Humans are social creatures, bound by relationships. This dissertation investigates the structure and function of such interpersonal ties in two different contexts: patient-doctor trust and the diffusion of public health interventions across social networks. Each chapter of the dissertation focuses on a particular type of relationship: in Chapter 1, the patient-doctor relationship; in Chapter 2, friendship; and in Chapter 3, ties among friends, spouses, and siblings. The hope is that a better understanding of the nature of these ties and the work they do will enable policymakers to develop reg

Resumen
The purposes of the study were to investigate homeless men's experiences of taking medications while living in a homeless shelter and the factors that affect medication adherence; and to describe medication adherence, as it pertains to homeless men residing in a shelter, from the perspective of the shelter staff. This is a manuscript-based thesis. The first manuscript provides a description of a proposed ecological model, based on clinical experience of the author and from the literature. The second paper is a summary of the findings of the qualitative research on medication adherence and home

Resumen
<p> Objective: Under-reporting of surgical errors inhibits development of knowledge and strategies that can lead to lower error rates. Mandatory error reporting programs have proliferated among states as one means of reducing the incidence of errors. Evidence suggests that errors are under-reported. Little is known of the perceptions of states regarding the risk of under-reporting, their responses to it and the ways they use reported data to improve patient safety. A qualitative study was conducted to assess the perceptions of state managers regarding the risk of under-reporting and the role o

<p> <b>Purpose:</b> Empirical evidence on impact of CABG report cards on the quality of CABG surgery is patchy, mixed and mainly comes from uncontrolled and short term studies. CABG report cards are associated with a small decline in mortality after controlling for an overall declining trend. Whether report card publication has caused this decline is not known. </p><p> <b>Methods:</b> I use difference-in-differences and difference-in-differences-in-differences with fixed and random effects in OLS, Logistic, FGLS and GMM regression frameworks in a quasi-experiment set up in State-level and pa

<p> The United States (US) is plagued by a high-cost health care system producing lower than desired patient quality outcomes. In 2012, the Patient Protection and Affordable Care Act was enacted to financially incentivize cost-effective models of care that improve the health of US citizens. One emerging solution is engaging patients with chronic conditions in self-management practices. </p><p> Guided by Krieger's Eco-Social Theory, this study used semi-structured interviews, scales and a questionnaire to detect factors that facilitate patient activation of self-management in patients with ty

<p> A number of states have adopted certification programs for community health workers (CHWs) to increase oversight, allow for direct funding, and improve recognition of CHWs as members of the health care team. More states are considering CHW certification programs to increase the use of CHWs by health care organizations with the hopes of improving health outcomes and decreasing health disparities. There has been little research into the impact of state CHW certification on the adoption and dissemination of CHWs into the existing health care system. This study examined the impact of state CHW

<p> Participation of colorectal cancer survivors (CRC) in the workforce has been described by clinicians, survivors, and researchers as a way to improve mood, quality of life (QOL), and survival. Maintaining self-esteem and financial independence have also been attributed to continued employment of the CRC survivor. The purpose of this cross-sectional survey was to describe patterns of employment of the CRC survivor and to examine the individual, disease, and work-related factors that influence presenteeism and perceived adequacy of sick pay (ASP) policy. Although the Special Supplemental Food Program for Women, Infants and Children (WIC) and the Commodity Supplemental Food Program (CSFP) have operated for years, little is known about utilization of program foods by participants. Using ethnographic interviews, this study examined food use, factors affecting use, and satisfaction among WIC and CSFP clients. WIC foods were consumed by most participants, but frequently not in the total amount. Four of seven WIC foods were found to be substitutions for foods previously consumed. CSFP foods were consumed less frequently, and in smaller amounts, than

A descriptive design was used to study the subjective sleep characteristics of permanent night shift workers, examine the validity and reliability of the Visual Analog Sleep (VAS) Scales, and to compare the scores to a previously tested non night shift working sample. The VAS Scales, which employ a visual analog format to measure subjective perceptions of sleeps were reworded slightly to apply to this population, giving rise to the VAS/NS (Night Shift) Scales. A sample of 40 night shift workers completed the VAS/NS Scales on two separate days. Descriptive and inferential statistics were used t

<p> Mexico's innovative health reforms have attracted scholarly attention beyond its own borders, making it a valuable case to study how countries set priorities. This dissertation examines the multifaceted topic of priority setting through a multidisciplinary approach: each of the three papers of this dissertation employs one of three disciplinary perspectives: historical, quantitative or normative. The dual focus on mental health and HIV--two highly stigmatized diseases with almost opposite histories of prioritization--further underscores the social and historical aspects of health priority

Resumen
This thesis explores the use of the Mental Health Act (MHA)(1983) with older people(65+) by providing a multi-perspective insight as expressed by those involved in the process. In particular, it focuses on the personal and social circumstances in which decisions to compulsorily detain older people are made. The thesis comprises two elements; one documentary, one qualitative. The documentary study was over a four year period (2000-2003) gathering demographic data around various themes including numbers of older people detained, gender, age, diagnosis and the relevant section used to detain the

Resumen
<p>The 2001 destruction of the World Trade Center and the subsequent anthrax attacks highlighted the inability of an antiquated public health system in the United States to respond effectively to emergencies. Little documentation exists to define how public health agencies can improve performance. The overarching research question was the extent to which drills and exercises improve performance in public health emergencies. Adult learning theory and deliberate practice theory were explored in this context. The research data were from 50 state public health departments, which were required to r
