Objectives Adherence to the allergic rhinitis clinical practice guideline is being considered as a potential focus for national performance metrics. To help inform this discussion, we assessed patient- and clinician-reported medication administration among nationally representative populations of patients with allergic rhinitis. Study Design Cross-sectional analyses. Setting and Subjects Home health assessments, ambulatory visits. Methods Participants in the National Health and Nutrition Examination Survey and the National Ambulatory Medical Care Survey / National Hospital Ambulatory Medical Care Survey were assessed. The primary outcomes were the percentage of patients reporting receipt of antihistamines and/or nasal steroids among those with allergy-related symptoms and the percentage for whom a clinician administered these medications when diagnosing allergic rhinitis. Secondary outcomes included assessments of those with worse quality of life, confirmatory allergy testing, and leukotriene receptor antagonist use. Results Within the National Health and Nutrition Examination Survey, an estimated 29.2 million patients were diagnosed with “hay fever,” while 92.2 million were diagnosed with “allergies.” Patients with symptoms of allergic rhinitis reported that antihistamines or nasal steroids were prescribed in 21.1% to 24.0% of cases. Leukotriene receptor antagonists were given to 1.7% of those without asthma or use of other allergy medications. Within the National Ambulatory Medical Care Survey / National Hospital Ambulatory Medical Care Survey, observations representing 149.5 million visits for allergic rhinitis demonstrated that nasal steroids were administered in 29.6% of cases, while nonsedating and sedating antihistamines were given in 22.4% and 17.2%, respectively. Conclusions Despite a high prevalence of allergic rhinitis, per patient report and clinician entry, a substantial number of affected patients do not receive antihistamines and nasal steroids.

Participants: A convenience sample of 5,172 currently employed nurses.Main Outcome Measure: The main outcome of this study is the documentation of overall familiarity with emergency preparedness and response knowledge among nursing staff. Logistic ordinal regression statistical analysis was completed to determine the significance of individual domains impacting the overall familiarity score.Results: Findings based on the results of the “overall familiarity with response activities related to a large-scale emergency incident” question documented most staff (78.45 percent) have little or no familiarity with their role in disaster response. Six domains or focused education areas were identified as having a statistically significant impact (p 0.0001 –p = 0.0195) on the results of the overall familiarity question.Conclusions: These study results support the need for more education (academic and/or institutional) related to nursing emergency preparedness and response.

The aim of this paper is to examine the influence of foreign exchange risks on manufacturing activities (<em>monozukuri</em> in Japanese) and the function of derivatives as a countermeasure against such risk from the viewpoint of management accounting. From this perspective, we examine the Comprehensive Profit Opportunity and Lost Opportunity Control (COLC) model, discussed previously in this journal, and further its practical development and application. To this end, this paper first clarifies the actual situations of major Japanese manufacturing companies in terms of foreign exchange fluctuation earnings and derivative instruments (including hedge accounting). Then, after investigation of the prior research on the interrelation between risk management and management accounting, we theoretically analyze the relations between risks, derivatives, and hedge accounting from the synthetic viewpoint of profit opportunity, risk, and opportunity cost. As a result, this analysis can play an important role in outlining the landscape in which business strategy and enterprise risk management align, both proactively and reflectively, with contemporary management accounting.

Background: Urinary tract infections (UTIs) are one of the most common hospital-acquired infections; ~70%–80% are attributable to an indwelling urethral catheter. Daily risk of bacteriuria acquisition varies from 3% to 7% with a catheter. CAUTIs are associated with increased mortality, cost, and inappropriate treatment of asymptomatic bacteriuria which promotes antimicrobial resistance and Clostridium difficile infection. NHSN CAUTI criteria is most commonly met when a patient has a positive urine culture and a fever. Although fever can be associated with many sources, it cannot be excluded from UTI determination even when attributable to another recognized source. Given the high prevalence of bacteriuria in catheterized patients and the many sources of fever, the NHSN definition lacks specificity. Objective: To better classify CAUTI using enhanced criteria to so that appropriate reduction efforts would be utilized. Methods: A retrospective review was conducted to evaluate NHSN-defined CAUTIs from July 2017 to December 2018. Patients with NHSN defined CAUTI were evaluated to determine elements present to meet criteria. Overcaptured (O-CAUTIs) were defined as follows: (1) O-CAUTI 1, a positive culture with fever attributable to an infectious source; (2) O-CAUTI 2, a positive culture with fever attributable noninfectious source; (3) O-CAUTI 3, repeated positive cultures outside the RI period; (4) O-CAUTI 4, a positive culture with symptoms attributable to another source and no fever. Classifications were discussed with the medical and clinical leadership to determine appropriate opportunities for improvement. Results: Overall, 49 NHSN CAUTIs were identified with 11 of 49 (22%) being true CAUTIs and 38 of 49 (78%) O-CAUTI. O-CAUTI 1 was most common, with 17 of 38 (45%). The most frequent attributable source of fever for O-CAUTI 1 (infectious source) was respiratory (7 of 17, 59%) followed by gastrointestinal (6 of 17, 35%). Also, 14 of 38 (37%) were O-CAUTI 2. Central fever was the most frequent source of fever for the noninfectious source (9 of 14, 64%) followed by drug fever (2 of 14, 14%). Of 38 patients, 3 (8%) had both an infectious and noninfectious reason for fever (CAUTI 1 and 2); 4 patients had no fever. Furthermore, 2 were O-CAUTI 3 (repeat culture positive) and 2 were O-CAUTI 4 (1 with hematuria and renal cell carcinoma and 1 with dysuria without leukocytosis). Conclusions: NHSN CAUTI definitions capture UTIs and other events. In FY2018, there were no true CAUTIs in 5 of 12 months (42%). Also, 50% of CDC CAUTIs were not UTI but could lead to inappropriate antibiotic use. Reviewing only CAUTI reduction work in O-CAUTIs prevents the assessment of other appropriate opportunities for improvement.Funding: NoneDisclosures: None

