Student mistakes in communicating mathematical ideas are still widely practiced. Therefore, it is essential to analyze students' mathematical communication errors in solving mathematical problems so that learning planning can be better. This study aims to describe students' mathematics communication errors in solving the higher-order thinking skill's problems in linear algebra and matrix subject. The type of research is a qualitative descriptive study. They were 155 students as subject research. The data analysis started by collecting students' answers and then grouped them according to mathematics communication skills criteria. Later identified and analyzed the errors made by students of each mathematics communication criteria. The results showed that mathematical communication errors on the indicators of writing mathematical situations were concept errors and principle errors. The declaring idea's mathematical communication error is a concept error, a principle error, and an operation error. Furthermore, mathematical communication errors on the indicator state the results of solving-problem using the language itself is a concept error and operator error.

The Universal Asynchronous Receiver Transmitter (UART) is the very simple and significant sequential communication protocol which is basically utilized for microprocessors & microcontroller systems. It is a shorter range communication protocol, which able to perform half-duplex and full-duplex type of communication at baud rates. Though, UART is a type of shorter range communication still they are not resistant to noisy channel which leads to communication errors by flipping or loosing of bits. These kinds of signal errors are named as forward-errors. The correction of forward errors is a mechanism to handle and rectify those errors (i.e. Burst errors and Random bits error). Thus in this methodology, have introduced a UART-SEC-DED communication module design which utilizes the Hamming encoder and decoders to achieve the forward error correction. Finally, the proposed system will simulated and implemented on FPGA board and experimental outcomes shows the better efficiency in single error correction and detection of double errors.

Communication has always been a key aspect of effective perioperative care. Operating departments continually attempt to improve communication between all parties in the perioperative setting, and articles on this subject are common in professional publications. However it is also true that examples of bad communication abound. In one study which examined communication during 48 operative procedures, a total of 421 communication events produced 129 instances of ‘communication failure’ (Lingard et al 2004). Sometimes these failings have severe consequences on the patient, the trust and the practitioner. Lessons in error management drawn from high risk areas of healthcare such as operating theatre practice have been derived from aviation and aircraft incidents. Frequently, errors will have similar root causes. Psychologist Robert L Helmreich has argued that one of the main parallel areas of error generation is flawed communication. He cites his own studies which show that, in one hospital under review, two thirds of doctors and nurses quoted ‘better communication’ as being the most useful way of reducing errors (Helmreich 2000).

Abstract A fundamental measure of safety communication is the residual error probability, i. e., the probability of undetected errors. For the detection of data errors, typically a Cyclic Redundancy Check (CRC) is applied, and the resulting residual error probability is determined based on the Binary Symmetric Channel (BSC) model. The use of this model had been questioned since several error types cannot be sufficiently described. Especially the increasing introduction of security algorithms into underlying communication layers requires a more adequate channel model. This paper introduces an enhanced model that extends the list of considered data error types by combining the BSC model with a Uniformly Distributed Segments (UDS) model. Although models beyond BSC are applied, the hitherto method of the calculation of the residual error probability can be maintained.

In wireless communication systems reducing bit/frame/symbol error rate is critical. If bit error rates are high then in wireless communication system our aim is to minimize error by employing various coding methods on the data transferred. Various channel coding for error detection and correction helps the communication system designers to reduce the effects of a noisy data transmission channel. In this paper our focus is to study and analysis of the performance of Reed-Solomon code that is used to encode the data stream in digital communication. The performances were evaluated by applying to different phase sift keying (PSK) modulation scheme in Noisy channel. Reed-Solomon codes are one of the best for correcting burst errors and find wide range of applications in digital communications and data storage. Reed-Solomon codes are good coding technique for error correcting, in which redundant information is added to data so that it can be recovered reliably despite errors in transmission or retrieval. The error correction system used is based on a Reed-Solomon code. These codes are also used on satellite and other communications systems.Â

Accuracy is at the core of what journalists do and it amounts to journalistic commitment to report without errors. This tenet of journalism is now in danger, because of the influence of digitalization, changes in media landscapes, and the utilization of the assertation model of journalism. In this study, we used a combination of content analysis and visual network analysis to investigate how subjective errors are disseminated through an online environment, how time/speed influences the propagation of errors, and what the error correction procedures/routines are. The results demonstrate that 69% of the analyzed stories contained errors, and the main cause of such errors was the use of secondary sources, instead of primary ones, these errors transcend national borders and, time/speed had only a minor role in the emergence and correction of the errors, etc. Out of the 107 media websites analyzed, only seventeen provide certain modalities of requesting error correction.

