Humans are increasingly disconnected from nature. Urbanization, resource exploitation, and changes in ways of living have diminished people's access to nature. Exposure to nature is beneficial to human beings in many aspects. Researches in environmental psychology and public health have shown the positive impacts of nature connections for people's happiness, concentration, and restoration. In creating the living environment with the connectedness to nature, various researches have been invested, such as the study of green space in the living environment, the application of virtual nature in psychiatric and medical care, the implementation of natural scenery in augmented reality. However, the idea of imitating natural phenomena in the built environment via tangible building systems has not been explored yet. This thesis aims to provide people with the perception of connectedness to nature in the built environment by embedding the sensory experience of nature, Komorebi, in the building system. Komorebi is a Japanese term that describes the dappled sunlight filtered through tree foliage. Through analysis of this visual effect and experimenting with various materials and actuators, a daylight-filtering system is developed to bring the dappled light phenomenon into the built environment. Environmental performance simulations of the Komorebi system is conducted in comparison with no-shading and the Venitian blind. The system builds on the existing infrastructure to integrate elements of improvisational nature into the building system, creating natural sensory experiences in the built environment. In practice, it would have great potential at places where natural connections are limited, and relinking occupants to nature would be highly beneficial. The impact of this work includes 1) creating a port for people who have limited access to nature due to work demand or mobility limitation, 2) invoking people's memories in nature, and encouraging more exposure to nature.

Texto completoObjective: Patients with ST-elevated myocardial infarction (STEMI) treated by primary angioplasty (pPCI) will develop left ventricular (LV) remodeling in one third of the cases. The purpose of this study is to determine early predictors of LV remodeling after acute STEMI and pPCI. Methods: 210 patients with a first acute anterior STEMI treated by pPCI were included. All participants underwent echocardiography in the first 24hrs and again after 6 months, after which they were divided into two groups: remodeling (n=55; 26%) and nonremodeling (n=155; 74%). Results and discussion: The most powerful independent early predictors were: "no reflow" after pPCI (OR=30.0 95% CI, p<0.0001), diastolic dysfunction in the first 24hrs (OR=27.7 95% CI, p<0.0001), increased diameter of the left atrium - LA (OR=5.0 95% CI, p=0.044) and at admission Killip class 2-4 (OR=3.4 95% CI, p=0.003), by multivariant regression analysis. Also, strong predictors were incomplete ST-resolution - STR (OR 2.0 95% CI, p=0.024) and Wall motion score index - WMSI >2 (OR 21.6 95% CI, p<0.0001), by univariant regression analysis. Total ischaemic time had no influence on LV remodeling. The group with remodeling had more frequent major adverse cardiac events (MACE) during one year follow-up: re-hospitalisation (61.8% vs 22.6%; p<0.0001) / mostly re-hospitalisation due to heart failure (40% vs 2.6%; p<0.0001) and mortality 5.5%; reinfarction (20% vs 7.1%; p=0.007); recoronagraphy (45.5% vs 18.1%; p<0.0001); revascularisation (30.9% vs 11%; p=0.001). Conclusion: For the patients with a first acute anterior STEMI, treated by pPCI, development of "no reflow" after pPCI, diastolic dysfunction, increased LA and heart failure on admission are the most powerful early independent predictors for LV remodeling. Incomplete STR and WMSI>2 are strong predictors too. Remodeling patients will have a more frequent incidence of MACE and mortality.

Texto completoObjective: The purpose of this study was to determine the influence of changes in function, morphology, and geometry of the left ventricle (LV) and a non-dipping arterial blood pressure (BP) pattern on the intrauterine growth restriction (IUGR) and preterm delivery in pregnant women with gestational hypertension (GH), reversibility of these changes after delivery and connection between BP in family with GH. Methods: This prospective study included 90 pregnant women, 30 normotensive, 30 with GH and dipping BP pattern and 30 with GH and non-dipping BP pattern. All participants underwent a complete two-dimensional, pulsed and tissue Doppler echocardiography and 24-h ambulatory blood pressure monitoring in the third trimester and 6 weeks after delivery. Results and discussion: Participants with GH had more impaired parameters of the LV systolic, diastolic and global function (EF, s’, E, A, E/A, E/e’, DTE, IVRT, IVCT, ET, Tei index, CO, CW, Ees), morphology (IVSd, PLWd, RWT, myocardial mass, p<0,0005) and geometry (abnormal geometry 67,7% vs 3,3% in normotensive, p<0,0005). The greatest changes were noticed in non-dippers. All changed echocardiographic parameters became improved, while 96,7 % non-dipper participants became dipper after delivery. Arterial hypertension in family was present in 80% women with GH vs 26,7% in normotensive (p<0,0005). Analyses revealed that maximum night-time diastolic BP, mass index and total vascular resistance were identified as independent predictors of IUGR. Average systolic night-time BP, mass index and EF were identified as independent predictors of preterm delivery. Conclusion: Changes in LV function, morphology and geometry and a non-dipping pattern of BP in GH predicts IUGR and preterm delivery.

