Based on fishbone diagrams, the problem was obtained that the implementation of handover of patient patients was not optimal.Results: To optimize the implementation of handover, implementation was carried out in the form of improving SPO, improving nurses' effective communication with SBAR and making role play video handover.Conclusion: The result is the handover can be optimal with the role and function of management of the head nurse especially a directing role. Recommendations given by follow up and revising the hand over standars, make a pilot project nursing ward and evaluate the implementation of the patient's handover through regular supervision.Keywords: Fishbone Diagram; Handover; Head Nurse; Optimization; DirectingPendahuluan: Serah terima perawat antar shift jaga merupakan salah satu komponen penting dalam mencapai keselamatan pasien dan mempertahankan kualitas asuhan.Tujuan: Meningkatkan optimalisasi fungsi pengarahan kepala ruang dalam pelaksanaan serah tertima antar shift di RS X Jakarta.Metode: Menggunakan study analysis situasi dengan pendekatan diagram tulang ikan, dimulai dengan mengisi kuesioner, observasi dan wawancara dan kemudian dianalisis menggunakan diagram tulang ikan. Berdasarkan diagram tulang ikan, masalah diperoleh bahwa pelaksanaan serah terima pasien pasien belum optimal.Hasil: Untuk mengoptimalkan pelaksanaan serah terima, implementasi dilakukan dalam bentuk meningkatkan SPO, meningkatkan komunikasi efektif perawat dengan SBAR dan membuat role play video serah terima.Simpulan: Hasilnya adalah serah terima dapat dioptimalkan dengan peran dan fungsi manajemen kepala ruang khususnya fungsi pengarahan. Rekomendasi yang diberikan dengan menindaklanjuti dan merevisi SOP serah terima, membuat ruang percontohan dan mengevaluasi pelaksanaan serah terima pasien melalui pengawasan rutin.

Abstract:
In the development of a system, quality assurance is needed. A system can be said to be of good quality if it meets the needs of the user. Quality can be measured from various points of view. One point of view to measure software quality is to focus on service management systems. To determine the quality of a system, it is necessary to analyze the quality of the system. The school library system at MTs Muhammadiyah 8 Ujungpangkah has implemented a lot of service management systems to support the process of activities in the library. However, it is not yet known whether the system is in accordance with the assessment standards or not.The author uses the ISO 20000 standard to assess the requirements for obtaining a certificate in the library application by conducting various analyses. This study aims to analyze the application of the library system with ISO 20000. The analytical research methods used include determining clauses, checklists, Likert scale, maturity levels, and fishbone diagrams. The references that I take are based on the results of the analysis of the writing team that I have made including graphs, diagrams, clause scores, checklist scores, Likert scales, maturity levels, and fishbone diagrams. Based on the results of the author's analysis, the school library application system at MTs Muhammadiyah 8 Ujungpangkah does not meet the ISO 20000 standard so that there is still much that needs to be improved in the library system. The Likert scale score is only 11%, while to meet the ISO 20000 standard the Likert scale score must reach at least 60%. The overall problem with this analytical assessment that results in the Library System application not passing the ISO 20000 quality standard is the lack of a Likert scale score and fishbone diagram. With fishbone diagram, the main root cause is found with organitation structure that is not clear to monitor the application working. The hypothesis in this study can be the basis for future research so that the application can meet the feasibility test of the ISO 20000 quality standard.

