Texto completoA metapsicologia consiste no núcleo fundamental da teoria psicanalítica. De acordo com a metapsicologia freudiana, a pulsão só se manifesta no psiquismo na forma de representantes psíquicos, que são de duas naturezas distintas: representações e (quotas de) afeto. A presente pesquisa pretende investigar o desenvolvimento da teoria dos representantes psíquicos da pulsão, privilegiando a abordagem do ponto de vista econômico da metapsicologia freudiana, principalmente no que diz respeito à primeira divisão tópica do aparelho psíquico. Também é apresentada a contribuição de Green para a teoria dos afetos, bem como a concepção de Laplanche acerca do recalque originário e do processo de tradução psíquica. Em seguida, partimos para uma investigação da angústia na obra freudiana, destacando o desenvolvimento do quadro psicopatológico de neurose de angústia. Por último, é feita uma abordagem do pânico, aproximando o quadro psicopatológico contemporâneo de transtorno de pânico do quadro psicopatológico freudiano de neurose de angústia, mostrando que também é possível desenvolver outras explicações de caráter metapsicológico para o pânico, sem abrir mão de categorias utilizadas pelo próprio Freud (a exemplo da noção de desamparo), mas que não foram suficientemente desenvolvidas e associadas a uma teoria mais ampla acerca do pânico. Assim, a presente pesquisa visa contribuir para o estudo das psicopatologias ditas contemporâneas, em especial para o conhecimento da psicodinâmica do transtorno de pânico. Por outro lado, este trabalho também pretende oferecer uma revisão sistemática de alguns dos conceitos fundamentais da metapsicologia freudiana, acrescentando ao corpo teórico da metapsicologia desenvolvimentos realizados por teóricos pósfreudianos.
Metapsychology consists of the basic nucleus of the psychoanalytical theory. According to the Freudian metapsychology, drive is only manifested, in psyche, as psychical representatives, which have two distinct natures: presentations and (quotas of) affect. The present research intends to investigate the development of the theory of the psychical representatives of the drive, being privileged the boarding of the economic point of view of Freudian metapsychology, mainly the first topical division of the psychic apparatus. Also the contribution of Green for the theory of affects is presented, as well as the conception of Laplanche concerning the primal repression and the process of psychic translation. After that, we make an inquiry of the anxiety in Freudian works, detaching the development of the psychopathological set of anxiety neurosis. Finally, a boarding of panic is made, approaching the contemporary psychopathological set of panic disease of the Freudian psychopathological set of anxiety neurosis, showing that it is also possible to develop other explanations, of a metapsychological character, for panic, without abandoning the categories used by Freud himself (for example, the notion of helplessness), but that have not been enough developed and associated to an ampler theory concerning panic. Thus, the present research aims at to contribute for the study of the, said, contemporaries psychophatologies, in special for the knowledge of the psychodynamics of panic disease. On the other hand, this work also intends to offer a systematic revision of some of the basic concepts of Freudian metapsychology, adding to the theoretical body of metapsychology, developments added by post-Freudian theoreticians.

Texto completoPast research has shown contrasting and controversial results concerning difficulties encountered by gifted children (GC). The objective of this study is to link potential difficulties in gifted children's to their self-evaluation and individual characteristics. A sample of 98 children aged between 10 and 15 months whose intelligence quotient (IQ) was equal to or higher than 130 participated in this study. Two methods were selected: a) a collect of anamnesis data from the mother; b) child auto evaluations questionnaires (Personality and Motivation). Analyses are organized in three distinct sections all of them emphasizing endogenous and exogenous factors exacerbating the gifted children's adaptation problems (sociorelational, educative, psychopathological…). Results of descriptive, factorial and typological analyses suggest that gifted children are a heterogeneous population, extreme and often at risk. Having most difficulties was more strongly associated with contextual factors than biological predispositions. Clinical case studies allow us to illustrate the complexity that these difficulties can imply in individual developmental trajectories. Our findings are important for programs aimed at prevention of psychopathological and/or developmental difficulties

