129 Background: While Electronic Medical Records (EMRs) have been widely adopted within primary care, practices are at various stages of maturity in their use. Cancer Care Ontario (CCO) has embarked on an EMR Optimization initiative, in collaboration with primary care providers (PCPs) and OntarioMD. The project is intended to support PCPs in leveraging the functionality in their EMR systems in order to support cancer screening through the development of training guides and tools. Working with the CANES Family Health Team (FHT), CCO is piloting the usefulness and effectiveness of these tools in meeting these objectives and using the results to inform how to support provincial pilots. Methods: The EMR Optimization guides and tools provide practices with training on five key areas: practice workflows, data discipline, searches, reminders and preventative care summary reports. Working with a multi-disciplinary team at the CANES FHT, CCO provided the team with the EMR Optimization tools and training on these five key areas. The training was supplemented with exercises on topics such as data discipline and practice workflow. The practice team then had opportunities to work with the guides and tools and integrate them into their daily workflow. Results: Preliminary results of an EMR maturity survey show that even after a short period of time, the team has developed an awareness of and a focus on improving data quality through standardization. In addition, they have learned how to leverage the functionality of their EMR system for preventative care purposes. Participant feedback indicates that they found the EMR guide and tip-sheets easy to understand and follow. Conclusions: The CANES pilot has resulted in practice staff moving up the EMR maturity curve and has taught the team how to leverage their EMR to support cancer screening. In addition, the pilot has helped inform CCO’s training strategy and refine the guides and tools. The next steps are to develop self-directed training tools and resources that will support a broader scale pilot to be implemented throughout 14 regions in Ontario.

Abstract: In 2014-2015, the Montreal Museum of Fine Arts (MMFA) and Concordia University's School of Extended Learning conducted a joint pilot project with the objective of determining the effectiveness of a new approach for training volunteer museum guides based on a dialogic method. In this paper, we address: (i) the conceptualization of alternative training based on dialogic concepts; (ii) the development of a new course outline; (iii) the trial period during which volunteers took part in test training; (iv) and, finally, aspects of what we learned during this undertaking. Purpose. The objective of the article is to assess the quality of services provided by tour guides and tour group leaders within the context of deregulation of these professions. Method. A survey questionnaire consisting of 14 questions characterising both traits and skills of tour guides and tour group leaders was used as a research tool to achieve the defined objective. The survey was conducted in the period from March 15 to May 31, 2018 among tourists participating in domestic and foreign trips. Correctly completed questionnaires (274) were collected in the course of the study, of which 121 referred to work quality of mountain and field guides, 103 concerned the quality of services provided by tour group leaders and 50 related to work quality of city guides. Findings. The average assessment of all analysed traits was very high and amounted to 4.58 (!) on a five-point scale. The highest rated traits and skills were as follows: punctuality (4.75), knowledge of topography (4.73), personal manners (4.71), level of professional knowledge (4.7), efficiency in realising the itinerary (4.67), patience and self-control in action (4.6) and neat and good appearance (4.6). In turn, the lowest assessed features were the ability of selecting information (4.3), the skill of making contact (4.4) and the ability of expressing oneself correctly (4.41). Research and conclusions limitations. Based on the conducted research, it cannot be determined whether the deregulation of both professions, i.e. tour guide and tour group leader, had any impact on the quality of provided services. The survey covered tour guides and tour group leaders who obtained their licences before the deregulatory regulations entered into force. No group of tour guides or tour group leaders who worked without completing any training was identified. Practical implications. The research results can be used by diverse institutions introducing various types of certification systems for tour guide services as well as the Ministry of Sport and Tourism in the course of implementing the Sectoral Qualifications Framework in tourism. Originality. The analysis of both tour guide and tour group leader quality of services is not often carried out in our country. The value of the study is the possibility of comparing current research results with the ones from 2006. Type of paper. The article presents empirical research results focused on the quality of services provided by city, field and mountain guides as well as tour group leaders.

A pilot guides a ship through a safe waterway based on extensive experience and knowledge of the region for piloting so that the ship can berth quickly and safely. There are insufficient studies on pilots who play crucial roles in pilotage, and most of the available studies have focused on psychological stress or tension. In this study, we group pilots through quantitative analysis of data to help in the safety management of pilots. In particular, berthing velocity and berthing energy were employed as basic data, and the mean and standard deviation values for each pilot after preprocessing were used for analysis. As a result of using the agglomerative clustering algorithm, we could group pilots into three types: cautious, efficient, and hazardous. Further, analysis of variance (ANOVA) showed that there was a significant difference between the cautious and hazardous types.

Objective. To investigate if 3D printed guides and preoperative planning can accurately control femoral stem anteversion. Methods. A prospective comparative study was carried out from 2018 to 2020, including 53 patients who underwent hip arthroplasty for femoral neck fracture. The target rotation center of the femoral head is determined by three-dimensional planning. In group A, planning was made by 2D templates. In group B, preoperative 3D planning and 3D printed osteotomy/positioning guides were performed. After the operation, 3D model registration was performed to calculate the accuracy of anteversion restoration. Results. We screened 60 patients and randomized a total of 53 to 2 parallel study arms: 30 patients to the group A (traditional operation) and 23 patients to the group B (3D preoperative planning and 3D printed guide). There were no significant differences in demographic or perioperative data between study groups. The restoration accuracy of group A was 5.42 ° ± 3.65 ° and of group B was 2.32 ° ± 1.89 ° . The number and rate of abnormal cases was 15 (50%) and 2 (8.7%), respectively. Significant statistical differences were found in angle change, restoration accuracy, and number of abnormal cases. Conclusion. Three-dimensional preoperative planning and 3D printed guides can improve the accuracy of the restoration of femoral anteversion during hip arthroplasty.

