Objective: To evaluate the curative effect of blue shark skin collagen composite gel on oral mucosal ulcer using the rat oral ulcers model stimulated by glacial acetic acid. Methods: Collagen from blue shark skin was isolated and physiochemically characterized by FTIR, SDS-PAGE and scanning electron microscopy (SEM). Seventy standard male rats were divided into seven groups. The surface and the area of the ulcer were observed and calculated daily. After 12 days of administration, rats in the model group and the control group were killed and the ulcer and surrounding tissues were cut to pieces about one mm3 size. The specimens were stained with 10% formalin solution, paraffinembedded sections, HE staining and light microscope were used to observe the histopathological changes in ulcer tissues. Results: The high-dose group had the fastest ulcer healing effects after 12 days of treatment with blue shark skin collagen composite gel. The composite gel was found to significantly accelerate the healing of oral ulcers in a dose-dependent manner. Conclusion: The blue shark skin collagen composite gel in this study may be a good biomedical material candidate for the treatment of oral ulcers in the near future. Potential of other marine fish skin collagen comples on healing oral ulcers should be also considered.

Background:Oral ulcers, the hallmark lesion of Behçet’s syndrome (BS) can be disabling and impair eating, drinking and speaking. Despite recent advances in systemic medications for the treatment of oral ulcers, some patients do not achieve complete remission. Topical agents may help such patients by decreasing the pain and duration of oral ulcers. Pentoxyfilline (PTX) is a methylxanthine derivative that inhibits phosphodiesterase and is thought to have immunomodulatory effects in addition to improving blood flow which is its main reason for use in peripheral vascular disorders.Objectives:The aim of this study is to assess the efficacy and safety of PTX gel for oral ulcers in patients with BS. We also aimed to explore the best tools for the assessment of treatment response to topical agents in randomized controlled trials (Clinicaltrial.gov ID: NCT 03888846).Methods:This was an open-label, randomized, parallel group study comparing PTX gel in addition to colchicine (PTX-COL) with colchicine alone (COL). Patients with BS who were treated with colchicine and not using any other systemic medications for BS, having at least one oral ulcer that appeared during the last 48 hours were included. PTX 5% gel with a dose of 1000 mg/day was applied in 4 divided doses per day for 14 days. Patients were contacted daily for 14 consecutive days. Photographs were taken every 24 - 48 hours and graphical processing software was used to calculate the area of the index ulcer. Duration of the index ulcer, time to start of index ulcer shrinkage, time to 50% reduction in oral ulcer pain on a 10 mm visual analog scale (VAS), change from baseline in the area of the index ulcer over time, total number of oral ulcers and adverse events were evaluated.Results:A total of 41 patients were randomized, 39 patients (18 in the PTX-COL group and 21 in the COL group) completed the study and 2 patients in PTX-COL group withdrew from the study due to unacceptable dysgeusia and nausea. Mean duration of index ulcer, time to start of index ulcer shrinkage, time to 50% reduction in oral ulcer pain, and number of patients with no detectable ulcers on day 4 in each group were lower in the PTX-COL group as presented in the Table. Change from baseline in the area of index ulcer and pain score over time is shown in the Figure. There were no serious adverse events. Fifteen (75%) patients reported nausea, 11 (55%) reported dysgeusia and 2 reported vomitting in the PTX-COL group, while 2 patients (10%) reported nausea in the COL group.Conclusion:This pilot phase 2 open label, randomized controlled study supports the hypothesis that topical PTX in addition to colchicine accelerates the healing of BS oral ulcers compared to colchicine alone. Aim:Adipose tissue Derived Mesenchymal Stem cells (ADMSCs) represent a promising tool for new clinical concepts in supporting cellular therapy. The goal of this study was to investigate the effects of ADMSCs transplantation on oral ulcer healing in dogs.Study design:Mesenchymal stem cells were isolated from adipose tissues of dogs obtained by suction-assisted lipectomy (liposuction), by dish adherence and were expanded in culture. Oral ulcers were induced by topical application of formocresol in the oral cavity of 18 dogs. The dogs were classified into 3 groups. Either autologousADMSCs, Corticosteriod (Dexamethasone) or vehicle (saline) was injected. The healing process of the ulcer was monitored histopathologically. Gene expression of vascular endothelial growth factor (VEGF),platelets derived growth factor (PDGF),epidermal growth factor (EGF) and collagen was assessed in biopsies obtained from all ulcers “as healing markers”, by real time polymerase chain reaction (PCR). Results:ADMSCs group showed significantly accelerated oral ulcer healing compared with the Dexamethasone and control groups. There was increased expression of VEGF, PDGF,EGF and collagen genes in ADMSCs-treated ulcers compared with Dexamethasone and controls. Conclusion:ADMSCs transplantation may help accelerate oral ulcer healing, possibly through the induction of angiogenesis by VEGF and PDGF, as well as epithelial and connective tissue proliferation as evidenced by increased EGF and collagen gene expression.

