Resumen

This dissertation uses a modified systematic literature review to look at working with parents and carers within child and adolescent psychotherapy, and to consider this tasks relationship to therapeutic outcomes for children and adolescents. The topic is important because psychotherapy with children and adolescents inevitably involves additional relationships. The literature indicates the way this undertaking has been regarded has varied through the history of psychodynamic child and adolescent psychotherapy. Numerous writers reflect on the ongoing neglect and absence of systematic thinking in relation to the task of work with parents and carers. This lack of attention is understood to have been influenced by the traditional model of child and adolescent psychotherapy where the source of the child or adolescent’s distress or difficulty was regarded as being primarily intrapsychic. What is now known regarding the current and active nature of the child or adolescent’s relationship with the parent or carer, and the power and persistence of the parent-child bond has resulted in an acknowledgement of the need for a more equitable balance of focus between internal and external factors. In acknowledging that the external can no longer be seen as peripheral there are compelling clinical reasons to work with parents and carers. This undertaking should not be seen as dependant on the therapist’s orientation or interest. The significant scope of possibilities for work with parents and carers within child and adolescent psychotherapy is explored; however there is a lack of data relating to the clinical effectiveness of these approaches. The future need is for systematic thinking, and the development of practice guidelines for this clinical task.

Adolescents have a high prevalence of mental health problems but are unlikely to seek help. Adults, including parents are important within this process. The study therefore aimed to develop a theory of the influence of parents upon adolescent help seeking. Method Eighteen semi-structured interviews were conducted with adolescents, their parents and clinicians working within Child and Adolescent Mental Health Services (CAMHS). A grounded theory analysis allowed for the in-depth exploration of participants’ experiences. Findings A model was developed identifying help seeking as a family journey. Parents were highly influential in the help seeking journey, and parents who were able to be more available to their adolescents tended to be more involved in the help seeking process. Other adults were utilised within the help seeking process. Once adolescents were engaged with the help seeking process they were often able to then seek further help independently. Conclusions The findings suggest that consideration should be given to making services accessible to adolescents. CAMHS services should explore ways with adolescents to give control over parental involvement, and ways with parents to develop availability. Future research should consider the experiences of older and younger adolescents separately, and the transition into adult services.

Resumen

The systemic orientation to behavioral change and the use of paradoxical interventions has been predominantly associated with the family therapy movement. Recently practitioners have been experimenting with the use of strategic/systemic methods in residential treatment centers, schools, and hospital inpatient settings. The literature suggests that these interventions may be ideally suited for oppositional or reluctant clients who resist cooperating in the treatment process. This thesis provides a case study description of an adolescent treatment home which has developed a strategic/systemic approach to residential care. The implementation of a systemic perspective to residential treatment required substantial changes to traditional child care philosophy and practice. These changes are described and discussed and actual intervention examples are presented which help to illuminate this novel approach to residential treatment.

Resumen

Even though there has been extensive research on depression in adults, relatively little is known about depression in adolescents and children. This is especially true for adolescents and children of different ethnic backgrounds. Because depression at younger ages has been shown to predict depression in adults, and because we are becoming more ethnically diverse as a nation, the paucity of research in this area is a serious oversight. The goal of the present study was to identify the prevalence of depression in an ethnically diverse group of adolescents, as well as to identify demographic variables that may contribute to depression and life dissatisfaction in this group. The present study investigated 11 ethnic groups of adolescent males and females: Caucasian, African American, Hispanic, Cambodian, Vietnamese, Laos/Hmong, Chinese, Japanese, Pacific Islander, Filipino, and other. Participants were recruited from a public high school in the Stockton Unified School District. Each participant was administered the Reynold's Adolescent Depression Scale (RADS), a self-report measure of depression; the DSM Scale for Depression (DSD), a DSM-IV -referenced measure of major depressive disorder; and the Life Satisfaction Scale for Adolescents (LSS-A), a measure of life satisfaction for specific domains of adolescent life. Results showed that females scored significantly higher on both depression measures. Ethnic differences were found on the measure for overall life satisfaction. Cambodians, compared to other ethnic groups, showed the lowest life satisfaction. Other significant differences were found between males and females on specific domains of life satisfaction. Socioeconomic status, family structure, and immigration status were found to contribute significantly to higher depression scores.

