Texto completohowever, with its widespread clinical use over the last decade, a few cases of cardiotoxicity related to gemcitabine use have been reported. Cardiomyopathy after the use of gemcitabine monotherapy is extremely rare
and only one such case has been reported in detail previously.CASE PRESENTATION:We report a case of a 56-year-old African American man with pancreatic cancer who presented with signs and symptoms of congestive heart failure after being treated with gemcitabine for two cycles (six doses). A two-dimensional echocardiography showed left ventricular ejection fraction of 15 to 20 percent with global hypokinesia. With the absence of significant risk factors for coronary artery disease and a strong temporal relationship with the initiation of chemotherapy, it was concluded that our patient's cardiomyopathy was related to the use of gemcitabine. Gemcitabine was discontinued and our patient responded well to standard heart failure therapy. Two months later, a repeat echocardiogram showed significant improvements in left ventricular systolic function.CONCLUSIONS:Gemcitabine should be considered as a potential cause of cardiomyopathy in patients receiving chemotherapy with this drug. We need further studies to look into potential mechanisms and treatments of gemcitabine-induced cardiac dysfunction.

Texto completoBackground and Significance: The utilization of IVC filters for pulmonary embolism prevention has increased significantly over the past decade as the indications continue to expand. Although the risks associated with IVC filters are small, a well‐known complication is filter fracture and subsequent embolization of the fragment. Case reports have been published on the devastating effects of fragment migration to the heart, causing intense chest pain, pericardial effusion, cardiac tamponade and death. Research Question: There is a paucity of experience and guidelines for treating patients with a metallic foreign object lodged within the heart. Is there a consensus on the proper management of these cases? How do these patients present and what are the outcomes of treatment? Some clinicians have chosen to observe and monitor, while others have gone to the operating room for open‐heart surgery and retrieval of the fragment. Methods: In an attempt to answer these questions, a systematic review of the published literature was conducted between 1985 and 2015. Only articles related to IVC filter fracture and subsequent fragment migration to the heart were included. The clinical presentation, workup, management, treatment and outcomes were collected as available. Results: A total of 23 articles were published consisting of a prospective study, retrospective series and case reports. There were 37 migrated fragment to the heart reported in 29 patients. The most common clinical presentations were chest pain (69.0%) and no symptoms (27.6%). Regarding treatment, ten patients underwent observation, three had successful endovascular retrieval, 12 went to the operating room for open‐heart surgery and four cases were unreported. Of the 12 patients with reported pericardial effusion, 11 (91.7%) underwent open surgical repair. Of the eight asymptomatic patients, seven (87.5%) were ultimately in observation and the management of the other was unreported. Conclusions: There appears to be a consensus in the literature that observation and close follow up are appropriate options for asymptomatic patients. Symptomatic patients with pericardial effusion may benefit from open‐heart surgery. Cardiovascular compromise such as cardiac tamponade should be managed with open surgery. Based upon these findings and other details in the cases, we have proposed a management algorithm.

Texto completoA 65-year-old gentleman presented with left groin swelling over the course of two months. Physical exam revealed nontender left inguinal adenopathy, and computed tomography scans detected multiple lymph nodes in the mesenteric, aortocaval, and right common iliac regions. An excisional lymph node biopsy was performed. Pathologic evaluation demonstrated follicular center site which stained positive for PAX5, CD20, CD10, Bcl-2, Bcl-6, and mantle zone cells. These findings demonstrated CCND1 and CD5 positivity, suggesting composite lymphoma comprising follicular lymphoma (FL) with in situ mantle cell lymphoma (MCLIS). FL is known as indolent non-Hodgkin lymphoma; however, the clinical significance of a coexisting MCLIS continues to be elusive, and optimal management of these patients remains largely unknown. This case illustrates the diagnostic and therapeutic challenges of composite lymphomas. This paper also discusses advances in molecular pathogenesis and lymphoma genomics which offer novel insights into these rare diseases.

Texto completoBackground : Participation and person-centered care are key concepts in the care that are being given today. In order for the patient to participate, they must be involved in the daily work such as shift reporting. At each shift, reporting occurs from one nurse to another which often lacks patient involvement. A reporting model is bedside report, which means reporting takes place in the patient's presence. Aim : The aim of this study was to describe patient’s participation in care during bedside report. Method : A literature review. Results : Bedside report made patients feel more involved in their care, partly because they felt valued as a participant in the report. The patients were considered a person in the first hand. Both patients and nurses felt that the reporting method was crucial for the participation. Most patients appreciated being included in the reporting and that they received valuable information that they considered justifiable. However, not all patients wanted to participate, which was due, among other things, to the fact that the report was considered for the nurses. Conclusion : Bedside report increased patient involvement in care, according to both the patient and the nurse. However, not all patients wanted to participate and the reporting method were important for the level of participation.