Texto completoAlthough early psychiatric symptoms usually appear in adolescence, etiological studies seldom concern this period of life. Since his first writings on attachment theory, Bowlby posited that early ties impact emotional regulation capacities and are thus determining for preventing psychopathology. Empirical findings have evidenced associations between attachment and psychopathology, alexithymia and psychopathology and between attachment and alexithymia, but none articulate these three variables together. The first study —conducted on a sample of 107 adolescents with psychiatric disorders (versus 107 controls)— suggests that attachment (security and model of self) predicts alexithymia and that both these variables are protective factors in the development of psychiatric disorders in adolescence. The second study confirmed this finding on a more specific clinical sample composed of 54 adolescents with borderline personality disorder (PBD, vs 51 controls). The third study evidenced high prevalence of disorganized attachment among borderline adolescents (N=30 vs 31 controls). Discriminant analysis showed that among the attachment dimensions considered, deactivation towards the mother, insecurity with the father and the number of disorganized relationships predicted group membership (clinical vs control) for 90% of the participants. This finding points to the cumulative effect of each attachment relationship, which increases adolescent vulnerability when disorganized

Texto completoWhat can be the efficiency of coarse scales in emotional information processing? Recently, psychological findings about emotional processing of visual information reported a particular link with low spatial frequencies (LSF), coarse and blurred but rapid, which might offer a very fast signal to emotional system compared to high spatial frequencies (HSF) more intricate and detailed. Plus, LSF information might be essential in danger detection and consequently in negative emotions classification. These two types of visual information would take different neural pathways driving to visual information segmentation in the brain. In psychopathological view, neurological disorders as Tourette syndrome or treatment-resistant depression are well known to produce social interaction troubles in which emotions are obligatory. Neurological and psychobiological dysfunctions belonging to these diseases implicate specific neural structures located at peripheral or inside the brain that are bind to functional dichotomy of visual information. One of the simplest ways to examine that processing is the categorization of emotion faces. This research has investigated according to theoretical and practical aspects the extent to which visual information or spatial frequency scaling (SF) might be implicated in categorization of emotional facial expressions (EFE). Thus, both in danger detection and EFE classification, among Tourette syndrome and treatment-resistant depression, we have studied behavioural responses during the first steps of visual information interpretation providing emotional cues. A comparison with healthy control population has given more precise effects of FS filtering in categorization processing with the hypothesis of a benefice to process coarse scales (LSF) compared to detailed signals (HSF) for the identification of difficult EFE in respect with the disorder. Our results have suggested a best identification of specific EFE filtered in LFS compared to HSF or intact images called broad spatial frequencies (BSF). Our empirical findings were argued in the perspective of visual information segmentation in the brain requesting specific neuronal circuits favouring visual information access to emotional complex. Given implicated brain areas and neuronal activities regarding studied disorders, dopaminergic innervation might explain our factual data

Texto completoThe immediate postpartum period contains the basics of the anthropological situation, for the baby as well as for the mother and the father and the socius. Psychic life first matrix subject to “the basic dyadic space” comes from the complex alchemy between the range of maternal investment and disposition and the baby’s original control abilities, without ignoring the play within a play which surrounds mothering. From an extended study in the perinatal field, this research takes an interest, with a double methodology of qualitative study (chapter 1) and quantitative “equipped” one (chapter 2 and 3), in the construction of a dyadic space, essential for the baby’s development and for the maternal investment, to this construction ups and downs, for regulatory functions of these rituals (relayed by current perinatal cares) which gives a scansion to childbirth work. Dyadic communication between mother and baby, with a climax around two months old with the first protoconversations, is studied as an asymmetrical transmodal complex co-genesis by both psychoanalysis and attachment theory, by developmental approaches and neurosciences. This unique and original space, which is renewed after every birth, is woven from “the forming of maternal investment” composed from psychic life vectors and their combination which provides enough energy for the mothering situation. From the three major schools of conceptual thoughts and from therapeutic treatments of early relationship disorder, we identify the forming “in transformation”, the “transmission” one and the forming “in seduction”. These breakthroughs have a triple purpose: to built a new reading of primary intersubjectivity, the building of baby’s self-awareness, normal and pathological mechanism of identification and empathy, to support the principle of cares and the edification of a dyadic semiology of the early psychopathology and to lead to a model that will include the aspect of the “early” and its symbolism form in adult therapy and the institutional support of fragile or borderline population

