Diagnosis and treatment of acute stroke has advanced considerably in the past 2 decades. Most notably, in cases of ischemic stroke, intravenous alteplase has become the standard of medical treatment despite its multiple contraindications and limited time window. More recently, trials have proven that endovascular thrombectomy is superior to medical therapy alone, advancing the standard of care for patients who present with acute ischemic stroke from a large vessel occlusion and salvageable brain tissue. The treatment of hemorrhagic stroke now involves the use of novel pharmacological agents and advanced minimally invasive technology. Important changes have also occurred at the levels of hospital organization and treatment decision-making. Such changes in organization and designation of hospitals with distinct levels of stroke care and the variety of stroke protocols now requires team work of emergency medical services (EMS), Emergency Department, stroke neurologists, neurosurgeons, and neurointerventionalists. This book provides an overview of the modern medical and surgical options for the treatment of patients with acute ischemic and hemorrhagic strokes. The pivotal role of EMS in prehospital evaluation and triage of a stroke patient and the levels of stroke systems of care are discussed. In addition, the current guidelines on the management of acute stroke, with the focus on early care of acute stroke patients at the Emergency Department and the first 24 hours of hospital admission, are reviewed. Each chapter contains a discussion of common clinical scenarios including initial management steps, practical points, and common pitfalls.

Management has become more demanding and difficult considering the fierce competition, increased complexity, and diversity of services. Hospitals and clinics face increased demand for their services along with pressure to contain costs. This chapter suggests that the solution is the implementation of novel managerial approaches to identify and improve relevant value drivers. These innovative approaches include the theory of constraints (TOC), Lean, and Six Sigma. These managerial approaches are based on common sense, have evolved from field practice, and negate the input–output myth (“To do more, you more resources”). Instead, these approaches emphasize “doing more with what you have.”

The use of coercion is one of the defining issues of mental health care and has been intensely controversial since the very earliest attempts to contain and treat the mentally ill. The balance between respecting autonomy and ensuring that those who most need treatment and support are provided with it has never been finer, with the ‘move into the community’ in many high-income countries over the last 50 years and the development of community services. The vast majority of patients worldwide now receive mental health care outside hospital, and this trend is increasing. New models of community care, such as assertive community treatment (ACT), have evolved as a result and there are widespread provisions for compulsory treatment in the community in the form of community treatment orders. These legal mechanisms now exist in over 75 jurisdictions worldwide. Many people using community services feel coerced, but at the same time intensive forms of treatment such as ACT, which arguably add pressure to patients to engage in treatment, have been associated with improved outcome. This volume draws together current knowledge about coercive practices worldwide, both those founded in law and those ‘informal’ processes whose coerciveness remains contested. Charles Bell (1774–1842) was a Scottish anatomist–surgeon whose original ideas on the nervous system have been equated with those of William Harvey on the circulation. He suggested that the anterior and posterior nerve roots have different functions, and based on their connectivity he showed that different parts of the brain have different functions. He noted that individual peripheral nerves actually contain nerve fibers with different functions, that nerves conduct only in one direction, that sense organs are specialized to receive only one form of sensory stimulus, and that there is a sixth (muscle) sense. In addition to the facial palsy and its associated features named after him, he provided the first clinical descriptions of several neurological disorders and important insights into referred pain and reciprocal inhibition. Bell helped to change the way art students are taught, described the anatomical basis of facial expressions, initiated the scientific study of the physical expression of emotions, and stimulated the later work of Charles Darwin on facial expressions. His teachings influenced British and European art. Bell was a renowned medical teacher who founded his own medical school, subsequently took over the famous Hunterian school, and eventually helped establish the University of London and the Middlesex Hospital Medical School in London. However, his belief in intelligent design caused him to be left behind by the evolutionist thought that developed in the nineteenth century. He was a brilliant but flawed human being who contributed much to the advance of knowledge.

War often appears to be definitionally outside the realm of structures such as law and literature. When we speak of war, we often understand it as incapable of being rendered into rules or words. Lawyers struggle to fit the horrors of the battlefield, the torture chamber, or the makeshift hospital filled with wounded and dying civilians into the framework of legible rules and shared understandings that law assumes and demands. In the West’s centuries-long effort to construct a formal law of war, the imperative has been to acknowledge the inhumanity of war while resisting the conclusion that it need therefore be without law. Writers, in contrast, seek to find the human within war—an individual story, perhaps even a moment of comprehension. Law and literature might in this way be said to share imperialist tendencies where war is concerned: toward extending their dominion to contain what might be uncontainable. This volume on war is the sixth in the University of Chicago Law School’s Law and Literature series. The papers are intended to address the many ways in which war affects human society and the many groups of people whose lives are affected by war. Some of the papers concern the lives of soldiers; others focus on civilians living in war zones who are caught up in the conflict; still others address themselves to the home front, far from the theater of war. By collecting such diverse perspectives within one volume, we hope to examine how literature has reflected the totalizing nature of war and the ways in which it distorts law across domains.
