Resumen
Abstract Introduction Endothelial function is impaired in adults with sickle cell anaemia (SCA), but limited data exists in children. Endothelial damage occurs from chronic inflammation, oxidant damage, immune cell activation and ischemia-reperfusion injury. In addition, availability of nitric oxide (NO) as the major vasodilator may be reduced as a result of scavenging by plasma haemoglobin and reduced arginine substrate for endothelial nitric oxide synthase. Methods Tanzanian children (N=119) with SCA (HbSS) aged 8-11.9 years enrolled in the Vascular Function Intervention Trial (ISRCTN74331412/NCT01718054) underwent baseline assessment of endothelium-dependent and -independent vasodilatation. All children were determined clinically well at assessment, hydroxyurea naive, on no long-term medication and not receiving chronic blood transfusions. Blood pressure and vasomotion were assessed after 10 minutes recumbent rest in a temperature controlled room between 08-13:00 hrs. An identical protocol as published in children (Donald & Charakida et al. Eur Heart J; 2010: 31; 1502-10) was used. In brief, brachial arterial endothelium dependent dilatation was assessed by 1 of 3 trained technicians using ultrasound imaging (Ultrasonix SonixTouch with a 12Mz probe & stereotactic holder) to assess flow-mediated dilatation (FMD) in response to reactive hyperaemia induced after release of transient blood pressure cuff occlusion (5 min, 200 mmHg, Hokanson, USA) using an automated air regulator (Logan Research, UK). Automated B-mode image edge detection was used to measure maximum change in arterial diameter (Brachial Tools) expressed as a percentage of resting baseline diameter (FMDmax). Endothelium-independent responses to 2.5µg sub-lingual glyceryl-trinitrate (GTN) were also assessed. All recordings were over-read by an experienced researcher in the UK. Venepuncture for full blood count, clinical chemistry and amino acids was conducted after FMD assessment. Results Patient characteristics are described in Table 1. Mean brachial artery diameter at baseline was 2.61mm (95% CI 2.55 – 2.67mm). Mean FMDmax was 7.70% (95% CI 7.09 – 8.32%). Endothelium-independent vasodilation (GTNmax) was 4.15% (95% CI 3.83 – 4.47%). The FMDmax response was on greater than the GTNmax response (Figure 1). No effect of room or skin temperature on FMDmax or GTNmax was observed. There was a strong inverse association between baseline artery diameter and FMDmax (-3.46, P<0.001) (Figure 2). The time to peak brachial artery diameter in response to hyperaemia was positively skewed (median 55s (IQR: 43-79s)) and was not associated with FMDmax. The only patient characteristic associated with FMDmax was age with a non-significant inverse correlation (-0.52, P=0.06) but was reduced when adjusting for baseline diameter. Baseline heart rate was positively associated with FMDmax and GTNmax (P=0.01 & 0.025). Discussion We have characterised peripheral vascular function in a large cohort of children with SCA. Mean FMDmax was slightly lower than that observed in predominantly Caucasian non-SCA British children of similar age (8.1% [SD3.4]) (Donald & Charakida et al. Eur Heart J 2010: 31; 1502-10), but higher than reported in 21 older French children with SCA (5.6 +/- 0.2) (Montalembert et al. Haematol 2007: 92; 1709-10) which might reflect deterioration of endothelial function with age. There was no apparent association between FMDmax and hemolytic markers or with nutritional status at baseline. Recruitment and assessment of appropriate local non-SCA controls for comparison is planned. Amino acid analyses are ongoing. Disclosures: No relevant conflicts of interest to declare.

Resumen
We consider a homogeneous isotropic Universe filled with cold matter (with zero pressure) and dynamic dark energy in a form of a scalar field. For known scalar field potential V(φ), the Friedmann equations are reduced to a system of the first order equation for the Hubble parameter H(z) and the second order equation for the scalar field as functions of the redshift z. On the other hand, knowledge of H(z) allows us to get the scalar field potential in a parametric form for a known cold matter content and three dimensional curvature parameter. We analyze when the accepted model mimics the dependence H(z) derived in the framework of the other models, e.g., hydrodynamic ones. Two examples of this mimicry are considered. The first one deals with the case when H2(z)~ Ωm(1+z)3+ΩΛ, but Ωm parameter overestimates the input of the cold matter (dark matter+baryons). The resulting scalar field potential is V(φ)=a+bsinh2(cφ), where the constants a,b,c depend on the Ω – parameters of the problem. In the other example we assume that some part of the dark matter has a non-zero equation of state p=wε, -1<w<1. In this case H2(z)~ Ωdm1(1+z)3(1+w)+ Ωb+Ωdm2)(1+z)3+ΩΛ. The corresponding potentials are defined for positive values of φ. For both signs of w potential V(φ) is a monotonically increasing function with typically an asymptotically exponential behavior; though for some choice of parameters we may have a singularity of V(φ)on a finite interval. Then we consider fitting of the potential for w from the interval [-0.2,0.2] for three different values of Ωdm2 by means of a simple formula Vfit(φ)=p0+p1exp(p2 φ). The dependencies pi(w) are presented and the approximation error is estimated.