Background: Drug switching describes switching between drug products with the same active substance. Drug switching occurs commonly in the Netherlands and mostly between generic drug products, however, the specific reasons for switching are incompletely understood. Methods: To document reasons for drug switches between products with the same active substance in the Netherlands. Results: In total, 207 drug switches were recorded. Most drug switches were caused by nationwide drug shortages (32%, n = 66) and the Dutch price-based tender system (23%, n = 47). Other reasons for switching included deals between pharmacists and wholesalers (12%, n = 25), distribution issues at wholesalers (11%, n = 22), and a financially favourable margin for pharmacists (11%, n = 21). Conclusion: This study indicates that drug shortages and the Dutch price-based tender system are likely to be major causes of drug switches in the Netherlands. However, other reasons, such as drug product distribution issues and local economic incentives, were also identified.

Particular emphasis is dedicated to Knowledge Management activities within the EURAD (European Joint Programme on Radioactive Waste Management) and PREDIS (Pre-disposal management of radioactive waste) projects to ensure the capture of existing knowledge, transfer of knowledge between Members States and management of the knowledge for future generations. The EURAD project has three work packages dedicated to knowledge management. One of them, the EURAD Guidance work package (WP12) is developing a comprehensive suite of specific guidance documents that can be used by Members States with radioactive waste management (RWM) programmes that are at an early stage of development but can be beneficial also to more advanced programmes. The PREDIS project does not have a specifically allocated work package for guidance development. Rather, such activities are integrated within deliverables produced as part of the Strategic Implementation and State of Knowledge actions of the Roadmap contributions on predisposal waste management. The EURAD guidance work is based on the existing PLANDIS guide on RD&D planning, developed by the Implementing Geological Disposal of Radioactive Waste Technology Platform (IGD-TP). The guidance documents complement other documents prepared in parallel knowledge management activities inside EURAD project: the State of Knowledge documents. The differentiation is that guidance documents explain in more detail how the process can be established and performed, including illustrative examples. The guides are self-standing documents and integrated with the EURAD Roadmap. The target end users of the guidance are primarily programme owners and managers (i.e., governments/administrations, Waste Management Organisations, Research Entities and Technical Support Organisations) responsible for planning and implementing the RWM programme and the supporting RD&D activities at a national level, even though they might also be of use and interest to other interested stakeholders, such as representatives of civil society. To produce a first list of prioritised topics for guidance documents with the aim to select a topic for a pilot guide, the Guidance WP has developed a screening process that includes review by experts and end users. Based on the priority list, the first pilot guide was developed with the title “Cost Assessment and Financing Schemes of Radioactive Waste Management Programmes”. Experience gained during the selection of topics for the pilot guide and during its production are being incorporated into the procedure for identification of new topics for which guides will be developed. First, the degree of coverage of the EURAD Roadmap themes by suitable guide documents will be analysed by the WP 12 team. The analysis will be combined with feedback from experts verifying the needs for missing guides. Finally, the potential end user community representatives will be given the opportunity to comment on the prioritisation of selected guidance documents and make additional suggestions. The potential end users stay involved also during the production of the guides. This procedure aims to optimise the scarce expert resources in relation to the identified needs of guidance documents. This article explains the approach for selecting topics for guidance documents and the results obtained both in EURAD and PREDIS.

In production processes uncertainty has a great impact on the product quality as well as production costs. In automotive industry the reaming of valve guides in a cylinder head of a combustion engine is a quality determining process. Due to the force fitting of the valve guides into the cylinder head the final reaming process has to deal with increased uncertainty. On the other hand, the finished hole is closely tolerated. To ensure the process reliability the admissible tolerance must be strictly met even in case of uncertainty. This paper presents a possibility to achieve process reliability by a modified process chain with an additional pilot reaming tool. Thereby, the effect of different cutting edge preparation is also analyzed. Further, the influence of the pilot reamer geometry on the final hole quality is investigated.

