Abstract:

Bakgrund: Smärtor i ben är vanligt förekommande och neuropati (nervskada) är en orsak som troligen är underdiagnostiserad. Bimanuell (tvåhändig/tvåsidig) nervpalpation och känseltest med sporre har visat sig vara ganska tillförlitliga och enkla test för att hitta nervskada men har inte testats på friska individer.

Syfte: Syftet var att undersöka om bimanuell nervpalpation i fossa poplitea framkallar smärta/obehag och om det finns skillnad mellan vänster och höger sida vid bimanuell undersökning med sporre på underben hos friska försökspersoner.

Metod: Ett bimanuellt palpationstest av nervi tibialis och peroneus i fossa poplitea och även ett bimanuellt känseltest med sporre över dermatomen L4, L5 och S1 på underben genomfördes. Urvalet var ändamålsenligt och totalt deltog 37 försökspersoner. Åldersspannet var 20 till 57 och medianålder 23.

Resultat: Vid palpationstestet hade intensiteten av smärta/obehag en median på 1 (variationsvidd 3) på den 11 gradiga skalan. En stor del skattade olika mellan sidorna i både palpationstestet (11 av 37) och känseltestet med sporre (25 av 37). Det var inte någon större skillnad mellan könen.

Slutsats: När man utför dessa nervtester måste man ta hänsyn till att även friska individer ofta anger en liten sidoskillnad och inte alltid skattar noll vad gäller smärta/obehag. Det behövs dock fler studier för att bekräfta dessa resultat.


Background: Leg pain is common and neuropathy (nerve disease) is one reason which probably is under diagnosed. Bimanual (bilateral) nerve palpation and sensory test with spurs has been shown to be quite reliable. Furthermore, the tests are straight forward detecting nerve disease but have not been tested on a healthy population.

Purpose: The purpose was to investigate whether peripheral nerve palpation in fossa poplitea induces pain/discomfort, and if side difference exists in a sensibility test with spurs on the lower leg in healthy subjects.

Method: A bimanual palpation test of the tibial and peroneal nerve in fossa poplitea and also a bimanual sensibility test with spurs of dermatome L4, L5 and S1 on the lower leg were carried out. In order to find healthy subjects a purposive sampling was made. A total of 37 subjects between 20 and 57 years with a median age of 23 participated in the study.

Results: At the palpation test the intensity of pain/discomfort had a median of 1 (range 3) in the 11 degrees of pain scale. A large part estimated differences between the sides in both the palpation test (11 of 37) and the sensibility test with spur (25 of 37). There was no significant difference between the sexes.

Conclusion: When performing these nerve tests it is important to keep in mind that even healthy individuals might perceive some pain/discomfort as well as side difference. However, we need more studies to confirm these results.

Introduktion: Elektroneurografi används för att bedöma nervfunktion och påvisa patologiska förändringar i det perifera nervsystemet. Det är en metod känslig för störningar och registreringarna kan påverkas av temperaturen i huden och underliggande vävnad, då värme ökar de perifera nervernas nervledningshastighet.   Fysisk aktivitet har bevisats ha positiva effekter på hälsan och involverar många av kroppens organsystem. En sådan effekt är en ökad metabolism i skelettmusklerna, vilket genererar värme.   Syfte och frågeställning: Syftet är att undersöka de perifera nerverna i en nedre extremitet hos en grupp neurologiskt friska individer utan diagnosticerad diabetes med elektroneurografi, för att se om fysisk aktivitet har någon effekt på den perifera nervfunktionen.   Metod och material: Tre perifera nerver i individernas (n=18) dominanta ben; nervus (n) suralis, n. peroneus och n. tibialis undersöktes med elektroneurografi före och efter måttlig fysisk aktivitet. N. suralis undersöktes sensoriskt och n. peroneus och n. tibialis undersöktes motoriskt och F-respons registrerades. Hudtemperatur och kroppstemperatur mättes inför samtliga registreringar och ansträngningsgrad vid den fysiska aktiviteten uppskattades.   Resultat: Signifikant skillnad mellan registreringarna från n. suralis kunde ses vad gäller latenstid och ledningshastighet, och den distala amplituden, proximala latenstiden samt ledningshastigheten för n. peroneus. Dessutom kunde signifikant skillnad ses i den proximala latenstiden för n. tibialis, och i F-M-latenstiden för n. peroneus och n. tibialis.   Slutsats: En positiv effekt kan ses i en del variabler, troligtvis beroende på temperaturskillnaderna mellan registreringarna. Dock skulle en större undersökningsgrupp potentiellt ge tydligare resultat.
Introduction: Electroneurography is used to assess nerve function and detect pathological changes in the peripheral nerve system. The method is sensitive to interference and measurements can be affected by temperature of the skin and underlying tissue, because heat increases nerve conduction velocity of the peripheral nerves.   It has been concluded that physical activity have a positive effect on health and involves many of the body´s organsystems. One of these effects is an increase in metabolism in skeletal muscles, which generates heat.   Purpose: The purpose is to examine the peripheral nerves in a lower extremity in a group of neurologically healthy individuals without diagnosed diabetes with electroneurography, to determine if physical activity has any effect on the peripheral nerve function.   Method and material: Three peripheral nerves in the individuals (n=18) dominant leg; nervus (n) suralis, n. peroneus and n. tibialis, were examined with electroneurography before and after moderate physical activity. N. suralis were examined for sensory function and n. peroneus and n. tibialis were examined for motor function and F-response. Skin temperature and body temperature were measured before every registration and the individuals assessed their degree of physical effort.   Results: Significant diffrence between the registrations could be seen in the latency and conduction velocity for n. suralis, in the distal amplitude, proximal latency and conduction velocity for n. peroneus, in the proximal latency for n. tibialis and the F-M-latency for n. peroneus and n. tibialis.   Conclusion: A positive effect can be seen on some variables, probably because of temperature diffrences between the registrations. However a larger test group could potentially yield more distinct results.