Resumen

The role of motivation in relation to youth symptoms and psychotherapy outcomes is not well understood. Some cross-sectional research suggests that motivation predicts youth treatment outcome in low-motivation populations. The purpose of this study was to examine patterns of change in youth motivation over the course of treatment and to elucidate the relation between motivation, youth symptoms, and psychotherapy outcomes in a routine community mental health setting. Participants and their caregivers were from three community mental health outpatient clinics and completed youth or parent forms of the Youth Outcome Questionnaire (Y-OQ) and Treatment Support Measure (TSM) at frequent intervals throughout treatment. Data were collected over a period of about 2 years. On average, youth motivation significantly increased over the course of therapy according to self- (p < .001) and parent-report (p < .001). This change followed a square root function better than linear and quadratic models. Initial motivation was not predictive of overall change in symptoms or rate of change at the p <.05 level after accounting for initial levels of youth symptoms. Individual rates of change for youth motivation varied significantly over the course of treatment (p < .001), which might suggest unique trajectories of motivation for different subsets of youth based on presenting concerns or other variables.

This study takes as its subject the clinical work with 7 older adolescents who attended for once-weekly psychoanalytic group psychotherapy, and focuses retrospectively on the first 15 months of this intervention, in which the researcher was a co-therapist. The clinical process notes formed the data set. The starting point for this thesis is our conception of an inherent, developmental relationship to groups, and to the intersubjective relating that exists in human beings. It then moves on to the psychoanalytic thinking about groups and the emotional disturbance that emerged during World War II found in the work of WR Bion and SH Foulkes. It further examines literature on adolescence as a developmental process, adolescent breakdown, and the particular psychosocial risks and challenges of later adolescence. The intrinsic complexity in the data precipitated initial conceptualisations – for example, borrowing Foulkes’ notion of figure-ground - to help apprehend the material. Then, using a form of Grounded Theory, the data set was examined methodically. This evidenced how members brought complex, changing constellations of feeling, and mental and bodily states to the group. Analysis revealed relational and developmental predicaments which would interweave inter-relationally at both conscious and unconscious levels. Using both narrative and tabular forms of presentation, it is demonstrated how this shared, multi-dimensional matrix of iv relationship and communication created the bedrock of the group therapeutic encounter. Emotional and psychological growth developed in the context of members’ capacities to bear emotional knowledge, and hold emotional states over time as individual preoccupations became less pressing within a heuristic relational encounter within the group. This conferred to the group the qualities of Bollas’ ‘transformational object’, while the matrix itself linked with Stern’s primary inter subjective matrix. It is suggested that group psychotherapy has much to offer young people whose relational and psychosocial struggles can be explored in the safety and stability of the clinical group setting.

Resumen

The purpose of this study was to explore and describe the potential use of Sandplay psychotherapy with an adolescent who presented with poor self-esteem. In conducting my research I assessed one adolescent’s selfesteem by means of quantitative and qualitative measures and identified potential aspects that might require intervention, related to self-esteem. After the initial assessment, Sandplay psychotherapy was implemented as intervention, after which the participant’s self-esteem was re-assessed, comparing the results with those obtained during the initial assessment (baseline data). For the purpose of my study, I fulfilled the role of researcher, involving an intern educational psychologist in assessing the participant and conducting the intervention, thereby fulfilling the role of therapist. I followed a triangulation mixed methods intervention research approach, primarily relying on the interpretivist paradigm yet being informed and elaborated upon by a post-positivist component. I studied a single clinical case situated within the context of psychotherapy outcome research. This enabled me to draw conclusions on how suitable and helpful Sandplay psychotherapy might be (or not) for an adolescent who presented with poor self-esteem. In collecting quantitative data, I employed the Self-Esteem Index (Brown&Alexander, 1991) both prior to and after the intervention had been completed. In support, a qualitative post-modern assessment was included in the form of the Rosebush technique (Oaklander, 1988). Other qualitative data collection and documentation measures include observations, semi-structured interviews (with the participant and her parents), photographs of the sand trays and a semi-structured reflection journal (of the participant), as well as reflection journals of the researcher and therapist. Based on the pre- and post-assessments I was able to compare results of the participant’s self-esteem before and after the Sandplay psychotherapy intervention had been completed. Subsequent to thematic qualitative data analysis and my comparison of the quantitative results I obtained, three main themes emerged. Firstly, the self-esteem of the participant seemed to have improved following the intervention, since her limited self-knowledge, feelings of inferiority and her uncertainty in unknown situations seemingly changed into age-appropriate self-knowledge, feelings of self-worth and facing the unknown. Secondly, she displayed changes in the emotional domain of development post-intervention. She seemed more content with her situation, reportedly feeling more safe and secure as opposed to feeling academically pressured, as was the case at the onset of my study, and having a positive future perspective. Thirdly, changes in her social domain of development occurred as she displayed age-appropriate skills, acted more assertively and felt more accepted and ready to establish relationships. Based on the findings of my study, I can conclude that it seems possible to facilitate the improvement of adolescents’ self-esteem by conducting Sandplay psychotherapy. Besides an improvement in terms of overall self-esteem, the participant in my study seemingly displayed improvement in the areas of relationships and social skills, as well as conflict resolution skills.