Resumen
In this study, the main aim is to improve performance of the voting feature intervals (VFIs), which is one of the most effective machine learning models, using two robust ensemble techniques, namely, AdaBoost and MultiBoost for landslide susceptibility assessment and prediction. For this, two hybrid models, namely, AdaBoost-based Voting Feature Intervals (ABVFIs) and MultiBoost-based Voting Feature Intervals (MBVFIs) were developed and validated using landslide data collected from one of the landslide affected districts of Vietnam, namely, Muong Lay. Quantitative validation methods including area under the ROC curve (AUC) were used to evaluate model performance. The results indicated that both the newly developed ensemble models ABVFI (AUC = 0.859) and MBVFI (AUC = 0.839) outperformed the single VFI (AUC = 0.824) model. Thus, ensemble framework-based VFI algorithms can be used for the accurate spatial prediction of landslides, which can also be applied in other landslide prone regions of the world. Landslide susceptibility maps developed by ensemble VFI models can be used for better landslide prevention and risk management of the area.

Resumen
The impact of longitudinal anthropometric changes during adjuvant chemotherapy on long-term survival in non-metastatic colon cancer is unclear. Herein, we analyzed the prognostic significance of computed tomography (CT)-measured body composition changes in colon cancer patients who underwent surgery followed by adjuvant FOLFOX (folinic acid, 5-fluorouracil, oxaliplatin) chemotherapy. Data of 167 patients with stage III or high-risk stage II colon cancer were analyzed. Skeletal muscle index (SMI), skeletal muscle radiodensity (SMR), visceral fat index (VFI), subcutaneous fat index (SFI), and total fat index (TFI) changes during chemotherapy were calculated using preoperative and postchemotherapy CT image data. The Cox proportional hazard model was used to determine the correlation between changes in anthropometric values and overall survival (OS). The median changes (%) in SMI, SMR, VFI, SFI, and TFI over 210 days during chemotherapy were 8.7% (p < 0.001), 3.4% (p = 0.001), −19% (p < 0.001), −3.4% (p = 0.936), and −11.9% (p < 0.001), respectively. Cut-off values of changes in SMI (skeletal muscle index change, SMIC) and SMR (skeletal muscle radiodensity change, SMRC) were defined at −2% and −2 Hounsfield units (HU) respectively, whereas those of changes in VFI (visceral fat index change, VFIC), SFI (subcutaneous fat index change, SFIC), and TFI (total fat index change, TFIC) were based on values that provided the largest χ2 on the Mantel–Cox test. Multivariable analysis revealed that low SMR measured on a postchemotherapy CT scan (hazard ratio, HR: 0.32, 95% confidence interval, CI: 0.15–0.70, p = 0.004) and visceral fat loss of at least 46.57% (HR: 0.31, 95% CI: 0.14–0.69, p = 0.004) were independent poor prognostic factors for OS. Severe visceral fat loss during FOLFOX chemotherapy and low skeletal muscle radiodensity measured on postchemotherapy CT scans are associated with poor OS in stage III and high-risk stage II colon cancer patients.