For the past 20 years, debates about public education have been dominated by standardized tests, but the emphasis on standardized tests in education rests on a set of false assumptions, explains Joshua Starr. First is the technocratic assumption that collecting the right data will naturally lead to the right kinds of improvements. Second, testing as currently performed assumes that mathematics and English language arts are the most essential domains of knowledge and skill. Resumen
Enterprise slack, defined as undistributed profit and earned surplus, is a key factor to consider when developing enterprise strategy based on opportunity and risk management. Slack also provides an unrestricted, but indispensable, financial resource for firms to carry out improvement and innovation, exploitation and exploration, and financial activities according to their strategy. In the comprehensive opportunity and lost opportunity control model previously developed by the author (Nishimura, 2015; 2016), slack fulfills its function effectively. Therefore, to make the model more practicable and functional, this paper will examine the substance and functions of slack more definitely by analyzing financial slack and its relationship with business strategy in five innovative manufacturing companies in Japan.

Resumen
Purpose – The purpose of this paper is to assess the current use being made of the sustainability message within business improvement activity and project justification within individual businesses. Design/methodology/approach – This is a reflective paper that looks at an illustrative case study against the wider perspectives of Lean Six Sigma (LSS) implementation, market orientation and business improvement initiatives in general. Findings – The piece discusses the gains derived from the LSS projects and identifies gaps in potential benefits in particular the paper questions how well any potential marketing messages were exploited by the organisation. The findings critically evaluates the impact that marketing function involvement could have in helping to promote the wider productivity message and its contribution to the wider corporate social responsibility (CSR) strategy for the organisation. The document also suggests how the company could leverage internally focused projects to adopt a companywide marketing orientation and use such leverage to support future LSS project selection and justification. Research limitations/implications – The observations are limited to a single case study, although tempered by the authors’ wider experience. Further empirical research is required to validate the observations and conclusions. Practical implications – The proposed approach to future improvement initiatives can help to increase the wider productivity message and assist organisations to exploit their internal projects in external communications and wider CSR strategy. Originality/value – The paper proposes an approach that can be used to widen organisational involvement in improvement initiatives allowing future project justification to be less internally cost focused, more holistic with a focus on customer voice.

Resumen
Purpose – The purpose of this paper is to present coaching opportunities and applications in the workplace as well as to point out that organizations that want to leverage the benefits of coaching must be mature enough to have certain processes and practices in place. A further purpose of this paper is to gain some insight regarding several critical success factors are not well understood by organizations and to identify possible areas for improvement for them. Design/methodology/approach – The authors first presents the environment in which coaching was developed, from which disciplines was it affected, and how it was shaped into its current form. Then the authors focus on coaching used as a business development tool and critical factors that play an important role in the effectiveness of coaching from the organizations’ side are presented. The paper ends with some qualitative conclusions. Findings – The value that organizations realize form coaching is proportional to the quality of coaching delivered. Organizations that invest in quality coaches, have, themselves, a clear understanding of what coaching really is and actively support coaching initiatives at every aspect of coaching’s procedure, can reap the greatest benefits from it. Originality/value – Several studies have been conducted to determine the organization-dependent factors that affect coaching and the quality of the result. However, do date it has not been highlighted that organizations that want to leverage the benefits of coaching must be mature enough to have certain processes and practices in place. Furthermore, possible areas for improvement for companies are identified regarding several critical success factors that are not well understood by them.
