Abstract This article examines the survival strategies of Lutheran dissenters in the eighteenth-century Dutch Cape Colony. The Cape Colony was officially a Reformed settlement during the rule of the Dutch East India Company (V.O.C.) but also had a significant Lutheran community. Until the Lutherans received recognition in 1780, part of the community chose to uphold their faith in secret. The survival of Lutheranism in the Cape Colony was due to the efforts of a group of Cape Lutheran activists and the support network they established with ministers of the Danish-Halle Mission, the Francke Foundations, the Moravian Church and the Lutheran Church in Amsterdam.

- The article's goal is a diagnosis on why utilization of evaluation in Italy encounters critical difficulties. One of the main factors that delay an effective use of evaluation in Italy seems to be the fact that supply doesn't meet a demand for this reason we interviewed experts, who can give their impression about the growing of evaluation use in Italy. Four issues were identified: a cultural problem, a careless attitude toward evaluation; a lacking awareness and interest towards the functions of evaluation among the politicians; the importance of skills and training; the development of an evaluation market. Résumé :
Purpose: The present study aims to report the reoperation of an unsuccessful orthognathic surgery with the use of heterogeneous graft and hybrid rigid internal fixation. Case Report: We report the case of V.M.S., a 19-year-old female patient with the main complaint of “crooked chin.” The patient revealed that she previously underwent two operations that were conducted by a different team. The first surgery was carried out to correct the dentofacial deformity, while the second one was intended to eliminate a postoperative infection. After a detailed anamnesis, facial analysis, cephalometric analysis with tomography, and examination of gypsum models, we observed mandibular left-handedness and vertical maxillary excess on the right side. In addition to these previously reported problems, there was a nonunion in the region of the sagittal fracture of the left jaw. Results: The patient underwent surgery to correct dentofacial deformity using the Le Fort I maxillary osteotomy technique and bilateral sagittal osteotomy with hybrid rigid internal fixation plus mentoplasty. Conclusion: Six months following the procedure and orthodontic completion, the patient has a satisfactory occlusion and stability of nonunion without the laterognathism in the skeletal and soft tissue profile.

In an essay of extraordinary range and depth, which it is difficult to summarise without distortion, Jacob van Leur is above all making an appeal for the autonomy of Asian history in relation to that of Europe. He was reviewing volume IV by Godée Molsbergen of Geschiedenis van Nederlandsch Indië, which dealt with the eighteenth century. To Molsbergen the activities of the V.O.C. in Asia in the eighteenth century had characteristics distinct from those of the seventeenth-century Company or from what was to follow in Indonesia in the nineteenth century. These characteristics essentially reflected those of the Netherlands during the eighteenth century. Assuming that eighteenth-century European history has unifying characteristics (an assumption that he was inclined to question), Van Leur asked: ‘Is it possible to write the history of Indonesia in the eighteenth century as the history of the Company?’ His answer was a resounding ‘no’. In giving his answer he widened the issue from Indonesia to Asia as a whole. ‘A general view of the whole can only lead to the conclusion that any talk of a European Asia in the eighteenth century is out of the question, that a few European centres of power had been consolidated on a very limited scale, that in general – and here the emphasis should lie – the oriental lands continued to form active factors in the course of events as valid entities, militarily, economically and politically.’ He concluded that diere was an ‘unbroken unity’ of Asian history from the seventeenth to the nineteenth century. Until well into the nineteenth century Europe and Asia were ‘two equal civilisations developing separately of each other’.