ABSTRACT Three experiments were conducted in the period between July and November with non-lactating red deer hinds to describe the effects of treatment with melatonin during this period on voluntary food intake (VFI), the onset of the breeding season, coat changes and plasma concentrations of prolactin and tri-iodothyronine (T3), and to examine whether prolactin mediated the observed effects. In experiment 1, eight animals were treated orally each day with either 10 mg melatonin at 16.00 h or 10 mg melatonin at 16.00 h plus 10 mg domperidone (a dopamine antagonist) given twice daily for 120 days from July; eight animals were maintained as controls. In experiment 2, the same numbers of animals per treatment were used to compare treatments in which 10 mg melatonin or 20 mg bromocriptine (a dopamine agonist) were given orally each day at 16.00 h for 119 days from late June and compared with an untreated control group. In experiment 3, six animals were treated daily for 105 days from mid August with 5 mg domperidone given i.m. and compared with six control animals. In experiments 1 and 2, the VFI of control animals reached a peak in late August and thereafter declined. Melatonin-treated animals showed a similar pattern but the peak in VFI was significantly (P<0·05) advanced by 2 weeks compared with controls, although the VFIs of both groups were similar in November. The mean date of onset of the breeding season of the melatonin-treated animals was advanced significantly (P < 0·05) by 23 days in both experiments and the coats of these animals had less undercoat and were pale coloured and patchy compared with the controls. The changes in VFI, coat and the onset of the breeding season were associated with the rapid decline in plasma prolactin concentration after the start of the melatonin treatment and significantly (P<0·01) lower plasma T3 concentrations than those of control animals. In experiments 1 and 3, plasma prolactin concentrations in animals treated with domperidone were higher than those of controls for periods of 2–3 weeks. These short-term increases in plasma prolactin concentration were not associated with changes in VFI, coat or onset of the breeding season compared with controls. In experiment 2, the pattern of decline in plasma prolactin concentrations was the same in bromocriptine-treated animals as in the melatonin-treated animals; plasma T3 concentrations were also similar in the two groups. The pattern of change in VFI over time in bromocriptine-treated animals was significantly (P<0·05) different from that of melatonin-treated animals and there was also a reduced amount and length of winter coat in the bromocriptine-treated animals. The mean date of onset of the breeding season in bromocriptine-treated animals was not significantly different from that of controls. It was concluded that a reduction in plasma prolactin concentration induced by bromocriptine produced different effects from that induced by melatonin treatment and that the effects of melatonin are unlikely to be induced through changes in contemporary plasma prolactin concentrations. Resumen
Vertical field-effect transistor (VFET) structure has the advantage in improving both the breakdown voltage (BV) and on-resistance (RON) of semiconductor power devices when compared with the lateral devices. GaN-based normally-off VFET device is designed and demonstrated in this work and an additional back current block layer (B-CBL) is proposed and employed to further improve the device performances. By introducing the B-CBL design, the electric field distribution along the gate aperture is more uniform which leads to an obvious increase (~30%) of BV in the proposed device while the RON is kept nearly constant. Therefore, the figure of merit (FOM) value of the proposed VFET is improved apparently in comparison with that of the conventional GaN-based VFET devices.

Resumen
Regeneration from VFNT Cherry tomato was optimized prior to transformation. Cotyledon and hypocotyl sections from 7-day-old in vitro grown VFNT Cherry tomato were cultured on medium containing MS salts, B5 vitamins and the following per liter: sucrose, 30g; zeatin, 1 mg; IAA, 0.1 mg; agar, 8.0g or GelriteR, 2.2g. Culture conditions investigated included cotyledon and upper hypocotyl explant lengths, light vs. dark germination, and agar vs. GelriteR. The conditions which resulted in the highest average number of shoots per explant were cotyledon basal explants 2 mm in length, 3.95; cotyledons from dark germination, 6.2; and hypocotyls from light germination, 8.6. An equal number of shoots regenerated on medium containing agar or GelriteR, however, shoots regenerated on medium containing agar were more vigorous. Cotyledon and hypocotyl sections were cocultivated with the Agrobacterium tumefaciens binary vector pBI121 containing the neomycin phosphotransferase II (NPTII) and B-glucuronidasc (GUS) genes. Transformants were selected by regeneration and rooting on medium containing kanamycin. Southern blot and PCR analysis indicated regeneranrs contain the NPTII and GUS genes. Mapping the chromosomal location of the NPTII gene is in progress.

Resumen
A gallium nitride (GaN) semiconductor vertical field-effect transistor (VFET) has several attractive advantages such as high power density capability and small device size. Currently, some of the main issues hindering its development include the realization of normally off operation and the improvement of high breakdown voltage (BV) characteristics. In this work, a trenched-gate scheme is employed to realize the normally off VFET. Meanwhile, an additional back current blocking layer (BCBL) is proposed and inserted into the GaN normally off VFET to improve the device performance. The electrical characteristics of the proposed device (called BCBL-VFET) are investigated systematically and the structural parameters are optimized through theoretical calculations and TCAD simulations. We demonstrate that the BCBL-VFET exhibits a normally off operation with a large positive threshold voltage of 3.5 V and an obviously increased BV of 1800 V owing to the uniform electric field distribution achieved around the gate region. However, the device only shows a small degradation of on-resistance (RON). The proposed scheme provides a useful reference for engineers in device fabrication work and will be promising for the applications of power electronics.

Resumen
AbstractRecently machine learning researchers are designing algorithms that can run in embedded and mobile devices, which introduces additional constraints compared to traditional algorithm design approaches. One of these constraints is energy consumption, which directly translates to battery capacity for these devices. Streaming algorithms, such as the Very Fast Decision Tree (VFDT), are designed to run in such devices due to their high velocity and low memory requirements. However, they have not been designed with an energy efficiency focus. This paper addresses this challenge by presenting the nmin adaptation method, which reduces the energy consumption of the VFDT algorithm with only minor effects on accuracy. nmin adaptation allows the algorithm to grow faster in those branches where there is more confidence to create a split, and delays the split on the less confident branches. This removes unnecessary computations related to checking for splits but maintains similar levels of accuracy. We have conducted extensive experiments on 29 public datasets, showing that the VFDT with nmin adaptation consumes up to 31% less energy than the original VFDT, and up to 96% less energy than the CVFDT (VFDT adapted for concept drift scenarios), trading off up to 1.7 percent of accuracy.

