There are four main components in this thesis: a literature review of program evaluation, a description and discussion of the current status of program evaluation in the crisis intervention literature, results and discussion of the formative evaluation which is the primary element of the thesis, and a report on the use of the Goal Attainment Follow-up Guide (GAFG) (Kiresuk & Sherman, 1968) and the Brief Derogatis Psychiatric Rating Scale (B-DPRS) (Derogatis, 1978) for community-based mobile crisis intervention programs. The data for the evaluation were gathered using both quantative and qualitative methods. There were 150 participants in the study: 89 females and 61 males. The mean age was 35. The GAFG was completed by 81 of the participants; 33 of the participants were administred the B-DPRS. There were three major findings in this evaluation. the participants contacted significantly more community agencies and spent less time in hospital after using the crisis program and the GAFG and B-DPRS were found to be unsuitable as outcome instruments for a community-based mobile crisis program.

Background Persons with severe mental illness (SMI) are vulnerable to mental health (MH) crises in the community. These crises often resulted in costly hospital admissions. Crisis Resolution Team (CRT) was implemented based on limited empirical evidence to address this problem. In 2001, UK implemented a national policy making CRT mandatory nationwide. Since then, more studies were performed on the effectiveness of CRT. It appeared CRT is cost effective in reduction of admissions. There was evidence of positive clinical outcome in comparison with standard care. Hong Kong does not have a specific community crisis program for people with SMI. It is desirable for policy makers to consider alternatives to reduce hospital admissions and to strengthen community support for people with SMI. Aims To evaluate the feasibility on the implementation of CRT Model in Hong Kong as a service to reduce hospital admissions. Method A review of current evidence on the effectiveness of CRT model and assess the feasibility and desirability of such model for implementation in Hong Kong. Results Evidence from observational studies and randomized controlled trials show CRT to be effective in the reduction of hospital admissions and inpatient days in the UK. CRT is also associated with higher acceptance from patients and family members than from standard care. CRT appears to be feasible against other service to address the acute needs of persons with SMI in reducing hospitalization. Conclusion A preliminary assessment ruled out a number of alternatives in dealing with mental health crises in the community. Preliminary results suggested the CRT model might be a feasible and desirable solution. There is evidence on the effectiveness of such teams in the UK. Successful implementation of CRT service in Hong Kong will be a challenge for stakeholders of the mental health system.

ABSTRACT This dissertation has the following specific aim: to measure outcomes of social work emergency certificates produced by a Mobile Crisis Team to determine the effectiveness of these certificates at linking clients to services. Linkage to services is a programmatic goal and is achieved by ensuring clients receive adequate crisis and follow up services as a result of Emergency Certificates. In doing so, risks of homicide, suicide and grave disability are mitigated. Under its administrative umbrella, Mobile Crisis Team operates a police-social work collaboration known as Crisis Intervention Team, and both are administered by the State of Connecticut Department of Mental Health and Addiction Services in southeastern Connecticut. The data comes from 233 consecutive cases that involved Emergency Certificate's to transport clients to the local emergency department for psychiatric evaluation and treatment. Data was gathered post-hoc by reviewing copies of each specific certificate and cross-referencing those certificates with an agency risk management report and an electronic data base that stores demographic data on all clients. Descriptive, bivariate and multivariable analysis, such as cross tabulations and binary logistic regression, were used to analyze the data in this study. Additionally, the chi-square automatic interaction detector (CHAID) was used to construct outcome trees to describe subgroups of interest. This research is a continuation of previously published research on the various permutations and outcomes of mobile crisis programs, and contributes two unique programmatic features: the use by social workers of a legally proscribed coercive tool to send people to the emergency department for assessment, and the effectiveness of police-social work collaborations compared to social work only referrals. Overall, linkage occurs in over 80% of cases, with significant predictors of linkage identified as substance abuse, Hispanic ethnicity, criminal justice involvement, suicide, affective disorders, and insurance status. Implications for program evaluation, future research and limitations of the study are also discussed