"Um dia no cais", by João Antônio, was firstly published in the magazine Realidade under the categorization of "short story-report". This designation makes us suppose a mixture, in text, of two different textual genres originated from literature and journalism: the short story and the story-report. This study investigates the textual resources that allowed the term, unknow of at that time, to be created and designated to the aforementioned text. Besides, our aim is to discuss the elements that approach the term and, at the same time, make it distance it from an assertive use above its supposed “subgenre” status. To get the results, we used theoretical concepts that define both short story and report in such a way to intersect them. And in order to consider the environment and the context in this analysis, we take the magazine Realidade and the conditions of Brazil (military dictatorship and the incipient development of a Brazilian cultural industry) into consideration at the time of the publication of the text in question, also intending to understand the emergence of this term that still characterizes "Um dia no cais" as the first Brazilian "short story-report".
“Um dia no cais”, de João Antônio, foi publicado na revista Realidade sob a categorização de “conto-reportagem”. Tal designação nos faz supor a mistura, em texto, de dois diferentes gêneros textuais oriundos da literatura e do jornalismo: o conto e a reportagem. A dissertação de que trata este resumo pretende investigar quais recursos textuais permitiram que o termo, inédito à época, fosse criado e designado ao texto supracitado e, para além de seu estatuto de suposto “subgênero”, discutir quais elementos o aproximam e o distanciam de um uso assertivo. Para a obtenção dos resultados pretendidos fazemos uso de teorias que se propõem a encontrar maneiras de definir tanto o conto quanto a reportagem, procurando nelas um caminho possível de intersecção. E para que também sejam considerados nessa análise o meio e o contexto, levaremos em conta a revista Realidade e as condições do Brasil (a ditadura militar e o incipiente desenvolvimento de uma indústria cultural brasileira) no momento de publicação do texto em questão, também intencionando entender o surgimento deste termo que ainda caracteriza “Um dia no cais” como o primeiro “conto-reportagem” brasileiro.

Texto completoJournalism and literature are two areas that have always been linked. When the first newspapers were published writers were responsible for their content. As a result, were present journalism elements in literary works. With this approach, and contamination of areas, a new genre has arisen: the literary journalism, which came up from journalism and uses the resources of literature to inform. Considering this contamination, this paper aims to show the mutual influences in each one of this areas. What is the literature contribution to journalism and what is the journalism influence in literature? These questions wil be answered by analyzing of the literary elements in the book-report Abusado, o dono do morro Dona Marta, written by Caco Barcellos, and the journalistic elements in the Paulo Lins novel Cidade de deus.
Jornalismo e literatura são duas áreas que sempre estiveram ligadas. Quando os primeiros jornais foram editados, eram os escritores os responsáveis pelo seu conteúdo. Em consequência disso, elementos jornalísticos também estiveram e estão presentes em obras literárias. Com essa aproximação e contaminação das áreas, surgiu um novo gênero: o Jornalismo Literário, que parte do jornalismo e utiliza recursos da literatura para informar. Sabendo dessa contaminação, este trabalho pretende apontar as influências de uma área sobre a outra. O que a literatura contribui para o jornalismo e o que o jornalismo contribui para a literatura? Essas perguntas serão respondidas ao analisar os elementos literários no livroreportagem Abusado, o dono do morro Dona Marta, de Caco Barcellos; e os elementos jornalísticos no romance Cidade de Deus, de Paulo Lins.

