There are 22 million veterans in the U.S. Armed Forces. Past research on the musculoskeletal health of military veterans has explored the prevalence of musculoskeletal disorders (MSDs) but largely avoids situating findings within a theoretical framework. This article uses Pierre Bourdieu’s theory of cultural capital to contextualize veteran’s greater rates of MSDs compared to nonmilitary civilians. Cultural capital consists of objectified, institutional, and embodied capital that can be transubstantiated to capital in other areas. Embodied or physical capital is central to military service, and military veteran status is beneficial in accessing social and institutional capital. Using the 2012–2014 National Health Interview Survey, we show veterans are more likely to report activity-limiting MSDs, and at younger ages, compared to nonveterans. Physical capital is central to, and impaired by, status as a veteran.

Abstract Introduction Military deployments cause stress for both service members and their families. Returning Veterans often report significant trauma exposure, and experience increased stress and mental health problems following deployment. These factors can in turn increase family problems and parenting strain among Veterans who are parents, exacerbating mental health symptoms. Men are generally less likely to seek treatment for mental health problems, and male Veterans, in particular, report lower rates of mental health treatment use. Interventions that target fathering or parenting skills may be more acceptable and less stigmatizing to male Veterans while serving the dual function of improving parental relationships and reducing mental health symptoms. However, it is unclear whether Veteran fathers will engage in these services. Materials and Methods As a preliminary evaluation of the acceptability of fathering interventions, 50 returning Veteran fathers completed an anonymous survey designed to assess their needs and preferences regarding this type of service. All procedures were approved by the local Institutional Review Board and Research and Development Committee. Results Ninety-eight percent of participants reported experiencing at least one parenting issue either that started postdeployment or that got noticeably worse following postdeployment. The majority (86%) stated that they would be open to participating in a fathering program if offered. Conclusions Returning Veteran fathers demonstrate interest in and willingness to participate in fathering programs suggesting that parenting programs may be a way to engage Veterans in mental health care following deployment.

In 2009, the Department of Veterans Affairs (VA) set a goal to end veteran homelessness by 2015. Since then there has been a 36% reduction in homelessness due, in part, to the VA Supportive Housing (HUD-VASH) program. These services include the receipt of home-based services to the veterans’ home. However, safety concerns and the threat of violence toward health care workers remain problematic in non-institutional care settings. This article discusses the concept of access to care and how safety concerns act as a barrier to services and optimal patient outcomes. Our study provides information on the prevalence of patient violence toward health care workers in the HUD-VASH program in a large veterans’ health system. Results suggest 70% of home-based service providers were exposed to violence and aggression. Providing services to veterans outside of institutional care settings, and the goal of eradicating homelessness among veterans, warrants further examination of access barriers.

Abstract Introduction Family members are important supports for veterans with Posttrauamtic Stress Disroder (PTSD), but they often struggle with their own distress and challenges. The Veterans Affairs—Community Reinforcement and Family Training (VA-CRAFT) website was designed to teach family members of veterans with PTSD effective ways to interact with their veterans to encourage initiation of mental health services as well as to care for themselves and improve their relationships. This article presents a pilot investigation of VA-CRAFT. Materials and Method Spouse/partners of veterans who had screened positive for PTSD but were not in mental health treatment were randomized to either use the VA-CRAFT website (n = 22) or to a waitlist control condition (n = 19) for 3 months. Veteran mental health service initiation was assessed posttreatment. Spouse/partner distress, caregiver burden, quality of life, and relationship quality were assessed pre and posttreatment. The study was approved by the Minneapolis VA Health Care System Institutional Review Board (IRB). Results Differences between groups on veteran treatment initiation were small (Phi = 0.17) and not statistically significant. VA-CRAFT participants reported large and statistically significantly greater decreases in overall caregiver burden (η2 = 0.10) and objective caregiver burden (η2 = 0.14) than control participants. Effects were larger for those with greater initial distress. Effects sizes for other partner outcomes were negligible (η2 = 0.01) to medium (η2 = 0.09) and not statistically significant. Postintervention interviews suggested that only 33% of the VA-CRAFT participants talked with their veterans about starting treatment for PTSD during the trial. Conclusion Results from this pilot trial suggest that VA-CRAFT holds initial promise in reducing caregiver burden and as such it could be a useful resource for family members of veterans with PTSD. However, VA-CRAFT does not enhance veteran treatment initiation. It may benefit from enhancements to increase effectiveness and caregiver engagement.