This article presents a pilot experiment on subjects from three Social Sciences degrees at two public Andalusian universities. The aim is to analyze the teaching guides and their presentation to students as a means of communicating with them, primarily through the video format instead of the conventional written format. To this end, attractive, clear and relatable video teaching guides have been developed to present to students, aware of the success of short videos dominating digital social networks, especially among younger age groups. A knowledge test was given to 99 students on the subject contents, as well as a satisfaction test; the following information was processed through the SPSS program. The main conclusion is that students have a high degree of satisfaction and a better knowledge of the new video guide format than the conventional written model. 100 Background: Cancer Care Ontario endorses patient reported outcome measures to improve outcomes and experience for nearly 14 million Ontarians. The EPIC-CP tool, validated to screen/monitor symptoms and side-effects in men with localized prostate cancer, was selected to improve patient and provider experience, and facilitate symptom management. Two pilots tested EPIC-CPs feasibility and acceptability. Subsequent recommendations include: province-wide implementation, improving technological privacy, patient and provider education and communication processes. This abstract will describe the provincial strategy for implementation of EPIC-CP. Methods: The implementation approach involved stakeholder-driven practices based on Kotter’s organizational process framework. Clinical, technical, administrative and patient stakeholder representatives from 14 cancer centres formed working groups to create a climate for change, to engage centres to strategize locally, to implement and sustain change and to address the challenges identified by the EPIC-CP pilot. Results: The final pilot ended in June 2015, and executive endorsement for EPIC-CP provincial implementation in March 2016. A schedule for multi-site phased implementation was informed by stakeholder consultations and began in Oct 2016. Technological privacy improvements were informed by 95 representatives creating a multidisciplinary team tasked with provincial oversight, development of EMR guidelines and IT solutions. Five patient and five clinical educational guides were designed to assist in symptom management, each focusing on one domain of EPIC-CP. Creation of the guides drew on the clinical and scientific expertise among 12 clinicians of varying disciplines in collaboration with four patients. This team assisted in enhancing communication processes by designing 21 training materials, including FAQs and narrated guides, accessible on a central communications hub. Conclusions: Results indicate that this framework-based, stakeholder-driven approach was successful and could be applied to other wide-scale implementations of symptom management tools.

This article presents the results of a teacher leader’s pedagogical practices in a pilot test on innovation implemented in a Costa Rican primary school. The pilot test was conducted due to the need of renewing the technological equipment in schools belonging to Conectándonos MEP-FQT project run by the Department of Research, Development, and Implementation of the Ministry of Public Education and Quiros-Tanzi Foundation. This pilot test aimed to diagnose the pedagogical practices of a teacher leader through the exploration and use of didactic digital resources of Clamshell-type laptops and an interactive projector. This equipment was used to favor the teacher’s pedagogical mediation through the access, selection, and use of didactic digital resources. To collect the information, we used non-participant observation guides, a questionnaire with open questions, and a bibliographic review of the teacher’s didactic planning. The systematization of the observation guides permitted to put in evidence, naturally and concretely, the aspects covered by the objectives of the study. The information was analyzed with a qualitative approach of phenomenological type. This approach defined categories and subcategories that, later, were linked to establish coincidences, similarities, differences, and the presence or absence of terms and phrases. Likewise, the data were interpreted to relate the information collected through the analysis and the theoretical patterns. This interpretation permitted, afterwards, to reflect on the studied categories. Finally, the main conclusion drawn from the pilot test was that the collaborative work between students was promoted and the learning objectives in the didactic planning drawn up by the teacher were achieved.

Summary Objectives: The exchange of electronic medical data between healthcare providers constitutes an integral part of modern medicine, and its importance is growing. Efficient application on a national level requires a uniform approach to the management of healthcare data exchange, avoiding isolated solutions that are expensive and also incompatible. Methods: In this communication we explain the basic concepts of establishing a nationwide framework to guide healthcare data exchange in Austria. To achieve this goal, a three-step approach was adopted: (i) creating general guidelines to direct electronic medical data exchange; (ii) defining detailed standards for electronic messages; (iii) organizing pilot projects to implement these standards, and further improving the general guidelines based on the results of the pilot projects. Results: We present the MAGDA-LENA framework which guides healthcare data exchange in Austria, and compare it with the US framework HIPAA. We describe several communication scenarios for which concrete message standards were developed in recent years, based on the MAGDA-LENA framework. We further discuss the implementation of these standards in four pilot projects. Conclusions: The strategic approach of managing healthcare data exchange presented in this paper is expected to have a substantial impact on medical informatics in Austria over the next few years.

Background Posttraumatic stress disorder (PTSD) is a major public health concern among American Indian and Alaska Native populations. Primary care clinics are often the first point of contact for American Indian and Alaska Natives seeking health care and are feasible locations for trauma-focused interventions. Objective Web-based therapeutic interventions have the potential to reduce PTSD symptoms by offering psychoeducation and symptom self-management tools. We investigated the feasibility of a culturally adapted Web-based therapeutic intervention in two American Indian and Alaska Native–serving primary care sites. We developed and tested a self-guided Web-based therapeutic intervention aimed at improving knowledge and awareness of, and provision of guidance, support, and symptom-management for, PTSD symptoms. Methods A community-based participatory research process was used to refine adaptations to the veteran’s administration’s PTSD Coach Online, to develop new content, and to guide and interpret the results of the feasibility pilot. This process resulted in a 16-guide intervention “Health is Our Tradition: Balance and Harmony after Trauma” website. The feasibility pilot included 24 American Indian and Alaska Natives aged 18 years and older who scored positive on a primary care PTSD screener. Enrolled participants completed a demographic questionnaire, an experience with technology questionnaire, and baseline behavioral health measures. Once measures were complete, research staff described weekly text messages, minimum study expectations for website use, and demonstrated how to use the website. Feasibility measures included self-reported website use, ratings of satisfaction and perceived effectiveness, and website metrics. Feasibility of obtaining measures for an effectiveness trial was also assessed to include behavioral health symptoms and service utilization through self-report instruments and electronic health record queries. Self-reported measures were collected at enrollment and at 6 and 12 weeks post enrollment. Electronic health records were collected from 12 months before study enrollment to 3 months following study enrollment. Changes between enrollment and follow-up were examined with paired t tests, analysis of variance or logistic regression, or the Wilcoxon signed rank test for nonnormally distributed data. Results The culturally adapted website and associated text message reminders were perceived as satisfactory and effective by participants with no differences by age or gender. The majority of participants (86%, 19/24) reported use of the website at 6 weeks and nearly all (91%, 20/22) at 12 weeks. At 6 weeks, 55% (12/22) of participants reported using the website at the recommended intensity (at least three times weekly), dropping to 36% (8/22) at 12 weeks. Participant use of modules varied from 8% (2/24) to 100% (24/24), with guide completion rates being greater for guides that were only psychoeducational in nature compared with guides that were interactive. There were no significant changes in patterns of diagnoses, screening, medications, or service utilization during exposure to the website. Conclusions “Health is Our Tradition: Balance and Harmony after Trauma” shows promise for an effectiveness pilot.

