Texte intégralThis theses is a reflection on the sociology of health and hospital from the nurses case. In the first part, a critical historical reading of sociology,is made, from the foundings texts of t. Parsons in the fifties to major present-day contributions dominated by interactionist analyses and sociology of organizations. In the second part, the nursing profession is presented diachronically and synchronically. Three coexisting figures are brought out : the "nun", dominated by religion, the "domestic" by science and technology through the doctor figure, and the industrial worker by a bureaucratic organization. Using the method of "sociological intervention" worked out by a. Touraine, the third part looks into the nursing experience from the relationships with other hospital workers and patients. Contradictory demands managed by the nurses appear to be at the heart of this experience in pieces, while the analysis of the nurses' mobilization still renforces the seeling of an identity crisis in the profession. The conclusion goes back to the sociology of health which touches on the question of the evolution of the health car system from a call to consider the pathient as a subject

Texte intégralA partir d'un échantillon (n=841) issu d'une enquête réalisée auprès de l'ensemble des infirmières d'un CHU en Suisse (n=1951), au travers d'une approche sociocognitive (BANDURA, 2003), la recherche des principaux facteurs favorisant le développement d'une « pratique de santé », identifiée comme la centration sur le malade et sa santé selon sa propre définition de la santé, amène à mettre en évidence une double dimension de l'agentivité: autonome et hétéronome.
Ainsi, les résultats montrent que la valeur attribuée par l'infirmière à sa « pratique de santé » détermine son orientation au sein d'un contexte qui reste prioritairement dirigé vers une « pratique de soins », centrée sur la gestion de l'ensemble des soins auprès du patient selon la définition de l'institution. Dès lors, l'autonomie dans les pratiques infirmières se présente comme l'exercice de l'agentivité, soit la puissance personnelle d'agir de l'infirmière, selon sa propre orientation : santé ou soins. Elle se développe significativement chez les professionnelles ayant un haut niveau de formation, santé ou soins, par une approche centrée sur l'apprentissage tout au long de la vie.
Cette thèse permet de tracer des perspectives relatives à la clarification des rôles et des missions des infirmières et de leurs développements, au sein des établissements hospitaliers universitaires en particulier. Elle pose la question de la construction d'un sentiment d'efficacité personnelle (SEP) qui ne correspondrait pas à l'attente prioritaire et explicite du contexte. Enfin, en distinguant l'agentivité de l'autonomie, elle contribue à la construction conceptuelle des questions liées à l'autoformation (CARRE, 2005).

Texte intégralThis thesis focuses on issues experienced by nursing students when learning their trade. These issues are considered from their ability to endorse the responsibility of their acts to their assumption of performance of their practices in light of their professional identity. The aim was to study the contribution of the nursing students’ activity analysis to the development of their professional identity throughout this reflexive activity. The indirect historical method of the clinical approach of activity has induced the development of the student’s reflection both on their way nursing and on their way of being regarding the profession they are learning. The dialogic observatory generated both by four nursing students discussing their care and by four expert trainers analyzing these reflections, has highlighted how the discursive activity between students has contributed to transform their care and their professional identity all together. Moreover, the trainers have discussed capitalizing students’ agreements for further use in teaching and questioned their own practices. Their exchanges, built upon watching significant video extracts of students’ confrontations, led them to a better understanding of how the students may go beyond the set tasks. The nursing students have thus become resources for the learning organization. The proposed analysis model can therefore be useful to nursing trainers as well as tutors

Texte intégralThe care is a particular form of knowledge. It differs from the human sciences in that it is not derived from philosophy. Knowledge of care is also separate from medicine in that it relies on a humanistic paradigm rather than on biology. Born of an empiric practice, however, care has developed a specific engineering. The lack of infrastructure pose theoretical difficulties for the development of knowledge and interpretation of the clinic. The research project is to build a speculative and theoretical for care from a philosophical reading of it, this from an approach that takes care as a phenomenon and in different historical situations of care. The analysis allows a description in which the care varies depending on the situation and to identify elements of its essence, then describe the construction and operation. The development of specific concepts, such as general care, care first, the care professional can specify how the task is being built. It is therefore possible to reflect the care process as a vehicle for the preservation and development of the person care rather than palliative for the disease or disability. I will be able to account for apparent contradictions and peculiarities of the device as caring relationship. At last the search led to laying the groundwork for developing an ethic of care and proposes a way of renewing the research carefully