Abstract Introduction Access to high-quality healthcare, including mental healthcare, is a high priority for the Department of Veterans Affairs (VA). Meaningful monitoring of progress will require patient-centered measures of access. To that end, we developed the Perceived Access Inventory focused on access to VA mental health services (PAI-VA). However, VA is purchasing increasing amounts of mental health services from community mental health providers. In this paper, we describe the development of a PAI for users of VA-funded community mental healthcare that incorporates access barriers unique to community care service use and compares the barriers most frequently reported by veterans using community mental health services to those most frequently reported by veterans using VA mental health services. Materials and Methods We conducted mixed qualitative and quantitative interviews with 25 veterans who had experience using community mental health services through the Veterans Choice Program (VCP). We used opt-out invitation letters to recruit veterans from three geographic regions. Data were collected on sociodemographics, rurality, symptom severity, and service satisfaction. Participants also completed two measures of perceived barriers to mental healthcare: the PAI-VA adapted to focus on access to mental healthcare in the community and Hoge’s 13-item measure. This study was reviewed and approved by the VA Central Institutional Review Board. Results Analysis of qualitative interview data identified four topics that were not addressed in the PAI-VA: veterans being billed directly by a VCP mental health provider, lack of care coordination and communication between VCP and VA mental health providers, veterans needing to travel to a VA facility to have VCP provider prescriptions filled, and delays in VCP re-authorization. To develop a PAI for community-care users, we created items corresponding to each of the four community-care-specific topics and added them to the 43-item PAI-VA. When we compared the 10 most frequently endorsed barriers to mental healthcare in this study sample to the ten most frequently endorsed by a separate sample of current VA mental healthcare users, six items were common to both groups. The four items unique to community-care were: long waits for the first mental health appointment, lack of awareness of available mental health services, short appointments, and providers’ lack of knowledge of military culture. Conclusions Four new barriers specific to veteran access to community mental healthcare were identified. These barriers, which were largely administrative rather than arising from the clinical encounter itself, were included in the PAI for community care. Study strengths include capturing access barriers from the veteran experience across three geographic regions. Weaknesses include the relatively small number of participants and data collection from an early stage of Veteran Choice Program implementation. As VA expands its coverage of community-based mental healthcare, being able to assess the success of the initiative from the perspective of program users becomes increasingly important. The 47-item PAI for community care offers a useful tool to identify barriers experienced by veterans in accessing mental healthcare in the community, overall and in specific settings, as well as to track the impact of interventions to improve access to mental healthcare.

Abstract Introduction Among US veterans, posttraumatic stress disorder (PTSD) is a high-prevalence condition; more than a million veterans of all combat eras are service-connected for this condition. Research on factors driving veterans’ decision-making regarding preferred setting for PTSD care has been limited. The purpose of this study was to understand factors associated with preferences for setting of future PTSD care among veterans service-connected for PTSD. Materials and Methods We conducted a cross-sectional mailed survey among veterans with service connection for PTSD in Texas and Vermont identified from the Veterans Services Network Corporate Mini Master File (VETSNET). Survey items were intended to elucidate PTSD healthcare decision-making and queried utilization, perceived need for care, and satisfaction and preferences for VA, community, and/or military setting for receipt of future PTSD care. Logistic regression was used to identify factors associated with preference for care setting. UT Health San Antonio’s Institutional Review Board determined this quality improvement project to be non-research. Results Veterans (n = 2,327) were surveyed with an overall response rate of 37.1%. Most veterans (72.4%) identified VA as a preferred site for their future PTSD care; a substantial, but smaller, number of veterans identified being interested in receiving care in community (39.9%) and military (12.7%) settings. Factors associated with preferences for future care setting included demographics (e.g., ethnicity, income), availability of healthcare coverage, prior experiences of care, and attitudes related to perceived quality of care and trust in VA. Conclusions These findings suggest that it is important to retain foundational mental health services within VA, as well as to continue to invest in building provider and network capacity in community settings, to ensure alignment with veteran preferences for care setting.

