Asperger’s syndrome was added to the psychiatric disease classifications in DSM-IV (1994), and removed from DSM-5 (2013) almost 20 years later. This is a short life for a psychiatric syndrome. This chapter examines the case in depth in order to see what can be learned from it about appropriate criteria for making changes in the DSM nosology. Scientific criteria, clinical considerations, and patient/family perspectives are considered. In general, I recommend broadening the criteria to include the impact on patient self-understanding and identity.

This chapter continues the exploration of semantics and its relevance for the art of written expression for interpreting Old Testament narratives. Contemporary and biblical examples of figurative expression are identified and analyzed in their literary and cultural contexts. Three examples from the book of Ruth are analyzed. Naomi’s bitterness in Ruth 1:20–21 is explored in the context of the prayers of lament in ancient Israel. The common description of Boaz and Naomi as persons of worth (hayil) contributes to their positive representation of an Israelite community ethos. The description of Boaz as a kinsman-redeemer offers an example of a term representing a social institution in Israel for the support or rescue of family members in distress.

A key objective of the study of work-family balance detailed in this report was to build an evidence base to inform policy creation or refinement targeting work-family balance and related implementation standards to ensure the protection and preservation of Qatari families. Two complementary projects were designed and implemented to achieve this key objective. The first project was a qualitative study involving in-depth interviews with 20 Qatari working adults (10 males and 10 females). The interviews were designed to learn the meaning of work-family balance among Qataris, identify the factors shaping work-family balance or the lack thereof, and collect firsthand detailed information on the use and value of policy-relevant work-family balance sup - ports for working Qataris. The second component was a survey designed to describe work-family balance among working Qatari adults, determine potential health and well-being consequences of poor work-family balance, and characterize Qataris’ use of and preferences for new work-family balance supports. The data from the qualitative interviews tell a very clear story of work-family balance among Qataris. Work-family balance is primarily viewed as working adults’ ability to meet responsibilities in both the work and family domains. Although work-fam - ily balance was valued and sought after, participants viewed work-family balance as an idyllic goal that is unattainable. Indeed, when individuals were asked about the last time they experienced balance, the most common response was “during my last vacation or extended holiday.” The challenge of achieving work-family balance was equally shared by males and females, although the challenge was heightened for females. Qataris recognized that “work” was essential to securing or providing a desirable family life; that is, work provided the financial wherewithal to obtain the features and comforts of contemporary family life in Qatar. However, the cost of this financial wherewithal was work hours and a psychological toll characterized as “long” and “exhausting” which left workers with insufficient time and energy for the family. Participants commented on the absolute necessity of paid maternity leave for work-family balance, and suggested it be expanded. Participants also discussed the importance of high-quality childcare, and the need for greater flexibility for attending to family responsibilities during the working day. Data from the quantitative national survey reinforce the results from the qualitative interviews. Work-family balance is a challenge for most working adults: if work-fam - ily balance were given scores like academic grades in school, the majority of both males and females would earn a "C" or lower (average, minimal pass or failure). As intimated in the qualitative data, working females’ work-family balance is statistically poorer than that of males. Poor work-family balance is associated with poorer physical and mental health, with particularly strong negative associations with depression. It appears the Human Resource Law of 2016 was effective in raising awareness of and access to paid maternity leave. However, a substantial minority of working Qataris lack access to work-family balance supports from their employer, and the supports that are provided by employers do not meet the expectations of the average Qatari worker.

Coffin-Lowry syndrome (CLS) is a relatively rare (1:50,000-100,000 incidence) sex-linked neurodevelopmental disorder that includes severe intellectual disability, dysmorphic features including facial and digital abnormalities, growth retardation, and skeletal changes. Most cases are sporadic with only 20% to 30% of cases having an additional family member. CLS is caused by variable loss of function mutations in the RPS6KA3 gene that maps to Xp22.2 and codes for the hRSK2 S6 kinase that phosphorylates the transcription factor CREB (cAMP response element binding protein) as well as other nuclear transcription factors. Phosphorylated CREB (pCREB) plays a major role in memory formation in fruit flies and mammals by activating specific genes through epigenetic histone acetylation.

Chapter 20 describes the way in which the support system provided by the traditional African extended family has given way in recent years to a new dependency by individuals on state organized health systems, charities, and private concerns. It discusses that the result has been increasing health costs and growing inequalities, and how it is now the poorest people who pay proportionately the highest amount for their healthcare out of their own pockets. The chapter sets out in broad terms the challenges faced by countries seeking to offer universal access to health and healthcare for their citizens. These are challenges that, as discussed in earlier chapters, require countries to find a way to meet these challenges that is based both on their culture and history, and on their financial capabilities and circumstances.