O presente estudo enfoca uma investigação discursiva quanto à reflexão de professores. Os sujeitos são quatro alunos do Curso de Letras da Universidade Federal de Santa Catarina, cursando a disciplina de 'Prática de Ensino', para a qual eles produzem avaliações crítico-descritivas de suas aulas. Nenhum dos sujeitos participantes do estudo tinha experiência prévia como professor(a) antes de ingressar nessa disciplina. O interesse principal desse estudo é investigar, através das práticas discursivas dos(as) estagiários(as), as percepções, os valores e as crenças que eles parecem ter com relação ao processo de ensino-aprendizagem de uma língua estrangeira e o tipo de conhecimento que mais influencia sua prática de sala de aula. Para perseguir o propósito desse estudo, são examinadas as representações de realidade dos(as) estagiários(as) - focalizando os papéis que eles(as) assumem como seus e como de seus(suas) alunos(as) - e as relações sociais que emergem de seus textos - focalizando na discussão de princípios relacionados ao ensino de língua estrangeira - sob a perspectiva da análise crítica do discurso (Fairclough, 1992) e da lingüística sistêmica (Halliday, 1994). Os resultados obtidos através da análise dos significados ideacionais e interpessoais indicam que os(as) estagiários(as), alguns mais do que outros, se vêem como participantes sociais dominantes e com mais poder do que os(as) alunos(as) e cujos papéis são mais ativos e centrais para o desenvolvimento das aulas. Ao abordar princípios relacionados ao ensino de língua estrangeira, os estagiários tendem a modalizar seu discurso, sinalizando intertextualidade (Bakhtin, 1981) entre os seus textos e e os do(a) seu(sua) supervisor(a) e/ou incertezas com relação aos conhecimentos teóricos que eles dizem ter. Por mais difícil que seja fazer uma distinção entre conhecimento teórico e experiencial, as interpretações levantadas corroboram a literatura na área de educação de professores (Bailey et al, 1996; Freeman, 1996a; Freeman, 1996b; Freeman & Johnson, 1998; Flowerdew, 1998) ao sugerir que professores(as)-estagiários(as) tendem a ser influenciados(as) pelo conhecimento experiencial a um grau mais elevado, reiterando, também, a afirmação de que o discurso tanto é constituído como também constitui práticas sociais (Fairclough, 1989; Kress, 1989).

Texto completoLather, Rinse, Repeat was an exhibition that culminated in my Graduate Thesis exhibition. This exhibition consisted of a set of four artworks that explore issues of authorship in relation to my own graduate art practice. This paper serves its purpose to offer reflection, insight and a brief description of the four artworks exhibited. Though Lather, Rinse, Repeat had humble beginnings, these artworks are the result of how I view relationships in my life, and how those relationships shape the aspects of self, authorship and authority. Lather, Rinse, Repeat's four artworks vary in production method but all stem from my experiences in and out of the classroom, and in the graduate school community. My hope is that these artworks and this paper provide a glimpse into how one artists brains struggles with the many roles it fulfills daily.

Este estudo tem como objetivo analisar as microcenas que emergem em notícias. As microcenas Benefactivas podem estar representadas por predicatores benefactivos básicos ou por predicadores de outros domínios semânticos que têm o Benefactivo perspectivizado naquela microcena. A análise foi realizada em 164 microcenas de texto jornalístico, retiradas do The Washington Post, em sua versão online # www.washingtonpost.com # durante quinze dias seguidos, numa tentativa de abarcar e destacar a noção benefactiva no contexto da língua em uso, aqui representada por notícias políticas. Fillmore (1968), Chafe (1970), Cruse (1973), Cook (1979, 1989), Nicolacópulos (1992,1995), Nicolacópulos & Conceição (1984), Oliveira (1999), Rocha (2003) e Weickert & Nicolacópulos (2005) têm estudado a multiplicidade de sentidos dentro da Gramática de Casos, incorporada à Semântica Relacional, a fim de compreender os novos significados e seus efeitos construídos no dinamismo da língua em uso. Nesta perspectiva, eles abordaram o significado da língua de diferentes formas, expressando agência, experiência, benfeciário, objeto, lugar, companhia, tempo e holismo. No entanto, ainda há muita pesquisa a ser desenvolvida nesta área, principalmente em relação ao Benefactivo, que expressa as noções de posse, ganho ou perda e transferência de propriedade, e como ele aparece de forma viva na língua Inglesa. Esta pesquisa considera uma microcena uma #oração extendida# que incorpora noções que não estão necessariamente expressas na superfície das microcenas, mas podem ser apreendidas pela macrocena, o contexto maior do texto e pelas associações que o verbo ou predicador mantém com os outros elementos nas microcenas Benefactivas. É possível de perceber que (i) este gênero textual favorece a presença de microcenas Benefactivas, (ii) a maioria delas são compostas por predicadores Benefactivos em seu sentido básico (87.80%), e (iii) algumas vêm de predicadores de outros domínios semânticos, mas que têm seu sentido Benefactivo perspectivizado (12.19%). Ao mesmo tempo, observou-se que muitos predicadores Benefactivos fazem um movimento metafórico (i) em direção a outros campos semânticos (Experiential, Comitativo, Tempo) ou (ii) em direção ao subdomínio Benefactivo chamado de Benefactive power. The present study aims at analyzing the Benefactive microscenes that emerge from news reports. The Benefactive microscenes may be represented by basically Benefactive predicators or by predicators form other semantic domains which have the Benefactive foregrounded. The analysis is carried out on a corpus of 164 microscenes from journalistic texts taken from The Washington Post online version # ww.washingtonpost.com # during a period of fifteen days in a row, in an attempt to encompass and enhance the Benefactive notion in the context of language in use, here represented by the political news reports. Fillmore (1968), Chafe (1970), Cruse (1973), Cook (1979, 1989), Nicolacópulos (1992,1995), Nicolacópulos & Conceição (1984), Oliveira (1999), Rocha (2003) and Weickert & Nicolacópulos (2005) have tried to tackle the multiplicity of meanings within Case Grammar, incorporated into Relational Semantics, in order to comprise the new senses and their effects constructed in the dynamism of language in use. Within this perspective, they have approached language meaning in different ways, be it expressing agency, experience, beneficiary, object, location, company, time, holism. However, there is still a lot to be researched in this area, mainly in the distinct notion of the beneficiary, which expresses the notions of possession, gain or loss, and transfer of property, and how it is lively represented in the modern English language. This work considers a microscene an #extended clause# which incorporates notions that are not necessarily expressed in the surface of the microscenes, but can be apprehended by the macroscene, the broader context of the text and by the associations the verb or predicator keeps with the other elements within the Benefactive microscenes. It was noticed that (i) this genre favors the presence of Benefactive microscenes, (ii) most of them emerge from basically Benefactive predicators (87.80%), and (iii) some from predicators from other semantic domains that in specific microscenes have the Benefactive sense foregrounded (12.19%). At the same time, it was observed that many basically Benefactive predicators make a metaphorical move (i) towards other semantic domains (Experiential, Comitative, Time) or (ii) towards a Benefactive subdomain, namely power-Benefactive.

