The problem addressed in this phenomenological study was the lack of documentation that supported the lived experiences of crisis intervention team (CIT) trained police officers related to their encounters with persons with mental illnesses (PwMI). The purpose of the study was to explore the lived experiences of officers among CIT-trained police officers to address the problem. Using the Giles's communication accommodation theory and Rogers's protection motivation theory (PMT), the purpose of this study was to examine the perceptions of CIT-trained police officers of PwMI during CIT encounters. Rogers's PMT was aligned closest with the teachings of CIT training as described by the study's participants. Participants provided data which was comprised of completed questionnaires and transcribed interviews. The method of analysis used was a combination of inductive coding and theme analysis that established the results of this study. Key findings of the study identified a significant amount of frustration expressed in the lived experiences of the CIT-trained police officers. Pushback from the public mental health facilities helped with the frustration experienced by CIT-trained police officers who applied the fundamentals of PMT and attempted to navigate treatment with the limited resources available to help PwMI in crisis. The positive social change produced from this study includes recommendations to police leadership and mental health advocates to encourage certain CIT training-related practices that directly impact CIT field encounters with PwMI in crisis. Specialized training may promote improved departmental outcomes, assist with injury reductions, and enable police officer accountability and reliability.

Police officers are often the frontline responders to calls involving people in mental distress. The lack of specialized mental health training can influence stigmatization and criminalization of people with a serious mental illness (SMI). Crisis intervention team (CIT) training is a tool to provide law enforcement with specialized mental health training to enhance their skills and comfort level when responding to crisis calls. There is a gap in the research examining officers' experiences with CIT training and how they apply that training when encountering people who have a SMI. The purpose of this qualitative case study was to explore a select group of police officers in Central Pennsylvania who are trained in CIT and how they use the tools learned from CIT training while on the job. Using the social distance theory and procedural justice theoretical framework, the researcher investigated the experiences of CIT officers by using the information gained from CIT training with individuals they encounter with a SMI. This study also examined the components that officers find most valuable from CIT training when responding to individuals who have a SMI. Participants in the study consisted of police officers trained in CIT, from a rural department, and have responded to mental health calls. Key findings showed that CIT training improved officers’ knowledge and understanding of mental health to be able to slow down on crisis calls and show empathy towards people who have a SMI. Implications for positive social change involve policy reform for law enforcement training. The data from this study reveals that CIT training should be utilized across police jurisdictions, as it has shown to improve the interactions between law enforcement and people who have a SMI.

Background Persons with severe mental illness (SMI) are vulnerable to mental health (MH) crises in the community. These crises often resulted in costly hospital admissions. Crisis Resolution Team (CRT) was implemented based on limited empirical evidence to address this problem. In 2001, UK implemented a national policy making CRT mandatory nationwide. Since then, more studies were performed on the effectiveness of CRT. It appeared CRT is cost effective in reduction of admissions. There was evidence of positive clinical outcome in comparison with standard care. Hong Kong does not have a specific community crisis program for people with SMI. It is desirable for policy makers to consider alternatives to reduce hospital admissions and to strengthen community support for people with SMI. Aims To evaluate the feasibility on the implementation of CRT Model in Hong Kong as a service to reduce hospital admissions. Method A review of current evidence on the effectiveness of CRT model and assess the feasibility and desirability of such model for implementation in Hong Kong. Results Evidence from observational studies and randomized controlled trials show CRT to be effective in the reduction of hospital admissions and inpatient days in the UK. CRT is also associated with higher acceptance from patients and family members than from standard care. CRT appears to be feasible against other service to address the acute needs of persons with SMI in reducing hospitalization. Conclusion A preliminary assessment ruled out a number of alternatives in dealing with mental health crises in the community. Preliminary results suggested the CRT model might be a feasible and desirable solution. There is evidence on the effectiveness of such teams in the UK. Successful implementation of CRT service in Hong Kong will be a challenge for stakeholders of the mental health system.

