Given the clinical and hormonal characteristics, as well as specific changes according to MRI, the diagnosis of "congenital hypopituitarism" did not cause doubt. After the purpose of the chorionic gonadotropin of 1500 units 2 times a week for 2 months, there was a positive dynamics from the genital organs.

Background and PurposeConstrained practice is routinely encountered by physical therapists and may limit the physical therapist's primary moral responsibility—which is to help the patient to become well again. Ethical practice under such conditions requires a certain moral character of the practitioner. The purposes of this article are: (1) to provide an ethical analysis of a typical patient case of constrained clinical practice, (2) to discuss the moral implications of constrained clinical practice, and (3) to identify key moral principles and virtues fostering ethical physical therapist practice.CaseThe case represents a common scenario of discharge planning in acute care health facilities in the northeastern United States.MethodsAn applied ethics approach was used for case analysis.ResultsThe decision following analysis of the dilemma was to provide the needed care to the patient as required by compassion, professional ethical standards, and organizational mission.Discussion and ConclusionsConstrained clinical practice creates a moral dilemma for physical therapists. Being responsive to the patient's needs moves the physical therapist's practice toward the professional ideal of helping vulnerable patients become well again. Meeting the patient's needs is a professional requirement of the physical therapist as moral agent. Acting otherwise requires an alternative position be ethically justified based on systematic analysis of a particular case. Skepticism of status quo practices is required to modify conventional individual, organizational, and societal practices toward meeting the patient's best interest.

Carcinoma of the parathyroid gland is a rare malignant tumor of the person, which manifests itself clinical syndrome of hyperparathyroidism. The tumor is characterized by structural signs of invasive growth and is characterized by an unfavorable clinical prognosis, often accompanied by metastasis and the formation of relapses. The article provides background information about the incidence of parathyroid carcinoma, the gender and age characteristics of patients with this disease, options for localization of the tumor in the gland itself (typical topic of the neoplasm), in other areas of the neck, thyroid, thymus, and so on. We discuss the features of clinical manifestations of parathyroid carcinoma, among which the most typical are manifestations of hyperparathyroidism and hypercalcemia, and also provide figures for five - and ten-year survival. The article presents the author’s observation of parathyroid carcinoma with an intrathyroid location in a 64-year-old man. After the examination, the patient underwent a cervicotomy with revision of the areas of the typical location of the parathyroid gland in the neck, hemithiroidectomy on the left with a parathyroid tumor incorporated in the thyroid tissue, which was later proved using immunohistochemical research methods. The article details the macroscopic, microscopic, and immunohistochemical characteristics of the operating material. A special feature of the case is a rare atypical intrathyroid localization of parathyroid carcinoma.

Cholecystitis at the current stage of clinical veterinary development attracts attention among specialists. The development methods of early diagnosis, expanding the knowledge of veterinarians and increasing the number of animals` diseases with suspected cholecystitis require detailed discussion and analysis. The article presents the results of the diagnosis of dogs with cholecystitis, which became a purpose of research. To achieve the purpose, we have performed the following tasks: analysis of the causes of cholecystitis and the use the various methods of diagnosing a disease. The scheme of diagnosis included: medical history, application of general research methods, laboratory blood tests and special methods (ultrasonography of the liver and gallbladder). We found that the reason for the development of cholecystitis of dogs was the presence of inappropriate food in their diet (sweets, pork and milk). Clinically the disease is manifested by disorders of the gastrointestinal tract (anorexia, alternating diarrhea, vomiting, faecal discoloration), hyperthermia (39.6 ° C), pale mucous membranes, dry skin, dull fur, depression of the animals` general condition, abdominal pain on palpation. There is a note that the dog bends its back up while lying down. In the blood there is an increase in erythrocyte sedimentation rate, oligocythemia, leukocytosis with a shift of the neutrophil nucleus to the left. Biochemical studies showed an increase in bilirubin to 11.4 μmol / l, increased activity of enzymes, in particular aminotransferases (ALT and AST), alkaline phosphatase, confirming the cytolysis of hepatocytes and cholestasis. At the same time, there is a doubling of alpha-amylase activity and it is characteristic for pathological processes in pancreas. Ultrasonographically cholecystitis of dogs was characterized by thickening of the gallbladder walls in the form of hyperechoic nodules (folding of the mucous membrane) and there was an increase in its volume. In the lumen flake-like hyperechoic sediment was observed, which moved freely along the wall of the gallbladder.