Abstract Coding theory has started with the intention of detection and correction of errors which have occurred during communication. Different types of errors are produced by different types of communication channels and accordingly codes are developed to deal with them. In 2013 Sharma and Gaur introduced a new kind of an error which will be termed “key error”. This paper obtains the lower and upper bounds on the number of parity-check digits required for linear codes capable for detecting such errors. Illustration of such a code is provided. Codes capable of simultaneous detection and correction of such errors have also been considered.

ABSTRACT This paper examines the relationship between audit seniors discussing their own experiences with committing and correcting errors (modeling fallibility), and audit juniors' thinking about errors and error communication (openly discussing their own self-discovered errors). The paper investigates the direct relationship between senior modeling fallibility and juniors' responses, and whether the relationship is mediated through error strain and error-related self-efficacy. Survey data from 266 audit juniors from two Big 4 Canadian accounting firms showed a direct positive association between audit senior modeling fallibility and audit juniors' thinking about errors, and error communication. This relationship is positively mediated through error-related self-efficacy. We also found that the relationship is mediated by error strain. However, although audit senior modeling fallibility was associated with reduced error strain, error strain was positively related to both thinking about errors and error communication, contrary to our hypothesis. The paper discusses the theoretical and practical implications of these results.

We investigated the perception and recognition of errors in a population of practitioners and academics in professional and technical communication. Specifically, we measured 303 participants’ botheration levels of 24 usage errors and then correlated those results against their ability to recognize the errors. Results indicated that practitioners were often more bothered by errors than academics and that participants’ overall botheration level might have fluctuated over the past 40 years. Participants’ botheration level also appeared to associate with their ability to identify error. Finally, we found that participants’ gender, job type, and years working in the field influence their error perception.

We examined the psychological and behavioral consequences of making a communication error in expressive crisis negotiations and instrumental suspect interviews. During crisis negotiation ( n = 133) or suspect interview ( n = 68) training, Dutch police and probation officers received preparation material that led them to make a factual, judgment, or no error. Across both studies, errors increased officers’ negative affect, with errors leading to more stress in crisis negotiations and more distraction in suspect interviews. When comparing factual with judgment errors, factual errors led to more distraction in crisis negotiations and more negative affect in suspect interviews. Analysis of the transcribed dialogues identified four categories of response: apologize, exploration, deflect, and no alignment. Of these, negotiators used all four regularly, whereas interviewers predominantly used exploration and deflect. Our findings revealed the potentially negative effects of errors on officers and offered insights into how they could best focus to induce an appropriate response.

In this work, a method for detecting and correcting errors in Modbus-RTU communications is designed, implemented, and assessed in a smart metering application. This is a low-cost solution for improving communication quality in conventional Modbus-RTU architectures with copper fieldbus. It consists of introducing error detection and correction devices in the network segments most susceptible to errors caused by electromagnetic interference. Experimental validations were conducted and demonstrated the effectiveness of this method for isolating noisy segments in the communication bus, maintaining full compatibility with commercial devices and improving the performance of the entire network.

This qualitative research described students pronunciation error mostly appeared in speaking for general communication class. The participants were the first semester students. This research was descriptive method. It involved describing, analyzing, and interpreting of condition that existed. In collecting the data, the researcher used documentation. Based on the result of data analysis, it could be concluded that students made of three types of pronunciation error. The pronunciation errors were pre systematic error, systematic error, and post systematic error. Furthermore, the students dominant error made was pre systematic error. The errors of students made were caused by three sources of students competence error in learning English as Foreign Language. Communication breakdowns among clinicians, patients, and family members can lead to medical errors, yet effective communication may prevent such mistakes. This investigation examined patients’ and family members’ experiences where they believed communication failures contributed to medical errors or where effective communication prevented a medical error (“close calls”). The study conducted a thematic analysis of open-ended responses to an online survey of patients’ and family members’ past experiences with medical errors or close calls. Of the 93 respondents, 56 (60%) provided stories of medical errors, and the remaining described close calls. Two predominant themes emerged in medical error stories that were attributed to health care providers—information inadequacy (eg, delayed, inaccurate) and not listening to or being dismissive of a patient’s or family member’s concerns. In stories of close calls, a patient’s or family member’s proactive communication (eg, being assertive, persistent) most often “saved the day.” The findings highlight the importance of encouraging active patient/family involvement in a patient’s medical care to prevent errors and of improving systems to provide meaningful information in a timely manner.