With the increasing amount of police calls involving persons experiencing a mental health crisis (PICs), agencies are looking for ways to reduce the overuse of emergency services and criminal confinement. Police-mental health collaborative (PMHC) programs were developed to utilize the expertise of both mental health and law enforcement practitioners to provide immediate linkage to psychiatric services in an effort to prevent unnecessary involvement in the criminal justice system. The theoretical framework for this study was built on the sequential intercept model (SIM) along with the theories of social network and social support. The SIM identifies 5 key points where PICs can be diverted away from the criminal justice system. PMHC programs fall within the first intercept where persons with mental illness can be diverted at their first initial contact with law enforcement. Limited empirical research exists that show PMHC programs are reaching their intended objectives. The purpose of this quantitative study was to determine the effect of PMHC services on the likelihood that PICs will have future mental health calls (MHCs), arrests, and emergency hospitalizations (EHPs). Archival data from 1 midwestern police agency and online public court records was used in the analysis. The study employed OLS and logistic regression techniques, which revealed no statistically significant relationships between the PMHC interventions and the likelihood of future MHCs, arrests, and EHPs. However, significance was achieved for several covariates including transient status, prior history of MHCs, arrests, and EHPs. These findings will contribute to positive social change by informing policymakers and practitioners on best practices in community mental health crisis response.

The Internet is becoming a more realistic means of accessing support, especially for those who are unable to or are uncomfortable with accessing supports in person. One population that experiences barriers to accessing face-to-face support is youth, and online counselling is an option available for youth to overcome these barriers (King, Bambling, Lloyd, et al., 2006). Online counselling can occur through e-mail and chat services; however, due to the text-based nature of these services, their effectiveness is criticized for having a lack of verbal and nonverbal communication (Cook & Doyle, 2002). Research, however, has suggested that it is possibly this lack of verbal and nonverbal cues that make online counselling effective. Research has also suggested that the effectiveness of counselling can be measured by the working alliance (Fletcher-Tomenius & Vossler, 2009; Hanley, 2009). This study explored if an effective working alliance can be established in online counselling mediums with youth who are in crisis. A quantitative research approach was taken, in which 91 youth completed an online survey that asked how they felt towards online support services. The results of this study suggest that youth respond positively to the lack of verbal and nonverbal cues; youth appear to enjoy an increase sense of anonymity that is created within online support services, which can contribute to open communication. In addition, the results suggest that an effective working alliance is established in online counselling services, as it was found that youth typical felt happy and satisfied with the services they received.

The Crisis Intervention Team (CIT) training program is a community partnership that provides education, understanding, interventions, relationships, and protection for responding officers, individuals in crisis and the community. The purpose of this study is to assess the effectiveness of CIT training amongst law enforcement officers in reducing stigmatizing attitudes and opinions on mental illnesses. The study uses a dependent means t-test to test the study participant’s attitudes and opinions on mental illnesses using a pre/post-test survey. A modified version of the Attitudes to Severe Mental Illness Scale was used to measure attitudes, beliefs, and opinions of mental illness. This modified version was developed based on CIT course material. The ultimate goal of this study was to gain insight within the law enforcement community as to whether or not CIT training is raising awareness on the issue of mental health. Findings from this study suggest that after participation, those in the CIT training reported lower levels of non-stigmatizing opinions and attitudes in two of the four factors being analyzed.

