Résumé :
BACKGROUND: Although the effectiveness of communication between nurses and ventilated patients has been identified by the American Association of Critical-Care Nurses as an area of concern, there are few reports of research in this area. OBJECTIVES: To examine the interactions between nurses and patients on ventilators and the relationship between characteristics of these nurses and their communication with patients. METHODS: An analytical, cross-sectional, experimental design was used to examine the work experience of 30 nurses with ventilated patients, the perceived level of consciousness of their ventilated patients, and the actions and reactions of nurses in relation to these patients. RESULTS: Significant correlations were found between the nurse's perception of the patient's degree of responsiveness and the number of positive and negative interactions with the patient, and between the length of time the nurse cared for the patient and the number of positive nurse reactions. Interaction patterns are also apparent from the data collected. CONCLUSIONS: Findings suggest that nurses' perceptions of patients' responsiveness and length of time nurses care for patients will influence nurse-patient interactions. Patterns of interaction suggest that nurses spend more time providing patients with information that the nurses consider important, rather than assessing or responding to patients' needs.

Résumé :
Background There is an increasing need for the voice of children and young people to be more evident in planning and designing services, including the delivery of higher education, for school nurses to ultimately influence service delivery. Aim To raise awareness of the importance of children's rights by embedding them in postgraduate school nurse education to influence school nurse service delivery. Methods A model was developed through direct partnership with representatives of the Children's Parliament in Scotland. This model facilitated the inclusion of children and young people's opinions in the co-design of postgraduate nurse education in order to incorporate children and young people's rights and opinions and improve the delivery of the school nurse service in Aberdeen. Results In a supported environment, children and young people were able to negotiate relationships with multiagency professionals on an equal basis, building connections and trust with them in partnership, based on their perceptions of school nurses' daily actions and characteristics. Their negotiation of relationships identified various change management categories for improving the delivery of higher education and services. Conclusions Children and young people have clear opinions and views when consulted in an inclusive age-appropriate way through rights-based participation. It is important that listening and taking due cognisance of children's voices becomes the norm in influencing higher education, and that children and young people have a direct influence on improving children's services.

Résumé :
Background: The process to obtain valid informed consent in healthcare reflects many aspects. Healthcare professionals that take care of the patient must provide him all the necessary information and verify his understanding, considering individual characteristics. Nurses are one of the main participants in this process. Objective: This study assesses nurses’ perceptions of their role in the informed consent process. Material and Methods: An observational study involving 300 nurses operating in 13 wards of the Padua Hospital, through the submitting of a questionnaire in the period November–December 2018. Results: The final sample is made up of 206 nurses—27 males (13.11%) and 179 females (86.89%). Work experience, on average 15 years, is significant in determining the answers to questions about opinions and experiences. Age is significant in determining how often nurses provide information to the patient’s family members about the actions to be taken after discharge. The ward was decisive in the responses related to information provided to patients on the nursing care level and the actions to be taken after discharge, and the definition of the nurse’s duties. Conclusions: The data collected show the need for interventions to reduce the causes of difficult that the nurse has in informing patients.

Résumé :
Objective: Clinical judgement has long been an important component of safe and effective nursing care. However, despite extensive research being conducted in this area it remains difficult to articulate and observe clinical judgement in action. Nursing actions could reflect the characteristics of clinical judgement, yet this alignment has not previously been reported in the literature.Aim: The aim of the study was to identify whether the nursing actions performed could be aligned with the characteristics of clinical judgement. Methods: The setting for this study was a simulation laboratory housing a high-fidelity manikin in a large Australian university. The study used a descriptive method collecting both qualitative and quantitative data. Data was collected using a Multiple-Choice Questionnaire (MCQ), a checklist of nursing actions and associated characteristics of clinical judgement and post simulation interviews were audio recorded. Results: The nursing actions could be clearly aligned the 12 characteristics of clinical judgement. In this study it was difficult to differentiate between two of the characteristics: experiential knowledge and practical knowledge. Conclusions: Aligning the nursing actions to the characteristics of clinical judgement could assist in educating students to develop their clinical judgement. The use of aligning nursing actions to the characteristics could be a more accurate way of observing clinical judgement in action.

