Testo completoABSTRACT Keywords: multidose drug dispensing system, LSS, nurse, quality improvement, medical treatment Background: The nurse has a great responsibility regarding medical treatment follow-ups and must also seek a good interaction with other participants in the continuum of care, in order to assure the quality of care. Aim: From a nurse’s perspective describe routines for medical treatment follow-ups, regarding patients with multidose drug dispensing system and accommodations according to LSS, as well as identify possible well-functioning and deficient routines. Method: The study has a descriptive design with qualitative approach. Five interviews were conducted with nurses, all working at accommodations according to LSS in Uppsala Municipality. The data was analyzed according to Graneheim and Lundman's (2004) method of content analysis. Results: There are some deficient routines and a need of better communication and more clear guidelines for the division of responsibilities regarding the patients’ medical treatment follow-ups. Overall, the participating nurses are content with the multidose drug dispensing system and there is nothing indicating that the deficient routines are associated with this system. Conclusions: The study shows that certain routines should be overhauled and reconsidered and that a clarification of the division of responsibilities, between nurses and doctors, could lead to improvements at medical treatment follow-ups. However, to elucidate a broader perspective of routines for medical treatment follow-ups, further studies are required.

Testo completoEn stor del av Sveriges befolkning över 65 år får sina läkemedel expedierade via Apotekets dosdispenserade läkemedel, ApoDos. E-dos är ett förskrivarverktyg som nu används vid förskrivning till dessa patienter. Införandet av e-dos är tänkt att förenkla hanteringen av ordinationer, att öka graden av automatisering av produktion av doser och att öka säkerheten i processerna. Trots detta så kommer en stor del av ordinationerna fortfarande till DosApoteken via fax.
Syftet med denna studie är att öka förståelsen för den underliggande problematik som ligger till grund för att e-dos inte används i större utsträckning än cirka 50-60 % vid ordination av läkemedel till ApoDos-patienter.
Studien har ett kvalitativt tillvägagångssätt där intervjuer med läkare från Kalmar kommun har genomförts. Data samlades in från så väl öppen- som slutenvårdsläkare under ett par veckor i april 2010.
Resultaten visar att läkarna tycker att e-dos är ett bra system, som ger en god helhetsbild över patientens samtliga läkemedel. Men på grund av att journalsystemet Cosmic och e-dos inte automatiskt delar information mellan sig så gör det att läkarna utsätts för ett tidskrävande dubbelarbete. En integrering av dessa system skulle med största sannolikhet minska läkarnas arbetsbörda och därmed leda till en ökad användning av e-dos. E-dos grafiska utformning upplevdes som komplicerad och svåröverskådlig. För att genomföra ändringar i patientens läkemedelsbehandling så krävs en stor mängd bakgrundsinformation och att läkaren genomför många moment innan en ordinationsförändring godkänns. Ledtiden och låsningen av e-dos är en orsak till att även vana e-dosanvändare väljer att gå utanför systemet, framförallt vid akuta och tillfälliga ordinationer.
Majoriteten av läkarna ansåg att användandet av e-dos är en säkerhet för patienten men under intervjuerna så framkom det att behovet av en integrering mellan journalsystemet och e-dos är stort. Läkarna upplevde att avsaknaden av en integrering var en riskfaktor för patienten och att det ökade på läkarnas arbetsbörda. En enklare utformning av e-dos önskades också, då ett stort antal läkare upplevde systemet som svåranvänt.
Objective. The aim of this study is to increase the understanding of why physicians don’t use e-dos (a computerized prescription system) to a greater extent than about 50-60 % when prescribing medicine to patients within the specific medication dispensing system (ApoDos).
Methods. This study has a qualitative approach. Data was collected via interviews with general practitioners and physicians from a hospital in the municipality of Kalmar during a couple of weeks in April 2010.
