Resumen
A Endothelial protective and antithrombogenic effects of Cimicifuga racemosa standardized extract (Klimadynon, Bionorica, Germany) in comparison with ethinylestradiol in experimental ovariectomy were investigated. Extract of Cimicifuga racemosa (klimadynon) contents phytoestrogens, that was the basis for studying of its effects in hypoestrogenic conditions for the purpose of prophylaxis and treatment of cardiovascular complications in climacteric syndrome as an alternatively of hormonal replacement therapy.The course administration of klimadynon (100 mg/kg p.o. daily during 14 days) decelerated the formation of intravascular thrombosis which was initiated by application of 10% FeCb solution on carotid artery. The ethinylestradiol in this conditions intensified prothrombogenic effect of application of 10% FeCh: blood flow in carotid artery stoped to 5 minutes of experiment and mass of the thrombus was on 28% more, than in control rats. For studying of possible mechanisms antithrombogenic action of klimadynon antiplatelet and vasodilator activity of endothelium were studied in rats after bilateral ovariectomy in comparison with effects of ethinylestradiol (25 µg/kg p.o. daily during 14 days). Treatment with klimadynon as well as ethinylestradiol increased the antiplatelet activity of endothelium, restoring its activity to level in sham-operated animals. The endothelium-dependent and endothelium-independence vasodilation by using of acetylcholine (5 µg/kg) and of sodium nitroprusside (30 µg/kg). respectively, were investigated. Coefficient of endothelium dysfunctions (CED) was estimated as ratio: area over curve of arterial pressure after sodium nitroprusside intravenous injection to value of this index after injection of acetylcholine. In rats after bilateral ovariectomy CED was increased by 66% in comparison with CED value in sharm-operated rats. The klimadynon and ethinylestradiol treatment decreased CED on 42% and 28% correspondingly in comparison with values of control group.In summary klimadynon has antytrombogenic and endothelial protective activities in the rats after ovariectomy. The ethinylestradiol decreases endothelial disfunction in ovariectomized rats, but unlike klimadynon demonstrates protrombogenic effect. The present findings suggest that klimadynon could be used as a therapeutic agent for menopausal vascular complication.

You study the influence of alternative therapies drug «Klimadynon®» in menopausal’nym patients with metabolic syndrome in the climacteric syndrome, weight, blood pressure, carbohydrate and lipid metabolism. Surveyed 38 patients aged 45 to 61 years (mean age 53,6±0,6 years) who were in natural menopause and had a series of violations within the framework of the metabolic syndrome (excess weight or obesity on abdominal’nomu type 2, type IMD dislipidemiu, arterial hypertension). Correction for climacteric disorders has been appointed an alternate method of treatment – «Klimadynon®» to 1 tablet 2 times daily. Estimated climacteric syndrome using modified menopausal index (E.V. Uvarova, 1983), carbohydrate, lipid Exchange, anthropometric parameters, level of HELL originally, after 3 and 6 months of initiating treatment. Against the backdrop of the reception of an alternative method of treatment with «Klimadynon®» we got the better of menopausal syndrome: in 13,3 % of heavy gravity menopausal syndrome has moved to middle level, in 52,6 % of the average degree of menopausal syndrome has moved into easy. Accurately noted improvements in the level of blood sugar on an empty stomach, reducing insulinorezisten- tnosti in women with menopausal’nym metabolic syndrome with therapy. «Klimadynon®» anticonvulsant therapy has statistically significant antiaterogennomu shift indicator of lipid Exchange in women. A tendency towards lower body mass index, waist, blood pressure. Thus, the application of the «Klimadynon®» in women with abnormal klimaksom and menopausal’nym metabolic syndrome could be a good option alternative therapies and significantly improve their quality of life.

Resumen
Due to the active professional activities of women aged 45-60, in the modern society, the after-care treatment of female patients with essential arterial hypertension (stages I-II , grades 1-2 ) under pressure of climacteric syndrome is of high social and economic importance. The specialists of medical rehabilitation are the problem of finding effective technologies of medical and non-medical series for improving the quality of life of women in this age group. This article presents the results of after-care treatment of women with essential arterial hypertension and mild or moderate climacteric syndrome (92 females aged 50±4.5) by means of the use of standard hypotensive pharmaceuticals (Candesartan, Bisoprolol, and Hydrochlorothiazide), non-hormonal pharmaceuticals (Klimadynon Uno), and spectral phototherapy as a physiotherapy approach. Such treatment resulted in reliable stabilization of blood pressure, vegetative functions, responsiveness, anxiety, encephalitic circulatory dynamics according to the REG data, and WAN indicators. The average clinical response of the control group amounts to 81.4 %, while that of the experimental group makes up 65.5 %. The key results are sustainable blood pressure stabilization, improved overall health, decreased frequency and severity of the climacteric syndrome along with stabilized vegetative functions, improved encephalitic circulatory dynamics, and psycho-emotional state.

Bakgrund: Menopaus definieras som tidpunkten för en kvinnans sista spontana menstruationsblödning. Cirka 65 000 svenska kvinnor passerar varje år sin menopaus och genomsnittsåldern för menopaus i Sverige och i övriga västvärden är 51-52 år. Substitutionsbehandling med östrogen och progesteron har länge använts för att lindra typiska menopausala symptom som värmevallningar och svettningar. Nyare studier visar att substitutionsbehandling med östrogen och progesteron ökar risken för att drabbas av kardiovaskulära sjukdomar och bröstcancer. Oro kring biverkningar liksom att kvinnor av annan anledning inte kan använda östrogen, har medfört ett ökat intresse för alternativbehandlingar som kan lindra menopausala symptom. En ny studie visar att mer än 50 % av kvinnor med menopausala symptom använder sig av någon sorts alternativbehandling för att lindra sina besvär. Extrakt av jordstam från silverax (Cimicifuga racemosa) har använts i Europa i mer än 40 år för att behandla klimakteriebesvär, och är idag den mest använda alternativbehandlingen. Syfte: I föreliggande litteraturstudie har syftet varit att undersöka om silverax kan minska upplevda vasomotoriska besvär, under perioden kring menopaus. Metod: Undersökningen är en litteraturstudie baserad på 5 randomiserade kliniska studier, hämtade från databaserna PubMed och Medline. Resultat: Silverax tycks vara ett effektivt behandlingsalternativ för att minska vasomotoriska symptom hos kvinnor som befinner sig i tidigt stadium av menopausen. Studierna visar en bra tolerans till extraktet utan allvarliga biverkningar. Diskussion: Fler stora randomiserade, kontrollerade och framförallt längre studier behövs för att man ska kunna dra slutsatser om effekten och säkerheten av silverax, och därmed visa om extraktet kan vara en effektiv behandlingsmetod vid menopausala symptom.
