Abstract:
This work deals with analysis of phonation in patients with Parkinson’s disease (PD). Approximately 90% of patients with Parkinson’s disease suffer from speech motor dysfunction called hypokinetic dysarthria. System for Parkinson’s disease analysis from speech signals is proposed and several types of features are examined. Czech Parkinson’s speech database called PARCZ is used for classification. This dataset consists of 84 PD patients and 49 healthy controls. Results are evaluated in two ways. Firstly, features are individually analysed by Spearman correlation, mutual information and Mann-Whitney U test. Classification is based on random forests along with leave-one-out validation. Secondly, SFFS algorithm is employed for feature selection in order to get the best classification result. Proposed system is tested for each gender individually and both genders together as well. Best result for both genders together is expressed by accuracy 89,47 %, sensitivity 91,67% and specificity 85,71 %. Results of this work showed that the most important vowel realizations for phonation analysis are sustained vowels pronounced with maximum or minimum intensity (not whispering).

Abstract:
This work deals with a design of hypokinetic dysarthria analysis system. Hypokinetic dysarthria is a speech motor dysfunction that is present in approx. 90 % of patients with Parkinson’s disease. Next there is described Parkinson's disease and change of the speech signal by this disability. The following describes the symptoms, which are used for the diagnosis of Parkinson's disease (FCR, VSA, VAI, etc.). The work is mainly focused on parameterization techniques that can be used to diagnose or monitor this disease as well as estimate its progress. A protocol of dysarthric speech acquisition is described in this work too. In combination with acoustic analysis it can be used to estimate a grade of hypokinetic dysarthria in fields of faciokinesis, phonorespiration and phonetics (correlation with 3F test). Regarding the parameterization, new features based on method RASTA. The analysis is based on parametrization sentences complicated for articulation. Experimental dataset consists of 101 PD patients with different disease progress and 53 healthy controls. For classification with feature selection have selected method mRMR.

Neither any of immunologic nor nonimmunologic factors tested in our study was found to represent a risk factor for early detected TxCAD development except of donor age and recipient's BMI. Despite proving survival to be worse in recipients suffering from early detected TxCAD in compare to the rest of OHTx recipients, we have failed to demonstrate, that early detected TxCAD is an independent risk factor for OHTx recipients negative clinical outcome. Similarly despite demonstration, that serious cardiac events are more frequent in subgroup of patients with early detected TxCAD, we have failed to prove early detected TxCAD to represent an independent risk factor for its occurrence.

Abstract:
Present work deals with the solution of the strain between Jewish-Christian picture of good and loving God and a tragic reality of AIDS phenomenon. The work looks for answers to questions such as: Is a human being able to influence their health by an ethical way of life? To what extend are we responsible for our diseases and difficulties? Is a disease God´s punishment?{\crqq} What is (can be) the sense of a disease? How can we explain the suffering of the innocent? The work looks for the connections between a disease and a sin, between AIDS and an immoral life. The work presents the HIV/AIDS issue as a phenomenon concerning all people without a difference. It solves if it is possible to perceive AIDS as a kind of God punishment for sinful and immoral behaviour of people, as a consequence of structural maleficence or a personal carelessness of a human person. It points out the methods of primary prevention and reminds the task of a Christian to bring Christian values into a human society. The way out is found in the change of thinking and behaviour of people: in the appreciation and acquirement of ethical demands and opening to God blessing for the assumption of belief.

