Dexmedetomidine is α2-adrenergic agent and commonly used in the intensive care setting. It is used primarily to sedate critically ill patients in various surgical, endoscopic and radiologic procedures. This medication gained superiority with other sedatives with a distinct advantage of less depression of the respiratory system. Although dexmedetomidine is often administered to septic patients, the vascular effect has not been fully studied in this clinical setting. Résumé :
The mechanism by which β-agonists induce skeletal muscle hypertrophy is believed largely to be through a reduction in protein degradation. These growth promoters are also known to effect the activity of the calcium dependent proteinases (calpains) and their specific endogenous inhibitor calpastatin. The changes in activity appear to be toward a decrease in the calpain system's proteolytic potential. In this study attempts were made to determine whether the altered activity of the enzymes and inhibitor were brought about by induced changes in gene expression as reflected by altered levels of sp

Cyclization of linear peptides restricts the number of conformations of the peptide, increasing the peptide's affinity to binding to the desired target. Cyclization also stabilizes the peptide, allowing the peptide to be resistant to proteases. This study explores the optimization of solution phase synthesis of an important integrin-media

For cardiovascular diseases such as high blood pressure, angina pectoris, and left ventricle hypertrophy; long-term activation of beta-adrenergic receptors is strongly linked to the progression of these diseases. A class of antagonistic drugs that target betaadrenergic receptors are collectively called beta-blockers. These drugs are commonly used to reduce the inotropic and chronotropic effects of beta-adrenergic receptor activation. This past decade has revealed that beta-blockers and other ligands are capable of functional selectivity at receptors. Functional selectivity describes the abilit

Résumé :
Beta-blockers are avoided in asthma and chronic obstructive pulmonary disease (COPD) due to the potential risk of drug induced bronchospasm. Despite these concerns, beta-adrenoceptor antagonism has recently been associated with potential therapeutics benefits in asthma. Furthermore the use of beta-blockers in COPD patients may potentially result in improved survival due to optimisation of treatment in those with concurrent cardiovascular disease. This thesis evaluates the role of beta-blocker use in asthma and COPD. The introduction outlines the pharmacological principles associated with the h

Beta-blockers and non-steroidal anti-inflammatory drugs (NSAIDs) are often avoided in asthma over risk of bronchospasm which may vary according to drug selectivity and duration of administration. This thesis attempts to quantify the risk of beta-blocker and NSAID exposure in asthma by synthesising clinical trial evidence and conducting observational studies using linked electronic medical records. As part of this thesis, three systematic reviews of clinical trials were conducted evaluating: the prevalence of aspirin-exacerbated respiratory disease (AERD); risk of selective NSAIDs/COX-2 inhibit

The number of myocardial infarctions (MIs) in population remains high and this event is a significant predictor of mortality. Information in the literature points to a reduction in mortality, reinfarction and sudden death in first year, especially in patients with high risk, if β-blockers (BBs) are used after MI. Two investigations were conducted to examine the influence of beta-adrenergic blockade on cardiovascular, respiratory, metabolic, and thermoregulatory responses to maximal and submaximal exercise in both highly trained and untrained subjects. In both studies, subjects received randomized and double-blind oral medication with atenolol (100 mg/day), propranolol (160 mg/day), and placebo. In the first study significant reductions in HR max and ‘VO₂ max resulted during the atenolol and propranolol treatments in both the trained and untrained subjects. However, the reductions in ‘VO₂ max were signi

Purpose. To examine the outcome of a 6-month treatment with carvedilol or metoprolol on peak and submaximal exercise performance and ventilatory efficiency in patients with heart failure (HF).<br>Methods. 27 patients with HF were randomized to receive either metoprolol or carvedilol for 6 months and compared with 12 healthy controls. Maximal exercise capacity was assessed at baseline and after 6 months with a symptom limited incremental treadmill protocol (RAMP). Submaximal exercise was determined to be the portion of exercise below a respiratory exchange ratio of 1.0. Peak heart rate (H