The criminal justice and mental health systems increasingly overlap as persons with mental illness (PMI) are disproportionately present throughout components of the criminal justice system, a concern to mental health and criminal justice professionals alike. In response, various initiatives (aimed across components of the criminal justice system) have been developed and implemented as a means of combating this overrepresentation. The following research will focus on one specialized police-based initiative, the Crisis Intervention Team (CIT), which aims to train police how to recognize mental illness, de-escalate persons in crisis, and to seek treatment-based alternatives to arrest, when appropriate (Schwarzfeld, Reuland, & Plotkin, 2008). Alternatives to arrest consist of various community-based mental health services such as public hospitals (some of which are designated as emergency receiving facilities, or ERFs) or private clinics. While the components of CIT training likely influence officers in unique ways, research has yet to empirically examine how CIT influences police perception, behavior or the incidence of referrals to mental health treatment. As an initial step, this research assessed the attitudes police have regarding the hospital and mental health system within their district. Specifically, this research provides a basic understanding of how police regard their local hospitals and mental health facilities that are posited as available alternatives to arrest, and help identify the role CIT plays in shaping these attitudes. This study found almost no significant difference in the attitudes CIT-trained officers had towards district ERF and the local mental health system as compared to non-CIT officers. Only in one of the six departments studied was there a significant difference between the attitudes of CIT-officers and non-CIT officers; with the non-CIT officers actually having more positive attitudes about their local mental health system than CIT-officers. The six departments studied had nearly similar attitudes of their mental health resources, which would barely be considered passing on a standard grading scale. While officers in this study do not have very positive attitudes towards the ERF they use to transport PMI or their districts’ mental health system, these attitudes may in fact be more positive than many police departments without any specialized approach or initiative. The significance and policy implications of these attitudes are discussed at length, as these findings speak to the need for increased attention by both the mental health and criminal justice systems. Recommendations for future research, including expanding this study to rural departments or agencies with no connection to CIT, are also outlined.

ABSTRACT This dissertation has the following specific aim: to measure outcomes of social work emergency certificates produced by a Mobile Crisis Team to determine the effectiveness of these certificates at linking clients to services. Linkage to services is a programmatic goal and is achieved by ensuring clients receive adequate crisis and follow up services as a result of Emergency Certificates. In doing so, risks of homicide, suicide and grave disability are mitigated. Under its administrative umbrella, Mobile Crisis Team operates a police-social work collaboration known as Crisis Intervention Team, and both are administered by the State of Connecticut Department of Mental Health and Addiction Services in southeastern Connecticut. The data comes from 233 consecutive cases that involved Emergency Certificate's to transport clients to the local emergency department for psychiatric evaluation and treatment. Data was gathered post-hoc by reviewing copies of each specific certificate and cross-referencing those certificates with an agency risk management report and an electronic data base that stores demographic data on all clients. Descriptive, bivariate and multivariable analysis, such as cross tabulations and binary logistic regression, were used to analyze the data in this study. Additionally, the chi-square automatic interaction detector (CHAID) was used to construct outcome trees to describe subgroups of interest. This research is a continuation of previously published research on the various permutations and outcomes of mobile crisis programs, and contributes two unique programmatic features: the use by social workers of a legally proscribed coercive tool to send people to the emergency department for assessment, and the effectiveness of police-social work collaborations compared to social work only referrals. Overall, linkage occurs in over 80% of cases, with significant predictors of linkage identified as substance abuse, Hispanic ethnicity, criminal justice involvement, suicide, affective disorders, and insurance status. Implications for program evaluation, future research and limitations of the study are also discussed

This study describes police officers’ perspectives of Crisis Intervention Team (CIT) training. Ethnographic interviews were used to gather data, and Transformational Learning Theory guided this study. Implications of CIT training indicate that CIT officers are a part of a subculture within police culture, and respond differently to mental health calls differently than their non-CIT counterparts. Outcomes of these different response styles include decreased criminalization, decreased injury to officers/consumers, decreased use of force, and increased confidence in responding to mental health calls for officers who are CIT trained. Implications of this study are discussed in relation to sustainability of partnerships between law enforcement officers and mental health professionals.

The Crisis Intervention Team (CIT) training program is a community partnership that provides education, understanding, interventions, relationships, and protection for responding officers, individuals in crisis and the community. The purpose of this study is to assess the effectiveness of CIT training amongst law enforcement officers in reducing stigmatizing attitudes and opinions on mental illnesses. The study uses a dependent means t-test to test the study participant’s attitudes and opinions on mental illnesses using a pre/post-test survey. A modified version of the Attitudes to Severe Mental Illness Scale was used to measure attitudes, beliefs, and opinions of mental illness. This modified version was developed based on CIT course material. The ultimate goal of this study was to gain insight within the law enforcement community as to whether or not CIT training is raising awareness on the issue of mental health. Findings from this study suggest that after participation, those in the CIT training reported lower levels of non-stigmatizing opinions and attitudes in two of the four factors being analyzed.