Resumen

Individualism, the assumption that persons are self-contained and primarily act for the sake of the self, is prevalent in American culture and has arguably perpetuated numerous psychological and societal ills. Relationality, the assumption that persons are always and inextricably constituted by relationships, has been posited as a philosophical and practical alternative to individualist culture. Several scholars, both inside and outside of psychology, have developed relational concepts and practices, including some who have elucidated a relational approach to psychotherapy (e.g., Slife and Wiggins, 2009). This study examines the implications and effects of this therapeutic approach, particularly exploring relationality's therapeutic success in countering the implications of individualism. Greenbrier Academy, an adolescent female boarding school located in West Virginia, has adopted relationality as its guiding therapeutic ethic. This study utilized hermeneutically modified grounded-theory methods to inquire into the lived experience of students at Greenbrier Academy. Eight students were interviewed and the researcher recorded observations of daily programing over a six-day period. Results indicated that Greenbrier's students' experienced marked changes in the quality and meaning of their interpersonal relationships. They increasingly cared for and served their relationships, engaged in more intimate relationships with others, approached (rather than retreated from) others' differences, viewed others more holistically, and accepted personal responsibility in relation to their context.

A prominent need in youth psychotherapy literature includes the examination of factors related to improved psychotherapeutic outcomes within the context of "real world" clinical settings, where the practice of psychotherapy differs significantly from that in controlled clinical trials. In examining those factors related to improved outcomes in youth psychotherapy, variables related to social support are important to consider. The purpose of the present study was to evaluate youth perceptions of social support as a potential predictor of successful treatment outcomes in a traditional community outpatient treatment setting for youth. In addition, this study examined whether perceptions of social support predicted both youth and parent-reported symptoms. Also examined were differential outcomes predicted by social support for youth with primarily internalizing vs. externalizing symptoms. Youth and parent-reported symptoms were measured using the Y-OQ and the Y-OQ-SR (Y-OQ; Burlingame, Wells, Lambert, & Cox, 2004; Y-OQ-SR; Wells, Burlingame, & Rose, 2003). Perceived social support was measured using the TSM social support subscale for youth reporters. Internalizing and externalizing symptoms were measured using the CBCL. Using a combination of Analysis of Variance and Hierarchical Linear Modeling with a sample of 199 youth, aged 10-17, and their parents, this study examined the relationship between perceived social support and youth progress in therapy. Results revealed that youth perceptions of social support were significantly associated with youth intake symptoms for both parent and youth-reported symptoms. In addition, youth-reported perceptions of social support improved significantly over the course of treatment. Also, increased overall change in perceived social support was associated with decreased overall symptoms for youth reporters. Finally, no significant differences were found for youth presenting with primarily internalizing vs. primarily externalizing symptoms. Examining the associations between perceptions of social support and youth psychotherapy outcomes is an important step toward identifying potential mechanisms of change in youth mental health treatment. The results of this study provide valuable information on the importance of attending to perceptions of social support in the assessment and treatment of youth mental health issues.

