INTRODUCTION:Newly developed antineoplastic drugs have resulted in improvements in morbidity and mortality from many forms of cancers. However, some of these new chemotherapeutic agents have potentially lethal side effects, which are now being exposed with their widespread use. Gemcitabine is a nucleoside analog, which is a commonly used agent for various solid organ malignancies. Phase 1 and 2 trials with gemcitabine did not show significant risk for cardiotoxicity
however, with its widespread clinical use over the last decade, a few cases of cardiotoxicity related to gemcitabine use have been reported. Cardiomyopathy after the use of gemcitabine monotherapy is extremely rare
and only one such case has been reported in detail previously.CASE PRESENTATION:We report a case of a 56-year-old African American man with pancreatic cancer who presented with signs and symptoms of congestive heart failure after being treated with gemcitabine for two cycles (six doses). A two-dimensional echocardiography showed left ventricular ejection fraction of 15 to 20 percent with global hypokinesia. With the absence of significant risk factors for coronary artery disease and a strong temporal relationship with the initiation of chemotherapy, it was concluded that our patient's cardiomyopathy was related to the use of gemcitabine. Gemcitabine was discontinued and our patient responded well to standard heart failure therapy. Two months later, a repeat echocardiogram showed significant improvements in left ventricular systolic function.CONCLUSIONS:Gemcitabine should be considered as a potential cause of cardiomyopathy in patients receiving chemotherapy with this drug. We need further studies to look into potential mechanisms and treatments of gemcitabine-induced cardiac dysfunction.

Patients with relapsed and refractory multiple myeloma have poor prognosis. A recent analysis of patients with relapsed and refractory multiple myeloma who were refractory to both proteasome inhibitors and immunomodulatory drugs showed the median overall survival of 9 months only. Daratumumab is the first-in-class human monoclonal antibody against CD38 cells which was studied in phase I/II trials for treatment of these patients with relapsed refractory multiple myeloma. It showed an overall response rate of 36% and a median overall survival (OS) of 17 months in these patients. We report a case of 40-year-old man with immunoglobulin D (IgD) multiple myeloma whose disease was refractory to at least 5 different chemotherapy regimens including proteasome inhibitors and immunomodulatory drugs. The clinical studies assessing daratumumab did not include any patients with IgD myeloma which is a rare form of multiple myeloma and to our knowledge is the first study reporting use of daratumumab in IgD myeloma.

Section 1 :Literature Review This review explores the recent research on the nature of fear of childbirth with a focus on methods of assessmen.t and psychological intervention. Using defined criteria, thirty-five articles were identified for review. Fear of childbirth was shown to have implications for the emotional well being of women both in the antenatal and postnatal period. A number of personal and social factors were reported to influence fear. Fear of childbirth was found to be a distinct dimension of anxiety and therefore measures of general anxiety lack validity. Studies investigating the impact of psychological interventions to treat childbirth fear show initial promise however methodological flaws limit conclusions. Findings are explored in relation to implications for women in the UK. Section 2: Research Report The expectations of childbirth that women develop during pregnancy are reported to influence the actual experience of childbirth. Existing measures of childbirth expectations have limited robustness and cultural validity for pregnant women in the UK. The aim of this study was to develop a reliable and valid English-language based scale to measure pregnant women's expectations of childbirth. Items were generated via exploratory interviews with members of the target population. An initial version of the scale was developed and piloted with a large sample of pregnant women. Questionnaire data was subjected to item analysis and principal components analysis revealed six underlying factors. A final version of the scale was developed and initial assessment of internal reliability and validity were carried out.

BACKGROUND:Some patients with mild or moderate thoracic scoliosis (Cobb angle <50-60 degrees) suffer disproportionate impairment of pulmonary function associated with deformities in the sagittal plane and reduced flexibility of the spine and chest cage. Long-term improvement in the clinical signs and symptoms of childhood onset scoliosis in an adult, without surgical intervention, has not been documented previously. CASE PRESENTATION:A diagnosis of thoracic scoliosis (Cobb angle 45 degrees) with pectus excavatum and thoracic hypokyphosis in a female patient (DOB 9/17/52) was made in June 1964. Immediate spinal fusion was strongly recommended, but the patient elected a daily home exercise program taught during a 6-week period of training by a physical therapist. This regime was carried out through 1992, with daily aerobic exercise added in 1974. The Cobb angle of the primary thoracic curvature remained unchanged. Ongoing clinical symptoms included dyspnea at rest and recurrent respiratory infections. A period of multimodal treatment with clinical monitoring and treatment by an osteopathic physician was initiated when the patient was 40 years old. This included deep tissue massage (1992-1996); outpatient psychological therapy (1992-1993); a daily home exercise program focused on mobilization of the chest wall (1992-2005); and manipulative medicine (1994-1995, 1999-2000). Progressive improvement in chest wall excursion, increased thoracic kyphosis, and resolution of long-standing respiratory symptoms occurred concomitant with a >10 degree decrease in Cobb angle magnitude of the primary thoracic curvature. CONCLUSION:This report documents improved chest wall function and resolution of respiratory symptoms in response to nonsurgical approaches in an adult female, diagnosed at age eleven years with idiopathic scoliosis.

