The functions of nurses in the department of gastroenterology are diverse. Nurses perform a wide range of medical appointments (injections, enemas, medication distribution, etc.), diagnostic procedures (body temperature measurement, gastric and duodenal intubation, etc.). If necessary, nurses should be able to calculate the respiratory rate and pulse rate, measure blood pressure, daily urine output, correctly collect the patient’s secretions (sputum, urine, feces) for analysis and send them to the laboratory. In urgent situations, nurses are required to be able to provide first aid (artificial respiration, chest compressions, the application of a hemostatic tourniquet) as well as first aid for gastrointestinal bleeding. This article presents evidence that the role of the nurse in gastroenterology is very important. Since some diseases in gastroenterology require urgent surgical intervention, a nurse is responsible for conducting a high-quality nursing process in the preoperative period and postoperatively for proper and timely nutrition of patients, organizes feeding of patients on strict bed rest, monitors the storage of food in refrigerators and bedside bedside tables, check the content and quality of programs.

Resumen
Objective - Nursing documentation is a part of the medical records. Which have important values such as administrative and legal aspects, etc. Therefore, the hospital manager should design an organizational culture that increases nurse commitment and motivation to complete medical records. The objective of this study was to find out the effect of direct and indirect perception of organizational culture toward nurse's commitment with motivation as a mediating variable. Methodology - The data were collected using questionnaires. The sample was 44 nurses of inpatient unit inthe Paru Batu Hospital. Data analysis used in this research was path analysis. Findings - The result of data analysis showed that there was an influence of organizational culture on nurse commitment at a value of 0,592. Moreover, there was an influence of organizational culture on the commitment with motivation as a mediating variable at value of 0,307. Novelty - Nowadays, only few studies have investigated the influence of organizational culture toward commitment with motivation as a mediating variable. In addition, studies examining the effect of it with nurses as the subject and nursing documentation as the object are very limited. The article provides an overview of the European experience in changing the role of a nurse, as well as Russian practices within the framework of a pilot project to expand the functions of nurses in accordance with the order of the Ministry of Health of Russia "On the approval of an action plan («road map») to expand the functions of specialists with secondary medical education. The author emphasizes that the new challenges of the millennium entail a shift in emphasis towards patient orientation, which implies an improvement in the quality of medical services, and this, in turn, will address the issues of competent distribution of powers between doctors and nurses. The article notes that patient-centered medical care entails an increase in the burden on nurses and their empowerment. The author analyzes both the advantages of expanding the role of nurses: career advancement with continuous professional and personal development through the acquisition of additional knowledge and competencies, and possible risks: psychological resistance of doctors and nurses to new forms of work, conflict of interests between doctors and nurses, unwillingness of nurses to work with documents, etc.

Purpose The purpose of this paper is to provide an overview of published literature on behavioural and psychological symptoms of dementia (BPSD) nursing in Japan and to highlight challenges that need to be resolved. Design/methodology/approach The criteria for retrieval of literature were as follows: a BPSD study conducted by a nurse in Japan, and it must have been published. Papers without conference proceedings and peer reviews and literature without English titles and abstracts were excluded. The PRISMA (preferred reporting items for systematic reviews and meta-analyses) was referenced. Findings Based on the analysis of 20 studies meeting the criteria, nurses tended to manage BPSD when all three of the following were clearly defined: attempts to understand BPSD, the provision of nursing intervention to improve the quality of care and clarification of the perception of BPSD. There were eight studies that implemented surveys considered to be helpful for nurses to understand BPSD with the aim of clarifying the symptomatic factors, meaning of each behaviour, etc. In the eight studies, nurses directly coped with BPSD in various ways. Four studies reported on how nurses perceive the associated behaviours and symptoms of BPSD patients. Originality/value This study suggests that not only implementing interventions but also aiming at improving nurses’ understanding of BPSD and their level of knowledge are crucial to promote BPSD nursing in Japan.

