The task poses ethical challenges to psychiatrists, who may be asked to participate in dehumanising and unjust administrative procedures as part of their professional role in the prison system. The author describes her personal experience as a prison psychiatrist earlier in her career, using a conceptual framework articulated by the legal scholar Robert Cover to navigate the decision of whether to participate in the prison's solitary confinement scheme.

Objectives: Real-time virtual sonography (RVS) is a promising navigation technique for percutaneous radiofrequency ablation (RFA) treatment, especially in ablating nodules poorly visualized on conventional ultrasonography (US). However, its cost-effectiveness has not been established. The purpose of this study is to evaluate the cost-effectiveness of RVS navigated RFA (RVS-RFA) relative to US guided RFA (US-RFA) in patients with small hepatocellular carcinoma (HCC) in China, from the modified societal perspective.Methods: A state-transition Markov model was created using TreeAge Pro™ 2012. The parameters used in the model, including natural history of HCC patients, procedure efficacy and related costs, were obtained from a systematic search of literature through PubMed, EMBASE, and Science Citation Index databases. The simulated cohort was patients with solitary, small HCC (<3 cm in diameter) and Child-Pugh class A or B, whose tumors are poorly visualized in B-mode US but clearly detectable by CT or MRI.Results: In this cohort of difficult cases, RVS-RFA was a preferred strategy saving 2,467 CNY ($392) throughout the patient's life while gaining additional 1.4 QALYs compared with conventional US guidance. The results were sensitive to the efficacy of US-RFA and RVS-RFA including complete ablation rate and local recurrence rate, the median survival for patients with progressive HCC, the probability of performing RFA for recurrent HCC, and the cost of RVS navigation, disposable needle or hospitalization.Conclusions: RVS-RFA is a dominant strategy for patients with small HCC unidentifiable in B-mode US, in terms of cost savings and QALYs gained, relative to the conventional US-guided method.

Abstract:
Background Patients with nonpalpable rib lesions (NPRL) requiring biopsy present a challenging problem for the thoracic surgeon. Ideally, a small incision directly over the NPRL should be performed to minimize morbidity, particularly if the lesion is benign. The Navigator probe is routinely used after lymphoscintigraphy by surgical oncologists to isolate sentinel lymph nodes requiring removal, but can also be used to guide resection of nonpalpable focal rib lesions demonstrating increased technetium-99m hydroxymethylene diphosphonate (Tc-99m HDP) uptake. This report describes our initial experience with this technique. Methods Over a 5-month period, 3 patients with focal NPRL underwent rib resection. All patients had solitary lesions demonstrated on recently performed Tc-99m HDP bone scanning. Prior cancers were reported in 2 patients, and pain in 2 patients. Before surgery, all patients underwent intravenous injection of 20 to 25 mCi Tc-99m HDP at least 2 hours before the Navigator probe-guided procedure. Results The Navigator probe identified all 3 lesions, allowing a single 4 cm or smaller incision in all cases. Histology included metastatic breast cancer (1), pathologic fracture secondary to metastatic palatal cancer (1), and eosinophilic granuloma (1). No patient required further resection. Conclusions Intraoperative localization of NPRL that are positive on Tc-99m HDP bone scanning using the Navigator probe is feasible and was 100% successful in our initial experience. This technique allows a minimally invasive approach, which is beneficial for those patients who do not require further resection.

Recent years have witnessed an increased use of ultrasound in evaluation of the airway and the lower parts of the respiratory system. Ultrasound examination is fast and reliable and can be performed at the bedside and does not carry the risk of exposure to ionizing radiation. Apart from use in diagnostics it may also provide safe guidance for invasive and semi-invasive procedures. Ultrasound examination of the oral cavity structures, epiglottis, vocal cords, and subglottic space may help in the prediction of difficult intubation. Preoperative ultrasound may diagnose vocal cord palsy or deviation or stenosis of the trachea. Ultrasonography can also be used for confirmation of endotracheal tube, double-lumen tube, or laryngeal mask placement. This can be achieved by direct examination of the tube inside the trachea or by indirect methods evaluating lung movements. Postoperative airway ultrasound may reveal laryngeal pathology or subglottic oedema. Conventional ultrasound is a reliable real-time navigational tool for emergency cricothyrotomy or percutaneous dilational tracheostomy. Endobronchial ultrasound is a combination of bronchoscopy and ultrasonography and is used for preoperative examination of lung cancer and solitary pulmonary nodules. The method is also useful for real-time navigated biopsies of such pathological structures.

Solitary fibrous tumor and desmoplastic small round cell tumor are primary peritoneal malignant tumors. Despite their morphological differences, these diseases are united by low morbidity, the lack of pathogenic symptoms, similar radiological signs, and scant knowledge of prognostic factors. Considering the low morbidity, reliable differential diagnostic signs have not been established at present, which greatly complicates the timely formulation of the diagnosis. On the contrary, cases of choosing the wrong treatment tactics and the lack of a unified management algorithm are increasingly being identified. Radiation and instrumental diagnostic methods such as computed tomography, magnetic resonance imaging, laparoscopy are crucial in determining the extent of the tumor, it is necessary to take into account the biological characteristics of the tumor (morphological type), which affect the nature of tumor growth. With a solitary fibrous tumor and a desmoplastic small-round cell tumor, the computed tomography picture is characterized by the presence of isolated nodular formations in any part of the abdominal cavity, which can also be accompanied by a large omentum, ascites, an increase in retroperitoneal lymph nodes, and distant metastasis to the lungs and bones is possible. In general, the main task of diagnosing a primary peritoneal neoplasm is to take sufficient quality biological material, with the leading role in the diagnosis being given to histological and immunohistochemical studies. On the other hand, the independent experience of many clinics does not exceed single surveillance. This information does not allow doctors to confidently navigate the problem; accordingly, there is a need for multicenter and, possibly, prospective, randomized scientific research in order to develop a unified algorithm for managing patients with primary peritoneal tumors. The approaches to the treatment of these tumors are currently not standardized and not well studied due to the rarity of the pathology. According to the available information, the treatment of desmoplastic small round cell tumors is based on multimodal therapy, including systemic chemotherapy and surgery, and possibly radiation therapy. Traditional treatment algorithms for this pathology include the surgical stage and systemic or intracavitary chemotherapy, while the treatment sequence may be different and depends primarily on the prevalence of the disease. For solitary fibrous tumor, surgical treatment, by contrast, is the method of choice. Thus, when choosing a treatment strategy for a primary peritoneal tumor, it is first of all necessary to take into account the morphological type of the tumor and rely on known data on the benefits of a particular method. In this article, we tried to present modern information on the diagnosis and treatment of rare forms of primary peritoneal tumors.