Résumé :
Bacterial infection is an important trigger of vaso-occlusive crisis (VOC) in sickle cell anaemia (SCA). SCA Patients with VOC have signs of inflammation and it is difficult to diagnose bacterial infection in them. This study was undertaken to evaluate serum procalcitonin (PCT) as a biomarker of bacterial infection in acute sickle cell vaso-occlusive crisis. Hundred SCA patients were studied at Sickle Cell Clinic and Molecular Biology Laboratory, V.S.S. Medical College, Burla, Odisha, India. SCA was diagnosed by haemoglobin electrophoresis, HPLC and molecular analysis. Patients were divided into 3categories namely Category-A (VOC/ACS with fever but without evidence of bacterial infection-66 patients); Category-B (VOC with fever and documentedbacterial infection-24 patients); and Category-C (Patients in steady statewithout VOC/ACS or fever-10 patients). Investigations like complete blood count, C-reactive protein estimation and PCT measurement was done in all the cases. There was no significant difference in total leucocytes count and C-reactiveprotein values between category A and B. In category A the PCT level was <0.5ng/mL in 83.3% and 0.5-2ng/mL in 16.7% of cases. In category B all the cases had PCT value >0.5ng/mL with 87.5% of cases having >2ng/mL. In category C, PCT value was <0.5ng/mL. The PCTvalue differed significantly (p<0.0001) in three categories. PCT had a highsensitivity (100%) and negative predictive value (100%) for bacterial infection at a cut-off value of 0.5ng/mL; whereas the specificity is excellent at a cutoff value of 2ng/mL. SCA patients with VOC/ACS with fever presenting with a PCT level of <0.5ng/mL do not have bacterial infection. In patients with VOC/ACS and fever, PCT value of >2ng/mL is indicative of bacterial infection necessitating antimicrobial therapy. Patients with indeterminate PCT value of0.5-2ng/mL, need a repeat PCT estimation or an empirical antibiotic therapyawaiting the availability of microbiological report as deemed necessary.