Although a specific program called the Crisis Intervention Team (CIT) has been generally recognized as the best-practice model that addresses the needs of the police officers in responding to mental health calls, many jurisdictions across the country have not only adopted the full CIT model but also have taken the liberty of adding new components and/or removing components of the original model in order to create a unique program that fits the needs of their individual community. The issue of differentiated adaptations of the original CIT model has created a controversy around best practice in the area of police response to individuals with mental health issues who are in crisis. Using an on-line survey and interview methods, this study examined a relationship between the degree of variation within specialized policing response models and their corresponding community characteristics. Previous research shows that the components of the original CIT model have positive influence on officers’ confidence in interacting with people with mental illness. Therefore, this study also hypothesized that a rating of an SPR police officers’ job satisfaction was likely to correlate with the degree to which an SPR program adhered to the original CIT model. The study found that mental health resources, extent of presence of special populations in a community, existence of SPR policies in law enforcement, mental health, and dispatch departments, and how much law enforcement and mental health administrators supported the program, all predicted the degree of total deviation of a program from the original CIT model. Population density, related to a distinction between rural and non-rural communities, did not predict the degree of deviation from the original CIT model. The study also found that the degree of deviation of a program from the original CIT model did not strongly predict the rating of SPR officers’ job satisfaction. The study discusses the possible reasons for the results as well as implications for stakeholders who are considering implementation of a Specialized Policing Response model in their communities. Limitations of the current study’s research design are also discussed.

The present study utilized a mixed-methods strategy to examine the effectiveness, diffusion, and institutionalization of the Memphis Crisis Intervention Team (CIT) model. To evaluate the effectiveness of the training component of the CIT model, a panel research design was employed in which a sample of 179 law enforcement officers and 100 correctional officers in nine Florida counties were surveyed on the first day of training (pretest), the last day of training (posttest), and one month following their completion of CIT training (follow-up). These surveys measured the extent to which CIT training achieved several officer-level objectives, including increased knowledge of mental illness and the mental health referral process, improved self-efficacy when responding to mental health crises, and enhanced perceptions of verbal de-escalation skills, mental health services in the community, and the mental health referral process. The results of these surveys revealed officers experienced a statistically significant increase on every measure of training effectiveness between the pretest and posttest data collection points. However, a significant decline was found among the 117 officers that responded to the follow-up survey on the measures associated with self-efficacy and perceptions of verbal de-escalation, which points to a measurable decay in the effectiveness of the training in the intermediate timeframe with regard to these two measures. To examine the extent to which the diffusion of the CIT model resembles a social movement in the field of criminal justice and to explore the impact of CIT institutionalization on the organizational structure of criminal justice agencies, an online survey was distributed to 33 representatives of law enforcement and correctional agencies known to participate in the CIT program in the nine Florida counties in which officers were surveyed. The results of this survey indicate interagency communication and external pressure from mental health providers and advocates largely contribute to the decision of criminal justice agencies to adopt the CIT model. In addition, the findings of this survey suggest criminal justice agencies modify their organizational structure in a number of different ways to internalize and institutionalize the CIT model. By coupling a training program evaluation with an assessment of diffusion and institutionalization, this study makes a unique contribution to organizational and evidence-based literature.

Idag innefattar en del av det polisiära arbetet bland annat att bemöta och handskas med psykiskt sjuka individer. Då den psykiska ohälsan ständigt ökar i samhället innebär det att allt fler anställda hos polismyndigheten möter psykiskt sjuka individer. Därför är det betydelsefullt att poliser har kunskap om psykisk ohälsa. Genom kunskap kan samspelet mellan polis och den psykiskt sjuke ske på bästa möjliga vis. Denna uppsats önskar att belysa hur CIT (Crisis intervention teams) påverkat polisens möte med personer med psykisk ohälsa. CIT har visat sig positivt kunna påverka mötet mellan polis och den sjuke. Den positiva inverkan av CIT diskuteras utifrån fyra övergripande teman; hänvisning till vård och antal gripande, ökning av kunskap, bruk av våld, säkrare möten samt förändring av attityder och uppfattningar om psykisk ohälsa. CIT visade sig ha en positiv inverkan på hänvisning till vård samt har visat en ökning av kunskap kring psykisk ohälsa vilket bidragit till förändring av attityder och uppfattningar gällande psykisk ohälsa. Det gick att finna visst stöd i uttalandet att CIT inneburit en minskning av våld och antal gripande.
A big part of police work today revolves around dealing with mentally ill individuals, and being able to do so is very important. An increase in mental illness in society has led to more police employees having to deal with individuals who suffer from mental illness. This systematic literature review aims to investigate how CIT can influence the meeting between officers and the mentally ill individual. Current research on CIT has shown a positive effect on improving the situations in which officers and individuals with mental health interact. These positive outcomes could be discussed in relation to four themes. Direction to mental health services & number of arrests and an increase in knowledge regarding mental health was two of the themes identified. The other two themes was use of force/safer interactions and a change in attitudes and perceptions about mental health. CIT was found to have a positive impact on direction to mental health services, an increase in knowledge and changes in attitudes and perception. Some support could be found for a decrease in use of force and amount of arrests after completion of CIT training.

