Patients infected with HIV are now living longer, and there is a growing prevalence of HIV-related and/or non-HIV-related comorbidities, often with an increasing need for chronic disease management beyond the treatment of HIV infection. The barriers to each of the steps in the HIV continuum of care need to be identified, anticipated, and addressed. Comprehensive HIV care requires the coordination of many providers. Successful management of referrals, communication between team members, and the challenges of an aging HIV-infected population are reviewed. The interdisciplinary team approach is essential to effective coordination of care for patients with multiple health and service needs.

This chapter reviews literature on policy networks examining descriptive, theoretical, and prescriptive accounts. It identifies three descriptive uses—policy networks as: interest intermediation, interorganizational analysis, and governance. It identifies two theories about policy networks: power-dependence and rational choice. It reviews three approaches to reforming and managing networks: instrumental, interactive, and institutional. It then discusses the central debates and challenges about comparing networks, explaining change, and managing the institutional void. Finally, the chapter identifies four main problems: the mix of governing structures; the problem of many hands; the holy grail of coordination; and steering not rowing. Policy network analysis has become one more locus for the endless debates about how we know what we know in the social sciences.

How do you craft a market? This chapter reviews some of the key institutions necessary to make markets function and flourish. A modern market economy requires much more than the rule of law and the protection of private property: corporate law, accounting systems, banking regulation, capital market regulation, corporate governance, labor regulation, antitrust policy, sector-specific regulation, intellectual property protection, and the deliberate fabrication of certain markets. These mechanisms structure markets by defining market actors, such as corporations; constructing goods, such as intellectual property rights; establishing market arenas, such as stock exchanges; setting the rules of exchange, such as trading practices; and promoting competition via regulation. In all of the substantive issue cases reviewed in this chapter, government regulation and private-sector coordination are not impediments to markets, but rather preconditions to their creation, expansion, and dynamism.

Optimal management of spinal epidural abscess requires early intervention and coordination with a multidisciplinary team, including emergency medicine physicians, infectious disease specialists, radiologists, neurosurgeons, orthopedists, internists, and hospitalists. This chapter reviews the epidemiology, microbiology, pathogenesis, clinical features, diagnosis, treatment, and outcome of spinal epidural abscess.

The core executive is a new concept replacing the conventional debate about the power of the prime minister and the Cabinet. It refers to all those organizations and procedures that coordinate central government policies, and act as final arbiters of conflict between different parts of the government machine. In brief, the ‘core executive’ is the heart of the machine. The chapter reviews the several approaches to studying the British executive: prime ministerial government; prime ministerial cliques; Cabinet government; ministerial government; segmented decision-making; and bureaucratic coordination. It then discusses several ways forward by developing new theory and methods. The Afterword discusses the core executive as interlocking networks, and the fluctuating patterns of executive politics.

The concept of team effort and delegation/sharing of responsibilities during a medical crisis should be present from the onset. As more responders arrive at the scene, the situation has the potential to become disorganized without consistent communication and leadership. By identifying specific roles and responsibilities, promoting use of good communication skills during crisis and practicing effective team work will help to minimize error. In this chapter, we review the communication and coordination skills required before, during, and after the activation of an RRT.

Chapter 4 introduces the ways in which institutional investors produce investment returns over time and space. In doing so, the chapter considers the 1937 theory of the firm by Coase and reviews the theory’s relevance in today’s environment. It then outlines the three building blocks underpinning the ways in which financial institutions produce investment returns in the context of spatially extensive financial markets: ecology of finance, managers and workers, and coordination. The chapter also demonstrates the distinctive attributes of financial institutions, especially vis-à-vis the power and authority of senior managers in relation to the institution’s goals and objectives. The chapter explores other influential factors, such as the ways in which location, particularly in large urban centres with extensive financial networks, can make a difference.

This chapter reviews, synthesizes, and builds on organizational dependent care support (ODCS) research, resulting in the development of a need-based model of ODCS use. Important unanswered questions remain, including, among others, the meaning to employees of having dependents; determinants of ODCS use and of need for ODCS; differences in need for nurturance, coordination, and financial caregivers in different combinations and profiles; differences in formal and informal ODCS; the mechanisms in play when organizations offer availability, but block, ODCS use; family and community moderators of ODCS use; and explicit consideration of different stakeholders. In the United States, dependent care is generally privatized to families and employing organizations; therefore how families and organizations handle dependent care becomes how U.S. society treats its dependents, making research on this topic vital to helping individuals, families, and organizations manage dependent care with efficiency and compassion.

The ability to regulate emotion promotes mental well-being in health and is disrupted in psychopathologies such as post-traumatic stress disorder (PTSD). The prefrontal cortex (PFC)—a region of the brain involved in executive function, behavioral coordination, and cognitive control—is particularly important in implementing the regulation of emotional response. This chapter reviews a decade and a half of neuroscientific research that has made considerable progress in advancing understanding of the neural basis of emotion regulation. This work, conducted in healthy individuals, provides a platform from which to understand the neural basis of emotion dysregulation that characterizes disorders like PTSD. Therefore, the proposed model could serve as a basis for explaining the etiology and/or maintenance of PTSD. The chapter concludes by summarizing the main findings and highlighting areas that need more work, including translation into the clinical domain.

Local governments frequently network with other local governments or other entities for efficient or effective delivery of local services. Networks enable local governments to discover ways to address externalities and diseconomies of scale produced by political fragmentation, functional interconnection, and uneven distribution of knowledge and resources. Local government networking can be informal or formal and bilateral or multilateral, in the form of deliberative forums or mutual aid agreements. This chapter uses the institutional collective action framework to underscore the link between problems of coordination and credibility of commitment that local governments face as they seek self-organizing solutions and the bridging and bonding networks they create in response to these problems. It then reviews the current state of scholarship in local government networks (LGNs) and shows that much progress has been made in both egocentric and whole LGN studies. Finally, it highlights important areas needing attention to advance LGN scholarship.

Sleep disordered breathing (SDB) may occur in a variety of ways. While obstructive sleep apnea is the most common of these, this chapter reviews the most common types of SDB that occur independently of upper airway obstruction. In many cases, there is concurrent upper airway obstruction and neurological respiratory dysregulation. Thus, along with attempts to correct the underlying etiologies (when present), stabilization of the upper airway is most often combined with flow generators (noninvasive positive pressure ventilation devices) that modulate the inadequate ventilatory pattern. Among these devices, when continuous positive airway pressure (CPAP) alone does not allow correction of SDB, adaptive servo-ventilation (ASV) is increasingly used for non-hypercapnic types of central sleep apnea (CSA), while bilevel PAP in spontaneous-timed mode (BPAP-ST) is more often reserved for hypercapnic CSA/alveolar hypoventilation syndromes. Coordination of care among neurologists, cardiologists, and sleep specialists will often benefit such patients.
