Abstract:

Abstract:
ObjectivesTo assess the association of fluoroquinolone use with tendon ruptures compared with no fluoroquinolone and that of the four most commonly prescribed non-fluoroquinolone antibiotics in the USA.DesignRetrospective observational study.SettingUS seniors enrolled in the federal old-age, survivor’s insurance programme.Participants1 009 925 Medicare fee-for-service beneficiaries and their inpatient, outpatient, prescription drug records were used.InterventionsSeven oral antibiotics, fluoroquinolones (ciprofloxacin, levofloxacin, moxifloxacin) and amoxicillin, amoxicillin-clavulanate, azithromycin and cephalexin.Primary and secondary outcome measuresAll tendon ruptures combined, and three types of tendon ruptures by anatomic site, Achilles tendon rupture, rupture of rotator cuff and other tendon ruptures occurred in 2007–2016.ResultsOf three fluoroquinolones, only levofloxacin exhibited a significant increased risk of tendon ruptures—16% (HR=1.16; 95% CI 1.06 to 1.28), and 120% (HR=2.20; 95% CI 1.50 to 3.24) for rotator cuff and Achilles tendon rupture, respectively, in the ≤30 days window. Ciprofloxacin (HR=0.96; 95% CI 0.89 to 1.03) and moxifloxacin (HR=0.59; 95% CI 0.37 to 0.93) exhibited no increased risk of tendon ruptures combined.Among the non-fluoroquinolone antibiotics, cephalexin exhibited increased risk of combined tendon ruptures (HR=1.31; 95% CI 1.22 to 1.41) and modest to large risks across all anatomic rupture sites (HRs 1.19–1.93) at ≤30 days window. Notably, the risk of levofloxacin never exceeded the risk of the non-fluoroquinolone, cephalexin in any comparison.ConclusionsIn our study, fluoroquinolones as a class were not associated with the increased risk of tendon ruptures. Neither ciprofloxacin nor moxifloxacin exhibited any risk for tendon ruptures. Levofloxacin did exhibit significant increased risk. Cephalexin with no reported effect on metalloprotease activity had an equal or greater risk than levofloxacin; so we question whether metalloprotease activity has any relevance to observed associations with tendon rupture. Confounding by indication bias may be more relevant and should be given more consideration as explanation for significant associations in observational studies of tendon rupture.

Abstract:
In-service material degradation of a high-pressure rotor steel has been investigated for remaining service life assessment. The samples for this study were taken from the cold and hot locations of a rotor that had operated for approximately 167,500 hours. The test results have indicated that the rotor steel is slightly softened and temper embrittled as a result of service exposure. Based on isostress creep rupture data of the rotor steel in the service-exposure. Based on isostress creep rupture data of the rotor steel in the service-exposed condition, it is concluded that crack initiation due to creep is not a concern in the near future. Considering the uncertainties associated with the extrapolation of short-time rupture data on remaining service life assessment, the rotor material should be re-evaluated for degradation of properties after the next 10 years of operation.

Abstract:
Case summary A stray female domestic shorthair cat was presented to the emergency service after being hit by a car. The patient was recumbent and vocalizing, with a small wound over the right lateral thorax, and two palpably firm swellings in the right cervical and thoracic soft tissues. The patient was sedated and humanely euthanized to prevent further pain and suffering. Post-mortem whole-body radiographs and subsequent necropsy revealed abdominal wall rupture with herniation of two near-term fetuses within the subcutaneous tissues along the right ventrolateral thoracic wall and neck. Within the abdomen, the right uterine horn was ruptured and a third extra-luminal fetus was identified. The left uterine horn remained intact, containing a fourth fetus. Relevance and novel information Rupture of the abdominal wall or diaphragm are well-known potential complications of blunt force trauma, such as motor vehicle accidents. While traumatic uterine rupture and diaphragmatic herniation of the gravid uterus have both been reported in the veterinary literature, abdominal wall rupture with subcutaneous fetal herniation is highly uncommon.

Abstract:
Background: Achilles tendon ruptures are devastating in elite athletes. There are currently no studies examining the effects of Achilles tendon rupture on performance outcomes in the Women’s National Basketball Association (WNBA). Hypothesis: Athletes in the WNBA who sustained an Achilles tendon rupture and underwent subsequent surgical repair will sustain declines in performance metrics when compared with their preinjury statistics and matched controls. Study Design: Cohort study; Level of evidence, 3. Methods: Seventeen WNBA players who sustained an Achilles tendon rupture from 2000 to 2019 were identified through publicly available injury reports and player profiles. Athlete information collected included age, body mass index, position, and service in the WNBA when the tear occurred. Statistics were collected for 1 season before and 2 seasons after the injury, and the player efficiency rating (PER) was calculated. Players were matched to uninjured controls by service in the WNBA, position, and performance statistics. Results: On average, players were 28 years of age at the time of Achilles tendon rupture, with a service time in the WNBA of 6.5 years. Four players never returned to play in the WNBA, while 7 players failed to play more than 1 season. Players who did return played significantly fewer minutes per game compared with preinjury in both postinjury seasons 1 and 2 (mean difference, –6.11 and –6.54 min/game, respectively; P < .01 for both) and had a significantly decreased PER in postinjury season 2 (mean difference, –2.53; P = .024). After returning to play, the injured players experienced significant decreases when compared with controls in field goals (–0.85 vs +0.20; P = .047), free throws (–1.04 vs +0.12; P < .01), steals (–0.48 vs +0.24; P = .018), and points scored (–2.89 vs +0.58; P = .014). Conclusion: WNBA players experienced significant decreases in performance metrics after Achilles tendon rupture compared with their preinjury levels and compared with uninjured controls. Overall, 23.5% of players failed to return to the WNBA, while 41.2% failed to play for more than 1 season.