Academic librarians face increasing challenges when trying to connect their services and resources with faculty and students, especially as courses move online and are taught by adjunct faculty. Online research guides, most commonly LibGuides, can be automatically inserted into course shells within Learning Management Systems (LMS) using Learning Tool Interoperability (LTI) protocols, but an alternative to automatic integration is for librarians to match and suggest appropriate subject guides to faculty for inclusion in their courses. Personally matching and recommending LibGuides to faculty can be an effective form of library outreach and relationship-building. This article describes a pilot project of suggesting specific LibGuides to faculty and its success in forging and strengthening several library-department connections.

The success of implant-supported restorations depends on the treatment planning and the transfer of planning through the surgical field. Recently, new computer-aided design and manufacturing (CAD/CAM) techniques, such as stereolithographic (SLA) rapid prototyping, have been developed to fabricate surgical guides to improve the precision of implant placement. The objective of the present case is to introduce a recently developed SLA surgical guide system into the rehabilitation of a 62-year-old male patient with mandibular edentulism. After obtaining a cone-beam computerized tomography (CBCT) scan of the mandible with a radiographic template, the images were transferred into a 3-dimensional (3D) image-based software for implant planning. The StentCad Beyond SLA surgical guide system, which is a combination of a currently used surgical template with pilot hollows and a surgical handpiece guidance apparatus, was designed to transfer a preoperatively defined implant position onto the surgical site without any drill-surgical guide contact. For the fabrication of this system, a surgical handpiece was scanned by a laser optical scanner and a mucosa-supported surgical guide was designed according to the patient's 3D model, which was attained from the CBCT images. Four dental implants were inserted through the SLA surgical guide system by a torque-controlled surgical handpiece to the interforaminal region via a flapless surgical procedure. Implants were assessed 3 months after surgery, and an implant-retained mandibular overdenture was fabricated. The present case emphasizes that CAD/CAM SLA surgical guides, along with CBCT images and scanning data, may help clinicians plan and place dental implants.

Introduction: As elsewhere in the world, Quebec (Canada) is currently facing the COVID-19 pandemic. Approximately 92% of deaths have occurred among people aged over 70, and approximately 100 long-term care (LTC) centers (termed CHSLDs in Quebec) were contaminated. This alarming situation is prompting stakeholders from healthcare networks to investigate the socio-economic repercussions of COVID-19. To the best of our knowledge, there is no valid and reliable tool to measure the costs associated with the management of COVID-19 in CHSLDs. Objectives: This research protocol aims to: i) adapt and validate for use in CHSLDs a combined guide, Cout-COVID19-SLD, developed from 2 guides used in acute care; ii) pilot the Cout-COVID19-SLD guide in CHSLDs and test its feasibility and afterwards resolve any barriers to its administration, and to conduct a partial estimate of costs brought about by COVID-19. Methods: A two-part prospective study will be conducted. Phase 1 will use a Delphi approach with 14 to 17 experts to validate the content of the Cout-COVID19-SLD guide. Phase 2 will pilot test the guide in a cross-sectional study in two CHSLDs. Discussion and conclusion: This study will provide a validated guide for the systematic measurement of costs associated with the management of COVID-19 (costs of preventive measures and costs of illness) in CHSLDs. Finally, this guide will serve as a valid and reliable instrument with which to better plan future research surrounding the socio-economic impacts of COVID-19 in CHSLDs.

The US Army Public Health Center developed the Creating Active Communities and Healthy Environments (CACHE) Toolkit to help military installations evaluate the quality of their built environments relative to healthy eating, physical activity, and tobacco-free living. This study sought to improve its implementation process and assess subsequent Action Plan Guides’ utility at 5 military installations. Baseline data included a knowledge, attitudes, and beliefs survey (N = 34); post-Toolkit implementation data included focus groups (N = 2) and interviews (N = 10). Although >80% of participants agreed the built environment affects healthy living, only 44%, 53%, and 35% agreed their installations’ built environments promoted healthy eating, physical activity, and tobacco-free living, respectively. Emerging themes comprised “Opportunities to Improve Toolkit and Action Plan Guide Functionality,” the “Sociopolitical Landscape Affects Toolkit Implementation,” and the “Sociopolitical and Physical Landscapes Affect the Toolkit’s Value and Utility.” This study provides concrete lessons for the CACHE Toolkit and other public health-based military initiatives.