Context.— Medical errors are unfortunately common. The US Institute of Medicine proposed guidelines for mitigating and disclosing errors. Implementing these recommendations in pathology will require a better understanding of how errors occur in pathology, the relationship between pathologists and treating clinicians in reducing error, and pathologists' experiences with and attitudes toward disclosure of medical error. Objective.— To understand pathologists' attitudes toward disclosing pathology error to treating clinicians and patients. Design.— We conducted 5 structured focus groups in Washington State and Missouri with 45 pathologists in academic and community practice. Participants were questioned about pathology errors, how clinicians respond to pathology errors, and what roles pathologists should play in error disclosure to patients. Results.— These pathologists believe that neither treating physicians nor patients understand the subtleties and limitations of pathologic diagnoses, which complicates discussions about pathology errors. Pathologists' lack of confidence in communication skills and fear of being misrepresented or misunderstood are major barriers to their participation in disclosure discussions. Pathologists see potential for their future involvement in disclosing error to patients, but at present advocate reliance on treating clinicians to disclose pathology errors to patients. Most group members believed that going forward pathologists should offer to participate more actively in error disclosure to patients. Conclusions.— Pathologists lack confidence in error disclosure communication skills with both treating physicians and patients. Improved communication between pathologists and treating physicians could enhance transparency and promote disclosure of pathology errors. Consensus guidelines for best practices in pathology error disclosure may be useful.

The quality of customer service, especially in realtor service, is centered at customer contact and communication. Because of the inherent diversity in customer demands and the consequent service procedures, the analysis of the service errors or failures in customer communication from the human factors perspective may provide an effective alternative toward the quality improvement in customer services beside the SERVQUAL-based approaches. This study investigates the pattern of communication errors occurred in realtor service operations and the associated latent failures for the implications to personnel training for quality services. Customers with realtor service experiences were interviewed to provide detailed description regarding the communication error(s) encountered while dealt with realtors in the past. Among all types of communication errors in realtor services, it is found that the active errors on providing insufficient or incorrect information regarding the target real estates and the communication errors involved in the price negotiation processes are prominent. The analysis of latent failures further reveals the importance on process transparency and the role of the information systems. For modern C2C service settings like realtors, it is recommended that a double customer framework with the emphasis of support information system should be considered for communication error analysis. Implications and suggestions to personnel training for quality improvement in realtor customer service, in terms of system knowledge and process management, are discussed in details.

AbstractBackgroundMiscommunication amongst providers is a major factor contributing to diagnostic errors. There is a need to explore the current state of communications between clinicians and diagnostic radiologists. We compare and contrast the perceptions, experiences, and other factors that influence communication behaviors about diagnostic errors between clinicians and radiologists.MethodsA survey with questions addressing (1) communication around diagnostic error, (2) types of feedback observed, (3) the manner by which the feedback is reported, and (4) length of time between the discovery of the diagnostic error and disclosing it was created and distributed through two large academic health centers and through listservs of professional societies of radiologists and clinicians.ResultsA total of 240 individuals responded, of whom 58% were clinicians and 42% diagnostic radiologists. Both groups of providers frequently discover diagnostic errors, although radiologists encounter them more frequently. From the qualitative analysis, feedback around diagnostic error included (1) timeliness of error, (2) specificity in description or terminology, (3) collegial in delivery, and (4) of educational value through means such as quality improvement.ConclusionsClinicians and radiologists discover diagnostic errors surrounding the interpretation of radiology images, although radiologists discover them more frequently. There is significant opportunity for improvement in education and practice regarding how radiologists and clinicians communicate as a team and, importantly, how feedback is given when an error is discovered. Educators and clinical leaders should consider designing, implementing, and evaluating strategies for improvement.

