Abstract:
Cilj: Cilj rada bio je procijeniti doprinos jednofotonske emisijske tomografije / kompjutorizirane tomografije somatostatinskih receptora (SR SPECT/CT) s 99mTc-EDDA/HYNIC-Tyr3-oktreotidom (99mTc-Tektrotyd) u dijagnostici i procjeni proširenosti bolesti kod pacijenata oboljelih od neuroendokrinih tumora (NET-ova). Ispitanici i metode: Retrospektivno je analizirano 120 SR SPECT/CT snimanja pacijenata s patohistološki dokazanim NET-om s obzirom na vizualizaciju primarnih lezija i metastaza. U 45 pacijenata učinjena je i pozitronska emisijska tomografija 18F-fluorodeoksiglukozom (18F-FDG PET/CT) te su nalazi uspoređeni s nalazima SR SPECT/CT-a i vrijednostima kromogranina A. Rezultati: Od 120 pacijenata 47 (39 %) je na SR SPECT/CT upućeno nakon odstranjenja primarne lezije. Od preostala 73 pacijenta (61 %), u 56 (77 %) primarni je tumor bio vidljiv SR SPECT/CT-om, a u 9 (12 %) poznata lezija nije akumulirala radiofarmak. U 8 (11 %) pacijenata s NET-om nepoznatog primarnog sijela nalaz je bio negativan. Od 68 (57 %) pacijenta s dokazanim metastazama, u njih 57 (84 %) bile su vidljive SR SPECT/CT-om, a u 11 (16 %) nisu akumulirale radiofarmak. Od 45 (38 %) pacijenata kojima je učinjen i 18F-FDG PET/CT, u 27 (60 %) detekcija primarnih lezija i metastaza bila je sukladna nalazu SR SPECT/CT-a. Osjetljivost SR SPECT/CT-a bila je 77 % za primarne lezije i 84 % za metastaze, a 18F-FDG PET/CT-a 75 % za primarne lezije i 76 % za metastaze. Vrijednosti kromogranina A nisu pokazale statistički signifikantnu korelaciju s nalazima slikovne dijagnostike. Zaključci: SR SPECT/CT ima visoku osjetljivost za detekciju NET-ova. Osim toga, potvrđena je komplementarnost s 18F-FDG PET/CT-om te kod pacijenata s negativnim nalazom SR SPECT/CT-a treba učiniti 18F-FDG PET/CT i obrnuto.

Abstract:
<zakljucak> Bolesnici sa metastatskom diseminacijom bolesti imaju srednje prezivljavanje od oko 6 meseci. Bolesnici sa malim brojem metastatskih lezija i produzenim intervalom bez bolesti mogu imati korist od hirurske ekscizije. Ukoliko je izvodljiva, hirurgija udaljenih metastaza ima znacaja u palijativnom smislu radi redukcije simptomatologije koju njihova pojava daje kao i radi poboljsanja kvaliteta i produzavanja zivota. Samo se operacijom metastaza, ukoliko je to izvodljivo, moze znacajno produziti zivot bolesnika sa metastatskim melanomom. Oko 25% bolesnika u IV klinickom stadijumu mogu biti kandidati za operaciju, bilo samu ili u sklopu kombinovanog lecenja koje ukljucuje jos i sistemsku imuno, biohemijsku i radijacijsku terapiju. Napredak u imunoterapiji, biohemioterapiji i radioterapiji nije doneo znacajnije poboljsanje u lecenju bolesnika sa metastatskim melanomom. Hirursko je, za sada, jedino standardno lecenje ovih bolesnika, dok se svi ostali modaliteti terapije primenjuju kroz kontrolisane klinicke studije.

Abstract:
The primary goal in the therapy of patients with cervico-facial cancers has been always the control of loco-regional disease. It is more difficult to control metastasis than primary tumor. According to numerous authors, metastases to cervical lymphonodus reduce the survival of patients with planocellular cancer of the upper aero-digestive pathways for about 50%. Precise classification of primary tumor and regional lymphonodus is highly significant for adequate and timely treatment of patients with cancers of cervico-facial region. The objective of our study was to make clinical classification of cervico-facial tumors and to establish the distribution of nodes according to node groups and cervical levels. In our series of 319 subjects, T2 category of primary tumors was most prevalent accounting for 40.44%. Clinically palpable lymphonodes were found in 87.15%, with most prevalent N1 category accounting for 42.95%. The incidence of clinically negative cervical nodus (NO) was reported in 12.85%. The nodes of the upper, medium and lower jugular group were most frequent in cancers of the larynx and pharynx. In cancer of the oral cavity, submental and submandibular nodes were the most commonly involved. In distribution of nodes based on oncological cervical levels, 45.86% of nodes at level II of the neck were found in laryngeal cancer, while 40% the neck level I was involved in the cancer of the oral cavity. In epipharyngeal cancer, 3.15% of cervical metastases were detected in the posterior triangle of the neck. The incidence of cervical metastases in specific primary localizations has a significant role for indications of one of the dissections of the neck.

