Discrimination and inequality of opportunity run counter to British values, are inefficient and waste talent, and can be a potent source of grievance. Discrimination also has highly negative consequences for the well-being and mental health of those affected. However, despite the various Acts of Parliament which made discrimination for jobs illegal, Blacks and Asians experience much the same level of discrimination as they did forty years ago. There is a rather more optimistic picture in the case of the gender wage gap, where the evidence suggests that there was some decline in unequal treatment. In contrast, the story for social class inequalities of opportunity is less encouraging, with little evidence of improvement over the post-war period. Most peer countries exhibit similar inequalities, but Canada appears to do better with respect to social class and France with respect to gender inequalities. There is scope for improvement.

Recent research has provided important insights on the long-term outcomes of patients who develop acute kidney injury (AKI) in the setting of critical illness. Large epidemiologic studies have demonstrated compelling associations between episodes of AKI and progressive kidney disease and death, respectively, although such studies do not establish causality due to the potential for confounding. Whether AKI is intrinsically toxic or a mere by-product of serious comorbidities (e.g. prior chronic kidney disease, heart failure, diabetes), there is no doubt that AKI survivors are a high-risk group who would likely benefit from close post-discharge follow-up. Recent studies have shown that a minority of patients with AKI receive specialized nephrology follow-up after discharge, suggesting an opportunity for quality improvement. Emerging research is evaluating factors that predict chronic kidney disease, end-stage renal disease, and death among AKI survivors. This work will, it is hoped, suggest new targets for prevention and treatment, with the goal of enhancing the likelihood of recovery following AKI.

This concluding chapter examines the recent trends in Massachusetts' effort to confront its image as out of touch with the rest of the country, turning to the themes of equality of opportunity and meritocratic individualism and how these had come to define liberalism. It shows that, in recent years, the Route 128 area had experienced a series of demographic changes which has since helped to lessen the pronounced whiteness in the Route 128 suburbs—but not the patterns of economic exclusivity. The chapter notes the improvements to the quality of life for Route 128's residents of color, noting however that such trends have yet to provide the solution to metropolitan, class, and racial inequities.

This book presents an evidence-based conceptual framework for planning and implementing organizational change processes specifically focused on human service organizations (HSOs). After a brief discussion of relevant theory and a review of key challenges facing HSOs that create opportunities for organizational change, a detailed conceptual framework outlines an organizational change process. Two chapters are devoted to the essential role of an organization’s executive or other manager as a change leader. Five chapters cover the steps of the change process, beginning with identifying a problem or change opportunity; then defining a change goal; assessing the present state of the organization (the change problem and organizational readiness and capacity to engage in change); and determining an overall change strategy. Twenty-one evidence-based organizational change tactics are presented to guide implementation of the process. Tactics include communicating the urgency for change and the change vision; developing an action system that includes a change sponsor, a change champion, a change leadership team and action teams; providing support to staff; facilitating the development and approval of ideas to achieve the change goal; institutionalizing the changes within organizational systems; and evaluating the change process and outcomes. Four case examples from public and nonprofit HSOs are used to illustrate change tactics. Individual chapters cover change technologies and methods, including action research; team building; conflict management; quality improvement methods; organization redesign; organizational culture change; using consultants; advancing diversity, equity, inclusion, and social justice; capacity building; implementation science methods; specific models, including the ARC model; and staff-initiated organizational change.

Physicians are among the few professionals who are expected to work extended duty shifts of 24 hours or more, often with little opportunity for rest. The physiological factors regulating sleepiness, including circadian rhythms, sleep homeostasis, and sleep inertia, are pushed to their limits when meeting the demands of training programmes and patient care. Sleep-deprived physicians experience reduced clinical performance and vigilance, putting patients at risk. Tired physicians are more likely to make both cognitive errors (e.g. diagnostic reasoning) and technical errors (e.g. surgical complications). Over recent decades, regulations have promulgated that limit physician hours in Europe and the United States. Studies of their impact have generally shown improvements in patient and physician outcomes, though have also revealed concerns about education and training which must also be addressed. As medicine evolves to meet our 24-hour on-demand society, physicians and patients will need to embrace new approaches to high-quality and safe care delivery.

This book shows how cities can be places of opportunity rather than places with problems. With strongly revived cities and suburbs, working as places that serve all their residents, metropolitan areas will thrive, thus making the national economy more productive, the environment better protected, the citizenry better educated, and the society more reflective, sensitive, and humane. The book argues that America has been in the habit of abusing its cities and their poorest suburbs, which are always the first to be blamed for society's ills and the last to be helped. As federal and state budgets, regulations, and programs line up with the interests of giant corporations and privileged citizens, they impose austerity on cities, short-change public schools, make it hard to get nutritious food, and inflict the drug war on unlucky neighborhoods. Frustration with inequality is spreading. Parents and teachers call persistently for improvements in public schooling, and education experiments abound. Nutrition indicators have begun to improve, as rising health costs and epidemic obesity have led to widespread attention to food. The futility of the drug war and the high costs of unwarranted, unprecedented prison growth have become clear. The text documents a positive development: progressive politicians in many cities and some states are proposing far-reaching improvements, supported by advocacy groups that form powerful voting blocs, ensuring that Congress takes notice. When more cities forcefully demand enlightened federal and state action on these four interrelated problems—inequality, schools, food, and the drug war—positive movement will occur in traditional urban planning as well, so as to meet the needs of most residents for improved housing, better transportation, and enhanced public spaces.

In October 2019, Abhijit Banerjee, Esther Duflo, and Michael Kremer jointly won the 51st Sveriges Riksbank Prize in Economic Sciences in Memory of Alfred Nobel "for their experimental approach to alleviating global poverty." But what is the exact scope of their experimental method, known as randomized control trials (RCTs)? Which sorts of questions are RCTs able to address and which do they fail to answer? This book provides answers to these questions, explaining how RCTs work, what they can achieve, why they sometimes fail, how they can be improved and why other methods are both useful and necessary. Chapters contributed by leading specialists in the field present a full and coherent picture of the main strengths and weaknesses of RCTs in the field of development. Looking beyond the epistemological, political, and ethical differences underlying many of the disagreements surrounding RCTs, it explores the implementation of RCTs on the ground, outside of their ideal theoretical conditions and reveals some unsuspected uses and effects, their disruptive potential, but also their political uses. The contributions uncover the implicit worldview that many RCTs draw on and disseminate, and probe the gap between the method's narrow scope and its success, while also proposing improvements and alternatives. This book warns against the potential dangers of their excessive use, arguing that the best use for RCTs is not necessarily that which immediately springs to mind, and offering opportunity to come to an informed and reasoned judgement on RCTs and what they can bring to development.
