The tribal people are more tradition bound and have poorer health indicators and limited access to healthcare services. However, with changing time and continuous effort of the government, they are now in a transitional phase between traditional and modern healthcare practices. This article attempts to explore context, reasons, and options behind the health-seeking behavior of the Tangsa (Hill Tribe) women of Margherita sub-division of Tinsukia district, Assam, along with their access to the public healthcare services. Using both simple random and purposive sampling method, primary data were collected in the field from the married women, adolescent girls, and mothers who have girl child of 0–9 years of age from two Tangsa villages, namely Kharangkong and Hassak. Data were collected using standard field methods, for example, observation, survey schedule, interview, semi-structured and open- ended questionnaires, case study, and participatory tools such as focused group discussion (FGD). The findings indicate that Tangsa women generally prefer traditional healing practices to cure many of the common diseases. But a portion of women ranging from 42 to 74 percent received the benefits from different government health schemes related to reproductive health. Institutional delivery is gradually increasing, but iron and folic acid (IFA) intake by pregnant women and adolescent girls is still very poor. The public health department should make more effort to increase the level of awareness and knowledge among the tribal women about the benefits that can be availed from various schemes and modern healthcare services.

Introduction: Adolescents are children aged 10-19 years. Nutrition influences the growth and development during infancy, childhood and adolescence. Adolescent girls are at higher risk of anemia and undernutrition. This research was aimed to find the prevalence of anemia among adolescent girls in a tertiary care hospital. Methods: A descriptive cross-sectional study was done in the adolescent girls attending the pediatric outpatient department of a tertiary care centre from October 2020 to May 2021. After the ethical clearance from the institutional review committee, 380 adolescent girls were taken using a convenient sampling technique. Anthropometric measurements, social demography and blood for hemoglobin estimation were taken and documented in predesigned proforma. Data were analyzed with Statistical Package for the Social Sciences version 20. Point estimate at 95% Confidence Interval was done, and frequency and proportion were calculated. Results: Out of 380 adolescent girls, 230 (60.5%) at 95% Confidence Interval (55.56-65.41) were anemic with mean hemoglobin of 11.138±1.954 gm/dl. The mean age was 14.57±2.107 years. Conclusions: This study showed a higher prevalence of anemia than the national data. Proper education regarding personal and menstrual hygiene, weekly supplementation of iron in school, dietary habits and uplifting of economic status can prevent anemia in this population.

ABSTRACT IMPACT: This change will improve primary care physicians and pediatrics ability to identify, intervene and prevent obesity related renal damage in the vulnerable population of young adults OBJECTIVES/GOALS: Obesity related glomerulopathy has a reversible stage manifested as hyperfiltration. Early intervention depends on the ability to identify hyperfiltration. Hyperfiltration prevalence is underestimated using the currently recommended formula We investigated whether calculating BSA-adjusted GFR will more readily identify hyperfiltration. METHODS/STUDY POPULATION: We extracted data from a large urban, multi-institutional Electronic Health Records (EHR) clinical data research network to construct an EHR data base of 60,549 women and girls ages 12-21 years from the New York metropolitan area. EGFR was calculated in two ways, 1) according to age appropriate formula, and 2) according to age appropriate formula and adjusted to body surface area (BSA). BMI-for-age values were classified according to the World Health Organization schema and grouped according to the CDC definitions. BSA was calculated according to the Du-Bois formula. Hyperfiltration was defined by a threshold of 135ml/min. The Bland Altman method assessed the agreement between formulas across the different BMI groups. RESULTS/ANTICIPATED RESULTS: Serum creatinine values were similar across different BMI groups. Comparing eGFR values, hyperfiltration rates were similar across BMI groups, ranging between 4%-6.6%. BSA-adjusted GFR was different across BMI groups: hyperfiltration rates were 0.81% for the underweight group, 2.56% for the normal weight, 12.18% for the overweight and 39% in the obese group. This trend of hyperfiltration paralleled the the rise in urine creatinine across BMI groups. DISCUSSION/SIGNIFICANCE OF FINDINGS: BSA-adjusted GFR more sensitively detects hyperfiltration due to obesity than does eGFR. Calculating BSA-adjusted GFR will improve primary care and pediatric physicians’ ability to identify, intervene and prevent early ORG. Changes in body composition may account for the increasing discordance between BSA-adjusted and eGFR as BMI rises.

Purpose The available research on specialized interventions for youth experiencing commercial sexual exploitation almost exclusively focuses on the impact and efficacy related to cisgender girls, despite the inclusion of youth who identify as transgender in these programs. This paper aims to present a case study on the experience of a transgender adolescent girl who experienced commercial sexual exploitation and provides a narrative of the multifarious challenges she faced while involved in institutional systems of care. Design/methodology/approach This paper conducted an in-depth case review of all records on “Jade,” a white adolescent transgender girl who experienced commercial sexual exploitation, from a specialty court program in the juvenile justice system between 2012 and 2016. Her experiences throughout childhood exemplify many of the unique challenges that transgender girls and young women with histories of exploitation or trafficking may encounter within service delivery and socioecological systems. This paper applied concepts adapted from the gender minority stress theoretical model to understand how minority gender identity can shape the experiences and outcomes of the youth impacted by commercial sexual exploitation. Findings Jade’s narrative underscores the interplay of gender-based sexual violence, heteronormative structural barriers, transphobia and their intersectional impact on her experience while receiving specialized care. The intersectional hardships she experienced likely contributed to adverse biopsychosocial outcomes, including high rates of medical and behavioral health diagnoses and expectations of further rejection. Originality/value This paper highlights the extraordinary challenges and barriers faced by an often under-recognized and overlooked subset of the youth impacted by commercial sexual exploitation, who may receive services that do not account for their unique needs related to gender expression and identity. This paper exemplifies how internalized stigma along with expectations of further rejection and victimization have implications for clinical and multidisciplinary intervention settings. Jade’s case underscores the need for improved access to supportive services for youth with minority gender identities, including peer community-building opportunities. Finally, this paper identifies a critical gap in US legislation and social policy. This gap contributes to the structural harms faced by transgender and gender-nonconforming youth receiving services during or following experiences of commercial sexual exploitation.

