AbstractThrough a case narrative set in the near future, this chapter explores some of the cultural and values issues that are raised by recent advances in genetics and associated personalised medicine. On first inspection, these advances might be thought to resolve or at any rate ameliorate the values issues that arise in the context of shared clinical decision-making as the basis of personalised mental health practice in genetic counselling. On the contrary, however, as the case narrative illustrates, the values issues are considerably amplified. This is an instance of the ‘Science Driven’ principle of values-based practice: that the practical impact of advances in medical science and technology is to widen patients’ choices and with choices go values. The result is that advances in medical science and technology drive the need for enhanced values-based as well as evidence-based practice in responding to the challenges presented by personalised clinical care. The emerging resources for responding to these challenges in the context of psychiatric genetic counselling are described.

AbstractThis case report presents the story of a young woman of Romani descent with a mixed dissociative (conversion) disorder within the contextual evidence-based and value-based medical framework. By painting the picture illustrating the course of her illness and the circumstances leading to the last clinical episode, compelling her most recent hospitalization, we delineate the contrast between common clinical phenomenology and the additional layers of the patient’s beliefs and values. Thus, we emphasize the importance of expanding the one-dimensional mainstream evidence-based approach, not only in cases of cross-cultural doctor-patient interactions but also in general medical practice, since the health attitudes and illness behaviors of every individual are influenced by their values and beliefs. In addition, the contemporary notion of medicine as a factual science requires a paradigm shift toward integrative multifaceted approaches if we as doctors are to treat human beings and not merely diseases.

AbstractThe term “precision medicine” is used to denote existing practices as well as how medical research and practice are imagined to become in the future. One important element of the imaginaries of precision medicine is the development of systems biology and computational models with the promise of numerical precision and conceptual rigour. If precision medicine were to become an exact science that relies on computational models, it might increase precision in diagnosis and treatment, specifying the right drug to the right patient at the right time. It should be noted, though, that computational models require explicit specification of the properties and boundaries of the system to be modelled, whereas cells, tissues and patients are predominantly open systems in their natural state. Accordingly, such models risk being precisely wrong instead of approximately right. Right and wrong, however, are value judgements that depend upon the aims and scope of the scientific and medical enterprise. In order for medicine to become an exact science, cells, tissues and patients would have to be reconceived and/or reconfigured as relatively closed systems with relatively deterministic behaviour. The realization of precision medicine as an exact science may thus be accompanied by a transition from a world of complex natural life to a world of reduced life or a simple delay of death; a transition to be likened with the crossing of the Styx.

But do these improvements lead to “good health” for cancer patients? “Good health” is one of the most important things in life, but what is the meaning of “good health” today, how do we talk about health, who is declaring a status of ‘good health’, do modern medicine have limitations in being able to declare ‘good health’, and by which perspectives are “good health” declared? These are all relevant questions to ask when defining and framing health, disease and illness in the era of precision medicine.Ovarian cancer is a serious and highly lethal disease. The different perspectives of health, disease and illness affects the physician-patient relationship and eventually the decision-making. The rapid progress in biomedicine demands knowledge and understanding, but are physicians and cancer patients living in the same world, understanding the same language, or are they all lost in the translation when communicating and understanding illness, disease and above all – health?

The promotion of healthy lifestyles amongst medical students is important in reducing alterations in their emotional state and the creation of healthier habits which, when adopted at this stage, could influence the promotion of better lifestyles for their patients. This current study intends to evaluate health risk behaviors and their relationship with the emotional state in medical students. An analytical cross-sectional study was carried out, evaluating 173 students of a public university between January and March of 2019, evaluating anxiety (Beck Anxiety Inventory, BAI), depression (Beck Depression Inventory, BDI-II), alcohol and nicotine dependence (Alcohol Use Disorders Identification Test, AUDIT, Fagerström Physical Nicotine Dependence), and Body mass index (BMI). sociodemographic and clinical data was collected through an interview. An association was found between anxiety and a sedentary (p = 0.01, OR = 0.446, CI = 0.24 - 0.83), and depression with alcohol consumption (p = 0.005, OR = 2.972, CI = 1.36-6.49). During college students face various demands that can unleash alterations in their emotional and physical state, the results obtained in this study allow the development of intervention programs against risky behaviors and promote healthy lifestyles.