Abstract:
Cinema ticket application services make it easy to purchase movie tickets. Various applications, especially in the business sector, certainly require information security that has been guaranteed to be applied and international information security certificates in ISO 27001 quality standards. Therefore, the requirements for obtaining information security certificates in international quality ISO 27001 need to be analyzed first. It requires a lot of preparedness that must be requested in meeting ISO 27001 quality standards. The purpose of this study is to analyze the application of cinema tickets for ISO 27001 eligibility. Analytical research methods used include clauses, checklists, Likert scales, maturity levels, and fishbone diagrams. The authors' references were based on the writer's team's analysis that the authors made, including graphs, charts, clause scores, checklist scores, Likert scales, maturity levels, and fishbone diagrams. Based on the author's analysis results, the cinema ticket application system does not meet ISO 27001 standards, so much needs to be improved in the system. Likert scale scores are still 19%, while to meet ISO 27001 standards, Likert scale scores must reach at least 60%. The overall problem with assessing this analysis, which results in cinema tickets being inappropriate or not passing ISO 27001 quality standards, is the lack of Likert scale scores and fishbone diagrams. This analytical research hypothesis can be the basis for future research that the application can meet the feasibility test of ISO 27001 quality standards.

Purpose - This study was conducted with the intention to find out what is included as non-value added activity that is happened in patient’s flow and how to reduce it using lean six sigma method in outpatient department of regional public hospital of Kerinci M.H A Thalib.Methodology - This research uses lean six sigma method with analysis tools in the form of value stream mapping and fishbone diagrams in order to reduce the activities identified as non value added in the outpatient units of the M. H A Thalib general hospital in Kerinci Regency.Finding - The results of the study showed that the lead time required in the flow of patients in this outpatient unit is 7300.27 seconds with a percentage of Process Cycle Efficiency is only 12.22%, and contributing factors such as databases that are not used effectively, delays in medical personnel, lack of infrastructure, and so on. The parking application service at the University of Muhammadiyah Gresik is an update from the previous system. Of course, this application requires a system that is practical, efficient, and also has good data storage security. One of the references that can be used that these things have been fulfilled in this application is an analysis using ISO 9241-11. The purpose of this study was to analyze the application of UMG parking for the feasibility of ISO 9241-11. Analytical research methods used include determining clauses, checklists, Likert scales, maturity levels, and fishbone diagrams. Clause, checklist use for getting assessment based on the park system, then from that result will be used to get the qualification of this system with liket scale in each clause of this standard, and assessment of all clause with likert scale , the result of level quality assurance of this system will be found. Next the using fishbone diagram can analyze to get the root cause of clause dan quality assurance in ISO 9241-11. Because that , with this study we can know the qualification and the root cause of park system. And the analysis using that tool, this system still not suitable to use ISO 9241-11. The parent of root cause is in efficiency clause in organitation like less costing and planning.

Abstract:
The paper deals with quality control in a Czech manufacturing company, especially at its quality department. The objective is to define the identified production failures, using selected quality management tools, and to determine the causes of technological problems. The following methods and tools were used: cause and effect analysis—Fishbone diagrams (Ishikawa diagrams); Pareto charts, flowcharts and others methods, whose results have been visually displayed. Based on a detail analysis of the production failures, the proposal to eliminate them as well as a draft implementation of the corrective action and its effect on enhancing quality in the production company are presented. The elements such as frequency of controls, adherence to regular inspections and replacement of filters in air conditioning units, cleaning and compliance with the work rules were monitored, as these elements have an impact on product quality and customer satisfaction. The implementation of the corrective actions and the related financial estimate are presented.

Abstract:
Freight forwarding is a logistics company which provide land delivery services rely heavily on trucking vendors. The available trucking vendors may not fulfill the company's expectations. Therefore the research was conducted to analyze the performance, selection process, management and development of 26 trucking vendors. The research began with the formation of a Critical Success Factor (CSF) and its development, namely a Key Performance Indicator (KPI) in accordance with company objectives. Then the CSF and KPI was weighted through the Analytic Hierarchy Process (AHP) method, determined the optimal performance value of each KPI, measured the performance of each vendor through the scoring method, analyzed the performance of the trucking vendor that was not optimal through Root Cause Analysis (RCA) by looking for the cause based on fishbone diagrams and FMEA tables, as well as providing suggestions regarding the management and development of trucking vendors. The results of the study were 11 CSFs and 28 KPIs that were formed. Based on the selection process conducted on 26 trucking vendors, there are 3 excellent vendors, 4 very good vendors, 11 good vendors, 3 matching vendors, and 5 unsatisfactory vendors. Furthermore, researcher conducted root cause analysis (RCA) of vendor representatives “matching” and “unsatisfactory” categories with fishbone diagrams and FMEA tables. Where the 5 highest RPN values from the FMEA table were given suggestions for improvement. Meanwhile, the excellent, very good, and good vendor categories are developed through strategic plans for the future.