At present, the error control method for high-speed serial data transmission obtains the errors by comparison and then controls them. If the data transmission channel is not denoised, the packet loss and error codes become serious, and energy consumption increases. The use of fuzzy classification is proposed to control data transmission errors. The method uses the combination of wavelet transform and transform domain difference to double denoise the channel, and it completes the clustering of data transmission errors by fuzzy classification. Considering packet loss, error codes, and energy consumption in data transmission error control, when the communication distance between two nodes is small, automatic repeat request is used to control data transmission errors. As the distance between nodes increases, forward error correction is used to control data transmission errors. When the communication distance gradually increases, data transmission errors are controlled by hybrid automatic repeat request. Experiments showed that the proposed method can reduce the data transmission error, control energy consumption, packet loss rate, and bit error rate, and enhance the denoising effect.

The purpose of this article is to explore the central bank's ability to management inflation forecast errors in Colombia. We present empirical evidence based on the Colombian experience with data from the period of 2008 to 2020. The communication channel selected for analysis is the press releases. The empirical evidence is divided into three steps: (i) regression analysis using an EGARCH model, (ii) use of VAR models, and (iii) variance decomposition analysis. The communications effects are significant for several months and that close to half of the forecast error variance can be explained by innovations in central bank communication. The results obtained allow monetary policymakers to develop more efficient strategies for anchoring expectations and strengthening the central bank credibility.

This study asks whether grammatical and mechanical errors bother business professionals, which of these types of errors are most bothersome, and whether such errors affect perceptions of the writer and their ethos. We administered a 17-question survey to roughly 100 business professionals whose roles are not primarily writing and communication within organizations. The findings show that business professionals are bothered by these errors and that the level of bothersomeness has increased from previous studies. Additionally, the findings show that participants have clear views of writers who make errors and that the context of the error matters. The authors conclude by offering implications for technical and professional communication.

This study reports intraindividual variations in the semantic and syntactic complexity of language and in the linguistic errors produced by mildly and moderately impaired aphasic and nonneurologically impaired control subjects in different communication contexts. Aphasic patients, compared to control subjects, evidenced as many, if not more, linguistic variations in response to changing communication requirements. In conditions that restricted visual contact between speaker and listener, aphasic patients produced fewer communicative gestures and more complex verbalizations. Verbal complexity and language errors also varied significantly with different contents of communication. Measures of verbal complexity and errors in verbal communications were found to vary independently across different communication contexts, contents, and tasks. These findings demonstrate that despite their linguistic impairments, aphasic patients show appropriate and predictable linguistic changes in response to nonlinguistic social contextual variables.

Today, to use home automation, intelligent home controls or remote controls in the office, electronic equipment is moving away from wireless communication in favor of Power Line Communication (PLC). In the standard PLC solutions, the corrections that result from error transmissions are based on complex digital modulation methods and algorithms for validating the transmitted data without paying attention to the causes of the errors. This article focuses on the implementation of a filtering system for interference and signals in the 120–150 kHz band (CENELEC band C), which is injected into the network by transmitters. Such a filter separates the desired signal from the interference that is occurring in the network, which can result in communication errors. Moreover, when used properly, the filter can be used as a subsystem separation element. The paper presents the requirements, design, construction, simulation and test results that were obtained under actual operating conditions. It is possible to use less complex methods for correcting errors in transmission signals and to guarantee an improvement in the transmission rate using the proposed filter system.

The increasing number of aircraft flying around the world has led to the requirement for air traffic controllers to improve their communication skills to face high demand traffic in the future. The paper examines the communication errors in the pilot-controller communication of six ab-initio air traffic controllers during simulation training. More than three hours of conversation were collected and analyzed qualitatively using conversational analysis. The transcribed data yielded a total of 62 instances of communication errors. The data revealed that clarity and pronunciation of ab-initio controllers contributed to problematic communication and reduced the efficiency of the air traffic controllers in communicating. In contrast, pronunciation errors rarely diminished comprehension amongst the controllers and pilots who share a similar first language and are familiar with the use of English in a lingua franca setting. The study also describes other instances of communication errors in pilot-controller communication. The results indicate that ab-initio air traffic controllers need to be proficient in three main areas in pilot controller communication to improve their performance: aviation phraseology, aviation English, and aviation knowledge. The findings suggest that pilots and air traffic controllers should achieve level 4 (operational) in aviation language proficiency test, before proceeding to aviation training that requires them to be proficient in their language skills.