Abstract:
A large proportion of the media around the world, especially those related to radio and television, belong to the state. In principle at least, there are three different terms to talk about these types of media: (1). The public media that draws on the treasury to present programming that is in the interest of the general population. They do not support any political party, not even the party in power. (2). National media owned by the state and using the treasury money, are also controlled directly by the state. (3). Government media that is owned by the ruling party and uses the treasury money, are also controlled by the ruling party. These three models coexist already in the Arab world since independence. This phenomenon almost removed the clear distinction that existed in principle between the government media and the public media. After the Arab Spring in 2011, however, this distinction remains important. The public broadcaster model was based on a principle that is still justified for most of the world and that the private media alone can not guarantee the pluralism of broadcasting. The problem, however, is that the government media have also largely failed. In several countries, the arrival of private media has pushed governments to exercise editorial control of the public media. The discussion of media regulation is aimed primarily at ensuring that the media financed by the Public treasury exercise their profession with the full independence of the government of the day to which they are entitled, rather than aiming to restrict the freedom of the media that already enjoy full editorial independence. In the Arab world, there have been some attempts to recover and modernize the ideal model of public media, as for example the case of Tunisia, Morocco and Jordan. This study aim to search if the Arab broadcasting meet the recognized standards and the requirements of the concept of public service?Int. J. Soc. Sc. Manage. Vol. 5, Issue-3: 98-112

Abstract:
The objective of this investigation was to evaluate the efficacy of different weld repair techniques as applied to service-aged 2-1/4Cr-1Mo steel weldments. A header which had been in service for 244,000 h at 1050°F (565°C) was utilized for the study. Three girth welds were partially excavated and subjected to repairs using gas tungsten arc welding (GTAW), shielded metal arc welding (SMAW) with postweld heat treatment (PWHT), and without postweld heat treatment using a temperbead technique. Results show that all the weld repairs improved the creep rupture lives of the ex-service weldments and that remaining lives of several decades could be achieved in the repaired condition. The SMAW-temperbead repairs resulted in increase of rupture life, tensile strength, and impact toughness compared to the SMAW-PWHT repairs. The GTAW-PWHT repairs also produced a superior combination of mechanical properties. Remaining creep rupture lives were a function of the extrapolation procedure and specimen size. These results are described here and discussed in comparison with results previously reported for a less severely degraded condition of the steel in order to delineate the effect of prior degradation on weld repair performance.

Abstract:
This study investigated the microstructural characteristics and property degradation of service-exposed reformer tubes. The reformer tubes were graded into three damage levels based on on-site ultrasonic testing. The microstructural and mechanical experiments were carried out included optical observations, scanning electron microscope (SEM) observations, room temperature tensile tests, high temperature tensile tests, and stress rupture tests. The results showed that the coarsen of carbides occurred during service. A decrease of tensile elongation was observed. The mechanical properties and rupture behaviours of reformer tubes for different inspection levels were also obtained. The results showed that the ultrasonic inspection levels of tubes could be significantly influenced by carbide morphology of tube materials. A residual life of 8.3 years was estimated for these tubes.

Abstract:
Category: Trauma Introduction/Purpose: Patients with acute tendo-achilles (TA) rupture require prompt diagnosis, investigation and treatment in order to ensure optimum outcomes. This can be prevented by delay in review of patients within secondary care. We present our experience of treating such injuries by establishing a “virtual” fracture clinic (VFC) and a one stop dedicated TA specialist clinic. Patients are referred to the service via the emergency department (ED) and are triaged remotely by the VFC, at which all orthopaedic cases including suspected TA injuries are reviewed by a consultant orthopaedic surgeon using all available medical records and imaging. Suitable patients are then diverted directly to the specialist TA clinic for definitive treatment. We wished to establish the speed and efficiency of this care pathway. Methods: Using our institutions prospectively maintained database, we identified patients treated in the specialist TA clinic between September 2016 and August 2017. We recorded demographic details, injury mechanism, time from injury to diagnosis, ultrasound scan findings, and the speed of progress of the patient along the agreed rehabilitation pathway. Results: Sixty two patients were referred to the VFC with suspected TA injury. Mean age was 49 years, M: F (44:18 female). Mean time to triage in the VFC was 3.5 days (with 42% of patients triaged in the VFC on the same day as initial ED presentation). Mean time from ED consultation to assessment in our TA clinic was 11.7 days. Patients were assessed by a specialist foot and ankle surgeon, supported by a same day musculoskeletal ultrasound service. 45 patients (74%) were diagnosed with TA rupture, of which 69% were acute complete ruptures. Eight patients fulfilled criteria for surgical repair. Mean time from ED consultation to surgery was 13.25 days. Three further patients treated non-operatively developed secondary ruptures requiring delayed operative intervention. Conclusion: We recommend the use of a virtual triage service and a one stop TA clinic led by a dedicated specialist team to improve the accuracy of diagnosis and efficacy of treatment of acute TA rupture. This service provides a robust system for avoiding mis-diagnoses and delays in treatment, as well as ensuring effective implementation of our local care pathway under specialist supervision in a one stop clinic. This also establishes an effective system for monitoring as well as maintaining a research database for further prospective studies.