Résumé :
The psychiatric reform that took place in Brazil carries characteristics of other movements that occurred in other parts of the world. The idea common to all movements is the struggle for the rights of the individual in mental suffering, seeking mainly the rupture of the mental model. These changes led to several transformations in the care scenario, for all professions directly linked to the patient. Nursing in turn has experienced and experiences significant changes in the provision of care. The aim of this study is to talk about nursing care for patients affected by mental disorder, making a temporal analysis of how this care occurred and how it presents itself in the current mental health conjuncture. The methodology is of the literature review type, which occurred through research in the databases BIREME, Lilacs, Scielo, BDENF and VHL. For this, the descriptors: nursing care for people with disorders were selected; nursing care for patients with mental disorders. In view of the results, it was evidenced that nurses are an important part of caring for patients with mental disorders, noting that these make up a multidisciplinary team and highlighting that care goes far beyond just caring for the patient, but that it consists mainly in the relationship with the patient's family, in bonding, in the work that aims at social reintegration and often also the family reinsertion of the individual. Profession that needs to undergo constant updates, but has experienced numerous transformations throughout this period of Reformation.

Résumé :
ObjectivesTo determine whether there was variation in nurse staffing across hospitals in Queensland prior to implementation of nurse-to-patient ratio legislation targeting medical-surgical wards, and if so, the extent to which nurse staffing variation was associated with poor outcomes for patients and nurses.DesignAnalysis of cross-sectional data derived from nurse surveys linked with admitted patient outcomes data.SettingPublic hospitals in Queensland.Participants4372 medical-surgical nurses and 146 456 patients in 68 public hospitals.Main outcome measures30-day mortality, quality and safety indicators, nurse outcomes including emotional exhaustion and job dissatisfaction.ResultsMedical-surgical nurse-to-patient ratios before implementation of ratio legislation varied significantly across hospitals (mean 5.52 patients per nurse; SD=2.03). After accounting for patient characteristics and hospital size, each additional patient per nurse was associated with 12% higher odds of 30-day mortality (OR=1.12; 95% CI 1.01 to 1.26). Each additional patient per nurse was associated with poorer outcomes for nurses including 15% higher odds of emotional exhaustion (OR=1.15; 95% CI 1.07 to 1.23) and 14% higher odds of job dissatisfaction (OR=1.14; 95% CI 1.02 to 1.28), as well as higher odds of concerns about quality of care (OR=1.12; 95% CI 1.01 to 1.25) and patient safety (OR=1.32; 95% CI 1.11 to 1.57).ConclusionsBefore ratios were implemented, nurse staffing varied considerably across Queensland hospital medical-surgical wards and higher nurse workloads were associated with patient mortality, low quality of care, nurse emotional exhaustion and job dissatisfaction. The considerable variation across hospitals and the link with outcomes suggests that taking action to improve staffing levels was prudent.

Résumé :
BACKGROUND: Critically ill patients have received little attention in the caring literature and yet are a population for whom caring behaviors are particularly important. OBJECTIVES: To describe patients' perceptions of caring exhibited by professional nurses in a critical care unit and to describe the meaning to the patients of these demonstrations of caring. METHODS: We used a phenomenologic approach for this descriptive study, which was conducted on 13 patients hospitalized in a critical care unit for at least 48 hours within 48 hours of their transfer from the unit. We asked them to respond to two open-ended questions and recorded all interviews on audio tapes that were transcribed verbatim. RESULTS: Caring in a critical care unit is attentive, vigilant behavior on the part of the nurse. This vigilance embodies nurturance and incorporates highly skilled, technical practices, as well as basic nursing care and beyond. Caring is a healing process of which lifesaving actions by the nurse are a part. Identifying the characteristics of the individuals involved in this healing process was one way of describing caring. Personal attributes of nurses, family members, and patients themselves are important in the descriptions of the caring process. These attributes are incorporated into the concept of mutuality. CONCLUSIONS: Caring is descriptive of a mutual process in which intentions are joined to form a shared experience. In this mutual process, healing is an outcome that might otherwise be elusive.

Résumé :
This study was designed to identify social life characteristics associated with the variance in charges and penalties against chemically dependent registered nurses (RNs) and licensed practical nurses (LPNs) in Arizona. Black's (1976) theory on the behavior of law was used as the basis for the conceptual framework of the study. Data were collected from a sample of 203 chemical dependency misconduct records at the Arizona State Board of Nursing. LPNs were more likely to have licenses revoked than RNs. Nurses holding staff positions were more likely to have licenses revoked than nurses holding administrative, supervisory or head nurse positions. Significant differences were also found between chemically dependent nurse offenders and non-offenders when comparing RNs to LPNs; married nurses to separated/divorced nurses; full-time nurses to part-time nurses; diploma or associate degree prepared nurses to baccalaureate or higher degree prepared nurses and male to female nurses.