Resumen
Wind tunnel based Virtual Flight Testing (VFT) is a dynamic wind tunnel test for evaluating flight control systems (FCS) proposed in recent decades. It integrates aerodynamics, flight dynamics, and FCS as a whole and is a more realistic and reliable method for FCS evaluation than traditional ground evaluation methods, such as Hardware-in-the-Loop Simulation (HILS). With FCS evaluated by VFT before flight test, the risk of flight test will be further reduced. In this paper, the background, progress, and prospects of VFT are systematically summarized. Specifically, the differences among VFT, traditional dynamic wind tunnel methods, and traditional FCS evaluation methods are introduced in order to address the advantages of evaluating FCS with VFT. Secondly, the progress of VFT is reviewed in detail. Then, the test system and key technologies of VFT for FCS evaluation are analyzed. Lastly, the prospects of VFT for evaluating FCS are described.

Resumen
This paper describes an impairment commonly encountered in an OFDM system that must be considered in the design that is peak average power ratio (PAPR). In these studies, a statistical approach to analysing PAPR is suggested. The PAPR statistics of OFDM and VFFT-OFDM are studied by simulation of the statistical distribution of the quantity that is the Complementary Cumulative Distribution Function (CCDF) of the PAPR. The simulation results show that the simulated CCDF of PAPR, the 1% PAPR of OFDM is about 11.40 dB, whereas for VFFT-OFDM this rises to less than 3% of the time. Simulations show that by employing VFFT in OFDM system results in a 0.55 dB deterioration in the PAPR 1% of time.

Resumen
Volume flow estimation in the common carotid artery (CCA) can assess the absolute hemodynamic effect of a carotid stenosis. The aim of this study was to compare a commercial vector flow imaging (VFI) setup against the reference method magnetic resonance phase contrast angiography (MRA) for volume flow estimation in the CCA. Ten healthy volunteers were scanned with VFI and MRA over the CCA. VFI had an improved precision of 19.2% compared to MRA of 31.9% (p = 0.061). VFI estimated significantly lower volume flow than MRA (mean difference: 63.2 mL/min, p = 0.017), whilst the correlation between VFI and MRA was strong (R2 = 0.81, p < 0.0001). A Bland–Altman plot indicated a systematic bias. After bias correction, the percentage error was reduced from 41.0% to 25.2%. This study indicated that a VFI setup for volume flow estimation is precise and strongly correlated to MRA volume flow estimation, and after correcting for the systematic bias, VFI and MRA become interchangeable.

Resumen
AbstractPerformance in pigs is greatly reduced during periods of heat stress through a reduction in voluntary food intake (VFI). However, little information is available as to what extent growth in piglets is affected by high temperature. The objective of this study was therefore to quantify the change in VFI as affected by environmental temperature. Piglets, initially 15·5 (s.e. 1·9) kg body weight (BW), were individually housed and exposed over a period of 17 days to either 19, 21, 23, 25, 27, 29, 31, 33, or 35ºC. Animals had ad libitum access to a starter diet and water. VFI was measured daily whereas BW was determined twice weekly. Over the 17 days, daily VFI and BW gain were proportionately 0·48 and 0·51 lower at 35ºC than at 19ºC. Due to the reduced VFI at high temperatures, the average BW during the experiment was greater at low temperatures than at high temperatures. Consequently, part of the difference in VFI is directly due to temperature and part may be explained by cascading, indirect effects (i.e. the increased BW). To account for this, VFI was expressed as a power function of BW (VFI = aBWb). It was assumed that environmental temperature affected the scalar (a) through a quadratic or a ‘plateau-linear decline’ function of temperature. The VFI appeared relatively constant between 19 and 25ºC (0·096 (kg/day)/(kg BW0·83)) and decreased thereafter. Between 25 and 35ºC, VFI decreased on average by proportionately 0·28 in a 20-kg pig.

Resumen
Abstract The enforced sedentary lifestyle and muscle paresis below the level of injury are associated with adipose tissue accumulation in the trunk. The value of anthropometric indicators of obesity in patients with spinal cord injuries has also been called into question. We hypothesized that the Body Mass Index recommended by the WHO to diagnose obesity in general population has too low sensitivity in case of wheelchair rugby players. The study group comprised 14 wheelchair rugby players, aged 32.6 ± 5.1 years, who had sustained CSCI (paralysis of lower limbs and upper extremities). The research tool was the Tanita Viscan visceral and trunk fat analyzer AB140 using the abdominal bioelectrical impedance analysis (BIA) to estimate the visceral fat level (Vfat) and trunk fat percentage (Tfat). The AB140 analyzer also allowed the measurement of body composition of those individuals who could not assume an upright position. Our analyses revealed high and very high correlation coefficients between Vfat and WC (r=0.9), WHtR (r=0.7) and Tfat (r=0.9) whereas the correlation between Vfat and the BMI was weak, especially in the subgroup with Vfat < 13.5% ( r=0.2). The subgroup with Vfat>13.5 exhibited a moderate-level relationship between the BMI and visceral fat increase. It was concluded that the BMI had a low sensitivity for predicting obesity risk in wheelchair rugby players after CSCI. The sensitivity of WC measurement was higher and thus, it may be stated that it constitutes an objective tool for predicting obesity risk in post-CSCI wheelchair rugby players.