This paper is aimed at investigating the damage and remaining life of a whole HP-Nb hydrogen reformer tube serviced for 12 years. The damage and remaining life evaluation of different parts of the tube were made using metallographic examination, ambient and high temperature uniaxial tensile tests, creep rupture tests and damage mechanics analysis. The results showed that different parts of the tube have the different damage and remaining life due to different service temperature, higher the service temperature, severer the damage and shorter the remaining life. Based on these research results, a case of local maintenance of the reformer tube was introduced.

Abstract:
Analytical life prediction methods are being developed for high-temperature turbine and valve studs/bolts. In order to validate the approach, the calculated results are compared to the results of uniaxial stress relaxation testing, bolt model testing, and service experience. Long time creep, creep-rupture, and stress relaxation tests were performed by the National Research Institute for Metals of Japan (NRIM) for 12 Cr-1 Mo-1 W-1/4V, Type 422 stainless steel bolting material, at 500, 550, and 600°C. Based on these results and limited tests for a service-exposed bolt, the creep behavior can be described using a two-parameter material model: ε/εr=1−1−t/trm+1δ where εr is the rupture strain, tr is the rupture time, and m and δ are material constants. For comparison with the measured uniaxial stress relaxation properties, the stress relaxation was calculated using the two-parameter creep equation and a strain-hardening flow rule. The rupture time data was correlated using time-temperature parameter methods. A power law was used for the rupture strain versus rupture time relationship at each temperature. The calculated stress versus time curves were in good agreement with the measured at all temperatures and for initial strain levels of 0.10, 0.15, 0.20, and 0.25 percent. [S0094-9930(00)01701-7]

Abstract:
We present a case of aneurysm rupture from severe blunt abdominal trauma due to fight in a patient who had endovascular aneurysm repair. The patient presented to the emergency service with computed tomography evidence of an endoleak and a large retroperitoneal hematoma. The contrast abdominal computed tomography demonstrated a type Ib endoleak, increase in the aneurysm diameter and hematoma in the retroperitoneum. The patient has been taken under interventional procedure for endovascular aneurysm repair revision under urgent condition. Type Ib endoleak was treated by placement of a covered iliac extension limb, but a second leakage from graft body was found in control computed tomography images and open surgical conversion was necessary. Operative findings included a type III endoleak from graft body, defect on fabric. It was seen that the aneurysm sac anterior and posterior parts were ruptured. Upon reviewing the literature, we found that it was an interesting case as the first rupture case which had been developed after severe blunt abdominal trauma during the follow-up period of a patient on which endovascular aneurysm repair procedure had been performed and progressed in this manner.

An aspergilloma is a well-recognized lesion of the lung caused most of the time by the fungus Aspergillus fumigatus. Its main complication is hemoptysis and has been very rarely associated with tension pneumothorax. We present the case of a 47-year-old man with a history of treated and healed tuberculosis, which was successfully managed in our service for a ruptured right upper lobe complexed aspergilloma, complicated by a massive and tension pneumothorax. The patient underwent thoracotomy and lung resection with quick recovery. Conclusively, although rare, an aspergilloma may rupture and cause a life-threatening air leakage.

Objective Clarify the safety and efficacy of the endovascular treatment of distal anterior cerebral artery (DACA) aneurysms, reporting outcomes of the aneurysms coiled in our service were compared with series of microsurgical treatment. The impact of embolization on ruptured or unruptured aneurysms remains controversial according to the current data, considering aneurysm from this topography should be aggressively treated due to their high incidence of rupture, currently there is a tend to prefer endovascular treatment. Methods We conducted a retrospective cohort study with 1092 patients admitted with cerebral aneurysm from October 2005 to March 2015 in our service. There were 31 cases of DACA aneurysms treated with the endovascular technique. These were compared with same topography aneurysms underwent to clipping. Results A total of 21 (67%) of 31 cases presented with ruptured aneurysms, 13 (59%) suffered clinical or radiological vasospasms, with modified Rankin Scale (mRS) scores of 3–5 in 7 patients (31%), and 4 deaths (mRS 6), reaching 92% of occlusion at one year. Conclusion Endovascular approach is associated with high angiographic occlusion rates and security.

