Texto completoThe number of foreign educated nurses (FEN) is in constant progression in Canada. Preceptorship is considered best practice to facilitate the professional transition of FEN in their new environment (Sherman & Eggenberger, 2008). During this period, the challenges for FEN are important because of the differences in culture and practice between the country of origin and the landed one (Johnston & Mohide, 2008). Successful transition is necessary for the well-being of the FEN and their retention in work places, but also for their possible impact on patient’s safety and quality of care (Kawi & Xu, 2009). Inspired by the middle-range theory of transition (Meleis, Sawyer, Im, Messias, & Schumacher, 2000), the goal of this research was to describe the perceptions of FEN and preceptors, in regards to strategies used in preceptorship to overcome barriers and facilitate professional transition of FEN in Quebec’s health system. The results of this descriptive qualitative research were obtained from the analysis of semi-structured interviews with six FEN of French origins and two preceptors. Several strategies addressed to the FEN, the preceptor, the unit’s team members and the hospital organization were identified. Those for the FEN serve to exchange and integrate, accept and optimize, understand and learn, as well as to compare and select; those for the professional community assist to support, guide, personalize, explain and justify. The perceptions of FEN and preceptors vary on many of these strategies. Recommendations for nursing education, practice and nursing research are also provided.

Texto completoIn today’s reality, nurses start to precept soon in their career (Cubit and Ryan, 2011), and often without any prior notice or preparation (Debra, 2014; Elmers, 2010). The purpose of the present study is to explore the experience of nurses becoming a preceptor for a Candidate to the Profession of Nursing Practice [CPNP], within 0 to 3 years of being CPNP themselves. Five young nurses who were newly preceptors participated in semi-structured interviews. Participants were between 24 et 27 years of age. Data was analysed using Benner`s (1994) and Paillé et Muchielli’s (2006) thematic analysis. The theoretical framework of Transition theory by Afaf Ibrahim Meleis (2010) directed the formulation of the purpose of this qualitative interpretive study. Meleis’s theory also accompanied the discussion of the results of the present study. The results of the present study suggest that the young nurses who are newly preceptors are living through the sense of responsibility of shaping the CPNP, they find themselves situated close to the CPNP experience and their experience of becoming a preceptor leads them to reflect on their personal experience as a nurse and a preceptor. Becoming a nurse preceptor early in their career seems to be an opportune moment. However, it would be recommended to standardise the preparation of the nurses for this new responsibility which could ease their experience of becoming a preceptor. Also pairing a more experienced nurse preceptor to guide the new nurse in the process of acquiring the new role of a preceptor would be an asset. Finally a further research could explore the need for support during the experience of becoming a preceptor.

Texto completoThe barriers in transition-to-practice for newly graduated nurses on intensive care units has been well documented. Independent professional practice is, in particular, one of the barriers shared by newly graduated nurses integrating an intensive care unit. This research aims to explore the perceptions of newly graduated nurses and preceptors regarding the professional autonomy development of the newly graduated nurses in a nursing residency program on intensive care units. Nursing residency programs were developed in order to facilitate transition-to-practice of newly graduated nurses. In these programs, the evaluation that assesses clinical competence development can be primarily based on the newly graduated nurse’s autonomy. However, few studies focus on the development of professional autonomy of newly graduated nurses. As study framework, a model that includes Benner's stages of clinical competence (1982), Tardif's competency assessment approach (2006) and a definition of professional autonomy as suggested by Létourneau (2017) is proposed. By conducting a descriptive exploratory study, semi-structured individual interviews collected the data from newly graduated nurses (n=6), while preceptors (n= 4) shared their perceptions during focus groups. Content analysis by Van der Maren (1996) was used for data analysis. The results include the barriers encountered in the early career of the newly graduated nurses in critical care setting, a definition of professional autonomy and the influences of the newly graduated nurses’ professional autonomy development. The influences of the newly graduated nurses’ professional autonomy development and the definition of professional autonomy have been linked in a concept map. Considering these findings, implications for practice, research, education and management in nursing were made.
