AbstractBoth poultry meat and eggs provide high-quality animal protein [containing sufficient amounts and proper ratios of amino acids (AAs)] for human consumption and, therefore, play an important role in the growth, development, and health of all individuals. Because there are growing concerns about the suboptimal efficiencies of poultry production and its impact on environmental sustainability, much attention has been paid to the formulation of low-protein diets and precision nutrition through the addition of low-cost crystalline AAs or alternative sources of animal-protein feedstuffs. This necessitates a better understanding of AA nutrition and metabolism in chickens. Although historic nutrition research has focused on nutritionally essential amino acids (EAAs) that are not synthesized or are inadequately synthesized in the body, increasing evidence shows that the traditionally classified nutritionally nonessential amino acids (NEAAs), such as glutamine and glutamate, have physiological and regulatory roles other than protein synthesis in chicken growth and egg production. In addition, like other avian species, chickens do not synthesize adequately glycine or proline (the most abundant AAs in the body but present in plant-source feedstuffs at low content) relative to their nutritional and physiological needs. Therefore, these two AAs must be sufficient in poultry diets. Animal proteins (including ruminant meat &amp; bone meal and hydrolyzed feather meal) are abundant sources of both glycine and proline in chicken nutrition. Clearly, chickens (including broilers and laying hens) have dietary requirements for all proteinogenic AAs to achieve their maximum productivity and maintain optimum health particularly under adverse conditions such as heat stress and disease. This is a paradigm shift in poultry nutrition from the 70-year-old “ideal protein” concept that concerned only about EAAs to the focus of functional AAs that include both EAAs and NEAAs.

Skeletal dysplasias are developmental disorders of the chondro-osseous tissues commonly resulting in short stature, which is often disproportionate. The underlying mutations are often in the structural genes encoding components of the matrix but may also involve growth factors or cell signalling. In contrast, the dysostoses tend to affect single bones or groups of bones, reflecting the transient nature of the many different signalling factors to which they are responsive during development. Abnormalities of bone density (high or low) may be due to primary deficiency of bone matrix synthesis (e.g. osteogenesis imperfecta and hypophosphatasia) but may also reflect an imbalance between bone formation and resorption. This may be caused by abnormalities of bone formation (e.g. hyperostosis/sclerosteosis and osteoporosis pseudoglioma syndrome) or bone resorption (e.g. classic osteopetrosis and fibrous dysplasia).

Without policy measures to offset the negative impact of the bonus culture, investment, productivity, and growth are likely to remain depressed. Given the slow growth of the working age population, the UK’s trend growth rate will thus be 1 per cent and that of the US 0.87 per cent, unless productivity improves. An alternative method of estimating US trend growth from the value data for tangible capital stock provides a slightly better rate of 1.1 per cent per annum. The prospects for the UK and US are so poor that policy measures to stimulate growth are vital. All growth is the result of changes in either TFP or NTV, so one or other must improve to avoid stagnation. There is no way to improve the former, but changes in NTV can be brought about through a lower hurdle rate, which requires the damage from the bonus culture to end.

The chapter shows that a sudden and sufficiently large contraction of bonds' liquidity could generate a fall in output, liquidity trap and price deflation, suggesting that there is no contradiction between Liquidity Crunch and Liquidity Trap, in line with the discussion in Chapter 2. The discussion is based on a simple model in which there are two types of liquid assets: fiat money and liquid bonds (e.g., Asset-Backed securities) under flexible prices. Fiat money is the liquid asset of choice for households, while under normal conditions firms have a preference for bonds. The model is employed to discuss quantitative easing (QE) in exchange for 'toxic' assets, and to show that the loss of bond liquidity could give rise to secular stagnation.

This period saw a surge in wartime credit, the end of the Reconstruction Finance Corporation (RFC), and the emergence of government support for venture capital and school loans. As postwar credit programs moved capital and pioneered new ways of lending, they shaped how and where U.S. companies lent money. They also helped lift a generation of white families and systematically exclude African Americans. Discriminatory and decentralized, off-budget and complex, the credit programs were a key component of America's peculiar developmental state. Indeed, credit programs are not aberrations in the system. They are a core aspect of how the system works.