Resumen
Patient-controlled epidural analgesia is widely used to control postoperative pain following major intra-abdominal surgeries. However, determining the optimal infusion dose that can produce effective analgesia while reducing side effects remains a task to be solved. Postoperative pain and adverse effects between variable-rate feedback infusion (VFIM group, n = 36) and conventional fixed-rate basal infusion (CFIM group, n = 36) of fentanyl/ropivacaine-based patient-controlled epidural analgesia were evaluated. In the CFIM group, the basal infusion rate was fixed (5 mL/h), whereas, in the VFIM group, the basal infusion rate was increased by 0.5 mL/h each time a bolus dose was administered and decreased by 0.3 mL/h when a bolus dose was not administered for 2 h. Patients in the VFIM group experienced significantly less pain at one to six hours after surgery than those in the CFIM group. Further, the number of patients who suffered from postoperative nausea was significantly lower in the VFIM group than in the CFIM group until six hours after surgery. The variable-rate feedback infusion mode of patient-controlled epidural analgesia may provide better analgesia accompanied with significantly less nausea in the early postoperative period than the conventional fixed-rate basal infusion mode following open gastrectomy.

Resumen
BackgroundEfficient transportation of blood through the left ventricle (LV) during diastole depends on vortex formation. Vortex formation time (VFT) can be measured by echocardiography as a dimensionless index. As elite athletes have supranormal diastolic LV function, we aim to assess resting and post-exercise VFT in these athletes and hypothesised that VFT may predict myocardial performance immediately post-exercise.MethodSubjects were world class speedskaters training for the Winter Olympic Games. Echocardiographic measurements were obtained before and immediately after 3000 m of racing. VFT was computed as 4×(1−β)/π×α³×left ventricle ejection fraction where β is the fraction of diastolic stroke volume contributed by atrial contraction, α is the biplane end diastolic volume (EDV)1/3 divided by mitral annular diameter during early diastole.ResultsBaseline VFT was 2.6±0.7 (n=24, age 22±3 years, 67% males). Post-exercise, heart rates increased (64±10 vs 89±12 beats/min, p<0.01); however, VFT was unchanged (2.9±1.0, p>0.05). VFT at rest correlated modestly with post-exertion early diastolic mitral in-flow velocity (E; r=0.59, p=0.01), tissue Doppler-derived early mitral annular velocity (E′; septal and lateral, both r=0.59, p=0.01) and systolic annular velocity (S′; septal: r=0.46, p=0.02 and lateral: r=0.48, p=0.02) but not late diastolic mitral in-flow velocity (A; r=0.06, p>0.05) or annular velocity (A′; septal: r=0.34, p=NS and lateral: r=0.35, p>0.05).ConclusionThere was no significant difference between VFT at rest and immediately post-exercise. However, VFT at rest correlated with immediate post-exercise augmented systolic and early diastolic tissue Doppler indicators of myocardial performance in elite athletes.

Resumen
Abstract Purpose Spectral Doppler ultrasound (SDUS) is used for quantifying reflux in lower extremity varicose veins. The technique is angle-dependent opposed to the new angle-independent Vector Flow Imaging (VFI) method. The aim of this study was to compare peak reflux velocities obtained with VFI and SDUS in patients with chronic venous disease, i. e., pathological retrograde blood flow caused by incompetent venous valves. Materials and Methods 64 patients with chronic venous disease were scanned with VFI and SDUS in the great or the small saphenous vein, and reflux velocities were compared to three assessment tools for chronic venous disease. A flow rig was used to assess the accuracy and precision of the two methods. Results The mean peak reflux velocities differed significantly (VFI: 47.4 cm/s vs. SDUS: 62.0 cm/s, p<0.001). No difference in absolute precision (p=0.18) nor relative precision (p=0.79) was found. No correlation to disease severity, according to assessment tools, was found for peak reflux velocities obtained with either method. In vitro, VFI was more accurate but equally precise when compared to SDUS. Conclusion Both VFI and SDUS detected the pathologic retrograde flow in varicose veins but measured different reflux velocities with equal precision. VFI may play a role in evaluating venous disease in the future.