Barns mentala ohälsa ökar kraftigt. Barnpsykoterapi med inslag av fri lek, här kallad lekterapi, har under många år varit den traditionella behandlingen av barn i behov av psykoterapi. För att ha en möjlighet att kunna jämföras vetenskapligt med andra dokumenterade behandlingsmetoder behöver evidensen för lekterapi som behandlingsmetod undersökas, stärkas liksom spridas. Syftet med föreliggande uppsats är att undersöka kunskapsläget inom lekterapi som behandlingsmetod inom barnpsykoterapi. Totalt 41 studier har granskats systematiskt enligt evidensbaserad medicinsk metod. Tjugoen av dem poängbedömdes medan övriga 20 delades upp i temagrupper för att visa ett ytterligare djup och bredd av de analyserade studierna. Residential placement is considered the most severe sanction for adjudicated youth, yet there is little consensus on best practices and interventions in residential settings. Demographic trends in the juvenile justice system further exacerbate challenges in studying residential placements. Disparities among minority youth, diverging state definitions of juvenile sex offenses, discrepancies in recidivism measures, and variations in local and state juvenile courts have contributed to a convoluted system that has struggled to identify the meaning of "success" in residential settings. Building on theories of engagement in residential care and program theory of change, this mixed methods study explores how various components of a residential program for adjudicated young males contribute to time in the program leading up to transition out of the program. Event history analysis was used to examine administrative program data. Then, follow-up interviews were conducted with program employees to gather further insight to supplement quantitative findings. Findings indicated that juvenile sex offenses and home visits played a significant role. Additionally, race, ethnicity, and duration and engagement in family, individual, and group sessions also played varying roles in youths' transition out of the program. Findings from the employee interviews further supported that race, ethnicity, culture, and family are all critical parts of residential treatment. The interviews also discussed the role that Collaborative and Proactive Solutions, a behavioral modification approach used in this program, plays in youths' overall engagement during their time in the program.

Since this is likely to become a more common family type, the thesis examined the literature on biracial stepfamilies , so as to reflect on the therapy and determine whether race and cultural differences were the most important cause of family dysfunction. While race and culture emerged as an important stressor, pathology in the family system was found to be crucial in the therapy. Considerations for counsellors are addressed and further South African research in this area is called for.

Therapistsʼ process notes - written descriptions of a session produced shortly afterwards from memory - hold a significant role in child and adolescent psychoanalytic psychotherapy. They are central in training, in supervision, and in developing oneʼs understanding through selfsupervision and forms of psychotherapy research. This thesis examines such process notes through a comparison with audio recordings of the same sessions. In so doing, it aims to generate theory that might illuminate the causes of significantly patterned discrepancies between the notes and recordings, in order to understand more about the processes at work in psychoanalytic psychotherapy and to explore the nature of process notes, their values and limitations. The literature searches conducted revealed limited relevant studies. All identified studies that compare process notes with recordings of sessions seek to quantify the differences between the two forms of recording. Unlike these, this thesis explores the meaning of the differences between process notes and recordings through qualitative data analysis. Using psychoanalytically informed grounded theory, in total nine sets of process notes and recordings from three different psychoanalytic psychotherapists are analysed. The analysis identifies eight core categories of findings. Initial theories are developed from these categories, most significantly concerning the role and influence of a ʻcore transference dynamicʼ between therapist and patient. Further theory is developed on the nature and function of process notes as a means for the therapistʼs conscious and unconscious processing of the session, as well as on the nature of the influence of the relationships – both internal and external – within which they are written. In the light of the findings, a proposal is made for a new approach for learning about the patient and clinical work, ʻthe comparison methodʼ (supervision involving a comparison of process notes and recordings), and, in particular, for its inclusion within the training of psychoanalytic psychotherapists. Further recommendations for research are also made.

Premature termination from child and adolescent psychotherapy is a prevalent problem for clients, their families, and mental health services. Rates of premature termination have been estimated at a range of 16-72%. Many variables have been examined as potential predictors of premature termination, yielding inconsistent conclusions. Researchers propose that part of this variability in rates and predictors is due to the inconsistent application of definitions of premature termination. The past literature identifies two main categories of definitions in this line of research: therapist judgment and number of sessions. This study aims to incorporate a relatively new definition, clinically significant change, in the evaluation of premature termination rates and predictor variables in a sample of adolescents receiving treatment in three community mental health clinics. Results showed that 65.7% of the participants were classified as premature terminators under the definition of clinically significant change. Premature termination was significantly correlated with parenting skills and ratings of the therapeutic alliance by both the parent and adolescent. However, the only variable that predicted PT was the therapeutic alliance as rated by the adolescent. It appears that, rather than having a small range of rates and universal predictors of premature termination, it may depend on the definition that is chosen.