Many sentinel events in acute care centers can be attributed to miscommunication of patient information at shift change. A growing body of evidence supports the implementation of bedside shift report as the standardized approach to ensure effective communication by staff. The purpose of this study was to identify the barriers that impede nurses from fully participating in bedside shift report in order to create an educational initiative to develop the nursing staff's proficiency in performing bedside report. Lewin's change theory served as the framework for the project. Data for the staff development project were collected using a focus group approach with 18 nurse participants. A classic method of analysis, defined and outlined by Krueger and Casey (2009), was used to identify themes and categorize results. A second evaluator of the data supported thematic findings. Nurses revealed barriers consistent with the literature including frequent interruptions, patient confidentiality, sensitive issues, and inconsistent report content. Barriers identified by this study, but not evident in the reviewed literature, included staff unavailability to answer call lights and having to receive report and transfers during report. An education plan was developed based on perceived barriers and evidence in the literature. Implications for positive social change include interventions to refine the current practice of bedside shift report in order to promote effective and efficient communication at change of shift. Findings can inform nursing units in the hospital and other facilities to achieve patient-centered care and improved outcomes. It is critical that nurses understand the positive impact that consistent, timely, and effective shift reports have on providing safe, quality patient care.

Background and Significance: The utilization of IVC filters for pulmonary embolism prevention has increased significantly over the past decade as the indications continue to expand. Although the risks associated with IVC filters are small, a well‐known complication is filter fracture and subsequent embolization of the fragment. Case reports have been published on the devastating effects of fragment migration to the heart, causing intense chest pain, pericardial effusion, cardiac tamponade and death. Research Question: There is a paucity of experience and guidelines for treating patients with a metallic foreign object lodged within the heart. Is there a consensus on the proper management of these cases? How do these patients present and what are the outcomes of treatment? Some clinicians have chosen to observe and monitor, while others have gone to the operating room for open‐heart surgery and retrieval of the fragment. Methods: In an attempt to answer these questions, a systematic review of the published literature was conducted between 1985 and 2015. Only articles related to IVC filter fracture and subsequent fragment migration to the heart were included. The clinical presentation, workup, management, treatment and outcomes were collected as available. Results: A total of 23 articles were published consisting of a prospective study, retrospective series and case reports. There were 37 migrated fragment to the heart reported in 29 patients. The most common clinical presentations were chest pain (69.0%) and no symptoms (27.6%). Regarding treatment, ten patients underwent observation, three had successful endovascular retrieval, 12 went to the operating room for open‐heart surgery and four cases were unreported. Of the 12 patients with reported pericardial effusion, 11 (91.7%) underwent open surgical repair. Of the eight asymptomatic patients, seven (87.5%) were ultimately in observation and the management of the other was unreported. Conclusions: There appears to be a consensus in the literature that observation and close follow up are appropriate options for asymptomatic patients. Symptomatic patients with pericardial effusion may benefit from open‐heart surgery. Cardiovascular compromise such as cardiac tamponade should be managed with open surgery. Based upon these findings and other details in the cases, we have proposed a management algorithm.

This thesis reports on a study which developed a profile of the reviewers of children's books in Australia. It then compared the profile with one which was developed by Kathleen Craver in 1984 of children's book reviewers in the United States. Five research questions were addressed by this study relating to reviewers and their opinions regarding review aspects, reviewer roles and review practices within the framework of their personal and professional background. Craver surveyed the reviewers from School Library journal because as a group, they provided the greatest potential for statistical significance of all the reviewing journals in the United States. As no Australian journal enjoys either the number of reviewers or the circulation of School Library journal, reviewers from eight journals which are most used by teacher and children's librarians were selected to form the population for this research. These journals are Fiction Focus, LINES, Magpies, Reading Time, Reviewpoin t, Review Bulletin, Scan and Tasmanian Resources Review. The reviewer profile which emerged from this study was very similar to Craver's in that it was not one which could be entirely defined in terms of group characteristics. Selected cross tabulations either with the particular journals, demographic details, or other variables, particularly those relating to reviewer experience failed to produce predictable behaviourial correlations. However, reviewers were united on certain issues which can be attributed to their own professional background.

Recent research indicates that children with language impairment (LI) often experience difficulties with social communication. Although the empirical basis for general social communication intervention is growing, information documenting the efficacy of these interventions for children with LI remains limited. The purpose of this study was to examine the effectiveness of a social communication intervention on teacher perceptions of sociability in five elementary-aged children with LI. The intervention focused on the presentation and use of children's stories to target aspects of emotion understanding. The two sociability subscales of the Teacher Behavior Rating Scale (TBRS), impulse control/likeability and prosocial behavior, were used as variables with which to measure teacher perception. Pre and post intervention measures of teacher ratings were taken and compared for each participant. Results indicated that four of the five participants received higher ratings for prosocial behaviors following treatment, with two participants scoring within typical range for their age. Two participants remained stable in their pre and posttreatment scores for impulse control/likeability, one participant increased in their ratings, and two of the participants had a decline in posttreatment scores for impulse control/likeability. This study revealed noteworthy improvements in prosocial behaviors in children with LI, even while problems with impulse control remained or increased. This was to be expected considering the intervention focused on emotion understanding which leads to prosocial behavior, whereas the intervention did not focus explicitly on impulse control. Implications of these results were discussed and suggestions for further research were offered.