Background: Autism spectrum disorder (ASD) affects the daily functioning of children and their families; however, in Argentina, there are no standardized tools to guide the description, evaluation, and follow-up of functioning and disability of children with ASD. To fill this gap, the overarching purpose of this study was to create a novel tool guided by the International Classification of Functioning, Disability, and Health (ICF) Core Sets for ASD for clinical practice. Methods: A multistep methodology was used to identify the most relevant ICF categories for an Argentinian clinical setting. The content of this ICF-based shortlist was piloted and revised according to the results. Subsequently, a toolbox of measures was proposed to operationalize each ICF category. Finally, profiles of the functioning of 100 children with ASD were created. Results: An ICF-based tool called TEA-CIFunciona was created, consisting of 32 ICF categories (10 body functions, 15 activities and participation, 7 environmental factors categories). The application of TEA-CIFunciona incorporated a family-centered approach in ASD evaluations and helped identify functional needs. Conclusions: TEA-CIFunciona is the first ICF-based instrument that guides the description of functioning of children with ASD in Argentina. TEA-CIFunciona standardizes collaborative assessments in pediatric ASD populations in Latin American contexts.

The temporomandibular joint (TMJ) is one of the most complex joints in the human anatomy. In advanced degenerative stages, conservative or minimally invasive surgical therapies have failed to restore joint function, and joint replacement with prostheses has been required. Stock prostheses, compared to custom-made prostheses, are much less expensive and require less pre-operative preparation time. Four patients followed for years for temporomandibular dysfunction and previously operated on by arthroscopy or open joint surgery that have been reconstructed with stock TMJ prostheses (STMJP) through virtual surgical planning (VSP) and an STL model with surgical and positioning guides were included. The median follow-up was 15 months; the median number of previous TMJ surgeries was 2. The mean preoperative MIO was 24.6 mm and at longest follow-up was 36.4 mm. The median preoperative TMJ pain score was 8, and the median postoperative TMJ pain was 3. All patients have improved their mandibular function with a clear improvement of their initial situation. In conclusion, we believe that stock TMJ prostheses with virtual surgical planning and surgical guides are a good alternative for TMJ reconstruction at the present time. Nonetheless, prospective and randomized trials are required with long-term follow up to assess their performance and safety.

The implementation of K to 12 curriculum in coastal villages in the Philippines posed a pressing concern on the dearth of contextualized instructional materials (IM). This study aimed to generate the cultural funds of knowledge and translanguaging praxis of migrant fishers as bases for the development of contextualized IM. Under the lens of narrative ethnography, knowledge, beliefs, and practices of migrant fishers were documented in memory bank charts from six months of immersion in the coastal villages. Translingual narratives of purposively chosen participants were analyzed in the QDA Lite Miner software. Three translingual storybooks and teacher’s guides were developed, validated, evaluated, and pilot tested. It resulted to “very acceptable” for the storybooks and “excellent” rating for the teacher’s guides. The study recommends that school heads, teachers, and parents in the coastal villages may interact beyond the classroom setting and their indigenous knowledge will be used in the development of IM.

Reflection enables students to develop a deeper degree of learning and identify appropriate opportunities for improvement. As such, remediation coaches should support lowperforming students to reflect on their learning experiences and, then, make positive changes through a guided, structured, and facilitated reflection. This study was an attempt towards structuring facilitated reflection (i.e. a structured and guided reflection) among low-performing students. Gibbs’ model was adopted as the theoretical framework to develop the reflection approach. Herein, there are three main steps in the reflection process: (a) Engagement: a remediation coach identifies and engages with appropriate students; (b) Active reflection: students complete and submit reflective journals, the remediation coach reads and highlights responses that require clarifications, and then the coach guides the students through their reflection; and (c) Monitoring and follow-up by the remediation coach. In a pilot implementation, students’ journal excerpts and dialogues were the supporting evidence used to analyse if effective reflection had taken place, with the student expressing positive and encouraging experiences of the facilitated reflection. These positive experiences suggest that this kind of structured and guided reflection is useful in encouraging reflective practice among low-performing students.

HIV pre-exposure prophylaxis (PrEP) reduces HIV acquisition among adolescent girls and young women (AGYW). Existing evidence suggests that uptake and adherence are low among AGYW and that relationship factors such as gender-based violence (GBV) are important barriers. Through a community-based participatory research (CBPR) process, a youth advisory board (YAB), service providers (SP), and a study team developed the Tu'Washindi na PrEP intervention to support AGYW PrEP use in the context of GBV. The YAB also guided the formative research and interpretation of results. The authors pretested the intervention with SP, AGYW and their partners, and community change agents, and then developed guides for AGYW support clubs, community-based male sensitization sessions, and couples-based events that included formulation of story lines for dramatized PrEP negotiation and information dissemination skills. Stakeholder engagement led to an intervention responsive to AGYW's needs for PrEP support in the context of their relationships, which was evaluated through a 6-month pilot community randomized controlled trial.

