Abstract:
Paresis of the recurrent laryngeal nerve (RLN) is one of the serious complications of thyroid and parathyroid gland surgery. The intraoperative neural monitoring (IONM) enables to verify the functionality of RLN. The aim of this study is to compare the incidence of postoperative RLN palsy in patients who underwent surgery with and without the use of IONM RLN and to evaluate the positive and negative predictive values, sensitivity, specifi city and accuracy of the method. Methods: Retrospective analysis of thyroid gland surgeries performed within the period from 1. 7. 2016 to 1. 7. 2018. A total of 467 operations were performed (780 nerves exposed). One hundred and thirty procedures (215 nerves) were carried out without IONM (group A). In total, 337 procedures (565 nerves) were performed with IONM (group B). Results: In group A, unilateral postoperative RLN paresis occurred in 7 cases (3.26%); 6 of them were temporary (2.79%) and 1 was permanent (0.47%). In group B, unilateral postoperative RLN paresis occurred in 33 cases (5.84%); 32 of them were temporary (5.66%) and 1 was permanent (0.18%). The incidence of postoperative RLN paresis related to the use of IONM was not considered statistically signifi cant. (Chi-square test: P = 0.146; Fisher‘s exact test: P = 0.2015, P = 0.4715). The sensitivity, specifi city, positive predictive value, negative predictive value and accuracy were 78.79%, 99.25%, 86.67%, 98.69%, and 98.05%, respectively. There was no case of bilateral postoperative RLN paresis. Conclusion: There was no statistically significant diff erence in the incidence of postoperative RLN palsy in patients who underwent surgery with IONM compared to the group without IONM. The high negative predictive value, specifi city and accuracy indicate high reliability of the method. The IONM provides the surgeon with valuable information regarding the functional status of the nerve. This knowledge allows for changing the operative strategy during the procedure. Streszczenie Wstęp: Podeszły wiek jest istotnym czynnikiem ryzyka chorób jelita grubego wymagających leczenia chirurgicznego. Celem fizjoterapii po dużych operacjach brzusznych jest zapobieganie powikłaniom i redukowanie ograniczeń funkcjonalnych. Celem badania było określenie związku pomiędzy poziomem aktywności fizycznej a stanem funkcjonalnym i sprawnością wentylacyjną płuc we wczesnym okresie pooperacyjnym. Materiał i metody: 34 pacjentów zakwalifikowanych do planowej dużej operacji brzusznej w wieku 65+ przydzielono losowo do grupy usprawnianej metodą z wykorzystaniem elementów koncepcji PNF lub grupy usprawnianej tradycyjnie. Spirometrię natężoną (FVC, FEV1, PEF) i testy funkcjonalne (prędkość chodu, wstań i idź) wykonywano przed operacją i w czwartej dobie po operacji. Analizowano poziom aktywności fizycznej, wiek, płeć, BMI oraz poziom pooperacyjnej samodzielności (SAP). Do analizy wyników zastosowano test Kołmogorowa-Smirnowa, test t-Studenta dla prób zależnych oraz analizy korelacji r-Pearsona. Przyjęto poziom istotności p<0,05. Wyniki: W teście prędkości oraz wstań i idź nastąpiło wydłużenie czasu wykonania po operacji w porównaniu do poziomu przed operacją. W FVC %, FEV1 %, PEF % nastąpił spadek wartości. W grupie osób z wysoką aktywnością nie odnotowano istotnych statystycznie zmian wartości dla tych pomiarów. Osoby o wysokim poziomie aktywności miały istotnie wyższy wynik na skali SAP niż osoby o średnim lub niskim poziomie aktywności. Wnioski: Duże operacje brzuszne w istotny sposób pogarszają funkcję wentylacyjną płuc oraz sprawność i prędkość chodu chorych po 65 roku życia, we wczesnym okresie pooperacyjnym. Wysoka przedoperacyjna aktywność fizyczna starszych pacjentów zakwalifikowanych do dużych operacji brzusznych, korzystnie wpływa na funkcję wentylacyjną płuc oraz poziom pooperacyjnej samodzielności, sprawności i niezależności w chodzie w okresie szpitalnym.