Research shows that children with language impairment (LI) often have deficits in social communication. The purpose of this study was to examine the effectiveness of a social communication intervention targeting emotion understanding on teacher perceptions of withdrawal in six elementary-aged boys with LI. The intervention incorporated four elements into treatment sessions: a) Story sharing of children's literature that was rich in emotional content and prosocial behaviors; b) Story enactment of the same children's literature; c) identification and discussion of pictures of facial expressions; and d) journaling to help internalize and reflect on emotions and social behaviors targeted. Two withdrawal subscales of the Teacher Behavior Rating Scale (TBRS), reticence and solitary-passive behavior, were used as variables with which to measure teacher perception. Pre and postintervention measures of teacher ratings were taken and compared for each participant. Results indicated that four of the six participants improved in ratings of withdrawn behaviors following treatment. Two of the boys improved 2 SD for ratings of reticence. Two additional participants did not show improvement, but remained stable in their pre and posttreatment scores for both reticence and solitary-passive withdrawal. These results indicate a generally favorable outcome of the social communication intervention with regard to social withdrawal in children with LI.

BACKGROUND: Left ventricular outflow tract (LVOT) obstruction and paravalvular leak (PVL) are relatively uncommon, but are serious complications of prosthetic valve replacement. CASE PRESENTATION: We present a case that displays the unique therapeutic challenges of treating a patient who developed both LVOT obstruction and mitral PVL after undergoing surgical aortic and mitral valve replacement (MVR). We also describe the use of alcohol septal ablation and albumin-glutaraldehyde (BioGlue) for septal ablation to percutaneously treat the patient's LVOT obstruction, followed by use of an Amplatzer vascular plug for percutaneous closure of an antero-medial mitral PVL associated with severe regurgitation. CONCLUSION: Percutaneous interventional management of these entities may be considered as an initial therapeutic option, especially in high-risk patients with significant morbidity and mortality of repeat surgical operations.

A 65-year-old gentleman presented with left groin swelling over the course of two months. Physical exam revealed nontender left inguinal adenopathy, and computed tomography scans detected multiple lymph nodes in the mesenteric, aortocaval, and right common iliac regions. An excisional lymph node biopsy was performed. Pathologic evaluation demonstrated follicular center site which stained positive for PAX5, CD20, CD10, Bcl-2, Bcl-6, and mantle zone cells. These findings demonstrated CCND1 and CD5 positivity, suggesting composite lymphoma comprising follicular lymphoma (FL) with in situ mantle cell lymphoma (MCLIS). FL is known as indolent non-Hodgkin lymphoma; however, the clinical significance of a coexisting MCLIS continues to be elusive, and optimal management of these patients remains largely unknown. This case illustrates the diagnostic and therapeutic challenges of composite lymphomas. This paper also discusses advances in molecular pathogenesis and lymphoma genomics which offer novel insights into these rare diseases.

We present an unusual case of a late recurrent central neurocytoma that was rediagnosed as an ependymoma and neurocytoma in accordance with changes in histological classifications. Case Description. A 56-year-old male teacher presented with incomplete transverse syndrome due to several intradural extramedullary tumors at the level of lumbar vertebrae 1–3. The histological diagnosis at the time was atypical ependymoma. One year later, two additional tumors were removed at the L5-S1 vertebral level. For 12 years, the patient remained tumor free on followup. Fourteen years after the initial diagnosis, the patient presented with thoracic paresthesias due to two new extramedullary tumors in the C7-T1 and the T8-T9 vertebral levels. After complete removal of the tumors, a radiological survey revealed an intracranial lesion in the third ventricle. Five months later, an additional lesion recurrence was removed surgically. The most recent histological diagnosis revealed an atypical central neurocytoma. In retrospect, the previous tumors were reclassified as neurocytoma according to the additional immunohistochemistry evidence. Discussion. There is no standard adjuvant treatment regimen for atypical neurocytoma; therefore, the patient is currently under close followup. Modern histopathological diagnosis is essential in these cases. Potential routes for dissemination of the tumor should be considered upon first recurrence.

Children with language impairment (LI) often demonstrate difficulties in social communication. Although a number of general social communication interventions have been suggested, there is relatively little work done to examine the efficacy of these interventions for school-age children with LI, and none reported to target general emotional competence. The purpose of this study was to examine the effects on teacher perception of an intervention designed to improve emotion understanding. The intervention was centered on the presentation and use of children's stories to introduce and practice aspects of emotion understanding. The withdrawn and sociable subscales of the Teacher Behavior Rating Scale (TBRS) were selected as variables on which to measure teacher perception. Following treatment five of the six participants scored higher ratings of prosocial behavior, with two demonstrating overall reductions in withdrawn behavior and increases in sociable behavior. For one of these participants, the reported progress was notable. The most positive indicator of change following treatment was the reduction in solitary-active withdrawal behavior reported for three of the participants. A reduction in this type of behavior would most likely have an important impact on the quality of social interactions experienced by these individuals.

My Bachelor’s Thesis is a comparative analysis that identifies a paradigm shift based on how imagining and portraying futuristic technology and human - computer (or machine) interaction within science fiction works and explore how they depict the technology and the future thinking of their own era. I use two very popular and influential works from two different eras: 2001 A Space Odyssey from the late 1960s and Minority Report from the early 2000s. In the first part of this analysis, I analyze the technology and human interaction with technology in 2001: A Space Odyssey and argue what this tells about the technology thinking of the late 1960s, the high time of the Space Race. During the second part, the analysis continues with the other primary source, Minority Report. However, in this part I make direct comparisons with 2001: A Space Odyssey in order to illustrate the paradigm shift with clear examples.