AbstractAnimals’ cognitive abilities can be tested by allowing them to choose between alternatives, with only one alternative offering the correct solution to a novel problem. Hermit crabs are evolutionarily specialized to navigate while carrying a shell, with alternative shells representing different forms of ‘extended architecture’, which effectively change the extent of physical space an individual occupies in the world. It is unknown whether individuals can choose such architecture to solve novel navigational problems. Here, we designed an experiment in which social hermit crabs (Coenobita compressus) had to choose between two alternative shells to solve a novel problem: escaping solitary confinement. Using X-ray microtomography and 3D-printing, we copied preferred shell types and then made artificial alterations to their inner or outer shell architecture, designing only some shells to have the correct architectural fit for escaping the opening of an isolated crab’s enclosure. In our ‘escape artist’ experimental design, crabs had to choose an otherwise less preferred shell, since only this shell had the right external architecture to allow the crab to free itself from isolation. Across multiple experiments, crabs were willing to forgo preferred shells and choose less preferred shells that enabled them to escape, suggesting these animals can solve novel navigational problems with extended architecture. Yet, it remains unclear if individuals solved this problem through trial-and-error or were aware of the deeper connection between escape and exterior shell architecture. Our experiments offer a foundation for further explorations of physical, social, and spatial cognition within the context of extended architecture.

Abstract Recent calls from senior managers, human rights groups and academics continue to scrutinize the impact of solitary confinement. But much less attention has been paid to prisoners’ own motivations for segregation. By analysing interviews with 16 segregated men in a high-security prison (Category-A) in England, this article foregrounds motivation. The argument involves a detailed description of the complex, and sometimes contradictory, motives that may lead prisoners into seeking isolation. It further attempts to explore the relationship between segregation and the wider prison environment. For many prisoners, segregation has a ‘negative benefit’ or amounts to a form of ‘lesser evil’. Such phrasing hints at the difficult decisions that prisoners navigate and offers an alternative perspective on solitary confinement.

Abstract In addition to foraging individually several species of ants guide nestmates to a goal by tandem running. We found that the Australian ant, Camponotus consobrinus, forages both individually and by tandem running to head to the same goal, nest-specific native Australian trees on which they forage. While paths of solitary foragers and initial paths of tandem followers showed no differences in heading directions or straightness, tandem followers moved at about half the speed of solitary runs. When leaders were experimentally removed, follower ants initially engaged in a systematic search around the point of interruption, following which they either (a) headed directly towards and successfully reached the foraging trees, or (b) continued searching or (c) returned to the nest. The high incidence of followers that successfully navigated towards the foraging trees on their own provides strong evidence that many tandem followers are in fact experienced foragers. Detailed analysis of the searching behaviour revealed that even seemingly lost followers displayed a directional bias towards the foraging trees in their search path. Our results show that in a foraging context follower ants in a tandem pair are not always naïve.

Doctoral studies are often described as solitary and challenging endeavors, dependent on candidates’ highly developed skills, self-driven nature, and commitment to engage in years of research activity. A range of university initiatives are specially crafted to support higher research degree students, for example, through digital and physical resources, workshops, group gatherings, and others. Our project examines what constitutes significant places for learning and experiences of inclusion amongst doctoral students, and ways of capturing and sharing these experiences with those learning at distance and on university campuses. Our focus is on connections between social values and the built environment, and on developing ways of expressing these values through students’ representations of places for learning. This paper reports on interviews with doctoral students’ discussing their connections to on-campus and distance places for learning – within the digital and physical landscapes of a multi-campus university, with provision for both, internal and distance students. Our findings reveal the ways doctoral students navigate the digital and physical realms of university spaces, the places they inhabit and value, their attachments to things, and how these, in turn, influence their feelings of inclusion, belonging, and learning purpose.

Abstract:
Introduction: Radical liver resection is the only method for the treatment of patients with colorectal liver metastases (CLM); however, only 20–30% of patients with CLMs can be radically treated. Radiofrequency ablation (RFA) is one of the possible methods of palliative treatment in such patients. Methods: RFA was performed in 381 patients with CLMs between 01 Jan 2001 and 31 Dec 2018. The mean age of the patients was 65.2±8.7 years. The male to female ratio was 2:1. Open laparotomy was done in 238 (62.5%) patients and the CT-navigated transcutaneous approach was used in 143 (37.5%) patients. CLMs <5 cm (usually <3 cm) in diameter were the indication for RFA. We used RFA as the only method in 334 (87.6%) patients; RFA in combination with resection was used in 36 (9.4%), and with multi-stage resection in 11 (3%) patients. We performed RFA in a solitary CLM in 170 (44.6%) patients, and in 2−5 CLMs in 211 (55.6%) patients. We performed computed tomography in each patient 48 hours after procedure. Results: The 30-day postoperative mortality was zero. Complications were present in 4.8% of transcutaneous and in 14.2% of open procedures, respectively, in the 30-day postoperative period. One-, 3-, 5- and 10-year overall survival rates were 94.8, 66.8, 43.9 and 16.6%, respectively, in patients undergoing RFA, and 90.6, 69.1, 52.8 and 39.2%, respectively, in patients with liver resections. Disease free survival was 63.2, 30.1, 18.4 and 13.1%, respectively, in the same patients after RFA, and 71.1, 33.3, 22.8 and 15.5%, respectively, after liver resections. Conclusion: RFA is a palliative thermal ablation method, which is one of therapeutic options in patients with radically non-resectable CLMs. RFA is useful especially in a non-resectable, or resectable (but for the price of large liver resection) solitary CLM <3 cm in diameter and in CLM relapses. RFA is also part of multi-stage liver procedures.