Texto completoCardiac surgery operative risk assessment in patients with imapired systolic left ventricular function using cardial biomarkers Evaluation of results in cardiac surgery involves monitoring the outcomes of operative treatment in a given time period. Typically, this interval includes 30 days from the date of operation. The most common criteria used for monitoring are the rate of mortality and morbidity, length of stay in the intensive care unit, the total length of hospitalization and medical costs. Risk stratification means that patients can be divided into groups depending on the number and importance of preoperatively identified risk factors, and that the outcome of surgery for each of the patients can be predicted preoperatively. In Europe, in the period of 1995-1999 on the basis of a multi-center study in 8 European countries and 128 cardiac centers in which 19,030 adult patients were operated on, EuroSCORE (European System for Cardiac Operative Risk Evaluation) model for risk stratification in cardiac surgery was developed. However, the inevitable changes and progress in the surgical treatment rendered the EuroSCORE model obsolete warranting updated system. It was in 2012 when a new system EuroSCORE II was introduced into practice At the Clinic for Cardiac Surgery of the Institute of Cardiovascular Diseases, EuroSCORE model was introduced in routine clinical use since the beginning of 2001. By analyzing the results, two years after application, it was shown that the model was accurate, and that there was no significant difference between the expected (3.7%) and the actual mortality (3.47%) In recent years, in patients who are candidates for cardiac surgery, more attention is paid to cardiac biomarkers in terms of evaluating their predictive power. The most significant biomarkers in cardiovascular medicine are: Troponin, creatine kinase MB isoenzyme (CKMB), N-terminal pro B-type natriuretic peptide (NT-proBNP), C-reactive protein (CRP), Lactate dehydrogenase (LDH), and uric acid (Uric uricum). The objectives of this study were to create a model to predict preoperative risk for cardiac surgery patients with impaired systolic left ventricular function on the basis of preoperative levels of certain biomarkers and to create a new model with a combination of the previous model and already existing EuroSCORE II model. The study included 704 patients with impaired systolic left ventricular function, ejection fraction less than or equal to 50%. All patients underwent cardiac surgery at the Institute of Cardiovascular Diseases, from January 20th 2014 until 20th April 2016. Patients were submitted to three types of operations: revascularization - coronary surgery, surgery of acquired heart defects - valvular surgery and combined operations. Following biochemical analyses were performed 24 hours prior to surgery: troponin I, creatine kinase, creatine kinase MB isoenzyme, mass creatine kinase, lactate dehydrogenase, C-reactive protein, NT-proBNP and uric acid. Postoperative mortality, postoperative onset of myocardial infarction and occurence of cerebrovascular accident and their correlation with preoperative values of listed biomarkers were registered. The study included all patients with acquired heart disease, older than 18 years, with the left ventricular ejection fraction less than or equal to 50% who were submitted to the following types of operations: revascularization - coronary surgery, surgery of acquired heart diseases - valvular surgery and combined operations - coronary and valvular surgery. The results showed that the postoperative mortality was 3.13%, new onset of postoperative myocardial infarction was detected in 7.95% of the patients and postoperative cerebrovascular accident developed in 9.23% of patients. Correlation of preoperative biomarkers values with postoperative myocardial infarction in patients with impaired left ventricular ejection fraction - elevated preoperative troponin I were associated with postoperative myocardial infarction. Correlation of preoperative biomarkers values with postoperative cerebrovascular incident occurence in patients with impaired left ventricular ejection fraction - elevated preoperative troponin I and CRP were associated with postoperative cerebrovascular incident. The influence of preoperative levels of all biomarkers, separetly, on the rate of significant adverse cardiac and cerebrovascular events - Major Adverse Cardiac and Cerebrovascular Events (MACCE) as the heart surgery outcome, in patients with impaired left ventricular ejection fraction. The following results were obtained: Increased preoperative levels of C-reactive protein (CRP) and postoperative MACCE were related. Increased preoperative levels of lactate dehydrogenase (LDH) and MACCE were related. The conclusions of this thesis are: 1. Independent predictor of postoperative myocardial infarction onset and significant adverse cardiac and cerebrovascular events in cardiac surgery patients with impaired systolic left ventricular function (ejection fraction less than or equal to 50%) is elevated preoperative value of troponin I. 2. Preoperative Troponin I value was poor marker for predicting postoperative myocardial infarction and significant adverse cardiac and cerebrovascular events in cardiac surgery patients with impaired systolic left ventricular function (ejection fraction less than or equal to 50%). 3. None of the studied variables showed influence on the postoperative cerebrovascular accident occurence, in cardiac surgery patients with impaired systolic left ventricular function (ejection fraction less than or equal to 50%). 4. Independent predictors of postoperative mortality in cardiac surgery patients with impaired systolic left ventricular function (ejection fraction less than or equal to 50%), that could be used to create a predictive model are: age and elevated preoperative value of NT-proBNP. 5. Developed model showed satisfactory results for predicting outcome after heart surgery in cardiac surgery patients with impaired systolic left ventricular function (ejection fraction less than or equal to 50%). 6. Elevated preoperative value of NT-proBNP may be a good marker for mortality prediction after the cardiac surgery in patients with impaired systolic left ventricular function (ejection fraction less than or equal to 50%). 7. EuroSCORE II model showed poor performance when predicting outcomes after cardiac surgery in patients with impaired systolic left ventricular function (ejection fraction less than or equal to 50%). 8. Validation of the newly-created model, considering low and medium risk patients, based on the value of left ventricular ejection fraction, showed that the model is a good marker for the mortality prediction in both groups.