Bakgrund : Delaktighet och personcentrerad vård är centrala begrepp i den vård som bedrivs idag. För att patienten ska vara delaktig krävs det att patienten är mer involverad i de dagliga händelserna som till exempel skiftrapportering. Vid varje skiftbyte sker överrapportering från en sjuksköterska till en annan, överrapporteringarna saknar ofta patientmedverkan. En rapporteringsmodell är bedsiderapportering vilket innebär att rapporteringen sker i patientens närvaro. Syfte : Syftet var att beskriva patientens delaktighet i vården vid bedsiderapportering. Metod : En litteraturöversikt. Resultat : Bedsiderapportering fick patienterna att känna sig delaktiga i vården, vilket delvis berodde på att de kände sig som värdefulla deltagare. Patienterna betraktades i första hand som en person. Både patienter och sjuksköterskor ansåg att rapporteringssättet var avgörande för patientens delaktighet i vården. De flesta patienter uppskattade att få vara med i rapporteringen och få viktig information som de ansåg sig ha rätt till. Dock ville inte alla patienter delta, vilket bland annat berodde på att rapporten ansågs vara till för sjuksköterskor. Slutsats : Bedsiderapportering ökade patientens delaktighet i vården enligt både patienten och sjuksköterskan. Dock ville inte alla patienter delta i rapporteringen och graden av delaktighet var beroende av på vilket sätt rapporten gavs.
Background : Participation and person-centered care are key concepts in the care that are being given today. In order for the patient to participate, they must be involved in the daily work such as shift reporting. At each shift, reporting occurs from one nurse to another which often lacks patient involvement. A reporting model is bedside report, which means reporting takes place in the patient's presence. Aim : The aim of this study was to describe patient’s participation in care during bedside report. Method : A literature review. Results : Bedside report made patients feel more involved in their care, partly because they felt valued as a participant in the report. The patients were considered a person in the first hand. Both patients and nurses felt that the reporting method was crucial for the participation. Most patients appreciated being included in the reporting and that they received valuable information that they considered justifiable. However, not all patients wanted to participate, which was due, among other things, to the fact that the report was considered for the nurses. Conclusion : Bedside report increased patient involvement in care, according to both the patient and the nurse. However, not all patients wanted to participate and the reporting method were important for the level of participation.

This essay has been carried out with the purpose of investigating the differences between teaching in a CLIL program and in a standard program. The results of this research are based on the perceptions of five teachers working in both programs. These teachers participated in a semi-structured group interview where they were asked questions and were given the opportunity to develop the discussions among themselves. The results have shown that there are fewer differences between these two programs than I originally thought, but the differences that do exist were unexpected. All of the teachers who participated in the study had taught English before they started to work in the IB, but are not trained CLIL teachers. Since there were only five participants and these participants are not trained CLIL teachers, this investigation does not show a general result, but instead gives the picture of how working with CLIL and a standard program simultaneously could work.

"Um dia no cais", by João Antônio, was firstly published in the magazine Realidade under the categorization of "short story-report". This designation makes us suppose a mixture, in text, of two different textual genres originated from literature and journalism: the short story and the story-report. This study investigates the textual resources that allowed the term, unknow of at that time, to be created and designated to the aforementioned text. Besides, our aim is to discuss the elements that approach the term and, at the same time, make it distance it from an assertive use above its supposed “subgenre” status. To get the results, we used theoretical concepts that define both short story and report in such a way to intersect them. And in order to consider the environment and the context in this analysis, we take the magazine Realidade and the conditions of Brazil (military dictatorship and the incipient development of a Brazilian cultural industry) into consideration at the time of the publication of the text in question, also intending to understand the emergence of this term that still characterizes "Um dia no cais" as the first Brazilian "short story-report".
“Um dia no cais”, de João Antônio, foi publicado na revista Realidade sob a categorização de “conto-reportagem”. Tal designação nos faz supor a mistura, em texto, de dois diferentes gêneros textuais oriundos da literatura e do jornalismo: o conto e a reportagem. A dissertação de que trata este resumo pretende investigar quais recursos textuais permitiram que o termo, inédito à época, fosse criado e designado ao texto supracitado e, para além de seu estatuto de suposto “subgênero”, discutir quais elementos o aproximam e o distanciam de um uso assertivo. Para a obtenção dos resultados pretendidos fazemos uso de teorias que se propõem a encontrar maneiras de definir tanto o conto quanto a reportagem, procurando nelas um caminho possível de intersecção. E para que também sejam considerados nessa análise o meio e o contexto, levaremos em conta a revista Realidade e as condições do Brasil (a ditadura militar e o incipiente desenvolvimento de uma indústria cultural brasileira) no momento de publicação do texto em questão, também intencionando entender o surgimento deste termo que ainda caracteriza “Um dia no cais” como o primeiro “conto-reportagem” brasileiro.