Background: Oral inflammatory ulcers are one of the common complaints of patients attending out-patient clinics. Previous in vivo studies had shown that an Aqueous M. citrifolia Fruit Extract (AMFE) possessed anti-inflammatory and ulcer healing activities. Therefore, a standardized topical bioadhesive gel containing AMFE-phytosome was developed and determined for its oral ulcer healing efficacy in a rabbit model. Methods: The AMFE phytosome (AMFE-P) was prepared by a complexation method with the required amount of AMFE: Phosphatidylcholine: Tween 80 to weigh ratio of 2:1:0.2. Poloxamer 407 was used as a gelling agent. The oral ulcer was induced in male New Zealand white rabbits by topical application of acetic acid. Each test compound was applied to the ulcer for 10 days beginning on the second day after the ulcer induction. Complete ulcer healing on the specimen obtained on day 12 was observed histologically using the histological scoring protocol. Results: The optimized gel containing AMFE-P equivalent to AMFE 10%w/w (10%AMFE-P gel) showed the best bioadhesive gel quality, a smooth and homogeneous texture with an optimum viscosity and pH range used in human oral cavity, a good physical and chemical stability and the highest percentage cumulative release of total phenolic and scopoletin content. It was found that a daily application of 10% AMFE-P gel exerted a superior ulcer healing efficacy and a significantly rapid ulcer healing process than a twice daily application of topical gel containing AMFE 10%w/w or chlorhexidine 0.2%. Conclusion: These findings demonstrated that 10% AMFE-P gel has potential as a safe and effective alternative therapeutic agent for oral ulcers.

Oral ulcer is the most common oral disease and leads to pain during meals and speaking, reducing the quality of life of patients. Recent evidence using animal models suggests that oral ulcers induce cyclooxygenase-dependent spontaneous pain and cyclooxygenase-independent mechanical allodynia. Endothelin-1 is upregulated in oral mucosal inflammation, although it has not been shown to induce pain in oral ulcers. In the present study, we investigated the involvement of endothelin-1 signaling with oral ulcer–induced pain using our proprietary assay system in conscious rats. Endothelin-1 was significantly upregulated in oral ulcers experimentally induced by topical acetic acid treatment, while endothelin-1 production was suppressed by antibacterial pretreatment. Spontaneous nociceptive behavior in oral ulcer model rats was inhibited by swab applications of BQ-788 (ETB receptor antagonist), ONO-8711 (prostanoid receptor EP1 antagonist), and HC-030031 (TRPA1 antagonist). Prostaglandin E2 production in the ulcers was suppressed by BQ-788. Mechanical allodynia in the model was inhibited not only by BQ-788 and HC-030031 but also by BQ-123 (ETA receptor antagonist), SB-366791 (TRPV1 antagonist), and RN-1734 (TRPV4 antagonist). In naive rats, submucosal injection of endothelin-1 caused mechanical allodynia that was sensitive to HC-030031 and SB-366791 but not to RN-1734. These results suggest that endothelin-1 production following oral bacterial invasion via ulcerative regions elicits TRPA1-mediated spontaneous pain. This pain likely occurs through an indirect route that involves ETB receptor-accelerated prostanoid production. Endothelin-1 elicits directly TRPA1- and TRPV1-mediated mechanical allodynia via both ETA and ETB receptors on nociceptive fibers. The TRPV4-mediated allodynia component seems to be independent of endothelin signaling. These findings highlight the potential of endothelin signaling blockers as effective analgesic approaches for oral ulcer patients.

BACKGROUND: Diagnosis of oral ulceration is always challenging and has been the source of difficulty because of the remarkable overlap in their clinical presentations. AIM: The objective of this review article is to provide updated knowledge and systemic approach regarding oral ulcers diagnosis depending upon clinical picture while excluding the other causative causes. METHODS: For this, specialised databases and search engines involving Science Direct, Medline Plus, Scopus, PubMed and authentic textbooks were used to search topics related to the keywords such as oral ulcer, oral infections, vesiculobullous lesion, traumatic ulcer, systematic disease and stomatitis. Associated articles published from 1995 to 2019 in both dental and medical journals including the case reports, case series, original articles and reviews were considered. RESULTS: The compilation of the significant data reveals that ulcers can be classified according to (i) duration of onset, (ii) number of ulcers and (iii) etiological factors. Causation of oral ulcers varies from slight trauma to underlying systemic diseases and malignancies. CONCLUSION: Oral manifestations must be acknowledged for precise diagnosis and appropriate treatment.

An oral ulcer is described by repeated or painful oral aphthae, in which oxidative stress apparently leads to its pathogenesis. The present study has been undertaken to evaluate the effects of fresh leaves and flowers of Tagetes erecta gel on oxidative stress and antioxidant levels on oral ulcer models in albino Wistar rats. The oral ulcer was induced by incision wound model and then treated with the Tagetes erecta flower and leaf extract gel for 7 days. The antioxidant activities of catalase (CAT), superoxide dismutase (SOD), the levels of glutathione (GSH) and malondialdehyde (MDA) were measured in rat buccal mucosal tissues. The antiulcer effects of Tagetes erecta leaf and flower extracts (2.5% and 5%) gel were intercompared with the standard drug, Triamcinolone. Ulcer induced rats exhibited elevated levels of MDA and reduced activity of SOD, GSH and CAT when compared to the control rats. The nonequilibrium levels of antioxidant were observed in rat mucosal tissues indicate that body’s antioxidant status was depleted with an oral ulcer. Tagetes erecta leaf and flower gel (5%) has been found to have higher significant antioxidant activity compared with ulcer induced rats by reducing the levels of MDA and by increasing the activities of SOD, GSH and CAT in response to oxidative stress due to induction of oral ulcers in a rat model. The findings thus justify the traditional use of Tagetes erecta to heal oral ulcer by increasing the antioxidant status and decreasing the free radical production.