IntroductionIn sub-Saharan Africa, adolescent girls and young women (AGYW) face a range of sexual and reproductive health (SRH) challenges. Clinical, behavioural and structural interventions have each reduced these risks and improved health outcomes. However, combinations of these interventions have not been compared with each other or with no intervention at all. The ‘Girl Power’ study is designed to systematically make these comparisons.Methods and analysisFour comparable health facilities in Malawi and South Africa (n=8) were selected and assigned to one of the following models of care: (1)Standard of care: AGYW can receive family planning, HIV testing and counselling (HTC), and sexually transmitted infection (STI) syndromic management in three separate locations with three separate queues with the general population. No youth-friendly spaces, clinical modifications or trainings are offered, (2)Youth-Friendly Health Services (YFHS): AGYW are meant to receive integrated family planning, HTC and STI services in dedicated youth spaces with youth-friendly modifications and providers trained in YFHS, (3)YFHS+behavioural intervention (BI): In addition to YFHS, AGYW can attend 12 monthly theory-driven, facilitator-led, interactive sessions on health, finance and relationships, (4)YFHS+BI+conditional cash transfer (CCT): in addition to YFHS and BI, AGYW receive up to 12 CCTs conditional on monthly BI session attendance.At each clinic, 250 AGYW 15–24 years old (n=2000 total) will be consented, enrolled and followed for 1 year. Each participant will complete a behavioural survey at enrolment, 6 months and 12 months . All clinical, behavioural and CCT services will be captured. Outcomes of interest include uptake of each package element and reduction in HIV risk behaviours. A qualitative substudy will be conducted.Ethics/disseminationThis study has received ethical approval from the University of North Carolina Institutional Review Board, the University of Cape Town Human Research Ethics Committee and Malawi’s National Health Sciences Research Committee. Study plans, processes and findings will be disseminated to stakeholders, in peer-reviewed journals and at conferences.

Background: Infant deaths from Bhavnagar rural areas were studied by using a verbal autopsy tool.Methods: Community visit based retrospective study of Bhavnagar rural by WHO verbal autopsy questionnaire.Results: Of the 92 deaths analyzed, 59 % (early), 12% (late), and 29% were during the post neonatal period. Male deaths were 55 (60%). The most common immediate causes were infection (39%), birth asphyxia (23%), and hyaline membrane disease (15%). Underlying causes were: maternal illness with feeding problem (45%), prematurity (26%), meconium aspiration syndrome (9%), and congenital/genetic anomalies (10%). Infant and neonatal deaths were seen more with illiteracy of mother, age of mother (25-29 years), third parity, anemia, and vaginal discharge; and non-breastfed, low birth weight and preterm. Birth asphyxia and hyaline membrane disease were during early, and meningitis and pneumonia were after the neonatal period. Verbal autopsy was accurate in 18/23 (78%) of the facility-based deaths where the cause of death was available. Ethics approval was obtained.Conclusions: Reproductive health education to adolescent girls and mothers, regarding the treatment of fever, vaginal discharge; and breastfeeding counselling with vitamin B12 should be used as more infant deaths are associated with anemia of mother. Health workers should be skilled in neonatal resuscitation, prematurity management, and referral, after stabilization, identification of congenital anomaly, antenatal screening by USG, and neonatal metabolic screen. Recent 2018 data obtained from Bhavnagar District Health Authority shows that over a period of eight years, institutional deliveries have increased and home deliveries, early neonatal deaths, HMD, and septicemia have decreased. Perinatal care should be check-list based, monitored, and mentored.

IntroductionIndividuals with Rett syndrome (RTT) experience impaired gross motor skills, limiting their capacity to engage in physical activities and participation in activities. There is limited evidence of the effectiveness of supported physical activity interventions. This study aims to evaluate the effects of a telehealth-delivered physical activity programme on physical activity, sedentary behaviour and quality of life in RTT.Methods and analysisThis is a multicentre study, conducted in Australia, Denmark and Israel. It is a randomised waitlist-controlled trial comparing an intervention to support physical activity with usual care. Participants are children and adults with RTT, recruited from the Australian Rett Syndrome Database, the Danish Center for Rett Syndrome and the Rett Syndrome Association of Israel. The intervention duration is 12 weeks, including fortnightly telephone contact to plan, monitor and develop individual activity programmes. Outcomes are measured at baseline, at 13 weeks and then at 25 weeks. The primary outcomes are sedentary behaviour assessed with an activPAL accelerometer and the number of daily steps measured with a StepWatch Activity Monitor. Secondary outcomes include sleep, behaviour and quality of life. Caregiver experiences will be assessed immediately after the intervention using a satisfaction questionnaire. Group differences for each outcome will be evaluated with analysis of covariance, adjusting for baseline values on an intention-to-treat basis.Ethics and disseminationEthics approval has been obtained in Western Australia from the Child and Adolescent Health Services (RGS3371), in Denmark from the Capital Region Ethics Committee (H-19040514) and in Israel from the Ariel University Institutional Review Board (AU-HEA-ML-20190331). Manuscripts on the development of the intervention from pilot work and the results of the intervention will be submitted to peer-reviewed journals. Results will be presented at conferences and consumer forums. IntroductionAdolescent girls and young women (AGYW) in sub-Saharan Africa are at high risk of HIV acquisition. Pre-exposure prophylaxis (PrEP) demonstration projects observe that AGYW uptake and adherence to PrEP during risk periods is suboptimal. Judgemental interactions with healthcare workers (HCW) and inadequate counselling can be barriers to PrEP use among AGYW. Improving HCW competency and communication to support PrEP delivery to AGYW requires new strategies.Methods and analysisPrEP Implementation for Young Women and Adolescents Program-standardised patient (PrIYA-SP) is a cluster randomised trial of a standardised patient actor (SP) training intervention designed to improve HCW adherence to PrEP guidelines and communication skills. We purposively selected 24 clinics offering PrEP services under fully programmatic conditions in Kisumu County, Kenya. At baseline, unannounced SP ‘mystery shoppers’ present to clinics portraying AGYW in common PrEP scenarios for a cross-sectional assessment of PrEP delivery. Twelve facilities will be randomised to receive a 2-day training intervention, consisting of lectures, role-playing with SPs and group debriefing. Unannounced SPs will repeat the assessment in all 24 sites following the intervention. The primary outcome is quality of PrEP counselling, including adherence to national guidelines and communication skills, scored on a checklist by SPs blinded to intervention assignment. An intention-to-treat (ITT) analysis will evaluate whether the intervention resulted in higher scores within intervention compared with control facilities, adjusted for baseline SP scores and accounting for clustering by facility. We hypothesise that the intervention will improve quality of PrEP counselling compared with standard of care. Results from this study will inform guidelines for PrEP delivery to AGYW in low-resource settings and offer a potentially scalable strategy to improve service delivery for this high-risk group.Ethics and disseminationThe protocol was approved by institutional review boards at Kenyatta National Hospital and University of Washington. An external advisory committee monitors social harms. Results will be disseminated through peer-reviewed journals and presentations.Trial registration numberNCT03875950

