Resumen
Sir Sultan Muhammad Shah Aga Khan III, a renowned Muslim leader, was instrumental in the freedom movement in Pakistan and provided significant support for Muslims in India. He was ideally positioned to play a big role in the international affairs of his time because of his extensive understanding of several cultural traditions, and his long public career had numerous facets. This study examines the contributions of Sir Sultan Muhammad Shah, Agha Khan III with particular attention to the social reforms and political achievements of his life. The article sheds light on his role in the Pakistan Movement as well as his social services for the boost up of Indian Muslims. This study is basically qualitative in nature and a descriptive method with a historical approach is applied.

Resumen
This article explores the changing relations between state officials and the Shiʿi Nizari Ismaʿilis in early 20th-century Ottoman Syria. It examines the history surrounding the founding of the School of Agriculture in the majority Ismaʿili town of Salamiyya. Ottoman authorities had only recently discovered that the Ismaʿilis were followers of an imam in Bombay, the Aga Khan III. Once the community was associated with a British Empire loyalist like the Aga Khan, officials suspected collusion. Subsequent criminal investigations sanctioned legal and political persecution against the Ismaʿilis. Arresting and imprisoning the Ismaʿilis, however, could only do so much. Officials decided on a policy to correct their beliefs through state schooling and turn the Ismaʿilis into loyal Ottomans. Provincial authorities, meanwhile, took advantage of Istanbul’s doubts over the Ismaʿilis’ loyalty to the Empire. They proceeded to arrest the Ismaʿilis and confiscate tens of thousands of gold liras in cash and jewellery from the community. The cash and valuables were buried in a fund which ultimately paid for the School of Agriculture. This article concludes that violence was mandated by all levels of government and prefigured any educational policy for the Ismaʿilis in Syria because of fears the community was a fifth column.

The article takes into consideration the unique data about Pamir Ismailism collected by the secretary of the Imperial Political Agency in Bukhara the Baron A. A. Cherkasov in 1904. There are presented the main provisions of the data reported by A. A. Cherkasov, to having the science enriched by new data on Pamir Ismailism. There were given some features of the doctrine of the Pamir Ismailis and how special this form of Ismailism is in comparison with other forms of Islam. This information make it possible to present the attitude of the Ismaili Imam Aga Khan III to the policy of Russia and England in the region and Russian officials’ perception of the Pamir population, Ismailis and Ismailism in general. In addition, they are supplemented by the information reported by A. A. Bobrinsky, A. A. Semenov and A. E. Snesarev on the doctrine and customs of the Pamir Ismailis.

AbstractThis paper analyzes the political project of secular Islam as outlined by the Indian political and religious leader, Muhammad Shah—also known as Aga Khan III (1877–1957). As first president of the All India Muslim League, Muhammad Shah facilitated the installation of separate electorates for Muslims as well as the call for Partition. The reformist notion of Islam he invoked for this separatist programme was informed by the secular and modernizing projects of the colonial public sphere. Simultaneously, however, Muhammad Shah claimed a divine role as Imam of the Ismaili Muslim community—a position validated by Ismaili beliefs and teachings of messianic Islam. The paper engages Muhammad Shah's writings and the devotional texts of the Ismailis to illustrate how the heterogeneous forms of practices peculiar to the vernacular history of Islam in early modern South Asia were displaced by the discourse of religious identity in the colonial period.