Abstract:
The aim of the dissertation is to treat psychiatry and mental illness from the anthropological point of view. The topic is treated at three levels. First, I describe psychiatry as a specific culture's product, and I explore how it constitutes itself as an unbiased science. I conclude that in spite of developing its scientific aspect, its human-science aspect is neglected, stuck in the 19th century's paradigm of evolutionism. Although there is a number of sources of a new less scientistic and ethnocentric and more reflexive paradigm nowadays, it still seems to be more a vision than reality due to the institutional toughness of the present paradigm. Second, I study "mental illness" as an abstract concept. I ask if the Western psychiatry's classification is universal and I explore various culturally specific forms of mental illness and its conceptualizations. Third, partially grounded in my fieldwork, I ask how abstract psychiatric terminology is embodied in a concrete human being and how the sufferer's point of view differs from the one of the professional in the mental health care. The dissertation intends to show that despite all the three levels being interlocked, their unity is not self-evident but is always constructed and contested.

The aim of the experimental part was to evaluate and quantify the cerebrospinal fluid and brain tissue of eight individuals suspected from prion disease, Creutzfeldt-Jakob disease specifically. Methods: To determine the presence of 14-3-3 protein in cerebrospinal fluid and prion protein in the native brain tissue an analytical method called Western blotting was used. Results were expressed only qualitatively. For better differential diagnosis, immunological examination of so-called CSF triplet (p-TAU, h-TAU, β-amyloid) was completed. The quantitative determination was performed by enzyme immunoassay (ELISA). Results: All eight subjects suspected from the prion disease were confirmed with the diagnosis of Creutzfeldt-Jakob disease. Determination of the prion protein post mortem has a great benefit for final confirmation of the disease. Detection of presence of...

This diploma thesis is dealing with issues of employer's liability for industrial injuries and occupational diseases and from that consequent obligation to compensate damage or non- material damage, through individual types of compensation. The first part provides a brief excursion into the development of the legislation on industrial injuries and occupational diseases and deals with the question of mandatory statutory insurance of the employer, implemented through two private insurance companies. The second part of the diploma thesis deals with the circumstances, which lead to emergence of employer liability for industrial injury. Substantial space is dedicated to situations where it may not be obvious at first sight that an industrial injury may occur. The industrial injuries resulting from teambuilding, business trip and injury originating from myocardial infarction are discussed in more detail. Further, here are defined the facts leading to the partial or total deprivation of liability by the employer. These facts are recklessness, own fault violation of regulations or instructions and drunkenness or misuse of addictive substances. The third part concerns with conditions under which the employer is liable for occupational diseases. The fourth part deals with individual types of compensation to...

This thesis is inspired by the theory of social construction of mental illness. The work is a case study that uses the metaphor of the story and narrative analysis of interviews examining the individual human experience with mental illness. My goal was to find out what makes us understand the disease and how this understanding can be used in a daily struggle with the disease. In the theoretical part, I drew from documented knowledge about mental illness, particularly for depressive disorder and schizophrenia simplex. Then I presented the narrative approach as a legitimate way of exploring understanding of lived experience. The practical part is implemented as a case study and through narrative analysis provides interesting insight into the way of understanding and coping with mental illness a person who has the expertise and also suffer from mental illness. Key words: mental illness, social construction, narrative, narrative approach, the dominant medical discourse, depression, schizophrenia simplex, stigma, coping strategies.

Abstract:
The study called ILLESS AS THE OPPORTUNITY TO CARE AND CONSIDERATION Is fenomenologically focused on a variety of seeing the world affected by an illness. Bio-psycho-social-spiritual model of illness is generally understood as the integration of the figures concerning body, mind and socially-spiritual conditions. The solution is arranged in a multi-feature attitude to the therapy, involving bodywork and psychotherapy in terms of mindcare aiming to reach recovering changes, respectively metamorphosis. The basis is found in respect to the body-organism as a self-organizing system creating its own world coming from self-relating autopoiesis. Showing respect to patients brings unexpected theoretical and therapeutical options, concerning that issue there is the contraversion of traditional kartezian western thinking with radical contructivism, which as a new-age empirically advenced alternative of scientific positivism is apparently not fully practised in medicine nowadays. Illnes is seen as disaretation-exasperation,disbalance.Treatment in terms of techne on kartezian thesis puts emphasis on total organism rehabilitation but health in terms of physis cannot be brought back anymore.