Cette recherche s’appuie sur une articulation clinico-théorique d’obédience psychanalytique. L’hypothèse générale est la suivante : dans le cadre d’une prise en charge psychothérapeutique, un adolescent handicapé mental peut produire une élaboration psychique de son handicap. Je nomme cette dernière « travail sur le handicap ». Le cadre de la prise en charge psychothérapeutique conduit l’adolescent handicapé mental à construire des représentations de son propre handicap. Ce travail de recherche s’étaye sur trois prises en charge psychothérapeutiques d’adolescents handicapés mentaux. Aussi riche que complexe, le matériel clinique issu des trois cas cliniques permet de rendre compte de la réalisation d’un « travail sur le handicap » sur la base d’une co-construction entre le thérapeute et le sujet. This work aims to demonstrate the relevance and the issue of a psychic elaboration work of the mentally handicapped adolescent subject in the setting of a psychotherapeutic caretaking. This research is supported by the clinical and theoretical articulation of psychoanalytical allegiance. The general assumption is as follows: in the setting of a psychotherapeutic caretaking, a mentally handicapped adolescent is able to produce a psychic elaboration of his handicap, which I name “work on the handicap”. The setting of the psychotherapeutic caretaking leads the mentally handicapped adolescent to build some representations of his own handicap. This research is supported by three psychotherapeutic care takings of mentally handicapped adolescents. Equally rich and complex, the clinical material coming from the three clinical cases demonstrates the carrying out of a work on the handicap, based on a co-construction between the therapist and the subject. The work on the handicap therefore could be the result of a psychic elaboration combining the work of mourning, the transformation of thought and representability. If the work on the handicap turns out to be painful in many regards for the mentally handicapped adolescent subject, it remains essential for his psychic construction

Youth psychotherapy literature is in need of more research related to understanding psychotherapy process and outcome in child psychotherapy in community settings. The purpose of this study was to examine how key parenting skills were associated with child and adolescent symptoms and outcomes over the course of treatment in an outpatient community mental health system. Much of the research on child and adolescent outcomes has been conducted in controlled research settings, which raises the importance of more research needing to be done in representative clinical practice conditions (Weisz & Jensen, 2001). Further, few child and adolescent studies have examined potential mechanisms of change in child and adolescent psychotherapy (Kazdin & Nock, 2003), including moderators and mediators of the relation between parenting skills and child and adolescent outcomes.Participants included 407 youth, ages 4-17 (mean age = 9.7 years), and their parents or guardians, receiving routine outpatient mental health services in a community mental health setting. The youth sample included 51% males, 49% females. Measures used included the Youth Outcome Questionnaire (Y-OQ; Burlingame, Wells, Lambert, & Cox, 2004; Burlingame et al., 2001), and the Treatment Support Measure (TSM). Data were collected starting at the intake session, each of the first five therapy sessions, then every three weeks thereafter for six months post-intake. Parenting Skills items from the TSM included domains of overreactivity, laxness, verbosity, monitoring, consistency, and positive reinforcement. Hierarchical linear modeling was used to examine changes in parenting behaviors and youth symptoms. Results indicated that parenting skills significantly improved over the course of treatment (p < .001) and best fit a logarithmic (natural log) function, such that most of the reported change in parenting skills occurred during the first few sessions of treatment. Further, there was a significant inverse relationship at intake between parenting skills and Y-OQ scores; specifically, lower parenting skills scores were associated with higher Y-OQ scores for parent and youth report (p < .001; p = .033). In addition, parenting skills at intake were associated with the subsequent rate of change of youth symptoms for parent report (p < .001) and youth report (p = .026). Lastly, improvements in parenting scores were associated with improved youth symptoms over the course of treatment for parent and youth report of symptoms (p = .021; p = .02). These findings can be generalized to other community outpatient settings and highlight the importance of attending to parenting skills as an avenue to improving child psychotherapy outcomes. Specifically, the results of this study emphasize the importance of parents implementing effective parenting skills and its influence on their child's overall symptoms at intake and outcomes in therapy.