Recurrent aphthous ulcers are the most prevalent oral mucosal disease, but the subset major aphthous ulcer is a less frequent type. These ulcers are refractory, may persist for several weeks to months, and interfere with the normal state of health. The aetiology is multifactorial and so is the treatment. We present a case of an adolescent male patient reported with multiple oral ulcers. He developed three ulcers simultaneously and suffered for 10 months despite using topical medications prescribed by different dental practitioners. We executed a multidisciplinary treatment approach that resulted in a long-term disease-free state. The treatment methods followed in our case could be a successful model to implement by medical practitioners and oral physicians when the situation demands.

Bakgrund: Trycksår är ett stort problem för patienter och en enorm kostnad för samhället. Sjuksköterskan har ansvar att förebygga att dessa uppstår och hjälpa sårläkningen. Att sår och nutrition har ett samband har länge varit omdiskuterat. Syfte: Syftet med denna litteraturöversikt var att undersöka om läkningen av trycksår kan påverkas av oral nutritionsbehandling. Metod: Studiens metod var en deskriptiv litteraturöversikt där tio artiklar inkluderades och analyserades. Sökning efter artiklar gjordes i PubMed och Cinahl. Vid urval av artiklar lästes sammanfattningar och inklusions- och exklusionskriterier jämfördes mot den aktuella studiens kriterier. Inkluderade artiklar i litteraturöversikten lästes utförligt, analyserades och kategoriserades. Resultat: I de analyserade studierna återfanns resultat som visat på att oral nutritionsbehandling ger signifikant förbättring av trycksårens läkning. Sammanfattningsvis kan oral nutritionsbehandling påverka läkningen av trycksår. Det är oklart hur och på vilket sätt interventionerna påverkar läkningen. Ett samband kunde ses mellan de interventioner som innehöll arginin och zink. Dessa näringsämnen kan potentiellt påverka läkningen positivt. Slutsats: Enligt den här litteraturöversikten kan förbättrad läkning av trycksår ses i samband oral nutritionsbehandling. Dock finns ingen evidens och ytterligare forskning behövs inom området.
Background: Pressure ulcers are a big problem for patients and a huge cost for the tax-paying society. Nurses have a responsibility to prevent the ulcers from emerging and help with the wound healing. A correlation between wounds and nutrition has been debated. Objective: The objective with this overview was to see if the healing of pressure ulcers can be affected by oral nutritional interventions. Method: The method of this study was descriptive literature overview and ten articles were included and analyzed. Original articles were searched for in PubMed and Cinahl. To find the articles needed the abstracts were read and the inclusion- and exclusion criteria were compared to the present study’s criteria. The included articles in the overview were read thoroughly, analyzed and categorized. Result: In the studies that were analyzed there were results that showed that oral nutrition interventions give a significantly improved healing of the pressure ulcers. In summary, oral nutrition interventions can affect the healing of pressure ulcers. It is unclear how and which way the interventions may affect the healing. A correlation between the interventions containing arginine and zinc could be seen. These nutrients can potentially affect the healing of the ulcers positively. Conclusion: According to this literature overview improved healing of ulcers can be seen in combination with oral nutrition interventions. However no evidence was found and more research is needed in this area.