Abstract:

Abstract:
Summary: Consilia played an important role in medieval but also early modern professional health literature. Literary "consilium" contained a written statement of one particular case, the patient's condition and disease as well as advice on a medical procedure where a doctor in accordance with the contemporary discourse analyzed symptoms, determined the diagnosis, prognosis and recommended its pharmacological treatment including possible technical interventions (venesection etc.). In the 16th century, the Consilia Literature was a common part of many eminent physicians' practice whereas nowadays it is unjustly neglected source of history of medicine, pharmacology, dietetics and so on. The first part of the dissertation is devoted to the definition of genre, the initial stages of its development and description of the specifics of the Middle Ages. Abstract:
This thesis deals with the scientific achievements of the inspirational scientist Marie Curie Sklodowska as well as with the issue of acute iradiation sickness. Two goals were set in the work. The first objective was to map the life and scientific work of Mrs. Curie. The second objective was to describe the causes and consequences of acute illness from radiation. At the beginning the theoretical part describes the life of Marie Curie Sklodowska. Her difficult beginnings at the University of Paris and her great achievements- discovering the elements of polonium and radium. Further, the work deals with acute radiation sickness together with basics of radiobiology. In the practical part, I tried to find an answer to this question. Why Mrs. Curie did not die from acute radiation sickness? I have gained a lot of information by studying literature, professional articles and internet resources. Most of the facts were obtained from foreign professional journals because there are not many sources in the Czech language. Thanks to this information, I could deal with this person more detailed. At the same time, in the case of toxic polonium-210, an estimated fatal dose for adults was calculated. In addition, calculations were made of how much Marie Curie Sklodowska would have to isolate and incorporate in order to die for the consequences of acute radiation sickness. For better clarity, the results are summarized in the tables. Safety precautions and health problems associated with work with radioactive elements would not be neglected. Marie Curie Sklodowska probably was not able to create a clean polonium, and therefore probably did not even accept this direct contact, which would be able to cause her death.

Abstract:
Huntington's disease (HD) is an inherited monogenic neuropsychiatric degenerative, progressive, and fatal condition. The disease onset is in middle age of the patient. The most prominent clinical features are motor impairment, progressive decline in cognitive functions, and personality changes. Any preventive or disease-modifying therapies are not available so far. Therapeutic interventions can only target symptoms. It is believed that the primary pathology of HD results from massive degeneration of neurons in the basal ganglia. However, the expression of mutant huntingtin was detected in all tissues. Thus the mutation in non- neuronal cells of the brain and in peripheral tissues contributes to the pathology of HD. Neuroinflammation, especially microglia activation, is involved in the pathogenesis of HD. Given evidence that mutated huntingtin is expressed in peripheral immune cells, it is possible that inflammatory changes detected in peripheral tissues may reflect the inflammatory process in central nervous system (CNS). Several recent studies indicated that the immune system could act as a modifier of HD neuropathology. In order to monitor the success of any disease- modifying drugs in the pre-manifest stage of HD it is important to identify robust biomarkers of the onset and disease progression....

Eating Disorders from a gender perspective: disease, or part of life? Bc. Kristýna Kaválková ABSTRACT This dissertation deals with a problem of "eating disorders", which are defined in medical literature, media and magazines as psychological disease, which relates to pathological way of dealing with food, body and himself/herself. It shows that that this area is not neutral from the gender point of view; eating disorders are usually women's matter. Based on semi-structured interviews within the framework of qualitative research, this work analyses opinions of women, who have experience with this, especially their experience of their physicality and daily life. It is also focused on how women perceive the presentation of eating disorders, which concerns them, in media, medical publications and society and how they interpret their status as those who are recovering from this illness. Women's testimonies point at the fact that their surroundings look at them as persons defined through their illness, as persons, who are exclusively connected with pathology and stigma. This view of surroundings, media and specialists, however, omit the diversity of human identities. It shows that women alone can interpret their experiences in different way. We will focus on the concept of suffering in the theology of John Paul II., especially his messages during the Days of the sick in this dissertation "Sickness and sick people in the messages of saint John Paul II. for the Worlds days of the Sick". Firstly we have to describe sickness in catholic theology to understand this topic better. We will continue further with the analysis of messages during the Days of the Sick under the pontificate of saint John Paul II. This in consideration of his other texts (especially the apostolic letter Salvifici doloris), John Paul's attitude towards the way sickness and the sick are viewed in different cultures, as well as his position on various kinds of suffering, will be streamlined and characterized in this work. We will also allow for the sickness in the life on John's Paul II. which particularly influenced his main view on sick people and we will also take into account his other writings.