This study was designed with an interest for the healing of emotional distress shared by the disciplines of psychology, philosophy, and spirituality. It sought to merge these disciplines into a coherent therapeutic context for human interaction that could be grounded in the shared human condition, the essential need for meaning, freedom, and relationship. This study also sought to examine the importance of spirituality across the lifespan to the practice of psychotherapy and particularly to explore the spiritual experiences of the adolescent male and discover the meaning and healing effects those experiences have for them, especially those who have experienced significant emotional or relationship difficulties in their lives. Using a phenomenological methodology for the qualitative data analysis, 10 interviews were conducted with young adult males who had participated in a unique spiritual retreat while in high school and during a particularly troubled time in their lives. An exhaustive document review took place. Two distinct phenomena emerged: Lost in Suffering, a state of life prior to the retreat and Found in Redemption: the results of a four day experience of bonding, healing, and transformation, each experience containing five distinct themes. The result is a deep understanding of the lived experience of the participants and a powerful implication regarding the place of spiritual experience in psychotherapeutic healing.

This study assessed the efficacy of the application of cognitive-behavioral self control therapy with a sample of psychiatrically impaired adolescents. Ten adolescent inpatients (5 male/5 female), residing within the Adolescent Unit of the State Hospital of a Southwestern state, were selected as subjects. Subjects ranged in age from 12 to 17 years and were paid volunteers. Subjects were assessed pretreatment for non self controlled behavior via the teacher rated Self Control Rating Scale (Kendall & Wilcox, 1979). This measure also served as the blocking variable utilized for random assignment to treatment or control group. Five adolescents were assigned to each group. Pretreatment measures of social perspective taking (Chandler Bystander Cartoons; Chandler, 1973) and social problem solving (Means-Ends Problem Solving test: Platt & Spivack, 1975) were taken additionally. All three measures were repeated at posttreatment and at 4 week follow-up. Treatment consisted of twelve 60-minute sessions held 3 times a week over the period of 4 weeks. The treatment consisted of a group application of Kendall's (1980) Cognitive-Behavioral Self Control therapy. The main treatment strategies included: (1) a problem solving approach, (2) self instructional training, (3) behavioral contingencies, (4) modeling, (5) affective education, and (6) role play exercises. The separate strategies were essentially interwoven. Except for the cognitive-behavioral self control training proper, subjects in both treatment and control groups were given similar tasks, task instructions, and performance feedback. Results found a range of behavioral self control skills distributed among the subjects but failed to support the hypothesis of associated poor social perspective taking and social problem solving skills. Treatment failed to improve teacher ratings of behavioral self control at posttreatment and at follow-up. Likewise, no significant improvement was found in social problem solving skills at posttreatment or follow-up, although a trend toward improvement was suggested. A significant improvement in social perspective taking skills was found in the treatment group at posttreatment. The improvement was maintained at 4 week follow-up. The results are interpreted as suggesting that cognitive-behavioral self control training can be useful in facilitating the further development and enhancement of previously inadequate cognitive capacities in psychiatrically impaired, non self controlled adolescents. Certain suggestions for enhancing the effectiveness and generalization of the treatment approach are discussed.

In recent years, an important focus has been placed on training counsellors to attend to cultural diversity. In an effort to assist counsellors in this process, models of Multicultural Counselling Competencies (MCC) have been developed for work with adult populations. Unfortunately, little attention has been given to adapting these models or developing specific models for counselling with adolescents. While clinical guidelines have been proposed for counsellors working with culturally diverse youth, they have yet to be empirically tested. The present study investigated the MCC which former adolescent clients believed to be important to them when they attended counselling. Participants included 20 women between the ages of 18 and 23 who sought counselling when they were between the ages of 12 and 19. Participants completed an in-depth qualitative interview about their counselling experiences and were asked to discuss the aspects of counselling they found to be most and least helpful. The interview data was analyzed using phenomenological methodology. The components of cultural competence (counsellor awareness, knowledge, and skills) proposed in the tripartite model of MCC and the multicultural counselling relationship were used as points of entry in the data analysis process. The results of this study provided support for the integration of these competencies in counselling with female adolescents. However, participant's accounts indicated that the competencies of awareness, knowledge, skills, and relationship are not sufficient for culturally competent counselling to occur. Additional MCC were found to be important for work with female adolescents, including cultural sensitivity, family interventions, and help beyond counselling. The results of this study have important implications for the training and practice of psychology. Obtaining the viewpoint of female clients who attended counselling during their adolescence was invaluable, as they offered a unique perspective, which was not previously available in the literature. This unique viewpoint informs us about the specific needs of female adolescents and provides useful information about the MCC counsellors may need to develop in order to work with this population.

