This book examines in detail the legal implications of cloud computing. In essence, 'the cloud' is a way of delivering computing resources as a utility service via the Internet. It is evolving very rapidly with substantial investments being made in infrastructure, platforms, and applications, all delivered 'as a service'. The demand for cloud resources is enormous, driven by such developments as the deployment on a vast scale of mobile apps and the rapid emergence of 'Big Data'. The book explains what cloud computing is and how it works; analyses contractual relationships between cloud service providers and their customers, as well as the complex roles of intermediaries; and explores specific arrangements for public sector cloud procurement and questions about ownership of data in clouds. It also studies the protection of personal data in clouds, governance challenges relating to access to data in clouds by law enforcement authorities, ways of facilitating competition between cloud service providers, and the consumer protection implications of cloud computing.

Telemedicine overcomes barriers to the delivery of health care services including, distance, cost, gaps in distribution and availability of providers. Anesthesia and surgery applications include remote robotic anesthesia and surgery, preoperative assessment at a distance, physiologic monitoring, remote mentoring, and comprehensive critical care services. The Patient Protection and Affordable Care Act and other legislation supports expansion of telemedicine by enabling telemedicine regulation and reimbursement. Tele-Anesthesia applications are expanding and future economically viable telehealth programs will address a variety of anesthesia specific challenges and domains including pain management, postoperative assessment, just in time training for new clinical applications and anesthesia procedures. Telemedicine techniques include synchronous and asynchronous interactions, virtual presence, mobile-health, and haptics enabled robotics, which are used alone or in combination. Focus areas for development of efficient and effective new Tele-Anesthesia programs include chronic care, remote emergency care, and disaster support. Provider acceptance in tele-anesthesia requires education and training to achieve the full advantages of telemedicine.

India, with its large rural population and limited mental health resources, must identify alternative strategies to deliver services. Telepsychiatry offers a pragmatic solution to redistribute sparse resources concentrated in urban pockets. Information available about telepsychiatry practice in India is largely anecdotal with limited literature available in the public domain with regard to technology used, processes undertaken, utilization patterns, or outcomes. The experience of the Schizophrenia Research Foundation (SCARF) in running a telepsychiatry network under its SCARF Telepsychiatry in Pudukottai (STEP) program is described in this chapter. This includes an account of the background to this initiative, description of the program methodology, the technology used, and the required infrastructure to establish mobile telepsychiatry clinics and fixed-line clinics in rural communities. The reach of the program, its coverage, major achievements, and challenges faced are described in detail.