According to the European Innovation Scoreboard (2017) report, Hungary’s summarised innovation score is 67.4 against the EU28 average of 102. This implies that the Hungarian economy has got rather serious disadvantages in the European Union community. This statement is more pronounced in the case of the food industry. From an innovation point of view food industry is seen as a slow one, which is lagging behind the technology pushed possibilities, but sometimes behind the customers’ desires and requirements as well. In our research, we determine why the food companies in Hungary do not engage in innovation activities and if they do so, what are the main drivers of their innovation performance? We use the Community Innovation Survey (CIS 2012a) data and employ double hurdle estimation because of the nature of the innovation distribution. This method also helps in overcoming the selection bias problem, which necessarily occurs in this situation. Results prove that networking scope as well as networking intensity, play an important role in explaining innovation performance. The size and market obstacles are also significant factors.

Differential diagnosis of articular syndrome is one of the leading issues of primary medical sanitary care. Timely and correct diagnosis of rheumatic diseases made by an internist in an out-patient setting is a cornerstone of successful treatment and prevention of possible complications. The masks of clinical course, subtle symptoms, as well as pathomorphosis of the disease make the accurate diagnosis difficult to establish. Among rheumatic diseases more and more often isolated hematological manifestations are observed on the onset of the disease. Among them the leading place is taken by Felty’s syndrome. Being a form of seropositive rheumatoid arthritis, it is often associated with cytopenia. The aim of the article is to draw attention to the problem of timely diagnosis of Felty’s syndrome. The presented clinical case of Felty’s syndrome with agranulocytosis was based on clinical, laboratory, instrumental data obtained on examination of a patient. The features of the course, diagnostic methods and treatment methods of the pathology are discussed. The treatment efficacy of corticosteroids and cytostatics, similarities in clinical presentation and course of the syndrome with lymphoproliferative diseases and liver cirrhosis are presented. According to the obtained data a conclusion was made about the need for early diagnosis and complex examination in case of Felty’s syndrome for successful treatment of the nosology.

Abstract This study analyses knowledge networking between educational institution/knowledge provider, associations and knowledge users in the agricultural sector within the local environment in Slovenia. It is based on the theory of expansive learning and the concept of knowledge networking. A qualitative research paradigm using ethnography and in-depth semi-structured interviews has been used and involved various actors included in selected activity systems (the educational institution, associations and knowledge users). Our findings indicate that cooperation between associations and the educational institution has advantages for all involved and that, according to research participants, it fosters the transfer of knowledge from the academic/research sphere into practice. In the case studied, we describe the development of new forms of connections between local knowledge and academic knowledge, both developing as a part of real-world complex learning environment. Knowledge networks have thus been built by enabling the transfer of explicit and tacit knowledge through social networks and by the development of new practices.

Purpose The purpose of this paper is to identify the importance of trade-offs between the United Nations Sustainable Development Goals (SDGs) and creating a holistic, integrated approach. Design/methodology/approach This study uses secondary data including official reports from three companies: Accor Hotels, Scandic Hotels and Atlantica Hotels. Third party references from research sources are included. Findings This study concludes that companies are adopting corporate social responsibility practices focussed on stakeholders’ needs. Additionally, the general trend in the case study hotel companies is towards similar sustainability approaches though less progress is reported in addressing the more complex issues relating to SDG 8 and SDG 10 because sustainability targets are more difficult to determine, implement and measure. Originality/value The three selected companies were chosen based on location criteria. Accor has a worldwide presence whereas the other two companies could be considered as multinational companies. Scandic is a Swedish company operating in Scandinavian countries whereas Atlantica is a Cyprus-based company operating in Greece and Egypt. The contrast among the selected companies displays different considerations regarding their commitment to SDGs.

Abstract Analysing teaching-practice offers an opportunity to answer questions like what is critical to making a pedagogy democratic, what are the factors that support a teacher to be critical in her teaching? Or what restricts the teacher in being critical in her work? This paper seeks to address some of these questions by presenting the findings of an investigation into the practice of teachers who are committed to the idea of critical pedagogy. The scope of the study is limited to understanding the critical aspects that are related to the teacher’s work within the classroom. The paper analyses the theoretical arguments that are relevant to critical pedagogy in relation to teachers’ practices as they emerged during the study. The study, conducted in the South Indian state of Kerala, reveals that teacher subjectivity and schooling situations interact in a dialectical fashion to shape the nature of classroom teaching. The political subjectivity of the teachers, shaped by their close interaction with the Kerala Science Literature Movement (KSSP) makes their pedagogy critical in nature. On the other hand, the standardized curriculum and mechanically disciplined school environment continuously challenge the teachers’ efforts at being critical in their work.

This research proposes a framework for value analysis derived from a practice. We link this concept with the consumer value in marketing. The approach adopted for the value derived from a practice is bivalent, including the various reasons for valuing and devaluing the activity. We then propose a model based on the assumption that the value derived from a practice changes with the intensity of the practice. The empirical study focuses on the practice of eco-friendly gestures that aim to save electricity. The results show that the more intensified the practice, the stronger the value creation while the loss of value is reduced. In addition, a multi-group analysis allows the diversification of the value to be observed when the intensity of the practice increases. The results reveal the importance of eudemonic value, which offers a new avenue for understanding the mechanisms of appropriation of practices with societal stakes.