Japanese nurses, like their counterparts in many other countries, are suffering from staff shortages and severe working conditions. The Japan Federation of Medical Workers’ Unions (Nihon Iroren) launched a campaign in 1989 for nurses called the ‘Nurse Wave’. Their demands were many: to increase the numbers of nursing staff, the regulation of night shifts, the implementation of a five-day working week everywhere, a fair appraisal of nurses’ work, better vocational training, etc. Nurses in white uniforms assembled at meetings, marched and took part in nationwide strikes. They collected over 5.4 million petition signatures, which they submitted to Parliament. After three years of campaigning, they won the enactment of a law for securing sufficient numbers of nursing personnel. This struggle, which highlighted the most pressing demands of nurses and was supported by the general public, was a new experience for Japanese nurses. It was a militant movement which demanded that the Government should change its health policy. Against the sluggish image of the Japanese labour movement, strikes and demonstrations organized by nurses dressed in white have made a strong impression on the Japanese people. As health care is a public service on which the life and death of people depend, a strike at a health institution naturally has constraints and must follow specific procedures. In this article, the measures taken by Nihon Iroren to minimize the consequences of strikes on patients and the local community, the impact of the nurses’ movement on the society, as well as the lessons drawn from the campaign, are presented.

Even though burnout syndrome is detected in professionals from various areas, prevalence is particularly high in service and care workers, especially health and care ones. Among those, nurses have been the subject of several studies, because they experience constant stressful labour situations, working in direct contact with patients who have different expectations and degrees of suffering. It is, therefore, necessary to conduct a study to determine the prevalence of the symptoms of burnout among nurses in hospitals in an effort to anticipate the negative impact of burnout on hospital nursing services. Consequently, this study aims to determine the level, knowledge and prevention among clinical nurses. A descriptive design was adopted for this study which was conducted in State Hospital, Ijaye, Abeokuta. A total of 100 respondents were used for the study, a stratified sampling technique was used to select the participants. A modified, structured, closed-ended questionnaire was used. Analysis was presented in tables, charts and percentages. Nearly all the causes listed in the questionnaire were seen as factors causing burnout in clinical nurses (99% of working long hours). 96%: from patients and families. Furthermore, it was found out that the majority of the nurses agreed to items listed in the questionnaire as things a nurse can do to prevent burnout, establishing social support; 94%, set goals for self; 99% etc. This implies that the majority of the nurses know what they can do to prevent burnout. Aside from free study grants, job opportunities that are most likely to attract young people, what can be done to retain nurses are to improve job satisfaction, patient safety, quality of health care and quality of life for both nurses and patients.

As recently as 20 years ago, the International Labor Organization has drawn the attention to the nurses’ working conditions, nursing and health. Nursing is one of the professions, mostly exposed to the risk of experiencing back injuries. One of the main precautions to prevent nurses' back injuries is the use of technical aids and the work environment provision with ergonomic means. There must be a lot and diverse devices in the workplace available for nurses to protect them from injuries [1]. The European Agency for Safety and Health at Work identifies several groups of risk factors that affect the health of nurses. They are work-related risks (monotonous movements, use of physical force, an uncomfortable body position), patient-related risks (a heavy patient, immobility, etc.), environment-related risks (slippery work surfaces, limited area, etc.), and other risks (lack of knowledge, inappropriate clothing and footwear, etc.) [2]. The aim of the research was to analyze the injuries experienced by nurses in changing the situation of patients. A quantitative survey was conducted, providing the questionnaires; its participants were nurses, working in Utena City health care institutions. The nurses usually experience various types of sprains of back, lumbar and shoulder girdle muscles. Injuries, suffered by nurses, are related to the lack of additional facilitators and knowledge on how to lift a patient in a correct manner.