Acts of incivility in nursing education and practice settings are a growing concern. Previous research has indicated that uncivil behaviors, especially among nurses in the healthcare workplace, create toxic work environments and pose risks to patient safety. Uncivil behaviors among nurse educators and students were found to erode the learning environment and lead to poor program outcomes. Researchers provided evidence to show varied levels of incivility in academic and workplace environments and recommended solutions to improve civility, yet little evidence exists to show how nurse faculty approach civility promotion in their practices. This qualitative, descriptive study was designed to explore the actions and decisions of prelicensure nurse faculty who promote civility in their baccalaureate programs. Fifteen full-time, prelicensure nurse faculty from varied baccalaureate programs across the southeastern United States volunteered to participate in individual, online interviews. Husted and Husted's theory of bioethical symphonology was used to support the study design and analyze the findings. Thematic analysis of participant interview transcripts revealed 4 themes to explain civility promotion actions and decisions which included (a) guiding civil professionalism, (b) championing civil communication, (c) negotiating civil partnerships, and (d) empowering civility awareness. The findings of this study are beneficial to nurse faculty seeking civility promotion solutions which will enhance awareness, knowledge, and professional civility skills among nursing students and effect positive social change as new graduate nurses are prepared to promote civility in the healthcare workplace.

Résumé :
The purpose of this study was two-fold: to identify role functions and characteristics perceived as important to Advanced Nurse Practitioners (ANPs) in Texas, and to identify differences in perceived importance of role functions and characteristics of ANPs according to gender, educational preparation, practice setting, and whether the practitioner entered practice before or after the current rules and regulations for ANP practice were adopted. Two questionnaires and a demographic data form were mailed to 300 ANPs in primary health care in Texas, with 152 responding. Data from the questionnaire, "Characteristics of the Advanced Nurse Practitioner," were analyzed using independent group t-tests. The findings indicated that the characteristics "person-oriented," "perceptive," and "skilled in problem-solving" were the most valued by the practitioners, while aggressive" was the least valued. Independent group t-tests were used to analyze data from the instrument, "Advanced Nurse Practitioner Functions." The results of this analysis were not significant. The Chi square test was used to further examine data from this questionnaire to determine if the responses differed from chance. With the exception of four items, all of the responses differed significantly from chance. The responses "extremely important or important" were selected significantly more frequently that any of the other options. Not appropriate," was selected significantly more often than could be expected by chance for two functions: suturing minor lacerations and performing incision and drainage of wounds. Fourteen functions were examined further using frequency, percentage of responses, and the Chi square test to determine if there were differences in responses between groups. There were significant differences in responses between ANPs in nurse-managed settings and those in physician-managed settings on four functions: ordering diagnostic tests, prescribing medications in consultation with a physician, suturing minor lacerations, and performing incision and drainage of wounds. Practice setting and educational preparation were the variables which had the greatest impact on perceived importance of the characteristics and functions to ANP practice.

Résumé :
This study's purpose was to describe the practice patterns of nurse practitioners (NPs) in Tennessee--specifically, the demographic characteristics and health problems of their clients and the therapeutic services they provide. A random sample of NPs practicing 20 or more hours per week in primary care in Tennessee provided data on a total of 680 clients seen during one selected day of care. An instrument adapted from the National Ambulatory Medical Care Survey (NAMCS) allowed comparison of the NP findings with a national survey of office-based physicians in five areas: client demographics, client health status, diagnostic tests ordered, therapeutic interventions provided, and client disposition. Although many similarities were seen, differences included the tendency of NPs to care for more younger and female clients, to perform fewer office surgical procedures, and to provide more health teaching/counseling interventions.