Jiří Motl Spiritualita a duševní nemoc Role spirituality v životě lidí se zkušeností s psychotickým onemocněním Spirituality and Mental Disorder: The Role of Spirituality in the Lives of People with Experience of Psychosis Abstract The dissertation investigates the relationship of mental illness and religion. It focuses on the role of spirituality in people with experience of psychosis. In the theoretical part, the problem of drawing the line between pathological and normal spirituality is discussed, stressing the importance of a functional point of view and the need for a dialogic relationship with the ill person. The theoretical part goes on with mapping the potential of spirituality in the recovery process. Spirituality often helps the afflicted to perceive their illness as a meaningful process, making it easier to cope with the illness. At the end of the theoretical part, four alternative or complementary conceptual frameworks are presented. The empirical part consists of a qualitative research of nine subjects (believers with the experience of psychosis). The data collection and analysis is based mainly in the IPA method (interpretative phenomenological analysis). The research aims to map how mental illness affected the subjects' spirituality and vice versa....

Abstract:
The thesis deals with the lived everyday practice of patients with epileptic seizures and the types of knowledge they are working with in relation to seizures. Particular attention is paid to patients' embodied knowledge as the practical knowledge based on the experience of epileptic seizures and their bodies in general. The thesis shows how the types of knowledge (such as triggers, auras, forms of seizures or their management) are based on, created and learnt through the embodiment. Lifestyle regimens associated with seizure management are introduced as a link between general recommendations and individual regimens, with seizure management being negotiated with regard to other aspects of patient life. While adhering to management, there is some communication, cooperation and negotiation between the patient and his body. Finally, other forms of knowledge about seizures are outlined, such as those created by people close to the patient and those created by assistance dogs. (Furthermore), it is also explained what these forms of knowledge mean to the patient and how to work with them.

Abstract:
Huntington's chorea is a dominantly inherited disease caused by trinucleotide (Cytosine-Adenine -Guanine) expansion in a gene coding huntingtin protein. Carriers of these mutation show symptoms associated with motor impairment, a cognitive and psychiatric disturbance, which is called Huntington's disease (HD). The major sign of HD is striatal atrophy in the middle age of life. Since it is known that huntingtin protein participates in a lot of cellular processes, such as transcriptional regulation and metabolism, these processes change by its mutation. One of the features observed in HD pathogenesis is the presence of oxidative stress. The aim of the work was to monitor the molecular changes preceding the HD manifestation in the knock-in minipig model. As a material for monitoring molecular changes leading to this condition, primary fibroblasts were used. Whereas, the oxidative stress arises from an imbalance between oxidants and antioxidants, level of reactive species and lipid peroxidation together with expression of antioxidant response associated genes was measured. At the same time, expression of metabolic and DNA repair related genes was monitored. Although the differences in oxidative stress level or the expression of antioxidative response genes were not detected, the changes in the...

