Texto completoObjectives: To investigate the occurency of hearin problems in children with Robin Sequence (RS) aged 1 to 3 years old with cleft palate not submitted to palatoplasty. To compare the frequency of hearing problems in children with RS only (group 1), children with other anomalies associated to RS (group 2), at the some age, not submitted to palatoplasty. Results: The findings demonstrate that 100% of children in group 2 are submitted to hospital admission compared to 56% in group 1. Only 26% of parents or caretakers of group 1 and 48% in group 2 were able to describe some kind of hearing complaints. The most frequent complaints were otalgia (70% in group 1 and 14% in group 2) and only 10% demonstrated doubts related to hearing loss observed only in group 1. In both groups the type B curve prevailed (76% in group 1 and 83% in group 2) considering timpanometry measurements. The hearing loss was detected in 33% of children in group 1, where 78% of the hearing loss were mild. In group 2 the hearing loss occurred in 81% of children, where 47% were mild,41% moderate and 12% were severe. Conclusions: Children with RS showed risks to hearing problems due to the possibility of hospitalization in the first months of life, alterations in middle ear due to cleft palate and the presence of respiratory obstruction characterized by this anomaly. Parents and caretakers show difficulties to observe symptoms or characteristics that prove the possibility of hearing loss. Children with RS only showed mild hearing loss, the increasing gravity of hearing loss could be related to factors such as genetic syndromes and othe congenit anomalies. The visual refoircement audiometry and the timpanometry are efficient instruments to detect possible hearing problems in children with RS. The audiologic findings must be added to the treatment protocol in children with, supplementing the rehabilitation, with special attention in genetic syndromes, specially the Stickler syndrome. The delay on palate surgery is not cousing damages on hearing in children with RS only.

Concurrent hash tables are among the most important primitives in concurrent programming and have been extensively studied in the literature. Robin Hood hashing is a variant of linear probing that moves around keys to reduce probe distances. It has been used to develop state of the art serial hash tables. However, there is only one existing previous work on a concurrent Robin Hood table. The difficulty in making Robin Hood concurrent lies in the potential for large memory reorganization by the different table operations. This thesis presents Bolt, a concurrent resizable Robin Hood hash table engineered for high performance. Bolt treads an intricate balance between an atomic fast path and a locking slow path to facilitate concurrency. It uses a novel scheme to interleave the two without compromising correctness in the concurrent setting. It maintains the low expected probe count and good cache locality of Robin Hood hashing. We compared Bolt to a wide range of existing concurrent hash tables in a comprehensive benchmark. We demonstrate that on a 36-core machine with hyper-threading, Bolt is between 1.57x - 1.73x faster than the fastest publicly available non-resizable concurrent hash table and 1.45 - 2x faster than the fastest publicly available concurrent resizable hash table. It also achieves speedups between 15x - 35x over a highly optimized serial implementation at 36-cores with hyper-threading.

Texto completoThe Pierre Robin sequence (PRS) is considered a congenital disease, characterized by three main anomalies: micrognathia, glossoptosis and palate cleft, showing a prevalence of over 1/8500 birth with a variable etiology, widely discussed on literature. It can be associated to some other syndrome or malformation (syndromic PRS) or to the isolated type (no syndromic or isolated PRS). Because it is an alteration that affects the medium third section of the face, the oral cavity may be involved. Thus, the aim of this study was to investigate the dental phenotypes, determining specifically the prevalence of the dental abnormalities on a brazilian group with IPRS from HRAC/USP. Material and methods: A retrospective study was done with panoramic radiographs from 110 individuals of HRAC/USP, with superior age to 11 years old. These radiographs were digitalized and evaluated to the diagnostic of taurodontism, dental agenesis, root dilacerations, dental transposition, supernumerary teeth, macrodontia and microdontia. In order to get a precise diagnostic of taurodontism, linear measurements from molars were performed using Software Image J. The Fishers exact test was employed for the comparison between the genders and the types of clefts. Results: From the 110 individuals with IPRS on this study, 94.54% showed at least one type of dental abnormality, being taurodontism, the most prevalent, observed in 92.72% from the studied population, followed by dental agenesis and the root dilacerations, with 22.72% and 15.45%, respectively. In a lower proportion a prevalence of 0,91% for dental transportation was observed and no case found of supranumerary teeth, microdontia or macrodontia. Conclusion: From the dental phenotypes investigated, taurodontism and dental agenesis were the most prevalent dental abnormalities, being the superior arcade and molars the most injured by dental agenesis. It was not observed significant differences among the cleft types and the gender, except the root dilacerations, with a higher prevalence to the female gender. Therefore, IPRS brazilian individuals have a high frequence of dental abnormalities, with considerable clinical implications on the dealing and managing the rehabilitation treatment of these individuals.

