirms the predictive impact of many well-­‐established predictive factors. It also implies, that other less known pathological factors might also have predictive importance. Finally, it suggest the existence of a link between epithelial-­‐mesenchymal transition and BCR.

Abstract:
This thesis reports the results of a comparative analysis of the attitudes held by police recruits about police work, before and after training. By employing an instrumental case study methodology combining quantitative and qualitative techniques, the researcher was able to demonstrate the early stages of development of traditional police values, over the course of basic police training at the Justice Institute of British Columbia. An abundance of police based literature has shown that traditional police tactics are ineffective in reducing crime and that some aspects of the traditional police culture can contribute to police misconduct. As a result, in the later part of the twentieth century community policing has become the philosophy endorsed by police experts and managers across North America. The philosophy of community policing promotes values that are in many cases diametrically opposed to those of the traditional police philosophy. While the Justice Institute of British Columbia and police training institutions across the continent teach police recruits about the community policing philosophy, this research demonstrates that recruits show a closer alignment with the values implicit in community policing before basic training, than they do after training. The researcher sees this distancing of self from the values associated with community policing not as any indictment of the police academy, but as an expression of the dominant discourse of the police culture through the curriculum of the police academy. Through an exploration of the police value system inferred by behaviours and attitudes demonstrated by police recruits, the researcher argues that community policing is resisted not because it is ineffective but because it is unattractive to police officers. Traditional police attitudes and behaviours are promoted in the police culture because they are more suited to the already masculinized nature of the job and are simply more desirable to most police officers than the behaviours emphasized under the community policing model. The researcher believes that for this reason, policing has failed to transform to a model based on the principles of community policing, and that the police culture will continue to resist these changes until the values implicit to the culture are challenged. Changing the values of the police culture must logically start at the recruit training level, but to be effective the researcher believes this change must also involve a process of introspection on behalf of all police practitioners.

Canada and British Columbia have legislation in place to protect gays and lesbians from discrimination based on their sexual orientation. A growing number of BC school districts have policy protecting gays and lesbians. However, some gay and lesbian teachers still hide their sexual orientation. Organizational theory recognizes that formal rules do not define the organization. In-depth interviews with 13 gay and lesbian teachers indicate that they would rather not maintain their classroom closets; but that they needed to know that they would be safe coming out. The methodology followed the tradition of narrative inquiry by collecting stories. Initial recruitment was through email, print, and word-of-mouth advertising. An on-line form was used to filter prospective participants to include urban, rural, Muslim, Catholic, closeted, and politically active participants. The great silence with respect to gays and lesbians in the workforce, paired with a history of negative messages needs to be offset by the frequent and ubiquitous dissemination of positive messages.

Queer Outbursts' investigation of the Vancouver publication concentration (node) contributes to the fields of Canadian literature, queer and lesbian literature, Asian Canadian literature, and women‘s literature through three interwoven tasks. The first two tasks develop and combine node theory and node methodology to produce an original approach to materializing micro-histories minoritarian literatures. The third task demonstrates the nodal approach by materializing a node in Canadian queer women‘s writing centred in the relational geography of Vancouver in the mid-1990s. The queer aesthetics of the novels under consideration are inseparable from the queer bodies and the material contexts that produce them; literary works are not discrete, static creations springing spontaneously from the mind of an inspired isolated writer. Node work reflects this understanding as it oscillates between material, social, and literary analyses and archival fieldwork. The literary and political context of the Vancouver publication node is historicized through a close reading of the 1988 conference, Telling It, which convened authors from First Nations, Asian, and Lesbian communities in the first public and explicit linking of the issues of racialization and sexuality. Social analysis of the node relies on both actor-network theory and Pierre Bourdieu's analysis of cultural production. Literary analysis is focussed on Larissa Lai‘s When Fox Is a Thousand as the primary representative text, and the social analysis is primarily based on the material circumstances of Fox‘s production and distribution. Close reading of Lai‘s novel demonstrates how the political concerns of the enabling communities are taken up literarily. It also demonstrates an inter-nodal connection, through Lai‘s literary strategies that engage the work of Nicole Brossard, which represents another node of Canadian queer women‘s writing circa 1980 and centred in Montréal. Secondary close readings of three other node novels reveal a common ethical interest in community and difference that is expressed through a literary strategy that I have named "literary thirdspace". Shani Mootoo‘s Cereus Blooms At Night, Persimmon Blackbridge‘s Sunnybrook, and Daphne Marlatt‘s Taken each opens to a site of possible literary thirdspace that explores the qualities necessary to live difference productively within community: hybridity, instability, kindness, witnessing, safety, and radical acceptance.