The social problem of intimate partner violence affects approximately one-half to two million individuals each year in the United States (Catalano, 2007; Tjaden & Thoennes, 2000). Commonly the criminal justice system mandates completion of a group-based intervention intended to prevent violent behavior (Dalton, 2007). These groups are typically referred to as a batterer intervention program (BIP). Despite the popularity of this intervention approach, research findings examining the efficacy of these programs remain inconsistent (Babcock, Green & Robie, 2004). Nonetheless, 45 U.S. states including the District of Columbia, have implemented standards that aim to proscribe and regulate elements of program functioning. To gain insight regarding the effects that standards implemented in the state of Oregon in 2006 have had on the functioning and characteristics of BIPs, this study examined survey data collected in 2001, 2004, and 2008 from a total of 76 BIPs functioning in Oregon. Several hypotheses were tested. First, it was hypothesized that program compliance with state standards would increase from 2001 to 2004 and from 2004 to 2008. Overall compliance did increase, though this change was not statistically significant. Consistent with this hypothesis, a statistically significant increase in one component of compliance, program length, was found between 2004 and 2008. Additionally, some components, such as collaboration with community partners, did not change in the expected direction. Second, the analyses tested whether programs that began functioning after the creation of the standards in 2006 would be more compliant with the standards than those operating prior to 2006. This was not the case; there was not a significant difference in the compliance ratios for programs that began functioning before and after 2006. Third, it was hypothesized that program characteristics of program size, location, and barriers to compliance would predict program compliance. This hypothesis was not supported; program size, location and barriers did not predict program compliance. These results indicate that some portions of the standards are being met by programs regardless of their program characteristics, while other components are not. Understanding which components of state standards programs are and are not in compliance with provides valuable insight into which components of standards may be difficult for programs to adhere. This information is important for understanding how programs may need assistance to comply with specific components and whether enforcement or formal monitoring of programs is necessary.

The purpose of this study is to develop a theoretical framework for social workers in children’s homes that may be used for play techniques during social casework intervention with a children’s home child. The influence that the placement in a children’s home has on the child, creates the context for this study. Specific behaviour- and emotional problems that exist within the child were identified and play techniques for the solving of these problems was described. The research was done based on an extensive literature study, which focused on the role and function of a children’s home, the needs, behaviour- and emotional problems of the children’s home child and the usage of a practice framework and play techniques by social workers. A combined qualitative and quantitative research method and an explorative and describing research design have been used in this study, since this combination resulted in reaching the goal of the study. The empirical research investigated the usage of play techniques by social workers during social casework intervention with a children’s home child. The overall sample consisted of the 23 children’s homes in the Cape Metropole where currently 31 social workers are employed. Semistructured questionnaires were used as an interview instrument with an availability test sample consisting of 18 social workers. In light of the findings derived from the literature study and empirical research, appropriate conclusions and related recommendations were made. The main conclusion of the study is that the participants use play techniques randomly and not in conjunction with a practice framework. The main recommendation of this study is that social workers should use play techniques during social casework intervention, based on a specific practice framework in order to assure responsiveness. Continuous education in this regard is the responsibility of each social worker working in a children’s home.

Child maltreatment negatively impacts physical, emotional, and the health and well-being of a person, often with lifelong implications. The importance of this study derives from the very necessity of mental health services for children who have suffered trauma. Children’s Assessment Centers (CAC) assess and refer children who have experienced abuse to a therapist in the community. This study examines the use of an enhanced referral process used to connect some CAC clients more quickly to therapeutic services in the community. The study examined whether children who received this expedited referral service were more likely than children who received the standard referral process to see a therapist within three months. The study used data provided by one CAC in California. The CAC provided information on clients who received either the expedited or standard referral process within a 3-month period in 2017. The researcher then contacted the clients’ caregivers to ask whether the child saw a therapist within 3-months of the child’s referral from the CAC. The study used a Chi-square test to analyze whether children who received an expedited referral to mental health services were more likely than children who received the standard referral to receive mental health services within three months. Results indicated no statistically significant difference in access to mental health services for the two groups. However, during data collection, the researcher learned that some children were already engaged in therapy at the time they received the referral, perhaps rendering the referral unnecessary. Implications for social work practice, policy, and research are discussed.