A úlcera de perna representa demanda crescente nos serviços de saúde. Por tratar-se de uma doença crônica, recorrente e de difícil manejo, a úlcera de perna é um problema que vem desafiando profissionais, pacientes e famílias. A necessidade de entender melhor a problemática existente no cuidado oferecido pelo enfermeiro ao paciente com úlcera e sua família, motivou a pergunta do estudo, a qual buscou responder como a família influencia e é influenciada pela úlcera de perna. O estudo teve como objetivo conhecer as percepções e crenças do paciente e de sua família em relação à úlcera de perna, bem como compreender como essas crenças influenciam o enfrentamento da família na experiência. O estudo de caso foi o método adotado, e as técnicas da história oral foram utilizadas para a coleta de dados. O estudo foi realizado com um casal idoso, o qual convive com a úlcera de perna da mulher, focando o impacto desse evento na história da família. As narrativas do casal permitiram a construção da memória coletiva organizada em três temas distintos os quais se inter-relacionam: Envelhecimento e doença, Jornada do tratamento, A ferida e o tempo. Os resultados permitiram compreender que a família influencia e é influenciada pela úlcera de perna e esta dinâmica determina um grande impacto na vida do casal que se encontra na fase tardia do ciclo de vida da família. O estudo identificou que as crenças do casal exercem grande influência sobre o impacto da doença em sua vida, além de determinar seu enfrentamento diante da situação. Este estudo, por considerar a família como categoria de análise, lança um novo olhar ao problema e acrescenta um conhecimento diferente no que se refere à relação entre a família e a ferida. Os resultados permitem uma reflexão acerca da importância do conhecimento profundo dos enfermeiros sobre as experiências dos pacientes e famílias que convivem com úlceras de perna. A valorização e o reconhecimento das forças da família no enfrentamento da doença e da mudança de comportamento do enfermeiro na prática clínica, a qual deve integrar um olhar além da ferida, considerando a interface entre a doença e o contexto familiar, são elementos capazes de estimular uma prática clínica centrada na família.
A leg ulcer is increasing demand on health services. Because it is chronic, recurrent and difficult to handle, leg ulcer is a problem that has defied professionals, patients and families. The need to better understand the problems existing in the care offered by nurses to patients with ulcer and their families led to the question of the study, which sought to answer how the family influences and is influenced by leg ulcer. The study aimed to understand the perceptions and beliefs of the patient and the family regarding leg ulcer as well as understand how these beliefs influence family coping experience. The case study method was adopted, and the techniques of oral history were used for data collection. The study was conducted with an elderly couple, who lives with woman\'s leg ulcer, focusing on the impact of this event in the family history. The narratives of the couple allowed the construction of collective memory organized into three distinct themes which are interrelated: Aging and illness, Journey of treatment, Wound and time. The results allowed to understand that family influences and is influenced by leg ulcers; family dynamics determines a great impact on the couples lives experiencing the late stage of the family life cycle. The study identified that the couples beliefs have great influence on the impact of the disease on their lives, and determine how they cope with the situation. This study, seeing the family as category of analysis, proposes a new perspective at the problem and adds new knowledge to the interaction between family and wound. The results allow a discussion about the importance of deep nursing knowledge about the experiences of patients and families living with leg ulcers. This new perspective requires a change in behavior of the nurse in clinical practice for a vision beyond the wound. The appreciation and recognition of the strengths of the family in coping with the illness and considering the interface between the disease and the family context are elements capable of stimulating a nursing family-centered practice.

Bakgrund: Trycksår är en komplikation som bidrar till ökat lidande för patienter, därav behövs en strukturerad behandlingsstrategi för trycksår. Efter forskning inom ämnesområdet har det blivit konstaterat att nutritionen har en påverkan på sårläkningsprocessen. Nutritionens betydelse för specifikt trycksårsläkning har det även gjorts forskning på, men än idag finns det kunskapsluckor. I nuläget finns etablerade, preventiva åtgärder att tillgå för sjuksköterskor, men tydliga riktlinjer och specifika orala nutritiva åtgärder saknas för trycksårsbehandling vid befintliga trycksår. Legitimerade sjuksköterskor förväntas ha kunskap om sambandet mellan nutrition och hälsa. Av relevans för sjuksköterskors yrkeskompetens och omvårdnadsarbete behövs orala nutritiva åtgärders effekt på läkningsprocessen vid befintliga trycksår belysas. Syfte: Att sammanställa vilken effekt orala nutritiva åtgärder har på sårläkningsprocessen vid befintliga trycksår. Metod: En litteraturstudie som inkluderar 15 vetenskapliga artiklar med kvantitativ ansats. Resultat: De vetenskapliga artiklarnas resultat och statistiska signifikans presenterades och sammanställdes under tre teman: effekten av orala näringstillskott berikad med arginin, effekten av orala näringstillskott berikad med övriga nutrienter och biverkningar. Konklusion: Litteraturstudien påvisade att orala nutritiva åtgärder hade en främjande effekt för sårläkningsprocessen vid trycksår. Det fanns olika näringstillskott som bedömdes gynna trycksårsläkningen men näringstillskott berikad med arginin uppvisade ha bäst effekt.
Background: Pressure ulcers are complications that contribute to increased suffering for patients, therefore is a structured treatment much needed. After research in the subject area it has been concluded that nutrition has an important role in the wound healing process. The importance of nutrition for specifically pressure ulcers has also been researched on, but still to this day there are knowledge gaps. There are currently preventive actions established for nurses, but not clear guidelines nor specific oral nutritional interventions for existing pressure ulcers. Registered nurses are expected to have knowledge of the connection between nutrition and health. Therefore, it’s essential to compile oral nutritional interventions effect on the healing process of existing pressure ulcer. Aim: To compile oral nutritional interventions’ effect on the healing process in existing pressure ulcers. Method: A literature review based on 15 scientific, quantitative studies. Results: The scientific studies’ result and statistical significance was presented and categorised into three categories: the effect of oral nutritional supplements enriched with arginine, the effect of oral nutritional supplement enriched with other nutrients and side effects. Conclusion: This literature review showed that oral nutritional interventions had a favourable effect on the healing of pressure ulcers. Various nutrition supplements were deemed to favour the healing of pressure ulcers, but especially supplements enriched with arginine.

