Pre-glomerular vessels are regulated by membrane potential alterations affecting the activity of L-type voltage-activated Ca2+ channels; whereas voltage-independent mechanisms regulate the efferent arteriole, notably influenced by angiotensin II and therefore by angiotensin converting enzyme inhibitors, and by non-steroidal anti-inflammatory drugs.These properties underlie the physiologic control of glomerular capillary pressure, for example, by prostaglandin E2, during conditions of reduced renal perfusion and the stabilization of glomerular capillary pressure when the kidney is exposed to pressure fluctuations. A wealth of hormones affects the tone of renal vessels. The chapter focuses on basic regulatory and signalling mechanisms, emphasizing the unique aspects of the renal vasculature and the underlying features that facilitate the independent regulation of pre-glomerular and post-glomerular tone.

Abstract (sommario):
Peripheral nerves consist of afferent and efferent neurones with different functions. TENS can be used to excite somatic efferents to influence the activity of skeletal muscle, and autonomic efferents to influence the activity of smooth muscle, cardiac muscle, and glands. There are physiological rationale to support the use of TENS to manage various non-painful conditions. Clinical experience suggests TENS is often beneficial. The purpose of this chapter is to describe the mechanism of action, clinical use and clinical efficacy for TENS when used to manage non-painful conditions. The chapter covers the effects of TENS on the autonomic nervous system, circulatory system, tissue regeneration, and psychomotor conditions. It also considers the use of TENS for incontinence, constipation, ileus and gastrointestinal discomfort, post-surgical symptoms, and antiemesis.

The spinal cord and peripheral nerves carry motor and autonomic efferents, as well as sensory afferents connecting the cerebrum with the body. Efferent and afferent fibres form predictable tracts within the spinal cord, forming spinal nerves as they exit the spinal canal. Peripheral nerves are often formed from complicated plexuses of spinal nerves in the cervical, lumbar, and sacral spine. Dermatomes are formed from spinal nerves that innervate specific areas of skin, while myotomes innervate a specific set of muscles. The detailed anatomy of these structures are discussed. Knowledge of the anatomy of these structures is relevant to many clinical situations encountered in the intensive care unit especially with caring for neurological, neurosurgical, orthopaedic, and trauma patients.
