Atypical demyelinating lesions in multiple sclerosis (MS) (tumefactive demyelinating lesions, lesions with white matter cavitation, leukodystrophy-like lesions), although rare, pose serious differential diagnostic and therapeutic problems. We present 3 clinical cases of children, one with Balo concentric sclerosis and two with multiple sclerosis (MS), who had atypical magnetic resonance findings during the course of the disease or during the initial clinical manifestation. The first patient presented with an acute hemiparesis and hemihypesthesia with MRI evidence of a rounded tumor-like periven

Resumen
Balo's concentric sclerosis (BCS) is a rare demyelinating disease of the central nervous system, characterized by concentric areas of demyelination alternating with areas of preserved myelin in the form of a pathognomonic pathomorphological substrate of varying degrees of severity in the form of «annual rings on a tree cut». The pathogenesis of BCS is diverse and complex, today there are several theories of its occurrence, among which hypoxic, vascular and closely related genetic, as well as a number of other theories are distinguished. The BCS clinic is variable, represented by both focal neu

Balo concentric sclerosis is considered a fulminant demyelinating disease that is characterized by the formation of specific concentric foci in the white matter of the brain that resemble sawing trees. This article describes the clinical case of a patient who has come with complaints of weakness and stiffness in the lower limbs, more in the left leg, a feeling of stiffness and tension in the left hand, the inability to walk without aids. Taking into account the complaints, the clinical picture and the pre-test: laboratory examination of blood and liquor, magnetic resonance imaging (MRI) of the

This chapter describes and illustrates the morphologic changes observed in inflammatory demyelinating diseases. These are defined by the selective destruction of myelin sheaths and oligodendrocytes, arising in a background of acute or chronic inflammation. The spectrum of inflammatory demyelinating diseases includes multiple sclerosis, acute disseminated encephalomyelitis, Balo concentric sclerosis, and neuromyelitis optica. The etiology and pathogenesis of the diseases are discussed and criteria for the diagnosis of inflammatory demyelinating diseases on brain biopsies are provided.

Resumen
This chapter describes the basic neuropathology of multiple sclerosis and other inflammatory demyelinating diseases. It specifically addresses the nature of multiple sclerosis lesions, characterized by inflammation, selective primary demyelination with partial preservation of axons, remyelination, and reactive astroglial scar formation. Attention is paid to the importance of demyelinated lesions in the gray matter, and it discusses the differences of the pathology between the relapsing and the progressive stage of the disease. Finally, it summarizes the similarities and differences between mul

Abstract This chapter describes the basic neuropathology of multiple sclerosis (MS) and other inflammatory demyelinating diseases. MS lesions, characterized by inflammation and selective primary demyelination with partial preservation of axons, include foci of remyelination and reactive astroglial scar formation. It has been recognized that demyelinated lesions are present in the gray matter, and cortical and subcortical injury may play a significant role in progression of neurologic symptoms. There are differences in the pathobiology of early MS lesions, the relapsing stage, and the progressi

Resumen
Introduction: Baló Concentric Sclerosis (BCS) is a rare inflammatory demyelinating disease of the central nervous system. Commonly reported symptoms and its classic neuroimaging findings can lead to a mistaken diagnosis of expanding lesions. The aim is to report on a case of a rare etiology of altered parietal lobe cortical functions, and the importance of topographic reasoning. Review of the patient’s file for clinical history, neurological exam, and complementary tests was performed. Case report: A 36-year-old male patient presented to the emergency room (ER) with complaints of sudden diffic