Abstract:
Deşi frecvenţa hepatoblastomului este redusă, acesta reprezintă cea mai comună tumoră hepatică primară malignă a copilului. Prognosticul bolii s-a îmbunătăţit considerabil în ultimele decenii, datorită progreselor terapeutice oncologice şi chirurgicale. Totuşi, tumorile aflate într-un stadiu avansat la momentul diagnosticului au în continuare un prognostic rezervat. Prezentăm cazul unui copil de 2 ani şi 9 luni, diagnosticat în februarie 2014 cu hepatoblastom, cu risc înalt (metastaze pulmonare), pentru care s-a intervenit chirurgical practicându-se excizia completă a tumorii. Ulterior, s-a iniţiat chimioterapie conform protocolului SIOPEL-4 modificat: astfel, blocurile de chimioterapie care ar fi trebuit administrate preoperator au fost utilizate postoperator. După finalizarea chimioterapiei, examenul CT toraco-abdominal efectuat în septembrie 2014 a indicat răspuns complet al bolii la tratament. Evaluarea periodică a pacientului a relevat până în prezent absenţa semnelor de recidivă locală tumorală, absenţa metastazelor şi un status de performanţă de 80 pe scala Lansky. Utilizarea terapiilor moleculare ţintite, a transplantului hepatic şi a unor noi chimioterapice poate reprezenta în viitor o modalitate de îmbunătăţire a prognosticului pacienţilor cu hepatoblastom cu risc înalt. De asemenea, modificarea protocoalelor de chimioterapie ar putea reprezenta o opţiune pentru îndeplinirea acestui scop.

Giriş ve amaç: Her yıl 1.8 milyon yeni akciğer kanseri tanısı konulmakta, bu nedenle 1.6 milyon ölüm bildirilmektedir. Akciğer kanserli hastaların halen büyük bir bölümü ileri evrede tanı almaktadır. Amacımız hastanemizde bir yıl içinde akciğer doku biyopsisi yapılan olguların özelliklerini değerlendirmek ve 1 yıllık sağ kalımlarını araştırmaktır. Yöntem: : Ocak 2018- Ocak 2019 tarihleri arasında kliniğimizde izlenip akciğer doku biyopsisi yapılan 222 olgunun dosyaları retrospektif olarak incelendi. Olguların demografik özellikleri, tanı yöntemleri, biyopsi sonrasındaki komplikasyonları, histopatolojik tanıları, uzak organ metastazları ve 1 yıllık sağ kalımları değerlendirildi. Bulgular: Olguların 192’si erkek, 30’u kadındı. Yaş ortalamaları 63,9±12,7’dü. Çalışmamızdaki erkeklerin yaş ortalaması 64.4±12.2 iken, kadınların yaş ortalaması 60.8±15.6’ydi (p=0.727). 159 olguya yapılan perkütan transtorasik akciğer biyopsisinin 26’sı ultrasonografi eşliğinde 133’ü bilgisayarlı tomografi eşliğinde yapıldı. Transtorasik biyopsi ile tanı oranı % 78,6 saptandı. 57 olguya bronkoskopi, 6 olguya ise açık akciğer biyopsisi yapıldı. 222 olgunun 159’una tanı konulmuştu ve bunların 153’ü neoplastikti. Adenokarsinom tanısı alan 65 olgunun 23’ü (% 35,4) bir yıl içinde exitus olmuştu. Malignite tanısı alan olguların %38,7’sinde tanı anında metastaz saptanmamış, %61,3’ünde tanıda metastaz saptanmıştı. Tartışma ve sonuç: Adenokarsinom dünyada görüldüğü gibi bizim çalışmamızda da en sık görülen histopatolojik tiptir. Akciğer kanseri tanısı koymakta kullanılan transtorasik akciğer biyopsisi etkin ve komplikasyonu düşük bir tetkik olmakla birlikte çoğu hasta akciğer kanseri tanısını ileri evrede aldığından daha erken aşamada tanı konulabilecek yöntemler için çalışmalara ihtiyaç vardır.