Abstract This study examined the effect of the Safe Adolescent Transition and Health Initiative (SATHI) programme on the use of maternal care services among rural, pregnant adolescents in India. This was an intensive community-based, multi-site intervention project conducted in Maharashtra state between 2008 and 2011. Its aims were to improve the reproductive health of married adolescent girls and avert the adverse consequences of early motherhood. It had a quasi-experimental, case-control, pre-post design to enable rigorous evaluation. This study used cross-sectional data from 644 married girls aged under 19 years at baseline and 802 at endline to assess the maternal care outcomes of antenatal care, delivery and postnatal services and nutrition during pregnancy. Difference-in-differences analysis showed that all outcomes improved significantly in the study sites between baseline and endline, and the improvement in study sites was significantly larger than in the control sites. Multivariate analysis showed a statistically significant dose–response effect of intervention participation for antenatal care, pregnancy nutrition and postnatal care. Study participation was not statistically significantly associated with higher rates of safe or institutional delivery. The analysis suggests that training and supporting community health workers to work with married adolescent girls using interpersonal communication and interacting frequently with them and their families and communities can significantly improve the use of maternal care services among this population. With almost a million community health workers and 200,000 auxiliary nurse midwives at the community level providing primary level care in India, this intervention offers a proven strategy to replicate and scale-up to reach large numbers of married adolescent girls who do not currently use maternal care services.

Objective: The siblings of patients suffering from Anorexia Nervosa (AN) are potentially affected by a disturbed emotional experience that often remains undetected. In order to bring them a psychological support, the Maison de Solenn proposed a support group program for these siblings. The current research explores their mental representations of AN and their emotional experience in the support group named “sibling group.”Method: This exploratory study is based on a phenomenological and inductive qualitative method. Four girls and three boys aged between 6 and 19 participating in the “sibling group” were included in a one-time focus group session using a semi-structured interview guide. The thematic data analysis was performed by applying the methods of interpretative phenomenological analysis.Results: Themes that emerged from the interview fall into four categories: AN explained by siblings; the individual emotional experience of siblings; the family experience of siblings and the experience inside the “sibling group.”Discussion: According to our participants, the “sibling group” thus functions as a good compromise between keeping an active role in the anorexic patient's care and taking a step back to avoid being eaten up by the illness. Sibling-group participants retrieved a sense of belonging, which is normally one of the functions of being a sibling. It is important to note that the “sibling group” is part of the comprehensive (or global) family-based approach included in an institutional multidisciplinary integrative care framework.

In this article, I study how an adolescent girl with an institutional background voices her previous experiences of a statutory sex crime relationship. The data were generated during 2013–2017 and analysed using the Listening Guide method in order to find different contrapuntal and situated voices and contextual layers from the girl’s story.The results indicate that different temporal, situational and contextual aspects can be part of the perceptions of sexual consent and abuse. Furthermore, the impression and experiences of being cared for in an abusive relationship interrelate in how the abusive sexual experiences are voiced even after a long period of time. A concept of abusive illusion of care was introduced to describe one of the aspects related to this phenomenon and it was suggested that it should be included in Jenny Pearce’s social model of abused consent.It is proposed that sexual consent could be approached as a situated and contextualised issue and from the perspective and recovery process of the victim; and, as such, an ongoing and proceeding negotiation on consent should also be allowed afterwards. In addition, new and caring ways of creating safe connections in order to confront sexual violence without silencing any voices are needed.<br />Key messages<br /><ul><li>More nuanced knowledge is needed about especially vulnerable adolescent girls polyphonic voices about sexual consent and its connection to temporal experiences of caring.</li><br /><li>Sexual consent could be considered as an ongoing and proceeding negotiation which takes into account situational and contextual aspects.</li></ul>

Introduction Discourses of queer youth suicide regularly represent non-heterosexual young men as vulnerable and as victims who are inherently without strategies for coping with adversity (qv. Rasmussen; Marshall; Driver 3). Alternatively, queer youth are sometimes marked as fundamentally resilient, as avid users of tools of resilience and community such as the internet (Smith & Gray 74; Wexler et al. 566; Hillier & Harrison; Bryson & McIntosh). In the latter approach, protective factors are typically presented as specific to queer youth (e.g., Russell 10), therefore also minoritising and essentialising resilience. Both approaches ignore the diversity of queer young lives and the capacity for a subject to be both vulnerable and resilient—concepts which need to be unpacked if we are to further our understanding of minority lives. Significantly, both approaches also ignore the fact that growing up occurs in a series of transitions, cultural encounters and circumstantial changes. Queer (LGBT) youth are neither all victims and vulnerable, nor are they all self-reliant and resilient. Recent research has indicated that non-heterosexual youth continue to have a higher rate of suicide and self-harm (Cover, Queer Youth Suicide), although this is by no means indicative that vast numbers of LGBTI require support, intervention or preventative measures throughout all aspects of the transition into adult life. This article has two objectives, both of which are best addressed together in order to come at an understanding as how best to frame approaches to queer youth suicide as an ongoing social concern. Firstly, to ask what human, psychological and subjective ‘resilience’ might be said to mean in the context of public discourses of queer youth suicidality, and secondly to ask what a concept of ‘resilience’ does for queer youth identity in terms of relationality. Neither objective, of course, can be met alone in a short article—the purpose here is to open thinking on the topic in ways that question normative assumptions about the conditions