Results. The physicians feel that e-dos is a good system, that provides a complete picture of the patient’s prescriptions. But because the electronic medical record systems “Cosmic” and e-dos aren’t able to share the same information in between them, this leads to an extra workload for the physicians. An integration of these two systems would decrease the workload and maybe also increase the use of e-dos. The graphic design is experienced as complicated and difficult to grasp. To make a change in a patient’s medication within e-dos requires a great quantity of information. Physicians are required to carry out a number of steps before a change is completed. Even experienced users choose not to use e-dos in some circumstances, i.e. then the prescription is urgent or temporary, and they use ordinary prescriptions instead. After a change in a patient’s medication is prescribed, the system is locked until the change is approved by a pharmacist. This is a course of irritation among the physicians.
Conclusion. The majority of physicians considered the use of e-dos as safe for the patient. They also pointed on the need of integration between the electronic medical record system, “Cosmic” and e-dos. Physicians expressed that the lack of integration was a risk factor for medical errors and that it increased the physicians’ workload. Several physicians also described the system as difficult to handle and wished a simpler design.

Testo completoThe Bible states that man was created by God with the characteristic ability to name and to speak (Redmonds, 2004). Albaigès (1998) and Darlington (2015) refer to the issue of names as a fascinating topic but it had only been regarded as a pastime (García-Cornejo, 2001; Redmonds, 2004). Although in recent times the interest in this subject has grown, until now most of the research has been devoted to the exploration of surnames and place names (Postles, 2002; Alomar, 2005). The names themselves as well as the reactions to names are full of expressivity (Evans & Green, 2006). Many different factors can be involved in the choice of a name, e.g. euphony, fashion, family tradition, religion, books, films (Withycombe, 1971; Zittoun, 2004). This study uses a comparative perspective in order to provide more empirical evidence for the significance of forenames, hypocorisms and nicknames in our lives by exploring more closely their richness and colourfulness, as portrayed in people’s reactions to them, in two different languages, English and Spanish. The participants were 425 males and females over 25 year-old whose usual place of residence was the metropolitan district of Murcia or Leeds. A semi-structured questionnaire with two versions, in Spanish and English, was used. The study was conducted over a period of two years (2010-2012). First of all, the metropolitan districts were chosen. Our participants were then selected from those who met the conditions previously mentioned (quota non-probability sampling strategy). Later, the questionnaire was designed. A personal interview modality was used (Rincón, 1991), which took about 10 minutes to complete. The data were analysed by using the statistical package SPSS 19.0.0 for Windows. Univariate, bivariate and association analysis techniques were used. Qualitative analyses of the quantitative information and a purely qualitative analysis of spontaneous comments from the participants were undertaken. The findings reveal that in both municipal districts, the younger the participant, the higher the rate of preference for shortened, changed or lengthened name forms. However, other factors (e.g. religion or beauty) may also have a bearing on the extent to which one form is chosen over another. Although most forenames assessed in terms of sound are liked in Murcia and Leeds, some such as Jéssica or Jénnifer are not favourably positioned in Spain, origin being involved in the fact that these names receive negative liking ratings. In Murcia and its surrounds respondents find more funny connotations than in Leeds and districts, especially as a result of the different associations evoked. The name versions Antonia-Antonella and Sonia-Sofía/Sophia elicit a similar rate of preferences in the two municipal districts, with a tendency towards Antonia and Sofía/Sophia, respectively. In the latter group most of the participants prefer the sound of the complete name rather than a sound or sounds in particular. Non-pejorative reasons (simply disliking the sound) are more common in both countries. The most disliked name in Murcia and districts is always the female counterpart while the choice is more balanced in terms of gender in Leeds and its surrounds. Emotional reactions are found in which neither the interpretations of questions nor the answers are categorical. Association is the factor par excellence, with a clear tendency towards associating names with football and T.V. celebrities on the part of Spaniards and literature on the part of Britons. The lexis used by participants is very varied and rich. Spaniards are more direct and explicit. Comments about names are full of expressive language: we find hyperboles, metaphors, metonymies, similes, symbols, sound symbolism and word play. Like Tsirópulos (1987), now we also feel the richness of names, their LIFE. The results obtained in this research project have provided us with some important findings, but there are obvious limitations. Using a more substantial number of name exemplars would be recommended in order to further substantiate and validate the conclusions of this study.