Résumé :
AbstractThis chapter places the preceding papers into a wider context. As part of the Innovative Learning Environment and Teacher Change (ILETC) project, seven Transitions symposia were held in five cities across Australasia, Europe and North America during 2017, 2018 and 2019. Each aimed at investigating how teachers adapt to innovative learning environments. The resulting accumulation of approximately 150 papers by graduate researchers and research groups, of which this book’s chapters are a sample, constituted a reasonable representation of international thinking on this topic. When added to three years of ILETC case studies, surveys, systematic literature reviews and teacher workshops, the project team was able to identify consistent patterns in teachers’ spatial transition actions. This chapter places the material of this book within that larger picture, specifically in terms of one project output—the development of a Spatial Transition Pathway. The Pathway emerged from these data and can be seen as an output of the material sampled in previous chapters. Certainly, the considerable work teachers had been doing to re-conceptualise their pedagogies for new spaces (done both intentionally, and at times, without realising) deserved to be mapped as a resource for others undertaking this journey. This chapter makes the case that while each teacher or school’s journey from traditional to ‘innovative’ spaces is unique, there exists some common issues that most seem to face at some time, in some way. It provides a description of fourteen ‘grand themes’ that appear commonly through the data and describes how these can be organised in a way that provides temporal and theme-based strategies and tools, developed by fellow educators to assist in this transition. This final chapter leads the reader to consider ‘where to now’? It celebrates the fact that teachers have enormous capacity to work out how to utilise innovative learning environments well and provides a framework for evidence-based actions into the future.

Résumé :
AbstractThis chapter presents an analysis of students’ concerns, values, engagement, and intended future participation on environmental issues in relation to their home socioeconomic background, gender, and migrant status. Analyzing IEA International Civic and Citizenship Education Study (ICCS) 2016 data of Nordic countries, we first present descriptions of student responses to all questions related to environmental issues and compare Nordic results with European and international averages. Then, we construct a composite score of student environmental citizenship for investigating its relationship with student background factors such as gender, migrant status, and home socioeconomic status through comparing means between student groups with different background characteristics. Lastly, we apply factorial ANOVA analysis method to examine the effect sizes of student background factors and the interactions between them on youth environmental citizenship in the four countries. The results show that there are both similarities and small variations in elements of student environmental citizenship among the Nordic countries and in comparison with their European and international peers. Nordic students stand out as the concerned environmental citizens while they are somehow lower than their European and international peers in engagement, values, and intended participation of environmental citizenship. We find that student environmental citizenship is socially divided in all Nordic countries as it differs significantly between students from different socioeconomic strata and genders. Although not all differences of student environmental citizenship by migrant status are statistically significant among the Nordic countries, we find some significant influence of migrant status interaction with socioeconomic statuses and genders.

Résumé :
<p>Masonry structures are probably the most popular and ancient type of buildings all over the world. Easy access of its constitutive materials, which are basically stones, bricks, and mortar (which varies from region to region), makes masonry one of the everlasting construction methods from small residential buildings to the most important ancient and historic monuments. <p>Some masonry buildings have proved to be resistant structures even in seismic prone areas, due to some specific structural characteristics that have been observed throughout the years and after many destructive earthquakes. In this chapter, an effort will be made to refer to and describe the most characteristic deficiencies in unreinforced and reinforced masonry buildings under seis-mic actions. Design recommendations for new earthquake-resistant structures will follow, and some retrofitting and strengthening strategies for existing masonry buildings will be proposed.

Résumé :
Estimated at 25 percent of Americans, the number of low-income single mothers is on the rise (Loprest & Austin, 2011). Described as the disconnected, these women are needy in many areas. Most have less education, and sometimes, more learning disabilities. Some have problems of substance abuse, depression, mental illnesses, and other physical health problems (Wincup, 2014; Jayakody & Stauffer, 2000; Tolman, Himle, Bybee, Abelson, Hoffman, & Van Etten-Lee, 2015). These characteristics have called for action on the plight of these women as their numbers become a strain on welfare systems leading to reduced benefits for some, with others being denied altogether (Silver, Heneghan, Bauman & Stein, 2006). The chapter presents a blueprint to train these mothers into Licensed Practical Nurses to reduce dependence on public finances and to enhance their self-images (Atkins, 2010). The blueprint explores how these women learn, and the services they need to complete the program.