of queer youth in the context of liveable lives and the positioning of resilience as reliant on normative accounts of identity. The article begins with a brief overview of the different uses of resilience in the context of broad social representations of queer youth. It goes on to discuss the It Gets Better video site which aimed to produce resilience among predominantly bullied queer youth by ‘imparting hope’. Some remarks on the relationship between identity, sexuality, sociality and resilience will conclude. Resilience and the Queer Youth Subject Developed by Crawford Holling in the 1970s, the concept of resilience was used to describe the capacity of a system to “absorb change and disturbance and still maintain the same relationships between populations or state variables” (Holling 14). In terms of ecology and the physical sciences, the notion of resilience operates within an assumption that future events will not be known but will be unexpected, thereby requiring a capacity to accommodate those events whatever form they take (21). When later used in the psychological sciences, the term resilience likewise assumes disruption and uncertainty in lived experience, requiring a resilient subject to be capable in both learning and adaptation. In the context of queer youth, resilience, then, can be applied to mean an adaptation to new situations which exacerbate vulnerability to suicidality for those who are positioned to seek escape from intolerable emotional pain or the perception of life as unliveable (Cover, Queer Youth Suicide 10, 148). Resilience in this use presumes that, for example, bullying has a detrimental causal relationship with suicidality when it newly occurs if the subject does not have the capacity to adapt and incorporate it into everyday life. Bullying, however, is generally related to suicide only by virtue of its ongoingness rather than it being a sudden shift in social relations. Striking about much of the discourse of resilience in the psychological sciences is that the concept of resilience presumes a unitary subject who is a subject prior to relationality and sociality (e.g. Leipold & Greve; Singh et al.; Smith & Gray). Resilience is thus seen as a capacity to cope with adversity as if adversity arises prior to the subject rather than being a form of relationality that conditions the subject. In that context, the queer youth subject is understood in essentialist terms, whereby sexual subjectivity is represented simultaneously as both a norm and abnormal, and is a factor of subjectivity that is understood to pre-exist sociality. That is, the queer youth subject is queer before relationality with others, thereby before the kinds of relationalities that might demand resilience. An alternative is to understand queer youth not as vulnerable because they are queer, but as subjects constituted in the (inequitably distributed) precarity of corporeal life in sociality, and thereby already formed in (inequitably distributed) resilience to the sorts of shifts, changes and adversities that shift one from an experience of vulnerability to an experience of a life that is unliveable (Butler, Precarious Life; Frames of War). Approaching queer youth suicide from a perspective not of risk but through the simultaneous fostering and critique of resilience opens the possibility of providing solutions that aid younger persons to resist suicidality as a flight from intolerable pain without articulating the self as inviolable and thereby losing the ethical value of the recognition of vulnerability. The question, then, is whether such critique can be found in sites of resilience discourse in relation to queer youth. Queer Youth and It Gets Better The video blogging site It Gets Better (http://www.itgetsbetter.org) was begun by columnist Dan Savage in response to a spate of reported queer student suicides in September/October 2010 in the United States. The site hosts more than a thousand video contributions, many from queer adults who seek to provide hope for younger persons by showing that queer adulthood is markedly different from the experiences of harassment, bullying, loneliness or surveillance experienced by queer youth in school and family environments. This is among the first widely-available communicative media form to address directly queer youth on issues related to suicide, and the first to draw on lived experiences as a means by which to provide resources for queer youth resilience. The fact that these experiences are related through video-logs (vlogs) provides the texts with a greater sense of authenticity and a framework which often addresses youth directly on the topic of suicidality (Cover, Queer Youth Suicide). Savage’s intention was to produce resilience in queer youth by imparting ‘hope for young people facing harassment’ and to create ‘a personal way for supporters everywhere to tell LGBT youth that … it does indeed get better’ (http://www.itgetsbetter.org/pages/about-it-gets-better-project/). Hope, in this context, is represented as the core attribute of queer youth resilience. The tag-line of the site is: Many LGBT youth can’t picture what their lives might be like as openly gay adults. They can’t imagine a future for themselves. So let’s show them what our lives are like, let’s show them what the future may hold in store for them (http://www.itgetsbetter.org/). Hope for the future is frequently presented as hope for an end to school days. In the primary video of the site, Dan Savage’s partner Terry describes his school experiences: My school was pretty miserable … I was picked on mercilessly in school. People were really cruel to me. I was bullied a lot. Beat up, thrown against walls and lockers and windows; stuffed into bathroom stalls. . . . Honestly, things got better the day I left highschool. I didn’t see the bullies every day, I didn’t see the people who harassed me every day, I didn’t have to see the school administrators who would do nothing about it every day. Life instantly got better (http://www.itgetsbetter.org/pages/about-it-gets-better-project/) Such comments present a picture of school life in which the institutional norms of secondary schools that depend so heavily on surveillance, discriminative norms, economies of secrecy and disclosure permit bullying and ostracisation to flourish and become, then, the site of hopelessness in what to many appears at the time as a period of never-ending permanency. Indeed, teen-aged life has often been figured in geographic terms as a kind of hopeless banishment from the realities that are yet to come: Eve Sedgwick referred to that period as ‘that long Babylonian exile known as queer childhood’ (4). The emphatic focus on the institutional environment of highschool rather than family, rural towns, closetedness, religious discourse or feelings of isolation is remarkably important in changing the contemporary way in which the social situation of queer youth suicide has been depicted. The discourse of the It Gets Better project and contributions makes ‘school’ its object—a site that demands resilience of its queer students as the remedy to the detrimental effects of bullying. Here, however, resilience is not depicted as adaptability but the strength to tolerate and, effectively, ‘wait out’, a bullying environment. The focus on bullying that frames the dialogue on queer youth suicide and youth resilience in the It Gets Better videos is the product of a mid-2000s shift in focus to the effects of bullying on LGBT youth in place of critiques of heterosexism, sexual identity, coming out and physical violence (Fodero), regularly depicting bullying as directly causal of suicide (Kim & Leventhal 151; Espelage & Swearer 157; Hegna & Wichstrøm 