This pilot study assesses the accuracy of 2 bone-supported stereolithographic surgical templates with respect to placement of implants in originally planned positions, and it tests the precision of dental volumetric tomography planning. Two mandibles retrieved from formalin-fixed human cadavers were scanned by dental volumetric tomography for planning of the implant positions, leading to stereolithographic models and fabrication of surgical guides. The situation immediately following drilling and implant insertion by an experienced surgeon was scanned and the outcome compared with the initial planning. The 3-dimensional discrepancies were then analyzed and determined. The results show deviations of the placed implants from the original planning, especially in the vertical direction, making the seating of a prefabricated denture impossible. At present, the flapless surgery technique based on stereolithographic surgical templates appears unsafe; further improvement is required.

This study presents a pilot study in suburban households in Salvador, Brazil, inserted in the context of a framework developed to aid water demand management strategies. The framework aims to understand the barrier of subjectivity while identifying consumption habit patterns in households. Six key sets of components create the framework architecture: (1) characterization of the area based on: context, climate, population/area, population growth rate, and water management challenges; (2) a survey to obtain socio-demographic and physical property data of the sample; (3) smart metering and data processing systems to monitor sample water end use; (4) determining daily consumption patterns; (5) analyzing qualitative data through theoretical consumption models to identify relevant variables for the next step; and (6) construction of representative mathematical models of consumption for each daily practice (this item was not included on pilot). It provides a starting point to understand how water demand management strategies can be supported at the user and decision-making level. As a result, improvements to the interview guides used in the pilot were suggested. Furthermore, customized measures to promote rational water consumption were identified in the study area, and policies could be proposed.

Background and Purpose:Parents’ competencies in coparenting are critical to adaptive and competent caregiving of an infant with complex congenital heart disease. To date, feasible interventions to support parents in working together—coparenting—for caregiving of these infants have not been developed and systematically examined. The purpose of this feasibility study was to examine the efficacy of the participatory teaching/learning intervention, Guided Participation (GP) on parent dyads’ competencies in interactive problem-solving tasks in preparation for a randomized controlled trial.Methods:Nurse guides used GP to support mother and father couples in developing coparenting competencies through the first 6 months after birth. Couples, enrolled from two regional heart centers, were randomly assigned either to the usual care group (n= 10) or the GP group (n= 24). Intervention involved nurse guided GP in hospital and, following the infant’s discharge, monthly telephone GP sessions between 2-months and 6-months infant age. In-home data collection visits at 2 and 6 months included video-recorded parent interaction problem-solving tasks with two goals, infant caregiving and the parent couple’s relationship. The Iowa Family Interaction Rating Scales were used to score observed interactive competencies.Results:Retention was 82%, and results revealed small to moderate effect sizes for GP on problem-solving constructs for mothers and for the parent couple dyad.Implications for Practice:Our findings support further study in a fully powered randomized trial with a more diverse sample, handbook-enhanced GP, and examination of the effect on a broader spectrum of outcomes, including infant growth and development.

More and more people go/return to nature seeking recreation, relaxation, stress/anxiety relief, socializing with like-minded persons, etc. Not all of them know what they can stumble upon walking through various terrains. We wanted to check how much hikers/mountaineers and those who took a course for excursion and hiking guides know about venomous snakes present in Serbia and the basic first aid in the case of a bite. For this purpose, we designed a questionnaire containing 20 simple questions that addressed recognition, ecology, distribution, the basics of venomous snakes' conservation, first-aid measures, and people's feelings regarding these reptiles. Results suggest that our target group is agreeably well informed given the amount and quality of easily accessible data for Serbia. However, some misconceptions were detected that must be corrected. There are indications of an increase in mountain/adventure tourism in Serbia, which is partially conditioned by the COVID-19 pandemic. Therefore, we must invest additional effort in examining and educating both mountaineers and "plain" tourists so both the people and snakes would stay safe. To achieve this long-term goal, we should exchange knowledge and experiences with experts from various fields, i.e. biology, medicine, tourism, economics, geography, etc.

Introduction: Post-operative misalignment of the lower limb mechanical axis following total knee arthroplasty (TKA) is thought to be associated with clinical failure. In a balanced knee, a neutral global mechanical axis following the use of intra-medullary (IM) guidance does not necessarily imply a horizontal TKA joint line. Using femoral and tibial axes separately would be more accurate in evaluating TKA alignment. Thus, the aim of the study is to define a pre-operative mechanical tibial and/or femoral angle threshold value for post-operative optimal alignment correction using IM guides. Methods: This single-center prospective continuous pilot study included 50 patients treated with a TKA for primitive osteoarthritis. Femoral and tibial cuts were performed using intramedullary (IM) guide. Three angles were calculated and analyzed pre and post-operatively on standing antero-posterior views using long-leg radiographs: femorotibial angle (FTA), mechanical femoral angle (MFA), and mechanical tibial angle (MTA). Statistical analysis was performed for the whole sample and for the three following FTA subgroups; normo-axial, varus and valgus. Results: The pre-operative MTA is the only parameter for which a threshold value was observed; when pre-operative MTA exceeded the value of 94°, an optimal correction might not be obtained post-operatively. Discussion: Our results suggest that the bony correction obtained via IM guiding depends exclusively on the primary deformation of the tibia. In cases of a varus of more than 94°, the IM guide was found to yield sub-optimal corrections. Thus, other solutions need to be investigated.

