A dilemma for market organizers is created by the fact that sellers want to know as much about buyers as possible, whereas buyers rarely want the seller to have that information—not least for privacy reasons. This dilemma is affected by market organization, and market organization may also be used to try to change imbalances in the conflicting information interests. In the market for personal insurance, insurance sellers require in-depth information about the buyer’s health conditions, in order to make an accurate categorization. As this information is sensitive to buyers, however, and can potentially exclude them from the market, many buyers are concerned about sharing it. This chapter demonstrates how sellers have spent considerable resources trying to organize buyers. However, the considerable imbalance in favour of the sellers’ interests triggered buyers and their advocates to call for market reorganizations. Eventually the state reacted and reorganized the market, but only modestly so.

Considering incidents that make headline news internationally, given the modern information and communication technology revolution, the facility of citizens to rapidly mobilize represents a considerable threat to autocratic survival. While the speed with which popular movements emerge has increased exponentially, and the news of their existence spreads faster and farther, civil unrest has threatened the stability and survival of dictators for centuries. The paranoia and machinations of dictators depicted in films, such as the portrayal of Ugandan dictator Idi Amin in The Last King of Scotland, while sensationalized, capture the astounding array of threats with which unelected leaders must concern themselves. On the one hand, they must worry about insider threats to their standing, such as conspiratorial plots from people within the dictator’s own circle or mutiny among government soldiers. On the other hand, dictators also must monitor threats originating from non-regime actors, such as new alliances forming among once-fragmented opposition groups or the possibility of sustained insurgency or a popular revolution. From force to finesse, autocratic leaders have developed a broad and evolving range of tactics and tools to diminish both internal and external domestic threats to their reign. The success of dictators’ endeavors to insulate their regimes from forces that might challenge them depends on accurate and reliable information, a resource that can be as valuable to the leader as would a large armory and loyal soldiers. Dictators invest significant resources (monetary as well as human capital) to try to gather useful information about their existing and potential opponents, while also trying to control and shape information emitted by the regime before it reaches the public. New information and communication technologies (ICTs), which have drawn a great deal of scholarly attention since the beginning of the 21st century—present both risks and rewards for dictators; inversely they also create new opportunities and hazards for citizens who might utilize them to mobilize people opposed to the regime. While civil unrest could encompass the full range of domestic, nonmilitary actors, there also needs to be a specific focus on various forms of mass mobilization. Historically, more dictators have been forced from office by elite-initiated overthrows via coups d’état than have fallen to revolution or fled amid street protests. Civil unrest, in its many forms, can affect autocratic survival or precipitate regime breakdown. While mass-based revolutions have been a relatively rare phenomenon to date, the actions of many 21st-century dictators indicate that they increasingly concern themselves with the threats posed by popular protests and fear its potential for triggering broader antigovernment campaigns. The ease of access to information (or the lack thereof) help explain interactions between authoritarian regimes and citizens emphasizes. The role of information in popular antigovernment mobilization has evolved and changed how dictators gather and utilize information to prevent or counter civil unrest that might jeopardize their own survival as well as that of the regime.

Many studies have supported the idea that human interaction relies on cooperation and joint action, implying that everyday communication has primarily a social motivation. According to Grice, interlocutors are expected to meet the informational needs of their interactional partner(s) in both accuracy and informativeness. However, conversional principles incompatible with Grice’s maxims have been found to be implicitly applied in traditional societies from Madagascar, the Pacific Islands, and Mesoamerica. This chapter considers the management of epistemicity among the Yucatec Mayas of Mexico, focusing on the function and use of evidential particles and the broader cultural context into which they fit. Another is to present the implicit rules that adults and children follow to evaluate and endorse claims of knowledge. Such rules might explain why Yucatec Mayas tend to be linguistically accurate in stating and evaluating knowledge sources. Because they expect others to lie or withhold information, they constantly monitor how their and others’ assertions and information are shared.

It is important to know the information is true and accurate and does not represent ‘fake news’ or alternative facts. Patents describe inventions and contain accurate information, as patents are examined and their accuracy can be challenged. This book shows how patents and the inventions they describe have shaped the modern world, that is the world in the twenty-first century. Patent documents that date from the mid-nineteenth century to the present time are used in the text and the subject matter covers many technical areas: for example, Morse code, the diode, triode, transistors, television, frozen foods, ring-pulls for soft drink cans, board games such as Monopoly, gene editing, metamaterials, MRI, computerised tomography, insulin and monoclonal antibodies such as Herceptin. Up to a page of text is used for each entry and the text is backed up by drawings from patent documents. Patent numbers are included to allow interested readers to trace the documents. Inventions described in the patents are placed in a historical perspective. For example, the role of the cavity magnetron and radar are described in the context of the Second World War, whereas the diode is discussed in the development of broadcasting at the beginning of the twentieth century. Entries cover examples from life sciences, engineering and physical sciences in the modern world.

It is therefore of critical importance that as much accurate information about each individual case is obtained. This is achieved with a variety of diagnostic procedures which allow the multidisciplinary team to reach correct decisions about management. The types of investigations performed typically include radiology and pathology, but clinically important information may also be obtained by other methods, for example surgical staging, clinical examination, endoscopy, and blood tests. As well as directing therapy, accurate staging also allows a more precise estimation of prognosis or the probability of treatment success, knowledge which is of obvious importance to the patient.

QA may be both an internal and an external process. QA relates to all aspects of a service, so not only should reports be timely, accurate, and useful, but the patient’s experience should be a safe and positive one. All aspects of a service need to be continuously monitored and optimized if quality is to be maintained and furthermore improved. Particular focus is needed on reporting and measurement with continuous checking for reporting errors and assessment of measurement variability. A systematic quality assurance programme needs to be an essential part of any echo laboratory. Life-determining clinical decisions may depend on it.

It is non-invasive and avoids patient exposure to ionizing radiation. MRI has the advantage of establishing diagnoses in a painful knee without the morbidity of surgical intervention. It is now widely available and has moved from a simple diagnostic adjunct into a key planning tool. It offers improved management of theatre resources and it allows for more accurate planning of postoperative rehabilitation.The role of MRI in management of the injured knee is determined by its cost-effectiveness and its ability to augment the diagnostic accuracy of clinical examination. Accuracy of clinical examination by specialist orthopaedic surgeons is comparable to MRI when interpreted by specialist radiologists (Table 8.3.1). Increasingly, MRI has been shown to be cost neutral. Whilst costs are high, diagnostic information reduces the need for unnecessary surgery.

More than any other branch of medicine, the practice of neurology depends on the classical methods of intuitive conversation, structured examination, and selective investigation. We teach the importance of eliciting an accurate neurological history. The key symptoms at onset are identified and their subsequent course defined. For the experienced clinician, this process becomes routine, efficient, and quick. The competent neurologist is the one who instinctively senses relevant components of the history, appreciates the most likely underlying disease mechanisms, reliably elicits the relevant physical signs, knows which investigations are necessary and assesses their relevance in the clinical context, provides a sensible clinical formulation, and communicates the situation accurately and sensitively to the patient and relatives. Rather than slavishly collecting an encyclopaedia of facts, in which the key issues may be lost in a surfeit of redundant information, the critical components are sifted and the subsequent conversation steered down an algorithm that seeks anatomical, physiological, and pathological explanations for what the patient describes.