Introduction:There are many health challenges faced by those returning from military service. Posttraumatic stress disorder (PTSD) is a serious problem in veterans. PTSD is a risk factor for suicide in veterans. Standard treatments include medication and talk therapy. Non-traditional treatments include civil service and leadership training.Aim:Assess the effectiveness of Veteran Focused Train-the-Trainer (TTT) Community Disaster Response and First Aid (DRAFA) Programs in promoting health, wellness, reintegration, and decreasing PTSD symptoms of veterans.Methods:A longitudinal cohort study was conducted using a convenience sample of veterans living in Denver, Colorado or Reno, Nevada. The sample size was over 50 (N=50+), with 25+ case-matched veterans at each location. This is an ongoing project lasting through the end of 2020. Inclusion criteria selected veterans interested in DRAFA training and education. Exclusion criteria disqualified those who are not a veteran or those unable to perform physical tasks required by curriculum. The null hypothesis was that there is no relationship between the DRAFA TTT program and the health, well-being, and reintegration of veterans back into their communities. Statistical tools used were SPSS Statistics (Version 25) and NVivo 12-12.2.0.3262. Research activities were conducted under the auspices of the University of Colorado and guided by the principles of the Institutional Review Board (IRB).Results:Results are being evaluated using a mixed methods impact model. The main outcomes measured health, wellness, and reintegration using Veterans RAND-12 Health Quality of Life Survey, the Military to Civilian Reintegration Survey, and a satisfaction survey. Preliminary analysis may indicate a correlation between participation in the DRAFA TTT program and improved health/wellness outcomes, better reintegration into society, and decreased PTSD.Discussion:There is growing evidence that expedited structured reintegration programs in community preparedness and disaster leadership roles for veterans alleviate PTSD symptoms and improves quality of life.

Responding to identified needs for increased veterans’ access to healthcare, in 2010 the United States Department of Veterans Affairs (VA) launched the Veteran Community Partnership (VCP) initiative to “foster seamless access to, and transitions among, the full continuum of non-institutional extended care and support services in VA and the community”. This initiative represents an important effort by VA to promote collaboration with a broad range of community organizations as equal partners in the service of veteran needs. The purpose of the study is an initial assessment of the VCP program. Focus group interviews conducted in six sites in 2015 included 53 representatives of the local VA and community organizations involved with rural and urban VCPs across the US. Interview topics included the experiences and practices of VCP members, perceived benefits and challenges, and the characteristics and dynamics of rural and urban areas served by VCPs. Using a community-oriented conceptual framework, the analyses address VCP processes and preliminary outcomes, including VCP goals and activities, and VCP members’ perceptions of their efforts, benefits, challenges, and achievements. The results indicate largely positive perceptions of the VCP initiative and its early outcomes by both community and VA participants. Benefits and challenges vary by rural-urban community context and include resource limitations and the potential for VA dominance of other VCP partners. Although all VCPs identified significant benefits and challenges, time and resource constraints and local organizational dynamics varied by rural and urban context. Significant investments in VCPs will be required to increase their impacts.

AbstractHospital-based “firms” provide a means for combatting the fragmentation experienced by both patients and caregivers in the modern teaching hospital environment. A “firm” is an academic group practice that includes attending physicians, physician trainees, nurses, other staff, and patients. Each person's relationship with a firm lasts throughout his or her association with a particular institution. This article describes the firm system that was recently implemented on the Medical Service of the Cleveland VAMC. This system incorporates both inpatient and outpatient general medical services and provides for unbiased assignment of patients, physicians, and nurses.