Essa Dissertação de Mestrado em Psicologia Clínica teve como foco a possibilidade de desenvolvimento da capacidade de mentalização na psicoterapia de uma adolescente que apresentava sintomas depressivos e havia buscado atendimento no Serviço de Psicologia de uma Universidade do Sul do Brasil. Com base nas contribuições da vertente psicanalítica da teoria do apego, mais especificamente nos conceitos de função reflexiva e capacidade de mentalização, foi realizado um estudo pautado pela abordagem qualitativo-exploratória, adotando-se o procedimento de estudo de caso. A participante desse estudo foi uma adolescente de 18 anos, que foi atendida em psicoterapia pelo período de seis meses, duas vezes por semana. Antes e depois do processo psicoterápico, a adolescente foi avaliada com o Children’s Depression Inventory (CDI) para o levantamento dos indicadores de depressão e com uma Entrevista Estruturada, sobre a qual foi aplicado o Checklist para Avaliação Clínica da Mentalização. Além desses instrumentos, foi utilizado também o dispositivo da escrita em psicoterapia a fim de explorar seus benefícios tanto para o desenvolvimento da capacidade de mentalização quanto para a redução dos sintomas depressivos na adolescência. Os resultados obtidos apontaram que a adolescente apresentava uma capacidade de mentalização comprometida no início do tratamento, mas que pôde ser desenvolvida ao longo deste, contribuindo significativamente para a diminuição dos seus sintomas depressivos. O uso da escrita nesta abordagem psicoterápica facilitou o desenvolvimento da capacidade de mentalização da paciente, ajudando-a a alcançar uma compreensão mais profunda da própria experiência afetiva, atribuindo novos significados aos afetos que se traduziram em formas novas e mais flexíveis de lidar com a sua problemática.
This study in Clinic Psychology focused the possibility of development of mentalization in the psychotherapy of an adolescent who had depressive symptoms and had sought treatment at a Department of Psychology University of Southern Brazil. Based on the contributions of the attachment theory, specifically the concepts of the reflective function and capacity of mentalization, a study guided by the qualitative-exploratory approach was made adopting the procedure of Case Study. The participants of this study was a adolescent of 18 years which was treated in psychotherapy for six months, twice a week. The adolescent were evaluated before and after the psychotherapeutic process, with Children's Depression Inventory (CDI) for the analysis of depression indicators and a Structured Interview, which was applied on the Checklist for Evaluation of Clinical Mentalization. Besides these instruments, was also used the device of writing in psychotherapy in order to exploit its benefits both for the development of mentalization as for reducing depressive symptoms in adolescence. The results showed that the teenager had committed a mentalization at the beginning of treatment, but that could be developed over this, contributing significantly to the reduction of their depressive symptoms. The use of writing in this psychotherapeutic approach facilitated the development of mentalization of the patient, helping them to achieve a deeper understanding of their own affective experience, giving new meaning to the affects that were translated into new and more flexible to cope with its problematic.

This study examined interpersonal predictors of early therapeutic alliance in a cognitive-behavioral treatment study for adolescents with anxiety and/or depressive disorders. Client, therapist, and observer rated measures of alliance were obtained from the third session of treatment in a sample of twenty-seven adolescents. Results indicate that alliance ratings across the three perspectives are significantly associated with one another, but that pretreatment interpersonal variables were differentially associated with varying informant perspectives. Adolescents’ perceptions of their relationships to their caregivers positively related to their perceptions of the therapeutic alliance, such that adolescents with more positive representations of relationships with their attachment figures were more likely to endorse stronger working alliances early in treatment. Adolescent-reported symptom severity at pretreatment predicted observer ratings of alliance, such that adolescents who indicated greater symptoms at pretreatment were rated as having stronger early alliances with their therapists by independent observers. Adolescent perceptions of social support also predicted observer ratings of alliance. Therapists perceived having weaker alliances with adolescents showing clinically significant depression, as compared to adolescents diagnosed with anxiety disorders alone. Future research is needed to examine whether identification of relevant interpersonal factors can help improve therapeutic engagement and outcomes for the psychosocial treatment of adolescents with anxiety and depressive disorders.