Six farms were examined, each from a different sector of Scottish agriculture. Surveys were carried out to identify both diffuse pollution risks and options for habitat conservation and enhancement. Financial data were also gathered to determine the current sources of farm income, both from sale of produce and from grants. Whole farm plans were produced aimed at bringing about reductions in diffuse pollution to water, soil and air and also habitat improvements. The assembled information was used to devise a possible agri-environment grant scheme to aid the implementation of the whole farm plans.

Background and objectives: Siesta, which is a short afternoon nap, is a habit that is commonly practiced in the Mediterranean and tropical areas. Data on the association between siesta and coronary artery disease has been conflicting. A protective effect has been demonstrated in the countries that commonly practice siesta, but a harmful effect has been observed in the countries that infrequently practice the siesta habit. Information on the association between siesta and ischemic stroke has been, however, lacking. Hence, the purpose of our study was to determine the effect of siesta on ischemic stroke. Materials and Methods: This was a case-control study, conducted on the patients with acute ischemic stroke who came for their first follow-up visit to the neurology clinic. Controls were randomly selected from the patients visiting the neurology clinic on the same day as the patients with ischemic stroke. In addition to basic demographics and the occurrence of established stroke risk factors, information about siesta practice was also collected from both groups. A multivariate logistic regression analysis was utilized to determine the relationship between siesta practice and ischemic stroke. Results: A total of 206 patients were recruited from the neurology clinic of King Khalid university hospital; of which only 194 subjects were included in the analysis (98 ischemic stroke cases and 96 controls). The mean age of the participants was 59.68 ± 13.75 years and 98 (50.52%) were male. Interestingly, 43% of the whole study cohort practiced regular siesta. However, when compared to the stroke population, the control group practiced siesta more frequently (30% vs. 56%). In a multivariate logistic regression analysis, hypertension, diabetes mellitus, excess body weight (body mass index > 25 kg/m2) and dyslipidemia were found to increase the risk of ischemic stroke (OR 2.12, 95% CI: 1.02–4.66, p = 0.005; OR 2.72, 95% CI: 1.94–4.88, p = 0.014; OR 2.94, 95% CI: 1.5164–5.7121 p = 0.0014; OR 3.27, 95% CI: 2.42–5.199, p ≤ 0.001, respectively). On the contrary, the practice of regular siesta lowered the risk of ischemic stroke (OR 0.58, 95% CI: 0.3551–0.9526, p = 0.031). Conclusions: Siesta was associated with a reduced risk for the occurrence of ischemic stroke. Large prospective longitudinal studies should be conducted to verify the protective effect of siesta on stroke.

Administrative corruption in a simple definition is violation of a law for personal benefits by utilization of job position. It is a phenomenon within today’s world as one of the most important obstacles on the way of societies’ progress. The impact of different factors in forming corruption has given it a complicated nature. Administrative corruption is a correlative issue and it is different according to value system of each society. This article is trying to present a solution in order to fight against administrative corruption through classification of staff with the help of explaining the relation of their perception, sensation and commitment towards corruption and corrupted situations. Present study in terms of purpose is developmental-practical, and in terms of execution and based upon research purposes is a survey. The study population is the staff of Ministry of Economic Affairs and Finance in 2015. In order to explain perception, sensation and the behavior of staff, the questionnaire of administrative corruption is designed in three dimensions: perceptive aspect (cognitive) which includes items for assessing the status of awareness and recognition of staff from instances and consequences of administrative corruption; sensation aspect which includes items for assessing the vision and tendencies of staff towards corruption, and behavioral aspect which is formed from items for assessing the behavior of staff in facing with corruptive situations. In order to identify the reasons of corruption two questionnaires of “National identity” and “Organizational culture” have been designed. The result of the research was that the perpetrators of corruption can be divided into three categories. First category is the staff that their perception is positive, it means that they have adequate cognition from manifestations and corruption consequences and their feeling about corruption is negative, it means that they consider it as an ominous phenomenon, but they are guilty of corruption. Second category is the staff that their perception is positive, they don’t see corruption as an ominous phenomenon. Consequently, their feeling toward corruption is positive, and they perpetrate it. The third category is the staff whose perception is negative, their feeling is uncertain, and they perpetrate it. The behavior of these categories will be interpreted in form of related theories. The significant note is that despite the tendency average to practice administrative corruption among governmental staff is lower than assumed average and it indicates that there is a protection of corruption perpetration, but the rank of our country is not favorable the International Organization report. This warning made the researchers to provide new solutions to help resolving this social issue by reviewing current solutions for prevention, and fighting against administrative corruption, regarding the richness of evaluating system in Iran. This article explores the importance of quality practices in underpinning the person-centred approach at a Community Options Program (COP) case management service in northern NSW. The NSW community care sector does not have a statutory excellence body to identify, promote and support improved practices and quality and safety across community services, and therefore the COP provider decided to establish a dedicated role to focus on the quality improvement of its service. The subsequent quality improvement initiatives have included mapping the clients’ journey through the service, identifying areas to standardise practice, and creating service pathways. The clients’ journey was used as the framework to identify where standardised practice was required, and a robust process was implemented to develop over 25 good practice guidelines and tools that addressed the variations in practice and enabled the service pathways to be developed. Prior to trialling the guidelines and tools, staff received education sessions on the anticipated changes to practice, and the practicality and applicability of the guidelines were evaluated at the end of the trials. This information was reviewed and the guidelines were amended accordingly before being rolled out. The guidelines have been in use for over 12 months and have provided the benchmark against which to audit practice, and have resulted in key performance improvements such as an increase in client review rates and a rise in the feedback response rate from clients, with a noticeable shift in the comments about the brokered support worker to acknowledging the role of their case manager. Formalising informal supports for those clients that lived alone also increased, which means these people are less reliant on services and there is a reduced risk of social isolation.