This study was to explore the perceptions of Filipino nurses’ spirituality and the provision of spiritual nursing care. A descriptive, cross-sectional, and quantitative study was adopted for this study. The study was conducted in the Philippines utilizing a convenience sample of 245 nurses. Nurses’ Spirituality and Delivery of Spiritual Care (NSDSC) was used as the main instrument. The items on NSDSC with higher mean scores related to nurses’ perception of spirituality were Item 7, “I believe that God loves me and cares for me,” and Item 8, “Prayer is an important part of my life,” with mean scores of 4.87 ( SD = 1.36) and 4.88 ( SD = 1.34), respectively. Items on NSDSC with higher mean scores related to the practice of spiritual care were Item 26, “I usually comfort clients spiritually (e.g., reading books, prayers, music, etc.),” and Item 25, “I refer the client to his/her spiritual counselor (e.g., hospital chaplain) if needed,” with mean scores of 3.16 ( SD = 1.54) and 2.92 ( SD = 1.59). Nurse’s spirituality correlated significantly with their understanding of spiritual nursing care ( r = .3376, p ≤ .05) and delivery of spiritual nursing care ( r = .3980, p ≤ .05). Positive significant correlations were found between understanding of spiritual nursing care and delivery of spiritual nursing care ( r = .3289, p ≤ .05). For nurses to better provide spiritual nursing care, they must care for themselves through self-awareness, self-reflection, and developing a sense of satisfaction and contentment.

Background: Pharmacovigilance is multidisciplinary field of the detection, assessment, understanding, and prevention of adverse drug reactions (ADRs). The incidences of ADRs about 95% are undocumented worldwide. However, there is a lack of information on ADR burden in developing countries due to lack of resources, infrastructure, and expertise.Methods: A cross-sectional questionnaire-based study was conducted on 90 staff nurses 90interns and 90 postgraduates. The validated questionnaire contained items to check knowledge including 7 questions, 9 questions to analyze attitude and 5 for practices. Comparison between knowledge, attitude and practice data obtained from resident doctors and nurses was performed using Chi square test.Results: A total of 270 healthcare professionals participated in the study. In our study the participants had good knowledge regarding the purpose of monitoring and reporting ADRs etc. The result shows that health care professionals felt reporting of ADRs is a professional obligation and all ADRs should be reported. There was no significant difference in the knowledge and attitude between interns and nurses but few questionnaires show significant variations between post graduate and staff nurse. The practice of ADR reporting was significantly higher in postgraduate compared to nurses.Conclusions: This study concluded that post graduate, intern and staff nurse had good knowledge and positive attitude towards pharmacovigilance and ADR reporting, but unfortunately the actual practice of ADR reporting is still deficient among staff nurse and intern. This can be improved by adequate training and motivation.

The purpose of this clinical project was to determine if nurses in an acute care setting would report a need for debriefing following a patient death. The review of current literature revealed mixed results regarding the effectiveness of debriefing as an intervention. Literature shows that emotional expression is perceived as cathartic by nurses and is viewed as a positive coping mechanism. However, randomized controlled trials of debriefing models did not demonstrate that participation in debriefing prevented distress symptoms. A survey was created to assess nurses' views regarding debriefing following a patient's death. Surveys were placed in mailboxes of Registered Nurses and Licensed Practical Nurses in a 220 bed hospital in Montana. There were 55 respondents. Demographically, the average participant in the survey was 38 years of age, held a bachelors degree, had been a nurse for 9 years, worked in a critical care unit, and had experienced approximately 15 patient deaths over their career. The majority of nurses surveyed felt that an information-sharing and event processing session among peers (debriefing) would be helpful in coping with their emotions after a patient's death. Thirty of those surveyed reported they would be very likely to participate or would definitely participate in a debriefing session if it were available. The overwhelming majority (n=52, 94.3%) reported experiencing one symptom of distress after a patient death. Comments written on the surveys were grouped into themes. These themes included catharsis/sharing, positive coping, past experiences with debriefing, prevention of death, perceived need for formal debriefing, no perceived need for formal debriefing, emotional distancing, and debriefing only for special circumstances. Surveys revealed that the majority of nurses surveyed felt that debriefing sessions would be helpful in processing their emotions following a patient death. The author wrote a nursing policy regarding the use of debriefing in a hospital setting for nurses following a patient death and made this available to the organization surveyed.