Résumé :
Abstract Introduction Cancer-related Venous Thromboembolism (VTE) requires treatment with Low Molecular Weight Heparin (LMWH), which is more effective and safer than warfarin; however, the risk of major hemorrhage still remains clinically relevant (up to 5%). This rate is even higher in case of impaired hemostasis, such as during thrombocytopenia (due to myelosuppression or chemo-therapy) where the bleeding risk is directly related to the platelet count level. At the present, the best management of adult patients with acute or non-acute cancer-related VTE during thrombocytopenia is uncertain. Objective To develop a consensus about the platelet cut-off for a safe LMWH administration in cancer patients with acute (lasting < 1 month) or non-acute VTE and thrombocytopenia, based on RAND/UCLA Appropriateness Method (RAM). Materials and methods A systematic review of the literature was performed via electronic databases (MEDLINE, EMBASE, and Cochrane Library Central Registry). Topics and research terms were: cancer, venous thromboembolism, platelets, risk of bleeding, anticoagulant drugs, low-molecular-weight heparin, and treatments. The main study outcomes were rates of VTE (first event, recurrence, and catheter-related DVT), major and minor bleeding, thrombocytopenia, and death. A panel of experts was identified; the literature review and the list of indications were sent to all members of this panel. For each indication, the panel members rated the benefit-to-harm ratio of the procedure on a scale of 1 to 9, where 1 means that the expected harms greatly outweigh the expected benefits, and 9 means that the expected benefits greatly outweigh the expected harms. A middle rating of 5 means either that the harms and benefits are about equal or that the rater cannot make the judge for the patient described in the indication. The working group comprised 9 hematologists expert in thrombosis and haemostasis (G.S., M.M., M.C., V.D.S., A.F., A.F., F.R., A.T., S.S.) including two methodologists (G.S. and M.M.) and one coordinator (S.S.). Consensus Development the RAND was construed identifying 3 platelet count cut-offs (i.e., PLT >50.000 <100.000 µL; PLT >30.000 <50.000 µL; PLT <30.000 µL) and 3 doses of LMWH (weight adjusted fixed-dose, prophylactic dose, and half of the weight adjusted fixed-dose) by administering 13 questions to the experts. Results The panel of expert reached the following consensus: 1. Cancer patients with acute VTE and a platelet count <100.000 µL - >50.000 should receive full therapeutic dose LMWH ; 2. In cases of non-acute VTE with platelet count <100.000 µL - >50.000, the dose of LMWH should be reduced to 75% of the full dose; 3. In cancer patients with acute VTE and platelet count <50.000 - >30.000 µL, the LMWH dose should be reduced to 50% of the full therapeutic dose; 4. In case of non-acute VTE and a platelet count <50.000 µL - >30.000, the expert panel considers uncertain a treatment with a reduced dose to 50% or a low dose (i.e. 4.000 IU anti-FXa/d); 5. In case of platelet count below 30.000 µL, the expert panel agreed to suspend LMWH both in acute and non-acute VTE. Conclusions This is the first expert opinion based on RAM to establish the safe platelet cut-off to administer LMWH therapy in cancer patients affected by acute and non-acute VTE. The present panel of experts suggests as appropriate the use of dose-adjusted LMWH according to platelets count. Further investigations by means of well designed prospective clinical trials are needed to establish the best management of cancer-related VTE in patients with thrombocytopenia. Résumé :
Imaging buried geological boundaries is one of a major objective during the interpretation of magnetic field data in Geophysics. Therefore, edge detection and edge enhancement techniques assist a crucial role on this aim. Most of the existing edge detector methods require to obtain special points such as in general the maxima of the resulting image. One of the useful tools in estimating edges from magnetic data is the tilt angle of the analytical signal amplitude due to its value slightly dependence on the direction of magnetization. In this study, the maxima of the tilt angle of analytical signal amplitudes of the magnetic data was determined by a curvature-based method. The technique is based on fitting a quadratic surface over a 3×3 windows of the grid for locating any appropriate critical point that is near the centre of the window. The algorithm is built in Matlab environment. The feasibility of the algorithm is demonstrated in two cases of synthetic data as well as on real magnetic data from Tu Chinh-Vung May area. Résumé :
Based on the data of daily maximum temperature in 26 meteorological stations in the North Center Region, Vietnam over the period of 1980 to 2013, the authors conducted the research on the spatial distribution of the number of hot days. The initial result shows that in general, in the north of the study area, the large number of hot days occurred in the plain, and tended to decrease westward and eastward. In the south, this number tends to increase from the west to the east. Especially, the largest number occurred in two areas: The Ma and Ca River's valleys (Thanh Hoa and Nghe An provinces) and the coastal areas (Thua Thien Hue province), creating two heat centers in Tuong Duong district, Nghe An province and Nam Dong district, Thua Thien Hue province.ReferencesAdina-Eliza Croitoru, Adrian Piticar, Antoniu-Flavius Ciupertea, Cristina FlorinaRosca, 2016 Changes in heat wave indices in Romania over the period 1961-2015. Résumé :
During the 20th century, population growth and changes in the business environment also led to a change in mobility requirements. Gradually, there was an increase in transport performance in the area of passenger and freight transport. These trends are also characteristic of the 21st century, when it is also possible to talk about the fundamental development of modern information and communication technologies. The sustainable and green city logistics deals with all transport (passenger and freight) and includes material and goods flows and movements of people inside and outside the city and agglomeration with respect to the sustainability pillars. This article deals with the possibilities of supplying Lidl Česká republika v.o.s. (hereinafter Lidl) stores in Prague agglomeration from the planned logistic centre in Buštěhrad. Two scenarios are tested in terms of total carbon dioxide emissions produced: standard way of distributing goods to stores (scenario A) and sustainable and green way of distributing goods to stores (scenario B). The scientific methods and approaches are used in this article, there are: scenario analysis, vehicle routing problem with pickup and delivery with time windows and carbon dioxide emissions calculation approaches.