Abstract:
The following essay constitutes a theoretical journey through the landscape of the NAMES Project AIDS Quilt, one narrated from the perspective of those who live on to mourn, to remember. To some critics, my approach might at once appear radical or unorthodox since I focus on how Quilt spectators engage with the artifact and are thereby implicated in the history of the epidemic, rather than on how the dead are made to speak from beyond the grave. My intent is not to invalidate the claims made or the conclusions reached by other critics who have written persuasively about how the Quilt facilitates a voice (both individual and collective) for those who have died of AIDS-related illnesses; indeed, such critics have contributed a wealth of significant knowledge to critical understandings of the pedagogical and political functions of the Quilt. My intent, rather, is to respond to a set of as yet unexplored questions about how the ever-evolving landscape of the AIDS epidemic has altered the nature and purpose of Quilt spectatorship. When the Quilt was created in 1985, “naming names” was an important strategy for political survival, especially given the institutional apathy that silenced and marginalized all who were infected and/or affected by the epidemic. However, over the past sixteen years, apathy has slowly given way to increased attention by medical and media institutions. No longer is memory and reverence enough. Now, we must ask ourselves how the Quilt can continue to be used to combat the emergent obstacles that have sprung up in the wake of apathy and silence. This is not to suggest that remembrance, mourning, and reverence are not still significant responses to the epidemic; we cannot forgot the past, lest we repeat it. But it is to suggest that as we look back, we also must move forward and continue to chart new horizons for how our minds and bodies engage with the Quilt as a social and political space. Throughout this essay, then, my conclusions are at best tentative, offered more as a gesture of hope than as a model for survival. It is my hope that critics can continue to press against social spaces both with caution and determination because those actions matter. We must act with caution because there can be devastating consequences of asserting claims to visibility and location. We must act with determination because there are equally perilous consequences of not doing so. Since its meager beginnings, the NAMES Project AIDS Quilt has undergone exponential changes in size, shape, and scope, but critical responses to the Quilt have remained stagnant with most critics attributing to the Quilt a single meaning and purpose: to revere the dead. Cultural critic Peter S. Hawkins, for instance, argues that "the Quilt . . . is most profoundly about the naming of names" (760), while journalist Jerry Gentry suggests that the Quilt bespeaks "a national and international constructive expression of grief" (550), a grief which most powerfully resonates in the loss of individual lives. While "naming names" is a politically important function of the Quilt, critics who read the artifact as only motivated by memory assume that exhibitions of the artifact facilitate a static model of performance. For such critics, the Quilt represents a mass graveyard--both as a place of interment and as a place in which ideological meanings are circumscribed by the fixity and stillness of reverence. (This reading is partly enabled by the fact that each panel measures the size of a human grave.) Not only does this reading universalize the meaning of the Quilt but also it establishes a monolithic viewing position from which to receive that meaning. Here, I outline an alternative model of reception which is implicit in the design and display of the Quilt. While this model acknowledges reverence as one potential response to the Quilt, it does not foreclose other ways of reading. Rather than facilitating a grave performance, then, the Quilt enables performances within the grave. These performances are constituted in/through a dynamic exchange between speaker and listener, text and context, and work to produce a range of ideological meanings and subject positions. To understand how the Quilt locates its viewers within a particular subject position, it is first necessary to specify the speaker-listener relationship established in displays of the Quilt. This relationship is played out through a series of confessional utterances which, as Michel Foucault explains in The History of Sexuality, Volume 1, imply a dialectic relationship between speaker and listener in which subjectivity is predicated on subjection (61-62). The speaker's desire to confess necessitates the presence of a listener, one not wholly passive since his/her presence incites and enables the will to confess. In this way, both speaker and listener are marked as active/passive agents in an exchange characterized by reciprocity and negotiation. Because speakers and listeners simultaneously serve as subject and object of the confession, the exchange cannot be represented as static (active/passive) or unidirectional (sender-message-receiver). Since many critics already have carefully delineated the processes through which the Quilt directs the address of the panels and constitutes the dead as subjects, here I want to focus on how spectators are constructed as subjects who bear witness. Critics typically posit viewers of the Quilt as unified, coherent, monolithic subjects; yet Foucault's discussion of the confessional exchange assumes a subject-in-process. For me, this process is most accurately characterized as schizophrenic. My use of schizophrenia is tropic rather than diagnostic, in that the term works figuratively to describe subject formation rather than to identify the nature of psychiatric disturbance. Two characteristics (which are derived from the symptomatology of the psychic disorder schizophrenia) define the schizophrenic spectator: one, the loss of "normal" associations; and two, the presence of "auditory hallucinations." The first characteristic of schizophrenic spectators is the loss of "normal" associations. Remi J. Cadoret notes that in schizophrenics, "[t]hought processes appear to lose their normal associations, or usual connecting links, so that the individual is often unable to focus his [sic] thinking upon a particular mental task" (481). For schizophrenics, conventional relational markers (such as chronology, causality, temporality, and spatiality) no longer order cognition; instead, these markers are distorted (if not entirely ineffectual), creating for the schizophrenic a fractured sense of self in/and world. Without these normal associations, the schizophrenic wanders aimlessly (and often in isolation) through a chaotic world in search of structure, meaning, and purpose. Temporal associations provide perhaps the most common means of ordering experience. Viewed as a linear progression characterized by movement, change, and renewal, time structures the historical and the everyday by sequencing, demarcating, and hierarchizing events. Within the Quilt, this sense of progression is supplanted by perpetual repetition of the present. Elsley has offered a similar observation, noting that the Quilt operates in a "transitory present" tense, it "exists in a continual state of becoming" (194). In one sense, this perpetual present tense derives from the fact that no two displays of the