Texto completoShort-term, mid-range and long-term psychological evaluation of earthquake survivors reveals that certain psychiatric symptoms frequently exist in these populations, specifically PTSD and Depression. The principal objectives of this work were to address current gaps in scientific knowledge related to potential relationships between specific pre, peri and post-traumatic variables and the development of posttraumatic psychopathology in vulnerable groups following the 2010 earthquake in Haiti. Study no 1- Objective : This study examines associations between religious beliefs regarding the origin of the 2010 earthquake in Haiti and posttraumatic symptomatology as well as resilience among survivors. The severity of depressive symptoms was associated with a score <80 on the CD-RISC (x2 = 7.49, df = 1,p <0.01) and were more prevalent among Voodoo Adherents than among their counterparts with different belief systems (x2 = 9.05, df = 1, p <0.01).Study no 2- Objective : This study, conducted one year after the 2010 earthquake, assessed the effect of a psycho-social intervention implemented among children residing in a displacement camp in Port-au-Prince compared with a control group. The two groups were homogeneous in terms of the intensity of the peritraumatic distress they experienced. No significant difference was noted between the experimental and control group in their endorsement of PTSD symptoms or in their scores on a depression scale. In three out of the four sub-scales of the Child Behavior Check-List, there was no significant difference noted either. Study no 3- Objective : In this study, we examined the psychometric properties of a Haitian Creole version of the PTSD checklist among 364 mothers exposed to the 2010 earthquake and the prevalence of PTSD symptoms among them, 3 years after the event. Our findings included that 22,3% of our sample would possibly meet the criteria for PTSD even three years after the earthquake. The creole version of PCL-S is a reliable measure of PTSD symptoms among female creole speakers. Study no 4- Objective : This study investigated the prevalence of autistic behaviors in a cohort of young children (n-364) who were exposed in utero to the devastating 2010 earthquake in Haiti, 3 years after theevent. A significant minority (2.2%) of our sample met criteria for severe autistics behaviors. Study no 5- Objective : This study, conducted 3 years after the 2010 Haiti earthquake targeted women who were pregnant during the earthquake and their offspring who were in utero at the time of the event. Specifically, the study assessed the association between maternal exposure to the earthquake with autistic, behavioral, and emotional symptoms of the offspring at age three. Current maternal PTSD symptoms was an independent predictor of the offspring’s autistic behaviors (=. 370, p<. 001) as well as behavioral and emotional problems (=. 467, p<. 001). Study no 6- Objective: This study comprised a qualitative analysis of a single mother and child dyadscreened in the quantitative survey. In addition to displaying symptoms of autism, the young child also suffers from recurrent seizures. Maternal history of physical and psychological trauma has implications forco-morbidity of autism and epilepsy offspring who were in utero at the time of the trauma and such dyadsshould be targeted for therapeutic intervention and follow-up in the future

Texto completoA psychological study of health within a general anthropology of public health -and quite apart from medical or sociological studies - has not been hitherto attempted as far immigration, its causes and pathologenic consequences are concerned. With this aim in view, the present study has drawn on both traditional and novel clinical methodological approaches. The clinical method (interviews, questionnaire, life stories, projective tests, group psychodrama, psychoterapy) has served as a basis and anchored the research within the domain of social psychiatry. Given the thematic field explored - "double bind and psychopathology of immigration and uprooting" - the research has centred around the construction and experimentation of the normativity of the group as defined in both epidemiological and psychopathological terms. The major hypothesis underlying the work is the suggestion that those who suffer take part in some ambivalent way in the excluding process that affects them in terms of exile, wandering and travel. Closely linked to this hypothesis is the notion that a culture is always so constructed that a blind spot - a kind of 'black hole' - emerges, concerning first and foremost the internal condition of the foreigner within it. This structural condition the foreigner has to address may become systematic and define itself historically, politically, sociologically and clinically in the extreme forms of xenopathy and 'sinistrosis' as well as in cultural hysteresis and psychosocial anachronism

Texto completoBackground: Mental health problems are prevalent in all countries and make up approximately one-third of consultations for general practitioners (GP) in large cities. In France, GPs provide service delivery to the majority of common psychopathological disorders. According to the National College of GP Educators, GPs are well suited to provide follow-up to these patients. Yet, many factors make this type of service delivery difficult: initial medical school training, the particularities of the French health system including a lack of networked service delivery, and poor communication between GPs and other health service providers, to name a few. These difficulties result in an over-usage of prescription psychotropic medications and this to the detriment of other types of therapies. Objectives: The objective of the current work is to better understand practices, attitudes and expectations of French GPs with regards to mental health treatment and the usage of CBT, an empirically validated approach to mental health problems that is still largely underutilized in France. Method: A study was implemented with the participation of French GPs. Results: The results show an overall need for better training of GPs. These results underline theoretical concepts in CBT and how they could provide a better foundation in both initial and continuing education of GPs, as well as how they could be integrated into the GP’s daily practice. Discussion: Limits of the work are presented along with a detailed inventory of CBT concepts that could be provided through an internet-based continuing education program for primary care doctors