A new analytic approach is presented for determining the total volatile fatty acids (VFAT) concentration in anaerobic digesters. The approach relies on external determination of the inorganic carbon concentration (CT) in the analyzed solution, along with two strong-acid titration points. The CT concentration can be determined by either a direct analysis (e.g., by using a TOC device) or by estimating it from the recorded partial pressure of CO2(g) in the biogas (often a routine analysis in anaerobic digesters). The titration is carried out to pH 5.25 and then to pH 4.25. The two titration results are plugged into an alkalinity-mass-based equation and then the two terms are subtracted from each other to yield an equation in which VFAT is the sole unknown (since CT is known and the effect of the total orthophosphate and ammonia concentrations is shown to be small at this pH range). The development of the algorithm and its verification on four anaerobic reactor liquors is presented, on both the raw water and on acetic acid-spiked samples. The results show the method to be both accurate (up to 2.5% of the expected value for VFAT/Alkalinity >0.2) and repetitive when the total orthophosphate and ammonia concentrations are known, and fairly accurate (±5% for VFAT >5 mM) when these are completely neglected. PHREEQC-assisted computation of CT from the knowledge of the partial pressure of CO2(g) in the biogas (and pH, EC and temperature in the liquor) resulted in a very good estimation of the CT value (±3%), indicating that this technique is adequate for the purpose of determining VFAT for alarming operators in case of process deterioration and imminent failure.

Resumen
Background Direct quantitative measurement of GFR (mGFR) remains a specialized task primarily performed in research settings. Multiple formulas for estimating GFR have been developed that use the readily available endogenous biomarkers creatinine and/or cystatin C. However, eGFR formulas have limitations, and an accurate mGFR is necessary in some clinical situations and for certain patient populations. We conducted a prospective, open-label study to evaluate a novel rapid technique for determining plasma volume and mGFR.Methods We developed a new exogenous biomarker, visible fluorescent injectate (VFI), consisting of a large 150-kD rhodamine derivative and small 5-kD fluorescein carboxymethylated dextrans. After a single intravenous injection of VFI, plasma volume and mGFR can be determined on the basis of the plasma pharmacokinetics of the rhodamine derivative and fluorescein carboxymethylated dextrans, respectively. In this study involving 32 adults with normal kidney function (n=16), CKD stage 3 (n=8), or CKD stage 4 (n=8), we compared VFI-based mGFR values with values obtained by measuring iohexol plasma disappearance. VFI-based mGFR required three 0.5-ml blood draws over 3 hours; iohexol-based mGFR required five samples taken over 6 hours. Eight healthy participants received repeat VFI injections at 24 hours. Results VFI-based mGFR values showed close linear correlation with the iohexol-based mGFR values in all participants. Injections were well tolerated, including when given on consecutive days. No serious adverse events were reported. VFI-based mGFR was highly reproducible. Conclusions The VFI-based approach allows for the rapid determination of mGFR at the bedside while maintaining patient safety and measurement accuracy and reproducibility.

Resumen
Abstract Background Increased visceral adipose tissue (VAT) is associated with systemic inflammation through amplified production of pro-inflammatory cytokines like TNF-α and interleukin-6. Higher visceral fat index (VFI), which is the ratio of VAT to subcutaneous adipose tissue (SAT) and reflects the degree of excess VAT in relation to SAT, is associated with complex IBD. However, its relevance to treatment response is unclear. We aim to evaluate the effect of VFI on treatment response and risk of surgery in IBD patients starting anti-TNF therapy. Methods IBD patients starting anti-TNF agents between 1/1/2009 to 7/31/2019 at two academic medical centers were included. Three dimensional VAT and SAT volumes were measured from CT scans with Aquarius (iNtuition, Foster City, CA). VFI was calculated with the ratio of VAT:SAT. Patients were categorized by predefined VFI cutoffs: <0.33, 0.33–0.66, >0.67. Primary outcomes included composite endpoint of corticosteroid-free response (CFR) at 6 and 12 months defined by meeting one of the following: a) clinical response based on Harvey Bradshaw Index for Crohn’s disease (CD) or Lichtiger score for ulcerative colitis (UC), b) endoscopic improvement based on ulcer healing for CD and Mayo score for UC compared to baseline, or c) 50% improvement or normalization of CRP or fecal calprotectin. Secondary outcomes included IBD-related surgery at 6 and 12 months. We performed a multivariable logistic regression on CFR and surgery, adjusting for age, gender, IBD diagnosis, disease duration, active tobacco use, and immunomodulator exposure, to calculate adjusted odds ratio (aOR) and 95% confidence interval (CI). Results We included 181 patients. Table 1 summarizes differences in baseline characteristics. There were no differences in disease severity or activity. There were no differences in CFR at 6 and 12 months (Figure 1a and b). With respect to surgery within 6 months, patients with VFI >0.67 were significantly more likely to undergo surgery (aOR: 41.92 [95%CI 5.09–345.59]), but no difference in patients with VFI 0.33–0.66 was observed when compared to patients with VFI <0.33 (aOR: 2.23 [95%CI 0.48–11.21]). Similarly at 12 months, patients with VFI >0.67 were more likely to undergo surgery (aOR: 15.55 [95%CI 2.85–80.56]) but no difference was observed in patients with VFI 0.33–0.66 compared to patients with VFI <0.33 (aOR: 1.38 [95%CI 0.36–5.29]). Conclusion In this retrospective, multicenter, cohort study, we found higher VFI is associated with increased odds of surgery in IBD patients starting anti-TNF therapy. If confirmed by future prospective studies, VFI may be employed as a biomarker to help identify IBD patients at risk for medically refractory disease and would benefit from an early surgical referral, especially considering the growing application of artificial intelligence in medical imaging.