Quilt are identical. Panels are ordered differently, new names and panels are added, older panels begin to show signs of wear-and-tear. A perpetual present tense also derives from the fact that the Quilt charts the progression of an epidemic that is itself ongoing, incomplete. Thus, the landscape of the Quilt is re-mapped in light of advances in HIV-treatment, softening/tightening of social mores, and changes in AIDS demographics. Another common means of ordering experience is though spatial associations. Location orders the social through architecture, urban planning, and zoning, endowing spaces with a well-defined purpose and layout. Yet the Quilt provides few signals regarding how spectators are intended to navigate its surface. As Weinberg notes, the Quilt is a "great grid" with "no narrative, no start or finish" (37). By describing the Quilt as a "grid," Weinberg implicitly ascribes to the artifact a controlling logic, a unified design--what in quilting parlance is termed a patchwork sampler. This design pattern, however, does not direct the flow of spectators in a single stream of traffic. This is so because, unlike a Drunkard's Path or Double Wedding Band pattern in which the individual panel blocks work together to create a unified design across the surface of the quilt, a patchwork sampler is constituted by a series of single panel blocks, each with a unique design, history, and logic. As a result, patrons' movements are guided by associations and punctuated by pauses, interruptions, and abrupt changes in course. The randomness of engagement is further enabled by the muslin walkways which visually separate the panels, marking each as distinct and disallowing any sense of continuity (narrative, spatial) among them. The routes which visitors of the Quilt traverse thus are transitory and ephemeral, simultaneously charted and erased in the moment of passing by. The second characteristic of schizophrenic spectators is the presence of auditory hallucinations. Cadoret defines these hallucinations as "the perception of auditory stimuli, or sounds, where none are externally present . . . . The voices . . . may repeat his [sic] thoughts or actions, argue with him [sic], or threaten, scold, or cure him [sic]" (481). Auditory hallucinations can lead the schizophrenic to believe that s/he is under constant surveillance or can cause the schizophrenic to slip further into a self-contained, isolated world of delusion. That the Quilt is made up of "a myriad of individual voices" (Elsley 192) is immediately apparent in the number of individuals who have taken part in its construction and display. With each quilt panel, spectators are confronted with multiple voices--the person who has died, the person(s) who made the block, the person(s) who stitched the block to others for a specific display, and so on. Moreover, the Quilt places these "individual voices . . . in the context of community" (Elsley 191). For Quilt spectators, then, memories of a life lived coexist with grief over a life cut short, anger at institutional apathy and systemic homophobia, faith in the import of remembering those who have died, and so on. Each of these voices vie for the spectator's attention, facilitating a gaze that is dynamic, multidirectional, mobile. Because the gaze is not fixed, the Quilt cannot convey a solitary truth claim to its viewers; rather, spectators must immerse themselves within the delusion and confusion of voices, imposing some sense of order on their own viewing experiences. Given that persons with AIDS continue to be marginalized within American culture, my use of the schizophrenic spectator to trace the reception dynamic of Quilt exhibits might appear to perpetuate, rather than unsettle, dominant ideological formations. Of course, this is the critical conundrum at the center of all investigations into subject formation: that is, "how to take an oppositional relation to power that is, admittedly, implicated in the very power one opposes" (Butler 17). Despite the potential pitfalls, I nonetheless use the trope of schizophrenia precisely because it recognizes the ways in which Quilt spectators, persons with AIDS, and persons who have died of AIDS-related illnesses are divested of the power and authority to speak even before they begin speaking. Furthermore, because the schizophrenic subject is founded on ever-shifting affinities (in time, across space), the position enables spectators to chart alternative lines of relation among institutional practices, ideological formations, and individual experiences, thus potentially mobilizing and sustaining a shared political program. Abstract:
It all started with a tweet. On the afternoon of 2 June 2013, Professor Geoffrey Miller, an evolutionary psychologist at the University of New Mexico (UNM) and visiting instructor at New York University (NYU), tweeted out a message that would go on to generate a significant social media controversy. Addressing aspiring doctoral program applicants, Miller wrote:Dear obese PhD applicants: if you didn’t have the willpower to stop eating carbs, you won't have the willpower to do a dissertation #truthThe response to Miller’s tweet was swift and fiery. Social media users began engaging with him on Twitter, and in the early hours of the controversy Miller defended the tweet. When one critic described his message as “judgmental,” Miller replied that doing a dissertation is “about willpower/conscientiousness, not just smarts” (Trotter). The tweet above, now screen captured, was shared widely and debated by journalists, Fat Acceptance activists, and academic social media users. Within hours Miller had deleted the tweet and replaced it with two new ones:My sincere apologies to all for that idiotic, impulsive, and badly judged tweet. It does not reflect my true views, values, or standards andObviously my previous tweet does not represent the selection policies of any university, or my own selection criteriaHe then made his Twitter account private. The captured image, however, continued to spread. Across social media, users began to circulate a campaign that called for Miller to be formally disciplined (Trotter). There was also widespread talk about potential lawsuits from prospective students who were not selected for admission at UNM (Kirby). Indeed, the Fat Chick Sings blogger Jeanette DePatie offered her own advice to Miller: #findagoodlawyer.Soon after the controversy emerged a response appeared on UNM’s website in the form of a video statement by Professor Jane Ellen Smith, the Chair of the UNM Psychology Department. Smith reiterated that Miller’s statements did not reflect the “policies and admissions standards of UNM”. She also stated that Miller had defended his actions by claiming the tweet was part of a “research project” where he would deliberately send out provocative messages in order to measure the public response to them. This claim was met with incredulity by a number of bloggers and columnists, and was later determined to be incorrect in an Institutional Review Board inquiry at UNM, which concluded Miller’s tweets were “self-promotional” in nature. Following a formal investigation, the UNM committee found no evidence that Miller had discriminated against overweight students. It did however pass a motion of censure that included a number of restrictions, including prohibiting Miller from sitting on any graduate admission committee at UNM.The #truth about Fat