BACKGROUND: Loneliness is more prevalent among people with mental health problems than in the general population. However, loneliness has not been a particularly prominent focus in recent research on outcomes of mental illness. Loneliness interventions have also received little attention. AIMS: 1) To review literature for the definition and conceptual model of loneliness and its closely related concepts, and for well-developed measures of these concepts. 2) To systematically review the impact of loneliness and perceived social support on mental health outcomes 3) To explore the severity of loneliness among people leaving Crisis Resolution Teams (CRTs), and identify factors cross-sectionally associated with loneliness. 4) To determine whether loneliness at baseline predicts poor outcomes at 4-month follow-up, including overall symptom severity, affective symptoms, self-rated recovery and health-related quality of life. 5) To examine whether there is any difference in loneliness at 4-month follow-up between a peer-provided self-management intervention group and a control group in a randomised controlled trial. METHOD: A conceptual framework of loneliness and related concepts was developed and measures identified of main concepts. A systematic review of longitudinal studies examining the effect of loneliness and perceived social support on mental illness prognosis was conducted. The sample (n=399) was taken from patients participating in a research trial from CRTs. Participants in the trial intervention group were offered up to ten meetings by a peer support worker and a self-management workbook. Participants in the control group were only provided a self-management workbook. Respondents (n=310) completed the follow-up measurement four months after baseline. RESULTS: A model with five domains was proposed to incorporate all terms relating to loneliness. Well-developed measures assessing each domain or covering multi-domains were identified. Perceived social support and loneliness were associated with mental health outcomes. The severity of loneliness was high among people leaving CRTs. Greater loneliness was significantly associated with small social network size, limited social capital, severe affective symptoms and long-term mental illness history. Greater loneliness at baseline predicted poorer health-related quality of life at 4-month follow-up. Loneliness was also a better predictor of clinical outcomes than objective social isolation and social capital. Loneliness at follow-up was not significantly different between the intervention group and the control group. DISCUSSION Loneliness is an important issue in mental health service users. It could be a promising target to improve recovery for people with mental health problems. The efficacy of peer-provided self-management intervention on loneliness was not confirmed. More research is necessary to explore beneficial loneliness interventions so as to aid the development of recovery-oriented mental health services.

The main aim of this thesis is to analyze how the general public and registered churches are informed about the Psychosocial intervention team of the Czech Republic. In addition, the thesis focuses on the analysis of available sources of information about psychosocial intervention (aid) and crisis intervention in emergency situations. The churches were asked by using interviews with their representatives. These were mainly personal interviews, telephone communications, and e-mail correspondence. The awareness of the general public was identified by using a questionnaire. The questionnaire was based on the general information about the Psychosocial Intervention Team of the Czech Republic and emergency situations, in the second part of the questionnaire the questions were whether the respondents met with this team and in what case it was. This thesis is the first one that deals with the awareness about the Czech Psychosocial Intervention Team. It can therefore be used as a basis for further research on this topic. It has been found that the general public has some awareness of the Psychosocial Intervention Team, but it is at a very low level and only one of the 105 respondents has personally met with this team. As far as the churches are concerned, the awareness was higher, but only a few churches closely cooperate with this team. It should be noted that it would be desirable to deepen the awareness about this team by general public and to ensure better cooperation between this team and churches.