There is a higher rate of recidivism for U.S. veterans compared to the general population of offenders. To address the unique needs of veterans, separate housing units for veterans (VSUs) are now operating within correctional facilities in 29 U.S. states. Despite reports that VSUs are having a positive impact on recidivism, little is known of the experiences of correctional administrators who have implemented a VSU. The purpose of this qualitative phenomenological study was to explore the lived experiences of several individuals who have implemented a VSU in their correctional facility. Guided by the quality implementation framework (QIF), data collected through semistructured interviews conducted with 7 U.S. correctional administrators were analyzed by reducing the information to significant statements, when combined into themes provided a descriptive analysis. Results from this study affirm that implementing a VSU is a feasible option for many correctional administrators with the desire to address the needs of veteran offenders. Key findings indicate most steps taken to implement a VSU align with quality implementation. Additional results indicate that presently there may be less consideration for VSU implementation processes associated with quality in the areas of ensuring staff training to work with the veterans, and in conducting process evaluations including outcomes tracking. VSUs have a profound and nearly immediate, effect on veteran inmate behaviors and reducing recidivism. This examination of the phenomenon of VSU implementation may offer implementers with evidenced-based practices to advance understanding of VSU implementation in the future, ultimately to benefit veteran offenders and the communities in which they reintegrate.

The federal government has been inconsistent in meeting the mandated contracting set-aside goals of 3% to aid service-disabled veteran small business owners. Guided by the general systems theory, the purpose of this qualitative exploratory case study was to explore what strategies an owner and 2 senior managers of a small service-disabled veteran-owned business in the Washington, DC metropolitan used to obtain federal contracts. The owner and senior members represented those responsible for strategic vision, development of business opportunities, and the company acquisition process. Data were collected from semistructured face-to-face interviews and corporate documents. Member checking and transcript review were completed to strengthen trustworthiness of interpretations of the participants' responses. Based on the methodological triangulation of the data sources collected, 6 themes emerged from the data analysis as key strategies to obtain federal contracts: business practices, business development of opportunities to attain growth, marketing to obtain opportunities to bid, networking to gain industry knowledge on trends and markets, understanding federal government contracts which emphasized the federal acquisition system access and availability of procurement information, and contract bidding which emphasized the importance of solution development. The findings from the study may contribute to social change by providing insights and strategies for service-disabled veteran small business owners in sustaining profitability through obtaining government contracts. The data from this study may contribute to the prosperity of the veteran small business owners, their employees, their families, and local communities.

Access to federal contracts is often a challenge for service-disabled, veteran-owned business (SDVOB) leaders because of business size and competition in the environment. The purpose of this qualitative, multiple case study was to explore the strategies that 5 SDVOB leaders from 5 different businesses in the Northeastern United States used to win federal contracts. Porter's generic strategies for competitive advantage was the conceptual framework for the study. Five company leaders who won $1 million or more in federal contracts were contacted from the Vendor Information Pages database of the Department of Veterans Affairs to participate in the study. Data were collected via semistructured interviews and archival documents. Data analysis consisted of compiling the data, coding for emergent and a priori codes, disassembling the data into common codes, reassembling the data into themes, interpreting the meaning, and reporting the themes (strategies). Eight themes regarding winning federal contracts emerged. The eight themes were process improvement/optimization, understanding requirements, preventing trial and error, personalizing services, understanding the client, access to external capital/resources, understanding the procurement process, and forward-planning. SDVOB leaders may use the results of this study to secure larger contracts in less time by adopting successful strategies that have won federal contracts. Positive social change implications include the potential for further empowerment, success, and profitability of SDVOBs, as well as other minority-owned firms. Further success of SDVOBs may provide long-term employment and increased tax revenue for communities.

Increasingly more service members are separating from the military as the United States draws down the force and moves towards a post-war era. Tens of thousands of these veterans will leverage their GI Bill tuition and housing benefits in an attempt to access Southern California community colleges and bolster their transition into mainstream society. Some of these men and women have served multiple tours in combat zones and carry with them the burden of physical and mental injuries. After 4 years of GI Bill benefits costing over $20 billion and supporting 750,000 veterans or their dependents, it is unclear whether these veterans have been successful in higher education. This study starts the dialogue for further quantitative research on this diverse population.