This research has a double theoretical framework: the anthropological theory of the didactic (TAD) and more particularly the notion of praxeology and the communities of practice. Texte intégralResearch shows that while various disciplines are inspired by the philosophy of science to examine how their own discipline progresses and produces its knowledge, nursing is in its early stages to evaluate and criticize its existing knowledge by means of the philosophy of science. In the present thesis, using the philosophy of science, we attempted to highlight the conceptions of some authors who have proposed ways of advancing the nursing discipline, particularly those of Fawcett and Risjord. Both theorists who are the representatives of two different schools of thought in the nursing discipline: the first one approaches the pure scientific thought of Martha Rogers (1972) and the second approaches the practical thought of Dickoff et coll. (1968b).These theorists inspired the philosophies of science of Popper and of Quine respectively, and their conception of the progress of scientific knowledge. The criteria which they used to assess the production of scientific knowledge and its progress of nursing science is exhibited. Based on the conceptions of Popper and Quine, an analysis was made of the work of Fawcett and Risjord to trace their proposals to ensure the development of the discipline, the influence of the two philosophies of science in developing their proposals of disciplinary knowledge, and also to analyze the suggested structure and to estimate their impact on the advancement of nursing. The results clearly show the direct influence of Popper and Quine on developing conceptions of Fawcett and Risjord about the structure of scientific knowledge and its progress. Also, the results show through bibliometric method, the influence, inspired by the vision of Risjord is prevalent in the work of the nursing discipline from that was inspired the proposal of Fawcett. Thus, research based on the middle range theories and on borrowed theories (RBT: research-based theories) are dominant to research based on nursing conceptual models (RBMI). In conclusion, in the first part of the advancement of the science, that is the degree of use of theories, the researches based on middle range theories and borrowed theories from other disciplines (RBT) was more numerous than the conceptual models (RBMI). For the moment, the results do not allow us to conclude about the capacity to use these theories to advance scientific nursing knowledge, nor the progress made in this direction by the examined research. -- Keywords: epistemology, advancement, discipline, philosopher, scientific knowledge, Popper, Quine, Fawcett and Risjord.

Texte intégralThe growing prevalence of chronic diseases and limited access to a primary health care professional represent a concern in Canada and Quebec. To address these problems, health care restructuring has led to expanded roles in nursing, and substantial increases in the different types and number of advanced practice nursing roles. As of 2007, the nurse practitioner (NP) role has emerged in Quebec in response to growing healthcare demands, especially in primary care. However, a limited amount of research in Quebec is focused on the innovation of practice models where nurses, including the registered nurse role and the nurse primary health care nurse practitioner (PHCNP) role, are the first contact for patients. In this paper, a case study will be presented based on the Nursing Role Effectiveness Model (Irvine, Sidani, & Hall, 1998a, 1998b) to explore the process of PHCNP role within a multidisciplinary care model and to identify NP-specific health outcomes for patients with chronic diseases. Semi-structured interviews were conducted on 14 key informants (10 patients and 4 PHCNP) to explore their perceptions of the NP practice in chronic disease management and follow-up. Observations and medical record review allowed information triangulation. Results showed that PHCNP emphasize health promotion and provide comprehensive, accessible, coordinated and quality family healthcare service for patients with chronic diseases. The discussion presents strategies to address barriers related to scope of practice as well as organizational issues affecting continuity of NP care. A better understanding of the added value of the PHCNP role will certainly contribute to optimize efficiency and better health and wellness outcomes for patients in primary care.