Abstract:
The article was inspired by the authors’ reviews of modern productions of Handel’s Baroque operas in 2019, presented in the three German cities: Halle, Bad Lauchstädt and Bernburg. Halle, where Georg Friedrich Handel (1685–1759) was born, is also a venue of the annual International Handel Festival, dedicated to the works of the great Saxon and his contemporaries. The concept of the festival in 2019, Sensitive, heroic, sublime: Handel’s women, was devoted to studying the female images embodied in his operas. In considering the scientific and artistic concept, the authors concluded that the directors’ understanding of these operas has expanded through the integration into musical drama of the compositional principles of film editing and the expressive means of modern, primarily screen arts. To study this phenomenon, we turned to the scientific tools developed in Russia by the two Soviet researchers who have become seminal in their field. One of them was the psychologist Lev Vygotsky, who, exploring the spiritual world of the protagonists in fine literature, revealed their psychological contradictions, expressed in the conflict of the narrative and the plot. Another The other, Sergei Eisenstein, was well-versed in Vygotsky’s manuscript of his study, Psychology of Art, and, influenced by some of these ideas, created his own “psychology of art”, which is set out in his works of various years. The core of this concept was “the transition from the Expressive Movement to the image of the art work… as a process of interaction of layers of consciousness” [1, p. 188], which allowed for multiple entries into the artistic image (one of the authors has applied this method of analysis in a work devoted to the integration of the principles of painting and cinema: [2, pp. 63–86]). Some features of the cinematograph also support such entries, and the first among equals here is the principle of intellectual editing developed by Eisenstein. In his montage theory, other types of editing—linear, parallel, associative—have been generalized and developed into a large-scale system for exploring the psychology of heroes in art. The essence of these processes, identified in the article, made it possible for the authors to discern the phenomenon of building the meaning in the Halle directors’ interpretations of Handel’s operas, which arises from the merger of the two seemingly irreconcilable and conflicting layers of consciousness: Baroque and eclectic modern, which emerged at the turn of the last century.

Abstract:
Cel pracy: Ocena skuteczności i bezpieczeństwa zabiegu fakoemulsyfikacji z głęboką sklerotomią wysokiej częstotliwości u pacjentów ze współistniejąca zaćmą i z jaskrą otwartego kąta. Materiał i metody: Dziewięciu chorych, w wieku średnio 77 lat, poddano łączonemu zabiegowi fakoemulsyfikacji i głębokiej sklerotomii wysokiej częstotliwości. Oceniono zmiany ciśnienia wewnątrzgałkowego oraz liczbę stosowanych miejscowych leków przeciwjaskrowych. Wyniki przedstawiono jako średnią arytmetyczną z odchyleniem standardowym i poddano analizie statystycznej testem t-Studenta. Za istotne wartości przyjęto p < 0,05. Wyniki: W 6. miesiącu po operacji u wszystkich chorych obserwowano obniżenie ciśnienia wewnątrzgałkowego z wyjściowego średnio 23,56 ± 2,7 mmHg do 16,89 ± 1,54 mmHg. U czterech chorych całkowicie odstąpiono od podawania leków przeciwjaskrowych, a u pięciu zredukowano ich liczbę. Średnia liczba leków przed operacją i po niej wyniosła odpowiednio 2,44 ± 0,53 i 0,67 ± 0,71. Zmniejszenie lub zaprzestanie stosowania leczenia zachowawczego u znacznego odsetka osób wskazuje, że mikroinwazyjna operacja w postaci głębokiej sklerotomii wysokiej częstotliwości wykonana łącznie z operacją fakoemulsyfikacji jest skuteczną metodą leczenia chirurgicznego chorych z jaskrą otwartego kąta przesączania.

Abstract:
Purpose The Social Return on Investment (SROI) framework has been developed for mapping and measuring social impact. It may be used for legitimating organisations and projects. The framework is often criticised for its overemphasis of the SROI ratio, i.e. the relationship between monetised benefits and costs. This study aims to demonstrate how the SROI method legitimates organisations or projects with multiple other discursive ways besides the SROI ratio. It also discusses the status of these other ways of legitimation in relation to the quantifying and monetising core tendency of SROI. Design/methodology/approach The empirical data consist of an SROI guidebook and 12 SROI reports. Their study applies Theo van Leeuwen’s ideas for analysing the discursive legitimation of social practices. The study takes place broadly in the framework of Norman Fairclough’s critical discourse analysis, aided by qualitative content analysis. Findings In the analysis, the full spectrum of the van Leeuwenian legitimation means used by SROI – authorisation, rationalisation, moral evaluation and mythopoetical narration – is brought out in the data and the status and social context of the legitimation means are assessed and discussed. It is shown that there is existing potential for broader and more visible use of different legitimation means. Practical implications Based on the findings of the study, suggestions for the improvement of SROI reporting by a more balanced explicit use of the multitude of legitimation means are presented. Originality/value The study is original both in its subject (the spectrum of legitimation in SROI) and its method (qualitative discursive and contentual analysis of SROI as a legitimating discourse).