Since the gender issue in India is a unique issue in India, understanding the representation of female nurses in films is of paramount importance. Mostly, the characters of female nurses are represented in a bad light. Sometimes their characters are comically or sexually depicted. Films in India have a more significant influence on society and vice versa. Moreover, the Indian film industry has its own identity. The way the films depict the characters of nurses raises ample issues in the nursing profession as well as in public. Nurses wonder why writers, journalists, producers, directors, etc. do not have an accurate image of the nursing profession. This chapter attempts to understand the characters of female nurses in Indian films.

This chapter investigates best practices for nurses who are involved in supporting families and family caregivers in palliative care. It views the family as a specialized unit of care which contributes to the healthcare system in a substantive way. There are significant implications for families and family caregivers in taking on these responsibilities, and nurses play a vital part in maintaining the integrity and health of the family and family caregiver. With a focus on palliative care, specific suggestions are made for nurses’ assessments in identifying needs and providing subsequent interventions. Particular sites of care (home, acute, etc.) and implications arising from a range of disease categories are discussed. The chapter concludes with recommendations for identifying community resources.

This chapter makes the case for mobile health care and its solutions in the non-acute community health care environment, where critical issues include usability, adoption, interoperability, change management, risk mitigation, security and privacy, and return on investment. A proposed community health care application demonstrates how these issues are addressed.

In this essay, the author attempts to discuss the perception by doctors and nurses of the nature of territories of the Caucasus front and the relationship of representatives of the Russian medical and sanitary services with the local population. In this study, the author refers to reports, diaries, memoirs, and press reports of doctors, nurses, orderlies and representatives of the military sanitary department, the Russian Red Cross Society, the Zemstvo and city Union, and other organizations. Contemporaries and participants of the studied events repeatedly point to the “peculiarity” of the Caucasian front. The “special conditions” of the Caucasian theatre had a serious impact on the conduct of hostilities there and, of course, on the work of the medical service. Most of the medical and sanitary representatives of the service of the Caucasian theatre of military operations came from the European part of Russia and for the first time were faced with different natural, geographical, and sanitary conditions, as well as the traditions and mentality of the populations of Transcaucasia, Turkey, and Persia. Some of the staff had lived previously in the Caucasus, but nevertheless encountered a different cultural and natural environment being in Asia Minor and Persia. In these conditions, it was more difficult to organize the medical care of the army, the refugees, and to improve the sanitary situation in the territories occupied by the Russian army. It is important to highlight the unusual nature of that time: it was the first opportunity for women to be involved in this process. Women of the “East” lived in the territories occupied by Russians, while women of the “West” held positions as doctors, nurses, heads of the economy, etc., and they came there together concerning the institutions of sanitary and medical care. At the Caucasian Front, we can discern a forced meeting of “East” and “West”, which had a mutual influence on each other.

In Romania, dementia care has gradually improved in the last decade. Accessibility of psychiatric services and availability of psychiatric and/or psychotherapeutic treatment have been significantly enhanced by new legislations and regulations. Public and private sectors of home care for the elderly have also been developed. Despite these changes, the management of dementia remains challenging because of delay in diagnosis and/or inappropriate treatment. Dementia care in the future in Romania should adopt a professional approach by improving education and training, as well as changing attitudes, not only among doctors (general practitioners, psychiatrists, neurologists, etc.), but also among all those involved in care (nurses, social assistants, relatives, etc.) and the general population. Reducing stigma and discrimination will contribute to early diagnosis and appropriate treatment. Collaborative efforts from a medico-social and community perspective, with the support of government authorities, could provide a source of new funding, which should be distributed according to the needs of each county in Romania.

Gone are the days when the traditional classroom is the only way to teach concepts to students. Now, the digital age allows professors a new territory to embark on. Currently, professors can add program features inside Blackboard or other courses which allow students to take quizzes, game simulations, and real life virtual simulations of cases (e.g., nurses giving care to consumers, students giving instructions on the correct theories to apply to situations, students participating in quizzes that require them to produce lighting via digital cameras in a field type environment etc.). With these new ways to teach in the digital age, there may be more challenges to introduce safeguards for cheating when the student is not face-to-face with the instructor monitoring their progress on examinations etc. Consequently, cheating in Higher Education (HE) classrooms is rampant at some universities. This chapter provides a discussion on cheating. In addition, authors discuss their digital learning activities and their experiences in which students have cheated and state safeguards to guard against cheating. In noting their perceptions of digital cheating, further discussions will compare and contrast the experiences of the faculty. This work provides recommendations and suggests solutions to combat cheating.