Resumen
Aim: the purpose of this study was to translate and adapt the English version of the Vocal Fatigue Index (VFI) into the Spanish language. Methods: the English version of the Vocal Fatigue Index (VFI) was translated into Spanish by two bilingual speech-language pathologists, and then was back-translated into English. The Spanish VFI (o “Índice de Fatiga Vocal”, IFV) was administrated to a pilot group of 10 individuals, which revealed some small typographical and grammatical adjustments to the index. The final updated version was then administrated to 34 subjects (21 with voice disorders, and 13 without voice disorders). Internal consistency and scale reliability were analyzed using Cronbach alpha coefficient. Results: a high Cronbach alpha coefficient for the three factors (0.87) was obtained. The results of the item role in reliability of the Spanish VFI demonstrated that all of them showed a positive role according to this criterion. The results of the ANOVA indicate a statistically significant difference between groups on the three scores of the Spanish translation of the VFI. In comparison to the healthy participants, those with voice disorders obtained statistically significant higher scores for the Spanish VFI subscales. Conclusion: the present study suggests that the Spanish translation of the Vocal Fatigue Index has a good internal consistency and high reliability on each of the three factors. The results suggest that the Spanish VFI can be used reliably to identify persons with vocal fatigue and has good clinical validity.

Resumen
Metabotropic glutamate receptors (mGluRs) form covalently linked homodimers and contain large, N-terminal extracellular ligand binding, “venus fly trap” (VFT) domains. These domains, when expressed separately, are secreted as disulfide linked dimers and can dimerize with full-length receptors. mGluR splice variants have been described that contain only this domain, but the consequences of their interaction on receptor signaling have not been explored. Here it is shown that an mGluR1 mutant containing only the VFT is retained on the cell surface when a full-length receptor is co-expressed. Further, when expressed in rat superior cervical ganglion (SCG) neurons and modulation of native calcium currents is used as an assay for receptor activity, the VFT acts as a dominant negative with respect to mGluR1 signaling. Although full-length mGluR1 and mGluR5 are not known to heterodimerize, the mGluR5 VFT partially occludes mGluR1 signaling and the mGluR1 VFT potently occludes mGluR5 signaling in SCG neurons. In addition, an mGluR1 point mutant, mGluR1 C140G, which cannot covalently dimerize, functions like the wild-type receptor when expressed alone. The C140G mutant is inhibited by the mGluR1 VFT construct but does not retain the mGluR1 VFT on the cell surface, suggesting that the loss of C140 renders the interaction reversible. Finally, a peptide designed to disrupt mGluR1 dimerization reduced signaling through the C140G mutant receptor, but only when applied intracellularly for several hours, indicating that loss of signaling requires disruption of dimerization prior to plasma membrane insertion.

Various studies have reported that the voluntary intake (VFI) of hays (Hovell, Ngambi, Barber & Kyle. 1986, Khazaal, Dentinho, Ribeiro & 0rskov. 1993) and straws (Ørskov, Reid & Kay 1988, Blummel & Ørskov, 1993) is predicted better by rumen degradation characteristics than by whole tract digestibility. Prediction of grass silage VFI is in general poor but is related to both whole tract digestibility and silage fermentation characteristics. However no attempts have been made to relate silage VFI to rumen degradation characteristics. It was the purpose of this study to attempt to predict silage VFI from rumen degradation characteristics measured using either the polyester bag technique or the in vitro Menke gas production technique.