Objective: To determine the frequency and extent of artefacts in magnetic resonance imaging and/or computed tomography scans of head caused by fixed dental prosthesis. Method: The retrospective study was conducted at Aga Khan University Hospital from July to December 2021, and comprised magnetic resonance imaging and/or computed tomography scans from January 2015 to December 2020 of the head of individuals with existing fixed dental prosthetic work at the time of exposure. They were analysed for the presence of artefacts. The association between artefacts and the presence of fixed dental prosthesis was explored. Data was analysed using SPSS 23. Results: Of the 297 images evaluated, 173 (58%) were magnetic resonance imaging scans, and 124(42%) were computed tomography scans. The most common artefacts was grade I 148(49.8%), followed by grade II 140(47.1%) and grade III 9(3%). There was no significant association between fixed dental prosthesis and the artefacts (p>0.05). Conclusion: There should be no reservations in placing fixed metal prosthesis in individuals on account of future brain scans. Background. Acute Kidney Injury (AKI) is common in elderly people (EP). There is paucity of data on predictor of mortality in EP with AKI. Objective. This study was done to know more about factors associated with inpatient mortality in EP with AKI. Methods. We retrospectively reviewed medical records of patients aged 65 years or above hospitalized with a diagnosis of AKI at Aga Khan University Hospital, Karachi, between January 2005 and December 2010. Binary logistic regression models were constructed to identify factors associated with mortality in EP with AKI. Results. 431 patients had AKI, with 341 (79.1%) having stage I AKI, 56 (13%) having stage II AKI, and 34 (7.9%) having stage III AKI. Out of 431 patients, 142 (32.9%) died. Mortality increased with increasing severity of AKI. Mortality was 50% (17/34) in AKI stage III, 44.6% (25/56) in AKI stage II, and 29.3% (100/341) in AKI stage I. Factors associated with increased inpatients mortality were presence of stage III AKI (OR: 3.20, P=0.04, 95% CI: 1.05–9.72), presence of oliguria (OR: 3.42, P=0.006, 95% CI: 1.42–8.22), and need for vasopressors (OR: 6.90, P<0.001, 95% CI: 2.42–19.65). Median bicarbonate 18 versus 17 between those who survived and those who died was associated with less mortality (OR: 0.94, P=0.02, 95% CI: 0.89–0.99). History of hypertension (OR: 0.49, P=0.03, 95% CI: 0.25–0.95) and high admission creatinine (OR: 0.68, P=0.01, 95% CI: 0.50–0.91) were also associated with less mortality. Conclusion. Mortality in EP increases with increasing severity of AKI. Presence of stage III AKI, oliguria, and hemodynamic instability needing vasopressor are associated with increased mortality. Increased median bicarbonate, presence of hypertension, and high admission creatinine were various factors associated with decreased inpatient mortality. Increasing age and need for dialysis did not increase mortality in elderly population.

Background Illness predictive scoring systems are significant and meaningful adjuncts of patient management in the Intensive Care Unit (ICU). They assist in predicting patient outcomes, improve clinical decision making and provide insight into the effectiveness of care and management of patients while optimizing the use of hospital resources. We evaluated mortality predictive performance of Simplified Acute Physiology Score (SAPS 3) and Mortality Probability Models (MPM0-III) and compared their performance in predicting outcome as well as identifying disease pattern and factors associated with increased mortality. Methods This was a retrospective cohort study of adult patients admitted to the ICU of the Aga Khan Hospital, Dar- es- Salaam, Tanzania between August 2018 and April 2020. Demographics, clinical characteristics, outcomes, source of admission, primary admission category, length of stay and the support provided with the worst physiological data within the first hour of ICU admission were extracted. SAPS 3 and MPM0-III scores were calculated using an online web-based calculator. The performance of each model was assessed by discrimination and calibration. Discrimination between survivors and non–survivors was assessed by the area under the receiver operator characteristic curve (ROC) and calibration was estimated using the Hosmer-Lemeshow goodness-of-fit test. Results A total of 331 patients were enrolled in the study with a median age of 58 years (IQR 43-71), most of whom were male (n = 208, 62.8%), of African origin (n = 178, 53.8%) and admitted from the emergency department (n = 306, 92.4%). In- hospital mortality of critically ill patients was 16.1%. Discrimination was very good for all models, the area under the receiver-operating characteristic (ROC) curve for SAPS 3 and MPM0-III was 0.89 (95% CI [0.844–0.935]) and 0.90 (95% CI [0.864–0.944]) respectively. Calibration as calculated by Hosmer-Lemeshow goodness-of-fit test showed good calibration for SAPS 3 and MPM0-III with Chi- square values of 4.61 and 5.08 respectively and P–Value > 0.05. Conclusion Both SAPS 3 and MPM0-III performed well in predicting mortality and outcome in our cohort of patients admitted to the intensive care unit of a private tertiary hospital. The in-hospital mortality of critically ill patients was lower compared to studies done in other intensive care units in tertiary referral hospitals within Tanzania.