As proteínas de choque térmico (HSPs) são proteínas presentes nas células em condições normais e supra-expressadas em condições de estresse e choque térmico. O laser de diodo tem sido utilizado em diversos tratamentos na odontologia, dentre eles na atenuação de sintomas e na aceleração do reparo de úlceras traumáticas. O objetivo deste estudo foi verificar, através de análise imunoistoquímica da expressão das HSPs (Hsp27 e Hsp 47) e da medição de temperatura por intermédio de câmera termográfica, se o laser de diodo modifica, (e quanto modifica) a temperatura local, bem como se há alteração no padrão de expressão das proteínas citadas em três regiões distintas do fragmento analisado. Para este estudo, foram feitos testes in vitro e in vivo. Para os testes in vivo, foram utilizados 56 ratos, nos quais foi induzida úlcera em ventre lingual. Os animais foram divididos em 4 grupos: GL - grupo ulcerado com posterior irradiação com laser (parâmetros: laser diodo, 0,5 W de potência, pulsado (10 Hz), por 40 segundos (método varredura), 80J/cm2 de energia total (área de 0,25cm2) Má, e a energia por ponto?); GN - grupo ulcerado sem nenhum tratamento posterior; CP - grupo controle sem úlcera e irradiado com laser (mesmos parâmetros de GL); e CN - grupo controle sem úlcera e sem nenhum tratamento posterior. O teste in vitro foi destinado à medição da temperatura durante a irradiação laser, quando utilizaram-se línguas de ratos previamente ulceradas e extirpadas, as quais foram irradiadas com os parâmetros para o teste in vivo. Na superfície irradiada, houve aumento médio de temperatura em torno de 6,7C e, na região mais distante dessa superfície, de 2,7C. Na análise semiquantitativa da expressão imunoistoquímica da Hsp27, observou-se padrão de marcação mais intenso em GL comparando-se a GN e aos demais grupos. Na análise quantitativa da Hsp47, houve maior quantidade de células positivas no GL em relação aos demais grupos, principalmente nas regiões mais próximas da superfície irradiada. Concluiu-se que a irradiação laser provocou aumento de temperatura local, bem como desencadeou padrões de intensidade maiores e diferentes da Hsp27 e da Hsp47 em relação aos demais grupos. Isso indica que o tecido irradiado sofreu maior estresse celular, com resposta tecidual de intensa migração e diferenciação celular, bem como de maior síntese colagênica.
Heat shock proteins (HSPs) are proteins present in cells under normal conditions and over-expressed in terms of stress and thermal shock. The laser diode has been used in various treatments in dentistry, to reduce symptoms and accelerate the repair of traumatic ulcers. The aim of this study was to verify, by means of immunohistochemical expression analysis of HSPs (Hsp27 and Hsp 47) and temperature measurement using a thermographic camera, whether diode laser changes the local temperature (and how much the temperature changes), and whether there is a change in expression pattern of the above-mentioned protein in three distinct regions of the analyzed fragment. For this study, some of the tests were conducted in vitro and others in vivo. For the in vivo tests, 56 rats were used, in which ulcers were induced on the ventral surface of the tongue. The animals were divided into 4 groups: GL-group with ulcer and later laser irradiation (diode laser parameters: 0.5 W power, pulsed (10 Hz), for 40 seconds (defocused), 80J/cm2 total power in area (0.25cm2) GN- group with ulcer and without any further processing; CP- control group without ulcer and with laser irradiation (same parameters as in GL); and GN-control group without ulcer and without any further processing. The in vitro tests were conducted to measure temperature during laser irradiation, using the tongues that had previously been ulcerated in rats, and later removed and irradiated with the same parameters as those used for the in vivo test. In the irradiated surface, there was an increase in mean temperature of around 6.7oC, and in the region more distant from this surface, an increase of 2.7 oC. In the semi-quantitative analysis of immunohistochemical expression of Hsp27, greater expression of Hsp27 was shown in GL in comparison with the other groups in general. In quantitative analysis of Hsp47, there was a larger quantity of positive cells in GL, when compared with the other groups, particularly in the regions closest to irradiated surface. It was concluded that the irradiation laser caused local temperature increase, and also set off greater and different patterns of intensity of Hsp27 and Hsp47, in comparison with those of the other groups. This indicates that the irradiated tissue suffered higher cellular stress, with tissue response of intense migration and cell differentiation, as well as increased collagen synthesis.