The higher education institutions are hereby an important sector of any society since it defines the overall quality of human lives. This research will investigate the application of Total Quality Management (TQM) tools at the higher education institutions; specifically at ALHOSN University. In this study five tools were implemented at ALHOSN University’s engineering college including: Quality Function Deployment, Affinity Diagrams, Tree Diagrams, Pareto Charts, and Fishbone Diagrams. The research will reveal that the implementation of TQM tools has a great benefit for higher education institutions where they have uncovered many area of potential improvement as well as the main causes of some of the problems the Faculty of Engineering is facing. Also, it will show that the implementation of TQM tools on higher education institution systems will enhance the performance of such institutions.</p>

Abstract:
Quality is one aspect that needs to be maintained in increasing consumer confidence and minimizing production costs. A motorcycle tire manufacturing company has a production defect of 5028 during February 2018. This study aims to reduce the number of defective products in the green tire cooking process in the curing area. The methods used in this study are seven tools, especially Pareto diagrams, and fishbone diagrams and analyzed the cause is 5W1H. Of the six types of green tire defects, under cure is the most dominant factor (40%) that causes disability. The improvement in Pneumatic Problems which the problem of under cure is human factors, machinery, and the method of being able to reduce defects by 48% from 891 pieces in February to 463 pieces in May. The decrease in defects in pneumatic problems affects the reduction in the number of defects in green tire products from 5206 pieces in February to 4699 pieces in May.

Magister Commercii - MCom<br>This research project focuses on the application of process improvement techniques in a clothing manufacturer to address delay problems in workflow in the factory. The objective of the research is threefold; investigate delays at the beginning of production and make suggestions, show the usefulness of continuous improvement techniques in improving activities in a clothing manufacturer and demonstrate how action research can be used in doing research in production and operation management. Using tools such as flow charts, check sheets, pareto analysis, fishbone diagrams, interviews and the "ask why five times" tool, an investigation into delays led to a second investigation into sewing defects. This established that these sewing defects are caused mainly by time constraints, the malfunctioning of machines, the wrong handling of garments, and previous operations. After an investigation using the above tools it was established that these defects could be addressed by setting realistic targets, doing regular maintenance on machines, cautioning operators to be more careful during their operations, and encouraging regular checks on garments before the next operation. The action learning methodology led to the following lessons; selecting a correct measuring tool is important, that not all tools need to be used, and that it takes time to do a research project using this method.