PhDs?Readers may be wondering why Miller’s tweet continues to matter as I write this article in 2015. It is my belief that the tweet is important insofar as it affords an insight into the cultural scene that surrounds the fat body in higher education. The vigorous debate generated by Miller’s tweet offers researchers a diverse array of media texts that are available to help build a more comprehensive picture of fat embodiment within higher education.Looking at the tweet in the cold light of day it is difficult to imagine any logical links one might infer between a person’s carbohydrate consumption and their ability to excel in doctoral education. And there’s the rub. Of course Miller’s tweet does not represent a careful evaluation of the properties of doctoral willpower. In order to make sense of the tweet we need to understand the ways cultural assumptions about fatness operate. For decades now, researchers have documented the existence of anti-fat attitudes (Crandall & Martinez). Increasingly, scholars and Fat Acceptance activists have described a “thinness norm” that is reproduced across contemporary Western cultures, which discerns normatively slender bodies as “both healthy and beautiful” (Eller 220) and those whose bodies depart from this norm, as “socially acceptable targets for shaming and hate speech” (Eller 220). In order to be intelligible Miller’s tweet relies on a number of deeply entrenched cultural meanings attributed to fatness and fat people.The first is that body-size is primarily a matter of self-control. Although Critical Fat Studies researchers have argued for some time that body weight is determined by complex interactions between the biological and environmental, the belief that a large body size is caused by limited self-control remains prevalent. This in turn supports a host of cultural connotations, which tend to constitute fat people as “lazy, gluttonous, greedy, immoral, uncontrolled, stupid, ugly and lacking in willpower” (Farrell 4).In light of the above, Miller’s message ought to be read as a moral one. I have paraphrased its logic as such: if you [the fat doctoral student] lack the willpower to discipline your body into normatively desired slimness, you will also likely lack the strength of character required to discipline your body-mind into producing a doctoral dissertation. The sad irony here is that, if anything, the attitudes that might hamper fat students from pursuing a doctoral education would be those espoused in Miller’s own tweet. As Critical Fat Studies researchers have illuminated, the anti-fat attitudes the tweet reproduces generate challenging higher education climates for fat people to navigate (Pausé, Express Yourself 6).Indeed, while Miller’s tweet is one case that arose to media prominence, there is evidence that it sits inside a wider pattern of weight discrimination within higher education. For example, Caning and Mayer (“Obesity: Its Possible”, “Obesity: An Influence”) found that despite similar high school performances, ‘obese’ students were less likely to be accepted to elite universities, than their non-obese peers. In a more recent US-based study, Burmeister and colleagues found evidence of weight bias in graduate school admissions. In particular, they found that higher body mass index (BMI) applicants received fewer post-interview offers into psychology graduate programs than other students (920), and this relationship appeared to be stronger for female applicants (920). This picture is supported by a study by Swami and Monk, who examined weight bias against women in a hypothetical scenario about university acceptance. In this study, 198 volunteers in the UK were asked to identify the women they were most and least likely to select for a place at university. Swami and Monk found that participants were biased against fat women, a finding which the authors interpreted as evidence of broader public beliefs about body size and access to higher education.In my examination of the media scene surrounding the Miller case I observed that most commentators associated the tweet with a particular affective formation – shame. Miller’s actions were widely described as “fat-shaming” (Bennet-Smith; Ingeno; Martin; Trotter; Walsh) with Miller himself often referred to simply as the “fat-shaming professor” (King; ThinkTank). In this article I wish to consider the affective-political dimensions of Miller’s tweet, by focusing on one digital community’s response to it: Fuck Yeah! Fat PhDs. In following this path I am building on the work of other researchers who have considered fat activisms and Web 2.0 (Pausé, Express Yourself); fat visual activism (Gurrieri); and the emotional politics of fat acceptance blogging (Kargbo; Bronstein).Imaging Alternatives: Fuck Yeah! Fat PhDsBy 3 June 2013 – just one day after Miller’s tweet was published – New Zealand-based academic Cat Pausé had created the Tumblr Fuck Yeah! Fat PhDs. This was billed as a photo-blog about “being fatlicious in academia”. Writing on her Friend of Marilyn blog, Pausé explained the rationale behind the Tumblr:I decided that what I wanted to do was to highlight all the amazing fat individuals who are in graduate school, or have completed graduate school – to provide a visual repository … and to celebrate the amazing work being done by these rad fatties!Pausé sent out calls for participants on Twitter, Tumblr and Facebook, and emailed a Fat Studies listserv. She asked submitters to send “a photo, along with their name, degree, and awarding institution” (Pausé Express Yourself, 6). Images were submitted thick and fast. Twenty-three were published in the first day of the project, and twenty in the second. At the time of writing, just over 150 images had been submitted, the most recent being November 2013.The Fuck Yeah! Fat PhDs project ought to be understood as part the turn away from the textual toward the digital in fat activist movements (Kargbo). This has seen a growth in online communities that are interested in developing “counter-images in response to the fat body’s position as the abject, excluded Other of the socially acceptable body” (Kargbo 162). Examples include a multitude of Fatshion photo-blogs, Tumblrs like Exciting Fat People or the Stocky Bodies image library, which responds to the limited diversity of visual representations of fat people in the mainstream media (Gurrieri).For this article, I have read the images on the Fuck Yeah! Fat PhDs Tumblr in order to gain an impression about the affective-political work accomplished by this collective of self-identified fat academic bodies. As I indicated earlier, much of the commentary following Miller’s tweet characterised it as an attempt to ‘shame’ fat doctoral students. As Elspeth Probyn has identified, shame frequently manifests itself on the body “most experiences of shame make you want to disappear, to hide away and to cover yourself” (Probyn 329). I suggest that the core work of the Fuck Yeah! Fat PhDs Tumblr is to address the spectre of shame Miller’s tweet projects with visibility, rather than it’s opposite. This visibility also enables the project to proliferate a host of different ways of (feeling about) being fat and doctoral.The first image posted on the Tumblr is Pausé’s own. She is pictured smiling at the 2007 graduation ceremony where she received her own PhD, surrounded by fellow graduates in academic regalia. Her image is followed by many others, mostly white