A perda de sensibilidade na zona da pinça do primeiro e segundo dedos causa um importante deficit funcional com grande impacto na qualidade de vida do doente e capacidade de trabalho manual. As transferências nervosas sensitivas cada vez mais são utilizadas para “re-inervar” a face volar destes dedos, com resultados variáveis. Visando a otimização dos resultados e redução do tempo de reinervação, levantou-se a hipótese de se efetuar uma transferência do Ramo Superficial do Nervo Radial (RSNR) para os ramos sensitivos terminais do nervo mediano, distalmente na mão. Para verificar a compatibilidade entre os referidos nervos, foi levado a cabo um estudo de disseção e microdissecção detalhado dos ramos terminais do RSNR – SR1, SR2 e SR3, de dorsal para volar - num membro superior (amostra reduzida devido aos constrangimentos da pandemia COVID-19) e foram colhidas secções nervosas para análise histomorfométrica. A distância entre o Processo Estiloide do Rádio (PER) e o ponto de emergência do RSNR foi de 90 mm (42% do comprimento do antebraço) e entre o PER e a primeira divisão do RSNR de 69 mm (32% do comprimento do antebraço). Entre o PER e as bifurcações do tronco comum SR1-SR2 e do ramo SR2 foram medidos 0 e 20 mm (9% do comprimento da mão), respetivamente. As distâncias entre o PER e os ramos SR1, SR2 e SR3 foram de 0, 0 e 12 mm, respetivamente. As distâncias medidas entre o eixo do segundo osso metacárpico e os ramos SR1 e SR2 no ponto medio-diafisário foram de 1,9 e -3,3 mm. Não foi possível proceder à análise histomorfométrica. Devido ao reduzido tamanho da amostra, não é possível confirmar a exequibilidade da transferência nervosa no nível proposto, tomando como base as caraterísticas anatómicas e histomorfométricas dos nervos em questão.
Upper limb nerve injuries resulting in loss of sensation over the thumb-index pinch represent an important functional deficit impacting on both patient’s daily activities and capacity for “blind” handwork. Sensitive nerve transfers are increasingly being used to restore sensation in the hand and different transfers have been used to re-innervate the pinch grip with variable outcomes. Aiming for the best outcomes and the shortest reinnervation period, we hypothesise that a transfer from the Superficial Branch of the Radial Nerve (SBRN) to the terminal sensitive branches of the median nerve is possible distally in the hand. To check for nerve compatibility, a detailed gross and microdissection study of SBRN’s terminal branches – from dorsal to volar SR1, SR2 and SR3 - was performed in one cadaver extremity (reduced sample due to COVID-19 pandemic constraints) and nerve sections were harvested for histomorphometric analysis. The distance from the Radial Styloid (RSt) to the point of emergence of the SBRN was 90 mm (42% of forearm’s length), that between the RSt and the first division of the SBRN was 69 mm (32% of forearm’s length), and the distances between the RSt and the bifurcations of the SR1-SR2 common trunk and SR2 branch were of 0 and 20 mm (9% of hand’s length), respectively. The horizontal distances between the RSt and the SR1-SR2 common trunk and SR3 branches were 0 and 12 mm. Distances between the axis of the second metacarpal bone and the SR1 and SR2 branches measured at the level of the midshaft were 1,9 and 3,3 mm, respectively. Histomorphometric analysis could not be performed. Even though measurements were in line with other reports, due to sample limitations it is not possible to ascertain whether the proposed nerve transfer is possible based on nerve anatomical and histomorphometric characteristics.

This ethnographic study describes the teaching and learning experiences of private music teachers, students, and students’ parents among the Taiwanese Canadian community in Greater Vancouver, British Columbia, Canada. The following elements are examined in this study: the context of private music lessons of Taiwanese Canadians; the private music teaching and learning experience of Taiwanese Canadians; the cultural beliefs underpinning Taiwanese Canadian private music lessons; and the valuing of private music lessons by Taiwanese Canadians. Five Taiwanese Canadian private music teachers and five of their students and the students’ parents living in Greater Vancouver, British Columbia participated in this research. Data were collected through formal and informal interviews and observations of video-recorded lesson. The findings indicate that the context of Taiwanese Canadian private music lessons is similar to that of the Western private music lessons, a consequence of the emphasis on the talented and Western teaching styles in private and school music education in Taiwan. Cultural values nevertheless have a strong but unacknowledged influence in the private music lessons of Taiwanese Canadians. The Confucian value of obedience to authority and a desire for Western independence are the two seemingly conflicting values that surfaced in the study. Taiwanese Canadians private music teachers teach to remove students’ doubts about music. The students, on the other hand, learn to put the skills and knowledge they have acquired during their private music lessons into practice. In addition, parents were involved in the private music lessons through various interactions associated with “pei.” Engaging their children in private music teaching and learning represents a status symbol for Taiwanese Canadians. Lastly, teaching privately is often a fall back job which provides financial security for private music teachers.

Queer-identified women in Canada and elsewhere are underserved as a community with regard to the provision of support for drug use related problems. In order to provide much needed inclusive support services, researchers, policy makers and treatment providers must recognize and act on the interface of oppression with substance use in populations of queer women. The homophobia and heterosexism endemic to our society is an issue that necessitates the exploration, development, and inclusion of responsive policies and services for queer women who seek - or desire to seek - support for problematic substance use. This research study explores the support needs of queer-identified women who experience difficulties as a result of drug use. Through qualitative, interview-based research, my inquiry examines responses to the question: What are the support needs of women who are impacted by the confluence of heteronormativity and problematic drug use? Data are derived from nine, semi-structured in-depth interviews with women in the Vancouver Island and Lower Mainland areas of British Columbia. The methodological framework incorporates a critical feminist approach. A thematic analysis technique was utilized to analyse the interviews, with data categorized into three primary themes of discrimination, resistance, and support. Findings indicate that queer women require distinct support services for problematic substance use issues in an effort to redress systemic heteronormativity.