Dissertação de Mestrado em Engenharia Informática apresentada à Faculdade de Ciências e Tecnologia<br>Este relatório de mestrado, dissertação sob o tema “Transition Design – Shared Online Canvas”, tem por objetivo apresentar o trabalho realizado, ao longo do ano letivo 2019/2020, no âmbito da unidade curricular de estágio, integrada no mestrado de Engenharia Informática da Faculdade de Ciências e Tecnologia da Universidade de Coimbra. Através do estudo efetuado ao longo dos primeiros três meses fez-se uma síntese sobre os conceitos-chave relacionados com a temática do Design de transição, Design participativo e suas relações com media dinâmicos interativos. Essa investigação decorreu, principalmente, da necessidade de desenvolver uma plataforma computacional que, através de ferramentas web, permitisse a partilha de posições entre “stakeholders”, em tempo real, utilizando “canvas”. O projeto desenvolvido, visa suportar processos de Transition Design, utilizando uma plataforma informática de colaboração, permitindo que um conjunto de utilizadores possam desenhar soluções que respondam às necessidades de mudança ou inovação social. Com o objetivo de desenvolver um protótipo para o propósito anteriormente descrito, facilitando processos colaborativos, fez-se um primeiro levantamento dos requisitos e das especificações do projeto. A conceção da ferramenta web desenvolvida implicou também o desenho das interfaces com o utilizador, necessárias para acomodar os canvas delineados para o design de transição. Para garantir a interligação entre o frontend de utilizador e o backend do servidor, e respetiva base de dados, projetou-se uma arquitetura adequada ao respetivo suporte de dados. Como o sistema precisava de ser cooperativo, e o desenvolvimento duma plataforma desse tipo envolveria muito tempo e elevada complexidade, utilizou-se uma forma inovadora de alcançar este objectivo através de substratos web (webstrates), que permitem criar uma interface distribuída e cooperativa, aproveitando potencialidades dos media dinâmicos existentes na web. Dessa forma, utilizando dois servidores, um servidor web e um servidor de “webstrates” implementou-se a plataforma de canvas partilhados para Transition Design, de uso cooperativo, partilhado e na web, que agora se apresenta.A plataforma desenvolvida, nas suas funcionalidades, permite a credenciação e o login dos utilizadores, a definição e consulta de projetos, o lançamento de temas e objetivos em cada projeto, e a criação de diagramas associadas a cada projeto, além de permitir um blog de discussão entre os participantes em cada projeto. Os canvas que são possíveis associar aos projetos são: wicked problem diagrams, fishbone diagrams, future vision, spatio-temporal matrices, e stakeholder perspective maps. A plataforma tem por principal objetivo, permitir a discussão de ideias e o desenho de soluções através da cooperação entre stakeholders envolvidos no projeto de transição.<br>This master's dissertation report, under the theme “Transition Design - Shared Online Canvas”, aims to present the work carried out, during the academic year 2019/2020, within the scope of the internship curricular unit, integrated in the Master of Informatics Engineering of the Faculty of Science and Technology of the University of Coimbra.Through a study carried out over the first three months, a synthesis was made about the key concepts related to the theme of Transitional Design, Participatory Design and its relations with dynamic interactive media. This investigation resulted mainly from the need to develop a computational platform that, through shared web tools, would allow sharing positions between stakeholders, using canvas. The developed project aims to support Transition Design processes, allowing to design solutions that would respond of change and social innovation needs.In order to develop a prototype, with the aim previously described, and facilitating collaborative processes, the requirements and specification of the project were initially surveyed. For the conception of this web tool, the user interfaces were also designed, necessary to accommodate the canvas outlined to the transition design. An architecture was designed to guarantee the interconnection between the user frontend and the server backend and the respective data support database. As the system needed to be cooperative, and given that there is not enough time and resources to develop a platform of such complexity, an innovative way was used to do it through web substrates (webstrates), which allows a distributed and cooperative way, taking advantage of potentialities of dynamic web media. Thus, using two servers, a web server and a webstrate server, the Transition Design platform was implemented, offering shared canvas for Transition Design, for cooperative use, shared on web.The developed platform, in its functionalities, allows the accreditation and login of users, the definition or consultation of projects, the launching of themes and objectives in each project, and the creation of canvas associated with each project, and in addition, allow a blog of discussion between project participants. The canvas that can be associated with the projects are: wicked problema diagrams, fishbone diagrams, future visions, spatio-temporal matrices, and stakeholder perspective maps. The main objective of the platform is to allow the discussion of ideas and the cooperative design of solutions between stakeholders.