PurposeThe purpose of this paper is to present a conceptual framework for benchmarking implementation in small medium‐sized enterprises (SMEs) taking into consideration their characteristics.Design/methodology/approachThe paper begins with the review on the definition of SME and a comparison of the characteristics of SMEs and large organizations. It presents the need for a framework and its relationship with benchmarking and TQM. This is followed by reviewing the benchmarking implementation frameworks proposed by researchers and discusses these frameworks based on their strengths and weaknesses from SMEs perspective. The frameworks were categorised into two broad types based on the different writer's background and the approach on how they view the benchmarking implementation process.FindingsThe paper suggested a conceptual framework for benchmarking implementation dedicated to the automotive manufacturing SMEs. This framework guides them through from the start to end of the benchmarking process. The framework was validated at six pilot case study companies, which gave useful comments and suggestions regarding the usefulness and applicability within the SMEs context.Research limitations/implicationsThe conceptual framework is still in the development stage and research is undertaken to include the pilot study companies suggestions and comments into the final version of the framework.Practical implicationsThis guidance and framework provides a useful guide for companies to adopt and adapt before embarking on their benchmarking journey.Originality/valueThis paper fulfils an identified knowledge gap and offers practical help to SMEs starting out a benchmarking implementation effort.

Pellet fluidized bed reactors have been widely used to soften water. Reports from laboratory-scale research on the particle growth kinetics of calcium carbonate in pellet reactors have been put forward. However, the reports have not been comprehensive as they only consider the influence of supersaturation on the calcium carbonate growth process. The influence of three factors, namely, the superficial velocity (SV), particle size (L0), and supersaturation (S) on the particle growth rate of calcium carbonate were investigated in a pilot-scale study, and two models of particle growth rate and fixed bed height growth rate were built. The linear particle growth model G = 3.90 × 10−SV1.93L0−1.56S2.13 at the bottom of the pellet reactor was built based on a pilot-scale study of particle growth kinetics influenced by SV, L0, and S. The growth of the fixed bed height is closely related to the particle growth and also influenced by the three factors. The fixed bed growth model Rh = 5.19 × 10−8SV1.65L0−0.93S2.58 also incorporates SV, L0, and S, and provides a method for calculating the fixed bed height. The two models were built based on the pilot-scale experiment and were different from those previously reported. They are applicable as pellet discharge guides and are used in the management of pellet reactors.

This paper examines the recent growth of government-led affordable housing in Guangzhou, addressing a paucity of global housing studies that explore experimental and contextual policy approaches in China. It also addresses the lack of Chinese housing studies recognizing the impact of housing design governance, including regulatory controls and design standards, on housing preferences, supply and lifestyles. Since 1995, the supply of affordable housing has surged, now surpassing that of market housing for the first time. This response to failures in the private housing market and a lack of equitable access to housing signifies a significant shift, acknowledging the need to re-establish a state-led and long-term public housing supply after decades of housing marketization. Employing an architectural design research perspective, this paper investigates the interplay between affordable housing supply and the emergence of housing standards, examining resulting housing design outcomes. It poses the question: What changes in housing policy and interventions in housing markets are necessary to increase public rental housing supply, and how do these changes affect housing outcomes? The paper explores these questions through a discussion of the key moments in affordable housing policy and housing estate development in Guangzhou that facilitated the creation of widely accessible public housing and long-term housing assets. This provides new insights into China’s unique approach to translating central government social welfare and housing policy through contextual design experimentation and pilot housing projects, departing from the conventional top-down policy implementation found in most other countries.

New digital technologies are improving the accuracy of orthognathic surgery. One of the new approaches transfers the surgical plan into real surgery without using an occlusal splint. This pilot study aims to validate the splintless approach to orthognathic surgery on a series of cases. Five patients were enrolled. Surgeries were planned using a digital surgical simulation method thanks to three-dimensional images. The splintless surgical approach was planned for maxillary reposition. This consisted of cutting guides and three-dimensionally (3D) printed custom titanium plates. These two were created using the computer-aided design and computer-aided manufacturing (CAD-CAM) technique and were used intraoperatively to guide the osteotomy and repositioning of the bony segments without the use of the surgical splint. The difference between the planned surgery and the real final position was analyzed thanks to superimposition techniques and landmark analysis. Statistical tests were performed to detect significant differences. No difference was found in any of the landmarks. Midline landmarks differed from the planned position by 0.34 mm. Higher variability was found in the posterior landmark. These findings suggest that a splintless approach is useful in transferring the surgical plan without using an occlusal splint.