A gengivoestomatite linfoplasmocítica felina (GELF) é uma síndrome clínica frequentemente observada na medicina de felinos. Trata-se de uma síndrome provavelmente multifatorial que envolve o sistema imune dos animais, agentes infecciosos e não infecciosos, de modo que a etiologia ainda não foi totalmente determinada. Diversos agentes infecciosos como o calicivírus felino (FCV), herpesvírus felino (FHV), vírus da leucemia felina (FeLV) e vírus da imunodeficiência felina (FIV) têm sido investigados. Este trabalho apresenta uma caracterização clínico, patológica, imuno-histoquímica e molecular de 27 felinos com GELF. Quanto à raça, 85,2% eram sem raça definida, 7,4% Siamês e 7,4% Maine Coon. Os machos corresponderam a 67% dos casos e fêmeas 33%. Quanto à idade, 10% apresentavam entre 1 e 3 anos, 20% entre 4 e 6 anos, 30% entre 7 e 9 anos, 15% entre 10 e 12 anos e 25% tinham mais do que 13 anos. A idade média dos felinos foi de 8,8 anos. Os principais sinais clínicos observados foram: disfagia (83,3%), halitose (72,2%), sialorreia (44,4%), emagrecimento (38,8%), intenso desconforto oral (33,3%), sangramento oral (22,2%), pelos opacos e quebradiços (11,1%) e intensa dispneia (5,5%). Macroscopicamente, eram lesões bilaterais no arco palatino e que se estendiam até a face lateral da base da língua. Em 77,8% elas eram difusas, de aspecto proliferativo, coloração vermelho intenso, friável e que sangravam facilmente durante a manipulação e em 22,2% as lesões eram multifocais a coalescentes, por vezes formando múltiplas vesículas, no arco palatino de aspecto avermelhado e edematoso. Microscopicamente, 14,8% apresentaram uma inflamação moderada (grau 2) e 85,2% dos animais apresentaram uma inflamação grave (grau 3). Os antígenos virais do FeLV foram identificados através da técnica de IHQ no epitélio e células do infiltrado inflamatório de 29,6% dos animais que apresentavam a GELF. Os antígenos virais do FIV foram identificados nas células do infiltrado inflamatório em 3,7% dos casos. Os antígenos virais do FCV não foram identificados nas lesões de GELF. Dezoito por cento dos animais foram positivos para FIV e 37% foram positivos para FeLV no exame de PCR.
Feline chronic gingivostomatitis (FCGS) is a clinical syndrome frequently observed in feline medicine. This is probably a multifactorial syndrome which involves the immune system of animals, infectious and noninfectious agent, though the causes have not been fully determined. Several infectious agents such as feline calicivirus (FCV), feline herpesvirus (FHV), feline leukemia virus (FeLV) and feline immunodeficiency virus (FIV) have been investigated. To determine clinical, pathological, immunohistochemical (IHC) and molecular characteristics of 27 cats with FCGS was developed. Regarding race, 85.2% were mixed breed, 7.4% Siamese and 7.4% Maine Coon. Males were 67% of cases and females 33%. Regarding age, 10% were between 1 and 3 years, 20% between 4 and 6 years, 30% between 7 and 9 years old, 15% between 10 and 12 years and 25% were older than 13 years. The average age of the cats was of 8.8 years. The main clinical signs were: dysphagia (83.3%), halitosis (72.2%), sialorrhoea (44.4%), weight loss (38.8%), intense oral discomfort (33.3%), oral bleeding (22.2%), hair opaque and brittle (11.1%) and severe dyspnea (5.5%). Grossly, the lesions were bilateral in the palatal arch and extending to the lateral side of the base of the tongue. In 77.7% they were diffuse, proliferative, intense red, crispy and bled easily during handling and in 22.3% lesions were multifocal to coalescing, sometimes forming multiple vesicles, the palatal arch reddish and edematous appearance. Microscopically, 14.8% showed moderate inflammation (grade 2) and 85.2% of the animals showed severe inflammation (grade 3). FeLV viral antigens were identified by IHC reaction in the epithelium and inflammatory cells of 29.6% of the animals showing FCGS. FIV viral antigens were identified in inflammatory cells in 3.7% of cases. FCV viral antigens were not identified in lesions of FCGS. Eighteen per cent of animals were positive for FIV and 37% were positive for FeLV in the PCR.