Abstract:
A component in service experiences stress conditions that change continuously with time. Since service conditions are usually difficult and expensive to reproduce in laboratory, the creep behaviour of alloys in service has to be extrapolated from a limited number of creep tests at constant loads and temperatures. Empirical rules have been proposed to forecast the effects of variable load and temperature both on the time to rupture, as the life fraction rule (LFR), and on the accumulation of creep strain with time, as the strain hardening rule (SHR). Two directionally solidified (DS) nickel based superalloys have been investigated with creep tests at constant and variable loads and constant temperature. Nickel based superalloys, for the typical stresses experienced in service, are often characterised by a small negligible primary, a minimum of strain rate with no secondary state, and a dominant accelerating creep caused by dislocation multiplication. The damage mechanisms causing the final rupture appear only in the very last percentage of life. In the present work, simulation results are reported to show that the physical-sounded model used to describe the accelerating creep due to dislocation multiplication can be employed to better predict the times to rupture and the creep curves of the two DS nickel based super-alloys with step-like variable stress than the empirical LF and SH rules.

Abstract:
The purpose of this theoretical essay is to reflect on the current discussion about the management of the Judiciary. We are currently living in a moment of rupture, in a scenario of disproportion between the offer of services and the number of conflicts to be resolved. Co-production has been advocated as an alternative through the participation of users of these services in decision-making. For this discussion, we bring in the New Public Management that proposes reforms based on the same logic of action that governs private companies. And the New Public Service, which defends productivity and efficiency in a larger context of democracy and public interest. We conclude that there is no dichotomous thought in the construction of a management model that is coherent with the difficulties and the complexity of the provision of justice. But there is a rupture with the conservative past, the search for a new identity, without losing its legitimacy and commitment to democratic principles.

Abstract:
Infallible creep rupture life prediction of high temperature steel needs long hours of robust testing over a domain of stress and temperature. A substantial amount of effort has been made to develop alternative methods to reduce the time and cost of testing. This study presents a finite element analysis coupled with a ductility based damage model to predict creep rupture time under the influence of multiaxial stress state of ex-service and as-received Grade 91 steel at 873 K. Three notched bar samples with different acuity ratios of 2.28, 3.0 and 4.56 are modelled in commercial Finite Element (FE) software, ABAQUS v6.14 in order to induce different stress state levels at notch throat area and investigate its effect on rupture time. The strain-based ductility exhaustion damage approach is employed to quantify the damage state. The multiaxial ductility of the material that is required to determine the damage state is estimated using triaxiality-ductility Cock and Ashby relation. Further reduction of the ductility due to the different creep mechanisms over a short and long time is also accounted for in the prediction. To simulate the different material conditions: ex-service and as-received material, different creep coefficients (A) have been assigned in the numerical modelling. In the case of ex-service material, using mean best fit data of minimum creep strain rate gives a good life prediction, while for new material, the lower bound creep coefficient should be employed to yield a comparable result with experimental data. It is also notable that ex-service material deforms faster than as-received material at the same stress level. Moreover, higher acuity provokes damage to concentrate on the small area around the notch, which initiates higher rupture life expectancy. It also found out that, the stress triaxiality and the equivalent creep strain influence the location of damage initiation around the notch area.

Abstract:
Abstract13.5 year service exposed (SE) catalyst primary reformer tube material made of H39WM micro-paralloy grade used in feritilizer plant was assessed for remaining life. The investigation includes mechanical properties evaluation; microstructural analysis and accelerated stress rupture tests. Failed tube portions showed coarsening primary carbides of Chromium and Niobium along the grain boundaries. Degradation of Niobium carbide (NbC) into Ni-Nb-Si phase and partial conversion this phase back to NbC was observed. Secondary carbides within grains were almost absent. Degradation in tensile strength for a range of temperature from 1123 to 1223 K was also observed but they were within the specified limits. Premature failures within 3–5 years service exposure are more common in reformer tubes. The failure was attributed to localized overheating leading to creep damage. The cast tube material may undergo microstructural changes during service exposure which is the main cause of degradation in strength and hardness changes. Accelerated stress rupture tests were performed in the range of 1173–1248 K on samples machined from 13.5 years at 1191 K and 15.4 MPa exposed reformer steel tubing, did not reveal any degradation of rupture behavior compared to that of the virgin alloy. An additional life of at least 10.6 years is predicted at the operating stress-temperature conditions.