Hospital readmissions place a heavy burden on healthcare in Canada. Communication errors often occur at vulnerable time points like peridischarge, and these can lead to downstream adverse drug events necessitating rehospitalization. Existing mechanisms to address this problem still have room for improvement. Discharge summaries, for example, may be late, erroneous, or incomplete. Medication reconciliation processes directly integrated with electronic medical records have demonstrated advantages in terms of legibility and timeliness but unfortunately encourage risky behaviours like indiscriminate copy and pasting, leading to new errors of a different sort. A single unified medication list that could be kept on a patient’s person at all times can ensure that medication information is always present at medical appointments and is synchronously updated among all involved practitioners. Near-field communications (NFC) may facilitate the creation and maintenance of such a list. It is a relatively new wireless technology that has advantages over radio-frequency identification and Bluetooth. It allows information to be recorded to and read from objects as small and thin as a sticker. Technologies based on NFC may prove useful for consolidation patient health information into “one source of truth”.

Purpose Outpatients with cancer receive complicated medication regimens in the clinic and home. Medication errors in this setting are not well described. We aimed to determine rates and types of medication errors and systems factors associated with error in outpatients with cancer. Methods We retrospectively reviewed records from visits to three adult and one pediatric oncology clinic in the Southeast, Southwest, Northeast, and Northwest for medication errors using established methods. Two physicians independently judged whether an error occurred (κ = 0.65), identified its severity (κ = 0.76), and listed possible interventions. Results Of 1,262 adult patient visits involving 10,995 medications, 7.1% (n = 90; 95% CI, 5.7% to 8.6%) were associated with a medication error. Of 117 pediatric visits involving 913 medications, 18.8% (n = 22; 95% CI, 12.5% to 26.9%) were associated with a medication error. Among all visits, 64 of the 112 errors had the potential to cause harm, and 15 errors resulted in injury. There was a range in the rates of chemotherapy errors (0.3 to 5.8 per 100 visits) and home medication errors (0 to 14.5 per 100 visits in children) at different sites. Errors most commonly occurred in administration (56%). Administration errors were often due to confusion over two sets of orders, one written at diagnosis and another adjusted dose on the day of administration. Physician reviewers selected improved communication most often to prevent error. Conclusion Medication error rates are high among adult and pediatric outpatients with cancer. Our findings suggest some practical targets for intervention, including improved communication about medication administration in the clinic and home.

Medication administration error is one of medication error that happened due to unfulfillment of drug instruction or drug administration that is different with the recipe. From the initial survey conducted in 2015, there were 13 events medication errors known in 2014-2015 in RSU Haji Surabaya. Based on Kepmenkes RI nomor 129/Menkes/SK/II/2008 about Minimum Service Standards, medication incident should not be any error occurred in the Hospital.This study was an observational descriptive study aimed to identify factors that contribute to the incidence of medication errors. Respondents were 56 nurses that worked on 7 inpatient wards. Variablesi n this research are: the skills of nurses, nurse's knowledge, and communication between nurse and patients. The result showed that the incidence of medication error in 2014-2016 amounted to 14 events that occurred in most of the inpatient ward (57.1%). Most of inpatient ward (57.41%) had good skill of nurses, most of the inpatient wards (57.1%) had a sufficient knowledge and communication of nurses.From this study it can be concluded that the skills of nurses, nurse's knowledge, communication between nurse and patients are contributing to the incidence of medication errors in RSU Haji Surabaya. Cross-Technology Communication (CTC) emerges as a technology to enable direct communication across different wireless technologies. The state of the art on CTC employs physical-level emulation. Due to the protocol incompatibility and the hardware restriction, there are intrinsic emulation errors between the emulated signals and the legitimate signals. Unresolved emulation errors hurt the reliability of CTC and the achievable throughput, but how to improve the reliability of CTC remains a challenging problem. Taking the CTC from WiFi to BLE as an example, this work first presents a comprehensive understanding of the emulation errors. We then propose WEB, a practical CTC approach that can be implemented with commercial devices. The core design of WEB is split encoding: based on the probabilistic distribution of emulation errors, the WiFi sender manipulates its payload to maximize the successful decoding rate at the BLE receiver. We implement WEB and evaluate its performance with extensive experiments. Compared to two existing approaches, WEBee and WIDE, WEB reduces the SER (Symbol Error Rate) by 54.6% and 42.2%, respectively. For the first time in the community, WEB achieves practically effective CTC from WiFi to BLE, with an average throughput of 522.2 Kbps.