Oral ulcers and oral thrush could be indicative of serious illnesses such as oral cancer, HIV and other sexually transmitted infections (STIs), among others. There are many different health care workers that can be approached for advice and/or treatment for oral ulcers and oral thrush (sometimes referred to as mouth sores by patients), including pharmacists. In fact, the mild and intermittent nature of oral ulcers and oral thrush may most likely lead the patient to present to a pharmacist for immediate treatment. In addition, certain aspects of access are exempt at a pharmacy such as long queues and waiting times, the need to make an appointment and the cost for consultation. Thus pharmacies may serve as a reservoir of undetected cases of oral cancer, HIV and other STIs. Aim: To determine how community pharmacists in the Western Cape manage oral ulcers and oral thrush. Objectives: The data set included the prevalence of oral complaints confronted by pharmacists, how they manage oral ulcers, oral thrush and mouth sores, their knowledge about these conditions, and the influence of socio-economic status (SES) and metropolitan location (metro or non-metro) on recognition and management of the lesions. Method: A cross-sectional survey of community pharmacists in the Western Cape was conducted. A random sample of pharmacies was stratified by SES (high and low), and metropolitan location. A structured questionnaire was used to conduct a telephonic interview. The questionnaire was faxed to pharmacists 24 hours prior to the interview. Pharmacists were also telephoned 24 hours prior to the interview, but after the questionnaire was faxed, in order to gain informed consent for participation in the study, and to confirm a convenient time to conduct the interview. Results: Two thirds (63%) of pharmacists managed oral problems nearly everyday, and 30% managed these more than once a week. More pharmacists in high SES (73%) areas managed oral problems nearly everyday (Fisher Exact, p=0.0005). Just over half (56%) and 49.2% of pharmacists said that ulcers and thrush, respectively, was the most common oral problem that they encounter. The prevalence of oral thrush was significantly higher in non-metro areas (58%) (RR=0.7 (0.5-1.0) ChiSquared=4.0, p=0.04), and it was also significantly lower in low SES areas (RR=1.6 (1.1-2.4), Chi Squared=6.5, p=0.01). Half the pharmacists reported that they would manage the patient comprehensively. Most would take a history but the quality of the history is poor, thereby compromising their ability to manage these cases appropriately. Only a third would refer a simple oral ulcer, thrush or mouth sore to a doctor/dentist but all pharmacists would have referred a longstanding ulcer to a doctor/dentist. In terms of knowledge, only 33% of pharmacists were aware that oral ulcers and thrush could be indicative of HIV infection, and only 8% linked oral ulcers with oral cancer. There was no discernable pattern of management of oral ulcers and thrush, or of knowledge of the link between these lesions with underlying diseases, by SES and metropolitan locations (Chi Squared, Fisher Exact, p>0.05). Conclusion: The result of this study strengthens the current view of pharmacists as oral health advisors as they encounter oral problems regularly, most commonly oral ulcers and oral thrush. Therefore, the pharmacist can play an important role in the early detection of HIV and oral cancer. However many pharmacists fail to refer these cases to a dentist. Most pharmacists lacked specific knowledge about the relationship of oral ulcers and thrush with underlying conditions, which may explain why many pharmacists are not managing these cases correctly. There is a definite need to train pharmacists at both an under- and post-graduate level with specific emphasis on the proper management of oral ulcers, oral thrush and mouth sores, as well as comprehensive training that outlines why vigilant management of oral ulcers and oral thrush is necessary.

The oral mucosa is a frequent site for traumatic ulcers, which can be caused by mechanical or chemical agents. There is no a gold standard treatment for the management of these lesions, which can be painful and have a negative impact on quality of life. Although studies suggest the use of chamomile and curcumin in the treatment of ulcerated lesions of the oral mucosa, there is no a consensus as to its efficacy in the management of these lesions. The literature has shown that both phytotherapics modulate the levels of inflammatory cytokines such as TNF-? and IL-1?. Therefore, in the first article of this dissertation, an integrative literature review was conducted investigating the efficacy of topical interventions with chamomile and curcumin in the management of pain and in the healing of ulcerated lesions of the oral mucosa. Few controlled clinical trials have been found and they suggest a beneficial effect of chamomile and curcumin in the healing process and in the reduction of symptoms in patients with oral mucositis, recurrent aphthous ulceration and oral lichen planus. Studies in animal demonstrated a reduction in the inflammatory wound healing phase, increased deposition of collagen fibers and epithelization. The second article of this dissertation presents an experimental study, which was carried out to evaluate the effect of the 10% chamomile extract and 2% curcumin in the repair of traumatic ulcers on tongue of rats. Thirty-nine Wistar rats were divided into three groups: control, chamomile extract and curcumin. A 5-mm-diameter excisional wound was made in the center of ventral tongue. The substances were applied as a gel twice daily for five days. The animals were euthanized 7 days after the lesions were made. In the macroscopic evaluation, there was no significant difference among chamomile, curcumin and control groups regarding to area of residual ulcer (P= 0.20). Histological analysis also showed no difference among the groups regarding newly formed epithelium area (P=0.97) or intensity of inflammatory infiltrate (P=0.72). Considering the experimental model, the findings of the present study showed that 10% chamomile extract and 2% curcumin did not accelerate the repair of traumatic oral ulcers or reduced the intensity of the inflammatory infiltrate, contradicting previous findings in the literature.

At? o momento, n?o existe um tratamento padr?o-ouro para o manejo dessas les?es, as quais podem ser dolorosas e ter impacto negativo na qualidade de vida dos pacientes. Apesar de estudos sugerirem a utiliza??o da camomila e da curcumina no tratamento de les?es ulceradas da mucosa bucal, n?o h? consenso sobre a sua efic?cia no manejo dessas les?es. A literatura tem demonstrado que ambos os fitoter?picos modulam os n?veis de citocinas pr?-inflamat?rias como TNF-? e IL-1?. No primeiro artigo desta disserta??o foi realizada uma revis?o integrativa da literatura investigando a efic?cia das interven??es t?picas com camomila e curcumina no manejo da dor e na cicatriza??o de les?es ulceradas da mucosa bucal. Poucos ensaios cl?nicos controlados foram encontrados e estes sugerem um efeito ben?fico da camomila e da curcumina no processo cicatricial e na redu??o da sintomatologia em pacientes com mucosite oral, ulcera??o aftosa recorrente e l?quen plano oral. Na avalia??o macrosc?pica, n?o houve diferen?a entre os grupos camomila, curcumina e controle quanto ? ?rea de ?lcera remanescente (P=0.20). Tularemia, caused by the gram-negative coccobacillus Francisella tularensis, is an extremely infectious bacterial zoonosis. Symptoms depend on site of exposure; they can be nonspecific and may include fever, lymphadenopathy, ulcer or papule, and nausea/vomiting. Natural transmission occurs via small mammals, such as rabbits, or arthropod bites. IV or IM antibiotics are preferred over oral forms. Supportive care is also critical; some patients may require respiratory support. If used as a biological weapon, aerosolized F. tularensis would be the most likely route of transmission. Clinical symptoms would include those of pneumonic tularemia. In the event of a bioterrorist attack, oral administration antibiotics can be used, as the health care system may not be able to accommodate intravenous or intramuscular treatment. Antibiotic resistance should also be considered if patients deteriorate despite use of recommended antibiotics.