Abstract:
Background: Pectoralis major tendon ruptures are commonly described as rare injuries affecting men between 20 and 40 years of age, with generally excellent results after surgical repair. However, this perception is based on a relatively small number of case series and prospective studies in the orthopaedic literature. Purpose: To determine the incidence of pectoralis major tendon ruptures in the active-duty military population and the demographic risk factors for a rupture and to describe the outcomes of surgical treatment. Study Design: Case control study; Level of evidence, 3. Methods: We utilized the Military Health System Data Repository (MDR) to identify all active-duty military personnel surgically treated for a pectoralis major tendon rupture between January 2012 and December 2014. Electronic medical records were searched for patients’ demographic information, injury characteristics, and postoperative complications and outcomes. Risk factors for a rupture were calculated using Poisson regression, based on population counts obtained from the MDR. Risk factors for a postoperative complication, the need for revision surgery, and the inability to continue with active duty were determined using univariate analysis and multivariate logistic regression. Results: A total of 291 patients met inclusion criteria. The mean patient age was 30.5 years, all patients were male, and the median follow-up period was 18 months. The incidence of injuries was 60 per 100,000 person-years over the study period. Risk factors for a rupture included service in the Army, junior officer or junior enlisted rank, and age between 25 and 34 years. White race and surgery occurring >6 weeks after injury were significant risk factors for a postoperative complication. Among the 214 patients with a minimum of 12 months’ clinical follow-up, 95.3% were able to return to military duty. Junior officer/enlisted status was a significant risk factor for failure to return to military duty. Conclusion: Among military personnel, Army soldiers and junior officer/enlisted rank were at highest risk of pectoralis major tendon ruptures, and junior personnel were at highest risk of being unable to return to duty after surgical treatment. Although increasing time from injury to surgery was not a risk factor for treatment failure or inability to return to duty, it did significantly increase the risk of a postoperative complication.

AbstractObjectives:To describe the pattern of ear injuries sustained by all British servicemen serving in Iraq and Afghanistan between 2006 and 2009; to identify all servicemen evacuated to the Royal Centre for Defence Medicine following blast injury; to ascertain how many underwent otological assessment; and to calculate the incidence of hearing loss.Design and setting:A retrospective analysis of data obtained from the Joint Theatre Trauma Registry and the Defence Analytical and Statistics Agency, together with audiometry records from the University Hospitals Birmingham National Health Service Trust. Results:Ear damage was present in 5 per cent of all British servicemen sustaining battle injuries. Tympanic membrane rupture occurred in 8 per cent of personnel evacuated with blast injuries. In 2006, 1 per cent of servicemen sustaining blast injury underwent audiography; this figure rose to 13 per cent in 2009. Fifty-three per cent of these audiograms were abnormal. Conclusion:The incidence of tympanic membrane rupture was higher than that found in previous conflicts. Otological assessment prior to and following military deployment is required to determine the incidence of ear injury amongst British servicemen following blast trauma.

Abstract:
This article describes a technique to calculate the risk from failure of passive components over time, and demonstrates the technique by applying it to a weld in the auxiliary feedwater (AFW) system. It uses a modified version of the PRAISE computer code to perform a probabilistic structural analysis to calculate the probability that crack growth due to aging would cause the weld to rupture. It then uses the weld rupture probability as input to a modified existing PRA to calculate the change in plant risk with time. The results show an insignificant effect on plant risk because of the low calculated rupture rate of the weld in this particular calculation over 48 yr of service. A decreasing yearly rupture rate for this weld is calculated. This results from infant mortality; that is, most of those initial flaws that will eventually lead to rupture will do so early in life.

Abstract:
Our aim is to examine the effects of climatic conditions on the incidence of ruptured abdominal aortic aneurysms (rAAA) in Catalonia, Spain. We combined clinical data from the Public Health official registries in Catalonia, Spain (HD-MBDS) of all rAAA with local climatic data obtained from the closest meteorological station (69 stations, National Meteorological Service: MeteoCat) from 2008 to 2017. We analyzed the median, maximum, minimum, and variability of atmospheric pressure (hPa) and air temperature (°C), solar irradiation (MJ/m2), humidity (%), accumulated precipitation (mm), median wind, and maximum flaw direction and velocity (°, m/s), recorded on the days of events, the previous day, and mean results for 3, 7, and 30 days before, as well as seasonality. Seventy-five control days were randomly selected in a 1-year period around every rAAA day at the same meteorological station, and compared. A total of 717 days and locations with rAAA were identified, and 53,775 controls were randomly selected. For the rAAA days, there were significantly lower temperatures, lower solar global irradiation, and higher mean humidity levels in all time periods ( p < 0.001, p < 0.001, p < 0.05); higher atmospheric pressure variability during 1 week and 1 month before ( p = 0.011, p = 0.007); and they often occurred during autumn/winter (57.6%, p < 0.001). Logistic regression identified low mean temperatures on the days of ruptures and high mean humidity the week before as independent rupture predictors. In conclusion, low median temperatures the same day and high humidity during 1 week before were identified as independent predictors of rAAA occurrence. The role of climate on pathophysiologic mechanisms may require further investigation.