This trend has resulted in the development of a variety of tools and technologies to support the various forms of collaboration. Many collaborative processes, e.g. strategy building, scenario analysis, root cause analysis and requirements engineering, require various collaboration support tools. Within these synchronous collaborative applications to create, evaluate, elaborate, discuss, and revise graphical models, e.g. data flow, fishbone and brainstorming diagrams, play an important role. Currently, the necessary tools are not integrated and flexible enough to support such processes. In this thesis, we present a synchronous collaborative brainstorming diagram editor that is integrated in a flexible group support system. By this our approach goes beyond the current state of the art as we can be seamlessly integrated with other collaboration support tools such as text-based brainstorming or voting.<br>A necessidade de colaboração suportada por computador tem crescido nos últimos anos e fez dos processos colaborativos um factor importante dentro das organizações. Esta tendência resultou no desenvolvimento de uma variedade de ferramentas e tecnologias para apoiar as diversas formas de colaboração. Muitos processos colaborativos (por exemplo, análise estratégica, cenários de causa ou engenharia de requisitos), exigem o suporte de várias ferramentas de colaboração. Dentro destas aplicações colaborativas síncronas para criar, avaliar, elaborar, discutir e rever modelos gráficos destacam-se dois diagramas de fluxo de dados (data flow), diagrama “espinha de peixe” (fishbone diagrams) e brainstorming, desempenham um papel importante. Atualmente, as ferramentas necessárias não estão integradas e flexíveis o suficiente para suportar tais processos. Nesta dissertação, apresentamos um editor síncrono de diagramas de colaboração de brainstorming, integrado num sistema de apoio a grupos (GSS) flexível para este efeito. Assim, esta abordagem vai além do nosso estado da arte atual, uma vez que pode ser perfeitamente integrado com outras ferramentas de colaboração, tais como brainstorming baseado em texto ou sistemas de votação.

Abstract:
<p>The internal and external challenges construction companies face such as variability, low productivity, inefficient processes, waste, uncertainties, risks, fragmentation, adversarial contractual relationships, competition, and those resulting from internal and external challenges such as cost overruns and delays negatively affect company performance and profitability. Though research publications abound, these challenges persist, which indicates that the following gaps exist. Lean construction, process improvement, and performance improvement research have been conducted wherein improvement principles, and best practices are used to ameliorate performance issues, but several knowledge gaps exist. Few companies use these improvement principles and best practices. For those companies applying improvements, there is no established link between these improvements and performance/profitability to guide companies. Further, even when companies use improvement principles and best practices, they apply only one or two, whereas an integrated application of these improvement principles and best practices would be more effective. The other gap the author identified is the lack of strategic tools that construction companies can use to improve and manage their profitability. This thesis tried to fill the knowledge gap, at least partially, by developing a two-part excellence model for profitability improvement of construction companies. The excellence model lays out strategies that would enable companies to overcome the challenges and improve their profitability. The excellence model also gives an iterative and recursive continuous improvement model and flowchart to improve the profitability of construction companies. The researcher used high impact principles, guidelines, and concepts from the literature on organizational effectiveness, critical success factors, strategic company profitability growth enablers, process improvement, and process maturity models, performance improvement, and organizational excellence guidelines to develop the two-part excellence model.</p> <p>The author also translated the two-part excellence model into the diagnostic tool and Decision Support System (DSS) by use of process diagrams, fishbone diagrams, root cause analysis, and use of improvement principles, countermeasures and best practices at the most granular (lowest intervention) levels to do away with root causes of poor performance. The author developed the diagnostic tool and Decision Support System (DSS) in Access 2016 to serve as a strategic tool to improve and manage the profitability of construction companies. The researcher used improvement principles, and best practices from scientific and practitioner literature to develop company and project process flow diagrams, and fishbone (cause and effect) diagrams for company, department, employee, interactions and project performance for the profitability improvement, which are the engines of the diagnostic tool and DSS. The diagnostic tool and DSS use continuous improvement cycles iteratively and recursively to improve the profitability of construction companies from the current net profit of 2-3 percent to a higher value.</p>

Hospitals lose a large amount of money through waste as defined by lean. Processes have been in place many years at medical facilities and waste is often accepted as unavoidable in the rightful “patient first” mentality. The Plan, Do, Check, Act cycle is used for the control and continual improvement of the waste management process at the hospital where the study took place. By adapting and utilizing lean and statistical tools such as scatter diagrams, histograms, Pareto charts, fishbone diagrams, and control charts, waste can be identified and reduced or eliminated. The use of the statistical tools provides a visual presentation of the message that the team conveyed. They are especially useful to summarize the information that would normally require much time and effort to elucidate its complexity. Further, they provide a visual summation with still a great deal of details leading to increased satisfaction, efficiency, and creates competitive advantage.