Abstract:
Syftet med studien var att utvärdera patientens upplevelse av den postoperativa smärtan och behandlingen av denna när den perifera nervblockadens effekt avtagit efter operation av distal radiusfraktur. Metoden som använts är en empirisk kvantitativ deskriptiv enkätstudie. Tjugo patienter deltog i denna pilotstudie. Demografin stämde väl överens med de statistiska data, som visar att distrala radiusfrakturer är vanligt förekommande hos kvinnor i åldern 55 år och äldre. Resultatet av studien har påvisat att många patienter har upplevt mer smärta än vad de själva varit beredda på. Slutsatsen utifrån visuell analog skala, VAS, visar att smärtan har varit svår för patienterna att behandla hemma trots att de haft tillgång till smärtbehandlade läkemedel. Informationen som patienten erhållit har varit otillräcklig och kan vara en källa till den svårbemästrade smärtan.
The aim of the study was to evaluate patient´s experience of the postoperative pain and its treatment after peripheral nerve blockade for operation of a distal radius fracture had ceased. The method used was an empiric, quantitative, descriptive study using enquiries. Twenty patients participated in this pilot study. Demography was according to statistical data, that distal radius fractures are common in women aged 55 or more. The results showed that many patients experienced more severe pain than expected. The conclusion from visual analogue scale, VAS, shows that the pain was difficult to treat at home in spite of prescription of pain killers. The information given to the patient was insufficient, which may have contributed to the difficulty to control pain.

 ABSTRACT  Background: Since its first mention by Kirk et al. in 1967 and its recognition as a full- fledged rheumatologic disorder, the hypermobility syndrome (HMS) has been increasingly investigated and reported in the scientific literature. Expeditiously renamed benign joint hypermobility syndrome in the patent absence of life-threatening complications, its relatively innocuous character has been progressively reconsidered. In fact, the HMS tends to date to be considered analogous to the Ehlers-Danlos syndrome-hypermobility type, a heritable disease of connective tissue, and therefore emerges as a chiefly rheumatologic disorder with possible widespread reverberations in practically all organs and systems. The condition thence goes beyond the sole involvement of the musculoskeletal system and is recurrently associated with seemingly-unrelated and more or less severe conditions (cardiovascular, pulmonary, gastro- intestinal…). However, neurologic implications of the hypermobility syndrome remain poorly documented, particularly those regarding the peripheral nervous system. Ranking amongst the afflictions of the latter, nerve entrapment syndromes (NES) comprehend a multitude of categories, notably the thoracic outlet syndrome (TOS). This thesis summes up findings concerning the pathogenesis, clinical picture, diagnostic, and the therapy of peripheral paresis of the facial nerve, with accent on physiotherapeutical methods of treatment of such disorder. Here is also a description of Polyelectromyography that could become one possibility how to objectively judge the differences in the activation of mimical muscles in patients diagnosed with peripheral paresis of the facial nerve during various kinds of mimical movements. The practical part is concerned with and is talking about the use and application of my knowledge of the theory in practice with providing therapy to my patients suffering from peripheral paresis of the facial nerve by applying Vojta's reflex locomotion. Abstract:
There is rising importance of the brachial plexus injury firstly due to its increasing incidence as a result of higher survival rate of the patients after polytrauma and due to the higher success rate of the surgical treatment. Recently, the obstetric brachial plexus palsy is the area of interest of many articles. The indication criterion is one of the most important problems to be solved as well as increasing awareness about the possibility of the surgical treatment. Many of injured elements of the brachial plexus are reconstructed by a donor nerve transfered onto the recipient nerve, i.e. by neurotization. This method can have better outcome than the suturing of the proximal parts of the plexus especially due to its main advantage - suturing as close to the muscle as possible. It is still not clear which donor nerve is better to use for neurotization of specific recipients due to the lack of large randomized clinical trials. End-to-side anatomosis (ETS) was re-discovered at the beginning of 90's at the same time when the Oberlin method (OT) was firstly described in the literaure. These techniques are applicable only in the Erb's palsy with functional lower part of the plexus. Title: Electromyographic and clinical evaluation of vincristine-induced peripheral neuropathy in pediatric patients after treatment of acute lymphoblastic leukemia and correlation with the Bruinkins-Oseretsky Test of Motor Proficiency Second Edition Objectives: The aim of this work was to characterize the neurological consequences of vincristine-induced peripheral neuropathy (VIPN) clinically and electromyographically and to evaluate motor skills of pediatric patients after the end of treatment of acute lymphoblastic leukemia. We also determined the relationship between the results of the clinical and electromyographic evaluation of VIPN and the correlation with the results of motor skills tests. Methods: The study involved 35 probands (19 girls and 16 boys) with a mean age of 10.7 years (SD ± 4.3) and a mean time since the last dose of vincristine of 2.3 years (SD ± 1.2). VIPN was assessed using a clinical pediatric-modified Total Neuropathy Score (ped-mTNS) and nerve conduction studies (NCS). Motor skills were assessed using the Bruinkins-Oseretsky Test of Motor Proficiency, Second Edition (BOT-2).
