Phencyclidine (PCP, ‘angel dust’) is an arylcyclohexylamine dissociative anaesthetic. It was first abused in the United States in New York and San Francisco in the 1960s, but abuse declined when a broad range of adverse complications was noted. Agents that alter perception and mood without disorientation typify hallucinogenic drugs. They have been known and used for millennia for purposes ranging from magical to medical. Hallucinogenic drugs comprise not so much a single class of compounds, but a multiple classes affecting different neuronal receptors. This chapter looks at the epidemiology, acute physiological effects, and adverse effects of both PCP and hallucinogens. it also covers PCP delirium, PCP-induced psychotic disorder, PCP abuse, dependence, and organic mental disorder, and finally human experimentation with hallucinogens.

Delirium is one of the most common and often serious neuropsychiatric complications in the management of cancer patients that is characterized by abrupt-onset, fluctuating disturbance in awareness, attention, cognition, and perception. Delirium is associated with increased morbidity and mortality, causing distress in patients, family members, and staff. Delirium is a medical emergency, a sign of significant physiologic disturbance, usually involving multiple medical etiologies, that needs to be prevented, identified, and treated vigorously. Unfortunately, delirium is often underrecognized or misdiagnosed and inappropriately treated or untreated in the medical setting. Psycho-oncologists who care for patients with cancer must be able to diagnose delirium accurately, undertake appropriate assessment of etiologies, and understand the benefits and risks of pharmacologic and nonpharmacologic interventions currently available. This chapter presents a comprehensive overview of the epidemiology, pathophysiology, clinical features, assessment, and management of delirium in cancer patients.

Cannabis use continues to be a public health concern and has become a major clinical and nonclinical research construct in numerous fields for numerous reasons. First, the slight alterations in the criteria for cannabis use disorder (CUD) diagnosis required validation and prevalence statistics. Second, the shift in social perceptions of cannabis use and its legalization by various states have led to significant increases in cannabis use, abuse, and individuals meeting the criteria for CUD. Additionally, understanding the psychological and neural mechanisms associated with CUD will allow for innovative and tailored intervention and treatment methods for those suffering from the condition. This chapter provides a brief overview of the central, peripheral, and autonomic processes associated with CUD. It then considers the current treatments for CUD that are informed by pharmacological and epidemiological information. Finally, the authors contrast CUD with cardiovascular dysfunction, providing inferences for CUD as a potential contributor to the leading cause of death: cardiovascular disease.

Qualitative research provides an adaptable, open-ended, rigorous method to explore local perceptions of an issue. Qualitative approaches are effective at revealing the subjective logic motivating behavior. They are particularly appropriate for research questions that are exploratory in nature or involve issues of meaning rather than magnitude or frequency. Key advantages of qualitative approaches include speed, flexibility and high internal validity resulting from emphasis on rapport building and ability to probe beneath the surface of initial responses. Given the time-intensive nature of qualitative analysis, samples tend to be small and purposively selected to assure every interview counts. Qualitative studies can be done independently or embedded in mixed-method designs. Qualitative data analysis depends on rigorous reading and rereading texts ideally with more than one analyst to confirm interpretations. Computer software is useful for analyzing large data sets but manual coding is often sufficient for rapid assessments in field settings..

Few would disagree that a diagnosis of childhood cancer is shocking and frightening to the child and to the child’s family. In addition to the sudden and dreadful diagnosis, the intensive and often lengthy treatment is extremely stressful. The combination of diagnosis and subsequent treatment frequently leaves children and parents feeling helpless. However, life-threatening illness has historically been categorized as stressful but not “traumatic” in the way that interpersonal violence or natural disasters are traumatic. In fact, the third edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-III) (American Psychiatric Association [APA], 1987) stated that “chronic medical illness” was specifically excluded as a potential precipitant of a formal psychiatric diagnosis of posttraumatic stress disorder (PTSD). This was important because PTSD is the only diagnosis in the DSM that includes a cause or precipitating event within the diagnostic criteria rather than simply offering a description of typical symptoms. Clinical observations challenging the perception that cancer was not sufficiently “traumatic” to precipitate the symptoms of PTSD began to be described in the psychiatric literature in the 1980s and prompted a series of field trials in preparation for the fourth edition of the DSM. A group of 24 adolescent cancer survivors and their mothers were interviewed, and findings revealed that a substantial minority of both groups met criteria for a diagnosis of PTSD (Alter et al., 1996). Life-threatening medical illness was subsequently included as a potential precipitating event for PTSD in the text of the fourth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) in 1994. The accompanying text specified not only that the person experiencing the medical life threat might respond with posttraumatic stress but also that this might also be a precipitant for family members. This allowed a common framework for researchers to investigate posttraumatic stress responses to medical life threat in adult patients, spouses of patients, and childhood patients, their siblings, and their parents. In this chapter, we review the growing body of literature that has emerged regarding the epidemiology, correlates, and predictors of posttraumatic stress symptoms in those exposed to medical life threat.

These common parasomnias include NREM parasomnias, considered to be disorders of arousal from NREM sleep and which include a heterogeneous spectrum of sleep behaviors encountered following arousal from NREM sleep exemplified by the phenotypes of sleep terrors, sleep walking, confusional arousals, sleep-related eating disorder, and sexsomnias; the REM parasomnias, including nightmare disorder, recurrent isolated sleep paralysis, and REM sleep behavior disorder, which is strongly associated with prodromal or overt alpha-synucleinopathy neurodegenerative disorders such as Parkinson disease and dementia with Lewy bodies; and other parasomnias, including a spectrum of “leftovers” that do not map well to NREM or REM sleep specifically such as exploding head syndrome, sleep-related hallucinations, sleep enuresis, and parasomnias ascribed to medical disorders, medication or substance use, or unspecified etiologies. It is important for a psychiatrist to be conversant with the complete range of parasomnias given the importance of effective management of frequent psychiatric and psychological comorbidities, which serve as drivers of the frequency and severity of nocturnal events, and to ensure timely referral for polysomnography or other necessary sleep diagnostics where appropriate.
