Texto completoIntroduction: Rheumatological diseases, ranked 3rd in terms of disability (Cross, Smith, Hoy et al., 2014), have a major impact on health. Chronic inflammatory rheumatism (CIR) is one of them. Patients suffering from these conditions are subject to recurrent fibromyalgia-like pain and anxiety-depressive co-morbidities. With a very different symptomatology, diabetes also has a major impact on society: it is the 7th leading cause of death according to the WHO in 2003, with possibility of serious complications and emergence of negative emotions, as such as anxiety and depression. Like diabetes, ICRs require patients to adopt new behaviors and to maintain them over time. They are then given recommendations about their treatment, as well as about their lifestyle. It can be difficult for them to adhere to these recommendations. Enhancing mindfulness skills can help them to develop positive emotions, coping strategies and acceptance skills, as well as satisfying the psychological needs that are fundamental to their balance. The aim of this thesis is to identify the levers for action among mindfulness skills, satisfaction of basic psychological needs and beliefs about treatment, in order to suggest ways of optimizing patients care for the professionals around of them.Method: The first part of the thesis consisted of adapting, in the chronic disease context, and validating a scale for measuring the satisfaction and frustration of psychological needs derived from the self-determination model (Chen et al., 2015). This tool using 24 items assessed by a 5-point Likert scale, was tested on 2 samples of subjects recruited online: a group of patients with CIR (rheumatoid arthritis, ankylosing spondylitis, or psoriatic arthritis) and a group of diabetic patients. The psychometric properties of the scale were tested using quantitative analyses based on two theories: the classical test theory, and the theory based on the item response model. The validated scale was then used with other variables of interest, such as mindfulness skills, beliefs about medicine and anxiety-depressive affects, in multiple linear regression analyses to identify their impact on life satisfaction, quality of life, and adherence, in order to propose interventions targeting the identified predictors.Results: The scale, adapted to the context of chronic illness, is a reliable and valid instrument that can be used in both research and clinical practice. It comprises 8 dimensions and distinguishes two populations in relation to patients: their relatives and healthcare professionals. Life satisfaction can be improved by supporting patients’ autonomy, and by taking into account patients depressive affects. Quality of life can be improved by satisfying the need for competence, by reducing depressive affect and the frustration of the need for autonomy and competence. Anxiety affects have an ambivalent impact on quality of life, improving its physical dimension and altering its mental dimension. Adherence to treatment can be improved through mindfulness skills, beliefs about treatment, and by reducing the frustration associated with relationships with relativesConclusion: In order to develop mindfulness skills, to support patient autonomy, and to remove obstacles linked to beliefs of treatment, integrative approaches should be promoted to improve overall patient care

Texto completoPatients with lipodystrophies dues to mutation in the LMNA gene encoding A-type lamins, or HIV-infected patients (Human Immunodeficiency Virus) receiving antiretroviral therapy (ARV) are prone to develop early atherosclerosis and vascular calcifications associated with comorbidities linked to premature aging. Our study focused on the impact of LMNA mutations or antiretroviral treatment in vitro on vascular smooth muscle cells (VSMC) or endothelial cells from human coronary arteries. The results obtained during my thesis showed that some ARV (the protease inhibitors lopinavir or atazanavir associated with ritonavir) induce a cellular premature senescence with associated dysfunctions in endothelial cells, whereas others have little or no consequences (maraviroc, dolutegravir, maraviroc/dolutegravir and darunavir/ritonavir). In addition, some ARV (lopinavir or atazanavir with ritonavir) or the expression of LMNA mutations p.R482W, p.D47Y or p.R133L induce premature senescence, osteogenic transdifferentiation with calcification and oxidative stress of VSMC. Our results reveal that the accumulation of farnesylated prelamin A (an immature lamin A) and the decreased expression of its processing enzyme ZMPSTE24 are, at least partly, responsible for these effects.This work shows the different effects of ARVs and highlights the existence of common pro-atherogenic pathophysiological mechanisms in HIV-infected patients receiving some protease inhibitors and in lipodystrophic patients with LMNA mutations, initiated by an accumulation of farnesylated prelamin A related to a decrease expression of ZMPSTE24. These abnormalities could give rise to new therapeutic perspectives

Texto completoSurvivors of early-stage breast cancer (BC) often face relevant late and long-term health-related sequelae, downstream functional impact of treatments and persistent symptoms. This thesis gathers several projects exploiting data of the CANcer TOxicity study (CANTO; NCT01993498), to understand the potential of health behaviors (HB), including maintenance of a healthy body mass index (BMI), adherence to physical activity (PA) recommendations, tobacco abstinence and limitation of alcohol consumption, to improve overall health and preserve quality of life (QOL) of BC survivors. CANTO is a nationwide, prospective cohort of patients with stage I-II-III BC. Longitudinal clinico-behavioral (e.g., nurse-reported BMI, tobacco and alcohol use, PA [GPAQ-16]), psychological (HADS), socio-demographic, and treatment data are available at diagnosis and yearly afterwards. Treatment-related toxicities are extensively assessed by patient-reported outcomes (PROs; EORTC QLQ-C30, -BR23, and -FA12) and by provider’s evaluation (CTCAE). A number of statistical analytic methods were involved in this thesis, including multivariable linear and logistic regression, multinomial regression, random-effects mixed-models, and latent-class analyses (i.e., group-based trajectory models). In the first part, this thesis aimed at (1) describing HB over the survivorship period, focusing on risk factors for persistent unhealthy behaviors, and at (2) characterizing the evolution of treatment-related toxicities and defining their interplay with HB. We assessed QOL in two specific and frail populations, at risk of severe downstream treatment impact: (2a) among patients receiving adjuvant chemotherapy (CT) and (2b) among obese BC survivors, where we also specifically studied the social impact of BC and its relationship with BMI. Key findings included the following: (1) most women pursue a healthy lifestyle and some are prone to improvements after diagnosis, although unhealthy behaviors tend to persist, with varying clinico-demographic and psychological determinants by HB; (2a) there is great variability in trajectories of post-CT QOL, with a clinically relevant cluster of patients reporting severely deteriorating QOL, never recovering to pre-treatment levels for over 4 years, and being also particularly prone to unhealthy behaviors; (2b) among obese survivors, those that lose weight report less of a decline in PROs, without evidence of worsened functionality or symptomatology in any QOL domain. Moreover, excess weight seems to represent a barrier to occupational reintegration, and weight loss is associated with higher rates of return-to-work after BC. In its second part, the thesis explored a core case-study, cancer-related fatigue (CRF). First, (3) prevalence and risk factors for severe CRF were evaluated, analyzing global CRF and its physical, emotional, and cognitive dimensions. We then (4) characterized the uptake of recommended management strategies to improve CRF, including PA. Findings revealed that: (3) there is a high prevalence of post-treatment severe CRF, which stays persistently elevated over time, with a multitude of risk factors including psychological distress, concomitant symptoms, and unhealthy behaviors, particularly excess weight and tobacco use (preliminary predictive equations were generated); (4) recommendations uptake, including PA adherence, seems suboptimal and in need of implementation in clinical practice. The overarching goal of this thesis was to inform targeted behavioral interventions for patients at risk of persistent toxicities. This work resulted therefore in (5) the generation of a study protocol for a phase III randomized trial: “Motivating to Exercise and Diet, and Educating to healthy behaviors After breast cancer - MEDEA”. This study, aimed to evaluate the impact of a personalized telephone-based weight loss intervention on CRF in overweight and obese BC survivors, is currently ongoing (NCT04304924)