Journalism and literature are two areas that have always been linked. When the first newspapers were published writers were responsible for their content. As a result, were present journalism elements in literary works. With this approach, and contamination of areas, a new genre has arisen: the literary journalism, which came up from journalism and uses the resources of literature to inform. Considering this contamination, this paper aims to show the mutual influences in each one of this areas. What is the literature contribution to journalism and what is the journalism influence in literature? These questions wil be answered by analyzing of the literary elements in the book-report Abusado, o dono do morro Dona Marta, written by Caco Barcellos, and the journalistic elements in the Paulo Lins novel Cidade de deus.
Jornalismo e literatura são duas áreas que sempre estiveram ligadas. Quando os primeiros jornais foram editados, eram os escritores os responsáveis pelo seu conteúdo. Em consequência disso, elementos jornalísticos também estiveram e estão presentes em obras literárias. Com essa aproximação e contaminação das áreas, surgiu um novo gênero: o Jornalismo Literário, que parte do jornalismo e utiliza recursos da literatura para informar. Sabendo dessa contaminação, este trabalho pretende apontar as influências de uma área sobre a outra. O que a literatura contribui para o jornalismo e o que o jornalismo contribui para a literatura? Essas perguntas serão respondidas ao analisar os elementos literários no livroreportagem Abusado, o dono do morro Dona Marta, de Caco Barcellos; e os elementos jornalísticos no romance Cidade de Deus, de Paulo Lins.

We present the case of a 50-year-old man who comes to the emergency department of Dos de Mayo Hospital, Lima, Peru, with fever, abdominal pain, diarrhea and jaundice. An intestinal infection arises as initial diagnosis. He is referred to the area of Internal Medicine where various laboratory tests and imaging studies were conducted, including an abdominal computerized tomography scan. The scan confirmed the diagnosis of pylephlebitis; treatment with antibiotics and anticoagulant therapy was immediately established. Response and evolution were favorable.
Se presenta el caso de un paciente varón de 50 años que acude a la emergencia del Hospital Dos de Mayo, Lima, Perú, por presentar fiebre, dolor abdominal, diarrea e ictericia. Se plantea un cuadro de infección intestinal como diagnóstico inicial. Es derivado al área de Medicina Interna donde se realizan diversos exámenes de laboratorio y auxiliares, dentro de los cuales se indica realizar una tomografía computarizada abdominal. El estudio imagenológico confirmó el diagnóstico de pileflebitis, iniciando tratamiento con antibióticos y anticoagulantes. La respuesta y evolución fueron favorables

This dissertation is a study of what selected literature teachers report to be their sources of practical knowledge. The data for the study was collected through open-ended interviews with three practicing public school literature teachers in two school districts in a large southwestern city between 1996 and 1998. The informants were selected more on the basis of convenience of access than any other criteria, though I also considered their length of time in the profession and limited the study to persons who were actually teaching literature at the time of the study. The interviews followed an extended observation. After the interviews were transcribed, I analyzed them by the "constant comparison" method (Merriam, 1988, p. 138), using a set of data codes derived from the interview data, then sorted the data according to the codes. That permitted me to bring together pieces of conversation from various points in the interview in a way that is most useful to me (Rubin and Rubin, 1995, pp. 238--241). While the selection and data collection methods were such that I cannot extrapolate the findings to other literature teachers, the data shows quite clearly that, for these teachers, there is a disjuncture between what the research literature assumes are teachers' primary sources of practical knowledge and what the teachers themselves think it to be. Both their statements about their sources of practical knowledge and the metaphoric language they use to describe themselves argue that, for these three teachers, alternative sources of practical knowledge---self, publisher-generated materials, reading in the professional literature, conversations with colleagues, and professional conferences and staff development workshops---are more important sources of their practices than are the sources upon which the research literature puts its primary focus---the apprenticeship of observation, content courses, and pedagogy courses. This finding suggests to me that a broader, more detailed study of this question is warranted.

This thesis will investigate the fictional discourse that has developed around academia and how this discourse has manifested itself in the New Zealand literary tradition, primarily in the works of M.K. Joseph, Dan Davin and James K. Baxter. These three writers have been selected because of their status within Kai Jensen's conception of “a literary tradition of excitement about masculinity”; in other words, the masculine tradition in New Zealand literature which provides fictional representations of factual events and tensions. This literary approach is also utilised in the tradition of British university fiction, in which the behaviour of students and faculty are often deliberately exaggerated in order to provide a representation of campus life that captures the essence of the reality without being wholly factual. The fact that these three writers attempt, consciously or unconsciously, to combine the two traditions is a matter of great literary interest: Joseph's A Pound of Saffron (1962) appropriates tropes of the British university novel while extending them to include concerns specific to New Zealand; Davin's Cliffs of Fall (1945), Not Here, Not Now (1970) and Brides of Price (1972) attempt to blend traditions of university fiction with the masculine realist tradition in New Zealand literature, though, as we will see, with limited success; Baxter's station as the maternal grandson of a noted professor allows him to criticise the elitist New Zealand university system in Horse (1985) from a unique position, for he was more sympathetic towards what he considered the working class “peasant wisdom” of his father, Archie, than the “professorial knowledge” of Archie's father-in-law. These three authors have been chosen also because of the way they explore attitudes towards universities amongst mainstream New Zealand society in their writing, for while most novels in the British tradition demonstrate little tension between those within the university walls and those without, in New Zealand fiction the tension is palpable. The motivations for this tension will also be explored in due course, but before we can grapple with how the tradition of British university fiction has impacted New Zealand literature, we must first examine the tradition itself.