The criminal justice and mental health systems increasingly overlap as persons with mental illness (PMI) are disproportionately present throughout components of the criminal justice system, a concern to mental health and criminal justice professionals alike. In response, various initiatives (aimed across components of the criminal justice system) have been developed and implemented as a means of combating this overrepresentation. The following research will focus on one specialized police-based initiative, the Crisis Intervention Team (CIT), which aims to train police how to recognize mental illness, de-escalate persons in crisis, and to seek treatment-based alternatives to arrest, when appropriate (Schwarzfeld, Reuland, & Plotkin, 2008). Alternatives to arrest consist of various community-based mental health services such as public hospitals (some of which are designated as emergency receiving facilities, or ERFs) or private clinics. While the components of CIT training likely influence officers in unique ways, research has yet to empirically examine how CIT influences police perception, behavior or the incidence of referrals to mental health treatment. As an initial step, this research assessed the attitudes police have regarding the hospital and mental health system within their district. Specifically, this research provides a basic understanding of how police regard their local hospitals and mental health facilities that are posited as available alternatives to arrest, and help identify the role CIT plays in shaping these attitudes. This study found almost no significant difference in the attitudes CIT-trained officers had towards district ERF and the local mental health system as compared to non-CIT officers. Only in one of the six departments studied was there a significant difference between the attitudes of CIT-officers and non-CIT officers; with the non-CIT officers actually having more positive attitudes about their local mental health system than CIT-officers. The six departments studied had nearly similar attitudes of their mental health resources, which would barely be considered passing on a standard grading scale. While officers in this study do not have very positive attitudes towards the ERF they use to transport PMI or their districts’ mental health system, these attitudes may in fact be more positive than many police departments without any specialized approach or initiative. The significance and policy implications of these attitudes are discussed at length, as these findings speak to the need for increased attention by both the mental health and criminal justice systems. Recommendations for future research, including expanding this study to rural departments or agencies with no connection to CIT, are also outlined.

Older people are at an increased risk of experiencing harmful effects from alcohol, particularly in conjunction with physical and mental health difficulties. Brief Alcohol Interventions (BI) aim to raise awareness of potential difficulties and enhance motivation to change drinking behaviour. There is a robust evidence base for using BI with adults drinking at hazardous/harmful levels. However, limited attention has been paid to how alcohol screening and BI can apply to older adult populations. The systematic review examined the literature investigating the effectiveness of using BI with older adults in primary care and the quality of the evidence evaluated. Although variation in the delivery of the BI and sampled populations was evident, evidence suggested that BI can be effective in reducing alcohol consumption for older adults, but less effective for those drinking at heavier levels. However, the literature was found to be limited in quality and number and using BI with older people or within secondary care remains under researched. A feasibility study of using alcohol screening and BI in mental health services for older people was developed. Community Psychiatric Nurses’ (CPNs) experiences of trialling the BI and attitudes towards addressing alcohol use with older people were explored through qualitative interviews, along with perceived barriers and facilitators for implementation. Challenges in undertaking research with older adults were highlighted and no hazardous drinkers identified to complete the BI. An overarching theme of anxiety about addressing alcohol and a lack of confidence in being able to influence the drinking behaviour of older people were identified. Older people had little knowledge about alcohol and its potential risks and differences emerged as to whether CPNs felt it their responsibility to address this. Several barriers to implementation were identified and the results indicated that offering BI within mental health services for older people was not feasible. For implementation to become successful, training and ongoing support is essential; to highlight the risks of alcohol for older people and the role CPNs can play. Further clinical implications and areas of future research are discussed.

Policy and professional guidance in England have emphasised the importance of Therapeutic Relationships (TRs) in community mental health care, yet there is no comprehensive model to guide practice or understand the process through which stronger TRs are generated. This thesis investigated TRs in community mental health for individuals with psychotic disorders and was embedded within the CRIMSON trial: a randomised controlled trial of the Joint Crisis Plan (JCP) intervention. JCPs contain service users’ treatment preferences for future care, which are jointly decided with clinicians. Qualitative analyses addressed participants’ views of TRs and JCPs. Quantitative analyses addressed the predictive utility of TRs for outcomes, associations of TRs and the effect of JCPs on service user and clinician-rated TR. Results suggested that significant ambiguity persists regarding what can and should be provided in community mental health, resulting in unhelpful experiences for service users and clinicians. A new model of TRs - Consistent Respect - was developed; it presents TRs as bi¬directional processes, jointly affected by clinicians’ and service users’ experiences of interactions and their roles defined by the wider context; the latter often being a barrier to the development of strong TRs. JCPs significantly improved service users’ appraisals of TRs by providing a structured protocol through which routine role enactments were limited and clinicians could demonstrate Consistent Respect. Positive effects were lost when there were deficiencies in the implementation of JCPs and/or engagement of clinicians. In conclusion, improving TRs through JCPs could facilitate better outcomes and more satisfactory treatment experiences for service users and clinicians. Changes in policy and practice are indicated to facilitate transparent goals and roles for clinicians and respectful interactions with service users.