Abstract:
ABSTRACT An 8-inch oil pipeline ruptured on Lookout Mountain in Chattanooga, Tennessee, in February 1996. The resulting response was complicated by the unusual geology at the spill site as well as by the proximity of surface waters and sensitive natural resources. Lookout Mountain contains extensive karst cave systems featuring nationally renowned tourist and spelunking destinations. The caves are also a natural resource managed by the National Park Service. A discharge of approximately 65,000 gallons of kerosene-fuel oil mix sprayed into a small ravine that drains toward the nearby Tennessee River. Responders arrived on scene promptly but found little product at the rupture site and so focused their preliminary efforts on containment and cleanup of free product in the ravine. As surface cleanup progressed, it became apparent that most of the discharge was unaccounted for. Responders quickly associated site geology with the “missing” product and feared that oil had entered the cave systems beneath the discharge zone. This paper discusses the strategies used and the problems encountered in attempting to track and clean up the discharge as well as some of the issues surrounding the impact on natural resources.

Abstract:
The hot components of gas turbine are loaded by high stresses in presence of high-temperature oxidation and hot corrosion environment during service process. It is important to simulate the interaction of stress and hot corrosion on the components in real service situation. Effect of the ratios of salt mixtures of Na2SO4/NaCl and the coating amounts of salt mixtures on the microstructure and the high temperature stress-rupture properties of a corrosion resistant directionally solidified superalloy at 850/500MPa were investigated. The results demonstrated that the stress-rupture life decreased with the coated salt mixture ratio of Na2SO4/NaCl changing from 3:1 to 9:1. Stress-rupture lives of the alloy decreased from 63 h to 42 h, while the coated salt mixtures amount increased from 1 mg/cm2 to 4 mg/cm2, hereby the salt mixture contained 75% Na2SO4 and 25% NaCl. When the coating ratio of Na2SO4/NaCl was 9:1, the stratified corrosion products layers were composed of the (Ni, Co)O, Ni (Al, Cr)2O4 layer, bright grey phase that be identified as CrWO4, and the underlying dispersed particle shape phase CrSx, the underlying layer forms the sulfides eutectic and leads to catastrophic destruction for the alloy, which decreased the stress-rupture lives dramatically.

Abstract:
The effects of high temperature exposure simulating service conditions on stress rupture properties were studied for the second generation single crystal superalloy DD6. The specimens with [001] orientation were exposed in air at temperatures of 980°C and 1070°C for 100h to 1000h. They were then tested using conventional mechanical tests at 1070°C/140MPa to determine the effects of exposure on stress rupture properties. The analysis indicated that stress rupture life decreased with increasing exposure time. At the temperature of 980°C, the stress rupture life is more than 180h after exposure for 1000h. When the test temperature increased to 1070°C, the stress rupture life exceeds 100h after 800h exposure. The morphology of γ prime phase after exposure was observed by using scanning electron microscopy (SEM). Morphologies evaluations have shown that alloy DD6 exhibits excellent microstructure stability after exposure. TCP (Topologically Closed Packed) phases have not been observed. It has been also found that the morphology and size of γ prime affected stress rupture life of the alloy. The decrement in stress rupture life with increasing exposure is a result of γ prime rafting.

Abstract:
The creep properties for SUS 316 HTB austenitic stainless steel were evaluated by using the small-punch creep test at 650 °C for loads of 234, 286, 338, 408, and 478 N and at 700 °C for loads of 199 and 234 N. The creep curves, determined by means of the small-punch creep test, were similar to those obtained from a conventional uniaxial creep test. That is, they exhibited clearly the three creep stages. The width of secondary creep stage and rupture time tr decreased with the increase in testing load level. The creep rupture strength for the service-exposed material was lower than that of the as-received material at high testing loads. However, the creep resistance behavior was opposite at relatively low load levels. This difference in creep resistance was explained on the basis of the difference in the creep deformation and microstructural evolution during tests. It was also found that the ratio between the load of small-punch creep test and the stress of uniaxial creep test was about 1 for having the same value of creep rupture life.

Abstract:
Since gas turbine blades in engines suffer centrifugal stress and gas corrosion during service, a good creep resistance in hot corrosion environment is one of the important considerations to evaluate service performance of the blade materials. In this work, the creep rupture behaviors of the directionally solidified superalloy DZ466 with and without salt deposition at 760°C/765MPa, 850°C/500MPa and 950°C/220MPa are preliminarily investigated based on the creep properties measurement and microstructure observations. The effects of hot corrosion on the creep properties and fracture mode are examined. The results show that the creep-rupture life in salt-deposit environment is lower than that in air-exposure environment at different temperatures. The creep-rupture life reduction caused by hot corrosion is increased with increase of the creep temperature. The fracture mode is exhibited by transgranular fracture in all crepted specimens. The propagation directions of all the secondary cracks are almost perpendicular to the crept specimen surface or the stress axis. The surface cracks are mainly produced in air-exposure environment at low temperature or in salt-deposited environment at high temperature. Induced by the stress concentration, the internal cracks are initiated surrounding the carbides in both air-exposure and salt-deposit environments. The creep-rupture life is dependent on the crack initiation at low temperature but on the crack propagation at high temperature. The reduction of the active load bearing area in transversal direction is the main reason why the creep-rupture life is decreased at 950°C.