Shewanella putrefaciens is a Gram-negative bacillus and marine pathogen that rarely causes disease in humans. We report a case of osteomyelitis by this organism in a 48-year-old male patient, who presented with pain and erythema of the right foot that was initially diagnosed as cellulitis and did not revert despite treatment. He was transferred to Lima where osteomyelitis was diagnosed and started on empirical treatment with partial regression. A biopsy and culture of the compromised area found S. putrefaciens. The infection was treated according to the antibiotic sensitivity profile of the pathogen. S. putrefaciens infection represents a rare opportunistic infection of devitalized or exposed areas of the body. It is associated with residence in coastal areas and commonly affects the skin and soft tissues. Exceptional cases of osteomyelitis have been reported, but this is the first that involves the metatarsal bones. Shewanella putrefaciens es un bacilo Gram negativo, patógeno marino que rara vez ocasiona enfermedad en humanos. Se presenta un caso de osteomielitis por este microorganismo en un paciente varón de 48 años, procedente de Chimbote. Presentó dolor y eritema en el pie derecho, inicialmente diagnosticado como celulitis, pero que no revirtió pese al tratamiento. Fue transferido a Lima donde se diagnosticó osteomielitis e inició tratamiento empírico con escasa mejoría. Por ello, se realizó una biopsia y cultivo de la zona comprometida, el metatarso, en el cual se aisló Shewanella putrefaciens. Se trató de acuerdo al perfil de sensibilidad. La infección por Shewanella putrefaciens representa una rara infección oportunista, que se localiza en áreas desvitalizadas o expuestas del cuerpo. Se asocia a vivir en zonas costeras, afectando comúnmente piel y tejidos blandos. Se han reportado casos excepcionales de osteomielitis. Este es el primero que involucra metatarso.

This thesis is a comparative study of two contemporary novels: Christa Wolf's: Cassandra (1983) and Margaret Atwood's The Handmaid's Tale (1985).
Wolf's Cassandra can be interpreted as a utopian projection. It is an expression of Wolf's not so modest proposal: "Literature today ought to be research on peace."
Atwood examines the underside of hope. While describing the present time as alarming, she speculates about the future. Juxtaposing the two novels provides a view of the political and philosophical imagination of the two authors. The cultural critique is esthetically expanded through the perspective of the protagonists. Both novels can be viewed as archeological work from a female perspective, as they attempt to provide a new vision by uncovering the blind spots of our western socio-political history.

O presente estudo enfoca uma investigação discursiva quanto à reflexão de professores. Os sujeitos são quatro alunos do Curso de Letras da Universidade Federal de Santa Catarina, cursando a disciplina de 'Prática de Ensino', para a qual eles produzem avaliações crítico-descritivas de suas aulas. Nenhum dos sujeitos participantes do estudo tinha experiência prévia como professor(a) antes de ingressar nessa disciplina. O interesse principal desse estudo é investigar, através das práticas discursivas dos(as) estagiários(as), as percepções, os valores e as crenças que eles parecem ter com relação ao processo de ensino-aprendizagem de uma língua estrangeira e o tipo de conhecimento que mais influencia sua prática de sala de aula. Para perseguir o propósito desse estudo, são examinadas as representações de realidade dos(as) estagiários(as) - focalizando os papéis que eles(as) assumem como seus e como de seus(suas) alunos(as) - e as relações sociais que emergem de seus textos - focalizando na discussão de princípios relacionados ao ensino de língua estrangeira - sob a perspectiva da análise crítica do discurso (Fairclough, 1992) e da lingüística sistêmica (Halliday, 1994). Os resultados obtidos através da análise dos significados ideacionais e interpessoais indicam que os(as) estagiários(as), alguns mais do que outros, se vêem como participantes sociais dominantes e com mais poder do que os(as) alunos(as) e cujos papéis são mais ativos e centrais para o desenvolvimento das aulas. Ao abordar princípios relacionados ao ensino de língua estrangeira, os estagiários tendem a modalizar seu discurso, sinalizando intertextualidade (Bakhtin, 1981) entre os seus textos e e os do(a) seu(sua) supervisor(a) e/ou incertezas com relação aos conhecimentos teóricos que eles dizem ter. Por mais difícil que seja fazer uma distinção entre conhecimento teórico e experiencial, as interpretações levantadas corroboram a literatura na área de educação de professores (Bailey et al, 1996; Freeman, 1996a; Freeman, 1996b; Freeman & Johnson, 1998; Flowerdew, 1998) ao sugerir que professores(as)-estagiários(as) tendem a ser influenciados(as) pelo conhecimento experiencial a um grau mais elevado, reiterando, também, a afirmação de que o discurso tanto é constituído como também constitui práticas sociais (Fairclough, 1989; Kress, 1989).