Research nos Estados Unidos é limitado a descrições de um pequeno número de CITs de alta qualidade e exames de sua eficácia no nível da comunidade. Este estudo utiliza uma amostra nacional de 1.887 instalações de internamento psiquiátrico da Pesquisa de Serviços de Saúde Mental Nacional para examinar (1) a diferença no número de pacientes psiquiátricos internados, entre estabelecimentos de execução de um CIT e aqueles que não o fazem por tipo de instalação e (2) a diferença em número de pacientes psiquiátricos internados entre instituições que oferecem crise psiquiátrica serviços de urgência e aqueles que não o fazem por tipo de instalação. As análises mostram que instituições psiquiátricas privadas prazo que operam a CIT ou serviço walk-in de emergência psiquiátrica relataram mais pacientes recebem cuidados em regime de internamento do que as instalações que relataram não enquanto as instalações administradas pelo governo que operam a CIT ou walk-in serviço relataram menos pacientes do que instalações que relataram nenhum CIT ou serviço walk-in. Estes resultados sugerem uma diferença no tipo de instalação que pode ser influenciando o número de pacientes que recebem tratamento hospitalar. Outras investigações devem considerar as características das instalações e dos dados demográficos da população que servem como fatores de internação em hospitais psiquiátricos.
ABSTRACT: Psychiatric crisis intervention teams (CITs) are growing throughout the United States. International research on the effectiveness of CITs in decreasing admissions to psychiatric hospitals has shown mixed results. Research in the US is limited to descriptions of a small number of high-quality CITs and examinations of their effectiveness on the community level. This study uses a national sample of 1,887 inpatient psychiatric facilities from the National Mental Health Services Survey to examine (1) the difference in number of inpatient psychiatric patients between facilities implementing a CIT and those that do not by facility type and (2) the difference in number of inpatient psychiatric patients between facilities offering psychiatric crisis walk-in services and those that do not by facility type. Analyses show that private-run psychiatric facilities that operate a CIT or psychiatric emergency walk-in service reported more patients receiving inpatient care than facilities that reported they did not while government-run facilities that operate a CIT or walk-in service reported less patients than facilities that reported no CIT or walk-in service. These results suggest a difference in type of facility that may be influencing the number of patients receiving inpatient treatment. Further investigation should consider characteristics of facilities and demographics of the population they serve as factors in admission to psychiatric hospitals.

In my thesis, I postulated the target to map the methods and procedures applied by the CR Police, Regional Police Headquarters of the South-Bohemian Region within the primary, secondary as well as tertiary prevention of posttraumatic reactions and disorder. A partial target of the thesis is finding out the rate of the satisfaction with availability of intervention care for policemen and employees of the Regional Police Headquarters of the South-Bohemian Region, finding out the rate of the awareness, usability and satisfaction with the care of the posttraumatic intervention team in the South-Bohemian Region, awareness of the service Help Line in crisis and its usability. Moreover I postulated the goal to map the interest in the possible internet helpline in the scope of IZS with elaboration of SWOD analysis. Everybody, also policemen may suffer from stress reaction. Also they can have the psychical and physical signs of an acute stress reaction. They perform the most of service acts, interventions in a routine way, however, besides this, also situations occur, differing from the normal life experience and policemen get in this way very often into the stress situations, which can arouse deep mental turbulences. The impact of feelings on the mental state in case of extreme experiences is a normal human reaction in a not normal situation. At present, each policeman has the possibility to ask for help a team of specially trained experts, the team of posttraumatic intervention care. In the team, not only psychologists are working, but also experienced policemen, experts of the branch of the Ministry of Interior and the priests prepared to help with their experience, erudition and knowledge acquired by a special training. Since 2001 the impacted person may call the anonymous Crisis Help Line, which is a supporting part of the posttraumatic intervention care for the policemen, firemen, employees of the branch of the Ministry of Interior and their family members. The crisis help line is also at disposal to the public in case of extraordinary events and crisis situations. For this reason, this thesis treats the methods of posttraumatic intervention care, awareness, usability, satisfaction and trust in these branch offices of the CR Police.

The main aim of this thesis was to gather information concerning psychological aspects in the field of critical management from available sources and to introduce a reader- friendly survey of this topic at the same time. Basic terms were explained, as well as various psychological dispositions of critical staff members and, last but not least, fundamental forms of psychological assistance with regard to citizens in extreme life situations. The psychological awarness survey of both critical staff members and people affected, was carried out by means of a public survey. The idea behind was to demonstrate up-to date level of psychological awarness in particular regions of the Czech Republic.

Options and application pastoral theology in posttraumatic team HZS CR. The thesis is divided into three sections. The first section dedicates to the process of developing psychological service in selected components of integrated rescue systém and the formation and other developing of teams who have to provide psychological assistance during some extraordinary events. It also pays attention to the theoretical cognition of stress problems, traumas and posttraumatic disorders. The significant chapter is the part that is concerned with communication which is the main resource while providing help. The second section dedicates to the practical theology, its origin and gradual development in Central Europe. They are listed two main concepts of developing the practical theology which are important for understanding next formation and the possibility of using practical theology in interdisciplinary activities. Analysis of the constitutive elements of the Church is the mainstay. Main attention is paid to diakonia and linking charitable service of the Church. The third and also closing part includes the practical verification findings via questionnaire. The results and comments gained thanks to questionnaire as well with theoretical knowledge and practice of professional life enable the final evaluation of the whole work. This file is supposted to help while seeking the answer on original question of possibility of cooperation and using already mentioned disciplines while providing help.