Abstract:
Background: The aim of the study was to characterize the clinical profile of patients with anterior communicating artery (ACoA) aneurysms and examine potential correlations between clinical findings, aneurysm morphology, and outcome. Methods: A review of medical records and diagnostic neuroimaging reports of patients treated at a neurosurgical service in Porto Alegre, Brazil, between August 2008 and January 2015 was performed. Results: During the period, 100 patients underwent surgery for ACoA aneurysms. Fifteen had unruptured aneurysms and 85 had ruptured aneurysms. Ruptured aneurysms had a higher aspect ratio than unruptured ones (2.37 ± 0.71 vs. 1.93 ± 0.51, P = 0.02). Intraoperative rupture occurred in 3%, and temporary clipping was performed in 15%. Clinical vasospasm occurred in 43 patients with ruptured aneurysms (50.6%). Overall, mortality was 26%; 25 patients in the ruptured group (29.4%) and one in the unruptured group (6%). The Glasgow Outcome Scale (GOS) was favorable (GOS 4 or 5) in 54% of patients, significantly more so in those with unruptured aneurysms (P = 0.01). In patients with ruptured aneurysms, mortality was associated with preoperative Hunt and Hess (HH) score (P < 0.001), hydrocephalus (P < 0.001), and clinical complications (P < 0.001). Unfavorable outcomes were associated with HH score (P < 0.001), Fisher grade (P = 0.015), clinical vasospasm (P = 0.012), external ventricular drain (P = 0.015), hydrocephalus (P < 0.001), and presence of clinical complications (P = 0.001). In patients with unruptured aneurysms, presence of clinical complications was the only factor associated with mortality (P < 0.001). Conclusion: Despite advances in the management of subarachnoid hemorrhage and surgical treatment of aneurysms, mortality is still high, especially due to clinical complications.

Abstract:
This paper discusses the results of evaluations performed on service-aged piping using both conventional postweld heat treatments and temperbead repair techniques. The two repair weldments were accomplished on two 2-1/4Cr-1Mo pipe girth weldments which were removed from a utility hot reheat piping system in the fall of 1992 after 161,000 h of operation at 1000°F (538°C). Each repair was performed around one-half of the diameter of a pipe girth weldment, while the remaining half of the girth weldment was left in the service-aged condition. Post-repair metallurgical and mechanical test results indicated that both weld repairs produced improved remaining lives when compared to the service-aged girth weldments. Since the two ex-service weldments that were utilized in weld repairs exhibited different stress rupture strengths to start with, the performance of temper bead and postweld heat-treated (PWHT) repair could not be compared directly. It was clear, however, that life extension periods exceeding 30 yr could be achieved by temperbead repairs, with improved toughness and with no loss of stress rupture ductility, tensile strength, or yield strength. The temperbead repair improved the toughness of the service-aged weldment, while the postweld heat-treated repair lowered the HAZ toughness.

It concerned all pregnant women who had been caesarized or had had a laparotomy for uterine rupture with complications and those who had not developed any complications. The parameters studied were types of complications, risk factors and maternal mortality. The Chi-square test was used to compare the two populations with a significance level p=0.05.Results: During the study period, 6141 hospitalizations were recorded among which 5682 surgical procedures were performed, i.e. 92.52% of hospitalizations. Caesarean delivery accounted for 90.55% of surgical procedures and laparotomy for uterine rupture for 1.10%. The overall maternal post-operative morbidity rate was 7.60%. Post-operative anemia was by far the most common complication (75.76%) followed by infection (23.46%). The maternal death rate was 0.92% with a ratio of 409.97 maternal deaths per 100,000 live births and more than 2/3 of these deaths were due to caesarean delivery. Anemia and septic shock were the main causes of death. Factors related to this post-operative maternal morbidity were: age greater than or equal to 40 years, multi-parity, illiteracy, emergency obstetric evacuation, low socio-economic level, poor quality of prenatal follow-up and rupture of membranes before admission. Conclusion: In the emergency context concerning majority of our cesarean deliveries and the totality of uterine ruptures predispose the mother to high significant morbidity and mortality.

Abstract:
A burst rupture test has been mainly used for evaluating the burst properties of internal pressurized tubes. Although the burst creep test provides accurate results, its complicated and material-wasting process makes it difficult to perform this test. In the burst test, it is known that the hoop stress is a main reason of burst, so it can be expected that the burst rupture properties are strongly related with the hoop creep rupture properties. A ring test is occasionally used for obtaining the hoop directional properties of tube-shaped structures. In this study, the creep rupture properties of Zirlo tubing are investigated at temperature ranging from 365 to 570 using the ring specimens. An analysis of the fractography was performed and the estimation of service life with Larson-Miller parameter was conducted. Finally, In view of the Dorn equation of power-law, the creep mechanism was determined and discussed.