Abstract:
(anglický jazyk) The most common senile dementia, Alzheimer disease (AD), is characterized by a decline of memory and high cognitive functions. Typical post-mortem brain lesions are extracellular amyloid deposits, intracellular neurofibrilary tangles and ruined cholinergic and other neurotransmitters systems. Connection between damaged central cholinergic system and beta-amyloid accumulation remains obscure. We examined parietal cortex of young adult (7- month-old) female APPswe/PS1dE9 double transgenic mice which develope beta-amyloid fragments at high rate. Cholinergic synapses of these mice demonstrate functional presynaptic (stimulated acetylcholine release) as well as postsynaptic (muscarinic receptor-induced G- protein activation) deficits and reduction of cholinergic markers. The mRNA levels of choline acetyltransferase, vesicular acetylcholine transporter and M1 to M4 subtypes of muscarinic receptors were determined in transgenic and littermate controls using qPCR. Obtained experimental data does not show any changes in measured mRNA levels. These observations indicate that reduction of cholinergic synaptic markers and function is due to posttranscriptional events.

Abstract:
Huntington's disease (HD) is an autosomal dominant inherited neuro-psychiatric disease with usual onset in the middle age. The mutation, located on the short shoulder of chromosome 4, is an expansion of a nucleotide triplet, containing cytosine, adenine, guanine (CAG), with critical limit of 40+ repetitions. The principal symptoms include motor symptoms (chorea, dystonia, disorders of voluntary movements), progressive cognitive deterioration and neuropsychiatric symptoms (behaviour disorders, affective symptoms and so on). The clinical diagnosis is confirmed by a genetic test, which may also be carried out presymptomatically in offsprings of the diseased person. The objective of the 1st study consisted in the characterization of differences in psychiatric examination and neuropsychological testing among the people at risk (PAR), in whom it was recommended to delay the test, and people at risk, who were recommended to continue in the so-called predictive protocol. The total of 52 people have been examined (32 females, 20 males). In addition to the common psychiatric examination we have also administered the Eysenck Personality Questionnaire (EPQ-A), self-rating scale of general psychopathology (SCL- 90), three short cognitive tests - Trail making test, test of Verbal fluency and...

The thesis is titled The causes of injuries and deaths in the Czech Republic. The main objective is to highlight the constantly high death rate caused by car accident injuries and by a hidden threat of heart and cardio-vascular diseases resulting in most people's deaths. The thesis contains two parts: theoretical and practical. The theory includes specific terminology such as health, health determiners and the lifestyle. The next chapter analyzes most frequented diseases in the Czech Republic such as heart and cardio-vascular diseases, tumors, respiratory and digestive tract diseases, death, the most frequented injuries in the Czech republic and suicides. The practical part describes the research findings and results. Two types of questionnaires were used for this purpose. The former one was handed to randomly chosen people who had suffered any injuries. The latter one was given to medical doctors in order to trace the death causes in the course of a year. The questionnaire data were transferred into graphs and charts. The graphs and charts as well as the discusssion outcomes are described in the final part of the thesis. The paragaphs of my thesis contain the bibliographical and website references. In the appendixes both questionnaires are inserted.

The diploma is about the topic of multidisciplinarity in community care of people with mentally illness. I chose this topic because I meet clients who have lived for various periods of time knowing that they suffer of mental illness. Over time, I began deeply perceive how mental illnesses affect all areas of client's lives. In the modern sense of community-based psychiatric services, multidisciplinary cooperation should become a provider of more comprehensive, better quality and more effective services for people with mental illness who require health care, but also social care that facilitates inclusion in the society of people who suffer of mental illness. Multidisciplinary collaboration is an interdisciplinary collaboration in social and health services, illustrates different theories that use various terminologies. The collaboration of experts from different disciplines, in addition to the term multidisciplinary, can be referred also as interdisciplinary or intersectoral. Another reason for choosing the topic of the thesis is the increas of discussions and even practical steps within the Strategy of Psychiatric Reform in the Czech Republic. An integral part of the thesis is an introduction to mental health issues, it's history and current planned transformation, which is a priority within the...