This essay focuses on the media’s response to sex crimes, how news reporting can affect people’s perceptions of sex offenders, and how all of these issues are connected to the development of law. The rate of sex crimes has been steadily declining for the past 20 years, and official rates of recidivism for sex offenders are actually low compared to other types of offenders, but the media tend to ignore these facts. Many people become oversaturated by the news, and continuous reporting can cause panic in viewers. The type of information that the media report increases people’s concern that they, or a close family member, might become a victim of a sex offense. With the fear increased—making the topic relevant in our culture—media outlets then spend more time reporting on such issues because people are interested and the stories will sell.

This chapter describes the evolution of collaborative mental health care in Canada over the past 15 years, and the ways in which integrated care is becoming an increasingly integral part of Canada’s provincial and territorial healthcare services. It explores the underlying principles and models that can be found across the country. There is a particular emphasis on three things: (1) changes any mental health service can make to improve collaboration, (2) programs to increase the mental health skills and capacity of primary care, and (3) the integration of mental health services within primary care.A program in Hamilton, Ontario, has successfully integrated mental health counselors and psychiatrists into the offices of 170 family physicians across a city of 500,000 people for the past 20 years. The authors present data from the program’s evaluation, as well as key lessons learned and advice for other programs looking to set up similar models.

The bowerbirds (family Ptilonorhynchidae) are famed for their unique bower-building behaviour. In some species, the bower can be a complex construction of sticks and other vegetable matter that can grow to two metres in diameter and one-and-a-half metres high. Many species are also accomplished mimics, and are able to copy the calls of other bird species, other natural and mechanical sounds and even human speech. The bowerbirds are confined to Australia and New Guinea and, due to the difficulty in accessing certain areas of their distribution, the study of their habits has been challenging. The 20 existing species are almost equally divided between the two regions, with eight species endemic to Australia, 10 to New Guinea and two species occurring in both. Bowerbirds condenses published knowledge into a format that will suit natural history enthusiasts at any level. While the emphasis is on Australian members of the family, with detailed accounts on each of the 10 species, the New Guinea representatives are discussed in general chapters and are included in a supplemental section that covers key areas such as breeding, identification and distribution. Behavioral health disorders also have generally stronger “cross-domain” effects, exacerbating the diagnosis and treatment of many physical disorders and chronic medical conditions. In addition to the persistent stigma surrounding behavioral health disorders, there are issues of reimbursement, costs of care, availability of care based on provider and/or geographic availability, policy limitations, formulary restrictions, transportation, provider knowledge, patient knowledge, attitudinal/evaluative barriers, medication compliance, family support (or lack of it), inconvenience or inability to obtain an appointment, and refusal of treatment. This chapter provides an overview of the issues surrounding behavioral health care from a population health perspective, including pharmaceutical treatment and competencies for community-based pharmacists.

This chapter provides an overview of current knowledge regarding the frequency, implications, and aetiology of comorbidity between ADHD and reading disorder (RD), a common childhood disorder that is defined by significant underachievement in reading. Results from community studies indicate that 20–50% of individuals with ADHD also meet criteria for RD, and the presence of comorbid RD is associated with increased functional impairment and less positive long-term outcomes. Family and twin studies indicate that RD and ADHD are both significantly familial and heritable, and multivariate analyses indicate that nearly all of the phenotypic covariance between RD and ADHD is due to shared genetic influences that are associated with slower and more variable cognitive processing speed. Key future directions include studies that incorporate a broader range of measures of reading and more sophisticated neuropsychological and neuroimaging phenotypes, along with studies of the treatment implications of comorbidity between ADHD and RD.

Resumen

Major depressive disorder is a serious, debilitating, life-shortening illness that affects many persons of all ages and backgrounds. The point prevalence is high (2.3%–3.2% in men, 4.5%–9.3% in women) and the lifetime risk is 7% to 12% for men and 20% to 25% for women. Major depression is a disabling disorder that costs the United States over $200 billion per year in direct and indirect costs. Depression also has detrimental effects on all aspects of social functioning, such as self-care, social role, and family life, including household, marital, kinship, and parental roles. While there have been several treatments that are efficacious, many individuals suffering from depression experience life-long challenges due to the often chronic and episodic nature of the disease. Identifying strategies to find the right treatments for the right patients is critical. Ongoing research has explored the importance of examining physiologic biomarkers as well as clinical characteristics to gain a better understanding of subtypes of depression, which will lead to improved treatments and better outcomes. This book provides an introduction to the etiology and pathophysiology of depression, common comorbidities and differential diagnoses, pharmacotherapy strategies, psychotherapeutic and neuromodulation interventions, novel and nontraditional treatment strategies, and considerations in special populations.