Objectives: This study sought to determine whether a computerised cognitive bias modification programme could be effective within a waiting-room setting for parents accompanying their children to CAMHS appointments. The primary objectives were to determine whether detectable changes to participants' emotion recognition could be observed in this setting, and whether this approach would be acceptable to the population. Secondary measures investigated whether the programme would lead to changes in participants' affect or changes in parents' appraisals of difficulties with children. Methods: A computerised emotion recognition training task was delivered to all participants during four weekly sessions. Participants in the experimental condition (n=17) received feedback aiming to shift their detection of positive facial emotions, while those in the control condition (n=14) received feedback which was not designed to elicit any shift in emotion detection. Results: Positive shifts in emotion recognition were observed in the experimental group, although no changes were observed in secondary measures in either control or experimental groups. Qualitative data indicated that the programme was acceptable and appropriately constructed. Conclusion: This study demonstrates that cognitive bias modification is possible within a waiting-room setting, although the extent to which this can lead to clinically significant improvements in mood or relationships remains uncertain. This work has implications for emotion recognition interventions for clinical populations known to present with negative emotional biases (e.g. anxiety and depression) and represents an important first research step towards developing interventions to improve parent-child relationships.

The purpose of this qualitative study was to describe the ways in which Orìsà priests and their clients conceptualize issues and concerns described by Western based approaches as mental health problems. The two research questions guiding this inquiry included: (a) how do Orìsà priests and their clients conceptualize issues and concerns associated with mental health problems in Western psychology and (b) what methods and techniques do Orìsà priests and their clients use to address issues or concerns associated with mental health problems in Western psychology? This study was grounded in African-centered theory by providing a cultural lens to guide the research design, data collection, and analysis. Data were collected during semi-structured individual interviews with four Orisa priests in a three phase model, for a total of 12 interviews. The study also included three focus groups (six informants per group), who did not participate in the individual interviews. Each group met for two sessions, for a total of six focus groups. Bracketing of assumptions by research team members and use of a reflexive journal was used to ensure credibility and dependability of the data (Creswell, 1998). Data analysis consisted of a recursive process divided into multiple steps, to help strengthen methodological rigor and verification of study procedures. The three part process included codebook development, code application, and data analysis. Three major themes emerged from the data. The first theme, The Conceptualization of Mental Health Problems as Spiritual Matters included one subtheme, Transgenerational Transmission. The second theme, Origins of Mental Health Problems, included three subthemes (Western Socialization, Spiritual Forces, and Ifa as a Healing System). The final theme, Addressing Mental Health Problems, included three subthemes (The Divination Process, Referrals, and Western Therapy). Results of this study indicate that respondents primarily conceptualize mental health problems as spiritual matters and seek to address these problems with the help of an Orìsà priest. In addition, some respondents sought the services of a Western trained therapist for the same issue. Practice and research implications are discussed.

The aim of the proposed study is to evaluate demographic factors and treatment characteristics contained in the historical records of those treated as in-patients at the Sonstraal Adolescent unit of Lentegeur Hospital, during the period 1986 to 1990. This demographic study details the following aspects of the adolescent in-patients: size, that is, numbers of those admitted to the unit; composition, including age, sex and area. Treatment characteristics such as reasons for admission, diagnosis of psychopathology, referral agent on admission and discharge and length of stay in the unit are considered. The data for the study have been extracted from the clinical records contained at Sonstraal, namely , the 'Clinical Summary on Discharge' form. This form is completed by the therapist of each adolescent attending the unit. The EpiInfo computer programmes have been utilised to create a database and to select the appropriate procedures and statistics which form the basis for data analysis and interpretation. Data interpretation includes an analysis of the emerging trends and details the implications for policy issues, unit staffing and treatment options. Analysis of the trends and comparisons with literature findings have facilitated the generation of hypotheses which could be tested in future studies. This study therefore provides a working document for future prioritising and planning of in-patient, out-patient and community mental health services to adolescents, their families and communities. This involves recommendations for intervention and community involvement. In addition, the study provides a basis for future research into adolescent mental health care.