35). Bullying, in these representations, is articulated as that which is, on the one hand, preventable through punitive institutional policies and, on the other, as an ineradicable fact of living through school years. It is, in the latter depiction, that experience for which younger LGBT persons must manage their own resistance. In depicting school as the site of anti-queer bullying, the It Gets Better project represents queer youth as losing hope of escape from the intolerable pain of bullying in its persistence and repetition. However, the site’s purpose is to show that escape from the school environment to what is regularly depicted as a neoliberal, white and affluent representation of queer adulthood, founded on conservative coupledom (Cover, “Object(ives) of Desire”), careers, urban living, and relative wealth—depictions somewhat different from the reality of diverse queer lives. The shift from the school-bullying in queer youth to the liberal stability of queer adulthood is figured in the It Gets Better discourse as not only possible but as that which should be anticipated. It is in that anticipation that resilience is articulated in a way which calls upon queer youth to manage their own resiliency by having or performing hopefulness. Representing hope as the performative element in queer youth resilience has precedence as a suicide prevention strategy. Hopelessness is a key factor in much of the contemporary academic discussion of suicide risk in general and is often used as a predictor for recognising suicidal behaviour (Battin 13), although it is also particularly associated with suicidality and queer teenagers. Hopelessness is usually understood as despair or desperateness, the lack of expectation of a situation or goal one desires or feels one should desire. For Holden and colleagues, hopelessness is counter to social desirability, which is understood as the capacity to describe oneself in terms by which society judges a person as legitimate or desirable (Holden, Mendonca & Serin 500). Psychological and psychiatric measurement techniques frequently rely on Aaron T. Beck’s Hopelessness Scale, which utilises a twenty-question true/false survey designed to measure feelings about the future, expectation and self-motivation in adults over the age of seventeen years as a predictor of suicidal behaviour. Beck and colleagues attempted to provide an objective measurement for hopelessness rather than leave it treated as a diffuse and vague state of feeling in patients with depression. The tool asks a series of questions, most about the future, presenting a score on whether or not the answers given were true or false. Questions include: ‘I might as well give up because I can’t make things better for myself’; ‘I can’t imagine what my life would be like in ten years’; ‘My future seems dark to me’; and ‘All I can see ahead of me is unpleasantness rather than pleasantness’. Responding true to these indicates hopelessness. Responding false to some of the following also indicates hopelessness: ‘I can look forward to more good times than bad times’; and ‘When things are going badly, I am helped by knowing they can’t stay that way forever’ (Beck). While these questions and the scale are not used uncritically, the relationship between the discursive construction through the questions of what constitutes hopelessness and the aims of the It Gets Better videos are notably comparable. The objective, then, of the videos is to provide evidence and, perhaps, instil hope that would allow such questions to be answered differently, particularly to be able to give a true response to the last question above. Hallway Allies liaison support group, which operates across university campuses and high schools to prevent bullying, stated in this representative way in the introduction to their video contribution: ‘Remember to keep your head up, highschool doesn’t last forever’ (http://www.itgetsbetter.org/video /entry/5wwozgwyruy/). Or, as Rebecca in the introductory statement of another video contribution put it: You may be feeling like this pain will last forever, like you have no control, it’s dark, oppressive and feels like there is no end. I know – I get it. but I promise … hang in there and you’ll find it … Wait – you’ll see – it gets better! (http://www.itgetsbetter.org/video/entry/wxymqzw3oqy/). As can be seen, such video examples respond to a discourse of hopelessness aligned with the framework exemplified by Beck’s scale, prompting queer youth audiences of these videos to imagine a future for themselves, to understand hope in temporal terms of future wellbeing, and to know that the future does not necessarily hold the same kinds of unpleasantness as experienced in the everyday high school environment. Sexual Identity, Resilience and the Normative Lifecycle In the It Gets Better framework, resilience is produced in the knowledge of a queer life that is linear and patterned through stages in relation to institutional forms of belonging (and non-belonging). That is, a queer life is represented as one which undergoes the hardship of being bullied in school, of leaving that institutional environment for a queer adulthood that is built on a myth of safety, pleasure, success and a distinctive break from the environment of the past (as if the psyche or the self is re-produced anew in a phase of a queer lifecycle). Working within a queer theoretical and cultural understanding of identity, sexual subjectivity can be understood as constituted in social and cultural formations. Overturning the previously-held liberal notion of power as the power which represses sex and sexualities, Foucault’s History of Sexuality provided queer theory with an argument in which power, as deployed through discourse and discursive formations, produces the coherent sexual subject. This occurs historically and only in specific periods. In Foucault’s analysis, homosexual identities become conceivable in the Nineteenth Century as a result of specific juridical, medical and criminal discourses (85). From a Foucauldian perspective, there is no subject driven by an inner psyche or a pre-determined desire (as in psychoanalysis). Instead, such subjectivity occurs in and through the power/knowledge network of discourse as it writes or scripts the subject into subjectivity. Canonical queer theorist Judith Butler has been central in extending Foucault’s analysis in ways which are pragmatic for understanding queer youth in the context of growing up and transitioning into adulthood. Her theory of performativity has usefully complexified the ways in which we can understand sexual identity and allowed us to overcome the core assumption in much queer youth research that heterosexual and homosexual identities are natural, mutually-exclusive and innate; instead, allowing us to focus on how the process of subject formation for youth is implicated in the tensions and pressures of a range of cultural, social, organisational and communicative encounters and engagements. Butler projects the most useful post-structuralist discussion of subjectivity by suggesting that the subject is constituted by repetitive performances in terms of the structure of signification that produces retroactively the illusion of an inner subjective core (Butler, Gender Trouble 143). Queer identity becomes a normative ideal rather than a descriptive feature of experience, and is the resultant effect of regimentary discursive practices (16, 18). The non-heterosexual subject, then, is performatively constituted by the very ‘expressions’ that are formed as recognisable identity performances in the context, here, of a set of lifecycle expectations built through a vulnerable queer childhood, being