This work focuses on summarizing suspected and proven links between personality traits and health. First we recapitulate various theories linking personality traits and health - Type A personality, optimism, pessimism, five-factor model of personality, self-esteem, self-efficacy and more. Next we concentrate on the perception of this connection with doctors through interviews on the topic of "personality and health" in the empirical part. In the empirical part semi-structured interviews with doctors on the topic of "personality and health" were used to observe how doctors themselves percieve this connection. The aim is to find out where the doctors find that connection while working with their patients and how they use it in their medical practice. Keywords: personality, health, disease, medical practice

Contemporary studies observed a phenomena that patients with COPD have increased resting energy expenditure (REE) that leads to loss of fat free mass and total weight. Our goal was to confirm or disprove this hypothesis. Our research ran in Department of research and development at University hospital Hradec Králové. In our study participated 30 patients, 26 men and 4 women, mean age 67 ± 8 let. Every patient was once examined after twelve-hour fasting. We determined REE and substrate utilization by method of indirect calorimetry. We confirmed increased REE from the average value of REE - 122 ± 14 % predicted by Harris - Benedict equation. None patient was hypometabolic, 7 were normometabolic and 23 hypermetabolic. Substrate utilization did not differ contrary to physiological values. Furthermore we discovered correlation among fat mass, fat free mass,...

Abstract:
Cognitive impairment is considered as essential feature of non-motor symptoms in Parkinson's disease (PD). It is a result of underlying pathological processes in the brain of PD patients and it leads to decreased quality of life. In this thesis an analysis of the structure and profile of cognitive impairment is presented with special emphasis on executive functions and memory. We take diagnostic entities developed for the description of PD cognitive spectrum such as mild cognitive impairment (PD-MCI) and dementia (PD-D) as examples of heterogeneity and different severity of cognitive impairment in PD. However, neuropsychological methods in Czech version that would measure these diagnotic units were not adequatly validated. In the experimental part we test a hypothesis, if gait disorder with falls in PD is interconnected with cognitive impairment, and if PD-fallers have more severe cognitive deficit than PD-non-fallers. On the basis of nine validity or normative data studies we show psychometric properties and clinical utility of several basic neuropsychological methods in the Czech population for memory (Rey Auditory Verbal Learning Test, California Verbal Learning Test, Second Edition, Memory For Intentions Screening Test and Enhanced Cued Recall Test), sustained attention and executive functions...

Abstract:
Thesis named: ?Pilgrimage and Religious Tourism as human need in health and illness? The phenomenon of pilgrimage and related religious tourism have been more and more discussed recently, although the history of pilgrimage origins in distant past. The main goal of the thesis was to describe two religious cultures, namely Christianity and Islam, to describe the history of pilgrimage and pilgrimage destinations of these two cultures and to compare them. The partial goals were to map attitudes of these cultures to health and illness, to identify the elements common for the pilgrims and to find out how religious and pilgrimage tourism influences these people in their relation to health and illness. The aim of the work was to identify and understand a person of different religion better, to grasp a human as an individual and to approach him/her this way. Qualitative research, for which four research questions had been set, was performed. The research itself had the form of an individual non standardized interview with a research sample consisting of four Christians and four Muslims. Case studies were elaborated upon the interviews. The case studies form the research base for categorization areas. The results based on the interviews were processed by means of frame analysis (of the categorized groups). The results show that Christianity and Islam have certain cultural differences. Pilgrimage and pilgrimage destinations differ substantially in Christianity and Islam. While Christians visit these places for spiritual and physical relaxation and to please for health, for Muslims the pilgrimage is a part of the ?five pillars of Islam?. This finding is the biggest difference of both the religious cultures in relation to pilgrimage and religious tourism. The topic of this thesis is not very typical for the nursing care field of study; however I would like to provide students and colleagues with valuable information based on the thesis. The thesis could be used as informational material for nurses and as study material for nursing care students.