Texto completoPurpose: The Robin Sequence (RS) It refers to the combination of micrognathia and glossoptosis that cause respiratory obstruction, and may be associated with this condition cleft palate. These clinical manifestations are heterogeneous, each newborn (NB) can present from mild respiratory difficulty and food to severe complications, thus making the first months of life decisive, it is in this age group that the clinical manifestations of the malformations are more pronounced and require Intervention so that there are no complications in development. The NB with RS need to go through periods of hospitalization for which the performance of the multidisciplinary team can cure their initial difficulties. So they are considered risk infants, with higher chances of having growth changes and neurodevelopment in the first years of life, creating potential problems in the future. Objective: To evaluate neurodevelopment of infants from zero to twelve months with Isolated Robin Sequence (IRS). Methods: The research was approved by the Human Research Ethics Committee of HRAC / USP under the opinion of No. 1,489,412 and CAEE 53941516.0.0000.5441. The subjects of the study were 20 babies with Isolated Robin Sequence (IRS), admitted to the Special Care Unit (SCU) of the HRAC / USP, in the age group from zero to one year of age. A chart review of each baby who was admitted to the SCU to verify eligibility for research was medical record, as soon as IRS was verified the parents were invited to participate in the survey. All the participants signed the Informed Consent Term (TICT). After consent, data were collected in the infant\'s charts through a protocol developed by the researcher and then an evaluation of the development was made using the \"Denver II Test\", which evaluates four areas of development, these being the folks social area , adaptive fine motor, language and coarse motor. The evaluations took place in the SCU, with a duration of 30 minutes each. In the end, all parents received the results of the evaluation and when developmental delays were found, the infants were referred to the HRAC-USP Occupational Therapy sector. Results: After evaluation, 85% of the babies presented a risk for development, the language area was the most affected with 70.6% of identified delays, followed by the coarse motor area, adaptive fine motor and folks social area, successively. Conclusion: With the accomplishment of this study we can verify that these babies, due to their clinical complications accentuated in the age group of 0 to 1 year, present delays in the development in this period. Thus, there is a need to follow up and intervene early to prevent possible future risks for the development of these children. The Occupational Therapist is the professional qualified to perform the monitoring and intervention with these babies.