Resumen
Since language is an important means of communication between human beings, it is held that writers or speakers can affect their readers or hearers by using certain linguistic means. The manipulation of semantics and syntax, namely the use of emotive language, is seen as an affective means resorted to by text producers to influence the people's acceptance of the truth. Emotional language aims ultimately at persuading the addressee to accept the facts as they are presented by writers. It is regarded as a necessary condition for persuasion to be successful. This is due to the persuasive force of emotive meaning exerted upon the receiver. In addition, the employment of emotive language may be attributed to ideological considerations. This will be demonstrated in Chapter II. Emotiveness, as a means of persuasion, can be expressed by using certain devices such as repetition, intertextuality, word-order, figures of speech, intensifiers ... etc. These strategies will be discussed in detail with reference to translation in Chapter III. Furthermore, I must say that some of my remarks have been based on the findings of outstanding grammarians and linguists, and therefore, I have been obliged to quote from such works to substantiate my points of view. Before proceeding with the investigation, I must point out that the entire data of my work will be confined only to news reports and editorials both in Arabic and English, and for this end a number of articles have been used from official newspapers in both languages.

Sweave combines typesetting with LATEX and data anlysis with S into integrated statistical documents. When run through R or Splus, all data analysis output (tables, graphs, ...) is created on the fly and inserted into a final LATEX document. Options control which parts of the original S code are shown to or hidden from the reader, respectively. Many S users are also LATEX users, hence no new software has to be learned. The report can be automatically updated if data or analysis change, which allows for truly reproducible research. (author's abstract)

Resumen

Lather, Rinse, Repeat was an exhibition that culminated in my Graduate Thesis exhibition. This exhibition consisted of a set of four artworks that explore issues of authorship in relation to my own graduate art practice. This paper serves its purpose to offer reflection, insight and a brief description of the four artworks exhibited. Though Lather, Rinse, Repeat had humble beginnings, these artworks are the result of how I view relationships in my life, and how those relationships shape the aspects of self, authorship and authority. Lather, Rinse, Repeat's four artworks vary in production method but all stem from my experiences in and out of the classroom, and in the graduate school community. My hope is that these artworks and this paper provide a glimpse into how one artists brains struggles with the many roles it fulfills daily.

This contribution explores notions of "popular narratology" in reports, memos and strategic research papers of the US military and affiliated institutions. It investigates the military´s analysis of emerging social media practices and corresponding efforts to adapt military organization, strategy, and culture to these new technological and cultural phenomena. Employing notions and techniques of constructing and disseminating a narrative that they borrow from literary theory, media studies, and advertising, these texts negotiate best practices to set political agendas and instigate institutional reforms within the military. Providing a cultural history perspective on these recent developments in military policy, this article emphasizes the military papers´reflections on the textuality of military information in order to reveal their agenda of furthering the military´s interests. It is thus invested in exploring how textual (i.e., "poetic") qualities serve as tools to pursue political goals.

Whether invoking the noble—or the cruel—savage, the image of Native Americans has always included notions of war and fighting. Non-Natives have attributed character traits to them such as cunning, stealth, endurance, and bravery; and they have used these im ages to denounce or to idealize Native Americans. In the U.S., a prolon ged history of frontier conflict, multiplied by popular frontier myths, has resulted in a collective memory of Indians as fighters. While images of fighting Indians have entered American everyday language, Germans have had no significant collective history of American frontier settlement and conflicts with Native Americans. Nevertheless, they have acquired a number of idioms and figures of speech relating to Indian images due to the romanticized euphoria for Native themes, spurred by popular nove ls and Wild West shows.

Este estudo tem como objetivo analisar as microcenas que emergem em notícias. As microcenas Benefactivas podem estar representadas por predicatores benefactivos básicos ou por predicadores de outros domínios semânticos que têm o Benefactivo perspectivizado naquela microcena. A análise foi realizada em 164 microcenas de texto jornalístico, retiradas do The Washington Post, em sua versão online # www.washingtonpost.com # durante quinze dias seguidos, numa tentativa de abarcar e destacar a noção benefactiva no contexto da língua em uso, aqui representada por notícias políticas. Fillmore (1968), Chafe (1970), Cruse (1973), Cook (1979, 1989), Nicolacópulos (1992,1995), Nicolacópulos & Conceição (1984), Oliveira (1999), Rocha (2003) e Weickert & Nicolacópulos (2005) têm estudado a multiplicidade de sentidos dentro da Gramática de Casos, incorporada à Semântica Relacional, a fim de compreender os novos significados e seus efeitos construídos no dinamismo da língua em uso. Nesta perspectiva, eles abordaram o significado da língua de diferentes formas, expressando agência, experiência, benfeciário, objeto, lugar, companhia, tempo e holismo. No entanto, ainda há muita pesquisa a ser desenvolvida nesta área, principalmente em relação ao Benefactivo, que expressa as noções de posse, ganho ou perda e transferência de propriedade, e como ele aparece de forma viva na língua Inglesa. Esta pesquisa considera uma microcena uma #oração extendida# que incorpora noções que não estão necessariamente expressas na superfície das microcenas, mas podem ser apreendidas pela macrocena, o contexto maior do texto e pelas associações que o verbo ou predicador mantém com os outros elementos nas microcenas Benefactivas. É possível de perceber que (i) este gênero textual favorece a presença de microcenas Benefactivas, (ii) a maioria delas são compostas por predicadores Benefactivos em seu sentido básico (87.80%), e (iii) algumas vêm de predicadores de outros domínios semânticos, mas que têm seu sentido Benefactivo perspectivizado (12.19%). Ao mesmo tempo, observou-se que muitos predicadores Benefactivos fazem um movimento metafórico (i) em direção a outros campos semânticos (Experiential, Comitativo, Tempo) ou (ii) em direção ao subdomínio Benefactivo chamado de Benefactive power. The present study aims at analyzing the Benefactive microscenes that emerge from news reports. The Benefactive microscenes may be represented by basically Benefactive predicators or by predicators form other semantic domains which have the Benefactive foregrounded. The analysis is carried out on a corpus of 164 microscenes from journalistic texts taken from The Washington Post online version # ww.washingtonpost.com # during a period of fifteen days in a row, in an attempt to encompass and enhance the Benefactive notion in the context of language in use, here represented by the political news reports. Fillmore (1968), Chafe (1970), Cruse (1973), Cook (1979, 1989), Nicolacópulos (1992,1995), Nicolacópulos & Conceição (1984), Oliveira (1999), Rocha (2003) and Weickert & Nicolacópulos (2005) have tried to tackle the multiplicity of meanings within Case Grammar, incorporated into Relational Semantics, in order to comprise the new senses and their effects constructed in the dynamism of language in use. Within this perspective, they have approached language meaning in different ways, be it expressing agency, experience, beneficiary, object, location, company, time, holism. However, there is still a lot to be researched in this area, mainly in the distinct notion of the beneficiary, which expresses the notions of possession, gain or loss, and transfer of property, and how it is lively represented in the modern English language. This work considers a microscene an #extended clause# which incorporates notions that are not necessarily expressed in the surface of the microscenes, but can be apprehended by the macroscene, the broader context of the text and by the associations the verb or predicator keeps with the other elements within the Benefactive microscenes. It was noticed that (i) this genre favors the presence of Benefactive microscenes, (ii) most of them emerge from basically Benefactive predicators (87.80%), and (iii) some from predicators from other semantic domains that in specific microscenes have the Benefactive sense foregrounded (12.19%). At the same time, it was observed that many basically Benefactive predicators make a metaphorical move (i) towards other semantic domains (Experiential, Comitative, Time) or (ii) towards a Benefactive subdomain, namely power-Benefactive.

