Full textInterprofessional collaboration (IPC) is an innovating concept which emerges in organization and management theory and which is mostly applied to the health sector. As a response to the evidence found in the literature as to the lack of precise definitions of IPC and to the existence of many underlying concepts which renders its applicability difficult, we propose a generic definition and three contextualized definitions of IPC based on the empirical study conducted in three wards of a pediatric unit of a teaching hospital in Lebanon. The case study approach used in this research allows us to compare between the three units, to propose models of IPC which take into consideration the specific environment of each unit and to develop a generic model of IPC. The unique interview grid on which is based this work limits bias from the researcher and subjectivity of the actors. This tool allows us to highlight the perception of actors of the IPC, the potential situations of IPC, forms of IPC, prerequisites of IPC, facilitating and restrictive factors of IPC and the outcomes

Full textThe particularities of services influence different elements on the innovation process. Although, the characteristics of the sector urge some questions related to research and development (R&D) activities. In this sense, we try to clarify the influences of these particularities in the characterization and definition of the dynamics of its R&D activities. Based on study cases from Brazilian and French university hospitals, this thesis tries to explain four convergent points. First, based on the definition of the product of services as a set of vectors, we propose an association between the dynamic of competences and different methods of knowledge processing, in a systematic way and with the purpose of solving scientific and technological uncertainties. Second, we associated service operations to the vector of technical characteristics. Third, the diversity of actors that compose the guiding forces of these dynamics was analyzed. Finally, some characteristics related to the R&D and innovation process were identified. The results demonstrated that the competences involved in the process of creating and increasing the stock of knowledge have theoretical and practical characteristics, but also organizational and relational ones. These competences, linked to a series of actors, trigger different service operations, which will support the knowledge processing. Therefore, it was possible to discuss some characteristics related to the R&D process and the different models of innovation found

Full textIn the last years, the field of peadiatric oncology has experienced significant medical improvements as well as the granting of a larger autonomy to sick children. This evolution is related to the movement of children rights, but also to the influence of psychoanalysis on nursing and medical staff and to a greater concern about ethical questions raised by the inclusion of children in clinical trials. Thus, the biomedical discourse seems to mirror the evolution of the children position in the society. This doctorate dissertation questions the place of the sick child at the hospital. The dissertation aims to follow the child progression in his path as a patient, guided by the search of his own speech. This work relies on a field study in a child-oncology unit in Marseille as well as on a specific work with former patients and the analysis of sick children'mother blogs. It is first a testimony of the harsh situations faced by children, their parents and the staff. Thanks to an interdisciplinary approach, this work has reached a dimension of clinical anthropology, having effects by itself. Through transference, this work could disclose unconscious or repressed datas to those players who took part in the study. As a conclusion, the dissertation raises some methodological questions. Does the double competence, being a psychoanalyst as well as an antrhopologist, imply a specific bond to the study field? This work aims to demonstrate that the deepening of dyadic relationships with the subjects of the ethnographist is not an obstacle to the triangulation of the information and to the comprehension of interactions

Full textA cross-national comparison of the organisation of six hospital wards in Germany, France and Great-Britain presents many national differences. These differences have been analyzed as the result of national institutions and norms. Functionalist and culturalist interpretations of cross-national comparison are criticized. Some similarities in teamwork in the three countries are highlighted. The quality of the relations between physicians depends on their respective career trajectories. On the other hand cooperation among nursing staff is good if there is a general agreement on the organisation of nursing work. Lastly the quality of relations between doctors and nurses depends on the results of negotiations of the division of work and the kinds of patients who are admitted to the wards. These cross-national similarities stem from the use of the same technical knowledge, identical hierarchical subordination between physicians and nurses, and career patterns which vary from one profession to another, but are the same in the three countries. The study of the links between professions and organisation is of the highest importance in order to understand teamwork. It is impossible, however, to explain the variety of cooperation and conflict observed by the sole analysis of national institutions and links between professions and organisations. The quality of teamwork depends also on the choices of the members of the team (for example which patients are admitted for treatment), especially the chief of service and the head-nurse. On a theoretical level, our research has gone beyond the classical stategic analysis of organisations, which gives the priority to the study of endogenous variables of the organisation, and it has shown that exogenous variables (national institutions, local environment) also influence teamwork in hospitals