Abstract This paper presents techniques for reverse engineering of objects containing faces with multiple loops (faces with holes, slots, etc.) wherein these internal loops are specified as constraints on the underlying surface. Non Uniform Rational B-spline (NURBS) surfaces are used for the representation of the faces with the constrained regions specified analytically. The algorithm for fitting of NURBS surfaces to a set of data points is first discussed and then the conditions necessary for joining two patches with the desired continuity (either C0, G1 or C1) are developed. Finally the concepts developed are illustrated with three examples. This paper thus brings together the initial results in the development of a constraints based system for reverse engineering of multiple loop faces in a B-Rep based modeler context.

Progress in the computer performance has enabled automatic optimization of the three dimensional shape of turbine blades with a large number of large-scale CFD (Computational Fluid Dynamics) calculations. This paper presents an advanced aerodynamic optimization system for turbine blades. The system can automatically find improved blade shapes that give better aerodynamic performance in a turbine stage and reduces human efforts to generate blade shape data, computational mesh, CFD input data etc. The system consists of three parts; parameter updating part, blade shape generation part, and aerodynamic performance evaluation part. In the parameter updating part, users can choose DOE (Design of Experiment) or evolutionary optimization method such as GA (Genetic Algorithm), ASA (Adaptive Simulated Annealing), etc. to define the parameters in each step. The shape generation part changes the blade shapes using NURBS curves whose control point parameters are defined in the parameter updating part. Three-dimensional CFD grid is automatically generated for the changed blade shapes and steady CFD calculation is used to evaluate the aerodynamic performance of the changed blades in a turbine stage. Stagger angle distribution in the radial direction was thought as one of the important design parameters of turbine blades because it determines the flow pattern in radial direction. Then it was chosen as an optimized parameter with NURBS curves in this system. First, DOE was used for the human optimization, in which the parameter range for the advanced optimization was estimated and the best shape obtained was used as the initial shape for the evolutionary optimization to explore better blade shape parameters. Stage loss of an exhaust stage of IP (Intermediate Pressure) turbine which contains relatively high aspect ratio was chosen as the objective. In spite that such kind of stage was considered not to be sensitive to three dimensional stacking, the results showed good performance enhancement.

Abstract C-frames constitute a large portion of machine tools that are currently used in industry. Examples of these frames include drilling machines, presses, punching and stamping machines, clamps, hooks, etc. The design parameters of these frames include the dimensions of their cross-sections, which should be chosen to withstand the applied loads and minimize the element’s overall weight. Traditionally, the cross-section of C-frame belonged to a set of primitive shapes, which included I, T, trapezoidal and rectangular sections. This paper introduces a new methodology for designing the frame’s cross-section. The cross-sectional shape is represented using non-uniform rational B-Spline (NURBS) in order to give it a form of shape flexibility. A special form of genetic algorithms known as real-coded genetic algorithms is used to conduct the search for the design objectives. Real-coded genetic algorithms are known to outperform the simple binary representation genetic algorithms when dealing with continuous search spaces. The results showed that the optimal shape was a semi I/T-section with the material bulk related to the applied load.

The diesel generator of HANARO provides Class III (standby) power during the loss of Class IV (off site) power events, for the important reactor system equipment such as the emergency water supply system, emergency ventilation system, etc. The HANARO has been stabilizing its operation since 1995, so the diesel generator failure data from 1995 to the end of 2002 has been collected. This study shows the failure-to-start probabilities and the failure-to-run probabilities of diesel generator within the 90% confidence bounds. A comparison with the NUREG and AECL studies is shown in this paper.