Client writing has been used in association with individual therapy for many years, but published research on its effectiveness is sparse. Such research could provide a potential tool for therapists, as well as expand scientific knowledge. This review includes all available reports on the topic of client writing in individual therapy. The studies fall into three general categories: the systematic, "experimental" studies; the anecdotal reports; and the didactic reports. All studies considered show some benefit from the use of client writing in therapy. Writing appears to be beneficial whether initiated by the client or directed by the therapist; beneficial to adolescents and adults of all ages; beneficial to hospitalized patients, clients in individual therapy, and to students in school counseling; beneficial with different forms of writing; beneficial with different types of therapy; and beneficial whether writing is the main focus of therapy or only an adjunctive process. However, the conclusion that client writing is beneficial is largely based on anecdotal reports from therapists who have accidentally stumbled on it. Much more systematic research is needed, beginning with a survey to determine how extensive is its use. Specific recommendations for research are made.

Este trabalho tem como objetivo fornecer orientações metodológicas que possibilitem a ampliação do acesso aos relatórios de pesquisas gerados na Fundação Oswaldo Cruz (FIOCRUZ), tomando a Unidade CICT como modelo. Para conhecer que valor o relatório de pesquisa tem para o pesquisador foram realizadas entrevistas abordando as questões de produção, representação e acesso a esses documentos. O panorama que se obteve a partir das opiniões apresentadas pelos entrevistados permitiu entender melhor o valor que esse documento produzido tem para a comunicação científica. A partir da análise documentária feita nos relatórios produzidos na Unidade foi possível identificar como esses documentos estão sendo elaborados, e a partir daí propor critérios para a padronização e inclusão de elementos em sua estrutura. Além disso, constatou-se que a dificuldade de acesso a documentos desta natureza é perene. A expectativa dos pesquisadores com respeito à gestão dos relatórios de pesquisa na Unidade reforça a necessidade de orientações metodológicas para a produção desses documentos. O relatório produzido a partir de critérios estabelecidos facilitará a sua representação e registro. Os relatórios ─ por conterem informações referentes aos métodos e resultados de pesquisas de forma detalhada ─ oferecem condições de informar de forma mais ágil uma comunidade científica, ainda que não formalmente publicados.
This work discusses methodological parameters aiming to improve the access to reports from studies carried out at the Oswaldo Cruz Foundation (FIOCRUZ), taking the output of one of its units (CICT) Unit as a model and source of information. To assess research reports’s relevance for researchers working at CICT, in-depth interviews were carried out, addressing issues related to scientific reports production, representation and retrieval. The picture emerging from the interviewees’ opinions and concepts fostered a better understanding of the value ascribed to the scientific reports and their meanings for scientific communication. The comprehensive analysis of the documents produced in the Unit permitted to disclose how scientific reports have been elaborated, and then to establish criteria for the standardization of such documents, with the inclusion of key elements in their structure. The study conclusions realized access to such documents constitutes a permanent challenge. The researchers' expectation with regard to the management of the Unit’s reports reinforces the need of clear and well-defined methodological parameters to be used in the elaboration of such documents. Standard reports, with sound criteria, will be easily represented and registered. Most reports contain detailed information on results and methodological approaches and procedures and as such constitute an invaluable source of information for the scientific community, even if not formally