Using quantitative methodologies based on a nontraditional student attrition model, 261 student veterans at three Southern California community colleges were surveyed to determine how military service affects their persistence. Background variables, academic and social integration, psychological outcomes, and military service perspective as related to student veterans' intent to persist, college GPA, and rate of attendance formed the framework for research. Findings show that student veterans' intent to continue their educational goals is affected by their military service perspective. Military service is a significant intervening variable that distinguishes veterans from other nontraditional students at community colleges. Military GPA was found to have a stronger association with college GPA than high school GPA. Although academic integration was the strongest predictor of college GPA, the most successful student veterans balance their work, family, and academic lives. Although some findings relate closely to existing higher education literature, the experience and perspective of military service produces both advantages and disadvantages for veterans in college. Accounting for veterans' persistence and acknowledging their diversity are measures which will strengthen their chances for academic success. Recommendations for policy, practice, and future research are addressed in this study.

Researchers examining issues of recent veterans of military conflicts in Iraq and Afghanistan have found evidence of psychological distress and difficulty adjusting to civilian life post-deployment. Changes in educational benefits offered to veterans will likely facilitate greater numbers of veterans to enter academia in the coming years. This survey of 49 college student veterans at a large Midwestern university was designed to reveal the frequency and intensity of psychological distress, PTSD and alcohol misuse. Relationships between these constructs and the constructs of social support, stigma and barriers to psychological care, life satisfaction, traumatic experiences, attitudes toward help seeking, and service utilization are reported. A history of mental health service utilization since military service was reported by many respondents. PTSD and alcohol misuse symptoms were reported by many college student veterans and incidence in this sample was greater than the incidence previously reported in the literature on veterans. Stigma was found to be negatively correlated with attitudes toward help seeking. Post deployment social support was found to be negatively associated with psychological distress and positively associated with service utilization. Implications and limitations of these findings are presented and future directions for research and intervention are discussed.

There is an established link between posttraumatic stress disorder (PTSD) and criminal activity. Of every 100 veterans who served during Operation Enduring Freedom (OEF)/Operation Iraqi Freedom (OIF), 11 to 20 percent are diagnosed with PTSD each year. Previous research has documented that veterans are incarcerated at higher percentages compared to nonveteran inmates, though little published research examines incidence of PTSD relative to violent crime. Using Agnew's general strain theory as the foundation, the purpose of this correlational study was to examine combat service, PTSD, and specific violent crimes as defined by the FBI as murder and nonnegligent manslaughter, forcible rape, robbery, and aggravated assaults; while controlling for branch of service, age, and sex. The sample consisted of 46 OEF/OIF inmates (federal and state) 9 of whom were convicted of a violent crime. Results from the multiple logistic regression analysis showed that combat service, PTSD, sex, and branch of service were not statistically significant predictors of imprisonment of OEF/OIF veterans for violent crime. Age, however, was statistically significant p = .029. The findings of this study contribute to social change by providing policymakers and prison administrators nuanced information (i.e., characteristic information - age, sex, branch of service, etc.) about the needs of this unique prison population with regards to reintegration. This may in turn contribute to improved reintegration initiatives to enrich the lives of veterans, their families, and the communities where veterans reside when returning from war. Focusing on reintegration will be very beneficial.

Afin de préparer la guérison, les pouvoirs publics ont réaffirmé la nécessité d'ouvrir les services hospitaliers à l'extérieur pour tenir compte des besoins spécifiques des enfants. Ainsi, des enseignants s'occupent de leur instruction, des éducateurs de jeunes enfants veillent à leur distraction et les parents assurent une présence continue. Tous ces acteurs participent à la matérialisation d'un espace de prise en charge réunissant diverses institutions ; rassemblent qui suppose une cohabitation entre la famille, l'école et l'hôpital. A quelles conditions cette collaboration institutionnelle peut-elle s'effectuer ? L'organisation des soins est-elle infléchie par al présence de ces nouveaux acteurs ? Ce fonctionnement pluri-institutionnel signe-t'il une moindre emprise de l'hôpital sur les malades ? En analysant la nature de la prise en charge hospitalière des enfants atteints de cancer, cette recherche étudie les effets durables d'une telle expérience sur leur construction identitaire.