Texto completoPurpose: The objective of this study was to compare speech nasality in children with isolated Robin sequence, operated with the Furlow palatoplasty technique, to the speech of children operated with the von Langenbeck technique. Research design: Prospective study. Research site: Department of Speech-Pathology and Laboratory of Experimental Phonetics at the University of São Paulo Hospital for Rehabilitation of Craniofacial Anomalies (HRAC-USP). Methods: Speech nasality was analyzed for 69 children with isolated Robin sequence, comparing the results for the 33 children who received primary palatoplasty with the Furlow procedure to the results of children who received the von Langenbeck (VL). Auditory-perceptual assessment of speech nasality involved: 1) the use of a 4-point scale (absence, mild, moderate and severe hypernasality), 2) the use of a cul-de-sac test in words, and 3) experienced listeners ratings of recordings of the phrase o bebê babou, establishing intra and inter-judge reliability. The instrumental assessment of nasality was done with Nasometry during repetition of the phrase o bebê babou, using the CUT-off score of 27% to interpret presence/absence of hypernasality. The significance of the differences between F and VL groups was studied, also analyzing the association of nasality with gender, age at palatoplasty, age at speech assessment, cleft severity, speech therapy and nasal snort. Agreement between the 4 modalities of assessment was studied and the analysis were repeated for the group of 47 participants without nasal snort . Results: Children Who received F presented with better speech nasality for all modalities of assessment studied. Absence of hypernasality, evaluated with 4-point scale live by the author, was found 26 (78,8%) children who received F e 17 (47,2%) who received VL. The difference between both groups was significant (p=0,012). When only the participants without nasal snort were studied (N=47), absence of hypernasality, was found 22 (91,7%) children who received F e 13 (56,5%) who received VL, with a difference also significant (p=0,008). Agreement between all 4 modalities of assessment as measured by Kappa statistics, was found between acceptable (0,32) to almost perfect (0,87) for the group of 69 participants, and between acceptable (0,32) and perfect (1,00) for the group without nasal snort. Significant association was found between nasality and nasal snort. Conclusion: Patients with Robin sequence who received primary palatoplasty with the F procedure presented with better speech nasality during auditory-perceptual and instrumental evaluations when compared with patients who received the VL procedure.

Texto completoObjectives: To assess the neurological and psychomotor development of children with Isolated Robin Sequence (IRS), submitted to the treatment of airway obstruction according to the protocol of the Hospital de Reabilitação de Anomalias Craniofaciais, Universidade de São Paulo (HRAC/USP), with nasopharyngeal intubation (NPI) or postural treatment. Verify if the neurodevelopment of the NPI group differs from postural group, and evaluate the effectiveness of NPI to prevent sequels of hypoxia. Materials and Methods: Prospective study conducted at HRAC/USP, with children SRI, with 2 to 6 years old, divided into two groups according to the type of treatment performed: NPI (Group 1) and postural (Group 2). Time data of use NPI, nasogastric tube (NGT), myringotomy surgery, socioeconomic status, education level of parents, among others, were collected. Participants were assessed through the development of Screnning Denver II Test (Denver II) and Neurologic Evolutionary Examination Adapted (NEEA). Results: The total of 62 children was evaluated, being 38 in the Group 1 and 24 in the Group 2. The results of the Denver Developmental Screening Test II showed that 73.7% of Group 1 and 79.2% of Group 2 presented with normal development. The results of NEEA had presented normal for 89.5% of children in the Group 1 and 87.5% in the Group 2. There was no significantly difference between the two groups at Denver Test (p=0.854) and in the NEEA Test (p=0.789). The myringotomy was performed by 47.3% of children in the Group 1 and 58.3% of children in the Group 2. The results showed language as the most impaired area, which may reflect fluctuations of hearing and velopharyngeal dysfunction. There was moderate agreement (k=0.563) between the results of the two tests applied in the Group 1, and substantial agreement (k=0.704) in the Group 2. The average time of use of the NPI was 60 ± 28 days. In the socioeconomic analysis of Group 1, 42.1% fit the low ranking higher, and 28.9% in the average lower. In the analyses of Group 2, 20.8% were in the low ranking lower, and 58.3% were in the low ranking the top. There was no different statistic between groups (p=0.211). There was no significant association between socioeconomic status and results of development tests performed. The levels of education most found between the participants parents were: completed graduation (Group 1), and secondary school (Group 2) with no statistical difference between groups. Conclusions: Most of children with IRS treated with NPI showed normal neurological and psychomotor development, similar of minor cases of postural group. The children treated with NPI didn\'t show evident clinical signs of neurological sequels of hypoxia.