Resumen
Individuals with incomplete spinal cord injuries (iSCI) often experience impairments in motor control of lower-limb muscles, causing a lack of balance when standing. These individuals are usually heavily dependent on others, reducing their self-sufficiency and quality of life. Our previous study has shown that visual feedback training (VFT) of tracking an individual’s center of pressure (COP) can improve balance stability of those with iSCI (1). Another of our previous studies demonstrated that functional electrical stimulation (FES) augments therapeutic effects in general (2-8). Thus, VFT in combination with FES is hypothesized to improve the standing balance ability of individuals with iSCI more effectively than a VFT or FES system. The purpose of this study was to develop a novel therapeutic system by combining VFT with FES. We developed this system by utilizing LabVIEW (17.0, National Instruments Corp., Austin, USA). The system hardware consisted of a force plate, an electrical stimulator, and a computer. The COP was calculated using the force plate signals, which was displayed in the monitor in front of the participant. We developed four COP-based games, each forcing the participant to shift their COP in various movement patterns. FES will provide electrical stimulation to participant plantarflexors and dorsiflexors to assist with movements, in which stimulus intensity will be modulated through a proportionalderivative (PD) feedback controller that mimics the physiological control system for standing (9,10). In this study, we tested the practicality of our VFT games with seven able-bodied participants (age 19-24 years). Each participant performed a balance assessment before playing the VFT games to determine their balance ability. Our focus was on developing effective VFT games and assessing each participant’s balance ability against their VFT game scores. Three out of four games showed a significant bivariate correlation between balance ability and VFT scores (r > 0.754 for n=7), indicating that the games were appropriately designed. The validity of the FES controller was demonstrated in another study to be effective. The parameters of the PD controller were optimized so that the stimulation intensity would match the natural muscle contraction of the plantarflexors and dorsiflexors, acquired through electromyography (EMG) signals. It is expected that rehabilitation combining VFT with FES will improve static and dynamic balance stability in a more effective way than isolated therapy techniques. Since this study has shown the validity of our VFT games, we would like to integrate the VFT and FES systems and run clinical trials on patients with iSCI, with the goal of improving their balance ability in the future. Through visual feedback training, the brain improves spatial awareness by visualizing their body displacement and orientation. Applying stimulation to the muscles re-trains the central nervous system and, over time, allows patients with iSCI to partially regain motor control. Thus, the proposed system will help individuals with iSCI improve their balance ability and increase their quality of life.

Resumen
Vestibular Assessment in patients with Myasthenia Gravis (MG) is challenging, as diagnostic evaluation requires good recording of eye movements. Reports on Vestibular Function Testing (VFT) in MG patients have been scant and it is arguable that VFT will have little clinical value in the MG population. A 75-year-old man, with late onset acquired autoimmune MG presented with dizziness for evaluation. He completed VFT with no significant abnormalities in all tests and was elated to have vestibular ruled out as a contributing factor to his dizziness and imbalance. However, his functional impairments were still addressed and managed regardless of the test results. MG is a heterogenous condition that may be well-controlled with treatment. Patients with dizziness can still be diagnostically evaluated to rule in or out a vestibular involvement and should not be precluded from VFT. Patients should also be assessed for their functional impairments and not based on symptom checklist and objective test results alone. Hence, patients with normal VFT results can still benefit from a hybrid of vestibular rehabilitation therapy (VRT) with focus on habituation.

Abstract Purpose Conventional pediatric echocardiography is crucial for diagnosing congenital heart disease (CHD), but the technique is impaired by angle dependency. Vector flow imaging (VFI) is an angle-independent noninvasive ultrasound alternative for blood flow assessment and can assess complex flow patterns not visible on conventional Doppler ultrasound. Materials and Methods 12 healthy newborns and 3 infants with CHD were examined with transthoracic cardiac VFI using a conventional ultrasound scanner and a linear array. Results VFI examinations revealed common cardiac flow patterns among the healthy newborns, and flow changes among the infants with CHD not previously reported with conventional echocardiography. Conclusion For assessment of cardiac flow in the normal and diseased pediatric heart, VFI may provide additional information compared to conventional echocardiography and become a useful diagnostic tool.

Resumen
In this study students judged suicidal ideation, attempted suicide, and completed suicide to be inappropriate responses to crises, though completed suicide was judged to be even more inappropriate than the lesser suicidal responses. In addition, condemnation of suicide was associated with lower scores on a test of general irrational thinking.

Resumen
The color of 15 red wines from several wineries within the renowned wine-producing region Rioja (Northern Spain) was measured by spectrophotometric and spectroradiometric techniques and was visually assessed in a pair-comparison experiment by a panel of 10 experienced observers having normal color vision. Correlation between instrumental color measurements made by spectrophotometric and spectroradiometric techniques was very low, as expected from major differences in the experimental conditions employed (different illumination, path lengths and glass effects). Spectroradiometric measurements at the center of the wine sampler and at positions displaced 1 cm in the horizontal and vertical directions were quite different, mainly because of an increase of the lightness L*, the average color differences between them being high (3.5 and 2.6 CIELAB units, respectively). A 50% acceptance percentage resulted for a color difference of 2.8 CIELAB units, using a reference anchor-pair of wine samples with 4.0 CIELAB units. Thus, a value around 3.0 CIELAB units should be considered a preliminary estimate of the acceptable tolerance by the human eye for red wines poured in standard wine samplers.