Résumé :
By the end of this chapter, you should understand… ● The range of possible sources of information about medicines ● Their positive and negative characteristics ● Some trustworthy sources of evidence ● The role of medicines information departments ● Some basic principles of critical analysis of evidence ● How the British National Formulary (BNF) is structured ● How to read a BNF monograph ● A selection of terms used in the literature about medicines…. While nurses will not usually be selecting medicines, they still need information to make the best use of the medicines prescribed for their patients. Information of all kinds is much more readily available today than it was a generation ago when the authors were students, but much of it is of low quality and today’s student must learn to test the quality of the evidence offered to see if it can be relied upon. In all fields of healthcare it has become usual to insist that practice must be evidence based. This is very desirable, but it begs the question—what is evidence? This chapter will examine some of the sources of evidence about medicines that are available and give some guidance on their reliability. Later, there will be an introduction to critical analysis of sources, and a description of some of the key terms used in evaluating clinical evidence about medicines. Sources of information may be conveniently divided into two main types—people and publications. It is natural that many healthcare professionals should rely upon their mentors and instructors to supplement the knowledge they gain in formal teaching. Indeed, for many years much of the practical information about medicines that junior doctors received came from participation in ward rounds under the tutelage of a consultant. In medical school they learned some general pharmacology, but the actions of many drugs were learned following graduation (Maxwell and Walley, 2009 ) The same will be true for nurses, and it will continue to be true throughout their careers. New medicines will come into use, and nurses will have to learn about them. It is therefore important to realize that pharmacology will be a lifelong study and does not end with registration as a nurse.

Résumé :
The diseases and health problems are concerns of managers of the Unified Health System has costs in more sophisticated care sector are high. The World Health Organization focused on prevention of chronic diseases to prevent millions of premature deaths in the coming years, bringing substantial gains in economic growth by improving the quality of life. Few countries appear to be aimed at prevention, if not note the available knowledge and control of chronic diseases and may represent an unnecessary risk to future generations. Early diagnosis of these diseases is the first step to successful treatment in any age group. The objective is to build a model, from the establishment of a Bayesian network, for the early diagnosis of nursing to identify eating disorders bulimia and anorexia nervosa in adolescents, from the characteristics of the DSM-IV and Nursing Diagnoses The need for greater investment in technology in public health actions aims to increase the knowledge of health professionals, especially nurses, contributing to prevention, decision making and early treatment of problems.

Résumé :
Rocket fairings, turbine blades, load carrying structure for solid rocket motor case, inter-stage joint, satellite-rocket joint etc., usually take the shape of conical shell sections. Conical shell has a large load carrying capacity per unit weight due to its high-strength, high-rigidity and light-weight properties. This paper is to evaluate spatially distributed microscopic control characteristics of distributed actuator patches bonded on conical shell surfaces. The converse effect of piezoelectric materials has been recognized as one of the best electromechanical effects for precision distributed control applications. The resultant control forces and micro-control actions induced by the distributed actuators depend on applied voltages, geometrical (e.g., spatial segmentation and shape) and material (i.e., various actuator materials) properties [8]. Mathematical models and modal domain governing equations of the conical shell section laminated with distributed actuator patches are presented first, followed by the formulations of distributed control forces and micro-control actions which can be refined to longitudinal/circumferential membrane and bending control components. Spatially distributed electromechanical microscopic actuation characteristics and control effects resulting from various longitudinal/circumferential actions of actuator patches are then evaluated.

Résumé :
Abstract Action recognition infrastructure can be applied anywhere behavior analysis is required and represents presently a domain of maximum actuality in security and surveillance. The model based on 3D Convolutions is a middle ground between simple key-frame approaches based on 2D convolutions, and other more complex approaches based on Recurrent Neural Networks. Behavior analysis represents a domain greatly improved by action recognition. By placing human actions in different categories it is possible to extract statistics regarding a person’s behavior, characteristics, abilities and preferences which can be processed later by specialized personnel, depending on the selected domain. The proposed model follows simple 3D convolution architecture. Hidden layers are composed of a convolution operation, an activation function and, sometimes, a pooling layer. Leaky ReLU was used as activation function to alleviate the problem of vanishing gradients. Batch Normalization is a technique used for scaling and adjusting the output of an activation layer, and it has been used to reduce over-fitting and decrease the training time. The 3D Convolution structure has the advantage of learning spatio-temporal features, because the convolution is applied over a sequence of frames. In the present paper is presented a proposed 3D convolution model that has average results, with an accuracy of approximately 55% on the NTU RGB+D dataset.

Résumé :
Climate change is a growing concern for forests across Minnesota. Foresters, land managers, and landowners are considering how to prepare for future conditions and how to evaluate risks for particular sites. This field guide is designed as a quick reference on climate change for northern Minnesota forests. The intent is to highlight key information that can be used during field visits or forest planning. We hope that this guide will help foresters consider climate change risks together with local site characteristics, and also that it will help people design adaptation actions that help meet management goals.