women, who attest to the academic attainments of fat individuals. My first impression as I scrolled through the Tumblr was to note that many of the images (51) referenced scenes of graduation, where subjects wore robes, caps or posed with higher degree certificates. Many more were the kinds of photographs that one might expect to be taken at an academic event. Together, these images attest to the viability of the living, breathing doctoral body - a particularly relevant response given Miller’s tweet. This work to legitimate the fat doctoral body was also accomplished through the submission of two historical photographs of Albert Einstein, a figure who is neither living nor breathing, but highly unlikely to be described as lacking academic ability or willpower.As I read through the Tumblr subsequent times, I noticed that many of the submitters offered images that challenge stereotypical representations of the fat body. As a number of writers have noted, fat people tend to be visually represented as “solitary, lonely figures whose expressions are downcast and dejected” (Gurrieri 202). That is if they aren’t already decapitated in the visual convention of the “headless fatty” used across news media (Kargbo 160). Like the Stocky Bodies project, the Fuck Yeah! Fat PhDs Tumblr facilitated a more diverse and less pathologising representation of fat (doctoral) embodiment.Across the images there is little evidence of the downcast eyes of shame and dejection that Miller’s tweet seems to invite of aspiring fat doctoral candidates. Scrolling through the Tumblr one encounters images of fat people singing, swimming, creating art, playing sport, smoking, smiling, dressing up, and making music. A number of images (12) emphasise the social nature of fat doctoral life, by picturing multiple subjects at once, some holding hands, others posing with colleagues, loved ones, and a puppy. Another category of submissions took a playful stance vis-à-vis some representational conventions of imaging fatness. Where portrayals of the fat body from side or rear angles, or images of fat people eating and drinking typically code an affective scene of disgust (Gurrieri), a number of images on the Tumblr appear to reinscribe these scenes with new meaning. Viewers are offered pictures of smiling and contented fat graduates unashamed to eat and drink, or be represented from ‘unflattering’ angles.Furthermore, a number of images offered alternatives to the conventional representation of the fat subject as ugly and sexually unattractive by posing in glamorous shots bubbling with allure and desire. In one memorable picture, blogger and educator Virgie Tovar is snapped wearing a “sex instructor” badge and laughs while holding two sex toys.Reading across the images it becomes clear that the Tumblr offers a powerful response to the visual convention of representing the solitary, lonely fat person. Rather than presenting isolated fat doctoral students the act of holding the images together generates a sense of fat higher education community, as Kargbo notes:A single image posted online amidst vast Internet ephemera is just a fleeting document of a moment in a stranger’s life. But in the plural, as one scrolls through hundreds of images eager to hit the ‘next’ button for what will be a repetition of the same, the image takes on a new function: it becomes an insistent testament to the liveness of fat embodiment in the present. (164)Obesity Timebomb blogger Charlotte Cooper (2013) commented on the significance of the project: “It is pretty amazing to see the names and faces as I scroll through Fuck yeah! Fat PhDs. Many of us are friends and collaborators and the site represents a new community of power.”Concluding Thoughts: Fat Embodiment and Higher Education CulturesThis article has examined a cultural event that that saw the figure of the fat doctoral student rise to international media prominence in 2013. I have argued that while Miller’s tweet can be read as illustrative of the affective scene of shame that surrounds the fat body in higher education, the images offered by the Fuck Yeah! photo submitters work to re-negotiate implication in social discourses of abjection. Indeed, the images assert that alternative ways of feeling about being fat and doctoral remain viable. Fat students can be contented, ambivalent, sultry, pissed off, passionate and proud – and Fuck Yeah! Fat PhDs provides submitters with a platform to perform a wide array of these affects. This is not to say that shame is shut out of the project, or the lives of submitters’ altogether. Instead, I am suggesting that the Tumblr generates a more open field of possibilities, providing “a space for re-imagining new forms of attachments and identifications.” (Kargbo 171). Critics might argue that this Tumblr is not particularly novel when set in the context of a range of fat photo-blogs that have sprung up across the Internet in recent years. I would argue, however, that when we consider the kinds of questions Fuck Yeah! Fat PhDs might ask of university cultures, and the prompts it offers to higher education researchers, the Tumblr can be seen to make an important contribution. I am in agreement with Kargbo (2013) when she argues that fat photo-blogs “have the potential to alter the conditions of visual reception and perception”. That is, through their “codes and conventions, styles of lighting and modes of address, photographs literally show us how to relate to another person” (Singer 602). When read together, the Fuck Yeah! images insist that a different kind of relationship to fat PhDs is possible, one that exceeds the shaming visible in Miller’s tweet. Ultimately then, the Tumblr is a call to take fat doctoral students seriously, not as problems in need of fixing, but as a diverse group of scholars who make important contributions to the academy and beyond.I would like to use the occasion of concluding this article to call for further conversations about fat embodiment and higher education cultures. The area is significantly under-researched, with higher education scholars largely failing to engage with the material and affective experiences of fat embodiment. Indeed, I would argue that if nothing else, this paper has demonstrated that public scenes of knowledge creation have done a much more comprehensive job of analysing the intersection of ‘fat + university’ than academic books and articles to date. While not offering an exhaustive sketch, I would like to gesture toward some areas that might contribute to a future research agenda. For example, researchers might begin to approach the experience of living, working and studying as a fat person in the contemporary university. Such research might examine whose body the university is imagined and designed for, as well as the campus climate experienced by fat individuals. Researchers might consider how body size could become a part of broader conversations about embodiment and privilege in higher education, alongside race, ability, gender identity, and other categories of social difference.Thinking about the intersection of ‘fat + university’ would also involve tracing possibilities. For example, what role do university campuses play as spaces of fat activism and solidarity? The L Word: Generation Q is the reboot of The L Word, a long running series about a group of lesbians and bisexuals in Los Angeles in the early 2000s. Both programmes are unique in their positioning of