Background – People with functional impairments have unmet needs and they are not given the support and service they are entitled to. According to international studies, early measures and treatment may slow down the outbreak of mental illness and relieve its course. Aims - To elucidate and compare both somatic poor health and social needs of people with either physical or mental functional impairments in a rural district, and to explore and compare how different personnel in the primary health care service and psychiatric services are able to detect early signs of psychosis. Moreover, to find out how early signs of psychosis are detected in primary health services, and to explore the patients’ pathways to the GPs. Methods – In studies I and II, people with severe functional impairments were offered a screening health examination followed by an interview. Three vignettes were presented to personnel in the primary health care service and the psychiatric services in study III. The participants were asked to detect any signs and symptoms of psychosis in the vignettes. In studies IV and V, notes in primary health care records were studied during a period of two years and six months, respectively, before a diagnosis of psychosis was made by the general psychiatric services. Results – People with severe functional impairments had poorer health and more problems with their ADL (Activities of Daily Life), economy/work and Quality of Life than people in general. Among the groups studied, people with mental impairments had the poorest living conditions. There were no differences between the participants in study III regarding sex, age and occupation; and the participants detected the signs and symptoms in the vignettes to a high degree (75% of all signs and symptoms). In all, 152 patients (22 with schizophrenia/ schizoaffective disorders, 41 with schizophrenia preceded by other psychotic disorders and 89 with persisting psychiatric disorders) with the diagnosis of psychosis made by the general psychiatric services were included in study IV and V. There were notes in 77% of the primary health records during the two-year study period, and 70% of these notes were about psychiatric signs and symptoms, which means that the GPs detected signs and symptoms of psychosis in 2/3 of the cases. The analysis of the patients’ visiting patterns to GPs showed that many patients did not visit their “own” primary health care centre or their “own” GP. Furthermore, many patients had no contact with the primary health care service at all, and the subgroup with schizophrenia/schizoaffective disorders visited the primary health care service less frequently than the other groups. Main conclusion – People with severe functional impairments must be granted regular contacts with a GP, whose role must be: to identify and motivate the patients; to detect when there are needs for care and social needs; to function as a representative for the patients; to inform the patients about their rights and to guide them to other social or health authorities. The GPs detected early signs and symptoms of an emerging psychosis to a high degree, which would make early intervention possible. The more visits to the GPs, the more symptoms were detected, and out of all signs and symptoms with psychiatric content noted, the GPs would have suspected an emerging psychosis in almost every second patient who visited them. To detect early signs and symptoms of psychosis is difficult, and whenever in doubt, primary health care personnel must be able to consult psychiatric professionals. Otherwise we may miss the opportunity to intervene in an early phase of the illness. Additional training could also mean better understanding and earlier detection of people at risk of an emerging psychosis.