Resumo: O tema desta pesquisa é a produção social de um santo católico. Para desenvolvê-la, parto de alguns pressupostos objetivando orientar minhas análises. Entendo que a produção de um santo, mais do que um fenômeno jurídico-teológico, possui uma dimensão social articulando e determinando a dinâmica deste processo. Para analisar esta questão, ocupo-me com o caso da produção da crença e da devoção ao padre salesiano polonês Rodolfo Komórek que viveu os últimos nove anos de sua vida em São José dos Campos, Estado de SP. Foi nesta cidade que, após sua morte em 1949, floresceu de forma efetiva sua santidade. Vários sãos os grupos envolvidos no processo de produção do santo, cada qual com seus interesses, objetivos e maneiras de participação. De forma clara existem dois que são principais: os especialistas do sagrado (representados por membros da Congregação Salesiana de São José dos Campos) e os devotos leigos. Os primeiros produzem uma série de bens simbólicos que eles mesmos distribuem para os fiéis objetivando encaixar o santo e a devoção a ele rendida nos moldes determinados pela Teologia. Os seguintes, mais do que meros consumidores desses bens, também são produtores ativos, pois, re-significam estes bens adaptando-os conforme suas necessidades contextuais. A primeira grande conclusão a que se chegou, a partir do estudo desta relação dialética entre especialistas do sagrado e leigos, é de que os bens simbólicos produzidos neste processo seguem um sentido de mão dupla, pois são gerados pela Igreja e lançados num campo social específico, depois são absorvidos pelos leigos, re-significados de acordo com as necessidades contextuais dessas pessoas e exteriorizados sob diversas formas, a mais óbvia é o próprio ato devocional. A segunda conclusão é de que cada etapa deste processo necessita da outra para que seu produto final seja efetivo. Este é um sistema que se retroalimenta e que também possibilita aumentar o capital simbólico dos produtores desses bens. Todas estas sanções e rituais de consagração que marcam as etapas pelas quais são atribuídos os bens simbólicos geram na verdade um sentido espiralar, ou seja, um sentido crescente na legitimidade do santo e também dos fazedores de santo.
Abstract: The theme of thios research refers to the social production of a catholic saint. To develop it, I start from some previous concepts aiminy to lead then to may analysin. I comprehend that the production of a saint, more than a theological-juridic phenomenon, also has a social dimension articulating and determining the dynamic of the whole process. To analyse this question, I occupy fully myself in the production of the belief and also in to the devotion to salesian Polish priest Rodolfo Komórek who lived his last nine years in São José dos Campos, São Paulo, Brasil. It was after his death in 1949, that his sanctity blossomed effectively in this town. Many groups are envolved in the process of the production of the saint, each group with their own interests and objectives. Ther two main groups: the experts on the sacredness and the non-experts. The first ones produce several symbolic assets which are distributed to the faithful people aiming to fit the saint ando devotion to him on the patterns determined by the Theology. The second group, more than consumers, are also manufacturers since they resignify these asset, adapting according to the context needs. The first great conclusion that came from this study of dialectual relationship between the sacredness and the layman, is that the symbolic assets manufactured in this process fallow two way traffic, since they are generated by the Church and launched into the specific social field, after that absorved by the laymen, resignified according to the contextual needs of the people and expressed on different ways, the most obvious one is the devotion act itself. The second conclusion is that each stage process needs one another to come to the final effective product. The system feeds it self and also makes possible increase the symbolic capital assets of the manufacturers. All these sanctions and rituals of the consecration that mark the stage on the symbolic assets are attributed and they generate, in fact, a coil sense, in other words, a crescentic sense of legitimating of the saint and the making saints.