PURPOSE The purpose of this research was to describe the sociodemographic and clinical characteristics of Kenyan women with metastatic breast cancer diagnosed and treated at Aga Khan University Hospital in Nairobi, Kenya from 2012 to 2018. PATIENTS AND METHODS We reviewed charts of Kenyan women with metastatic breast cancer and analyzed sociodemographic data, breast cancer risk factors, and tumor characteristics associated with stage at diagnosis, receptor status (ie, estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2 [HER2]), and site of metastasis using χ2, analysis of variance, two-sample t tests, and logistic regressions. RESULTS A total of 125 cases with complete medical records were included in the analysis. Forty women (32%) had metastases at diagnosis. Of the others, those diagnosed in stage III developed metastases sooner than those diagnosed in stage II ( P < .001). Fifty-eight percent of patients had metastases to bone, 14% to brain, 57% to lungs, and 50% to liver. Seventy-four percent of patients presented with more than one metastatic site. Metastases to bone were associated with greater age at diagnosis ( P = .02) and higher parity ( P = .04), and metastases to the brain were associated with early menopause ( P = .04), lower parity ( P = .04), and lack of breastfeeding ( P = .01). Patients whose tumors were triple negative (estrogen receptor-negative, progesterone receptor-negative, and HER2 negative) were more likely to develop brain metastases ( P = .01), and those whose tumors were HER2 positive were more likely to develop liver metastases ( P = .04). CONCLUSION Although our data on patterns of metastases and pathologic subtypes are similar to those in published literature, some unique findings concerning hormonal risk factors of women with metastatic breast cancer and specific metastatic sites need additional exploration in larger patient populations.

Mais ce projet, en harmonie totale avec celui de l'islah, s'accompagne d'une autre reforme. En effet, l'aga khan est l'imam (chef spirituel) des shi'ites ismaeliens. A ce titre, il realise un aggiornamento de la doctrine caracterise par une extension de son autorite sacree a travers la mise en place d'une centrazlisation des institutions communautaires. Cette double renovation converge sur la fonction rationalisatrice assignee a l'ethique, qui a elle-meme vocation a promouvoir un humanisme universel. C'est pourquoi l'esoterisme, element determinant dans la pensee ismaelienne historique, se deploie autour des notions de service et de bien-etre. En definitive, l'effort theologique deploye par l'aga khan lui permet d'elaborer une modernite veritablement universelle qui reactive un heritage islamique preexistant
The long life of sultan muhammad shan aga khan (1877-1957) coincided with the climax of the confrontaztion between the muslim world and the west. To face this challenge, he was convinced that islamic knowledge had to undergo reforms based on a critical return of its sources; the aga khan established a number of principles which were to constitute the common ground on which all the islamic schools of thought were to be brought together. But this objective, which totally matched the aim of islah. Was accompanied by a additional reform. The aga khan, as the imam (spiritual leader) of the isma'ili shi'ites, intiated a revival of isma'ili doctrine characterised by an enlargement of his autorithy through a centralisation of community institutions. Both changes have in common the rationalizing function applied to ethics, and ethics have the ultimate goal of promoting a universal humanism. That is why esoterism, a decisive element of historical isma'ili thought, spread out around the notions of service and welfare. Finally, the theological effort made by the aga khan enabled him to develop a genuine modernity, which regenerates a ancient islamic legacy