Este trabalho tem como objetivo fornecer orientações metodológicas que possibilitem a ampliação do acesso aos relatórios de pesquisas gerados na Fundação Oswaldo Cruz (FIOCRUZ), tomando a Unidade CICT como modelo. Para conhecer que valor o relatório de pesquisa tem para o pesquisador foram realizadas entrevistas abordando as questões de produção, representação e acesso a esses documentos. O panorama que se obteve a partir das opiniões apresentadas pelos entrevistados permitiu entender melhor o valor que esse documento produzido tem para a comunicação científica. A partir da análise documentária feita nos relatórios produzidos na Unidade foi possível identificar como esses documentos estão sendo elaborados, e a partir daí propor critérios para a padronização e inclusão de elementos em sua estrutura. Além disso, constatou-se que a dificuldade de acesso a documentos desta natureza é perene. A expectativa dos pesquisadores com respeito à gestão dos relatórios de pesquisa na Unidade reforça a necessidade de orientações metodológicas para a produção desses documentos. O relatório produzido a partir de critérios estabelecidos facilitará a sua representação e registro. Os relatórios ─ por conterem informações referentes aos métodos e resultados de pesquisas de forma detalhada ─ oferecem condições de informar de forma mais ágil uma comunidade científica, ainda que não formalmente publicados.
This work discusses methodological parameters aiming to improve the access to reports from studies carried out at the Oswaldo Cruz Foundation (FIOCRUZ), taking the output of one of its units (CICT) Unit as a model and source of information. To assess research reports’s relevance for researchers working at CICT, in-depth interviews were carried out, addressing issues related to scientific reports production, representation and retrieval. The picture emerging from the interviewees’ opinions and concepts fostered a better understanding of the value ascribed to the scientific reports and their meanings for scientific communication. The comprehensive analysis of the documents produced in the Unit permitted to disclose how scientific reports have been elaborated, and then to establish criteria for the standardization of such documents, with the inclusion of key elements in their structure. The study conclusions realized access to such documents constitutes a permanent challenge. The researchers' expectation with regard to the management of the Unit’s reports reinforces the need of clear and well-defined methodological parameters to be used in the elaboration of such documents. Standard reports, with sound criteria, will be easily represented and registered. Most reports contain detailed information on results and methodological approaches and procedures and as such constitute an invaluable source of information for the scientific community, even if not formally