Error in clinician–parent communication is not a new issue in pediatrics. It has been the impetus behind national initiatives, namely, family-centered rounds. While family-centered rounds have proven effective, their success is dependent on the family being present during rounds. This does not always occur during prolonged hospitalizations, particularly in the Neonatal Intensive Care Unit (NICU). Current communication methods with parents not present during rounds rely heavily on the provider’s prerogative and ability to multitask. Thus, errors in communication are commonplace and are largely accepted as inevitable. For the sake of the parents of a patient in the NICU, a high-fidelity communication system is urgently needed. NICUs must move beyond the telephone and use modern innovations in communication technology.

AbstractTimely and accurate communication is essential to safe and effective health care. Despite increased awareness over the past decade of the frequency of medical errors and greater efforts directed towards improving patient safety, patient harm due to communication breakdowns remains a significant problem. Communication problems related to diagnostic testing may account for nearly half of all errors made by typical primary care physicians in their medical practices. This article provides an overview of communication breakdowns in the context of radiology related diagnostic errors. In radiology, communication breakdowns between radiologists, referring clinicians, and patients can lead to failure of critical information to be relayed, resulting in delayed or missed diagnosis. New technologies, such electronic health records (EHRs), contribute to the increasing complexity of communication in health care, but if used correctly, they can provide several benefits to safe and effective communication. To address the complexity of communication breakdowns, a multifaceted sociotechnical approach is needed to address both technical and non-technical aspects of health care delivery. The article also provides some future directions in reducing communication breakdowns related to diagnostic testing, including proactive risk assessment of communication practices using recently released SAFER self-assessment guides.

Passive voice is one of the important English grammar that should be mastered because it is used at formal and informal communication. The EFL students tend to produce grammatical errors in transforming active to the passive sentence because of syntactical interference of their first language. The objectives of this research are to know the types of errors that students made in transforming active to passive sentences, to know the students' problems in transforming active to passive sentences and to know the proportion of the students' errors. The subject of this research is 20 English students at one private university in Lampung. The result of this research shows that the total number of errors that had been committed by the students based on surface strategy taxonomy is 280 errors, which falls into the following proportion of the four kinds of errors in surface strategy taxonomy: 126 errors of omission error, 18 errors of addition error, 120 errors of misformation error, and 16 errors of misordering error.

The error of term usage in communication can be caused due to the misunderstanding of the words' mean-ing, or even the users might even realize their error but somehow keep using it deliberately. Thus, this study was conducted to find errors in using scientific terms in communication that influence the scientific communication of prospective science teachers. The research design used was mixed methods with sequen-tial explanatory strategies. The data were gathered through a closed questionnaire referring to the optional Likert Scale of 5 options and analyzed quantitatively. There were 256 participants or prospective science teachers as research subjects. The mean of the r count was 0.64, and the r table was 0.12. Therefore, it was judged as valid. The reliability test was obtained by Cronbach's Alpha of 0.951 0.80. The results show that there were 13 errors of scientific terms performed by participants. Wilcoxon test obtained the Asymp value. Sig. of 0,000 0.005 meaning that there were differences in the use of the term science in society and university. Representing and presenting were two aspects of scientific communication, with 40% obtain-ing moderate criteria. The research concludes that the scientific terms' error affects prospective teachers' sci-entific communication and progressive education since the errors have become a long habit. It must be over-come because the habit scientifically will also influence the trust of scientific truth.