ABSTRACTObjectiveThe objective of this study was to develop and evaluate an evidence-based information technology (IT) application that guides clinical decision-making during the reverse-triage selection process in mass casualty incidents.MethodsBased upon 28 validated critical interventions (CI) relevant for determining whether a patient qualifies for early discharge, we developed the Reverse Triage Tool of Leuven (RTTL). The RTTL is compatible with the health electronic record (HER) of UZ Leuven, a tertiary hospital in Belgium. During a 3-week period in March 2015, we registered data from 2 groups of patients: a random group (no RTTL usage) and a filtered group (RTTL usage).ResultsWhen applying the original 28 CIs, we were able to select almost twice as many patients in the filtered group who qualified for early discharge compared with patients in the random group. The predictive validity was highly satisfactory.ConclusionsThe RTTL saves time in 2 ways. First, it reduces the patient population that needs to be evaluated for potential early discharge to one-third. Second, it doubles the probability of selecting an actual dischargeable patient. Each selected patient, however, still must undergo multidisciplinary reassessment in order to qualify for early discharge. Mathematics and Statistics Teaching (MAST) is a staff facing service set up by the University of Bath Mathematics Resource Centre in September 2021. The service was created to collaborate with academics involved in mathematics and statistics teaching in their academic departments. In September 2023, MAST ended its pilot phase and entered a new growth phase fine-tuning existing structures and practices. During the pilot phase, MAST worked with academics consulting on enriching their programme of study and/or teaching practices. Specifically, the key principle that guides MAST is engaging in long-term sustainable collaborations with academic departments. As a means to address the key principle, we explore the potential of building communities of practice and supporting a wider network across the range of disciplines who encounter similar objectives. Facilitating a wider community of practice could be challenging, especially considering the tight schedule of the academics involved. We discuss our plans to cultivate communities of practice within departments and how we are working towards meeting our goal of establishing best practice and collecting evidence to support a wider community.

The carbon emission trading policy attributes carbon dioxide emission rights to commodities, representing an important exploration to reduce carbon emissions by the market mechanism. Since 2013, China has implemented the carbon emission trading policy. In order to study whether the carbon emission reduction effect of emission trading policy in pilot areas and what is the pathway, this paper uses multi-period difference-in-differences method and the medintion effect method. An empirical analysis of panel data from 283 prefecture-level cities in China between 2006 and 2019.The research findings demonstrate that: (1) The policy about carbon emission trading significantly reduced carbon emissions,which is still valid after a series of robustness tests, and had a synergistic effect. (2) The policy primarily promotes carbon dioxide emission reduction through innovation in green technology and adjustments in industrial structure. (3) In comparison to the eastern and western regions, the policy performs better in central regions. Furthermore, the effectiveness of emission reduction depends on the intensity of environmental regulations, thus, stricter regulations are more conducive to carbon reduction. This paper provides strong evidence to the expansion of carbon emission trading pilots, and guides the operation of carbon emission trading market in China.

Intimate partner violence (IPV) is a common concern among military Veterans that negatively impacts health. The United States’ Veterans Health Administration (VHA) has launched a national IPV Assistance Program (IPVAP) to provide comprehensive services to Veterans, their families and caregivers, and VHA employees who use or experience IPV. Grounded in a holistic, Veteran-centered psychosocial rehabilitation framework that guides all facets of the program, the IPVAP initiated the pilot implementation of a novel intervention called Recovering from IPV through Strengths and Empowerment (RISE). This evidence-based, person-centered, trauma-informed, and empowerment-oriented brief counseling intervention is designed to support those who experience IPV and to improve their psychosocial wellbeing. This program evaluation study describes clinical outcomes from patients who participated in a pilot implementation of RISE in routine care. We examined changes in general self-efficacy, depression, and valued living, as well as treatment satisfaction among patients who received RISE and completed program evaluation measures at VHA facilities during the pilot. Results from 45 patients (84% women) indicate that RISE was associated with significant pretreatment to posttreatment improvements in self-efficacy, depression, and valued living (Cohen’s d s of 0.97, 1.09, and 0.51, respectively). Patients reported high satisfaction with treatment. Though preliminary results were similar across gender and IPV types, findings from the evaluation of the pilot implementation of RISE demonstrate the intervention’s feasibility, acceptability, and clinical utility in routine VHA care and inform the scalability of RISE. Additionally, findings provide preliminary support for the effectiveness and acceptability of RISE with men. Modification to RISE and its implementation are discussed, which may be useful to other settings implementing IPV interventions.

In some natural parks, introducing a shuttle bus system as an alternative to private cars can help reduce congestion and protect nature. This study examines the pilot program to effectively induce visitors to switch to the shuttle bus, while examining the visitors’ understanding of the expansion of the restricted vehicle section in Shiretoko National Park—a World Natural Heritage site known for its rich natural resources, especially as a high-density habitat for brown bears. There are concerns about conflicts between visitors and brown bears that appear along roads. Visitor restrictions, such as limited access to transportation, limit the use of a place and alter the behavior and experience. Therefore, careful consideration is needed before introducing visitor restrictions. The results of our questionnaire survey of visitors indicated that most favored the expansion of the shuttle bus section. The choice experiment revealed that the guides on the bus and an optional experience encouraged visitors to switch to the shuttle buses. Nonetheless, awareness of the pilot program was low, and some visitors preferred their cars, indicating the need for further promotion of awareness and understanding of these new measures.

Health technology is increasingly recognised as a feasible method of addressing health needs in low and middle-income countries (LMICs). Primary Care 101, now known as PACK (Practical Approach to Care Kit), is a printed, algorithmic, checklist-based, comprehensive clinical decision support tool. It assists clinicians with delivering evidence-based medicine for common primary care presentations and conditions. These assessment and treatment guides have been adopted widely in primary care clinics across South Africa. This paper focuses on the process of designing, developing, and implementing a digital version of the clinical decision support tool for use on a tablet computer. Lessons learnt throughout its development and pilot implementation could apply to the creation of electronic health interventions and the digitisation of clinical tools in LMICs.