Various techniques are used to diagnose problems throughout all levels of the organization within the manufacturing industry. Often times, this root cause analysis is ad-hoc with no standard representation for artifacts or terminology (i.e., no standard representation for terms used in techniques such as fishbone diagrams, 5 why’s, etc.). Once a problem is diagnosed and alleviated, the results are discarded or stored locally as paper/digital text documents. When the same or similar problem reoccurs with different employees or in a different factory, the whole process has to be repeated without taking advantage of knowledge gained from previous problem(s) and corresponding solution(s). When discussing the diagnosis, personnel may miscommunicate over terms used in the root cause analysis leading to wasted time and errors. This paper presents a framework for a knowledge-based manufacturing diagnosis system that aims to alleviate these miscommunications. By learning from diagnosis methods used in manufacturing and in the medical community, this paper proposes a framework which integrates and formalizes root cause analysis by categorizing faults and failures that span multiple organizational levels. The proposed framework aims to enable manufacturing operations by leveraging machine learning and semantic technologies for the manufacturing system diagnosis. A use case for the manufacture of a bottle opener demonstrates the framework.

Abstract This paper describes a schematic representation of product retirement specification that aids in design for recycling and reuse. In the past decade, a graphical representation of the assembly process, called the assembly fishbone diagram, has effectively assisted engineers to conduct design for assembly (DFA) and process failure modes and effects analysis (FMEA). On the other hand, environmentally conscious manufacturing requires engineers to make advanced planning for product retirement. This study investigates the use of the reverse fishbone diagram to model the disassembly and reprocessing sequence of a product at the end of its useful life. An industry-provided student project guided us to an initial definition of the reverse fishbone diagram that effectively led the students to analyze the recyclability and make practical redesign suggestions. The diagram is continuously adding more rigorous definitions and promises to be a central tool for evaluation of recyclability in a simultaneous engineering setting.

Armored Face Conveyor (AFC) is a kind of typical customized product. Different AFCs are similar in function, structure and parameter. In order to obtain new products more efficiently and effectively, it is essential for the AFC to implement Mass Customization. Product platform is common items shared by a set of products and a good way to achieve Mass Customization. Most methods and processes of designing product platform discussed in relative references are processes that common items are found, developed or redesigned. If differentiae of a set of similar products are identified, those parts associated with those differentiae will be customized parts that need to be redesigned each time, and the rest will be integrated into a platform. From that point a reasoning process of designing platform for AFC is put forward. The fishbone diagram, reverse fishbone diagram and a matrix method are introduced, and the analyzing process and steps are discussed.

Abstract This paper proposes an integrative design method of product architecture and assembly process plan by introducing a matrix-based modeling and analysis scheme. Design structure matrix (DSM) has been used to analyze and design the conceptual structure of product architecture. An assembly sequence diagram, such as a fishbone diagram, has been used as a representation scheme of the assembly process in the design-for-assembly (DFA) framework. The method of this paper is composed of a DSM-based procedure for generating an assembly sequence plan and its integration with a DSM-based design method of product architecture. In this paper, after general meanings of integration in product design are reviewed, the overall method for integration and mathematical procedure for optimizing an assembly sequence plan by compromising with components modularity are described. The procedure for generating an assembly sequence plan is demonstrated with an example of a desktop stapler, and the integrative design is demonstrated with a case study of air-conditioner units. This paper concludes with some discussion on the role of the proposed framework toward the integrity of product design.
