Major depressive disorder (MDD) is a significant mental health problem with deleterious effects, including poor health related quality of life and long-term disability. Epidemiological studies suggest that women in particular are more vulnerable to an increased risk of depression, relative to men, beginning at the time of menarche through the menopausal transition. Depression comorbid with chronic medical conditions can often exacerbate the risk of depression, as well as complicate its recognition and treatment. Depression comorbidity can lead to negative outcomes, including progression of the chronic medical condition, poor treatment adherence, and mortality. In this chapter, we explore chronic medical conditions that are associated with a greater prevalence of depression in women relative to men, including type 2 diabetes, fibromyalgia, and rheumatoid arthritis. An overview of epidemiology is followed by a discussion of theories explaining depression comorbidity and approaches to recognizing and treating depression in the context of these chronic medical conditions. Finally, we discuss future research directions with the goal of informing clinical research and practice.

Rates of chronic health conditions in childhood are increasing, and school-based professionals regularly encounter children with chronic health conditions in the school setting. Students with chronic health conditions often require accommodations, assessment, intervention, and close collaboration with medical providers and families. However, most school-based clinicians (school psychologists, counselors, social workers) who are charged with addressing the special needs of these children have not had coursework or experience related to common pediatric conditions. This book is a practical guide for school-based clinicians working with children with chronic health conditions. Section I provides a broad overview of school-related issues for children with chronic health conditions. This includes a review of common medical conditions and terminology and cross-cutting issues related to social and emotional and academic functioning, as well as the role of the school-based professional in collaborating across systems of care. The section also reviews legal and policy issues and alternative educational settings for students with chronic health needs. Section II focuses on prevention, assessment, intervention, and consultation strategies for individual students and entire school systems. Finally, Section III addresses common groups of medical conditions. Each chapter provides an overview of the condition(s), common school-related concerns, risk and protective factors, and cultural considerations, as well as practical strategies, resources, and handouts for the school-based professional. Case examples are used throughout the book to illustrate key concepts and implications for the school setting.

The introduction of highly potent antiretroviral agents has transformed HIV from a disease with a once dismal prognosis to a manageable chronic medical condition. The primary care provider as well as the HIV care provider must focus on aspects of preventive medicine that improve the quality of life and life expectancy of the HIV-infected person. Accurate record-keeping is essential, and examples of HIV primary care flow sheets are presented in this chapter. In addition, tuberculosis screening indications and methods are reviewed. Regular preventative dental and gynecological care should be given. Reviewing the treatment of traditional cardiovascular risk factors with patients will be helpful in educating them and reducing the risk of cardiovascular disease.

Changes in cancer treatment and improved survival rates mean that cancer is often experienced as a chronic condition. This chapter draws on contemporary models of chronic disease management, which define the capabilities required to promote self-management and identify the specific communication practices that achieve optimal outcomes for individuals living with a long-term condition. These capabilities require health professionals to provide person-centred care and achieve individual behavioural as well as organizational/system change. Communication skills which reflect these capabilities in practice include open questions and reflective listening, empathy and sensitivity to patient needs, and sharing of information. Communication skills to support motivational interviewing, collaborative problem identification, and organizational change, including communicating within a multidisciplinary team, are critical to achieving optimal outcomes for people living with cancer. These communication practices enable the patient to be a partner as they adjust to new health challenges, and a changed social and psychological context.

This book explores the theories and practices that help to facilitate self-management of chronic health conditions (also known as chronic impairments or chronic diseases). It consists of four parts, in addition to an introductory chapter and a chapter on defining self-management, for a total of 22 chapters. This book includes discussions about self-management models, psychological interventions, and collaborative care on both individual and systemic levels for the promotion of self-management. Self-management requires that individuals understand the range of symptoms related to their specific chronic impairment, what those symptoms may indicate, and what actions to take to address those symptoms. Healthcare providers are an integral part of providing self-management support (SMS) to these individuals. Self-management includes the micro-decisions that individuals with chronic health conditions make about their conditions and the macro-decisions (e.g., creating treatment plans) that healthcare providers make in collaboration with individuals with chronic health conditions. This book focuses on exploring a range of self-management practices that can empower individuals with chronic health conditions to be less dependent on healthcare systems and, ultimately, to be more in control of their lives.

This chapter considers the organization and delivery of care for osteoarthritis (OA) within the broader context of chronic condition management, and with a specific focus on the current OA literature. It describes factors that have influenced healthcare reforms for people with chronic conditions such as OA, and the development and characteristics of chronic condition models of care. The chapter also examines existing models of care for OA, which are commonly multidisciplinary in nature and situated within primary care, community care, or secondary/tertiary care settings. It summarizes current evidence for the effectiveness and cost-effectiveness of OA models of care, acknowledging that limited data are available regarding access and efficiency outcomes. Barriers to the successful development, implementation, and sustainability of OA models of care are discussed, as well as enablers that could facilitate the success of models of care. The chapter also presents points to consider when planning the implementation and evaluation of models of care, such as the development of a program logic, use of theoretical models or frameworks, and careful selection of appropriate research designs (including mixed-methods approaches). Finally, it considers future challenges for OA models of care, particularly the rise of multi-morbidity among patient populations that will necessitate integrated chronic condition management rather than stand-alone OA services. Future models of care may require the design of specific OA modules that can be integrated with generic chronic condition models that address risk behaviour modification and optimize self-management and mental health outcomes.

Chapter 4 examines the advancing field of mental health and psychosomatic disease from preconception to the postpartum period. The reader is reminded of the normal adaptation of different organ systems to pregnancy. This adaptation affects both physical and emotional functioning, and is further modified by the pregnant woman's social circumstances. The transition to the pathological or diseased condition may follow an exaggeration of the physiological alterations or could occur due to health conditions specific to pregnancy. This may result in manifestations due to mind-body interactions that cause psychosomatic disease. Common and unfamiliar psychosomatic clinical conditions associated with childbearing such as anxiety and mood disorders, eating disorders, hyperemesis gravidarum, and substance misuse are discussed, along with the unfamiliar, such as schizophrenia and seizures. Pregnancy-related acute-on-chronic psychosomatic presentations, besides those arising de novo in labour, are illustrated by vignettes representing real-life encounters. Controversies in management are debated to acquaint the less familiar with these clinical challenges, which require patient-centred care. Promoting health during childbearing not only pertains to the health of the mother, but also to the well-being of her infant. This entails concomitant attention to both in order to enhance the physical, mental and social health of the mother-infant dyad. An urgency for improved understanding of biopsychosocial initiating factors is reflected in an UK surveillance report, `Saving Lives Improving Mother's Care: It confirms the continuing fall in fatalities from 'direct' pregnancy-related physical causes, but a rise due to under-recognition of 'indirect' psychiatric causes that represent the psychosomatic interface.