Abstract:
OBJECTIVE - To study the diagnosis, prognosis and management of spontaneous rupture of the esophagus. DESIGN: This is a retrospective study through the analysis of two cases with delayed diagnosis and subsequent treatment at the Track Surgery Service. LOCALE: The study was performed at the Thoracic Surgery Unit of the Hospital do Servidor Público Estadual Francisco Morato de Oliveira in the city of São Paulo. This is a specialized service. PARTICIPANTS: The two patients reported on had suffered spontaneous rupture of the esophagus. They were transferred to the Thoracic Surgery Unit because of the worsening of their condition in the previous institution which they had been admitted into. MEASUREMENT: The two patients with esophagus pleural fistula received similar treatment, initially advocated by Kanashin in Russia and Hauer-Santos in the United States, which consists of washing the fistula and using continuous pleural aspiration. RESULTS: Although both patients had to spend a long period of time in hospital, their evolution was satisfactory with the treatment adopted, and the fistula closed. CONCLUSION: The authors conclude that the method of lavage of the mediastinum and continuous pleural aspiration, in patients who after spontaneous rupture of the esophagus developed a pleural esophagus fistula due to belated diagnosis, is an alternative and satisfactory therapy. Furthermore, in order to have the best outcome, an early diagnosis is recommended and thoracostomy as the surgical procedure, with primary suture.

Many investigations about superalloys and coatings have been done in the laboratory, but evaluating the degradation condition of hot section components during service is still important not only for repair and reuse but also for outage prevention. Time dependent degradation for 1st stage blades of gas turbine was investigated. The degradation analysis for used blades was divided into microstructure changes by position in the blade and mechanical tests. In the microstructure analysis, the rafting and coarsening of γ', MC decomposition and TCP(Topologically closed packed) phase formation occurred and progressed with increasing service time, and especially at the leading and trailing edge of top layer which should be a check points for used blades. Stress-rupture test results of 25,000 and 52,000 EOH(Equivalent Operating Hours) for used blades were also compared with serviced time and position in each blade.

Abstract:
Introduction Terson syndrome is described as an intraocular hemorrhage consequent to a spontaneous subarachnoid hemorrhage (SSAH). In the present article, we describe cases of patients who underwent neurosurgical treatment of ruptured cerebral aneurysm at our institution over a period of one year, and who were diagnosed with Terson syndrome. Methods The present study included patients with a diagnosis of SSAH by rupture of a cerebral aneurysm who underwent treatment in our neurosurgical service from December 2009 to December 2010. The patients were followed-up for a minimum of 20 months. We have also performed a literature review and compared the data with those available in the current literature. Results The present study included 34 patients, 18 (53%) of which underwent endovascular treatment, and 16 (47%) who underwent microsurgical clipping. In the sample, the mortality was 14.7% (5 patients), the same percentage of patients who were diagnosed with Terson Syndrome, which is an incidence of 14.7%. Regarding the ophthalmologic evaluation, all patients had vitreous hemorrhage detected by an ultrasound examination, which was unilateral in only two patients. Visual acuity improved in all patients, being incomplete in only one of them. Conclusion Terson syndrome is relatively common and is associated with higher mortality. With the existence of an effective treatment, it should be investigated in all patients with SSAH.

Abstract:
Welded high-temperature power plant components can experience a greater risk of failure by creep during service, when compared to similar as-wrought components. The heat-affected zone (HAZ) of alloy steels is usually the region of a weldment exhibiting poor mechanical properties. The arc welding of an ASTM A470, Class 8-rotor steel in this study identified the intercritical heat affected zone (ICHAZ) as the weakest region in terms of creep rupture life. The type of welding procedure significantly affects this region, but most important are the welding parameters utilized. Because of the microstructural heterogeneity of the HAZ and sensitivity of these microstructures to changes when exposed to elevated temperatures, their performance at later times is difficult to predict. Extrapolation techniques are limited in value for predicting service lives of homogeneous materials, because these do not incorporate the microstructure changes of the materials during high temperature operation. They are even less useful for predicting the operating lives of weldments. This paper considers the creep performance and structure stability of the ICHAZ of 12 percent Cr and 214-1Mo vanadium modified weldments produced on a retired CrMoV rotor steel. Abstract:
A large number of inclusion defects were found during the metal supervision process of 15Cr1Mo1V steel main steam pipeline of the Russian-made thermal power unit. In this paper, the tracking supervision research of 15Cr1Mo1V steel pipe elbow with inclusion defects is carried out. The variation law of strength, toughness, metallographic structure and creep rupture strength with inclusion defects at different operating time is studied. Type and composition of inclusions are analysed and creep fatigue crack evaluation of the most serious inclusion defects discovered is performed. The results show that with the increase of service time, the room temperature and high temperature strength of the material gradually decreased, the impact toughness deteriorated, the ductile-brittle transition temperature increased and greater than room temperature, the metallurgical organization aging grade rose from 3 to 4, creep rupture strength decreased, creep aging was increasingly serious, and creep residual life reduced. The main inclusions are plastic MnS、SiO2 and severe inclusion levels up to 3. Longitudinal inclusions are mostly long-chain features, and the ends are sharp but no sharp cracks are found at the sharp ends; the transverse inclusions are granular. Creep fatigue crack evaluation show that there was no obvious growth of material inclusion defects with the increase of service time, it is necessary to strengthen the supervision and inspection of brittle inclusions in the follow-up operation.