Texto completoCertain individuals show an astonishing behaviour while having recourse to imposture, sometimes throughout their life, without any tangible motivation being distinguished. With the help of a critical analysis of the psychopathological work on the subject, dialectized by a structural decomposition of the concepts of lie and identity implied in the phenomenon, are theorized the different functions which imposture can fulfil according to the structure of the subject. The study of the clinical field of the pathological lie thus results in questioning its possible defensive function with regard to the return of the repressed in the hysterical neurosis. The psychoanalytical contributions, for their part, mainly underline a non-neurotic clinical type of pathological impostors. Deepened in the light of the Lacanian developments on identification, these contributions make it possible to emphasize the compensation function which imposture regularly fulfils in the psychotic structure

Texto completoLiterature tells us that hearing impairment linked to old age, or presbycusis, generates a reduction in social skills, leading the deaf person into isolation, sometimes resulting in a state of depression and anxiety. Hearing-impaired people might also suffer from cognitive disorders. Several experiments have enabled us to check the presence of mnesic, emotional (depression, anxiety, social anhedonia) and psychosensory troubles (hyperacusis, sensory hyperesthesia) among presbycusic patients and to assess the impact of hearing aids on these disorders. Our study has also shown the presence of hyperacusis and sensory hyperesthesia among deaf people, thus suggesting cerebral reorganization after hearing-loss. Social anhedonia is more pronounced among hearing-impaired people, therefore contributing towards their becoming socially isolated. The appearance of mnemic trouble has also been reported. Hearing aids reduce psychosensory disability, pointing to post-equipment neuronal reorganization. Texto completoThis thesis, fit into the framework of clinical psychopathology, proposes an articulation between data of the neurosciences and the practice met in a CSAPA.The first part is devoted to the phenomenology of the addicting process with its successive phases that end in a request of care as a response to the final alienation produced by drug use. This request of care is, generally, at first, a request for a substitution treatment, which sometimes comes along with a psychological follow-up.In the second part, the hypothesis on the addiction’s genesis regarding the narcissistic vulnerability spotted in patients and their neurobiological bases is examined. Drug use would be a way to make arise the true self, thus overcoming the defensive system set up to protect oneself from traumas induced by the first environment. A first detailed clinical example represents an attempt to establish the existence of trauma, including of transgenerational origin, and the associated unrelenting course of addiction. Other clinical cases are examined, centered on a different aspect or on a specific moment of the support encounters, in order to illustrate other aspects of the addicting process. The neurobiological impact of traumas is also developed, that allows to articulate it with several concepts, particularly those of Winnicott.The third part proposes several functions to the addiction (defensive and anti-depressive roles, emotional regulation) with clinical examples and the neuroscientific bases currently known. Although experience in the psychoanalytical clinic is at a level of complexity much higher than what is currently accessible to the neurosciences, most of the research in this domain stay in line with psychological understanding of the addicting process.Finally, a hypothetical model of addiction in drugs and the most sensitive points concerning the therapeutic support are outlined

The Christian demonology theory, active in the western world since the 13e and 14e centuries, only proposes a religious therapy – exorcism. Ethnopsychiatry showed to be the most pertinent psychopathology research approach to deal with the diabolic etiology regularly revealed by Christian patients over the last years in Occident. This thesis discuss a modern psychopathology for the devil ; it is effective for psychotherapy. In the Christian Occident, in particular France, the devil is the etiology of mental, sexual and behavioral troubles, and of the disorders and misfortunes of an entire Christian group. The devil is also the most anti-Christian practice, always described by: “To make a pact with the devil”

Texto completoThis thesis aims at seizing the intrication of the cultural factors in the psychopathological disorders such as they arise in a consultation of ethnopsychology at Reunion island. Replaced in cultural context and psycho-history, the insistence of pathologies and therapeutics are articulated around the notion of secrecy and the importance of the dead. The issue of the relationship between the subject and the sacred serves as the common thread throughout the thesis. The concepts of identity, otherness, order and disorder form the disturbances which fall under universal categories of causality : mystic, animistic and magic. Most of them belong to an etiology of fright, a traumatic logic. They are commonly related with the dead's interference in the every day's life. Clinical observations and analysis of "traditional" healing methods show the narrow fastener of the occurrence of psychological disorders with the effration and the care. They take part of a universal concern : to reinforce social ties, the feeling of membership, identity, the psychical envelopes of the weakest within a framework of a creolizing world. In many cases, the taking in account of ethnopsychologic dimension is essential with comprehension and the psychological practice