Background and Purpose: Qualified education is characterized by qualified teaching and learning in the classroom, laboratory, and clinical practice. Producing nurses who are able to provide professional nursing care for patients requires learning process in practice by using learning models. The purpose of this study was to explore the experience and expectation of students and clinical instructors on practices learning model in Ners Program of STIKES Bali.Method: This study used qualitative design with phenomenology approach. There were 8 participants from students and clinical instructors selected by purposive sampling. Data were collected by in-depth interviews. Data was transcribed and analyzed using thematic analyses. Results: Findings indicated that there were five themes related to student and clinical instructor’s experience and expectation about practices learning model. The themes were supporting from institution and clinic, bed side teaching (BST), conference, lack of time of clinical instructor in supervising, and case study. BST, conference, and case study were some of learning models implemented in clinical practices. These methods will run properly if there are good supporting and optimal supervising. Conclusion and Recommendation: BST, conference, and case study are appropriate methods for clinical teaching. Lack of time in supervising is one of barriers found. It is essential for clinical instructors to spend sufficient time to supervise students in clinical practice.

ABSTRACTObjectiveAssess Hurricane Sandy’s impact on primary care providers’ services in the Rockaways.MethodsIn-person surveys were conducted in 2014. A list of 46 health care sites in the area of interest was compiled and each site was called to offer participation in our survey. Respondents included physicians and practice administrators who remained familiar with Sandy-related operational challenges.ResultsOf the 40 sites that opted in, most had been in their current location for more than 10 years (73%) and were a small practice (1 or 2 physicians) before Hurricane Sandy (75%). All but 2 (95%) had to temporarily close or relocate. All sites experienced electrical problems that impacted landline, fax, and Internet. Less than one-quarter (n = 9) reported having a plan for continuity of services before Hurricane Sandy, and 43% reported having a plan poststorm. The majority (80%) did not report coordinating with other primary care stakeholders or receiving support from government agencies during the Sandy response.ConclusionsHurricane Sandy significantly disrupted access to primary care in the Rockaways. Severe impact to site operations and infrastructure forced many practices to relocate. Greater emergency response and recovery planning is needed, including with government agencies, to minimize disruptions of access to primary care during disaster recovery. BackgroundSerious adverse events (SAE) at out-of-hours services in primary care (OHS-PC) are rare. It most often concerns missed acute coronary syndromes (ACS). Root cause analyses highlighted errors in the triage process, but these analyses are hampered by hindsight bias.AimTo compare triage calls at the OHS-PC of missed ACS with matched controls with chest discomfort but without a missed ACS; and to assess predictors of missed ACS.MethodA case-control study with triage recordings of calls of a missed ACS registered between 2013–2017. Controls were from the same period. Cases were matched 1:8 with controls based on age and gender. Clinical, patient and call characteristics were assessed, and 15 expert GPs rated the triage safety and quality, being blinded to the final diagnosis. We applied conditional logistic regression analysis.ResultsFifteen missed ACS calls and 120 matched control calls were included. Cases used less cardiovascular medication (38.5% versus 64.1%, P = 0.05), and more often experienced retrosternal chest pain (63.3% versus 24.7%, P = 0.02) than controls. Consultation of the supervising GP (86.7% versus 49.2%, P = 0.02) occurred more often in cases than controls. Experts rated the triage of cases more often as ‘poor’ (33.3% versus 10.9%, P = 0.001), and ‘unsafe’ (73.3% versus 22.5%, P<0.001) than that of controls.ConclusionIt seems nearly impossible to differentiate missed ACS at the OHS-PC from others with chest discomfort based on symptom presentation.