lesbian characters and have been well received by audiences and critics alike. These programmes present a range of characters and narratives, previously excluded from mainstream film and television, bringing a refreshing change from the destructive images typically presented before. We argue that the reboot Generation Q now offers more meaningful representation of the broader lesbian and transgender communities, and discuss its relevance in the changing portrayals of gay representation. Gay visibility has never really been an issue in the movies. Gays have always been visible. It is how they have been visible that has remained offensive for almost a century. (Russo 66) In 2004 The L Word broke new ground as the very first television series written and directed by predominantly queer women. This set it apart from previous representations of lesbians by Hollywood because it portrayed a community rather than an isolated or lone lesbian character, that was extraneous to a cast of heterosexuals (Moore and Schilt). The series brought change, and where Hollywood was more often “reluctant to openly and non-stereotypically engage with gay subjects and gay characters” (Baker 41), the L Word offered an alternative to the norm in media representation. “The L Word’s significance lies in its very existence” according to Chambers (83), and this article serves to consider this significance in conjunction with its 2019 reboot, the L Word: Generation Q, to ascertain if the enhanced visibility and gay representation influences the system of representation that has predominantly been excluding and misrepresentative of gay life. The exclusion of authentic representation of lesbians and gays in Hollywood film is not new. Over time, however, there has been an increased representation of gay characters in film and television. However, beneath the positive veneer remains a morally disapproving undertone (Yang), where lesbians and gays are displayed as the showpiece of the abnormal (Gross, "Out of the Mainstream"). Gross ("Out of the Mainstream") suggests that through the ‘othering’ of lesbians and gays within media, a means of maintaining the moral order is achieved, and where being ‘straight’ results in a happy ending. Lesbians and gays in film thus achieve what Gerbner referred to as symbolic annihilation, purposefully created in a bid to maintain the social inequity. This form of exclusion often saw controversial gay representation, with a history of portraying these characters in a false, excluding, and pejorative way (Russo; Gross, "What Is Wrong"; Hart). The history of gay representation in media had at times been monstrous, playing out the themes of gay sexuality as threatening to heterosexual persons and communities (Juárez). Gay people were incorrectly stereotyped, and gay lives were seen through the slimmest of windows. Walters (15) argued that it was “too often” that film and television images would narrowly portray gays “as either desexualized or over sexualized”, framing their sexuality as the sole identity of the character. She also contested that gay characters were “shown as nonthreatening and campy 'others' or equally comforting and familiar boys (and they are usually boys, not girls) next door” (Walters 15). In Russo’s seminal text, The Celluloid Closet, he demonstrated that gay characters were largely excluded from genuine and thoughtful presentation in film, while the only option given to them was how they died. Gay activists and film makers in the 1980s and beyond built on the momentum of AIDS activism (Streitmatter) to bring films that dealt with gay subject matter more fairly than before, with examples like The Birdcage, Philadelphia, To Wong Foo, Thanks for Everything! Julie Newmar, and In and Out. Walters argues that while “mainstream films like Brokeback Mountain and The Kids are Alright entertain moviegoers with their forthright gay themes and scenes” (12), often the roles have been more of tokenisation, representing the “surprisingly gay characters in a tedious romcom, the coyly queer older man in a star-studded indie hit, the incidentally gay sister of the lead in a serious drama” (Walters 12). This ambivalence towards the gay role model in the media has had real world effects on those who identify themselves as lesbian or gay, creating feelings of self-hatred or of being ‘unacceptable’ citizens of society (Gamson), as media content “is an active component in the cultural process of shaping LGBT identities” (Sarkissian 147). The stigmatisation of gays was further identified by the respondents to a study on media and gay identity, where “the prevailing sentiment in these discussions was a sense of being excluded from traditional society” (Gomillion and Guiliano 343). Exclusion promotes segregation and isolation, and since television media are ever-present via conventional and web-based platforms, their messages are increasingly visible and powerful. The improved portrayal of gay characters was not just confined to the area of film and television however, and many publications produced major stories on bi-sexual chic, lesbian chic, the rise of gay political power and gay families. This process of greater inclusion, however, has not been linear, and in 2013 the media advocacy group known as the Gay and Lesbian Alliance against Defamation (GLAAD) mapped the quantity, quality, and diversity of LGBT people depicted in films, finding that there was still much work to be done to fairly include gay characters (GLAAD Studio Responsibility Index). In another report made in 2019, which examined cable and streaming media, GLAAD found that of the 879 regular characters expected to appear on broadcast scripted primetime programming, 10.2% were identified as gay, lesbian, bisexual, transgender and or queer (GLAAD Where Are We on TV). This was the highest number of queer characters recorded since the start of their reporting. In January 2004, Showtime launched The L Word, the first scripted cable television to focus chiefly on lesbians. Over the course of six seasons it explored the deep bonds that linked the members of an evolving lesbian friendship circle. The central themes of the programme were the love and friendship between the women, and it was a television programme structured by its own values and ideologies. The series offered a moral argument against the widespread sexism and anti-gay prejudice that was evident in media. The cast, however, were conventionally beautiful, gender normative, and expensively attired, leading to fears that the programme would appeal more to straight men, and that the sex in the programme would be exploitative and pornographic. The result, however, was that women’s sex and connection were foregrounded, and appeared as a central theme of the drama. This was, however, ground-breaking television. The showrunner of the original L Word, Ilene Chaiken, was aware of the often-damning account of lesbians in Hollywood, and the programme managed to convey an indictment of Hollywood (Mcfadden). The L Word increased lesbian visibility on television and was revolutionary in countering some of the exclusionary and damaging representation that had taken place before. It portrayed variations of lesbians, showing new positive representations in the form of power lesbians, sports lesbians, singles, and couples. Broadly speaking, gay visibility and representation can be marked and measured by levels of their exclusion and