Este estudo buscou resgatar a vivencia dos familiares e sujeitos em sofrimento psÃquico durante situaÃÃes de crise. Historicizando esses fatos foi possÃvel retratar um contexto de vida que se ramifica de diversas formas para alcanÃar um atendimento digno e resolutivo. Trata-se de estudo qualitativo que utilizou a HistÃria Oral TemÃtica como mÃtodo de apreensÃo das narrativas. As narrativas estÃo apresentadas na Ãntegra, com textualizaÃÃo do autor, cujo objetivo foi manter a singularidade e especificidade contidas no discurso. Como narradores, tivemos setes sujeitos, seis mulheres e um homem, estes foram identificados de diversas formas, tendo como ponto zero o Sistema de Atendimento MÃdico de UrgÃncia (SAMU). Para desvelar as ideias contidas nas narrativas, utilizei o referencial metodolÃgico de Paul Ricoeur que considera a compreensÃo como ordenaÃÃo do enunciado narrativo e quando esta estrutura organizacional nÃo se estabelece, nÃo à possÃvel compreender a narrativa, requer uma explicaÃÃo. Objetivando alcanÃar esta explicaÃÃo, optamos por utilizar aspectos codificados, explicaÃÃo causal do sofrimento mental a partir da visÃo dos sujeitos do estudo do reconhecimento da crise a busca por apoio terapÃutico e seus descaminhos, a rede de atenÃÃo em saÃde mental como recurso de apoio buscado pelos sujeitos, o cuidado e suas dimensÃes: perspectiva da famÃlia e da rede, o porvir na visÃo do familiar e do sujeito em crise. As narrativas revelaram sofrimento psÃquico marcado por multicausalidades, potencializado pelo meio ao qual o sujeito encontra-se inserido. A ausÃncia de rede de assistÃncia emergencial Ãs situaÃÃes de crise foi registrada em todas narrativas coletadas, tendo como foco principal a carÃncia de estruturas de retaguarda aos serviÃos substitutivos, o que gera sofrimento psÃquico em familiares, sujeito e vizinhos. Marcaram, tambÃm, os relatos excessivos de violÃncia praticados pelo sujeito em situaÃÃes de crise. A terapÃutica com ervas ganhou notÃria importÃncia nos relatos dos familiares, seguido pelos atos espirituais, medidas buscadas para suprir a ausÃncia da assistÃncia adequada. A partir deste contexto, o cuidado ganha relevante importÃncia no compreender as situaÃÃes de crise instaladas, uma vez que à possÃvel encontrar diversas formas de tratamentos, inclusive mÃtodos arcaicos à segregaÃÃo prisional. Por fim, encontrei nas narrativas a esperanÃa de cura ou tratamento que permita aos familiares e sujeitos retornarem a uma vida saudÃvel e normal. Evidenciei no estudo relatos peculiares da realidade vivenciada no contexto das situaÃÃes de crises psÃquicas, como a nÃo formaÃÃo da trÃade de cuidado sujeito, famÃlia e rede de atenÃÃo, que a despeito de a famÃlia perceber que o hospital psiquiÃtrico nÃo à a estrutura assistencial desejada, lanÃa mÃo deste por ser o Ãnico recurso existente e que à preciso rever o suporte de retaguarda dos novos serviÃos instalados pela Reforma PsiquiÃtrica.
This study ought to redeem experiences of families and individuals in psychological distress during crisis situations. Historicizing these facts, it was possible to portray a life context that branches in various ways to achieve a dignified and decisive care. It is a qualitative study that used thematic oral history as a method of seizure of narratives. The narratives are presented in full, with textualization of the author, whose objective was to maintain the uniqueness and specificity contained in the speech. As storytellers, we had seven subjects, six women and one man, these were identified in several ways, with the Zero point the System of Emergency Medical Service (SAMU). To disclose the ideas contained in the narratives, I used Paul Ricoeurâs methodological referential, that consider understanding as a ordination of the narrative enunciation and when this organizational structure is not established , it is not possible to understand the narrative, requires an explanation. In order to achieve this explanation, I chose to use aspects encoded as therapeutic measures and their diversity; care and its dimensions in the family perspective in relation to the health care network, the future vision of the family and the individual in crisis. The narratives revealed psychological distress marked by multi-causalities, enhanced through which the subject is inserted. The absence of network emergency assistance to crisis situations was recorded in all narratives collected, focusing mainly on the lack of structures to rear services, which generates psychological distress in relatives, individuals and neighbors. Marked also the reports of excessive violence by the subject in crisis situations. Therapy with herbs gained remarkable importance in the accounts of family members, followed by spiritual acts, measures sought to address the lack of adequate assistance. From this context, care gain relevant importance in understanding the crisis installed, since it is possible to find different forms of treatments, including archaic methods to segregational prison. Finally, I found in the narratives hope of cure or treatment that enables individuals and families return to a healthy and normal life. Found in the study reports the peculiar reality experienced in the context of psychic crisis situations, such as non-formation of the triad of subject care, and family care network, which in spite of the family realize that the psychiatric hospital care structure is not desired, makes use of this because it is the only resource available and it is necessary to review the support behind the new services installed by the Psychiatric Reform.