Texto completoPsychoanalysis has focused since its foundation to the question of love from the clinic's love life as it is brought by the patients' speech. Freud was quickly confronted with the emergence of transference love in the logic of the analytical device. First resistance is apparent then, that love has taught on the very logic of true love. Lacan's teaching resume to formalize approaches based on the report to know. But this allows it to reduce the contribution of the broad field of Lacanian question of love? We revisit the approach of Lacan's love in its various figures throughout his teaching. From this abundance of proposals, is it possible to identify a Lacanian theory of love? But more importantly, what will we find contributions to allow us a clinical approach to the love life? It will, after such contributions held by a plan that will follow the structure of a knot of three registers Imaginary / Symbolic / Real, to test the thesis that love is psychopathological. Hence, the assumption is this: love is a metapsychological point of view, sheathed mainly in the Imaginary register, it will be his knotting two other records that will allow him not to pour into the shoot death of passion. Love can tie this according to three speak in a clinic beyond the narcissism and thereby create a fourth register which will be the symptom of love and / or sinthome and one who is experienced between two subjects in all these avatars sometimes verges on the clinical and psychopathology of psychoses

Texto completoA study into the process of freudian theorization, as applied to the psychopatholigical issues of bereavement in early childhood, entrances the patent enigma which lies with the relationship to someone else, through the enigmatic message stemming from the dead, as the node, the navel, of the mourning problematic. Talking up the knowledge of pain, we shall emphasize the forsaking of the notions of +pain experience ; and + outward frightful experience ;, as capping with a theory focused on otherness and exogenesis, and we shall insist upon the paramount importance of the process of avoidance (or diflection) of the memory of pain and fright: abwendung von der erinnerung. We suggest gathering the whole range of these experiences into the frame of the + human kin complex ; ( nebenmensch) as the primeval form of the distinctive relationship to others, in taking up the topics of pre-ambivalence, equivalence and the neuter issue. We shall further develop the negative psychic working weighing out the sequencing stages of denial, scotomisation, then negative hallucination. Thus early bereavement ensues a negative trauma, devoid of memory, a distorting of memory, a radical avoidance of otherness, related to a fate of presentation process, traces that signal the lack of traces, and that might show up through a range of negative symptoms telling off backward trends and + haunting ; avoided memories

Texto completoThis is a research on psychoanalytic epistemology, and develops the problematic status of psychoanalysis towards know, ledge. From an historical point of view, epistemologic thought in psychoanalysis was always dependant on positivist orienta, tions. These are based on the identity principle which enables knowledge's axiomatisation. Unconscious is the other of consciousness, and this onto- logical condition of being-other, requires other logical prin, ciples than those based on identity. These principles are sup, ported by a paradoxical structure where opposites bear a re, lation of complementarity and do not exclude one another. Contemporary french thought in psychoanalysis as well as in philosophy provides a reflection about otherness. A theoretical paradigm is thereby formed which can contribute to constitution of an apistemology adapted to psychoanalysis. Starting from a descriptive angle and a selection of authors and theories, a normative angle rises up in the re, search, concerning a possible psychoanalytic epistemology. Every theorisation in psychoanalysis carries a limited knowledge. This limit is connected with the unknown unconscious and with transformability of its notions. The negativerepre- sents these limits in our research. This integrative approach of otherness by means of a paradoxality logic and the negative, can contribute to the development of an epistemology suitable to psychoanalysis

Texto completoFrom clinical experience and from a theoretical reflection indexed in the Freudian approach we propose a psychoanalytical approach of the psychopathology of the obesity through three axes of reflection. The first axis concerns the impact of the encroachment by the first objects upon the future objectal investments of the obese subject. The noxious effects of the premature disturbances of the early feeding, by excess or by default, can be treated or by a series of negative mechanisms-what we suggest naming the white lineage-, or by an incorporation of the object : the body melancholy - that we associate with the black lineage-.The second axis of reflection is the question of transgression of the body limits. We propose an individually and a social and cultural study of this phenomenon. Finally, the third axis is interested in the constitution of obesity and in the aporias of the development of the psychosexuality

Using the psychoanalytical theory and its reference authors, especially Freud and Lacan, we will see how, because of the disappearance of Freud's morals end the deficiency of the paternal metaphor in our society, the difficulty is increasingly great for the men to maintain between them the user-friendliness relations resulting from impossibility to exceed the intrusion complex.
