Abstract:
During the last two decades, the attention of many scientists has been attracted by a newly discovered group of regulatory molecules - RNA interference. These RNA molecules are capable of very efficient gene silencing. The main pathways of this mechanism are used by two major types of small noncoding RNAs - microRNA (miRNA) and small interfering RNA (siRNA). These RNAs are a direct product of genes and are able to bind to mRNA molecules and influence their activity. Due to this knowledge, scientists started examining the effect of RNAi in cancer cells and tried to find a way how RNAi can be applied in therapy. The main topic of this thesis is the potential of miRNA replacement to harm cancer cell proliferation. In cancer cells, miRNA levels are downregulated or upregulated, depending on the nature of cancer and miRNA. Restoring levels of miRNAs can lead to elimination of cancer cell by apoptosis or stop cell in cell cycle arrest. miRNA replacement was used first to examine the function of a single miRNA in HeLa cancer cells. The change in the expression level of miRNA treated cells was verified by reverse transcriptase quantitative PCR and the proliferation inhibition was examined using proliferation assays. Abstract:
Introduction: Addition of a second drug used to be a strategy to achieve clinical stabilization of multiple sclerosis in many patients with on-going activity despite monotherapy. Modern immunosuppressive drugs used in monotherapy exert more specific mode of action. Methods: This retrospective observational study evaluated 5-year data from 85 patients with active multiple sclerosis despite monotherapy with either interferon beta or azathioprine, who received add-on azathioprine or interferon beta, respectively. In a subgroup of 23 patients 10- year data were analysed. In a second part of the study, a group of 126 patients switched either from interferon beta or glatiramer acetate to fingolimod was followed-up for one after the change of their treatment and a in a subgroup of 53 patients the 2-year data were assessed. Clinical (relapse frequency, disability) parameters were compared preceding and following the addition of second drug or the switch of treatment. Laboratory results and potential serious adverse events were evaluated in a group of patients with combination therapy. Results: The add-on treatment triggered a drop in annualised relapse rate by approximately 1.5 points sustained over 5 and 10 years. No effect on disability was observed. Title of diploma thesis: Topic and systemic treatment of acne vulgaris Acne vulgaris is a skin disease affecting the hair follicles and sebaceous glands. The disease manifests itself by increased sebum production, non-inflammatory (comedones) and inflammatory lesions (papules, pustules, nodules, cysts). It occurs predominantly in adolescents, but may persist into adulthood. It is a multifactorial disease, which is caused by several factors (internal and external stimuli). The major pathogenetic factors include increased sebum production, hyperkeratosis, P. acnes colonization and inflammation present. First, it is necessary to diagnose the type of acne in order to choose the right and effective therapy, because there is not only one type of acne. There are many types and variants of acne, and although they show similar symptoms (affecting the follicles of sebaceous glands), their cause often differs. There is no uniform classification system for acne vulgaris and it varies between authors. Some authors classify acne vulgaris according to severity into mild, moderate and severe, others into comedonic, papulopustular, nodulocystic...

The aim of this thesis is to study the effect of radiofrequency ablation (isolation of pulmonary veins and modification of the substrate) and following cardiostimulator implantation for preventive purposes on the incidence, duration of atrial fibrillation and quality of life of patients suffering from any of the chronic forms (persisting / permanent) of this type of arrhythmia. 1st Hypothesis: It is expected that by stabilising and maintaining the sinus rhythm beneficial modification of atrial fibrillation by catheter ablation and preventive-regime cardiostimulator implantation will improve the quality of life of patients. 2nd Hypothesis: Continuous rhythm monitoring following combined therapy provides evidence proving the actual number of asymptomatic AF episodes.

Colorectal cancer is a problem of all civilized countries. Czech Unfortunately, Africa is in the incidence of these tumors for morethan ten years on the first areas of statistics, of all oncological diseases in our country is 13 to 14%. (1) Every year there are more than 7000 new cases andmore than 6000 patients in the tumor dies. It is the most common malignanttumor gastrointestinal tract. It is the second most common cancer in mennow after prostate cancer and in women after breast cancer. (2)Mortality from colorectal cancer growing slower than its incidence. This positive trend is likely caused by the start of screening programs. Abstract:
Myasthenia Gravis (MG) is an autoimmune disease affecting neuromuscular transmission, in which the thymus is considered pathogenic organ. Earlier ideas suggesting that MG is only the receptors disease have been proven wrong. There are immunopathological changes in both target structures [specific receptors for acetylcholine (AChR] muscle-specific tyrosine kinase (MuSK) and low-density lipoprotein 4 (Lrp4)], the thymus, as well as in peripheral lymphoid organs. Initial findings of the humoral immunity defect with the decisive role of the pathologic autoantibodies, were corrected with findings of the immune dysregulation at the level of T lymphocytes. According to today's knowledge, the development and maintenance of MG involves almost all cell types of immune function in the autoimmune inflammation: helper CD4+ T lymphocytes, cytotoxic CD8+ T lymphocytes, regulatory CD4+CD25+ T lymphocytes, Th17 lymphocytes, B lymphocytes and plasma cells. Thymus plays a dominant immunopathogenetic role in younger patients with MG, while extrathymic mechanisms are applied in older patients. As a result of that, the thymectomy (TE) is generally accepted as part of treatment for MG. However, there is still no data verified by a prospective controlled study, which would demonstrate a useful result of this treatment...

Abstract:
The aim of the study was to evaluate the effect of human immunodeficiency virus infection (HIV) and antiretroviral treatment on the retinal nerve fibre layer (RNFL). The RNFL hickness defined by standard parameters(TSNIT average, Superior average and Inferior average) was assessed in 48 HIV positive patients using scanning laser polarimeter, GDx VCC device. Results were compared to normal values and tested against factors suspected to affect the RNFL thickness. The mean values of the RNFL standard parameters were for TSNIT average, Superior average and Inferior average, 57,65 ± 6,18  m, 69,38 ± 8,34  m, 68,89 ± 9,50  m respectively, in our cohort. The RNFL thinning was not confirmed in our HIV positive group compared to values on healthy population. No significant correlation between the RNFL thickness and the immune profile or antiretroviral therapy was detected. However, a significant negative correlation between the RNFL thickness with increasing duration of HIV infection was foundin our study that is hypothesized to be possibly on an immune pathological basis.