inclusion. Sedgwick said that the L Word was particularly important as it created a “lesbian ecology—a visible world in which lesbians exist, go on existing, exist in forms beyond the solitary and the couple, sustain and develop relations among themselves of difference and commonality” (xix). However, as much as this programme challenged the previous representations it also enacted a “Faustian bargain because television is a genre which ultimately caters to the desires and expectations of mainstream audiences” (Wolfe and Roripaugh 76). The producers knew it was difficult to change the problematic and biased representation of queer women within the structures of commercial media and understood the history of queer representation and its effects. Therefore, they had to navigate between the legitimate desire to represent lesbians as well as being able to attract a large enough mainstream audience to keep the show commercially viable. The L Word: Generation Q is the reboot of the popular series, and includes some of the old cast, who have also become the executive producers. These characters include Bette Porter, who in 2019 is running for the office of the Mayor of Los Angeles. Shane McCutchen returns as the fast-talking womanising hairdresser, and Alice Pieszecki in this iteration is a talk show host. When interviewed, Jennifer Beals (executive producer and Bette Porter actor) said that the programme is important, because there have been no new lesbian dramas to follow after the 2004 series ended (Beals, You Tube). Furthermore, the returning cast members believe the reboot is important because of the increased attacks that queer people have been experiencing since the election of Donald Trump in 2016. Between the two productions there have been changes in the film and television landscape, with additional queer programmes such as Pose, Orange Is the New Black, Euphoria, RuPaul’s Drag Race, and Are You the One, for example. The new L Word, therefore, needed to project a new and modern voice that would reflect contemporary lesbian life. There was also a strong desire to rectify criticism of the former show, by presenting an increased variation of characters in the 2019 series. Ironically, while the L Word had purposefully aimed to remove the negativity of exclusion through the portrayal of a group of lesbians in a more true-to-life account, the limited character tropes inadvertently marginalised other areas of lesbian and queer representation. These excluded characters were for example fully representative trans characters. The 2000s television industry had seemingly returned to a period of little interest in women’s stories generally, and though queer stories seeped into popular culture, there was no dedicated drama with a significant focus on lesbian story lines (Vanity Fair). The first iteration of The L Word was aimed at satisfying lesbian audiences as well as creating mainstream television success. It was not a tacky or pornographic television series playing to male voyeuristic ideals, although some critics believed that it included female-to-female sex scenes to draw in an additional male viewership (Anderson-Minshall; Graham). There was also a great emphasis on processing the concept of being queer. However, in the reboot Generation Q, the decision was made by the showrunner Marja-Lewis Ryan that the series would not be about any forms of ‘coming out stories’, and the characters were simply going about their lives as opposed to the burdensome tropes of transitioning or coming out. This is a significant change from many of the gay storylines in the 1990s that were seemingly all focussed on these themes. The new programme features a wider demographic, too, with younger characters who are comfortable with who they are. Essentially, the importance of the 2019 series is to portray healthy, varied representations of lesbian life, and to encourage accurate inclusion into film and television without the skewed or distorted earlier narratives. The L Word and L Word: Generation Q then carried the additional burden of countering criticisms The L Word received. Roseneil explains that creating both normalcy and belonging for lesbians and gays brings “cultural value and normativity” (218) and removes the psychosocial barriers that cause alienation or segregation. This “accept us” agenda appears through both popular culture and “in the broader national discourse on rights and belongings” (Walters 11), and is thus important because “representations of happy, healthy, well integrated lesbian and gay characters in film or television would create the impression that, in a social, economic, and legal sense, all is well for lesbians and gay men” (Schacter 729). Essentially, these programmes shouldered the burden of representation for the lesbian community, which was a heavy expectation. Critiques of the original L Word focussed on how the original cast looked as if they had all walked out of a high-end salon, for example, but in L Word: Generation Q this has been altered to have a much more DIY look. One of the younger cast members, Finlay, looks like someone cut her hair in the kitchen while others have styles that resemble YouTube tutorials and queer internet celebrities (Vanity Fair). The recognisable stereotypes that were both including and excluding have also altered the representation of the trans characters. Bette Porter’s campaign manager, for example, determines his style through his transition story, unlike Max, the prominent trans character from the first series. The trans characters of 2019 are comfortable in their own skins and supported by the community around them. Another important distinction between the representation of the old and new cast is around their material wealth. The returning cast members have comfortable lives and demonstrate affluence while the younger cast are less comfortable, expressing far more financial anxiety. This may indeed make a storyline that is closer to heterosexual communities. The L Word demonstrated a sophisticated awareness of feminist debates about the visual representation of women and made those debates a critical theme of the programme, and these themes have been expanded further in The L Word: Generation Q. One of the crucial areas that the programme/s have improved upon is to denaturalise the hegemonic straight gaze, drawing attention to the ways, conventions and techniques of reproduction that create sexist, heterosexist, and homophobic ideologies (McFadden). This was achieved through a predominantly female, lesbian cast that dealt with stories amongst their own friend group and relationships, serving to upend the audience position, and encouraging an alternative gaze, a gaze that could be occupied by anyone watching, but positioned the audience as lesbian. In concluding, The L Word in its original iteration set out to create something unique in its representation of lesbians. However, in its mission to create something new, it was also seen as problematic in its representation and in some ways excluding of certain gay and lesbian people. The L Word: Generation Q has therefore focussed on more diversity within a minority group, bringing normality and a sense of ‘realness’ to the previously skewed narratives seen in the media. In so doing, “perhaps these images will induce or confirm” to audiences that “lesbians and gay men are already ‘equal’—accepted, integrated, part of the mainstream” (Schacter 729).