Résumé :
Aim: The aim of this study was to establish the validity and reliability of the Tswanatranslations of three pain scales.Design: This was a cross–sectional study to validate and test the reliability of threepain scales.Participants: One hundred subjects participated in the study. They were selectedfrom the back schools of five hospitals in the North -West Province of South A frica andfrom workers in these hospitals who were employed as kitchen workers, laundryworkers and cleaners.Method: Translation of the pain scales and the stages of cross-cultural adaptation were followed as recommended byBeaton et al (2000). Pain tolerance of the subjects was measured using a P.T.M. (pressure threshold meter). The painscales used were the V.A .S. (visual analogue scale) one (nought and ten only), the V.A .S. (visual analogue scale) two(nought through to ten), the W.B.F. (Wong-Baker Faces pain measure) and the V.R.S. (verbal rating scale). The V.R.S.used came in two forms. The first form was written on cue cards which the subjects arranged in order and the second form was the questionnaire version of the V.R.S.The subjects were interviewed and asked five questions relating to their back pain. Upon completion of the interviewthe pressure threshold of the painful area (back) was tested. Subjects then filled in the three pain scales, namely the (V.A .S. one, V.A .S. two, the V.R.S. and lastly the W.B.F. pain scale). Approximately a third of the sample (37) was retested the following day following the same procedure asdescribed above. Results: There was no correlation between the pressure threshold meter readings and the pain scale measurements. Conclusion: From the statistical analysis of the results, it became apparent that the subjects tested did not have anunderstanding of any of the three pain scales. Future research needs to be done in developing entirely different scales for peoples of low literacy and differentlanguage and cultural groups in South Africa.

‘Title retention Clause’ was introduced in sale of goods contracts to prevent the transfer of ownership till the seller was fully paid. Exigencies of businesses have led to introduction of further clauses allowing the buyer to use or sub-sell the goods even before the transfer of ownership. The conflicting provisions, as the judgment of the United Kingdom Supreme Court in PST Energy 7 Shipping v O.W. Bunker Malta Limited brings out, changes the very character of the contract. The contract fails to be a contract of sale of goods depriving the parties of the valuable rights.

Mastology 30, Suppl 1 (2020). http://dx.doi.org/10.29289/259453942020v30s1004.

Texte intégral
Résumé :
Introduction: Occult breast carcinoma is a rare presentation of breast cancer, with histological evidence of axillary lymph node involvement and clinical and radiological absence of malignant breast lesions. Its survival is similar to that of the usual presentation. The treatment consists of modified radical mastectomy or axillary drainage with breast irradiation, resulting in similar survival, associated with systemic therapy according to the staging. Neoadjuvant therapy should be considered in N2-3 axillary cases. Differential diagnoses of axillary lymphadenopathies include: non-granulomatous causes (reactive, lymphoma, metastatic carcinoma) and granulomatous causes (infectious – toxoplasmosis, tuberculosis, sarcoidosis, atypical mycobacteria). Objectives: To report the case of a patient who needed a differential diagnosis among the various causes of axillary lymphadenopathy. Methods: This is a literature review conducted in the PubMed database, using the keywords "granulomatous lymphadenitis", "breast sarcoidosis", "occult breast cancer". Inclusion and exclusion criteria were applied. Case report: V.F.S., female, 51 years old, was referred to an evaluation of axillary lymphadenopathy in May 2019. She was followed by the department of pulmonology due to mediastinal sarcoidosis since 2017. Physical examination indicated breasts without changes. Axillary lymph nodes had increased volume and were mobile and fibroelastic. Mammography revealed only axillary lymph nodes with bilaterally increased density, and the ultrasound showed the presence of atypical bilateral lymph nodes. Neither presented breast lesions. Axillary lymph node core biopsy was compatible with granulomatous lymphadenitis. This result corroborates the diagnosis of sarcoidosis affecting peripheral lymph nodes. The patient was referred back to the department of pulmonology, with no specific treatment since she is oligosymptomatic. Discussion: Despite the context of benign granulomatous disease, malignancy overlying the condition of sarcoidosis must be ruled out. The biopsy provided a safe and definitive diagnosis, excluding the possibility of occult breast carcinoma. The patient will continue to undergo breast cancer screening as indicated for her age and usual risk.