Texte intégralThe presence of various sensory stimuli in the neonatal intensive care environment is a major determinant in the development of preterm infants. The impact of excess noise and light on the short, medium- and long-term development of the premature infant is documented by studies. Measures to optimize the care environment are implemented in neonatal units in the West and in other parts of the world, but they are not in Cameroon. However, this problem is more important in these units that the infant mortality rate is considerable. The research involved developing and implementing a nursing intervention protocol aimed at reducing noise and light in two neonatal units in Cameroon and assessing their acceptability and feasibility. To do this, the Sidani and Braden method taking place in four phases and using a mixed estimate was used. Three approaches to data collection are proposed by this method, namely experiential, empirical and theoretical approaches. Levine’s conservation care model and Mefford’s (2004) premature health promotion theory were used to situate the problems experienced by preterm infants from a nursing perspective and to suggest nursing interventions. For the first three phases of the study, the objective data were obtained by direct observation of the environment of the neonatal units and a measurement of the sound and light intensities on these units. The experiential data comes from individual and group interviews with nurses (n = 18), doctors (n = 11) and family members (n = 7). The empirical data are from a literature review and the theoretical data are from the study and the theoretical nursing literature. At the end of this stage, needs were identified, and an intervention protocol developed. Phase 4 consists of implementing the intervention protocol in the form of a pilot project. This phase also included the implementation of correctional measures on the two participating units and a set of interventions aimed at changing the behavior of the personnel of these units, and that of families and visitors in favor of protocol interventions. Based on the framework of Michie et al., Training Program and awareness sessions were offered, supported by various means (signs, flyers and reminders). Then, the implementation of the intervention protocol was carried out over a two-week period with the implementation of a cyclical light program on the participating units. The pilot project was carried out through the recruitment of 29 staff members (22 nurses and seven doctors). The results show that the correctional measures were implemented in full on one unit and partially in the other. The results show a high degree of acceptability and feasibility of such a protocol aimed at reducing noise and light on neonatal units in Cameroon. In conclusion, the study showed that local culture, lack of means and financial resources were not an absolute obstacle in terms of feasibility and acceptability and that a research program could better examine the conditions implantation, efficacy and effects on premature babies. Texte intégralHowever, the full learning potential of this program is sometimes limited by the lack of time available in clinical settings for reflection on action. A reflexive practice intervention with nursing externs to mobilize various nursing knowledge has yet been studied. Aim: to explore the contribution of a reflexive practice intervention based on experienced situations of care by nursing externs in the hospital setting to support their learning. Method: descriptive qualitative study with implementation of a reflexive practice intervention with nursing externs (n=14) divided in small groups. Data was collected from the reflexive practice group meetings (n=9), post meeting written perceptions and semi-structured individual interviews (n=14). A qualitative content analysis was performed. Results: reflexive practice allows nursing externs to mobilize several nursing knowledge, namely contextual, practical, moral and ethical, intrapersonal, interpersonal, scientific as well as life experience knowledge. Their mobilisation is made possible by several conditions described as facilitative. These allowed the coconstruction of learning by the nursing externs in the reflexive practice intervention. This coconstructed learning is then reinvested in their professional path, as nursing externs but also in their future nursing practice, in addition to being reinvested at a personal level. Conclusion: reflexive practice intervention with nursing externs is a relevant intervention that allows, with facilitative conditions, knowledge mobilisation, leading to the coconstruction of learning that is reinvested in their practice

Texte intégralEmergency management is one of the principal hospital missions. Since the 90s, hospitals have answered, through the emergency structure, an exponential request for not programmed care, which posed the problem of the dimensioning of the structure and its means. This situation provoked enormous organisational problems. In front of this situation, the hospital decision-makers miss tools witch are indispensable to re-size the emergency resources and to choose the new emergency organization. They shall also allow the planning of the emergency management as well as human and material resources allocation. In response to these needs, this thesis proposes an approach of assistance to the management of emergency systems. This approach tackles three principal problems. At first, we focus our attention on the modelling of emergency processes and its main paths. This modelling, coupled with patients flow simulation, will enable us to analyze the functioning of the emergency structure and to determine the parameters which influences the emergency process. We are interested, secondly, in the nursing staff’s allocation by taking into account their profile of competence evolution using the linear programming. In the third time, we shall study the problem of in hospital vital emergencies management. By means of a process modelling combined with a simulation, we proceed to an approach of conception and evaluation of in hospital survival chain. We tackle, in this last stage, the question of appointment of mobile resuscitation emergency team as well as the positioning of semi automatic defibrillator