Veteran-owned small businesses that possess ratings as service-disabled companies (i.e., service-disabled, veteran-owned small businesses [SDVOSB] are 1 of 5 socioeconomic small business contracting goals that U.S. government agencies seek to comply with U.S. Small Business Administration (SBA) set-aside goals. Little is known about collaborative strategies for improving training regarding single point of entry into Department of Defense (DOD) subcontracting. The research questions in this study focus on how veteran-owned small businesses pursue acquisition opportunities from training opportunities. Sabatier's advocacy coalition theory was used as the theoretical foundation for this study. This qualitative study was employed using 6 semistructured interviews of small business owners who possessed SDVOSB status and a content analysis of training policies from corporate supplier diversity offices, procurement assistance centers, small business development centers, and DOD Office of Small Business programs from a midwestern city. The emergent themes were (a) availability of training from procurement support assistance agencies is of limited value to mature small businesses; (b) additional support and training are needed for subcontractors desiring to enter the DOD subcontracting market; (c) significant obstacles are present in gaining access to federal subcontracting opportunities, including complex acquisition strategies and selection of contract type; and (d) understanding and enforcement of SDVOSB regulations were perceived as insufficient. SDVOSB entities can use these findings to comprehend what questions to ask about subcontracting training.

At least one-quarter of service members who have returned from combat in Iraq and Afghanistan meet the criteria for a mental health diagnosis, of which Posttraumatic Stress Disorder (PTSD) is the most common. Social support provided by close relationships has been shown to be a buffer against PTSD symptoms. However, PTSD can also have devastating effects on couple relationships, hampering this form of social support. One promising intervention for PTSD has been the use of service dogs specially trained to perform tasks related to PTSD symptoms. Anecdotally, there are promising individual outcomes for veterans with PTSD who are partnered with service dogs; however, the effects of these service dogs on the couple relationship for veterans who are married or in long-term relationships has yet to be explored. Seven couples participated in in-depth, semi-structured interviews related to their experiences of their relationship before, during, and after acquiring a service dog trained to respond to PTSD symptoms. Responses were analyzed using interpretative phenomenological analysis. Themes derived were loss of self, living with the enemy, to hope or not to hope, running the gauntlet, pawsitive reinforcements, and turning the tide. Results are weighed against existing literature in the field, clinical and public policy considerations are offered, and directions for future research are proposed.

The research conducted by lecturers and students at a university has a strong correlation with the quality of the institution. Unfortunately, the culture of writing and researching is still far from expectation. Meanwhile, some departments in the institution still manually collected the articles from the lecturer or researchers that takes big effort and time to produce a single volume of the journal. This research was conducted to improve research culture in the Petroleum Engineering Department by optimizing a well-known online submission system called Open Journal System (OJS) that has been used in several departments. OJS is configured and optimized based on lessons learned from existing journals so that it could ease the lecturers to submit and the reviewers to examine the articles. The methodology in this research includes identifying existing problems and constraints, creating questionnaires that were distributed to lecturers and students, analyzing the results of the questionnaire and the obstacles, developing and introducing this system to students and lecturers. From the questionnaire result, some problems were identified and subjected to be solved. At the end of the research, the lecturers gave good feedback to the online journal system for its simple view and easiness of publication.

During the Covid-19 pandemic, almost all universities conducted distance learning. Student responses to the implementation of online lectures need attention. Research on e-service quality on e-satisfaction in the context of pursuit is still very limited. The purpose of this study was to examine the effect of e-service quality on e-satisfaction in online learning during the Covid-19 pandemic. The study was conducted on 1,212 students who had participated in online learning by distributing online questionnaires. The analysis tools used are SPSS and WarpPLS. The results of this study indicate that e-service quality has a significant positive effect on e-satisfaction. This finding is useful for universities as evaluation material in order to improve the online learning process.