bullied, attaining hope, leaving school and fruition in queer adulthood. Resilience, in the It Gets Better discourse, then, is seen to be produced in understanding the stages of a normative queer life. An issue emerges for how queer youth suicide is understood within this particular formation that posits non-heterosexuality as the problematic source of suicidality emerges in the assumption that the vulnerability to suicidal behaviours for queer youth is the result singularly of sexuality, rather than looking to the fact that sexuality is one facet of identity – an important and sometimes fraught one for adolescents in general – located within a complex of other formations of identity and selfhood. This is part of what Diana Fuss has identified as the “synecdochical tendency to see only one part of a subject’s identity (usually the most visible part) and to make that part stand for the whole” (116). This ignores the opportunity to think through the conditions of queer youth in terms of the interaction between different facets of identity (such as gender and ethnicity, but also personal experience), different contexts in which identity is performed and different institutional settings that vary in response and valuation of non-normative aspects of subjectivity, thereby allowing a vulnerability not to be an attribute of being a queer youth, but to be understood as produced across a nuanced and complex array of factors. While the very concept of resilience invokes both an individualisation of the subject and a disciplinary regime of pastoral care (Foucault, Abnormal), queer youth in the It Gets Better discourse of hope are depicted multiply as: Inherently vulnerable and lacking resilience as a result of an essentialist notion of sexual orientation.Constituted in a relationality within a schooling environment that is conditioned by bullying as the primary expression of diverse socialityFinding resilience only through a self-managed and self-articulated expression of ‘hope’ that is to be produced in the knowledge that there is an ‘escape’ from a school environment. What the discourse of that which we might refer to as “resilient hopefulness” does is represent queer youth reductively as inherently non-resilient. It ignores the multiple expressions of sexual identity, the capacity to respond to suicidality through a critique of normative sexual subjectivity, and the capabilities of queer youth to develop meaningful relationships across all sexual possibilities that are, themselves, forms of resilience or at least mitigations of vulnerability. At the same time, “resilient hopefulness” is produced within a context in which a normative sociality of bullying culture is expressed as timeless and unchangeable (rather than historical and institutional), thereby requiring queer younger persons to undertake the task of managing vulnerability, risk, resilience and identity as an individualised responsibility outside of communities of care. Whether the presentation of a normative lifecycle is genuinely a preventative measure for queer youth suicidality is that which suicidologists and practitioners must test, although one might argue at this stage that resilience is better produced through a broader appeal to social diversity rather than the regimentation of a queer life that must ‘wait in hope’ for a liveability that may never come. (Photo credit: Author) Introduction Since the mid-1990s robotic and digital creatures designed to offer social interaction and companionship have been developed for commercial and research interests. Integral to encouraging positive experiences with these creatures has been the use of cute aesthetics that aim to endear companions to their human users. During this time there has also been a growth in online communities that engage in cultural production through fan fiction responses to existing cultural artefacts, including the widely recognised electronic companion, Hasbro’s Furby (image 1). These user stories and Furby’s online representation in general, demonstrate that contrary to the intentions of their designers and marketers, Furbys are not necessarily received as cute, or the embodiment of the helpless and harmless demeanour that goes along with it. Furbys’ large, lash-framed eyes, small, or non-existent limbs, and baby voice are typical markers of cuteness but can also evoke another side of cuteness—monstrosity, especially when the creature appears physically capable instead of helpless (Brzozowska-Brywczynska 217). Furbys are a particularly interesting manifestation of the cute aesthetic because it is used as tool for encouraging attachment to a socially interactive electronic object, and therefore intersects with existing ideas about technology and nonhuman companions, both of which often embody a sense of otherness. This paper will explore how cuteness intersects withand transitions into monstrosity through online representations of Furbys, troubling their existing design and marketing narrative by connecting and likening them to other creatures, myths, and anecdotes. Analysis of narrative in particular highlights the instability of cuteness, and cultural understandings of existing cute characters, such as the gremlins from the film Gremlins (Dante) reinforce the idea that cuteness should be treated with suspicion as it potentially masks a troubling undertone. Ultimately, this paper aims to interrogate the cultural complexities of designing electronic creatures through the stories that people tell about them online. Fan Production Authors of fan fiction are known to creatively express their responses to a variety of media by appropriating the characters, settings, and themes of an original work and sharing their cultural activity with others (Jenkins 88). On a personal level, Jenkins (103) argues that “[i]n embracing popular texts, the fans claim those works as their own, remaking them in their own image, forcing them to respond to their needs and to gratify their desires.” Fan fiction authors are motivated to write not for financial or professional gains but for personal enjoyment and fan recognition, however, their production does not necessarily come from favourable opinions of an existing text. The antifan is an individual who actively hates a text or cultural artefact and is mobilised in their dislike to contribute to a community of others who share their views (Gray 841). Gray suggests that both fan and antifan activity contribute to our understanding of the kinds of stories audiences want: Although fans may wish to bring a text into everyday life due to what they believe it represents, antifans fear or do not want what they believe it represents and so, as with fans, antifan practice is as important an indicator of interactions between the textual and public spheres. (855) Gray reminds that fans, nonfans, and antifans employ different interpretive strategies when interacting with a text. In particular, while fans intimate knowledge of a text reflects their overall appreciation, antifans more often focus on the “dimensions of the moral, the rational-realistic, [or] the aesthetic” (856) that they find most disagreeable. Additionally, antifans may not experience a text directly, but dislike what knowledge they do have of it from afar. As later examples will show, the treatment of Furbys in fan fiction arguably reflects an antifan perspective through a sense of distrust and aversion, and analysing it can provide insight into why interactions with, or indirect knowledge of, Furbys might inspire these reactions. Derecho argues that in part because of the potential copyright violation that is faced by most fandoms, “even the most socially conventional fan fiction is an act of defiance of