Texte intégralBackground: It increasingly appears that the sustainability of health systems depends in part on expanding the care provided by primary care teams. There are many examples of successful primary care models in several countries. However, Quebec generally fails to meet its objectives of accessibility, equity and efficiency. There is growing evidence on the importance of nursing care in primary health services, but it provides little actionable advice on how to reform primary care models given the diversity of clinical settings. The complexity of the interventions needed to reform primary care models is a challenge. Objective: This research reports the results of a realist review on optimizing the contribution of interprofessional and nursing teams in primary health care. Method: An analysis of the literature based on the realist review and the logic analysis was conducted to determine the desirable structural characteristics of health care services models that tend to an extended or optimized contribution of interprofessional teams. Results: The realist review helped to develop two analytical typologies to establish coherent operating bases to improve the performance of models of primary health services. Practice implications : These typologies ultimately aim to facilitate the use of evidence for decision makers to support the necessary transformation processes of primary health care in Quebec.

Texte intégralSeveral nursing authors acknowledge the essential value of the caring relationship and its benefits for the rehabilitation clientele. In spite of this importance, the therapeutic benefits of the caring relationship for this clientele remain an unexamined area. Currently, no Quebec, Canadian or international scientific nursing study is concerned with understanding the experience to « be with » the person being cared for in a rehabilitation context, which is central to the caring relationship according to Watson. Inspired by Watson’s Human Caring philosophy, this study aims to investigate, by qualitative interviews with seventeen rehabilitation nurses, the meaning of the experience to « be with » the person being cared for, as well as their perception of the contribution of this experience to the person’s rehabilitation. Fifty one interviews, therefore three interviews for every research participant, were analyzed using the « phenomenological » method entitled « Relational Caring Inquiry » developed by Cara (1997). The process of participant recruitment involved the nursing management team of both rehabilitation centres targeted by the study. A variety of participants was favoured (e.g.: gender, educational level, shift worked, and patient care unit). The process of data analysis allowed the discovery of five eidos-themes. Among these eidos-themes, four refer to the meaning of the experience to « be with » the person cared for (first research question), namely: (a) the importance of humanistic values at the core of the care, (b) the investment of the nurse and the person cared for, (c) the reciprocal and relational care dimensions and finally, (d) the irreplaceable care experience of contextual complexity. In more detail, the first eidos-theme reveals the humanistic basis of the experience to « be with » the cared person. The second expresses the substantial implication of the nurse and the person cared for. The third eidos-theme clarifies the reciprocity and relational dimension as being central elements of the experience to « be with » the person cared for. The fourth eidos-theme documents fundamental and complex origins of this unique care experience, as well as the contextual conditions which facilitate and impede it. The fifth and last eidos-theme emerging from this study, « strengthening the mind-body-spirit harmony of the person cared for and the nurse », illustrates the participants’ perception regarding the therapeutic contribution of the experience to « be with » the person cared (second research question). This contribution meets the patient’s needs, optimizes the rehabilitation progress of the person cared for, promotes the level of well being of the person cared for and the nurse and finally, raises the inner growth of the people engaged in this extraordinary care experience. The recognition of these five eidos-themes supports the emergence of the universal essence of the phenomenon being studied which is entitled « the profound, therapeutic, and transformative human relationship ». The present study contributes, in an innovative way, to knowledge development, in particular by allowing a better understanding of the meaning of rehabilitation nurses’ experience to « be with » the patient, and advancing evidence-based results to be used as a guide for the development of nursing care in the context of rehabilitation. By determining the ontological meaning of this care experience, the present study makes it possible to specify the place of the phenomenon to « be with » the patient as being central to the caring theory. The results arising from the second research question contribute to the development of a body of knowledge. These evidence-based results will be used to guide the renewal of nursing practice in the context of rehabilitation. Moreover, by identifying the benefits of this care experience, the present study recognizes the element at the heart of transpersonal caring relationship, which contributes to mind-body-spirit harmony for both, the person cared for and the nurse. This study also proposes recommendations for education and research in nursing sciences.