Short stature (SS) is defined as height less than the third percentile or below –2 standard deviation score (SDS) with reference to chronological age according to standard growth curves. Children are born small for gestational age (SGA) when their birth height and/or birth weight are below or equal to –2 SDS using standards such as Usher and McLean. In patients presenting with SS associated with abnormal physical features, malformations, or delayed development, a syndromic growth disorder should be considered. Whilst individually rare, there are many syndromes with short stature as a component—in the London Dysmorphology Database (Winter and Baraitser), there are 873 such syndromes, 175 of which are of prenatal onset. In these patients, malformations and/or sensorineural abnormalities should be systematically screened by complementary exams (skeletal X-rays, cardiac and abdominal ultrasound, complete eye and hearing evaluations). In some cases, these abnormalities could help in making the diagnosis (e.g. pulmonary stenosis suggestive of Noonan’s syndrome). Different chromosome disorders may present with SS. For this reason, chromosome studies, preferably high-resolution analysis, should be performed to search for chromosome abnormalities in these children. Specific gene analysis may be requested when a specific syndrome is suspected. In these syndromes, growth failure may be due to a wide variety of mechanisms, including growth hormone deficiency (GHD), growth hormone resistance (Laron syndrome, bone dysplasia) or in combination with nutritional issues with, in many, the underlying mechanisms still being unknown. A complete evaluation of growth hormone/IGF-1 axis is necessary in these children. There are many classifications of short stature, each with specific advantages and disadvantages. Indeed, syndromes with SS could be classified according to clinical presentation and in particular auxological and anthropometrical parameters (SS with normal prenatal growth, SS with intrauterine growth retardation, SS with obesity), or to pathophysiology (GHD or growth hormone insensitivity, bone disorders and idiopathic SS). Here, a classification based on clinical presentation is used. Those syndromes with SS that are most common and are often followed by paediatric endocrinologists namely Silver–Russell, Noonan’s, Turner’s and Prader–Willi syndromes will be reviewed, as well as some rarer syndromes.

Primary malignant tumours of bone are rare and comprise a large number of histological subtypes. The most common are osteosarcoma, Ewing’s family of tumours, and chondrosarcoma. Each of these has further subgroups which will be discussed in detail under their subheadings. The remaining subgroups are exceptionally rare and include the presentation of spindle cell sarcoma of bone, e.g. malignant fibrous histiocytoma (MFH), within the bone as a primary lesion. MFH of bone is treated as other primary bone tumours. Soft tissue sarcomas represent a rare collection of heterogeneous tumours characterized by malignant growth of mesenchymal tissue. Different subgroups can be divided by genetics, pathology, anatomical location, and clinical behaviour. Less than 1% of malignant tumours with an incidence of 30 per million per annum. Median age at presentation depends on histological subtype. For example, rhabdomyosarcomas are most common in children and adolescents, whereas leiomyosarcomas predominate over the age of 40. This chapter covers cancers of musculoskeletal system. Topics include bone tumours overview; osteosarcoma (metastatic, non-metastatic, and recurrent); Ewing’s sarcoma; rare bone tumours; soft tissue sarcomas (management of localized disease, and of locally advanced and metastatic disease); retroperitoneal sarcoma; gastrointestinal stromal tumours; and future directions.

Bone tumours comprise a heterogeneous mix of post-traumatic, developmental, benign, and malignant lesions. They can be subclassified according to their histological matrix as bone-forming, cartilage-forming, or fibre-forming, although some do not produce a specific matrix. Perhaps the most important distinction is between malignant tumours, which have a predisposition to invasive and destructive local growth and distant metastasis, and benign tumours, which do not metastasize, although some can demonstrate locally aggressive behaviour. Both benign and malignant bone tumours can present with unexplained pain and swelling. Radiographic investigation is mandatory to exclude bony malignancy, avoid delay in diagnosis, and prevent incorrect treatment such as arthroscopy or steroid injection when there is an underlying tumour. The three most common malignant tumours of bone are osteosarcoma, Ewing’s sarcoma, and chondrosarcoma. If radiography indicates that bone cancer is a possibility, an urgent referral should be made to a bone cancer multidisciplinary team. Wherever possible, patients should be entered into clinical trials. Some chemosensitive tumours (e.g. osteosarcoma or Ewing’s tumours) require a combined approach to treatment, classically with adjuvant (preoperative) chemotherapy, local therapy (usually surgery), then adjuvant chemotherapy.

Although dissolution reactions involving water can etch and decompose oxides, truly catastrophic failures of oxide structures usually involve fractures and mechanical failures. Geologists and geochemists have long recognized that water and ice both play key roles in promoting the fracture and crumbling of rock (see Chapter 17). Freezing and thawing create stresses that amplify the rate at which water attacks metal–oxygen bonds at the crack tip. The interplay between water and stressed oxides also leads to common failures in man-made objects, ranging from the growth of cracks from flaws in windshields to the rupture of optical fibers in communication systems. In this chapter, we outline how mechanical deformations change the reactivity of metal–oxygen bonds with respect to water and other chemicals, and how reactions on strained model compounds have been used to predict time to failure as a function of applied stress. The basic phenomenon of stress corrosion cracking is illustrated in Figure 16.1. Cracks can propagate through oxide materials at extremely fast rates, as anyone who has dropped a wine glass on the floor can attest. High-speed photography reveals that when glass shatters, cracks can spread at speeds of hundreds of meters per second, or half the speed of sound in the glass. At the other end of the spectrum, cracks in glass can grow from preexisting flaws so slowly that only a few chemical bonds are broken at the crack tip per hour. Because mechanical failures are associated with cracking, it is critical for design engineers to understand the factors that control crack growth rates for this enormous range of crack velocities (a factor of 1012). In addition, because it is difficult to measure crack velocities slower than 10−8 m/second, it is often necessary to make major extrapolations from measured data to predict the long-term reliability of glass and ceramic objects. Will an optical fiber under stress fail in 1 year or 10 years? Answering this question can require accurate extrapolations down to crack growth rates as low as 10−10 m/second.