corporate control…” (72). Additionally, because of the creative freedom it affords, “fan fiction and archontic literature open up possibilities – not just for opposition to institutions and social systems, but also for a different perspective on the institutional and the social” (76). Because of this criticality, and its subversive nature, fan fiction provides an interesting consumer perspective on objects that are designed and marketed to be received in particular ways. Further, because much of fan fiction draws on fictional content, stories about objects like Furby are not necessarily bound to reality and incorporate fantastical, speculative, and folkloric readings, providing diverse viewpoints of the object. Finally, if, as robotics commentators (cf. Levy; Breazeal) suggest, companionable robots and technologies are going to become increasingly present in everyday life, it is crucial to understand not only how they are received, but also where they fit within a wider cultural sphere. Furbys can be seen as a widespread, if technologically simple, example of these technologies and are often treated as a sign of things to come (Wilks 12). The Design of Electronic Companions To compete with the burgeoning market of digital and electronic pets, in 1998 Tiger Electronics released the Furby, a fur-covered, robotic creature that required the user to carry out certain nurturance duties. Furbys expected feeding and entertaining and could become sick and scared if neglected. Through a program that advanced slowly over time regardless of external stimulus, Furbys appeared to evolve from speaking entirely Furbish, their mother tongue, to speaking English. To the user, it appeared as though their interactions with the object were directly affecting its progress and maturation because their care duties of feeding and entertaining were happening parallel to the Furbish to English transition (Turkle, Breazeal, Daste, & Scassellati 314). The design of electronic companions like Furby is carefully considered to encourage positive emotional responses. For example, Breazeal (2002 230) argues that a robot will be treated like a baby, and nurtured, if it has a large head, big eyes, and pursed lips. Kinsella’s (1995) also emphasises cute things need for care as they are “soft, infantile, mammalian, round, without bodily appendages (e.g. arms), without bodily orifices (e.g. mouths), non-sexual, mute, insecure, helpless or bewildered” (226). From this perspective, Furbys’ physical design plays a role in encouraging nurturance. Such design decisions are reinforced by marketing strategies that encourage Furbys to be viewed in a particular way. As a marketing tool, Harris (1992) argues that: cuteness has become essential in the marketplace in that advertisers have learned that consumers will “adopt” products that create, often in their packaging alone, an aura of motherlessness, ostracism, and melancholy, the silent desperation of the lost puppy dog clamoring to be befriended - namely, to be bought. (179) Positioning Furbys as friendly was also important to encouraging a positive bond with a caregiver. The history, or back story, that Furbys were given in the instruction manual was designed to convey their kind, non-threatening nature. Although alive and unpredictable, it was crucial that Furbys were not frightening. As imaginary living creatures, the origin of Furbys required explaining: “some had suggested positioning Furby as an alien, but that seemed too foreign and frightening for little girls. By May, the thinking was that Furbies live in the clouds – more angelic, less threatening” (Kirsner). In creating this story, Furby’s producers both endeared the object to consumers by making it seem friendly and inquisitive, and avoided associations to its mass-produced, factory origins. Monstrous and Cute Furbys Across fan fiction, academic texts, and media coverage there is a tendency to describe what Furbys look like by stringing together several animals and objects. Furbys have been referred to as a “mechanized ball of synthetic hair that is part penguin, part owl and part kitten” (Steinberg), a “cross between a hamster and a bird…” (Lawson & Chesney 34), and “ “owl-like in appearance, with large bat-like ears and two large white eyes with small, reddish-pink pupils” (ChaosInsanity), to highlight only a few. The ambiguous appearance of electronic companions is often a strategic decision made by the designer to avoid biases towards specific animals or forms, making the companion easier to accept as “real” or “alive” (Shibata 1753). Furbys are arguably evidence of this strategy and appear to be deliberately unfamiliar. However, the assemblage, and exaggeration, of parts that describes Furbys also conjures much older associations: the world of monsters in gothic literature. Notice the similarities between the above attempts to describe what Furbys looks like, and a historical description of monsters: early monsters are frequently constructed out of ill-assorted parts, like the griffin, with the head and wings of an eagle combined with the body and paws of a lion. Alternatively, they are incomplete, lacking essential parts, or, like the mythological hydra with its many heads, grotesquely excessive. (Punter & Byron 263) Cohen (6) argues that, metaphorically, because of their strange visual assembly, monsters are displaced beings “whose externally incoherent bodies resist attempts to include them in any systematic structuration. And so the monster is dangerous, a form suspended between forms that threatens to smash distinctions.” Therefore, to call something a monster is also to call it confusing and unfamiliar. Notice in the following fan fiction example how comparing Furby to an owl makes it strange, and there seems to be uncertainty around what Furbys are, and where they fit in the natural order: The first thing Heero noticed was that a 'Furby' appeared to be a childes toy, shaped to resemble a mutated owl. With fur instead of feathers, no wings, two large ears and comical cat paws set at the bottom of its pudding like form. Its face was devoid of fuzz with a yellow plastic beak and too large eyes that gave it the appearance of it being addicted to speed [sic]. (Kontradiction) Here is a character unfamiliar with Furbys, describing its appearance by relating it to animal parts. Whether Furbys are cute or monstrous is contentious, particularly in fan fictions where they have been given additional capabilities like working limbs and extra appendages that make them less helpless. Furbys’ lack, or diminution of parts, and exaggeration of others, fits the description of cuteness, as well as their sole reliance on caregivers to be fed, entertained, and transported. If viewed as animals, Furbys appear physically limited. Kinsella (1995) finds that a sense of disability is important to the cute aesthetic: stubby arms, no fingers, no mouths, huge heads, massive eyes – which can hide no private thoughts from the viewer – nothing between their legs, pot bellies, swollen legs or pigeon feet – if they have feet at all. Cute things can’t walk, can’t talk, can’t in fact do anything at all for themselves because they are physically handicapped. (236) Exploring the line between cute and monstrous, Brzozowska-Brywczynska argues that it is this sense of physical disability that distinguishes the two similar aesthetics. “It is the disempowering feeling of pity and sympathy […] that deprives a monster of his monstrosity” (218). The descriptions of Furbys in fan fiction suggest that they transition between the two, contingent on how they are received by certain characters, and the abilities they are given by the author. In some cases it is the overwhelming