Human myiases can be caused by over 50 species of dipteran larvae. The numbers of human clinical myiasis reports, reflect their relative importance in the following order; cutaneous, ophthalmomyiases, nasal, oral, intestinal, ear, urogenital, and cerebral myiases. Myiasis producing flies are distributed worldwide, but most reported cases are from warm and developing countries. Molecular techniques have been applied to myiasis fly identification and classification, especially ostrids and calliphorines. Successful elimination programs have been carried out against Hypoderma spp. in the UK and Cochliomyia hominivorax in the USA, Mexico, Central America, Libya and the Caribbean Islands and another is ongoing against Crysomya bezziana in the Middle East. A beneficial myissis “Biosurgery or maggot therapy” is the intentional use of Lucilia sericata larvae applied in specially designed dressings to chronic and MRSA infected wounds. The growing larvae execration/secretion facilitate wound debridement and successfully treated leg and pressure ulcers, wounds associated with diabetes, and many other types of infected wounds in a shorter time compared to conventional treatment. Now knowledge of myiases producing flies is accepted in many countries as a forensic tool.

Genitourinary tract diseases in the palliative care setting most commonly involve urinary tract obstruction, intractable bleeding, fistulae, and bladder-associated pain. Sources of obstruction in the lower urinary tract include benign prostatic hyperplasia, invasive prostate or bladder cancer, urethral stricture, or bladder neck contracture. Upper tract obstruction includes intraluminal or extraluminal blockage of the renal collecting system and ureters, such as transitional cell carcinoma, fibroepithelial polyps, stricture, stones, pelvic or retroperitoneal malignancy, fibrosis, or prior radiation. Untreated, obstructive uropathy leads to elevated bladder, ureter, and kidney pressures, bladder dysfunction, urolithiasis, renal failure, pyelonephritis, or urosepsis. Intractable haematuria can cause problematic anaemia, frequent transfusions, clot retention, haemorrhagic shock, and death. In addition, urinary tract fistulae such as vesicovaginal and vesicoenteric fistulae are common in patients who have had prior pelvic surgery or radiation especially in the setting of immunocompromise, poor nutrition, and infection. Untreated, these symptoms lead to rash, skin breakdown, ulcers, chronic infection, and sepsis. Lastly, pelvic and bladder pain, depending on aetiology can be treated with oral medications, intravesical therapies, or surgical therapies such as palliative resection or urinary diversion. Selection of tests and treatment modalities in the palliative care setting should be based on using the least invasive means to achieve the most relief in suffering. Some genitourinary conditions are potentially fatal, and in the acute or subacute setting, require re-evaluation of the end-of-life goals and wishes of the patient and family.

Behçet's syndrome is a systemic inflammatory panvasculitis (affecting all sizes of vessels) of unknown aetiology. It is in vogue to include it among the systemic autoinflammatory conditions. Behçet's syndrome is more frequent along the ancient 'Silk Route' across Asia than it is in Western countries. The usual onset is the second or third decade, equally affecting either gender. However, young patients and male patients have more severe disease. Almost all patients have recurrent oral ulceration. Scar-forming genital ulcers, a variety of skin lesions including acneiform, erythema nodosum-like lesions, arthritis, potentially blinding panuveitis, thrombophlebitis, gastrointestinal disease, central nervous system (CNS) involvement, and life-threatening bleeding pulmonary artery aneurysms are seen. The pathergy phenomenon is a heightened tissue inflammatory response. The strongest genetic association is with HLA B51. There are immunological aberrations but not prominent enough to call it an autoimmune disease. Similarly, Behçet's syndrome does not fit easily into the broad concept of autoinflammatory diseases. The histopathology is also non-specific and the diagnosis is mainly clinical. Differentiation from Crohn's disease is very difficult. In more than one-half of the patients the disease burns out in time, thus only symptomatic therapy is indicated in some patients. However, eye involvement, pulmonary vascular disease, thrombophilic complications, CNS involvement, and gastrointestinal disease need prompt recognition and treatment. Brief courses of glucocorticosteroids along with immunosuppressives including the newer biologicals, interferon, and colchicine are commonly used. However, controlled clinical trials are not available for some of these medications especially when thrombophilia, CNS, and gastrointestinal disease are present.