Incorporation of a mental health focus into Early Intervention/Early Childhood Special Education (EI/ECSE) services is considered critical by many professionals due to the vulnerability of young children with disabilities to developing behavior problems, mental health disorders, and to experiencing maltreatment. This qualitative research study investigated ways personnel in EI/ECSE agencies in Oregon are addressing the mental health needs of the children and families they serve. How mental health is conceptualized by EI/ECSE providers, how well prepared providers are in addressing the needs, what kinds of strategies are used to serve children and families, and how adequately EI/ECSE families and children are served by the mental health system were explored. The study was divided into two parts, which included a state-wide study of 14 EI/ECSE agencies in five regions in Oregon, and an in-depth study of one EI/ECSE agency that has employed a mental health consultant. State-wide agencies were mainly located in rural parts of the state, although two education service districts in metropolitan areas were included. The perspectives of 10 mental health providers from community mental health centers were also collected. The most salient issues that surfaced in the state-wide study included the focus of EI/ECSE concern on behavioral and mental health problems in children ages three through five and strategies for related behavior management in classroom settings, the importance of collaboration with community agencies, and a desire for increased partnership with mental health. Interviews with both EI/ECSE and mental health agencies revealed problems in the mental health system including training of therapists in working with very young children, gaps in services for children who were not Medicaid-eligible and in services for parents with their own psychological issues, and in the capacity to serve all of the children who were referred. The in-depth study focused on one EI/ECSE agency that was developing staff competence in relationship-based work with families through support from a mental health consultant employed by the agency. Salient issues included funding for a mental health consultant in an educational agency, development of mental health services for early childhood through community collaboration, and supporting staff through reflective supervision.

Purpose The Johnson City Downtown Day Center (JCDDC) provides integrated inter-professional primary care, mental health, and social work case management services to homeless and under-served persons who have difficulty accessing traditional systems. Because of the exponential rise in substance abuse in the Appalachian region, the JCDDC providers and staff initiated SBIRT as recommended standard of care, as endorsed by SAMHSA, United States Public Health Services Task Force, and the National Institute on Alcohol Abuse and Alcoholism. The JCDDC has two mechanisms by which patients can choose to participate in substance abuse treatment: SMART Recovery, and psychiatric nurse practitioner (NP) referrals. The purpose of the project evaluates use of SBIRT at the JCDDC by determining process of (1) referral and (2) follow-up rates of those who received SBIRT; analyzing outcomes by measuring numbers of: (1) screens administered; (2) brief interventions; (3) positive screens; (4) referrals to either SMART Recovery or to the psychiatric NP; (5) participation in one follow-up. Review of Literature: Approximately 6.4 million people, or 2.4% of the U.S. population 12 years and older, currently misuse prescription medications. There is an undeniable and tangible correlation between the chronic disease of substance use disorder and unstable housing or homelessness (de Chesnay & Anderson, 2016). Similarly, substance use disorder was found to be much more common in people facing homelessness than in people who had stable housing (National Coalition for the Homeless, 2009). Substance Abuse and Mental Health Services Administration (SAMHSA) has been the most significant funding source for SBIRT proliferation in the United States. Despite a demonstrated need for substance abuse services among this vulnerable population, people who are homeless have substantially greater barriers to obtaining treatment and often go without. Summary of Innovation or Practice The current SBIRT process includes use of DAST-10 and AUDIT tools. Evaluating clinic processes and outcomes in vulnerable populations who have inconsistent erratic follow-up is challenging. However, new ways of understanding patterns and incremental outcomes is essential to addressing clinic practice that can impact outcomes in vulnerable groups. Implications for NPs The heterogeneity of the homeless population is often precipitated by a host of complicating factors including co-occurring mental illness, multiple chronic conditions, unstable income, and lack of transportation. Therefore, the importance of finding effective, cost-conscious processes that are population specific and patient-centered is essential for future research and policy. The inter-professional model of care also informs future practice by evaluating the feasibility of administering all of the elements of SBIRT in a single facility.