threat the Furby poses that extinguishes feelings of care. In the following two excerpts that the revealing of threatening behaviour shifts the perception of Furby from cute to monstrous in ‘When Furbies Attack’ (Kellyofthemidnightdawn): “These guys are so cute,” she moved the Furby so that it was within inches of Elliot's face and positioned it so that what were apparently the Furby's lips came into contact with his cheek “See,” she smiled widely “He likes you.” […] Olivia's breath caught in her throat as she found herself backing up towards the door. She kept her eyes on the little yellow monster in front of her as her hand slowly reached for the door knob. This was just too freaky, she wanted away from this thing. The Furby that was originally called cute becomes a monster when it violently threatens the protagonist, Olivia. The shifting of Furbys between cute and monstrous is a topic of argument in ‘InuYasha vs the Demon Furbie’ (Lioness of Dreams). The character Kagome attempts to explain a Furby to Inuyasha, who views the object as a demon: That is a toy called a Furbie. It's a thing we humans call “CUTE”. See, it talks and says cute things and we give it hugs! (Lioness of Dreams) A recurrent theme in the Inuyasha (Takahashi) anime is the generational divide between Kagome and Inuyasha. Set in feudal-era Japan, Kagome is transported there from modern-day Tokyo after falling into a well. The above line of dialogue reinforces the relative newness, and cultural specificity, of cute aesthetics, which according to Kinsella (1995 220) became increasingly popular throughout the 1980s and 90s. In Inuyasha’s world, where demons and monsters are a fixture of everyday life, the Furby appearance shifts from cute to monstrous. Furbys as GremlinsDuring the height of the original 1998 Furby’s public exposure and popularity, several news articles referred to Furby as “the five-inch gremlin” (Steinberg) and “a furry, gremlin-looking creature” (Del Vecchio 88). More recently, in a review of the 2012 Furby release, one commenter exclaimed: “These things actually look scary! Like blue gremlins!” (KillaRizzay). Following the release of the original Furbys, Hasbro collaborated with the film’s merchandising team to release Interactive ‘Gizmo’ Furbys (image 2). Image 2: Hasbro 1999 Interactive Gizmo (photo credit: Author) Furbys’ likeness to gremlins offers another perspective on the tension between cute and monstrous aesthetics that is contingent on the creature’s behaviour. The connection between Furbys and gremlins embodies a sense of mistrust, because the film Gremlins focuses on the monsters that dwell within the seemingly harmless and endearing mogwai/gremlin creatures. Catastrophic events unfold after they are cared for improperly. Gremlins, and by association Furbys, may appear cute or harmless, but this story tells that there is something darker beneath the surface. The creatures in Gremlins are introduced as mogwai, and in Chinese folklore the mogwai or mogui is a demon (Zhang, 1999). The pop culture gremlin embodied in the film, then, is cute and demonic, depending on how it is treated. Like a gremlin, a Furby’s personality is supposed to be a reflection of the care it receives. Transformation is a common theme of Gremlins and also Furby, where it is central to the sense of “aliveness” the product works to create. Furbys become “wiser” as time goes on, transitioning through “life stages” as they “learn” about their surroundings. As we learn from their origin story, Furbys jumped from their home in the clouds in order to see and explore the world firsthand (Tiger Electronics 2). Because Furbys are susceptible to their environment, they come with rules on how they must be cared for, and the consequences if this is ignored. Without attention and “food”, a Furby will become unresponsive and even ill: “If you allow me to get sick, soon I will not want to play and will not respond to anything but feeding” (Tiger Electronics 6). In Gremlins, improper care manifests in an abrupt transition from cute to monstrous: Gizmo’s strokeable fur is transformed into a wet, scaly integument, while the vacant portholes of its eyes (the most important facial feature of the cute thing, giving us free access to its soul and ensuring its total structability, its incapacity to hold back anything in reserve) become diabolical slits hiding a lurking intelligence, just as its dainty paws metamorphose into talons and its pretty puckered lips into enormous Cheshire grimaces with full sets of sharp incisors. (Harris 185–186) In the Naruto (Kishimoto) fan fiction ‘Orochimaru's World Famous New Year's Eve Party’ (dead drifter), while there is no explicit mention of Gremlins, the Furby undergoes the physical transformation that appears in the films. The Furby, named Sasuke, presumably after the Naruto antagonist Sasuke, and hinting at its untrustworthy nature, undergoes a transformation that mimics that of Gremlins: when water is poured on the Furby, boils appear and fall from its back, each growing into another Furby. Also, after feeding the Furby, it lays eggs: Apparently, it's not a good idea to feed Furbies chips. Why? Because they make weird cocoon eggs and transform into… something. (ch. 5) This sequence of events follows the Gremlins movie structure, in which cute and furry Gizmo, after being exposed to water and fed after midnight, “begins to reproduce, laying eggs that enter a larval stage in repulsive cocoons covered in viscous membranes” (Harris 185). Harris also reminds that the appearance of gremlins comes with understandings of how they should be treated: Whereas cute things have clean, sensuous surfaces that remain intact and unpenetrated […] the anti-cute Gremlins are constantly being squished and disembowelled, their entrails spilling out into the open, as they explode in microwaves and run through paper shredders and blenders. (Harris 186) The Furbys in ‘Orochimaru's World Famous New Year's Eve Party’ meet a similar end: Kuro Furby whined as his brain was smashed in. One of its eyes popped out and rolled across the floor. (dead drifter ch. 6) A horde of mischievous Furbys are violently dispatched, including the original Furby that was lovingly cared for. Conclusion This paper has explored examples from online culture in which different cultural references clash and merge to explore artefacts such as Furby, and the complexities of design, such as the use of ambiguously mammalian, and cute, aesthetics in an effort to encourage positive attachment. Fan fiction, as a subversive practice, offers valuable critiques of Furby that are imaginative and speculative, providing creative responses to experiences with Furbys, but also opening up potential for what electronic companions could become. In particular, the use of narrative demonstrates that cuteness is an unstable aesthetic that is culturally contingent and very much tied to behaviour. As above examples demonstrate, Furbys can move between cute, friendly, helpless, threatening, monstrous, and strange in one story. Cute Furbys became monstrous when they were described as an assemblage of disparate parts, made physically capable and aggressive, and affected by their environment or external stimulus. Cultural associations, such as gremlins, also influence how an electronic animal is received and treated, often troubling the visions of designers and marketers who seek to present friendly, nonthreatening, and accommodating companions. These diverse readings are valuable in understanding how companionable technologies are received, especially if they continue to be developed and made commercially available, and if cuteness is to be used as means of encouraging positive attachment.
